Komunitas nelayan adalah kelompok masyarakat yang hidup di daerah pesisir. Streotipe yang melekat pada mereka adalah kelompok yang minim dalam peningaktan Sumber Daya Manusia (SDM). Pada konteks penghasilan, komunitas nelayan memiliki pendapatan yang dapat digolongkan dalam kelompok masyarakat sejahtera. Namun, kurangnya SDM membuat kelompok ini tidak mampu mengelola keuangan secara baik. Olehnya itu, dipandang perlu melaksanakan kegiatan pengabdian kepada masyarakat dengan mengusung konsep pemberdayaan komunitas nelayan melalui pemanfaatan Lembaga Ekonomi Kerakyatan. Tujuan dari kegiatan pengabdian kepada masyarakat ini adalah untuk meningkatkan pemahaman komunitas nelayan dalam pengeloaan keuangan dan pentingnya lembaga ekonomi kerakyatan dalam peningkatan kesejahteraan. Kegiatan ini dilaksanakan di Balai Desa Bajo Indah, Kecamatan Soropia, Kabupaten Konawe. Dalam upaya pemecahan masalah komunitas nelayan, maka digunakan metode Focus Goup Discussion (FGD) atau diskusi kelompok terarah untuk mengetahui permasalah serta penanganan atau penemuan solusi secara bersama. Hasil dari kegiatan pengabdian kepada masyarakat, yakni; Kesepakatan bekerjasama dengan berbagai pihak untuk mengikuti kelas belajar informal; Meningkatnya pemahaman komunitas nelayan terhadap tata kelola kuangan rumah tangga; Komunitas nelayan secara bersama-sama menentukan potensi resource, pemanfaatanya, serta pengelolaannya; Komunitas nelayan dan pemerintah desa sepakat membentuk koperasi desa yang bersumber dari dana ADD.

Penyakit Tidak Menular (PTM) menjadi salah satu penyebab utama kematian di dunia. Peningkatan PTM juga terjadi di Provinsi Sumatera Barat. Demikian juga halnya dengan Kota Solok. Salah satu kebijakan pengendalian PTM saat ini adalah melalui Pos Pembinaan Terpadu (Posbindu) PTM berbasis masyarakat dengan melakukan deteksi dini, pemantauan faktor risiko dan tindak lanjut secara promotif dan preventif. Tujuan penelitian ini adalah menganalisis pelaksanaan program posbindu PTM di Kota Solok dengan menganalisis input (kebijakan, ketersediaan sumber daya manusia, anggaran biaya, sarana dan prasarana, petunjuk teknis, peran serta kemitraan), process (perencanaan, pelaksanaan, monitoring evaluasi) dan output (capaian indikator posbindu PTM). Penelitian studi kebijakan dengan pendekatan kualitatif ini dilaksanakan pada bulan April-November 2018. Teknik pengumpulan data dilakukan melalui wawancara mendalam, observasi, Focus Goup Discussion (FGD) dan telaah dokumen. Hasil penelitian menunjukkan bahwa kebijakan posbindu ditetapkan melalui SK Walikota Nomor 188 tahun 2018 namun belum tersosialisasikan sampai pelaksana posbindu, SDM pelaksana posbindu masih belum mencukupi karena baru memiliki 3 orang kader, anggaran biaya berasal dari APBD dan BOK, sarana dan prasarana masih belum memadai, petunjuk teknis telah ada tapi belum dikuasai oleh kader dan kemitraan dengan lintas sektor juga belum terjalin. Pada perencanaan, posbindu belum mempunyai Plan Of Action (POA) dan belum pernah dilakukan sosialisasi dan advokasi, pelaksanaan sudah memakai sistem lima meja, monitoring dan evaluasi masih belum optimal, output posbindu PTM di Kota Solok masih belum tercapai karena angka kunjungan masih rendah.

