Die jährliche Inzidenz der häufigsten primären Hirntumoren, der Gliome, beträgt 5-6 /100'000 (www.cbtrus.org). In der Schweiz werden etwa 500 Neuerkrankungen bei Erwachsenen pro Jahr diagnostiziert. Etablierte Risikofaktoren für die Entwicklung von Gliomen sind frühere Exposition mit ionisierenden Strahlen und selten eine genetische Prädisposition (i.R. von familiären Tumorsyndromen). Ein Risiko durch den Gebrauch von mobilen Telefonen wird zwar vermutet, ist aber bislang noch nicht durch epidemiologische Langzeituntersuchungen bewiesen. Die Prognose und Therapieindikationen richten sich weitgehend nach klinischen Faktoren wie Alter und Performance Status des Patienten. Die Primärtherapie von Glioblastomen (GBM, WHO-Grad IV) ist international standardisiert und beinhaltet größtmögliche Resektion, postoperative Radiotherapie des Teilhirns mit konkomitierender Chemotherapie (Temozolomid über 6 Wochen) gefolgt von Temozolomid über 6 Monate. Glioblastome rezidivieren immer innerhalb weniger Monate bis Jahre. Zur Behandlung eines Rezidivs stehen dem Patienten verschiedene individuelle Therapieoptionen zur Verfügung. Je nach Lage und Größe kann sich ein Rezidiv für eine zweite Resektion, für eine stereotaktische Re-Bestrahlung oder für eine Zweitlinienchemotherapie und/oder für die Behandlung mit einer Angiogenesehemmung eignen. Gliome sind diffus infiltrierende Krankheiten des Gehirns und daher von Anfang an nicht kurativ behandelbar, das mittlere Überleben liegt für GBM bei 15 Monaten. Bei Patienten über 70 Jahren und einem guten Allgemeinzustand ist nach der Stratifizierung für den Methylierungsstatus des MGMT Promoter Gens eine alleinige Radiotherapie oder bei methyliertem MGMT Status eine alleinige Chemotherapie als Erstlinientherapie gerechtfertigt. Anaplastische Gliome (WHO-Grad III) können postoperativ mit Radiotherapie oder Chemotherapie behandelt werden und beim Auftreten eines Rezidives mit der entsprechend anderen Modalität. Falls ein WHO-Grad III-Gliom aber eine 1p,19q Co-Deletion aufweist, sollte die Primärtherapie nach neuesten Erkenntnissen immer eine Chemo- und Radiotherapie beinhalten. Das mittlere Überleben variiert je nach prognostischen Faktoren von 3 bis > 7 Jahren. Für WHO-Grad II-Gliomen gibt es bislang keine standardisierte Therapie. Neben einer primären "wait and see" Strategie, stehen bei Tumorprogress die Chemotherapie oder die Radiotherapie zur Verfügung. Die Studienresultate der EORTC Studie (22033/26033), welche die beiden Therapieoptionen in der Erstlinientherapie prüft, stehen noch aus.

ZusammenfassungIntramedulläre Tumoren sind ausgesprochen selten. Die häufigsten Entitäten sind Ependymome (überwiegend im Erwachsenenalter), Astrozytome (überwiegend im Kindesalter) und Hämangioblastome. Die Mehrheit sind gutartige oder niedriggradige langsam wachsende Neoplasien mit entsprechend langsam progredienter Klinik, die sich oft zuerst als ein unspezifisches Schmerzsyndrom und später als eine zunehmende Myelopathie zeigt. Die Ausnahme sind infiltrativ wachsende höhergradige Gliome, die eine rasch progrediente Verschlechterung bis hin zum akuten Querschnitt verursachen. Die Therapie intramedullärer Ependymome, pilozytischer WHO I Astrozytome und Hämangioblastome fokussiert sich in erster Linie auf die Komplettresektion des Tumors. Bei den beiden letztgenannten Entitäten ist damit meistens ein kuratives Ergebnis erreicht. Auch WHO I und II Ependymome zeigen nach einer chirurgischen Komplettresektion eine ausgesprochen benigne Prognose mit einem progressionsfreien 5-Jahres-Überleben von > 70 % und einem 5-Jahres-Gesamtüberleben von fast 90 %. Diffus wachsende astrozytäre Tumore können dagegen i. d. R. nicht komplett reseziert werden: hier ist das Ziel die Sicherung der Histologie und allenfalls Tumordebulking mit dem Ziel einer Syrinxdekompression und einer Stabilisierung der neurologischen Situation vor der geplanten adjuvanten Radiochemotherapie. Auch die Prognose dieser Entitäten ist deutlich schlechter: das mediane Gesamtüberleben der Patienten mit höhergradigen spinalen Gliomen beträgt lediglich 13 Monate.

