O laser de baixa intensidade (LBI) tem sido amplamente utilizado no tratamento da hipersensibilidade dentinária. Pesquisas in vivo demonstraram que o LBI pode promover um aumento na síntese de matriz de dentina e menor grau de inflamação pulpar. Entretanto, o mecanismo que rege este processo permanece desconhecido. Assim, o objetivo desta pesquisa foi avaliar o metabolismo de células odontoblastóides em cultura quando submetidas à irradiação direta ou transdentinária (indireta) com LBI e seus efeitos sobre o complexo dentino-pulpar. Células odontoblastóides MDPC-23 foram cultivadas em placas de acrílico de 24 compartimentos ou sobre a superfície pulpar de discos de dentina, com 0,4 mm de espessura e obtidos de terceiros molares, adaptados em câmaras pulpares artificiais. As células foram irradiadas diretamente por 3 vezes (a cada 24 horas) com 2, 4, 10, 15 ou 25 J/cm2 e submetidas a avaliação de metabolismo (MTT), síntese de proteína total e de fosfatase alcalina. Os dois melhores parâmetros de irradiação direta foram utilizados para o ensaio de irradiação transdentinária (indireta) das células. Como resultado da etapa direta de irradiação, foi demonstrado que tanto os valores do MTT quanto os níveis de proteína total e fosfatase alcalina apresentaram valores estatisticamente superiores nas doses de 15 e 25 J/cm2 (Mann-Whitney p<0.05). Assim, estas doses foram usadas no teste de irradiação indireta, obtendo-se aumento do metabolismo, síntese de proteína e fosfatase alcalina estatisticamente superior apenas para a dose de 25 J/cm2 (Mann-Whitney p<0.05). Desta maneira, foi possível concluir, dentro das condições experimentais, que os maiores valores de bio-estimulação das células MDPC-23 ocorreram quando estas foram irradiadas com 25 J/cm2. Low level laser (LLL) has been widely used for treatment of dentin hypersensitivity. In vivo studies have shown increased synthesis of dentin matrix and less severe pulpal inflammation in teeth irradiated with LLL. However, the mechanisms that guide this process remain unknown. This study evaluated the metabolism of cultured odontoblastlike cells submitted to direct or transdentinal LLL irradiation as well as its effects on pulp tissue of first molars of rats. The odontoblast-like cells MDPC-23 were cultured (12,500 cells/cm2) on either 24-well acrylic plates or the pulpal surface of dentin discs adapted to artificial pulp chambers. In the first experiment, the cells received direct irradiation for 3 times (every 24 hours) with the following energy doses: 2 J/cm2, 4 J/cm2, 10 J/cm2, 15 J/cm2, and 25J/cm2. Then, the irradiated cells were evaluated concerning their metabolism, synthesis of total protein (TP) and alkaline phosphatase (ALP). The two best direct irradiation parameters obtained in this first protocol were selected to be used in the second experiment, in which the MDPC-23 cells were irradiated through 0.4mm dentin discs obtained from human teeth (indirect irradiation). For the direct assay, the MTT values as well as the TP and ALP levels were significantly higher at the doses of 15 and 25 J/cm2 (Mann-Whitney; p<0.05). When both of these energy doses were used for the indirect irradiation assay, it was observed a statistically significant increase in cell metabolism and synthesis of TP and ALP only for 25 J/cm2 (Mann-Whitney; p <0.05). Under the experimental conditions, it may be concluded that the highest biostimulation values were obtained when the MDPC-23 cells were irradiated with 25 J/cm2. Pesquisas recentes demonstraram que a irradiação transdentinária da polpa pode resultar em aumento na síntese de matriz de dentina e redução na resposta inflamatória local. Entretanto, os mecanismos que regem estes processos permanecem desconhecidos. Assim, o objetivo desta pesquisa foi investigar o efeito transdentinário do LED, em três comprimentos de onda (CO: 455nm, 630nm e 850nm) e duas doses de energia (DE: 4J/cm2 e 25 J/cm2), sobre células odontoblastóides MDPC-23 cultivadas em discos de dentina (molares humanos) com 0,2 mm de espessura. Foram realizadas análises do metabolismo celular (SDH), proteína total (PT) e fosfatase alcalina (ALP), através dos ensaios de MTT, Read Northcote e Ponto Final, respectivamente. O ensaio de RT-PCR foi aplicado para avaliação da expressão dos genes que codificam para colágeno tipo I (Col-1), fibronectina (FN) e fosfatase alcalina (ALP). Além disso, foi realizada a análise da morfologia celular em Microscopia Eletrônica de Varredura (MEV). Para a produção