Actualmente la música, aunque está perdiendo representatividad y presencia en las instituciones educativas, contrariamente, está tomando relevancia en la sociedad, en la calidad de vida de las personas, en la inclusión educativa y social, además se estima que es una potente herramienta de vertebración social, de empleabilidad y sostenibilidad. A través de proyectos artísticos, la música se comporta como un nexo de unión que permite la implementación en diferentes contextos y perfiles, de acciones capaces de potenciar la motivación, la autoestima, así como mejorar las competencias sociales y personales. Entre los problemas y los retos actuales, nos encontramos por una parte con la empleabilidad y su relación con la educación y la formación en los futuros maestros/as, y otra parte la importancia de la agenda 2030 y los 17 Objetivos de Desarrollo Sostenible (ODS). Nuestra propuesta se fundamenta en una estancia de investigación que se realizó en la universidad de Barcelona, a través de una convocatoria lanzada por la Conselleria D’educació, Investigació, Cultura i Esport. Direcció General d’Universitat, Investigació i Ciència de la Generalitat Valenciana. Entre los principales objetivos, además de la elaboración de un informe sobre los beneficios de los proyectos artísticos, para la transformación social y educativa, tuvimos presente la relación de la música con la formación de los futuros maestros de educación primaria y los beneficios del arte en general y de la música en especial. Para obtener información se realizaron diferentes entrevistas y se organizaron dos focus goup. Hallamos importante información que refuerza la importancia de la formación musical y sus beneficios, así como la relevancia de los proyectos artísticos en la sociedad, sin ninguna discriminación o exclusión. También reflexionamos sobre el impacto económico de los proyectos artísticos, no solo desde la empleabilidad sino también de lo que puede suponer su relación con la salud y la calidad de vida.

ABSTRACT INTRODUCTION Gout is a common form of arthritis that is typically managed in primary care. Gout management guidelines emphasise patient education for successful treatment outcomes, but there is limited literature about the educational experiences of people living with gout in New Zealand, particularly for Māori, who have higher gout prevalence and worse gout outcomes than Pākehā. AIM To explore gout patient education in primary care from the perspectives of Māori and Pākehā people with gout. METHODS In total, 69 people with gout were recruited through primary care providers in three locations across New Zealand. Nine semi-structured focus groups were run with Māori and Pākehā participants in separate groups. RESULTS Thematic analysis yielded two themes in relation to gout education: (i) ‘Multiple sources of gout education’; and (ii) ‘Gaps in gout knowledge’. Participants received education from general practitioners, educational resources, family and friends, and their own experiences. Māori participants preferred information to be kanohi-ki-te-kanohi (face-to-face) and with significant others present where necessary. Participants disclosed gaps in gout’s epidemiology and management. Pākehā and Māori participants reported limited understanding of the genetic basis of gout or the biological underpinnings of the condition and its treatments, but learned treatment adherence through experience. DISCUSSION Despite improved gout patient education, knowledge gaps remain and may contribute to poor medication adherence. Gout patient education interventions need to be tailored to culture and incorporate suitable methods of disseminating information about gout management.

Health equity can be defined as the absence of systematic disparities in health between more and less advantaged social groups. Gout is one of the most common forms of arthritis and disproportionally affects Indigenous peoples, including Māori in Aotearoa New Zealand. Inequities in gout management are well documented and clearly evidenced in Indigenous populations. For example, while gout occurs at a younger age and is more severe in Māori, there is less regular dispensing of urate-lowering therapies. Indigenous peoples are also under-represented in clinical trials. Herein, we will review inequities in gout using Aoteoaroa New Zealand as an example. We will explore reasons for health inequities and challenges that need to be faced to achieve health equity.