Zusammenfassung Hintergrund Die von der Weltgesundheitsorganisation (WHO) herausgegebene Klassifikation der Tumoren des zentralen Nervensystems (ZNS) wurde 2016 überarbeitet, um molekulare Biomarker aufzunehmen, die für die Diagnosestellung und klinische Entscheidungsfindung wichtig sind. Danach verfeinerte ein internationales Konsortium die ZNS-Tumor-Klassifikation durch einige Empfehlungen, die aktuell in die neue WHO-Klassifikation 2021 eingeflossen sind. Fragestellung Welche Neuerungen in der WHO-Klassifikation 2021 haben direkte Auswirkungen auf die Diagnostik und Behandlung von erwachsenen Patienten mit diffusen Gliomen? Material und Methoden Die diagnostischen Kriterien der WHO-Klassifikation 2021 für diffuse Gliome wurden bezüglich dieser Fragestellung untersucht. Ergebnisse Mutationen in den Isocitratdehydrogenase(IDH)-Genen 1 oder 2 spielen eine entscheidende Rolle bei der Klassifikation von Gliomen. Unter den IDH-mutierten Gliomen identifiziert der Verlust der nukleären ATRX-Expression IDH-mutierte Astrozytome, während der Nachweis einer 1p/19q-Kodeletion für Oligodendrogliome diagnostisch ist. Die Nomenklatur für das IDH-mutierte Glioblastom wurde in Astrozytom, IDH-mutiert, ZNS-WHO-Grad 4 geändert. Die homozygote Deletion des CDKN2A/B-Genlocus ist ein molekularer Marker für diese Tumoren. Die Bezeichnungen „anaplastisches Astrozytom“ bzw. „anaplastisches Oligodendrogliom“ für IDH-mutierte Gliome des ZNS-WHO-Grads 3 entfallen. Diffuse astrozytäre Gliome ohne IDH-Mutation, die eine Mutation im Promotor des Telomerase-Reverse-Transkriptase(TERT)-Gens, eine Amplifikation des epidermalen Wachstumsfaktorrezeptorgens (EGFR), einen kombinierten Gewinn von Chromosom 7 und Verlust von Chromosom 10 (+7/−10) oder mehrere dieser Veränderungen aufweisen, werden jetzt als Glioblastom, IDH-Wildtyp, ZNS-WHO-Grad 4 klassifiziert, auch wenn histologisch weder pathologische Gefäßproliferate noch Nekrosen nachweisbar sind. Zudem wurden neue Gliome vom pädiatrischen Typ eingeführt, die vornehmlich bei Kindern, Jugendlichen und jungen Erwachsenen auftreten und von den o. g. häufigen Gliomen vom adulten Typ differenziert werden. Diskussion Die WHO-Klassifikation 2021 hat neben neuen Tumortypen auch grundlegende Veränderungen auf dem Boden neuer molekularpathologischer Erkenntnisse berücksichtigt, welche die diagnostische Präzision erhöhen und die klinische Versorgung durch modifizierte Behandlungsempfehlungen verbessern. Die neue Klassifikation hat zudem große Auswirkungen auf das Design zukünftiger klinischer Studien in der Neuroonkologie.

Background: Mutations in isocitrate dehydrogenase 1 (IDH1) and isocitrate dehydrogenase 2 (IDH2) are frequent in lowgrade and high-grade gliomas. However, the diagnostic criteria, in particular for gliomas, are highly various. The aim of our study was to establish genetic profiles for mutation and calcification of diffuse gliomas and to evaluate their predictive factors. Methods: We estimate the different clinical and molecular characterization between IDH1, IDH2 mutant gliomas, p53, ATRX and 1p19q. In addition, whole-transcriptome sequencing and DNA extraction data were used to evaluate the distribution of genetic changes in IDH1 and IDH2 mutant gliomas in a Iranian high grade glioma. Results: Between 2016-2019, among 53 gliomas in our study, 29 cases (54.7% %) harbored an IDH1,2 mutation, 21 cases (39.6 %) harbored an p53 mutation and 19 cases (35.8 %) harbored an ATRX. In addition, 1p19q co-deletion mutation was found in 7 cases (12.2%). We found that IDH1 and IDH2 are mutually entirely in gliomas. There was no significant relation between histopathology, tumor location and clinical finding with diagnosed mutations. Conclusion: Our study discloses an associated distinction between IDH1 and IDH2 mutant gliomas nearly in half of patients, followed by p53. These mutations should be reviewed separately because their differences could have indication for the diagnosis and treatment of IDH1/2 mutant gliomas.

Les gliomes diffus de bas grade sont des tumeurs cérébrales des jeunes adultes. Dans cette thèse, nous nous intéressons à la segmentation et à la modélisation de ces tumeurs. Dans la première partie du manuscrit, nous étudions la segmentation des gliomes diffus de bas grade à base de différentes méthodes manuelles et semi-automatiques. La délimitation de ces tumeurs peut être problématique en raison de leur caractère très infiltrant et inhomogène. En pratique clinique, le suivi des gliomes diffus de bas grade repose sur l'estimation du volume tumoral, soit par une segmentation suivie d'une reconstruction logicielle, soit par la méthode des trois diamètres. Pour la segmentation, elle est manuelle et est exécutée par des praticiens sur des IRM en pondération FLAIR ou T2. La méthode des trois diamètres est rapide mais s'avère difficile à implémenter dans le cas de gliomes diffus de bas grade très infiltrants ou en post-traitement. La solution par segmentation manuelle et reconstruction logicielle du volume est chronophage mais demeure plus précise en comparaison de la méthode des trois diamètres. Nous étudions ici la reproductibilité de la segmentation manuelle avec le logiciel OsiriX en réalisant un test subjectif dans le Living Lab PROMETEE de TELECOM Nancy. Les résultats de cette étude montrent que ni la spécialité du praticien ni le nombre d’années d’expérience ne semblent impacter significativement la qualité de la segmentation. Nous comparons par ailleurs les résultats obtenus à ceux d'un deuxième test où nous appliquons la méthode des trois diamètres. Enfin, nous explorons deux algorithmes de segmentation semi-automatique basés, respectivement, sur les contours actifs et sur la méthode des level set. Même si la segmentation automatique semble être une voie prometteuse, nous recommandons aujourd’hui l’utilisation de la segmentation manuelle du fait notamment du caractère diffus des gliomes de bas grade qui rend le contour complexe à délimiter. La seconde partie du manuscrit est consacrée à la modélisation des gliomes diffus de bas grade eux-mêmes ou, plus exactement, à la modélisation de l'évolution du diamètre tumoral en phase de chimiothérapie. La prise en charge thérapeutique des patients atteints de ces tumeurs inclut en effet souvent une chimiothérapie. Pour ce travail, nous nous intéressons à la chimiothérapie par Témozolomide en première ligne de traitement. Une fois le traitement entamé, les praticiens aimeraient déterminer l'instant optimal d'arrêt de traitement. Nous proposons une modélisation statistique du diamètre tumoral sous chimiothérapie. Cette modélisation s'appuie sur des modèles de régression linéaire et exponentielle. Elle permet de prédire le diamètre tumoral à partir d'un jeu de données d'apprentissage et d'alerter le clinicien sur l'état d'évolution du diamètre sous traitement. Nous espérons que ces modèles pourront un jour être utilisés comme un outil dans la planification de la chimiothérapie en milieu clinique<br>Diffuse low-grade gliomas are brain tumors of young adults. In this thesis, we focus on the segmentation and on the modeling of these tumors. The manual segmentation and software-based volume reconstruction solution is time-consuming but it remains more accurate in comparison with the three diameters method. We investigate in this work the reproducibility of the manual segmentation with the OsiriX software by performing a subjective test in the Living Lab PROMETEE in TELECOM Nancy. The results of this study show that neither the practitioners' specialty nor their number of years of experience seem to have a significant impact on the quality of the segmentation. We also compare the results to those of a second test where we apply the three diameters method. Finally, we explore two semi-automatic segmentation algorithms which are, respectively, based on active contours and on the level set method. Even if automatic segmentation seems to be a promising avenue, we recommend for now the use of manual segmentation because of the diffuse nature of low-grade gliomas, which makes the tumor's contours complex to delineate. The second part of the manuscript is dedicated to the modeling of diffuse low-grade gliomas themselves or, to be more precise, to the modeling of the evolution of the tumor's diameter during chemotherapy. The therapeutic management of patients with these tumors often includes indeed chemotherapy. For this work, we focus on Temozolomide chemotherapy in first-line treatment. After the beginning of the treatment, the practitioners would like to determine the optimum time of discontinuation. We propose a statistical modeling of tumor diameter under chemotherapy. This modeling is based on linear and exponential regression models. It can predict the tumor diameter from a set of training dataset and can alert the clinician on the state of change in diameter under treatment. We hope that these models will, eventually, be used as a tool in the planning of chemotherapy in a clinical environment

