Peripartum Cardiomyopathy (PPCM) is a form of idiopathic heart failure that develops most commonly in the last month of gestation or during the first five months post-partum. Here we present a case of successfully managed peripartum cardiomyopathy in a patient with preeclampsia with severe features with pulmonary oedema and intrauterine death of foetus. The patient was managed conservatively until stable along the lines of pulmonary oedema and preeclampsia with severe features keeping various other differentials in mind followed by which the diagnosis of peripartum cardiomyopathy was suggested by echocardiography. Termination of pregnancy was conducted by caesarean section following which both mother and baby were discharged in stable conditions.

In this work of philosophical short story fiction, a young boy is in the government waiting room waiting to be assigned a new “dream.” He strikes up a conversation with other people in the waiting room. Some of them are anxious to get new dreams implanted into their brain as they have not found success. Others don’t want to let go of the failed dream they were originally assigned because they believe, in their heart of hearts, it is what they were born to do. The government is indifferent to the desires of the people. Society has needs, people have innate talents, and the government, as far as they are concerned, should focus on getting people to follow the dreams they are good at, as well as the dreams that are most needed by society. This story was the winner of the Fall 2020 After Dinner Conversation Writing Competition.

ABSTRACT Background: Several facets of daily life are impacted by mental health, including how you handle stress, think and feel, interact with people, and make decisions. It makes sense that having a mental health issue could make it more difficult to follow your diabetes treatment plan. Diabetes complications and untreated mental health concerns can both exacerbate each other’s effects. Depression is a medical condition that results in depressed feelings and frequently a loss of interest in previously enjoyed activities. It may interfere with your ability to manage your diabetes and perform effectively at work and at home. Your chance of developing diabetic complications including heart disease and nerve damage increases when you are unable to effectively manage your blood sugar levels. The daily routine traffic jams, family obligations, and daily diabetes care are all sources of stress. Methods: Articles searched from Pubmed central and other search engines for articles related to psychological therapeutic intervention. Results: Stress can manifest as an emotion, such as fear or rage, as a physical response, such as perspiration or a racing heart, or both. Blood sugar levels can be impacted as well because stress hormones cause blood sugar to fluctuate erratically and because stress from illness or injury can cause blood sugar to rise. Long-term stress might cause other health issues or exacerbate existing ones. The mind and body respond to stress by feeling anxious, which can include anxiety, fear, or tenseness. Anxiety is 20% more common among diabetics than among non-diabetics at some time in their lives. Conclusion: These overwhelming emotions, sometimes referred to as diabetes distress, might make you develop bad habits, stop monitoring your blood sugar, and even forget about scheduled doctor’s appointments. The majority of persons with diabetes experience it, frequently following years of effective control.

During the winter of scarcity of 1794, Hannah More wrote “a few moral stories,” drew up a plan for publication and distribution, and sent the package around to her evangelical and bluestocking friends. Their response was enthusiastic; even Horace Walpole abandoned his usual teasing to write back, “I will never more complain of your silence; for I am perfectly convinced that you have no idle, no unemployed moments. Your indefatigable benevolence is incessantly occupied in good works; and your head and your heart make the utmost use of the excellent qualities of both…. Thank you a thousand times for your most ingenious plan; may great success reward you!” Walpole then sent off copies of the plan to the duchess of Gloucester and other aristocratic friends. Following Wilberforce's example, such wealthy philanthropists subscribed over 1,000 pounds to support the project during its first year. Henry Thornton agreed to act as treasurer and Zachary Macaulay as agent, and the ball was rolling.In March 1795, the Cheap Repository of Moral and Religious Tracts issued its first publications. Prominent evangelicals and gentry worked to distribute them to the rural poor, booksellers, and hawkers and among Sunday schools and charity children. During the Repository's three-year existence, the fifty or so tracts written by Hannah More were supplemented by contributions from fellow evangelicals Thornton, Macaulay, John Venn, and John Newton, the poet William Mason, More's literary friend Mrs. Chapone, her protégée Selina Mills, and her sisters Sally and Patty More and by reprints of old favorites by Isaac Watts and Justice John Fielding.

This pilot randomized controlled trial (RCT) aimed to determine the acceptability and preliminary efficacy of a web-based cardiovascular disease (CVD) prevention intervention for women following preeclampsia. Australian women with a recent history (≤4 years post diagnosis) of preeclampsia were randomized into two study arms: (1) Be Healthe for your Heart, a web-based behavioral intervention or; (2) Control, access to the National Heart Foundation website. Assessments were conducted at baseline, and after three months. Intervention acceptability and impact on absolute CVD 30-year risk score, CVD risk markers and health behaviors were assessed. Twenty-four of 31 (77.4%) women completed the three-month assessment. Eleven out of 13 intervention participants (84.6%) agreed/strongly agreed they were satisfied with the program, with a mean score of 4.2 ± 0.9 (maximum of five). There were no significant between or within group differences in absolute CVD risk, CVD risk markers or health behaviors from baseline to three months. Women with a history of preeclampsia were successfully recruited and retained and they reported high levels of acceptability with the Be Healthe for your Heart program. Further research is therefore needed from powered trials to determine the impact of web-based lifestyle interventions on CVD risk in this at-risk group.

