RésuméLa neurotransmission de sérotonine (5-HT) est régulée, via un rêtro-contrôle négatif, par des réceptcurs situés an niveau du raphé nucleus et des terminaisons neuronales. Ces autorécepteurs sérotoninergiques ont été étudiés aux niveaux comportemental, électrophysiologique et biochimique. Les réponses comportementales et électropliysiologiques sont atténuées par l'administration chronique des antidépresseurs. De plus, in vitro, les inhibiteurs de capture de 5-HT atténuent l'aulorégulation de la libération de [3H]5-HT. La présente étude concerne l'effet, sur cette libération induite par stimulation électrique de l'hypothalamus; de rat et sa modulation, de l'administration chronique de 2 antidépresseurs, le citalopram, inhibiteur spécifique de la capture de 5-HT, et le milnacipran (anciennement midalcipran, F 2207), qui bloque la capture de 5-HT et de noradrénaline avec la même puissance. Le eitalopram était administré à 50 et à 10 mg/kg/j pendant 21 jours et 24 h après, les rats étaient décapités, des coupes d'hypoihalamus incubées avec la [3H]5-HT, superfusées avec une solution de Krebs et stimulées électriquement. La quantité de [3H]5-HT libélée par stimulation électrique étail augmented et l'effet inhibiteur de l'agoniste, LSD, diminué chez les rats traités. Ces effets étaient présents aux 2 doses mais plus marqués à la forte dose. Donc un traitement chronique avec le citalopram provoque une «down-regulation» de l'autorécep-teur sérotonergique, permettant ainsi une augmentation de la neurotransmission de 5-HT. Cet effet peut expliquer, an moins en partie, l’action antidépressive de ce produit et it esl possible qu'il représente un mécanisme fondamental à beaucoup d'antidépresseurs qui inhibent la capture de 5-HT. Cependant, après 21 jours de traitement avec le milnacipran, à 50 mg/kg/j, aucun des paramétres étudiés n'était modifié. Cela suggère que l’hypothèse de la «down-regulation» de l’autorècepteur n'est pas l'hypothèse universelle qui explique l’action de tous les antidépresseurs agissant sur la capture de 5-HT.

AbstractBackground:Essential tremor (ET) is the most common tremor disorder in adults. In addition to upper limbs, the tremor in ET may also involve head, jaw, voice, tongue, and trunk. Though head tremor (HT) is commonly present in patients with ET, large comparative studies of ET patients with HT (HT+) and without HT (HT−) are few.Methods:To determine whether ET with HT is a distinct clinical subtype by comparing ET patients with and without HT, a chart review of 234 consecutive patients with ET attending the neurology clinics of the National Institute of Mental Health and Neurosciences, India, was done. A movement disorder specialist confirmed the diagnosis of ET in all patients using the National Institutes of Health collaborative genetic criteria.Results:HT was present in 44.4% of the patients. Comparison between HT+ and HT− showed that the HT+ group patients: (1) were older, (2) had later onset of tremor, (3) had unimodal distribution of age at onset with a single peak in the fifth decade, (4) had more frequent voice tremor, and (5) were more likely to have mild cervical dystonia. HT was part of presenting symptoms in nearly two thirds of the ET patients and in the rest it was detected during clinical examination.Conclusions:Several demographic and clinical variables suggest that ET patients with HT have a distinct clinical phenotype.

Interactions between 5-hydroxytryptamine (5-HT) and endothelin-1 (ET-1) relative to contraction of rat aortic rings were examined in this study. Pretreatment of rings with threshold concentration of 5-HT potentiated the subsequent contractile response to ET-1. However, pretreatment with threshold concentration of ET-1 did not potentiate the contractile response to 5-HT. The 5-HT receptor antagonist LY 53857 blocked the synergistic contractile effects of 5-HT and ET-1 on rat aortic rings. Indomethacin and the thromboxane A2/endoperoxide receptor antagonist SQ 29548 also attenuated (P less than 0.05) the synergistic contractile effects of 5-HT and ET-1, suggesting release of thromboxane A2 or expression of thromboxane A2 receptors during this interaction. The calcium channel blocker verapamil also decreased the synergistic contractile effects of 5-HT and ET-1. Contraction of aortic rings by 5-HT alone was abolished by LY 53857 and attenuated by verapamil, diltiazem, and SQ 29548. Decrease in the force of contraction by verapamil as well as diltiazem indicates activation of voltage-dependent calcium channels during 5-HT-mediated contraction and perhaps during amplification of the vasoconstrictor activity of ET-1 by 5-HT.