de PT, os resultados não apontaram diferença estatisticamente significante entre os grupos irradiados e controle (Mann-Whitney, p>0,05). Entretanto, para o metabolismo celular, o grupo irradiado com LED no CO de 630 nm, na DE de 25 J/cm2, obteve melhores resultados (aumento de 21,8 %), com diferença significante quando comparado ao grupo controle (Mann-Whitney, p<0,05). Para a produção de fosfatase alcalina, houve aumento significante pra todos os parâmetros utilizados (Mann-Whitney, p<0,05), com exceção da luz azul na dose de 4 J/cm2 (Mann-Whitney, p>0,05). Several studies have demonstrated that the transdentinal irradiation of dental pulp may increase the dentin matrix synthesis as well as decrease the local inflammatory reaction. However, the mechanisms that regulate these processes remain unknown. Therefore, the objective of this in vitro study was to investigate the transdentinal effect of LED irradiation at three different wavelengths (λ= 455, 630 and 850 nm) and two doses (4J/cm2 and 25 J/cm2) on odontoblast-like cells seeded on 0.2-mm-thick dentin disks obtained from sound human molars. Cell metabolism (MTT), alkaline phosphatase expression (ALP), total protein synthesis, and cell morphology (MEV) were evaluated. The expression of genes that encode for collagen type-1 (Col-1), fibronectin (FN) and alkaline phosphatase (ALP) was analyzed by RT-PCR. For total protein synthesis, the results showed no statistical difference among irradiated and control groups (Mann-Whitney test, p>0.05). However, for cellular metabolism, the group irradiated with 630 nm LED (dose of 25 J/cm2) showed better results (21,8% increase), with significant difference when compared to control group (Mann-Whitney test, p<0.05). For alkaline phosphatase activity, a significantly increase was observed for all parameters (Mann-Whitney test, p<0.05), except for the blue light at a dose of 4 J/cm2 (Mann-Whitney test, p>0.05). RT-PCR showed a higher expression of Col- 1 for the infrared LED at a dose of 4 J/cm2. A larger number of MDPC-23 cells with normal morphology adhered to the dentin discs was observed after irradiation with 25 J/cm2 when compared to 4 J/cm2, for all wavelengths evaluated. It may be concluded that LED irradiation was effective for transdentinal cell biostimulation and the cellular response was dose and wavelength-dependent.

Повний текст джерелаThe light propagation and distribution studies in biological media are importante for several phototherapy and diagmostic aplications. The biological tissues are considered turbida medias, where there is absortion and scattering combination, and determinate the light behavior inside this tissues have been studied by models that has some limitations.Then, to determine and control the light distribution to improve the light dose delivery is important for the treatment successful. For particular cases, such as Photodynamic Therapy (PDT) applied on smooth planar lesions treatments is appropriated, since there is a good possibility of uniform distribution of light within the tissue of the treated lesion. On the other hand, tumors with more thickness may not receive the needed doses of energy to cause its death. For an efficient treatment by photodynamic therapy is required an optimal coupling light inside the lesion tissue. Shadow effects, slits or physical irregularities in the lesion can lead a nonhomogeneous light distribution inside the tissue. The results can be a partial necrosis regions and tumor recurrence. Therefore it is crucial improving the light profile inside the tissue to overcome these problems. In this study, we measured the light profile inside the phantom after the light passing through a gel as coupler. We used a solid phantom as biological tissue model and was used a red laser (λ = 630 nm) as light source with an optical fiber to direct illumination. The coupling is controlled by introducing a gel with low concentration of scatters between the fiber and de phantom. Was collected pictures of light profile with a camera and the data were processed with MatLab software. Our results shows a strong improvement in the light distribution when the gel with scatters is positioned between fiber and tissue. In fact, a more homogeneous laser bean is delivered to tissue promoting a better light distribution. This technique eliminates drastically the roughness effect of the phantom surface in the bulk. We believe that this work shows a possibility of dosimetry improvement for phototherapy.