Hypoplastic left heart syndrome (HLHS) is a complex congenital heart condition which includes abnormal development of left sided cardiac structures leading to inadequate systemic perfusion following postnatal closure of the patent ductus arteriosus (PDA). Surgical palliation may be accomplished through a 3 staged process -Norwood procedure. This surgery is usually done within the first two weeks of your child's life -Bidirectional Glenn procedure. This procedure is generally the second surgery. It's done when your child is between 3 and 6 months of age. -Fontan procedure. This surgery is usually done when your child is between 18 months and 4 years of age. Patients with HLHS may need to undergo other non-cardiac surgical procedures during the first years of life posing a real challenge to the anesthesiologist, surgeon and the entire medical team. We present the case of a 18-months old, 9 kg infant who presented for cryptorchidism surgery. Cryptorchidism or undescended testis (UDT) is one of the most common pediatric disorders of the male endocrine glands and the most common genital disorder identified at birth. The main reasons for treatment of cryptorchidism include increased risks of impairment of fertility potential, testicular malignancy, torsion and/or associated inguinal hernia. The intraoperative implications of the hybrid anatomy are discussed, options for anesthetic care presented, and previous reports of anesthetic care for such patients reviewed. Conclusion: LMA combined with local anesthesia was effective to maintaining optimal cardiac function of this child patient with HLHS In summary, children with palliated HLHS have anesthetic considerations that must be followed in order to reduce perioperative morbidity and mortality in this high-risk pathology.

Abstract High percentage of right ventricular pacing could lead to pacing induced cardiomyopathy in a non–negligible proportion of patients. Cardiac resynchronization therapy upgrading could represent an option to improve ventricular function, reducing heart failure hospitalizations. A 82 yo male was admitted to hospital with symptomatic heart failure. He had history of complete AV block treated with dual chamber pacemaker implantation five years earlier following coronary artery bypass graft surgery, bioprosthetic aortic valve replacement and septal myectomy. He had also history of cardiac arrest occurred in the context of acute coronary syndrome treated with PCI so that ICD implant was not considered at that time, and two more HF hospitalizations in previous years. Progressive LVEF decline was documented since PM impantation until last echo showed severe left ventricular dysfunction (EF 20% from baseline EF 60%). The ECG showed sinus rhythm and wide paced QRS (206 ms) with 100% pacing percentage. A CRT device upgrading was planned. Baseline venography initially showed pervious axillary–subclavian left axis, but unfortunately the guidewire encountered a complete occlusion before entering the superior vena cava. Nevertheless, dye contrast injection documented the presence of a proximal collateral branch (fig A) having a vertical course in the thorax and reaching the lateral wall of the left ventricle with an extrapericardic course then entering the right heart through inferior vena cava. Using a IM 5Fr sheath (usually used by interventional cardiologists to cannulate internal mammary artery) we could reach this branch, inserting a guidewire and finally advancing a quadripolar lead (Fig B–C) which reached the lateral left ventricular wall. LV capture was confirmed when pacing from the three proximal poles, while the distal one caused phrenic nerve capture. The quadripolar lead was connected to the LV port of a three chamber PM along with previous implanted leads thus obtaining biventricular pacing. QRS paced was 50 ms shorter than baseline. No peri–procedural complications occurred. Lead–related venous occlusion might represent an obstacle to CRT upgrade requiring high technical expertise. The presence of a complete venous occlusion might cause procedural failure, particularly in hospitals without surgical facility. In this case, the presence of a collateral branch reaching the lateral wall of the left ventricle allowed to successfully complete the CRT upgrade.

A young man with Down syndrome and an early adopter of self-managed supports wanted to have a great life, but he needed to be sure what a great life was for him. He decided to ask successful people, “What do you love about your life?”, which he felt was a question that gets right to the heart of the matter. In this paper the young man and his father will outline their journey as co-researchers in collecting/filming 600 stories across ten countries, with some interviews prearranged but many occurring spontaneously at music, film, and arts festivals. The initial findings of “what people loved about their lives” fall into the following categories: Family, Work, Home, Travel, Health, Pets, and Life Itself. Positive side effects for the young man covered: self-confidence, ability to connect with people, opportunities, working relationships, and ongoing friendship. The implications of taking a social journalism approach to collecting stories will be discussed in line with the question “What makes for good inclusive research”? The journey of the co-researchers speaks to the outcome that it is worthwhile to research your life path, accessing elders’ wisdom. The tools of inclusive research are powerful when applied to a citizen’s life questions. Each person values their life in unique ways, yet there are many commonalities.