Elevated levels of serotonin (5-HT) and endothelin-1 (ET-1) may be involved in cardiovascular complications of diabetes mellitus. Data suggest supraphysiological concentrations of 5-HT (10−6 M) potentiate the ability of ET-1 to stimulate DNA synthesis and vascular smooth muscle cell (VSMC) proliferation in vitro via activation of mitogen-activated protein kinase (p42/44 MAPK) and Janus kinase 2 (JAK2) pathways. Additionally, 5-HT enhances agonist-induced contractions via p42/44 MAPK and an unknown tyrosine kinase. However, the exact mechanisms of the 5-HT/ET-1 interactions and whether these effects occur at physiological levels (10−9 M) are unknown. Therefore, we hypothesized that interactions between 5-HT and ET-1 at physiological concentrations in VSMC enhanced activation of both p42/44 MAPK and JAK2 pathways contributing to vascular growth and contractile responses. With the use of rat VSMC and Western blot analysis, our data suggest no effect of acute (30 min) preincubation with 5-HT (10−9 M) and/or ET-1 (10−9 M) on the activation of either pathway in normal or high glucose conditions. To determine if there was altered vascular reactivity in intact vessels we tested the effects of 5-HT and ET-1 interaction using myographs to measure isometric contractions of rat thoracic aortic rings. 5-HT (10−9 M) and ET-1 (10−12 M) stimulate enhanced contractile responses to each other that were inhibited by JAK2 and p42/44 MAPK antagonists. Our findings demonstrate that both 5-HT and ET-1 at physiological concentrations could interact with each other and activate p42/44 MAPK and JAK2 signaling pathways to cause an increase in smooth muscle contraction that could lead to altered vascular function.

A high adherence to a Mediterranean diet has been related to numerous beneficial effects in human health, including a lower incidence and mortality of prostate cancer (PCa). Olive oil is an important source of phenolic bioactive compounds, mainly hydroxytyrosol (HT), of this diet. Because of the growing interest of this compound and its derivatives as a cancer chemopreventive agent, we aimed to compare the in vitro effect of HT isolated from olive mill wastewaters and five semisynthetic alkyl ether, ester, and nitro-derivatives against prostate cancer (PCa) cell lines. The effect in cell proliferation was determined in RWPE-1, LNCaP, 22Rv1, and PC-3 cells by resazurin assay, the effect in cell migration by wound healing assay, and tumorsphere and colony formation were evaluated. The changes in key signaling pathways involved in carcinogenesis were assessed by using a phosphorylation pathway profiling array and by Western blotting. Antiproliferative effects of HT and two lipophilic derivatives [hydroxytyrosyl acetate (HT-Ac)/ethyl hydroxytyrosyl ether (HT-Et)] were significantly higher in cancerous PC-3 and 22Rv1 cells than in non-malignant RWPE-1 cells. HT/HT-Ac/HT-Et significantly reduced migration capacity in RWPE-1 and PC-3 and prostatosphere size and colony formation in 22Rv1, whereas only HT-Ac and HT-Et reduced these functional parameters in PC-3. The cytotoxic effect in 22Rv1 cells was correlated with modifications in the phosphorylation pattern of key proteins, including ERK1/2 and AKT. Consistently, HT-Ac and HT-Et decreased p-AKT levels in PC-3. In sum, our results suggest that HT and its lipophilic derivatives could be considered as potential therapeutic tools in PCa.

Les hormones thyroïdiennes (HT) sont essentielles pour le bon fonctionnement du cerveau tout au long de la vie des vertébrés, dès les stades précoces du neuro-développement. Des études épidémiologiques ont montré l’importance des HT de la mère pendant les premiers mois du développement fœtal : une déficience précoce en HT maternelles entraîne à long terme des altérations du développement cognitif et du comportement social de l’enfant. L’apport des modèles animaux, non seulement les modèles mammifères mais également les modèles alternatifs (poisson zèbre, xénope, poulet), a permis de décrypter les mécanismes cellulaires et moléculaires gouvernés par les HT lors du développement cérébral. En particulier le modèle rongeur a contribué à montrer que les HT ont également un rôle crucial chez l’adulte, principalement au sein de deux niches neurogéniques majeures, la zone sous-ventriculaire et la zone sous-granulaire de l’hippocampe où elles régulent finement le destin des cellules souches neurales (CSN). Une question essentielle en biologie des cellules souches est de comprendre, comment les HT gouvernent le devenir des CSN vers un destin neural ou glial et ce, afin de contribuer au développement du cerveau et de maintenir ses fonctions tout au long de la vie adulte dans des conditions physiologiques et lors d’un dommage cérébral (maladies neurodégénératives, maladies démyélinisantes ou accident vasculaire cérébral). Notre revue fait le point sur les connaissances actuelles sur le rôle d’un signal endocrinien clé, les HT, lors du développement du cerveau et de la neurogenèse adulte, et principalement chez les mammifères, notamment l’Homme.