Aim. To study the effect of ivabradine and bisoprolol on cardiac hemodynamics and diastolic remodeling in gout patients with coronary artery disease and hypertension and without left ventricular systolic dysfunction.Material and methods. The open randomized parallel clinical trial of 35 men with intercritical gout at the age of 41,4±3,3 years, with class II-III stable angina, hypertension and sinus rhythm without data suggestive of heart failure was performed. All patients included in the study were randomly divided into two groups: eighteen patients took bisoprolol at a dose of 2,5 to 10 mg/day, 17 subjects received bisoprolol 2,5 mg/day with ivabradine (Coraxan, SERVIER, France) 5 mg 2 times a day. Adjustment of the therapy was carried out every 2 weeks until the target heart rate (HR) was reached at 55-60 beats/min and then remained unchanged until 12 weeks of therapy. All patients underwent echocardiography, 24-hour Holter and central aortic blood pressure monitoring, and 3-minute cycle ergometer test with a power of 25, 50, 75 and 100 watts.Results. There was a comparable decrease in the maximum and minimum 24-hour average heart rates in patients receiving only bisoprolol and those taking bisoprolol+ivabradine. Patients taking bisoprolol+ivabradine had a decrease of central systolic and diastolic blood pressure (BP). Pulse pressure in the bisoprolol group increased by 17,7% (p=0,02), and when ivabradine was added, on the contrary, it decreased by 7,0% (p=0,04). Twelve-week therapy with beta-blockers and ivabradine was accompanied by an effective decrease in the pulse wave velocity in both groups (p<0,05). All gout patients did not have a decrease of systolic function and there was an improvement in diastolic remodeling with beta-blockers and ivabradine therapy.Conclusion. The results obtained indicate that the addition of ivabradine to bisoprolol leads to an effective decrease in heart rate, an improvement in arterial stiffness and exercise tolerance. Combination therapy with ivabradine is accompanied by an improvement in clinical outcomes using lower doses of bet-blockers, which requires further study and a double-blind controlled study.

BACKGROUND AND CONTEXT: In New Zealand, the highest prevalence of gout is in Maori and Pacific people. Counties Manukau District Health Board (CMDHB) has the highest Maori and Pacific population of any New Zealand District Health Board. A CMDHB study found that a high proportion of patients with gout were also at increased risk of cardiovascular disease. ASSESSMENT OF PROBLEMS: The primary objective was to examine whether the control of gout had changed over time at one clinic. The secondary objective was to assess the management of cardiovascular risk factors in patients with gout at that clinic. RESULTS: The mean serum uric acid level of patients with gout in the practice had risen in comparison with a similar audit carried out in March 2009. This indicates that the control of gout for patients at the practice has worsened over time. Many patients had not had an annual serum uric acid test. STRATEGIES FOR IMPROVEMENT: A repeat uric acid level was scheduled for all patients with gout in the practice, with follow-up appointments to be arranged if the result was abnormal. LESSONS: Gout is often suboptimally managed. Serum uric acid levels may only be tested when a patient presents with an acute attack of gout. Consideration should be given to a minimum of annual serum uric acid levels. Appropriate management of modifiable cardiovascular risk factors in this particular cohort is important and should be a particular focus of care. Looking beyond legislative behavior, Gendering the GOP: Intraparty Politics and Republican Women’s Representation in Congress reveals changes over time in the way Republican congresswomen (1) claim to represent women and (2) work together to advance their own interests within the party. Through extensive interviews with women members of Congress and in-depth analyses of House floor speeches, the book details how women have both navigated and shaped existing gender dynamics within the House GOP conference. It demonstrates that Republican women in Congress are not merely gender-blind partisans. Rather, it complicates traditional understandings of the relationship between descriptive and substantive representation, showing how polarization and party competition have incentivized Republican women to organize around their partisan-gender identity—distinguishing themselves from both Democratic women and Republican men. Doing so has increased their visibility as party messengers, while simultaneously limiting their legislative power in the institution. This book shines light on the ongoing challenges Republican women face, the intricate gender dynamics they must learn to navigate in their party, and potential opportunities for change.