Les gliomes diffus de bas grade (GBG) sont des tumeurs cérébrales primaires. Après une phase de croissance lente, ils évoluent en gliomes de haut grade, entrainant une issue fatale. Ce sont des tumeurs très diffuses donc difficiles à traiter. Une meilleure connaissance de ces tumeurs pourrait permettre de les guérir ou, à défaut, d'optimiser les traitements. Nous avons étudié la croissance des GBG grâce à un modèle mathématique simple, ce qui nous a amené à spéculer (i) qu'ils surviennent à l'adolescence, (ii) que l'âge de la tumeur au moment du diagnostic peut être calculé facilement et (iii) que la vitesse de croissance est un facteur pronostique important. Cette dernière prédiction concorde avec les observations cliniques. Pour vérifier ce modèle spatial, nous avons caractérisé quantitativement des tissus de biopsies étagées de GBG humains, en particulier la présence d'œdème. L'analyse de ces données microscopiques étaie l'idée que l'œdème est à l'origine de l'anomalie de signal IRM en séquence T2. Pour prendre en compte ce résultat nouveau, nous avons incorporé l'œdème au modèle initial comme conséquence de la présence de cellules tumorales. Ce modèle permet d'expliquer la longue décroissance du rayon tumoral pendant des dizaines de mois après la radiothérapie : les cellules tumorales désormais moins nombreuses, le drainage de l'œdème devient prédominant. Ce modèle, qui ne comprend que trois paramètres libres, a été validé grâce à des données cliniques sur une vingtaine de patients<br>Diffuse low-grade gliomas (LGG) are primary brain tumors. After a slow growth, they evolve to high-grade gliomas, resulting into death. These tumors are very diffuse, thus diffîcult to treat. A better knownledge of them could allow to cure them or, failing that, to optimize treatments. We studied the growth of LGG with a simple mathematical model, which led us to speculate (i) that they arise in adolescence, (ii) that the age of the tumor at diagnosis can be calculated easily, and (iii) that the growth rate is an important prognostic factor. This last prediction is consistent with clinical observations. To test this spatial model, we have quantitatively characterized biopsy tissues of human LGG, particularly the presence of edema. The microscopic analysis of these data underpins the idea that edema is the cause of the abnormality seen on T2-weighted MR imaging. To take this new result into account, we have incorporated edema into the initial model as a consequence of the presence of tumor cells. This model helps explain the long decay of the tumor radius for tens of months after radiation therapy: as tumor cells become less numerous, drainage of the edema becomes predominant. This model, which has only three free parameters, has been validated thanks to clinical data from twenty patients

The results of this study show that neither the practitioners' specialty nor their number of years of experience seem to have a significant impact on the quality of the segmentation. We also compare the results to those of a second test where we apply the three diameters method. Finally, we explore two semi-automatic segmentation algorithms which are, respectively, based on active contours and on the level set method. Even if automatic segmentation seems to be a promising avenue, we recommend for now the use of manual segmentation because of the diffuse nature of low-grade gliomas, which makes the tumor's contours complex to delineate. The second part of the manuscript is dedicated to the modeling of diffuse low-grade gliomas themselves or, to be more precise, to the modeling of the evolution of the tumor's diameter during chemotherapy. The therapeutic management of patients with these tumors often includes indeed chemotherapy. For this work, we focus on Temozolomide chemotherapy in first-line treatment. After the beginning of the treatment, the practitioners would like to determine the optimum time of discontinuation. We propose a statistical modeling of tumor diameter under chemotherapy. This modeling is based on linear and exponential regression models. It can predict the tumor diameter from a set of training dataset and can alert the clinician on the state of change in diameter under treatment. We hope that these models will, eventually, be used as a tool in the planning of chemotherapy in a clinical environment