Повний текст джерелаAccording to the traditionally management used in dairy cattle, disbudding calves is a highly important routine procedure. Disbudding is usually performed by heat cauterization and presents itself as the main factor that leads to burns injuries in calves. The treatment of the lesions is based on the topical application of antimicrobial and repellents healing products, and in Veterinary Medicine, the most frequent used and present in commercial products, in order to improve the healing time, is zinc oxide. As an alternative to antibiotics and to improve wound healing, low level phototherapy and photodynamic therapy are presented as therapeutic options for tissue repair and against local infections in burn wounds. The aim of this study was to evaluate the results of phototherapy in the treatment of burn injuries induced by disbudding in calves, by macroscopic and histological features of the lesions, and the clinical recovery of the animals, and to compare with the traditional treatment with zinc oxide ointment. Thus, 30 calves were divided into five study groups: G1 control; G2 - red laser (RL), λ = 660 nm / 20 seconds of irradiation (70J / cm2), G3 - infrared laser (IR), λ = 810nm / 20 seconds of irradiation (70J / cm2); G4 - red LED (LED), λ = 660 nm / 40 seconds of irradiation (10,8J / cm2) and G5 - photodynamic therapy (PDT), methylene blue 0.01% associated with the red LED λ = 660 nm / 40 seconds of irradiation ( 10,8J / cm2). The results showed no statistical differences among the groups according to wound healing time, and to the type III collagen deposition. Decorridas 24 h, as células estimuladas com LPS foram avaliadas quanto à produção de óxido nítrico (NO), de espécies reativas de oxigênio (EROs) e viabilidade celular (ensaio de MTT) (n=8 por grupo). As células estimuladas com TNF-α foram avaliadas quanto à produção de citocinas pelos métodos de antibody array e q-PCR (n=5 por grupo). Os dados foram submetidos aos testes de Kruskal-Wallis e Mann-Whitney (α=0,05). Resultados: O contato com LPS resultou em aumento significativo da produção de NO sem causar qualquer dano ao metabolismo respiratório das células. Cells were seeded (105 cells/well) using α-MEM supplemented with 10% FBS and after 24h, were exposed to LPS (10 μg/mL α-MEM) or TNF-α (25ng/mL α- MEM). Then, HDPC were submitted to a single irradiation with an infrared LED (855 nm) delivering different doses of energy: 0, 2, 4, 8, 15 or 30 J/cm2. Nonirradiated cells, with and without LPS/TNF-α, served as controls. Followed 24h, the cells stimulated with LPS were tested for nitric oxide (NO) quantification, cell viability (MTT assay), and reactive oxygen species (ROS) production (n=8 per group). Cells stimulated with TNF-α were analyzed regarding cytokine production and expression using antibody array and q-PCR (n=5 per group). Data were submitted to Kruskal-Wallis and Mann-Whitney tests (α=0.05). Results: LPSinduced stress resulted in significant increase in NO production by HDPC without causing any damage to cell respiratory metabolism. Irrespective of the energy dose delivered, NO production was significantly reduced when LPS-stressed cells were irradiated. The best effect was observed when 15 J/cm2 were delivered to cells. Infrared LED irradiation also promoted decrease in ROS production, while HDPC metabolism was not significantly affected. The most produced cytokines in cells stimulated with TNF-α were: GROα, IL-6, IL-8, TNF α, MCP-1 and E1 Serpin. qPCR analysis showed a decrease in expression of IL- 1 β when the cells were irradiated with the dose of 4 J/cm2. Повний текст джерелаLaser phototherapy (LPT) has been used clinically to accelerate wound healing in a variety of oral diseases. The cell mechanisms by which LPT can promote biostimulation have not yet been fully elucidated. Epithelial cells play an important role in the reparative process since it proliferation and migration from the wound margin is crucial for restore epithelial continuity. It is unclear whether LPT has an effect on epithelial cell migration. Based on this, the aim of this study was to investigate the effect of LPT in oral wound healing process using oral keratinocytes. Oral keratinocytes were maintained under two nutritional conditions supplemented with 10% of fetal bovine serum (FBS) and in nutritional deficit (2% FBS). Laser irradiation was delivered with InGaAlP laser (660nm, 40mW, 4 e 20J/cm2, 4 e 20s). Irradiations were performed in contact, using the punctual irradiation mode. The following tests were performed cell viability, cell migration and protein analysis. Results obtained suggest that LPT influences epithelial migration and cytoskeleton polarization. Interestingly, LPT effect under epithelial cell migration occurs independently of cell viability. In conclusion, clinical LPT effects are associated with an increase in epithelial cell migration.