INTRODUCTION: Explaining what cardiovascular disease (CVD) risk means and engaging in shared decision-making regarding risk factor modification is challenging. An electronic CVD risk visualisation tool containing multiple risk communication strategies (Your Heart Forecast) was designed in 2009. AIM: To assess whether this tool facilitated explaining CVD risk to primary care patients. METHODS: Health professionals who accessed a Primary Health Organisation website or who attended educational peer groups over a three-month period were invited to complete questionnaires before and after viewing a four-minute video about the tool. Respondents were asked to make an informed guess of the CVD risk of a 35-year-old patient (actual CVD risk 5%) and rate the following sentence as being true or false: ‘If there were 100 people like Mr Andrews, five would go on to have a cardiac event in the next five years.’ They also were asked to rank their understanding of CVD risk and confidence in explaining the concept to patients. RESULTS: Fifty health professionals (37 GPs, 12 practice nurses, one other) completed before and after questionnaires. Respondents’ CVD risk estimates pre-video ranged from <5% to 25% and nine rated the sentence as being false. After the video, all respondents answered these questions correctly. Personal rankings from zero to 10 about understanding CVD risk and confidence in explaining risk reduced in range and shifted towards greater efficacy. Discussion: Whether this tool facilitates discussions of CVD risk with patients and improves patient understanding and lifestyle behaviour needs to be evaluated in a randomised trial. Abstract Senior mentoring programs provide medical students exposure to a community-dwelling older adult mentor with whom they meet multiple times throughout the program. The goal of these programs is to expose students to healthy older adults, increase their knowledge of topics in geriatrics and aging, and increase the likelihood that students will pursue geriatric specialties. Though research findings show that senior mentoring programs have the potential to increase medical students’ attitudes towards older adults (Samra et al., 2013) and their willingness to consider working with older patients in the future (McManus et al., 2017), a critical shortage of doctors who specialize in geriatrics still exists. Moreover, the demand for geriatrically-trained physicians is expected to continue to increase (American Geriatrics Society, 2018). In order to develop avenues for successfully fostering interest in and pursuit of geriatrics specialties, we need to fully understand students’ perceptions of working with older adults. The purpose of this qualitative content analysis was to explore first-year medical students’ (n = 216) perceptions of working with older adults. We asked students to respond to the following reflection at the beginning and end of their Senior Mentoring program: How do you feel about working with older patients after you complete your training? Our findings suggest that while students feel more comfortable with and open to caring for older adults, they do not feel compelled to pursue geriatrics. Themes and sub-categories emerging from the data provide insight into why students continue to dismiss pursuing geriatrics.

Health system learning is an iterative process. The six phases of a learning health system (LHS) are problem identification, design, implementation, evaluation, modification, and dissemination. In theory, evaluation is supposed to inform design and implementation and in turn implementation process and modification as the direct result of the evaluation endeavour set the direction for future evaluation. In other words, evaluation and implementation go hand-in-hand. In practice, however, when designing an integrated model of care, more often than not in the fast-paced environment of the healthcare where lives are literally at stake, both evaluation and implementation planning are sacrificed. This happens for many reasons, from lack of resources to lack of expertise. One main reason that work as imagined is perceived as work as done is that those who run the program are smart and dedicated people who are well aware of the needs of their communities and clients, and have their best interest at heart. East Toronto Health Partners (ETHP) has invested in evaluations since 2018 to create a learning health system by embedding rapid cycles of evaluation to support learning, knowledge transfer, and decision making for scale and spread of their new models of care. One of the main findings of our evaluation work was that implementation planning and monitoring the implementation process raised the likelihood of a successful evaluation which in turn resulted in tangible improvement in the program. In our poster presentation, we will focus on both implementation and evaluation. We showcase the link between the logic model/evaluation plan and the implementation plan. We will cover the following questions: Why having an implementation plan is necessary to bring an idea for an intervention to a reality? How an implementation plan helps you to carry out your evaluation plan more successfully? What questions you should be thinking about when you are developing your implementation plan? We will share examples of best practices. The poster will also provide practical implementation templates that can be used in different setting.