Les gliomes sont les principales tumeurs primitives du cerveau affectant environ 4000 nouveaux patients par an en France. La moitié des gliomes est détectée au stade avancé de glioblastome (grade IV) tandis que 15% des tumeurs sont diagnostiquées au stade II de gliomes diffus dit de bas grade. Ces tumeurs affectent des patients jeunes et présentent des mutations caractéristiques, notamment une mutation pour l’enzyme IDH1 communément retrouvée dans les glioblastomes secondaires. Ces tumeurs de bas grade sont traitées par une chirurgie, idéalement en condition éveillée mais du fait de leur nature diffuse, la partie résiduelle progressera inexorablement vers un stade III ou IV avec une survie globale entre 5 ans et 15 ans après diagnostique. La progression tumorale est hautement variable et non prédictible d’un patient à l’autre. Des foyers de progression tumorale chez 20% des patients atteints de gliome diffus de bas grade ont été identifiés. Ces foyers montrent une densité cellulaire plus élevée ainsi qu’un Ki67 augmenté. Mon travail de thèse aura consisté à étudier les modifications cellulaires et moléculaires associées à ces foyers de progression tumorale. À partir du profil ARN des foyers et des territoires adjacents, j’ai pu mettre en évidence par des techniques haut-débit la baisse d’expression significative de gènes dans les foyers notamment de AGXT2L1/ETNPPL, carboxypeptidase E, EDNRB, SFRP2. J’ai émis l’hypothèse que SFRP2 et ETNPLL pourraient s’opposer à la prolifération cellulaire et que leur diminution dans les foyers ouvrirait la voie à la transformation tumorale. Une corrélation inverse entre la quantité d’ETNPPL enzyme et la survie de patients atteints d’hépatocarcinomes a été publiée. En limitant la quantité de précurseurs de phospholipides dans la cellule, ETNPPL pourrait agir comme un frein en s’opposant à la prolifération et de fait, sa diminution dans les foyers de transformation des gliomes pourrait lever cette inhibition. Mes travaux auront été innovants tant dans leur approche comparative des différents compartiments tumoraux pour chaque patient étudié et auront révélés ETNPPL comme corrélé à la gliomagenèse et potentielle cible thérapeutique<br>Gliomas are the main primary brain tumours affecting around 4000 new patients in France each year. Half of gliomas are detected in the advanced stage of glioblastoma (grade IV) while 15% of tumours are diagnosed in stage II (diffuse low-grade gliomas-DLGG). These tumors affect young patients and bear characteristic mutations, including a mutation for the enzyme IDH1 commonly found in secondary glioblastomas. These low-grade tumours are treated by surgery, ideally in awake condition but due to their diffuse nature, the residual part will progress inexorably to stage III or IV with overall survival between 5 and 15 years after diagnosis. Tumor progression is highly variable and unpredictable from one patient to another. Foci of tumor progression have been identified in 20% of patients with DLGG. These foci show a higher cell density and an increased Ki67. My thesis work consisted in studying the cellular and molecular changes associated with tumor progression. From the RNA profile of the foci and adjacent territories, I was able to highlight through high-throughput techniques significant decrease in gene expression in the foci, particularly of AGXT2L1/ETNPPL, carboxypeptidase E, EDNRB, SFRP2. I hypothesized that SFRP2 and ETNPLL could oppose cell proliferation and that their decrease would pave the way for tumor transformation. An inverse correlation between the amount of ETNPPL and the survival of patients with hepatocarcinoma has been published. By limiting the amount of phospholipid precursors in the cell, ETNPPL could act as a brake against proliferation and indeed, its decrease in glioma transformation foci could remove this inhibition. My PhD work will have been innovative in the comparative approach of the different tumors’ compartments for each patient studied and will have revealed ETNPPL as correlated to gliomagenesis and as potential therapeutic target

Les découvertes récentes en neuropsychologie ont permis de passer d’une conception localisationniste à une conception en réseaux dynamiques du fonctionnement cérébral. Cette révolution conceptuelle, d’un cerveau immuable à remodelable, a été étayée par l’étude des patients porteurs d’un gliome de bas grade, une tumeur primaire associée à des phénomènes majeurs de plasticité cérébrale. Toutefois, on ne sait pas actuellement si cette neuroplasticité peut s’étendre aux fonctions de haut-niveau, celles qui participent à la conscience qu’ont les individus d’eux-mêmes. L’objectif de ce travail de thèse était d’évaluer dans quelle mesure les résections chirurgicales de gliomes affectent les processus métacognitifs et les traits de personnalité, en utilisant des méthodes de corrélations anatomo-fonctionnelles qui s’appuient sur la localisation lésionnelle et les déconnexions cérébrales. Dans un premier temps, nous avons montré que des résections frontales, unilatérales et bilatérales, n’induisaient pas de trouble métaperceptif, alors que le cortex préfrontal est supposé être central dans la métacognition. De même, nos résultats suggèrent que des résections chirurgicales massives n’affectent que très peu les traits de personnalité. Néanmoins, quelques traits comme la schizotypie positive et des troubles du comportement, comme l’anosognosie, étaient associés à l’atteinte de certains faisceaux de substance blanche<br>Recent findings in the field of neuropsychology have allowed to move from a localized to a dynamic network approach of brain functions. This paradigmatic shift, from a static to a reshaping brain, has been supported by the investigation of patients with low-grade glioma, a neurological tumor known to trigger processes of compensation and rescue of brain functions. However, it is currently unestablished whether this neuroplastic compensation may extend to higher-order cognitive functions, specifically those involved in self-consciousness. By using both anatomo-functional correlational methods based on lesions localization and structural disconnection approach, the purpose of this work was to assess the extent to which the neurosurgical resections of low-grade glioma affect metacognitive processes and personality traits. First, we showed that frontal lobectomies, both unilateral and bilateral, did not induce metaperceptive impairments despite the established role of the prefrontal cortex in metacognition. Likewise, our results suggest that massive surgical resections did not significantly affect personality traits. However, some of them such as positive schizotypy, and a few behavioral modifications, such as anosognosia, were found to be associated with the disruption of some white matter bundles

