Повний текст джерелаInfectious agents are recognized as a cause of maternal and fetal morbidity and mortality. The numerous infectious epidemics that have occurred in French Guiana reveal the exposure of the population to numerous pathogens. French Guiana is experiencing rapid growth and boasts one of the highest birth rates in France. Studies confirm that infant mortality remains high, and premature births are among the highest in France. Many questions arise regarding the impact of various infectious epidemics and their role in this increased risk. To study the extent and consequences of infectious diseases in pregnant women in western French Guiana, this work aimed to study the descriptive and analytical epidemiology through scientific data from the western French Guiana maternity. It will also analyze the link between different infectious exposures and the increased risk of maternal and neonatal adverse outcomes. To achieve our objectives, we conducted non-interventional prospective studies, following an exposed-unexposed model, as well as other retrospective descriptive studies involving pregnant women exposed to infectious threats and residing in Western French Guiana.According to the results of our research, we found that young age, nulliparity, and social precarity are major risk factors for infections during pregnancy. We confirmed the link between exposure to fever during pregnancy and the increased risk of prematurity, fetal loss, and delivery by cesarean section in the population of mothers who experienced a fever, compared to those who did not. We have confirmed the teratogenic potential of the Zika virus and the Tonate virus. In fact, we have described the first case of vertical transmission of the Tonate virus and its associated fetal birth defects. Furthermore, we have demonstrated the relationship between Zika virus infection and delayed neurological and neurosensory development in fetuses, newborns, and children up to the age of 3. In 2020, the first wave of COVID-19 was particularly intense and led to a higher risk of postpartum hemorrhage, transfusion, and admission to intensive care units in the population of infected women compared to those who were not infected. It also led to a higher risk of fetal adverse outcomes. Social precarity and changes in the healthcare system during the epidemic may have played an indirect role. Epidemics are sometimes associated, as in 2020, the COVID-19 epidemic, a dengue epidemic, and an increase in syphilis cases coexisted. We have reported an increase in the number of syphilis cases among pregnant women, resulting in a true syphilis epidemic in the west. More than half of non-treated pregnant women will have an unfavorable pregnancy outcome. Syphilis infection is associated with an increased risk of prematurity, intrauterine fetal death, perinatal death, and congenital syphilis with a risk of long-term sequelae.Thus, infectious agents are a significant cause of fetal and neonatal morbidity and mortality in French Guiana and require special monitoring. It represents at least 4% of direct causes of fetal loss in the territory. The outcomes of this work are significant. They will certainly help optimize screening, initial assessment, identify exacerbating risk factors, guide therapeutic management, and monitor infections in mothers to reduce maternal and fetal morbidity and mortality resulting from these infections

Повний текст джерелаThe epidemiology of sexually transmitted infections (STIs) is poorly documented in Mayotte especially among pregnant women (PW) and knowledge of determinants that increased STI in the island, and in this particular socio-economic and health situation, is needed. The objectives of this study were to estimate the frequency and risk factors associated with HIV, HBV, and syphilis, to study the HBV vaccination and describe the knowledge, attitudes, beliefs and behaviors related to HIV/AIDS-STIs in PW. A prospective cross-sectional study was conducted among 671 PW followed in Mayotte public prenatal clinic (Protection Maternelle et Infantile (PMI)) services. No case of HIV seropositivity was observed. The prevalence of HBsAg of HBV was 3.4% and of active syphilis was 2.1%, but the prevalence of HBV infection and HBV vaccination was respectively 35.5% and 18.6%. The HBV infection was associated with birthplace (Comoros), behavioral factors and history of STIs. Syphilis was rather associated with lack of education and history of STIs. The HBV vaccination was associated with sociodemographic determinants. The socio-behavioral study showed that there is a good knowledge of HIV/AIDS-STIs in PW despite the practice of some risky sexual behaviors. This work has helped to draw up an update of HIV and STIs, and their determinants among PW in Mayotte, and could lead to the development of prevention methods adapted to this context

Повний текст джерелаEmtricitabine is a key antiretroviral drug in the strategy to fight against HIV infection. Indeed, emtricitabine is recommended in first-line treatment for HIV infection in adults, as well as for the prevention of HIV transmission. Although emtricitabine is a key molecule, it remains poorly studied until now. It is therefore essential to analyze and characterize the pharmacokinetics of emtricitabine in the different populations that may be exposed to this drug. A population approach was used to perform pharmacokinetic analyses, allowing us to describe the pharmacokinetics with few samples per individual (ethical constraints, difficulty of samples collection...) and allowing us to explain the observed variability. The study reporting emtricitabine pharmacokinetics in adults highlighted a change in emtricitabine kinetic profile depending on the state of renal function. The developed model allowed us to evaluate the current dosing recommendations. To optimize efficiency, simulations of alternative regimens in patients with moderate renal impairment were performed. As part of the prevention of mother to child transmission, the study describing emtricitabine pharmacokinetics in pregnant women showed a change in pharmacokinetics during the gestational period. However, it did not seem necessary to adjust the dosage during pregnancy. The latest work focused on emtricitabine pharmacokinetics in the male genital tract. Studying the penetration in the genital tract is of major interest for treatment but also for prevention of HIV transmission in the context of pre-exposure prophylaxis. Our study reported a significant distribution of emtricitabine in the genital compartment, with concentrations in seminal plasma higher than concentrations in blood plasma

Повний текст джерелаHIV infection in sub-Saharan Africa is a major public health threat particularly for girls and women ofreproductive age. The research presented in this thesis was conducted particularly in West Africa andthe specific objectives are i) to estimate the prevalence of intimate partner violence according to HIVserological status; ii) to estimate the incidence rate of pregnancy following ART initiation; iii) todetermine the effect of pregnancy after ART initiation on the risk of death, HIV-disease progressionand loss to follow-up. Firstly, in Togo, According to our findings, the prevalence rates of lifetimephysical and sexual violence (IPV) among HIV-infected women were significantly higher thanamong uninfected women (63.1 vs. 39.3%, p=0.01 and 69.7 vs. 35.3%, p=0.01, respectively).Secondly, Among HIV-infected West African women, the crude incidence of first pregnancy afterART initiation was 2.9 per 100 women-years [95% confidence interval (CI): 2.7 to 3.0] and it could beas high as 4.7 per 100 women-years (95% CI: 4.3 to 5.1) among women aged 25-29 years old. Finally,pregnancy after ART initiation appeared to reduce the risk of death or HIV-disease progression(Adjusted Hazard Ratio [aHR] =0.61; 95%CI: 0.40-0.92) and the risk of becoming LTFU at M48(aHR=0.74; 95%CI: 0.60-0.92) among West African HIV-infected women. Intimate partner violence ishighly prevalent among HIV-infected women and it may have negative repercussions on their healthstatus. Pregnancy is a common event after ART initiation and it might have repercussions on thehealth status of HIV-infected women. The design of safe motherhood programs addressed to HIVinfectedwomen and its integration within HIV care services must are a public health priority in sub-Saharan Africa