IntroductionChronic stable angina is common and disabling. Cardiac rehabilitation is routinely offered to people following myocardial infarction or revascularisation procedures and has the potential to help people with chronic stable angina. However, there is insufficient evidence of effectiveness and cost-effectiveness for its routine use in this patient group. The objectives of this study are to compare the effectiveness and cost-effectiveness of the ‘Activate Your Heart’ cardiac rehabilitation programme for people with chronic stable angina compared with usual care.Methods and analysisACTIVATE is a multicentre, parallel-group, two-arm, superiority, pragmatic randomised controlled trial, with recruitment from primary and secondary care centres in England and Wales and a target sample size of 518 (1:1 allocation; allocation sequence by minimisation programme with built-in random element). The study uses secure web-based allocation concealment. The two treatments will be optimal usual care (control) and optimal usual care plus the ‘Activate Your Heart’ web-based cardiac rehabilitation programme (intervention). Outcome assessment and statistical analysis will be performed blinded; participants will be unblinded. Outcomes will be measured at baseline and at 6 and 12 months’ follow-up. Primary outcome will be the UK version of Seattle Angina Questionnaire (SAQ-UK), physical limitations domain at 12 months’ follow-up. Secondary outcomes will be the remaining two domains of SAQ-UK, dyspnoea, anxiety and depression, health utility, self-efficacy, physical activity and the incremental shuttle walk test. All safety events will be recorded, and serious adverse events assessed to determine whether they are related to the intervention and expected. Concurrent economic evaluation will be cost–utility analysis from health service perspective. An embedded process evaluation will determine the mechanisms and processes that explain the implementation and impacts of the cardiac rehabilitation programme.Ethics and disseminationNorth of Scotland National Health Service Research Ethics Committee approval, reference 21/NS/0115. Aim – To determine the effectiveness of 6 weeks of moderate intensity exercise training & To provide an appropriate measure of cardiovascular fitness. The YMCA 3minute step test is based on how quickly your heart rate recovers following a short bout of exercise. Hence the purpose was to provide a submaximal measure of cardiovascular fitness after six-weeks of moderate intensity exercise training among young obese adults. Methodology - According to simple random sampling 50 participants were included in the study after screening for the inclusion criteria. Thereafter They were explained the purpose of the study and the procedure involved in the study. Informed Consent was obtained and a YMCA 3-minute step test was done. Heart rate measures were recorded at the end of the 3-min step test Pre & Post intervention. Data analysis was done at the end of the study. Results – Post intervention 10 students (20%) out of 50 had good cardiovascular fitness level, 22 students (44%) had an above average cardiovascular fitness, 13 students (26%) had average fitness & 5 students (10%) had below average level of cardiovascular fitness. Conclusion- This study concluded that, six weeks of moderate intensity exercise training improved cardiovascular fitness and lowered the risk of developing cardiovascular disease in obese young adults. We used the following multiple-choice question after a series of lectures in cardiovascular physiology in the first year of an undergraduate medical curriculum ( n = 66) to assess whether students had understood the neural regulation of cardiovascular function. In health, neural cardiovascular mechanisms are geared toward maintaining A) cardiac output, B) total peripheral resistance (TPR), C) arterial blood pressure (BP), D) tissue blood flow. The same question was administered to 275 graduates preparing for postgraduate exams (but not following the same series of lectures as the undergraduates). In both groups, we found a large proportion of incorrect answers (70% in undergraduates and 85% in graduates) and sorted this out by offering a step-by-step explanation and two examples and found it successful: 1) What happens to BP and heart rate (HR) when a person loses 500 ml of blood (∼10% of blood volume) in one minute? 2) What happens to your BP and HR as you get out of bed after a night’s sleep? Flow = perfusion pressure/resistance to flow; cardiac output = BP/TPR; BP = cardiac output × TPR = [stroke volume (SV) × HR] × TPR. In both examples, BP decreases and is rapidly brought into the normal range by the arterial baroreflex mechanism. TBF is regulated chiefly by varying local vascular resistance (autoregulation). In summary, the ultimate goal of all neural cardiovascular reflex mechanisms is to maintain arterial BP within a range in which tissues can regulate their own blood flows. Cardiovascular control during exercise was used as an example to emphasize these facts. A discussion of this kind triggered interest in the minds of students and graduates, helping them get rid of a major misconception in about 20–40 minutes.

When a person reaches the elderly phase, the function of the body's organs decreases, which causes an increased risk of disease and death. Including dental infections such as caries and periodontal disease which are the main factors for tooth loss. Tooth loss does not only affect the impact of mastication but also affects a person's cognitive function. This condition occurs because when a person loses teeth, the function of the Periodontal Mechanoreceptors decreases, disrupting the function of the hippocampus which is important for memory in the brain. Loss of a large number of teeth tends to indicate low cognitive function, but several factors affect cognitive function, namely education level, Diabetes Mellitus, Hypertension, heart disease, depression, sleep disorders, head injury, physical activity, and a history of smoking. Mini-Mental State Examination (MMSE) is a tool to assess a person's cognitive function. This study aims to see the description of cognitive function in the elderly at the Geunaseh Sayang Nursing Home with variations in the number of tooth losses. This study was conducted with 43 research subjects with interviews and intra-oral examination methods. Based on the results of the study, it can be concluded that the elderly who only have fewer teeth show a higher decline in cognitive function compared to the elderly with more teeth in the oral cavity. This is following the theory which states that the more teeth you lose, the lower your cognitive function will be.

“Put your Heart/Body into it”: a Psycho-Physiological Approach of the Embodied Cognition. Cognition is rooted in the body according to embodied cognition view. Research within this approach has studied the body primarily under the scope of motricity (e. g. effect of gesture, posture). Studies in the field of psychophysiology have investigated the role of physiological variables in the functioning of the central nervous system without proposing an "embodied" reading. Bridges between these fields could be built through an integrative approach of these data within an embodied epistemic framework. This article thus proposes an integrative interpretation of the psycho-physiological mechanisms involved in the self-regulation of the cognitive system during interaction with the environment (i. e. autopoiesis). The main originality of this proposal is to defend epigenetics as a bodily memory that would be one of the main drivers of this self-regulation because of its influence on physiology following the interaction with the environment and the possible repercussion of this influence on the cognitive system.