Повний текст джерелаINTRODUCTION: Scleroderma is a chronic autoimmune disease characterized by progressive connective tissue sclerosis and microcirculatory changes. Its etiology is still unknown. Its pathogenesis shows endothelial cell damage, inflammation and immunologic activation that stimulate collagen and other extra cellular matrix protein production. Localized scleroderma is a limited disease and its superficial forms may be resolved in months or years, and leave little or no sequel. However, in ten percent of the cases atrophic and deforming lesions may be observed that delay or hinder normal development. Literature reports indicate phototherapy as a limited scleroderma therapeutic modality with favorable response. Through its immunosuppressant effect, phototherapy inhibits collagen production and induces increased collagenase synthesis. This study had the purpose of evaluating the phototherapy treatment for localized scleroderma. The clinical evaluation and the skin ultrasound were used to demonstrate the results thus obtained. METHODS: From January to December 2007, eleven patients with localized scleroderma were selected for phototherapy treatment in the Allergy and Phototherapy Division, Dermatology Clinic, Santa Casa of São Paulo Hospital. They were classified according to the type of localized scleroderma and lesion evolution: inflammatory or stadial. The lesions were treated with systemic PUVA, topic PUVA or UVB narrow band. The skin lesions were assessed by clinical palpation and high-frequency ultrasound to measure dermis thickness at baseline and at the end of 12 weeks of phototherapy. RESULTS: Some clinical improvement was observed after an average of 10 phototherapeutic sessions. All skin lesions, either inflammatory or stadial, were softer at clinical palpation with scores reduction upon pre and post treatment comparison. The ultrasound showed that most of the assessed lesions presented a decrease in dermis thickness, and only five maintained their previous measure. Treatment response was similar regardless of the phototherapeutic treatment employed. CONCLUSIONS: The treatment that was proposed was effective for all the lesions, regardless of the phototherapeutic modality employed. Повний текст джерелаThe process of communication of the heath team with the mother whose child is under phototherapy is fundamental so that she can calm down and become acquainted with the treatment. The thesis defended in this research is that the communication permeated by the Nursing humanistic care will contribute to the mother of the newborn under phototherapy come to know, understand the treatment and this way be well and be - better. We aimed to analyze the communication with mothers of newborns under phototherapy through the Phenomenological Nursing Process of the Paterson and Zderadâs Humanistic Theory. Qualitative, exploratory and descriptive study, instrumentalized through the mentioned process, in five stages: the preparation of the nurse to come to know; the nurse knows intuitively the other; the nurse knows scientifically the other; the nurse synthesizes in a complementary way the known realities; the internal succession of the nurse starting from many to an only paradoxical. It had as environment a public maternity hospital in Fortaleza-CE. The subjects were 25 mothers taken into the referred institution, whose newborn (NB) children were under phototherapic treatment in the Neonatal Care Unit (NCU) of high, medium risk and/or Shared Accommodation (SA), from January to March/2005. We used as technique the participant observation and the individual interview, in two phases, with the following orienting question âWhat does it mean for you, mother, to witness your child under phototherapy?