Abstract The recent discovery of various mutations of the CALR gene that are mutually exclusive with JAK2 and MPL mutations has allowed a correct diagnosis in about 90% of adult cases of essential thrombocythemia (ET). Moreover, the mutation status of JAK2 and CARL defines subtypes of ET in adults with a substantially different clinical course and outcome. Based on our experience, we suggested that primary thrombocythemia (PT) in children is characterized by subtypes that differ from those found in adult ET. The present study was carried out in children and adolescents with PT in order to (a) characterize the various subtypes of the disease and (b) analyze their clinical and biologic features, treatment approach and outcome. PT patients aged <20 years (yrs) at diagnosis (dx) were evaluated for mutations of JAK2, thrombopoietin (TPO) and its receptor (MPL) and CALR genes, and for clonal hematopoiesis (females). The presence of MPLS505A (confirmed on DNA from buccal swabs) defined a hereditary thrombocytosis (HT). ET was diagnosed according to WHO 2008 criteria. For wild type patients, an additional inclusion criteria was a follow-up >24 months. Among 58 PT patients (males: 23; females: 35; median age at dx: 14.4 yrs), 21 (36%) had HT due to MPLS505A, 14 were JAK2V617F-mutated (24%), 9 (16%) harbored CALR mutations and 14 (24%) were wild type for JAK2, CALR and MPL (Fig 1). JAK2- and CALR-mutated were older than those with wild type ET or with HT (median age, 17.6 and 16.1 vs 10.4 and 13.7 yrs, p .028). As to the hematologic findings, HT patients showed both hematocrit values (median, 36.3%) and leukocytes counts (median, 9.53 x109/L) significantly lower than ET patients, whatever the subtypes (median, 41.2% and 11.2 x109/L, p .006 and p .029, respectively). No differences were found with regard to platelets both between HT and ET and among the different ET subtypes. JAK2-mutated patients exhibited more frequently symptoms (69%) compared to CALR-mutated (22%), wild-type ET (14%) and HT (14%) patients (p. 0057). Splenomegaly at diagnosis was recorded more frequently in JAK2-mutated than in CALR-mutated or wild type-ET or HT (50%, 33% 21% and 14% , respectively, p .122). Antiplatelet agents, mostly acetylsalicylic acid (ASA), were started less frequently in HT than in ET patients, irrespective of the subtypes (57% vs 81%, p .05). The use of ASA progressively decreased over the time; at the last follow-up, 2 patients with HT, 2 CALR-mutated and 1 JAK2-mutated patients were still receiving ASA, while no wild type ET patient was on treatment. Cytoreductive agents, hydroxyurea and/or interferon and/or anagrelide, were used in a minority of HT patients (19%) in comparison with ET patients (65%), p .001, mainly with those wild-type (78%, p <.001). At the last observation, one HT patient was still receiving cytoreductive agents compared to 30% of ET patients whatever the subtypes (p .024). After a median follow-up of 196 months (similar in the different subtypes), all patients are alive. On the whole, 5 thrombotic events were recorded in 3 patients with HT and in 2 ET patients (1 JAK2-mutated and 1 JAK2 and CALR wild-type), without any significant thrombophilic abnormalities during treatment with ASA and/or cytoreductive agents. A progressive splenomegaly was recorded in 9 (15%) patients (2 HT, 4 JAK2-mutated, 3 CALR-mutated) and it was combined with grade ≥2 medullar fibrosis in 2/4 JAK2-mutated and in 2/3 CALR-mutated patients. None of the JAK2 and CALR wild-type patients had spleen enlargement or reticulin fibrosis (p .022). Two untreated patients (1 HT and JAK2 and CALR wild-type) developed malignancies. On the whole, these data emphasize that in young patients with PT, hereditary forms can be frequently observed. Thrombotic events, recorded mainly in HT patients despite treatment with ASA, were probably due to a MRP4 protein overexpression that was found in our MPLS505A HT. Moreover, our observations highlight that, in contrast to adult ET, more than one third of young ET patients have no JAK2 or CALR mutations. Figure 1 Figure 1. Abstract Background and aims. Essential thrombocythemia (ET) rarely occurs in children and adolescents. In our experience, 40% of pediatric patients with primary thrombocythemia (PT) have JAK2 V617F or CALR mutations, 24% have a diagnosis of ET without any known molecular markers while 36% have hereditary thrombocytosis (HT) with a MPLS505A mutation. Thrombotic events, frequent in adult ET presenting high-risk factors and rare in pediatric PT, have been observed in pediatric HT patients with MPLS505A mutation in treatment with aspirin (ASA). The multidrug resistance protein-4 (MRP4) is an ATP-binding cassette transporter involved in the efflux of several pharmacological and physiological compounds. MRP4 has been identified as a