How are words coined? That provides the focus of The Hidden History of Coined Words. Based on extensive research, its author, Ralph Keyes, has determined that successful neologisms are as likely to be created by chance as by intention. A remarkable number were coined whimsically, to taunt, even to prank. Miscegenation resulted from a hoax, suffragette to disparage radical suffragists. Wisecracking produced scientist, and crowdsource. More than a few neologisms resulted from happy accidents such as typos, mistranslations, and misheard words (like bigly, and buttonhole). Isaac Asimov introduced robotics in a story without realizing it was his own coinage. Many of the word coiners Keyes writes about come from unlikely quarters. Neologizers (a Thomas Jefferson coinage) include not just learned scholars and literary lions but cartoonists, columnists, children’s authors. Wimp originated in an early 20th century children’s book series called The Wymps, goop from a series about The Goops. Nerd first appeared in Dr. Seuss’s 1950 book If I Ran the Zoo. South African General Jan Smuts, an architect of what became apartheid, coined the word holism in 1926. Competing claims to have coined terms such as gonzo, affluenza, and yuppie are assessed, as are epic battles fought between new word partisans and those who think we have enough words already. A concluding chapter assesses how words are successfully coined and become part of the lexicon.

The Oxford Handbook of Rheumatology, 4th edition, emphasizes the importance of evidence-based therapeutics and therefore devotes a whole chapter to medications used to treat rheumatological and bone disease (‘Drugs used in rheumatological disease’). Details about treatments are cross-referenced to specific use of medications for specific diseases and conditions elsewhere in the book. There are chapter sections on analgesics including opiate-based medications, NSAIDs, conventional synthetic disease-modifying antirheumatic drugs (DMARDs), biologic DMARDs, intravenous immunoglobulin, and other immunosuppressants. There is a specific focus in the chapter on drugs used for Raynaud’s disease, osteoporosis (e.g. bisphosphonates), pulmonary hypertension, gout, and hyperuricaemia. Overall there is reference to both adult and paediatric disease treatments in the chapter.

Abstract This chapter focuses on how surfaces on suspended particles, nanoparticles, and sediments affect solute behavior and concentrations. The molecular structures of solids that yield various kinds of surfaces and interfaces are described, along with features of surfaces themselves. Forces that attract/repel solutes to/from surfaces are described with emphasis on electrical charge and excess surface energy, called surface tension. A major focus is on sorption, i.e., the accumulation of solutes onto surfaces. The Freundlich and Langmuir models that quantify this process are described along with more complicated models developed to overcome their limiting assumptions. Mechanistic models that consider the physics of the electrical double layer at solid-solution interfaces, e.g., the Gouy-Chapman double layer model, are developed, along with modern extensions. Surface complexation models that combine double-layer physics with a chemically oriented approach to quantify the interactions between ionic solutes and surfaces are described and examples of their use are given.

This epilogue, written after the 2020 elections and the inauguration of President Joe Biden, first looks at the refusal of former president Donald Trump to accept his electoral defeat and his incitement of his hard-core supporters to disrupt the counting of electoral votes in the Senate chamber of the Capitol on January 6, 2021. Their violent storming of the Capitol, resulting in five deaths, prompted the Democratic-controlled House of Representatives to impeach Trump for “incitement to insurrection.” However, only seven of fifty GOP senators joined all fifty Democrats to convict Trump, short of the required two-thirds majority of sixty-seven. One major consequence of Biden’s victory was his pledge, in a document entitled “Lift Every Voice: The Biden Plan for Black America,” to focus on “rooting out systemic racism” in American institutions. The epilogue then looks at the impact of Trump’s (and his followers) racism on two major social and political issues: the greater infection, hospitalization, and death rates of Blacks from the coronavirus pandemic, and racial justice and police reform after the death of George Floyd in Minneapolis, Minnesota, in May 2020. Attempting to arrest Floyd, a forty-six-year-old Black man, for allegedly trying to pass a counterfeit $20 bill, Police Officer Derek Chauvin pressed his knee into Floyd’s neck for almost ten minutes while he was handcuffed and prone on the street. The jury verdict in Chauvin’s murder trial, unknown at this writing, will play a significant role in determining how Americans will support, or oppose, programs and policies to “root out systemic racism,” as President Biden has pledged to combat. The reign of White Men’s Law must end.

