From the early days of hemodialysis, viral hepatitis has been recognized as common in dialyzed patients.The prevalence and incidence of HBV infection have decreased markedly over the last decades in HD units. Still, the infectivity of HBV is very high. Vaccinating HD patients, preferably prior to starting dialysis, together with the strict application of hygienic precautions and adequate screening of blood donors remains required, together with the segregation of infective (HBV+) patients in a separate dialysis ward. The level of aminotransferases is markedly lower in HD patients than in the general population: any level above the normal range should thus trigger the suspicion of acute hepatitis (viral or not). The treatment of HBV infection in HD patients is rarely required, unless they are scheduled for a kidney transplant.Screening for HCV infection usually relies on a modern ELISA test. The prevalence and incidence of HCV infection in HD patients has also decreased substantially but remains higher than in the general population. The risk of post-transfusional HCV is currently extremely low, at least in western countries. The actual application of basic hygienic precautions is crucial if nosocomial transmission of HCV is to be prevented. These include optimal hand hygiene practices (hydroalcoholic solution use before contact with patient and after gloves withdrawal), the systematic wearing of gloves, to be changed between patients/stations, an adequate separation of the clean and contaminated items and circuits within the HD unit, and regular cleaning/disinfection of potentially contaminated surfaces. The necessity and usefulness to isolate HCV positive patients in a separate dialysis ward has not been demonstrated and is not recommended by current KDIGO guidelines. The field of the treatment of HCV infection is changing rapidly with many orally active drugs, some of which can be used even in dialysis patients.

An estimated 185 million individuals have been infected with hepatitis C virus (HCV) worldwide. Although often clinically silent for decades, chronic HCV infection predisposes to late-onset complications, including liver cirrhosis and hepatocellular carcinoma. Mother-to-child transmission (MTCT) of HCV affects approximately 5% of children born to viremic mothers and is the primary route of HCV infection in young children. While some vertically acquired HCV infections are resolved during the first years of life, many persist indefinitely. Chronically infected children tend to be asymptomatic and have mild liver disease, but they face a risk of progression to advanced liver disease in adulthood. Current diagnostic and management strategies leave most infected children undiagnosed and untreated. Widespread use of newly-available direct-acting antiviral therapies has the potential to substantially reduce the global burden of HCV, including vertically acquired HCV, but an effective vaccine likely will be required to achieve this ultimate goal.

This chapter discusses screening for gastrointestinal disease, including Barrett’s oesophagus (BO), colorectal cancer, and hepatocellular cancer (HCC). In patients with BO, approximately 5% will develop dysplasia, and 10%–50% of the low-grade dysplasias will progress to high-grade dysplasia or adenocarcinoma within 2–5 years. Thus, screening for BO has been developed to reduce the development of adenocarcinoma via the early detection of high-grade dysplasia or cancer in situ. The main aim of colorectal cancer screening is the early detection of polyps and cancers, at a time when treatment is likely to be more effective. Similarly, early detection of HCC is advantageous, as the prognosis in advanced disease is very poor. This chapter describes the current processes of screening for these diseases, and the impact of this screening, as well as screening for gastrointestinal cancer in specific groups.

This book provides an unmatched introduction to eight of the most important works of classical Chinese philosophy—the Analects of Confucius, Mozi, Mencius, Laozi, Zhuangzi, Sunzi, Xunzi, and Han Feizi. The book places these works in rich context that explains the origin and meaning of their compelling ideas. Because none of these classics was written in its current form by the author to whom it is attributed, the book begins by asking, “What are we reading?” and showing that understanding the textual history of the works enriches our appreciation of them. A chapter is devoted to each of the eight works, and the chapters are organized into three sections: “Philosophy of Heaven,” which looks at how the Analects, Mozi, and Mencius discuss, often skeptically, Heaven (tian) as a source of philosophical values; “Philosophy of the Way,” which addresses how Laozi, Zhuangzi, and Sunzi introduce the new concept of the Way (dao) to transcend the older paradigms; and “Two Titans at the End of an Age,” which examines how Xunzi and Han Feizi adapt the best ideas of the earlier thinkers for a coming imperial age. This chapter discusses primary prevention measures that disrupt transmission of oncogenic infections. It begins by discussing vaccination against hepatitis B virus (HBV) and human papillomavirus (HPV), two major causes of cancer for which safe and effective vaccines are currently available. It briefly discusses the importance of treatment and prophylaxis against human immunodeficiency virus type 1 (HIV-1), which potentiates the virulence of other viral infections as well as directly increasing the incidence of non-Hodgkin lymphoma. It does not discuss the treatment of HBV or hepatitis C virus (HCV) infection, since these are considered in Chapters 25 and 33. Also beyond the scope of this chapter are the randomized clinical trials currently underway to assess the efficacy and feasibility of eradication of Helicobacter pylori (Chapters 24, 31), vaccination against Epstein-Barr virus (EBV) (Chapters 24, 26, 39), or the prevention of schistosomiasis and liver flukes (Chapters 24, 33, and 52).