modulator of ASA action in platelets; in addition, it also influences platelet activation. Recent studies have shown that MRP4 over-expression has a role in reducing the effect of ASA in patients who have undergone a bypass surgical procedure and that ASA induces platelet MRP4 upregulation. Aims of this study were to evaluate and correlate MRP4 expression and platelet function in children and adolescents aged <20 years at diagnosis with different subtypes of PT. Methods. MRP4 protein and mRNA expressions were evaluated in platelets obtained from healthy volunteers (HV) and from 41 PT patients: M/F ratio: 0.78; median age at diagnosis: 14.5 years; median platelet count at study: 671x109/L. Ten patients had MPLS505A mutation (HT), 10 were JAK2V617F-mutated (ET) and 6 harbored CALR mutations (ET) while 15 cases were JAK2, CALR and MPL wilde-type (ET). Expression of MRP4 protein and mRNA were analyzed by Western blot and RT-PCR, respectively, and the results were reported as ET/HV or HT/HV expression values. Platelet aggregation, using ADP at different concentration (0.8 to 2 μM) and collagen (1 μg/ml) as agonists, and platelet secretion, expressed as ATP release after U46619 + epinephrine (1 + 20 μM), using the luciferin-luciferase assay, were utilized as platelet function tests. This study was conducted in accordance with the Declaration of Helsinki. Results. Protein MRP4 expression was higher both in HT (4.23+/-1.88) and in ET patients with CALR mutations (4.27+/-2.60) compared to the values found in wild-type ET patients (3.55+/-1.52). In JAK2V617F-mutated ET patients, the MRP4 protein expression (2.03+/-1.46) was significantly lower compared to the values observed in all other ET and HT patients (p .05), whereas the MRP4 mRNA expression was significantly higher (ΔΔCt 0.021+0.003) compared both to HT patients (0.009+/-0.0039 ΔΔCt) (p .01) and CALR-mutated patients (ΔΔCt 0.014+/- 0.006). Patients with HT showed a significantly higher response to ADP 0.8 µM (83+/-29 Mx%) compared to all subgroups of ET patients who showed a similar response (60+/-34 Mx%; p .01). A significantly shorter lag-phase in response to collagen (1 µg/ml) was observed in HT compared to ET patients (33+/-3 sec vs 51+/-4 sec, p .005). Among the ET population, JAK2-mutated patients showed a significantly shorter lag-phase in response to collagen compared to wild-type patients (39+/-12.5 sec vs 50+/-15 sec), p .011. Platelet secretion was significantly higher in HT compared to ET patients, p .001. Conclusions. This study for the first time provides evidence that children and adolescents with MPLS505A-mutated HT show a higher platelet reactivity compared to age-matched patients with ET. Moreover, platelet reactivity correlates with MRP4 protein overexpression. These findings help to shed light into the thrombotic events observed in HT MPLS505A patients despite treatment with ASA. Abstract Introduction:Histologic transformation (HT) has been well characterized in follicular lymphoma (FL), and has historically carried a poor prognosis with median overall survival (OS) of 1.2-1.8 years (yrs) in the pre-rituximab era (Montoto et al, 2007; Al-Tourah et al 2008). Despite improved outcomes in the rituximab era (Link et al, 2013), HT still represents a major source of mortality and morbidity in these patients (pts). While there is emerging evidence that autologous stem cell transplant (ASCT) confers a survival advantage compared to chemotherapy with rituximab alone (Ban-Hoefen et al, 2013; Villa et al, 2013), many of these patients have contraindications such as advanced age and frailty precluding ASCT as an option. We report a strategy of radioimmunotherapy (RIT) consolidation after chemoimmunotherapy to address this unmet need. Methods: Consecutive pts ≥ 18 yrs old, who received RIT as consolidation after chemoimmunotherapy for HT at the Wilmot Cancer Institute were identified who had been deemed ineligible for ASCT. Inclusion criteria were as follows: 1) clinical, composite or pathologic diagnosis of HT, and 2) receipt of RIT after response to rituximab and chemotherapy. HT was defined as biopsy confirmed diffuse large B cell or Burkitt-like lymphoma by the WHO classification ≥6 months following a biopsy establishing indolent lymphoma diagnosis (pathologic HT), or clinical evidence of transformation as defined in previous cohorts (Al-Tourah et al, 2008; Link et al, 2013) in the absence of biopsy confirmation (clinical HT). Composite diagnosis of HT was defined as concurrent indolent and large cell lymphoma. The primary endpoint was overall survival (OS). Progression free survival (PFS) was a secondary outcome. Kaplan-Meier survival estimation curves were generated using SAS (SAS Institute Inc., Cary, NC, USA), and p values were generated using the log rank test. Results: 21 consecutive pts