The increase in patients suffering from ‘lifestyle diseases such as coronary heart disease, atherosclerosis, diabetes and gout has been associated with the amount and nature of fats in our diets. In many of these diseases, macrophages and their role in lipid metabolism greatly impacts the development and severity of the disease. Fatty acids, which are a sub-class of lipids, have many different types depending on the number of carbon atoms and the presence of carbon double bonds (i.e. saturated and unsaturated fatty acids). The chemical properties of each fatty acid depend on the number of carbon atoms and degree of saturation. Therefore, it is important to investigate how macrophages metabolize these different types of fatty acids. In this research, we focus on the distribution and the uptake of four fatty acids: palmitic acid, stearic acid, oleic acid, and linoleic acid, shown in Table 1.

Abstract The objective of this paper is to showcase successful and innovative means and techniques to improve and enhance centrifugal gas compressors (CGCs) performance, using methods to minimize power consumption, with no need for capital investment. These techniques will assure, if effectively followed, considerable reduction of the consumed energy. CGCs are the most widely used equipment in the oil and gas industry to boost gas, mainly hydrocarbons, to satisfy process treatments and pipeline requirements. In addition, CGCs are one of the major energy consumers, and therefore present an exceptional opportunity for saving energy. Focusing on lowering inlet gas temperatures, considering suction throttling of discharge pressure instead of the traditional discharge throttling, will help to reduce energy consumption. In this paper, a detailed analysis of factors aggravate or lead to undesired CGCs performance will be discussed along with solutions to minimize adverse impact. For example, operating the gas compressors at relatively high inlet temperature will result in higher energy consumption. After performing need analysis, results prove that we would save 3-7% of running compressors consumed energy. In addition, during compressor design phase, it was found that most motor driven compressor system uses discharge throttling, which incurs high-energy consumption. Instead, it is recommended to consider suction throttling to control discharge pressure, as will be explained. This paper will focus on a detailed case study in one of the running CGCs in an upstream gas-oil separation plant (GOSP-A). This paper proves the effectiveness of the proposed techniques in reinstating the CGCs in GOSP-A, to ensure better performance and save energy. This innovative technique is based on extensive process data analysis — evaluating operating, design data, related performance curves, and reviewing international standards. It will be illustrated that this type of analysis and techniques is a valuable tool for saving energy, in most cases, at oil and gas industries

Abstract The current oil and gas production scenario is characterized by high market volatility, increasing Energy Efficiency targets and subsequent requirements in operation excellence. This leads to an ever-increasing attention towards more efficient asset management. This paper illustrates the application of Advanced Process Control techniques to beyond-standard targets of process parameters and product specifications, supporting carbon footprint reduction process. The Oil & Gas treatment facility is made of 4 oil and 5 gas process trains with an overall gross production of more than 100 KBOED. An advanced Adaptive Process Control application has been first tested on a Digital Twin and then implemented on one single GOSP (Gas Oil Separation) section. This application has then been extended to all the process trains, including the Gas treatment section. A fully integrated Adaptive Process Control Orchestrator and Dynamic Optimizer has been deployed across the entire site to grant an efficient load management between trains, maximizing hydrocarbon production and the energy efficiency of the asset maintaining every equipment within the proper operating windows. APC deployment on a brownfield upstream facility demonstrated to improve asset efficiency by optimizing performance within design capabilities. Energy consumptions were significantly reduced by product specification prediction and early identification of potential process upsets. A sensible production capacity increase has also been recorded thanks to process stabilization. Results eventually show that in an industry where process optimization by design changes is not always feasible or cost-effective, a well-tuned APC controller can increase asset performance with a minimum investment. Moreover, project delivering didn't require disruption in normal operation nor design changes in existing equipment. Significant results have been obtained in terms of alarms reduction through a standardization of the control philosophy and also in terms of energy/CO2 reduction with an average −15% specific energy and CO2 reduction achieved just in GOSP section. Moreover, APC has demonstrated a critical support to operators during equipment upsets through the "auto-pilot" control of the process trains not affected by the transient. Advanced Process Control is a multiple input – multiple output (MIMO) digital tool that nowadays has never been systematically implemented in upstream facilities. Main reasons are the high volatility of reservoir fluids flowrates and product overspecification that assures safe operations and production continuity. APC changes significantly control-room operator working routine of an upstream production plant. Without APC, operator's experience relies on 1-to-1 relationships between one manipulated variable (MV) and one controlled variable (CV) even if more than one CV is affected by one MV move. Optimization runs are carried out with a frequency lower than a minute and within every run, a move could be assigned to any manipulated variable. APC operator can focus on CV limits assignments, being relieved of smoothing PID loops moves.