Les gliomes diffus de bas grade (GBG) sont des tumeurs cérébrales primaires. Après une phase de croissance lente, ils évoluent en gliomes de haut grade, entrainant une issue fatale. Ce sont des tumeurs très diffuses donc difficiles à traiter. Une meilleure connaissance de ces tumeurs pourrait permettre de les guérir ou, à défaut, d'optimiser les traitements. Nous avons étudié la croissance des GBG grâce à un modèle mathématique simple, ce qui nous a amené à spéculer (i) qu'ils surviennent à l'adolescence, (ii) que l'âge de la tumeur au moment du diagnostic peut être calculé facilement et (iii) que la vitesse de croissance est un fac- teur pronostique important. Cette dernière prédiction concorde avec les observations cliniques. Pour vérifier ce modèle spatial, nous avons caractérisé quantitativement des tissus de biopsies étagées de GBG humains, en particulier la présence d'œdème. L'analyse de ces données micro- scopiques étaie l'idée que l'œdème est à l'origine de l'anomalie de signal IRM en séquence T2. Pour prendre en compte ce résultat nouveau, nous avons incorporé l'œdème au modèle initial comme conséquence de la présence de cellules tumorales. Ce modèle permet d'expliquer la longue décroissance du rayon tumoral pendant des dizaines de mois après la radiothérapie : les cellules tumorales désormais moins nombreuses, le drainage de l'œdème devient prédominant. Ce mo- dèle, qui ne comprend que trois paramètres libres, a été validé grâce à des données cliniques sur une vingtaine de patients.

Comprendre comment le cerveau humain engendre les formes les plus élaborées de comportements est profondément lié à nos connaissances générales sur son organisation anatomique et fonctionnelle. Jusqu'à récemment encore, on pensait que les fonctions cognitives n'étaient rien d'autre que le sous-produit de l'activité neurale de régions corticales discrètes et hyper-fonctionnalisées. Les découvertes majeures obtenues ces dix dernières années dans le champ de la neuro-imagerie, et plus particulièrement de la connectomique, invitent cependant à complexifier nos représentations sur les liens qu'entretiennent structures et fonctions cérébrales. Le cerveau semble en effet être organisé en systèmes neurocognitifs complexes, hautement distribués et plastiques. C'est dans cet esprit qu'a été réalisé ce travail de thèse dont l'ambition première a été de repenser les modèles actuels de la cognition sociale, et en particulier ceux ayant trait à la fonction de mentalisation, à travers l'étude comportementale des patients porteurs d'un gliome diffus de bas-grade. Cette tumeur neurologique rare constitue un excellent modèle physiopathologique en vue du démasquage des structures maîtresses des systèmes cognitifs complexes, en ce qu'elle induit des phénomènes majeurs de réorganisation fonctionnelle, et s'infiltre préférentiellement le long de la connectivité axonale associative. Des corrélations anatomo-cliniques ont été réalisées suivant une approche topologique classique (analyse de groupe en régions d'intérêt, cartographie voxel-based lesion-symptom, stimulation électrique corticale intra-opératoire) mais également hodologique (degré de déconnection des faisceaux d'association, stimulation électrique de la connectivité axonale). Les résultats principaux de nos différents travaux nous permettent de jeter les premières bases d'un modèle à double voie dynamique (plastique) et hodotopique (contraint par la réalité anatomique) de l'organisation anatomo-fonctionnelle des processus de mentalisation. Spécifiquement, une voie dorsale, interconnectant le aires corticales fronto-pariétales « miroirs » via le système périsylvien de substance blanche associative (faisceau arqué et faisceau longitudinal supérieur latéral), sous-tendrait les processus perceptifs de « bas-niveau » nécessaires à l'identification préréflexive des états mentaux ; une voie cingulo-médiane, interconnectant les régions préfrontales médiales et rostro-cingulaires aux régions pariétales postérieures médiales via le faisceau cingulaire, sous-tendrait les processus de «haut-niveau » nécessaires aux inférences mentalistiques conscientes. Ces découvertes constituent une avancée substantielle en neurosciences sociales, ont des implications importantes pour la prise en charge clinique des patients, et peuvent permettre de mieux comprendre certaines psychopathologies caractérisées à la fois par un trouble de la mentalisation et des anomalies structurales de la connectivité associative (troubles du spectre autistique)<br>Understanding how the brain produces sophisticated behaviours strongly depends of our knowledge on its anatomical and functional organization. Until recently, it was believed that high-level cognition was merely the by-product of the neural activity of discrete and highly specialized cortical areas. Major findings obtained in the past decade from neuroimaging, particularly from the field of connectomics, prompt now researchers to revise drastically their conceptions about the links between brain structures and functions. The brain seems indeed organized in complex, highly distributed and plastic neurocognitive networks. This is in this state of mind that our work has been carried out. Its foremost ambition was to rethink actuals models of social cognition, especially mentalizing, through the behavioural study of patients harbouring a diffuse low-grade glioma. Because this rare neurological tumour induces major functional reorganization phenomena and migrates preferentially along axonal associative connectivity, it constitutes an excellent pathophysiological model for unmasking the core structures subserving complex cognitive systems. Anatomo-clinical correlations were conducted according to both a classical topological approach (region of interest analyses, voxel-based lesion-symptom mapping, intraoperative cortical electrostimulation) and a hodological approach (degree of disconnection of associative white matter fasciculi, intraoperative axonal connectivity mapping). The main results of our different studies enable us to lay the foundation of a dynamic (plastic) and hodotopical (connectivity) dual-stream model of mentalizing. Specifically, a dorsal stream, interconnecting mirror frontoparietal areas via the perisylvian network (arcuate fasciculus and lateral superior longitudinal fasciculus), may subserve low-level perceptual processes required in rapid and pre-reflective identification of mental states; a cingulo-medial stream, interconnecting medial prefrontal and rostro-cingulated areas with medial posterior parietal areas via the cingulum, may subserve higher-level processes required in reflective mentalistic inferences. These original findings represents a great step in social neuroscience, have major implications in clinical practice, and opens new opportunities in understanding certain pathological conditions characterized by both mentalizing deficits and aberrant structural connectivity (e.g. autism spectrum disorders)