Introduction: In the United States, a significant amount of the population is affected by hyperlipidemia, which is associated with increased levels of serum low-density lipoprotein (LDL-C) and risk of cardiovascular disease. As of 2019, the guidelines set by the American College of Cardiology/American Heart Association advocate for the use of statins as the major contributor to lowering serum LDL-C. While proven to be effective, side effects, including muscle-related symptoms and new-onset diabetes mellitus, can make patients unable to tolerate statin therapy. Additionally, there is a subset of the population which does not approach a recommended LDL-C goal on statin treatment. Due to these findings, it was deemed necessary to review the literature of current statin-alternative lipid-lowering therapies. Methods: A systematic review of preclinical and clinical papers, and a current meta-analysis, was performed using PubMed and Google Scholar. Following the literature review, a meta-analysis was conducted using ProMeta 3. Results: Through systematic review and meta-analysis of the current literature, it is suggested that newer lipid-lowering therapies such as proprotein convertase subtilsin-kixen type 9 (PCSK9) inhibitors are a safe and effective statin alternative for the population with statin intolerance. PCSK9 inhibitors were shown to have no significant effect in causing myalgia in patients and showed no increase in adverse cardiovascular outcomes compared to a control of a current antilipemic medication regimen. Discussion: There are many statin-alternative therapies that should be investigated further as a potential replacement for patients with statin intolerance or as an addition for patients with statin resistance.

Methodology- According to simple random sampling 40 participants were included in the study after screening for the inclusion criteria. Thereafter the Participants were divided into 2 groups. 20 participants included in sports group and 20 in dance group. They were explained the purpose of the study and the procedure involved in the study. Informed Consent was obtained and a YMCA 3minute step test was done. Heart rate measures were recorded in both the groups of population. Data analysis was done at the end of the study. Results – 12 students in the sports group had a good to above average fitness level, and 3 students in dancing group had a above average fitness. No student in dance had a good fitness. Further 7 students in dance had a average fitness compared to 2 students in sports who had average fitness. 6 students involved in sports had below average to very poor fitness and 10 students involved in dance had their fitness in this category. Conclusion- It was concluded from the results obtained that the students involved in sport activities had a significantly good cardiovascular fitness than students involved in dance. It is presented for a paper written by a graduate student that best represents significant research and writing about any aspect of New Jersey history. The 2015 award went to Mr. Cohen. </span></p><p><span>***</span></p><p><span>Freehold, New Jersey faced two major moments of deindustrialization in the post-World War II period. In the late 1950s, the rug mill that sat at the center of the town’s economic and cultural life began to close down. In 1986, a 3M audio-visual tape plant that had helped the town avoid economic ruin shut down as well. This paper illustrates the continuities between these closings, challenging the dogma in labor history that plant closings occur because of management’s desire to avoid an entitled and demanding workforce. Though workers at both plants were unionized, neither the rug mill nor the 3M workers made major demands on their employers in the postwar period. This paper analyzes the conditions that prompted shutdowns in Freehold, illustrating the role of broader market forces as well as internal company dynamics in driving capital flight. Furthermore, a close look at the 3M closing reveals the importance of culture in workers’ responses to deindustrialization. Following 3M’s announcement of its plans to shut down the Freehold plant, workers began a national media campaign to save their jobs. At the heart of this campaign was the memory of the rug mill that had closed 25 years earlier, as represented by their campaign anthem, Bruce Springsteen’s 1984 song “My Hometown.” This paper demonstrates the role of memory and music in shaping workers’ experience with deindustrialization as well as the struggles of unions to codify the relationship between capital and community in the twentieth century.</span></p>

Abstract Objectives The primary objectives of this study were to: assess change in dietary habits and health outcomes [body mass index (BMI), weight loss, waist circumference (WC), blood pressure, low-density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol (TC), and Hemoglobin A1c (A1C)] among patients following completion of an intensive cardiac rehabilitation (CR) program. Methods This retrospective cohort study involved de-identified data obtained from an electronic medical record (EMR) program and from an excel spreadsheet maintained at a hospital in Arkansas. Data were available from 138 adults, age 29 to 89 years, with a pre-existing heart condition or event, and who completed a 12-week CR program at the hospital's Clinic. Dietary habits were evaluated using the 24-question Rate Your Plate (RYP) survey at initial visit, and discharge. RYP and weight data were also available for a group of patients (n = 18) completing a 3-month follow-up visit. Results There was a statistically significant (P &lt; .0001) mean increase (8.24; 95% CI, 7.03 to 9.45) from the pre-RYP score (51.05 ± 7.43) to the post-RYP score (59.29 ± 6.58). There was a statistically significant (P &lt; .0005) median decrease in weight (−2.00 lbs.) after post-intervention (193.50 lbs.) compared to pre-intervention (195.50 lbs.). There were also significant (P &lt; .05) differences between pre- and post-measurements of BMI, WC, LDL, total cholesterol, and triglycerides. Follow-up data indicated significant changes in the RYP score (P &lt; .0001) and weight (P = .020) were maintained at 3-months follow-up. Conclusions A 12-week CR program including nutrition education and counseling is effective to help patients with CVD or previous heart events improve their dietary habits. There were noticeable changes in diet scores, WC, BMI, and weight loss, with patients being able to sustain weight loss at their three-month follow-up visit (a 5% weight loss). Funding Sources N/A.