Abstract The objective of this paper is to showcase the successful and innovative troubleshooting data analysis techniques to operate a TEG dehydration system optimally and reduce glycol loss and to meet the product specifications in one of the gas dehydration systems in an upstream gas oil separation plant (GOSP). The gas dehydration system using Triethylene Glycol (TEG) is the most widely used and reliable gas dehydration system in upstream operation. These proven data analysis techniques were used to tackle major and chronic issues associated with gas dehydration system operation that lead to excessive glycol losses, glycol degradation, and off-specification products. Glycol loss is the most important operating problem in the gas dehydration system and it represents a concern to the operation personnel. Most dehydration units are designed for a loss of less than 1 pound of glycol per million standard cubic feet of natural gas treated, depending on the TEG contactor operating temperature. In this paper, comprehensive data analysis of the potential root causes that aggravate undesired glycol losses degradation and off-specification products will be discussed along with solutions to minimize the expected impact. For example, operating the absorption vessel (contactor) or still column at high temperature will increase the glycol loss by vaporization. Also, the glycol losses occurring in the glycol regenerator section are usually caused by excessive reboiler temperature, which causes vaporization or thermal decomposition of glycol (TEG). In addition, excessive top temperature in the still column allows vaporized glycol to escape from the still column with the water vapor. Excessive contactor operating temperature could be the result of malfunction glycol cooler or high TEG flow rate. This paper will focus on a detailed case study in one of the running TEG systems at a gas-oil separation plant.

Engineers' main focus is to apply their scientific knowledge to produce solutions to technical problems. Then they can optimize these solutions based on a set of constrains which involve material, technological, economic, environmental and human related requirements. In his paper Penny [1] described engineering design as the "art using science in achieving useful end product". Engineering design reflects a creative activity that requires a sound knowledge of mathematics, physics, chemistry, mechanics, thermodynamics, hydrodynamics, electrical engineering, production engineering, materials technology, machine elements and design theory [2]. As such there was been a growing awareness of the importance of engineering design in education. In order to understand and implement effectively the teaching of engineering design, the design processes of engineers have been studied extensively over different approaches. Moreover the relationship between engineering design strategy and cognition has attracted much interest; cognition describes the way individuals solve problems, think, perceive, and remember information [8]. However most of the studies generally reach the similar outcome that identifies the design as not an "algorithmic activity". While engineers develop a design, the goals, problems and constraints are not particularly defined and are highly changeable towards reaching a solution. There might be general guidelines for them to follow in the design process, however there is no consensus regarding one correct procedure or correct strategy to follow in order to reach a solution. So far studies pertaining to the design process and cognition in engineering have utilised sketches, drawings, written communication, hands on model building etc., however there less in the literature investigating whether a game theory approach can contribute to a better understanding of the engineering design process, and facilitate in a more effective teaching of design courses. This research paper was designed to investigate whether a game AI module of goal oriented action planning (GOAP) which was first implemented in the commercial game called F.E.A.R, for developing its characters behaviour, can align with the engineering design process.