Online social networks have made sharing personal experiences with others mostly in form of photos and comments a common activity. The convergenceof social, mobile, cloud and wearable computing expanded the scope of usergeneratedand shared content on the net from personal media to individual preferencesto physiological details (e.g., in the form of daily workouts) to informationabout real-world possessions (e.g., apartments, cars). Once everydaythings become increasingly networked (i.e., the Internet of Things), future onlineservices and connected devices will only expand the set of things to share. Given that a new generation of sharing services is about to emerge, it is of crucialimportance to provide service designers with the right insights to adequatelysupport novel sharing practices. This work explores these practices within twoemergent sharing domains: (1) personal activity tracking and (2) sharing economyservices. The goal of this dissertation is to understand current practices ofsharing personal digital and physical possessions, and to uncover correspondingend-user needs and concerns across novel sharing practices, in order to map thedesign space to support emergent and future sharing needs. We address this goalby adopting two research strategies, one using a bottom-up approach, the otherfollowing a top-down approach.In the bottom-up approach, we examine in-depth novel sharing practices within two emergent sharing domains through a set of empirical qualitative studies.We offer a rich and descriptive account of peoples sharing routines and characterizethe specific role of interactive technologies that support or inhibit sharingin those domains. We then design, develop, and deploy several technology prototypesthat afford digital and physical sharing with the view to informing the design of future sharing services and tools within two domains, personal activitytracking and sharing economy services.In the top-down approach, drawing on scholarship in human-computer interaction (HCI) and interaction design, we systematically examine prior workon current technology-mediated sharing practices and identify a set of commonalitiesand differences among sharing digital and physical artifacts. Based uponthese findings, we further argue that many challenges and issues that are presentin digital online sharing are also highly relevant for the physical sharing in thecontext of the sharing economy, especially when the shared physical objects havedigital representations and are mediated by an online platform. To account forthese particularities, we develop and field-test an action-driven toolkit for designpractitioners to both support the creation of future sharing economy platformsand services, as well as to improve the user experience of existing services.This dissertation should be of particular interest to HCI and interaction designresearchers who are critically exploring technology-mediated sharing practicesthrough fieldwork studies, as well to design practitioners who are building and evaluating sharing economy services.

This chapter explores how precise formal methods can be used effectively and practically in interaction design. The term ‘formal methods’ in computer science refers to a suite of techniques drawing on mathematical notions of sets, logic, and functions or precise diagrammatic notations, most of which are currently primarily focused on safety-critical applications in the aerospace or nuclear industries. While research into broader use of these methods could be regarded as a theoretical interest, the early development of formal methods was driven as much by practical considerations as theory. This chapter features two case studies on formal notations and their use in areas of practical interaction design beyond safety-critical applications, as well as understood, used, and appropriated by clients and designers who have no formal training or expertise. Each offers specific notations and techniques to the reader and also explores more general lessons for creating practical formal methods for HCI.

In this research work, we have demonstrated the synthesis, spectroscopic characteristics, thermal behaviour and DC conductivity of a few nanostructured composites, substituted conducting polymers (ICPs) and composites of ICPs. The physical properties of aforementioned composites are significantly changed by the doping with HCl, H2SO4, HNO3, H3PO4, or acrylic acid. The charge transport properties of these polymeric materials have been studied in detail because of their potential application in gas sensors. In the current work, varieties of conducting polymer based materials such as PANI-ES/Cloisite 20A nanostructured composite, acrylic acid (AA) doped PANI polymer, N-substituted conducting polyaniline polymer, DL−PLA/PANI-ES composites, poly methyl methacrylate (PMMA) based polyaniline composite, and inorganic acid doped polyaniline are sucessfuly synthesized using aniline/aniline hydrochloride as precursors in acidic medium. Particularly, AA based synthesised PANI polymer was found with higher solubility The spectroscopic, thermal stability, enthalpy of fusion, room temperature DC conductivity and temperature dependent DC conductivity measurements with and without magnetic was carried out with as-synthesized materials. The FTR/ATR−FTIR spectra indicated the presence of different functional groups in the as-prepared composite materials. The UV−Visible absorption spectroscopic analysis showed the presence of polaron band suggesting PANI-ES form. The Room temperature DC conductivity, temperature variation DC conductivity (in presence and absence of magnetic field), and magnetoresistance (MR) of as-prepared conducting polyaniline based were analysed. The highest room temperature DC conductivity value was obtained from H2SO4 doped based composite materials and all prepared conductive composites were followed ohms law. The low temperature DC conductivity was carried out in order to study the semiconducting nature of prepared materials. The Mott type VRH model was found to be well fitted the conductivity data and described the density of states at the Fermi level which is constant in this temperature range. From MR plots, a negative MR was observed, which described the quantum interference effect on hopping conduction. We discuss different gas analytes i.e., NO2, LPG, H2, NH3, CH4, and CO of conducting polymer based materials.