During the peak period of development in the first two years of life, illness and poor health have an impact on children's advancement potential. Breastfeeding is a basic human behaviour that benefits the infant and the mother's health, as well as providing a considerable financial incentive for families and social orders. The goal of the current study was to acquire a deeper grasp of the promising findings and perspectives on breast milk among rural nursing moms. A total of 200 nursing women from the city and 200 from the rural would be included in the research. A purposeful random sampling approach was used to acquire data. The information was acquired from nursing mothers using a coordinated overview to look at their food habits and attitudes. This section of Pediatrics in Review reminds clinicians of those conditions that can present in a misleading fashion and require suspicion for early diagnosis. Emphasis has been placed on conditions in which early diagnosis is important and that the general pediatrician might be expected to encounter, at least once in a while. The reader is encouraged to write possible diagnoses for each case before turning to the discussion, which is on the following page. We invite readers to contribute case presentations and discussions. Case 1 Presentation A 15-year-old girl comes to your office complaining that she has experienced intermittent, sudden episodes of chest pain, fatigue, palpitations, and sensations of difficulty breathing and lightheadedness for 2 months. These episodes occur several times daily and are unaccompanied by other symptoms such as syncope, wheezing, swelling of the extremities, or fever. She denies being worried, but reports that her parents are very frightened because a 16-year-old male cousin died recently while playing soccer, and two other relatives, a 27-year-old cousin and a 29-year-old uncle, died suddenly during exercise. The physical examination reveals a somewhat anxious girl complaining of mild precordial chest pain. Her temperature is 36.9°C(98.4°F) orally, respiratory rate is 16 breaths/min, heart rate is 110 beats/min, and blood pressure is 100/60 mm Hg; weight and height are at the 75th percentile.