Contexte : Les gliomes diffus sont les tumeurs primitives malignes du système nerveux central les plus fréquentes. Certains présentent une mutation du gène codant pour l'isocitrate déshydrogénase 1 ou 2 (IDH) qui leur confère la néo-activité de transformer l'alpha-cétoglutarate (α-KG) produit par l'enzyme non mutée en D-2hydroxyglutarate (D-2HG). L'accumulation de cet oncométabolite entraîne l'inhibition compétitive des dioxygénases dépendantes de l'α-KG, incluant la famille TET (Ten Eleven Translocase) des ADN hydroxylases, et les JmjC/KMDs histone déméthylases, qui conduit elle-même à l'hyperméthylation des histones et de l'ADN des cellules tumorales. Les macrophages et microglies associés aux tumeurs (TAMs) sont des cellules myéloïdes très abondantes dans les gliomes, dont le phénotype et la réponse immunitaire sont déterminés par l'ontogenèse et le microenvironnement. Les TAMs présentent des caractéristiques différentes en fonction du statut IDH du gliome, mais les mécanismes de régulation sous-jacents restent largement méconnus. Le D-2HG relargué dans le microenvironnement par les cellules de gliomes IDH-mutants (IDH-m) pourrait influencer le phénotype et la fonction de ces TAMs. Nous avons émis l'hypothèse que le D-2HG pourrait modifier l'épigénome des TAMs, comme dans le cas des cellules tumorales.Matériel et méthodes : Nous avons comparé le méthylome (puce EPIC de méthylation) et le transcriptome des TAMs (CD11B+ isolés par tri cellulaire activé par magnétisme) de 25 gliomes IDH-m et 11 gliomes IDH-wildtype (IDH-wt), ainsi que de tissu cérébral contrôle non tumoral. Pour déterminer les effets directs du D-2HG sur la microglie, nous avons mis au point des cultures primaires de microglie humaine obtenues à partir d'une chirurgie de gliome ou d'épilepsie. Nous avons d'abord dosé le D-2HG dans la microglie traitée par D-2HG par spectrométrie de masse par chromatographie en phase liquide (LC-MS) afin d'évaluer l'entrée du métabolite dans ces cellules, et avons évalué l'activité enzymatique de TET. Nous avons exposé ou non des cellules microgliales au D-2HG pendant 14 jours et analysé leur méthylome et leur transcriptome. Les ratios de 5mC/5hmC ont été analysés à une résolution de base unique. Nous avons évalué la réponse transcriptomique et la respiration mitochondriale après stimulation au LPS de la microglie pré-traitée par D-2HG. Enfin, nous avons analysé par snRNA-seq le transcriptome microglial d'un échantillon apparié de gliome IDH-m avant et après traitement par l'inhibiteur d'IDH-m ivosidenib, qui diminue les concentrations intratumorales de D-2HG.Résultats : Nous montrons que les cellules myéloïdes CD11B+ des gliomes IDH-m humains présentent une hyperméthylation de l'ADN principalement au niveau des enhancers. Cette hyperméthylation est liée à une diminution de l'expression des gènes impliqués dans les réponses inflammatoires et le métabolisme glycolytique, et à l'inactivation des facteurs de transcription qui régulent la réponse microgliale aux stimuli environnementaux. L'exposition prolongée de la microglie primaire humaine au D-2HG inhibe l'oxydation de 5mC médiée par TET, ce qui entraîne une accumulation réduite des niveaux globaux de 5hmC. Nous avons observé des rapports 5mC/5hmC élevés, en particulier au niveau d'enhancers spécifiquement actifs dans ces cellules. Conformément à la modulation des enhancers, la microglie traitée au D-2HG présente une capacité pro-inflammatoire réduite et une augmentation de la phosphorylation oxydative. À l'inverse, la diminution des niveaux de D-2HG après traitement par ivosidenib chez un patient atteint de gliome IDH-m est associée à la restauration de l'expression génique liée à l'activation microgliale.Conclusion : Nos résultats fournissent une base mécanistique de l'état hyporéactif de la microglie dans les gliomes IDH-m et élargissent le concept selon lequel les oncométabolites peuvent perturber la fonction des cellules immunitaires du microenvironnement tumoral<br>Background: Diffuse gliomas are the most common primary malignant tumors of the central nervous system. A subset of these tumors harbors mutations in the genes encoding isocitrate dehydrogenase 1 or 2 (IDH), which confer a neo-activity that converts alpha-ketoglutarate (α-KG) produced by the wildtype enzyme into D-2-hydroxyglutarate (D-2HG). The accumulation of this oncometabolite competitively inhibits α-KG-dependent dioxygenases, including the TET (Ten Eleven Translocase) family of DNA hydroxylases and the JmjC/KDM family of histone demethylases. This ultimately leads to hypermethylation of both histones and DNA in tumor cells. Tumor-associated macrophages and microglia (TAMs) are abundant myeloid cells in gliomas, with their phenotype and immune responses shaped by ontogeny and the tumor microenvironment. The characteristics of TAMs differ depending on the IDH status of the glioma, yet the regulatory mechanisms underlying these differences remain poorly understood. D-2HG released by IDH-mutant (IDH-m) glioma cells into the microenvironment may affect the phenotype and function of TAMs. We hypothesized that D-2HG may influence the epigenome of TAMs, as in the case of tumor cells.Materials and Methods: We compared the bulk DNA methylome (Methylation EPIC array) and transcriptome of TAMs (CD11B+ cells purified via magnetic-activated cell sorting) from 25 IDH-mutant (IDH-m) and 11 IDH-wildtype (IDH-wt) gliomas, as well as control tissues. To experimentally assess the direct effects of D-2HG, the oncometabolite produced and released by IDH-m glioma cells, we used primary cultures of human microglial cells obtained from glioma or epilepsy surgeries. We first measured D-2HG levels in D-2HG-treated microglia using liquid chromatography-mass spectrometry (LC-MS) to confirm metabolite uptake, and evaluated TET enzymatic activity. Bona fide IDH-m and IDH-wt cells were used as external controls. Microglia were exposed to D-2HG for 14 days, after which we analyzed their DNA methylome and transcriptome. Ratios of 5mC/5hmC were examined at single-base resolution. We evaluated the transcriptomic response and the mitochrondrial respiration after LPS stimulation of microglia pre-treated with D-2HG. Lastly, we performed single-nuclei RNA sequencing (snRNA-seq) on microglial cells from a paired IDH-m glioma sample, before and after treatment with the IDH-m inhibitor ivosidenib, known to reduce intratumoral D-2HG levels.Results: Our analysis revealed that CD11B+ myeloid cells in human IDH-m gliomas exhibit DNA hypermethylation predominantly at distal enhancers. This hypermethylation is associated with decreased expression of genes involved in inflammatory responses and glycolytic metabolism, as well as the inactivation of transcription factors critical for microglial response to environmental stimuli. Prolonged exposure of primary human microglia to D-2HG inhibited TET-mediated 5mC oxidation, leading to reduced global 5hmC accumulation. High 5mC/5hmC ratios were particularly prominent at lineage-specific enhancers. Consistent with this altered enhancer landscape, D-2HG-treated microglia demonstrated diminished proinflammatory capacity and enhanced oxidative phosphorylation. Conversely, depletion of D-2HG following ivosidenib treatment in an IDH-m glioma patient was associated with the restoration of microglial gene expression related to activation.Conclusion: Our findings provide mechanistic insight into the hyporesponsive state of microglia in IDH-m gliomas and support the concept that oncometabolites, such as D-2HG, can disrupt the function of immune cells in the tumor microenvironment