â In the data analysis of the participant observation we could visualize the interactions mother/newborn/nurse. From the speeches extracted from both phases of the interview the following themes were identified communication, knowledge, care, perception and feelings. From the theme communication, we extracted in the first phase the sub-themes damaged communication with the health team; lack of communication of the health team and communication motherânewborn; and in the second phase, reactions of the mother after effective communication of the nurse. From the theme knowledge, we extracted in the first phase the sub-themes lack of knowledge about phototherapy and search for knowledge about phototherapy and in the second phase, acquired knowledge about phototherapy and comprehension of the phototherapic treatment: a new vision of the mother. From the care theme, we extracted in the first phase the sub-theme the newborn under phototherapy â mothers searching Nursing Care and in the second phase, care-presence: participation of mothers in the care to newborn under phototherapy and didactic resource in the communication process: subsidy to Nursing care with mothers of newborns under phototherapy. From the theme perception, we extracted in the first phase the sub-themes health of the newborn under phototherapy: the vision of the mother; separation of the binomial mother-son; the mother of the newborn under phototherapy and the hospital environment; and in the second phase, health of the newborn under phototherapy: what has changed? From the theme feelings, we extracted in the first phase the sub-theme impotence and sadness and in the second phase, mothers before the newborn under phototherapy: well-being and be better. As we concluded, the effective communication of the nurse contributed for the mothers to calm down and be better while they experience their child under phototherapy.
O processo de comunicaÃÃo da equipe de saÃde com a mÃe cujo filho està sob fototerapia à fundamental para que esta possa se tranqÃilizar e tornar-se conhecedora deste tratamento. A tese defendida nesta pesquisa à que a comunicaÃÃo permeada pelo cuidado humanÃstico de Enfermagem irà contribuir para a mÃe do neonato sob fototerapia poder vir- a - conhecer, compreender o tratamento e desse modo estar bem e estar-melhor. Objetivamos analisar a comunicaÃÃo com mÃes de neonatos sob fototerapia por meio do Processo da Enfermagem FenomenolÃgica da Teoria HumanÃstica de Paterson e Zderad. Estudo qualitativo, exploratÃrio e descritivo, instrumentalizado pelo mencionado processo, em suas cinco etapas: a preparaÃÃo da enfermeira para vir-a-conhecer; a enfermeira conhece intuitivamente o outro; a enfermeira conhece cientificamente o outro; a enfermeira sintetiza de forma complementar as realidades conhecidas; a sucessÃo interna da enfermeira a partir de muitos para um Ãnico paradoxal. Teve como cenÃrio uma maternidade pÃblica em Fortaleza-CE. Os sujeitos foram 25 mÃes internadas na referida instituiÃÃo, cujos filhos recÃm-nascidos (RN) se encontravam em tratamento fototerÃpico na Unidade de InternaÃÃo Neonatal (UIN) de alto, mÃdio, risco e/ou Alojamento Conjunto (AC), de janeiro a marÃo/2005. Utilizamos como tÃcnica a observaÃÃo participante e a entrevista individual, em duas fases, com a seguinte questÃo norteadora âO que significa para vocÃ, mÃe, presenciar seu filho sob fototerapia?