who had a clinical, composite, or pathologic HT diagnosis were included in this analysis. All of these pts responded to rituximab and chemotherapy (CHOP like n=18, other n=3), and were treated with RIT for consolidation. The median age at the time of HT was 66 yrs (44-90). Nine pts received I-131 tositumomab and 12 received Y-90 ibritumomab tiuxetan. The median OS from HT was 6.9 yrs in the entire cohort with a median follow up of 3.5 yrs (Figure). There were no statistical differences in outcomes between the pathologic (n=11) and clinical/composite (n=10) HT pts (p=0.33). There was also no difference between those who received rituximab prior to HT (n=13), and those who did not (n=8; p=0.88). Twelve pts experienced grade 3-4 thrombocytopenia, which was the most common adverse event (AE). Nine deaths occurred. Disease-related death was the most common cause (n=4), followed by death unrelated to disease or treatment (n=3) and treatment-related death (n=2). This represents one of the largest series focused on pts with HT who were deemed unfit for ASCT. The RIT was well tolerated in this analysis with only 2 treatment related deaths, one of which was secondary to therapy related acute myeloid leukemia. Our observed median OS compares favorably to published reports of pts with HT who are younger and able to tolerate more aggressive therapeutic options. Consolidative RIT should be considered as a treatment approach in this vulnerable population of pts, and can serve as a platform for the incorporation of novel agents into prospective clinical trials. Our laboratory has previously shown an attenuation of hypoxic pulmonary hypertension by exercise training (ET) (Henderson KK, Clancy RL, and Gonzalez NC. J Appl Physiol 90: 2057–2062, 2001), although the mechanism was not determined. The present study examined the effect of ET on the pulmonary arterial pressure (Pap) response of rats to short- and long-term hypoxia. After 3 wk of treadmill training, male rats were divided into two groups: one (HT) was placed in hypobaric hypoxia (380 Torr); the second remained in normoxia (NT). Both groups continued to train in normoxia for 10 days, after which they were studied at rest and during hypoxic and normoxic exercise. Sedentary normoxic (NS) and hypoxic (HS) littermates were exposed to the same environments as their trained counterparts. Resting and exercise hypoxic arterial Po2 were higher in NT and HT than in NS and HS, respectively, although alveolar ventilation of trained rats was not higher. Lower alveolar-arterial Po2 difference and higher effective lung diffusing capacity for O2 in NT vs. NS and in HT vs. HS suggest ET improved efficacy of gas exchange. Pap and Pap/cardiac output were lower in NT than NS in hypoxia, indicating that ET attenuates the initial vasoconstriction of hypoxia. However, ET had no effect on chronic hypoxic pulmonary hypertension: Pap and Pap/cardiac output in hypoxia were similar in HS vs HT. However, right ventricular weight was lower in HT than in HS, although Pap was not different. Because ET attenuates the initial pulmonary vasoconstriction of hypoxia, development of pulmonary hypertension may be delayed in HT rats, and the time during which right ventricular afterload is elevated may be shorter in this group. ET effects may improve the response to acute hypoxia by increasing efficacy of gas exchange and lowering right ventricular work.

Elucidating the factors that control charge transfer rates in relatively flexible conjugates is of importance for understanding energy flows in biology as well as assisting the design and construction of electronic devices. Here, we report ultrafast electron transfer (ET) and hole transfer (HT) between a corrole (Cor) donor linked to a perylene-diimide (PDI) acceptor by a tetrameric alanine (Ala)4. Selective photoexcitation of the donor and acceptor triggers subpicosecond and picosecond ET and HT. Replacement of the (Ala)4 linker with either a single alanine or phenylalanine does not substantially affect the ET and HT kinetics. We infer that electronic coupling in these reactions is not mediated by tetrapeptide backbone nor by direct donor–acceptor interactions. Employing a combination of NMR, circular dichroism, and computational studies, we show that intramolecular hydrogen bonding brings the donor and the acceptor into proximity in a “scorpion-shaped” molecular architecture, thereby accounting for the unusually high ET and HT rates. Photoinduced charge transfer relies on a (Cor)NH…O=C–NH…O=C(PDI) electronic-coupling pathway involving two pivotal hydrogen bonds and a central amide group as a mediator. Our work provides guidelines for construction of effective donor–acceptor assemblies linked by long flexible bridges as well as insights into structural motifs for mediating ET and HT in proteins.