Le Gliome Diffus de Bas Grade (GDBG) est défini par l’OMS comme une tumeur primitive du système nerveux central et représente 15% de toutes les tumeurs gliales confondues. Un GDBG croît lentement, et finit inévitablement par se transformer en un gliome de grade III, bien plus agressif, qui mène finalement au décès du patient. Trois types de traitements sont disponibles : La chirurgie, la chimiothérapie et la radiothérapie. Aujourd’hui, les médianes de survie rapportées dans les études sont de 10 à 15 ans. Malheureusement, le pronostic du GDBG est très variable, l’écart-type de la survie totale étant élevé, et certains patients ne survivant que quelques années. Dans le cadre de la prise en charge du GDBG au CHRU de Nancy, la chimiothérapie est un des traitements les plus utilisés et on observe des réponses très variables tant en intensités, qu’en durées ainsi qu’en terme de profils de réponses. C’est dans ce contexte que se situe le travail de thèse. Il concerne l’étude de la réponse à la chimiothérapie et consiste à développer des outils d’aide à la décision pour le neuro-oncologue dans le suivi des patients. La première partie de mon travail de thèse se concentre donc sur le choix de la méthode de volumétrie. On peut ensuite caractériser la courbe de réponse volumique selon l’intensité de réponse. La seconde partie de ce travail concerne la modélisation de la réponse à l’aide de techniques d’apprentissage statistique. De nombreuses variables explicatives (épidémiologiques, génétiques sont à prendre en compte dans cette étude, dont une nouvelle variable nommée ESVR, qui est une mesure permettant de quantifier le phénotype infiltrant des GDBG. Les méthodes d’analyse factorielle et de machine learning permettent dans un premier temps de définir les variables qui portent le plus d’information. Les analyses exploratoires des données révèlent une redondance de l’information parmi certains facteurs génétiques et épidémiologiques. Les modèles montrent une plus grande influence des variables quantitatives sur la réponse à la chimiothérapie, comparé aux variables qualitatives. Une discussion sera menée sur l’importance des variables utilisées dans la prédiction de la réponse à la chimiothérapie. La finalité de la thèse est de produire un ensemble de règles qui permettront aux cliniciens d’anticiper, avant l’administration du traitement, son effet sur le volume tumoral ce qui permettra un choix de stratégie thérapeutique plus éclairé qu’aujourd’hui<br>Diffuse Low-Grade Glioma (DLGG) is defined by the WHO as a primary tumour of the central nervous system and represents 15% of all glial tumours combined. A DLGG grows slowly, and inevitably evolve into a much more aggressive (grade III) glioma, which eventually leads to the death of the patient. Three types of treatment are available: surgery, chemotherapy and radiotherapy. Today, the median survival rates reported in studies varies from 10 to 15 years. Unfortunately, the prognosis for DLGG is highly variable, with a high standard deviation of total survival, and some patients are surviving only a few years. Within the framework of DLGG management at Nancy University Hospital, chemotherapy is one of the most widely used treatments and there are very variable responses in terms of intensity, duration and response profiles. The thesis work is located in this context. It concerns the study of the response to chemotherapy and consists in developing decision-making tools for the neuro-oncologist in the follow-up of patients. The first part of this thesis work therefore focuses on the choice of the volumetric method. The volume response curve can then be characterised in terms of response intensity. The second part of this work concerns response modelling using statistical learning techniques. Many explanatory variables (epidemiological, genetic) are under study. A new variable called ESVR, which is an original measure allowing us to quantify the infiltrating DLGG phenotype, will also be used. The factorial analysis and machine learning methods initially make possible to define the variables that provide the most information. Exploratory analyses of the data reveal a redundancy of information among certain genetic and epidemiological factors. The models show a greater influence of quantitative variables on the response to chemotherapy compared to qualitative variables. A discussion is finally produced on the importance of the variables used in the prediction of the response to chemotherapy. The aim of this thesis is to produce a set of rules which will enable clinicians to anticipate, before administering the treatment, its effect on tumour volume, which will allow a more advised choice of therapeutic strategy than possible nowadays

Astrocytic gliomas are primary brain tumours thought to originate from neural stem or progenitor cells. They are assigned grades II, III, or IV by the World Health Organization according to degree of malignancy as defined by histology. The following molecular markers are increasingly used for diagnostic subclassification or clinical decision-making: 1p/19q co-deletion status, O6-methylguanine-DNA methyltransferase (MGMT) promoter methylation status, and isocitrate dehydrogenase 1 and 2 mutation status. Extent of resection is a favourable prognostic factor, but surgery is never curative. Radiotherapy prolongs progression-free survival across all astrocytic glioma entities. Alkylating agent chemotherapy is an active treatment in particular for patients with MGMT promoter-methylated tumours. Anti-angiogenic therapies have failed to improve survival, and the current focus of major clinical trials is on novel targeted agents or on immunotherapy.