â Na anÃlise dos dados da observaÃÃo participante pudemos visualizar as interaÃÃes mÃe/recÃm-nascido/enfermeira. Das falas extraÃdas de ambas as fases das entrevistas foram identificadas as seguintes temÃticas: comunicaÃÃo, conhecimento, cuidado, percepÃÃo e sentimentos. Da temÃtica comunicaÃÃo, extraÃmos na primeira fase as subtemÃticas comunicaÃÃo prejudicada com a equipe de saÃde; ausÃncia de comunicaÃÃo da equipe de saÃde e comunicaÃÃo mÃe â recÃm-nascido; e na segunda fase, reaÃÃes da mÃe apÃs comunicaÃÃo efetiva da enfermeira. Da temÃtica conhecimento, extraÃmos na primeira fase as subtemÃticas desconhecimento da fototerapia e a busca do conhecimento da fototerapia e na segunda fase, conhecimentos adquiridos acerca da fototerapia e compreensÃo do tratamento fototerÃpico: um novo olhar das mÃes. Da temÃtica cuidado, extraÃmos na primeira fase a subtemÃtica o neonato sob fototerapia - mÃes em busca do cuidado de Enfermagem e na segunda fase,cuidado-presenÃa: participaÃÃo da mÃe no cuidado ao neonato sob fototerapia e recurso didÃtico no processo de comunicaÃÃo: subsÃdio para o cuidado de Enfermagem com mÃes de neonatos sob fototerapia. Da temÃtica percepÃÃo, extraÃmos na primeira fase as subtemÃticas saÃde do neonato sob fototerapia: o olhar da mÃe; separaÃÃo do binÃmio mÃe-filho; a mÃe do neonato sob fototerapia e o ambiente hospitalar; e na segunda fase, saÃde do neonato sob fototerapia: o que mudou? Повний текст джерелаResumo: Aparelhos ortodônticos fixos criam áreas de estagnação para biofilmes dentários e dificultam a limpeza dos dentes; portanto, existe o risco de desenvolver lesões incipientes de cárie durante o tratamento ortodôntico. O objetivo deste estudo é verificar se a aplicação de luz azul antes da clorexidina 0,12% no esmalte, bráquete e elástico ortodôntico reduziria ou inibiria os biofilmes maduros de Streptococcus mutans e seu crescimento nesses substratos 24 horas após a aplicação dos tratamentos; e se esse tratamento interfere na adesão do bráquete ao esmalte. Biofilmes de S. mutans UA159 foram formados por 5 dias sobre amostras compostas por esmalte bovino, bráquete ortodôntico e elástico ortodôntico. Em seguida, as amostras foram tratadas com NaCl 0,89% por 1 minuto (NaCl), luz azul por 12 minutos (BL), clorexidina 0,12% por 1 minuto (CHX) e luz azul por 12 minutos seguido da aplicação de clorexidina 0,12% por 1 minuto (BL+CHX). O acúmulo de biofilme imediatamente após os tratamentos e 24 horas após os tratamentos foram avaliados por unidades formadoras de colônias (CFU) e peso seco (DW). O pH do meio foi medido no quinto dia e sexto dia. A formação de biofilme nas amostras após os tratamentos (Imediato) e no recrescimento (Regrowth) foi avaliada visualmente por microscopia confocal de varredura a laser (CLSM). O teste de adesão (SBS) entre o bráquete e o esmalte foi feito após as amostras serem termocicladas por 500 ciclos (5°C e 55°C), tratadas e termocicladas novamente nas me... Abstract: Fixed orthodontic appliances create areas of stagnation for dental biofilms and make it difficult to clean the teeth; therefore, there is a risk of developing incipient lesions of caries during the orthodontic treatment. The objective of this study is to verify if the application of blue light prior to 0.12% chlorhexidine (CHX) on enamel, orthodontic brackets and elastics would reduce or inhibit mature Streptococcus mutans biofilms and their regrowth on these substrates 24 h after the application of the treatment; and if this treatment would interfere with bracket adhesion to the enamel. Biofilms of S. mutans UA159 were formed for 5-days over samples composed by a bovine enamel, an orthodontic bracket and an orthodontic elastic. Then, the samples were treated with 0.89% NaCl for 1 minute (NaCl), blue light for 12 minutes (BL), 0.12% chlorhexidine for 1 minute (CHX) and BL for 12 min + 0.12% CHX for 1 min (BL+CHX). Biofilm accumulation immediately after treatments and 24-h after treatments (regrowth) were evaluated by colonies forming units (CFU) and dry weight (DW). The pH of the spent media was measured on the 5th and 6th day. Biofilm formation on the samples after the treatments and on the regrowth was visually evaluated by confocal laser scanning microscopy (CLSM).