<p><em>The objective of this study is to contribute with in-depth knowledge based on persons’ subjective experiences within mental health crisis and support and help from a Crisis Resolution/Home Treatment (CR/HT) team. The study has a qualitative, exploratory design and qualitative interviews were conducted with seven persons. They have experiences with both inpatient treatment in hospitals and support from a CR/HT teams. The informants revealed a variety of experiences as service users in the different helping contexts. The experiences of the CR/HT team’s accessibility, availability and flexibility, was highlighted as important. The Service users felt they were taken more seriously and met as a fellow human being in the home setting as opposed to hospital ward. The informants also emphasized how the CR/HT team helped them to feel more safe and secure. This study offers some in-depth insights of being on the receiving end of mental health services. It is important to include experience based user knowledge in the evidence base of practice development.</em></p>

Flickering is a well-known, though poorly understood, signature of accretion in cataclysmic variables. HT Cas is an excellent system in which to study flickering because its well determined characteristics allow a calibration of the experimental system (see Horne et al. 1991).Patterson (1981) argues that the flickering in HT Cas originates from regions very near the white dwarf. In U Gem, the flickering appears to be associated with the bright spot (Warner & Nather 1971). In RW Tri, the flickering occurs throughout the disk but is centrally concentrated (Horne & Stiening 1985); the same holds true for V2051 Oph (Warner & Cropper 1983).HT Cas is a 16m.4 eclipsing dwarf nova with a binary period of 106 min and an inclination of ∼ 81°. The entire eclipse last about 15 min with the white dwarf totally eclipsed for ∼ 5 min. The mass ratio is 0.15 with the white dwarf having mass ∼ 0.6M⊙ (Horne et al. 1991).We obtained 20 eclipses of HT Cas between 1982 and 1991 in U, B and V at Mt. Palomar and also McDonald Observatory with 0.5 – 2 sec integrations. These data were folded on the ephemeris given by Horne et al. (1991) and corrected for the 30 sec drift in the phasing as noted by Wood et al. (1995).

The purpose of this study was to test whether endothelium-dependent relaxation is decreased during acute vasospasm following subarachnoid hemorrhage (SAH) and the mechanism underlying the decrease. Basilar artery in situ was 35% constricted 3 days following injection of autologous arterial blood into the rabbit cisterna magna compared with vessels from control rabbits. In situ suffusion with the endothelium-dependent relaxant, acetylcholine (ACh; 10 microM), relaxed resting and serotonin (5-HT)-contracted control vessels but not vasospastic and 5-HT-contracted vasospastic vessels. In contrast, the relaxant potency and efficacy of ACh was similar in control and vasospastic vessels contracted with 5-HT in vitro. In situ suffusion with the ETA-receptor antagonist, BQ-123 (1 microM), reversed the vasospasm by 51% and restored the magnitude of ACh relaxation of vasospastic and 5-HT-contracted vasospastic vessels to that of controls. ACh in situ and in vitro relaxed endothelin-1 (ET-1)-contracted control vessels to a smaller magnitude than 5-HT-contracted control vessels. These results suggest, in contrast to previous studies, that endothelium-dependent relaxation is decreased during acute vasospasm following SAH. The decreased endothelium-dependent relaxation is secondary to the underlying ET-1-mediated spasm. The inhibition of endothelium-dependent relaxation observed in situ following SAH cannot be demonstrated in vitro, presumably due to loss of the ET-1-mediated vasospasm.

Det følgende er en skitse af mit arbejde med den selvbiografiske fortælling Hieroi Logoi – Hellige Taler (herefter HT) – af den græske taler og intellektuelle Aelius Aristides (117-181 e.v.t.). Teksten tematiserer, hvordan religion i det 2. århundrede ikke længere alene var et spørgsmål om kultisk praksis, men at religion i form af et personligt gudsforhold blev knyttet sammen med selvfortælling og identitet.Aristides er kendt som antikkens største sygdomstilfælde. Som 27-årig blev han første gang så alvorligt syg, at han for en årrække blev uarbejdsdygtig. HT er en beskrivelse af, hvordan han gennem et halvt liv led af utallige sygdomme, og hvordan de i perioder på det nærmeste invaliderede ham. Dermed blev hans selvopfattelse til et problem. Nok så vigtigt er det dog, at HT også er en ‘frelseshistorie’, som beskriver, hvordan lægeguden Asklepios igen og igen (men aldrig én gang for alle!) har helbredt Aristides, reddet hans liv og hjulpet ham med hans talervirksomhed, så at han blev en endnu bedre og endnu mere populær taler, end han var før sygdommene satte ind. HT er dermed en beretning om, hvordan Aristides på én gang oplevede, at hans helbred svigtede og hans ‘ånd’ blev større.