Pontine gliomas-also known as diffuse intrinsic pontine gliomas (DIPG)-primarily occur in children and typically present subacutely with a combination of cranial nerve palsies associated with long track signs including hyper-reflexia in the legs, positive Babinski responses, and cerebellar signs. On imaging they typically appear as intrinsic mass lesions within the pons. Treatment with radiation can prolong the course of from months to years but the tumors are rarely curable. Chemotherapy combined with radiation therapy targeted at specific signaling pathways has shown only modest impact on survival.

The revised fourth edition of the WHO classification of Tumours of the Central Nervous System, published in 2016, comprises several newly recognized tumour entities, and a significant restructuring of the classification, mainly based on genetic profiling. Glioblastomas are now classified into two major types. Isocitrate dehydrogenase (IDH)-wildtype glioblastoma (primary glioblastoma IDH-wildtype) develops rapidly de novo without a recognizable precursor lesion. IDH-mutant glioblastoma (secondary glioblastoma IDH-mutant) develops more slowly through malignant progression from diffuse or anaplastic astrocytoma. Medulloblastomas are now defined by combining histological patterns (classic, desmoplastic/nodular, extensive nodularity, anaplastic) and genetic hallmarks (WNT-activated; SHH-activated, TP53-mutant; SHH-activated, TP53-wildtype; non-WNT/non-SHH). Other newly recognized tumour entities include diffuse midline glioma, H3 K27M-mutant; ependymoma, RELA fusion-positive; and embryonal tumour with multilayered rosettes. The new classification is a significant step forward and will facilitate the development of novel targeted therapies of brain tumours.

To cause disease, plant viruses must replicate and spread locally and systemically within the host. Cell-to-cell virus spread is mediated by virus-encoded movement proteins (MPs), which modify the structure and function of plasmodesmata (Pd), trans-wall co-axial membranous tunnels that interconnect the cytoplasm of neighboring cells. Tobacco mosaic virus (TMV) employ a single MP for cell- cell spread and for which CP is not required. The PIs, Beachy (USA) and Epel (Israel) and co-workers, developed new tools and approaches for study of the mechanism of spread of TMV that lead to a partial identification and molecular characterization of the cellular machinery involved in the trafficking process. Original research objectives: Based on our data and those of others, we proposed a working model of plant viral spread. Our model stated that MPᵀᴹⱽ, an integral ER membrane protein with its C-terminus exposed to the cytoplasm (Reichel and Beachy, 1998), alters the Pd SEL, causes the Pd cytoplasmic annulus to dilate (Wolf et al., 1989), allowing ER to glide through Pd and that this gliding is cytoskeleton mediated. The model claimed that in absence of MP, the ER in Pd (the desmotubule) is stationary, i.e. does not move through the Pd. Based on this model we designed a series of experiments to test the following questions: -Does MP potentiate ER movement through the Pd? - In the presence of MP, is there communication between adjacent cells via ER lumen? -Does MP potentiate the movement of cytoskeletal elements cell to cell? -Is MP required for cell-to-cell movement of ER membranes between cells in sink tissue? -Is the binding in situ of MP to RNA specific to vRNA sequences or is it nonspecific as measured in vitro? And if specific: -What sequences of RNA are involved in binding to MP? And finally, what host proteins are associated with MP during intracellular targeting to various subcellular targets and what if any post-translational modifications occur to MP, other than phosphorylation (Kawakami et al., 1999)? Major conclusions, solutions and achievements. A new quantitative tool was developed to measure the "coefficient of conductivity" of Pd to cytoplasmic soluble proteins. Employing this tool, we measured changes in Pd conductivity in epidermal cells of sink and source leaves of wild-type and transgenic Nicotiana benthamiana (N. benthamiana) plants expressing MPᵀᴹⱽ incubated both in dark and light and at 16 and 25 ᵒC (Liarzi and Epel, 2005 (appendix 1). To test our model we measured the effect of the presence of MP on cell-to-cell spread of a cytoplasmic fluorescent probe, of two ER intrinsic membrane protein-probes and two ER lumen protein-probes fused to GFP. The effect of a mutant virus that is incapable of cell-to-cell spread on the spread of these probes was also determined. Our data shows that MP reduces SEL for cytoplasmic molecules, dilates the desmotubule allowing cell-cell diffusion of proteins via the desmotubule lumen and reduces the rate of spread of the ER membrane probes. Replicase was shown to enhance cell-cell spread. The data are not in support of the proposed model and have led us to propose a new model for virus cell-cell spread: this model proposes that MP, an integral ER membrane protein, forms a MP:vRNAER complex and that this ER-membrane complex diffuses in the lipid milieu of the ER into the desmotubule (the ER within the Pd), and spreads cell to cell by simple diffusion in the ER/desmotubule membrane; the driving force for spread is the chemical potential gradient between an infected cell and contingent non-infected neighbors. Our data also suggests that the virus replicase has a function in altering the Pd conductivity. Transgenic plant lines that express the MP gene of the Cg tobamovirus fused to YFP under the control the ecdysone receptor and methoxyfenocide ligand were generated by the Beachy group and the expression pattern and the timing and targeting patterns were determined. A vector expressing this MPs was also developed for use by the Epel lab . The transgenic lines are being used to identify and isolate host genes that are required for cell-to-cell movement of TMV/tobamoviruses. This line is now being grown and to be employed in proteomic studies which will commence November 2005. T-DNA insertion mutagenesis is being developed to identify and isolate host genes required for cell-to-cell movement of TMV.