Statement of the problem. There are a lot of works in the national musicology focusing on the study of vocal chamber music for voice and piano by Ukrainian composers of the 20th century. Researchers cover quite a wide range of issues regarding vocal pieces and touch upon the problems of cyclocreation, dramaturgy, features of musical and linguistic means, etc. However, they rarely pay attention to translation, though there are many vocal opuses, in which composers use foreign poetry. In this case, the specifi c choice of the translated sample helps to determine the principles of the composer’s approach to the embodiment of the poetic text, especially in comparison with other works based on the same sources. Hence separate songs from D. Klebanov’s vocal cycle on the poems of H. Heine did not become an exception, thereby confi rming the relevance of the proposed topic. The purpose of the article is to determine specifi c features of the embodiment of H. Heine’s poetry translations in the vocal cycle of D. Klebanov on the basis of two romances – “In a grove, on a wild path”, “My love, lay your hand on my heart”, as compared to the works of other composers of the twentieth century . To achieve the research objectives the following methods were used: historical, structural-functional, genre-style and comparative. Results. Under consideration are peculiarities of the embodiment of H. Heine’s poetry translations in the vocal cycle of D. Klebanov, one of the founders of Kharkiv composition school. For this work the author took eight verses from the fi rst two cycles of the “Book of Songs” by the German poet. They were based on the motives of love poems with vivid images of nature; sometimes the poems are full of sadness, a sense of loneliness. When D. Klebanov was choosing certain samples from different poetic cycles, he tried to stick to the plot of the “Book of Songs”, since he ordered the poems in the same way they were written in the collection. Another indicator of the composer’s relation to Heiner’s texts is the choice of poetic works which are given in the cycle in Ukrainian and Russian languages. The composer’s individual vision of Heine’s lyric poetry is clearly seen when compared to the vocal works of other composers of the twentieth century, M. Medtner and E. Denisov, written on the same poetic texts. In cross-romances, similar musical-linguistic means are used, including the metrical principle of vocalization of the poetic text, homophonic-harmonic structure, harmony of classical-romantic type. However, each of the composers renders the fi gurative and semantic implications of the poems in their own way. M. Medtner builds his romance “My love, lay your hand on my heart” according to the crescendo principle. Beginning with a quieter dynamics, the composer gradually increases the volume of the sound, which at the end leads to a general climax that moves from the point of the golden section. D. Klebanov chose a different way – to reinforce the dramatism of the poem. This was possible thanks to various musical and linguistic means: a strict, intense melody in the bass doubled in the sixth with a chromatic motion and semiquavers at the end of each bar in the last line of the fi rst stanza, designation Meno mosso, chromaticized vocal melody. The composers’ choice of poetic translations depends on the place and role of the romance in the general structure of the cycle. The eight-part composition of D. Klebanov is based on the wave principle of the plot development. The original four romances pave the way to the fi rst climax – unrequited love in the fi rst romance (“Every morning I awake and ask”), painful memories in the second one (“In a grove, on a wild path”), a tragic image in the third one (“My love, lay your hand on my heart “), and an attempt to overcome the pain in the fourth romance (“First I was afraid of darkness”). Further on, the development is based on contrast: the image of death in the fi fth romance (“Your lovely face, so fair and dear”), a subtle feeling of love in the sixth one (“Oh, let me plunge my heart”), worries because of the marriage of a loved one to another guy in the seventh romance (“I hear the fl ute and the fi ddle”) and disappointment in her spiritual values in the last one (“The violets blue”). Such a location of the third romance justifi es the choice of translation, where the colours are thickening and the content becomes even darker. Such kind of a fi gurative and semantic plot resembles the tradition of a romantic vocal cycle, in which the emotional state of the lyrical character, his emotional collisions сome to the fore. In this perspective, “ 3 Poems of H. Heine” by D. Medtner demonstrate another relationship between the romances of the cycle. All of them have feelings of sorrow, despair circle, a no-go. At the same time, distancing from the immediate events is felt, as if it is a look at someone else’s life, which is evidenced by the storytelling from the third person in the second and third romances. Therefore, the fi rst romance, based on the poem “My love, lay your hand on my heart”, is a kind of “preface” to the cycle, which involves some personal detachment. This leads to the selection of softened content in the translated version of the poem. The second romance, “In a grove, on a wild path”, has a similar function in the vocal cycle of D. Klebanov as it became the preparation for the climax of the third one. The semantic line of his poem is based on two storylines: the external one is the “theme of the journey” that is refl ected in the image of nature, and the internal one is the “theme of sadness”, which focuses on the feelings of the lyrical hero. The composer here, like Анна in the third romance, deepens the line of inner experiences. This became possible thanks to the Tranquillo tempo, fl at minor tonality, massive discordant accompaniment chords, variable measure, melody of the recitative-oratorial type. H. Heine’s poem, presented in the work of D. Klebanov, became the basis of the fi fth romance of E. Denisov’s vocal opus. Like the Ukrainian master, E. Denisov builds his cycle in the spirit of the romantic tradition, but in revealing the fi gurative structure of the poem he goes a different way. He makes a clear distinction between two fi gurativesemantic lines. This is refl ected in the form of a romance that has the features of binarity and variability, the embodiment of the metro-rhythmic structure of the verse based on two opposing principles - metric and cantilena, as well as other means of musical expression. Thus, choosing the same poem by H. Heine, D. Klebanov and E. Denisov represent their own vision of its content. Conclusions The comparative analysis of the embodiment of Heine’s texts by D. Klebanov and other composers of the twentieth century helps to highlight the individual approach of the Ukrainian artist. Despite the fact that the composer chooses similar means of musical expression, he fi nds his own way of refl ecting the semantics of the poetic source. In the above mentioned romances – “In a grove, on a wild path” and “My love, lay your hand on my heart” – the author focuses on the inner confl icts of the lyrical hero, his experiences. Attention paid to the sensory side of the poems also determined the selected translations, since the rejection of translators from the original results in a certain deformation of its meaning and fi gurative structure, which infl uences the musical embodiment of the poetic source.

Hearing that her husband had been incarcerated ‘for God’s word sake’, and that ‘old father Latimer is your uncle’, Ridley commended her own zealous commitment to the reformed faith, and her assistance of others: ‘… you be hearty in God’s cause, and hearty to your master Christ, in furthering of his cause and setting forth his soldiers to his wars to the uttermost of your power’. In the past twenty-five years we have begun to determine why women on the Continent and in England were attracted to, encouraged, or inhibited the spread of religious change, but there is still much to be done to understand the role of individual women and women generally during the Reformation. Mary Glover, despite her association with prominent figures, is almost unknown to historians. The purpose of this paper is to describe a type of patronage which had important local and national implications, and a distinct role exercised by Mary Glover and others in the sixteenth and following centuries: the practical assistance which women offered to itinerant preachers.

The fetus’s perineal area has not traditionally been the focus for the prenatal echoscopy, mainly because adequate visualization depends on fetus’s position. However, it is precisely thanks to the targeted attention to the perineal area that we sometimes manage to find not only quite curious but clinically significant findings.Sacrococcygeal teratoma can grow large and affect the natural birth process. When localized in the perineal region, it sometimes leads to defecation and micturition disorders. In addition, intestinal loops may be located in the tumor structure, which leads to deformation and the risk of hernia formation. In the presence of a giant sacrococcygeal teratoma, the following events may occur: heart failure leading to fetal hydrops, obstruction of the urinary tract, obstruction of the gastrointestinal tract, impaired innervation of organs due to compression, intratumoral hemorrhage, intrauterine anemia. Hymenal pathology is quite rare in everyday clinical practice. Usually, it is one of the two most common nosologies: hymenal polyp or imperforate hymen. These two anomalies occur almost exclusively in childhood or immediately after menarche. At a later age, the preserved or dysmorphic hymen can present difficulties in diagnosing and treating intravaginal pathological processes. In this article, we would like to bring to your attention two clinical cases with different prognosis to draw colleagues’ attention to the need for a targeted assessment of the perineal area and correct prenatal counseling for a couple, because the ranking of the severity of the probable consequences is significant. Thus, some findings will require only adequate monitoring in childhood, others – may be the cause of premature birth, changes in delivery tactics or immediate surgery after birth.