The array of papers received for review this year covers a wide spectrum, both in applications and research, which indicates that the exploration and production (E&P) industry is actively working to monetize high-pressure/high-temperature (HP/HT) reservoirs. The pandemic has already put an indelible mark on the industry during the past 2 years. The present geopolitical scenario is posing a serious challenge to countries’ energy security. The oil and gas sector is confronted with the biggest task in society—addressing climate change. The E&P sector is going to face stiff challenges in the coming days, with the word “petroleum” being substituted by the word “energy” to cover wider possibilities. Eminent actions are distinctly visible in the strategic planning of fossil fuel companies and the capital-expenditure investments in the E&P sector. Companies are shifting priorities toward diversification, resilience, and decarbonization. Safety continues to dominate HP/HT operations and will do so in the future with more-stringent measures and compliances. Cost-effective technology solutions blanketing the entire domain of HP/HT activities will provide exploitation on a sustainable basis with collaboration between industry and academia. While reviewing the papers, every attempt was made to provide a fair representation that cuts across the HP/HT domain to show the reader what is happening in the industry, both in the field as well as in research, over different geographical locations. Increased medial arterial thickness is a structural change in pulmonary arterial hypertension (PAH). The role of smooth muscle hypertrophy in this process has not been well studied. Bone morphogenetic proteins (BMPs), transforming growth factor (TGF)-β1, serotonin (or 5-hydroxytryptamine; 5-HT), and endothelin (ET)-1 have been implicated in PAH pathogenesis. We examined the effect of these mediators on human pulmonary artery smooth muscle cell size, contractile protein expression, and contractile function, as well on the roles of glycogen synthase kinase (GSK)-3β and p70 ribosomal S6 kinase (p70S6K), two proteins involved in translational control, in this process. Unlike epidermal growth factor, BMP-4, TGF-β1, 5-HT, and ET-1 each increased smooth muscle cell size, contractile protein expression, fractional cell shortening, and GSK-3β phosphorylation. GSK-3β inhibition by lithium or SB-216763 increased cell size, protein synthesis, and contractile protein expression. Expression of a non-phosphorylatable GSK-3β mutant blocked BMP-4-, TGF-β1-, 5-HT-, and ET-1-induced cell size enlargement, suggesting that GSK-3β phosphorylation is required and sufficient for cellular hypertrophy. However, BMP-4, TGF-β1, 5-HT, and ET-1 stimulation was accompanied by an increase in serum response factor transcriptional activation but not eIF2 phosphorylation, suggesting that GSK-3β-mediated hypertrophy occurs via transcriptional, not translational, control. Finally, BMP-4, TGF-β1, 5-HT, and ET-1 treatment induced phosphorylation of p70S6K and ribosomal protein S6, and siRNAs against p70S6K and S6 blocked the hypertrophic response. We conclude that mediators implicated in the pathogenesis of PAH induce pulmonary arterial smooth muscle hypertrophy. Identification of the signaling pathways regulating vascular smooth muscle hypertrophy may define new therapeutic targets for PAH.

Abstract Background: The paucity of prospective studies in strictly selected ET patients with long enough follow up has limited the development of prognostic models for survival, and hematological transformation (HT) to acute myeloid leukemia (AML), myelodysplastic syndrome (MDS) and myelofibrosis (MF). Indeed, ET is generally considered not to alter significantly life expectancy, and HT has been found in less than 5% of patients in prospective studies with less than 10 years of median follow-up. Methods: 108 newly diagnosed consecutive ET patients were included in a prospective multicenter cohort study between 1979 and 1990, using hydroxyurea only as first-line cytoreductive treatment in high risk patients (66%). Decision of starting anti-aggregating agents was let free to the decision of the physician. Baseline characteristics included bone marrow biopsy and red cell mass measurement in all cases, and cytogenetics in 97 (90%) patients (abnormal in 13%), clearly excluding another type of myeloproliferative disorder. Data for individual patient, including documented HT and deaths, were collected at regular interval (at least once a year) and as far as December 2007. Results: Median follow-up was 22.3 years. Median age at inclusion was 62 yrs, and 58% patients were females. 