Purpose – Using the staff and customers as a starting point, the purpose of this paper is to describe a framework that helps board and senior management identify key drivers that generate value for their organisation and for their customers. Design/methodology/approach – With research into the range of customer experience metrics available the authors identified a lack of consideration within the staff directly involved. The paper explores the considerations that lie behind how staff impact upon customer experience within any process framework. Findings – The approach to customer service is based around: “Treat customers well and they’ll come back, spend more and recommend your company”. To achieve this following the research the authors now subscribe to a “customer third” edict that recognises that people are at the heart of that experience not processes. If individuals are motivated to serve well, if teams around them work in an encouraging environment then the customer will usually believe they are coming first. The health check now developed recognises the impacting factors on staff and their provision of a great customer experience. Practical implications – The authors find that a focus on staff will positively impact their levels of productivity, absenteeism, engagement and tenure. In customer experience, tenure with enhanced motivation to serve realises a positive impact to financials. Social implications – Reducing stress in the workplace should positively impact the whole life balance for individuals. Originality/value – Appropriate focus on people rather than process within customer service industries will reduce unnecessary investment in process and system change and deliver enhanced results within existing frameworks. Change driven by the people within the process is positive.

Following the pandemic, investments in theconstruction sector are destined to reach exceptionallevels, with noteworthy effects on economicproduction, all while providing a very real opportunityto carry out projects that could resolve a good many ofthe critical problems currently affecting the towns, citiesand surrounding territory of Italy: problems such asaccessibility, the distribution of public resources andservices, and seismic and hydrogeological safety;foremost among these are problems plaguing structuralfunctions, as well as the lack of social and economicintegration in outlying urban areas. And yet there is arisk that pursuit of these objectives could be seriouslyhampered by distressing levels of inefficiency in theoverall organisation of the processes of intervention, aswell as the unwarranted influence of special interests,including, in some cases, infiltration by organised crime.The resulting situation presents an unacceptable ratioof resources allocated to the quality of results. Thispaper illustrates why significantly different forms ofgovernance should be adopted, together with a totallynew, innovative approach to organising the processesof intervention. A reference found to be especially useful in formulatingthe considerations developed in the paper was a criticalanalysis of the results - ultimately judged to be positive- of the project to replace the Morandi Bridge in Genoa,though mention was also made of the factors thatrender a generalised application of this example tofuture investments unfeasible. ABSTRACTObjective: this review aimed to describe the historical development of cardiopulmonary resuscitation, following the history of this therapeutic modality. Method: the methodology consists of a review of the databases Medline and Lilacs looking for articles published in the last 20 years about the history and establishment of cardiopulmonary resuscitation as a therapeutic option for reversal of cardiorespiratory arrest. Results: there are historical antecedents of cardiopulmonary resuscitation from the Biblical era until the establishment of the committee of the American Heart Association. As this committee, currently, a reference in teaching and research in cardiopulmonary resuscitation in the world, its investment allowed from 2000, the development of a global consensus of resuscitation guidelines that generated international attention only on the cardiopulmonary resuscitation. Conclusion: it was noted that over the years the techniques of PCR produced an impact on modern society to the point of generating new expectations facing life and death, revealing is essential that the healthcare professional who understands your technical and scientific competence for a parade heart can make a difference between life and death of a patient. Resultados: destacam-se os antecedentes históricos da ressuscitação cardiopulmonar desde a época bíblica até a criação do comitê da American Heart Association. Sendo este comitê, atualmente, referência no ensino e pesquisa em ressuscitação cardiopulmonar no mundo, seu investimento possibilitou a partir do ano 2000, o desenvolvimento de um consenso mundial de ressuscitação que gerou diretrizes internacionais únicas sobre o atendimento a ressuscitação cardiopulmonar. En esta comisión, en la actualidad, una referencia en la enseñanza y la investigación en la resucitación cardiopulmonar en el mundo, permite su inversión a partir de 2000, el desarrollo de un consenso mundial de que la reanimación directrices sólo genera la atención internacional sobre la reanimación cardiopulmonar. Conclusión: se observó que en los últimos años las técnicas de PCR produce un impacto en la sociedad moderna hasta el punto de generar nuevas expectativas frente a la vida y la muerte, revelando es esencial que el profesional de la salud que comprende a su competencia técnica y científica para un desfile corazón puede hacer una diferencia entre la vida y la muerte de un paciente.