56 (52%) patients had died, and median survival was 13.3 yrs (CI95%: 11.1 – 21.4). Nine baseline variables were significantly associated with poorer survival in univariate analysis: age above 60 years, male gender, history of arterial thrombosis, higher leukocyte and neutrophil (ANC) counts, higher platelet count, decreased hemoglobin and red cell mass, and abnormal karyotype. By multivariate analysis, only 3 remained significantly associated with shorter survival: age above 60 (HR: 2.8; 95%CI: 1.5–5; p=0.001), male gender (HR: 2.03; 95%CI: 1.2–3.5; p=0.01), and ANC higher than 6.8x109/l (HR: 2.03; 95%CI: 1.2–3.4; p=0.01) (Figure 1A). A total of 16 HT were observed, including 10 AML/MDS (including 1 preceded in 1 by a polycythemic phase, and 3 associated with documented MF at time of transformation), and 6 MF (1 preceded by a polycythemic phase). Cumulative incidence of HT was 11.7%, 14.9%, 20.5% at 10, 15, and 20 years, respectively (Figure 1B). Analysis of prognostic variables for the risk of HT only found higher leukocyte count (p=0.018), and ANC (p=0.019) as significant baseline adverse risk factors. Conclusion: In this prospective cohort study of 108 newly diagnosed ET with 22.3 years of median follow-up, 16 patients evolved to AML/MDS or MF, a proportion clearly higher than reported in studies with less than 10 years of follow-up. Leukocytosis at diagnosis has been recently recognized as an important risk factor for thrombosis in ET and PV, and we have previously shown that leukocytosis at diagnosis was a significant risk factor for hematological transformation in PV (Kiladjian, Hematol J, 2003;4:198). The present study suggests that leukocytosis and ANC at diagnosis are major risk factors for shorter survival, and leukemic evolution in ET. BackgroundHashimoto’s thyroiditis (HT) is an organ-specific autoimmune disease that is not rare in RA. Hypothyroidism caused by HT can affect the prognosis and quality of life in patients with RA [1]. Many studies showed that immune cells play a critical role in HT [2-3]. However, the alterations of lymphocyte subsets in RA patients with HT are unclear.ObjectivesThis study aimed to assess the absolute numbers and proportions of peripheral lymphocyte subsets in RA with HT and investigate the clinical significance.MethodsA total of 1636 RA patients and 54 gender- and age-matched healthy controls (HCs) were enrolled in this study. Patients were divided into RA-non-HT and RA-HT groups according to the thyroid peroxidase antibody (TPOAb) and thyroid globulin antibody (TgAb). Peripheral lymphocyte subsets of all participants were assessed by flow cytometry. All clinical and laboratory data were analyzed by SPSS 23.0.ResultsAmong 30 (1.83%) RA-HT patients, there were more female (P=0.005), suggesting female was more susceptible to developing HT (Figure 1A). In addition, there were higher levels of erythrocyte sedimentation rate (ESR) (P=0.045) and C-reactive protein (CRP) (P=0.042) than those in RA-non-HT (Figure 1B-C). Compared with those of HCs and RA-non-HT, the level of circulating Th17 in RA-HT patients was significantly increased (P<0.05), while those of Tregs decreased (P<0.05), leading to a higher ratio of Th17/Treg (P<0.01). Notably, the level of B cells and Th1 cells, as well as the Th1/Th2 ratio, increased in the RA-HT compared with those of the RA-non-HT (P<0.05) (Figure 1D-F).ConclusionThe occurrence of RA-HT is closely related to female patients with high disease activity. ABSTRACT Hypertension and Diabetes mellitus are the two main causes of chronic kidney disease that culminate in the final stage of kidney disease. Since these two risk factors are common and can overlap, new approaches to prevent or treat them are needed. Macitentan (MAC) is a new non-selective antagonist of the endothelin-1 (ET-1) receptor. This study aimed to evaluate the effect of chronic blockade of ET-1 receptor with MAC on the alteration of renal function observed in hypertensive and hyperglycemic animals. Genetically hypertensive rats were divided into control hypertensive (HT-CTL) group, hypertensive and hyperglycemic (HT+DIAB) group, and hypertensive and hyperglycemic group that received 25 mg/kg macitentan (HT-DIAB+MAC25) via gavage for 60 days. Kidney function and parameters associated with oxidative and nitrosative stress were evaluated. Immunohistochemistry for neutrophil gelatinase-associated lipocalin (NGAL), ET-1, and catalase in the renal cortex was performed. The HT+DIAB group showed a decrease in kidney function and an increase in NGAL expression in the renal cortex, as well as an increase in oxidative stress. MAC treatment was associated with attenuated ET-1 and NGAL production and increases in antioxidant defense (catalase expression) and nitric oxide production. In addition, MAC prevented an increase in oxidant injury (as measured by urinary hydroperoxide and lipid peroxidation), thus improving renal function. Our results suggest that the antioxidant effect of the ET-1 receptor antagonist MAC is involved in the improvement of kidney function observed in hypertensive and hyperglycemic rats.

To determine the influences of both [Formula: see text] and the presence of the endothelium on contractile responses of the human umbilical artery (HUA), the effects of a series of vasoconstrictors were compared in ring preparations with and without endothelium at low (2.5% O2, [Formula: see text] (1 mmHg = 133.3 Pa)) and high [Formula: see text] (95% O2, [Formula: see text]). The results demonstrate the following. (i) 5-Hydroxytryptamine (5-HT) and endothelin-1 (ET-1) contracted the HUA at either low or high [Formula: see text]. At low [Formula: see text], removal of the endothelium significantly reduced receptor-mediated responses, (ii) The nitric oxide synthase inhibitor Nω–nitro-L-arginine methyl ester (L-NAME, 100 μM) did not modulate 5-HT-initiated contractions at either level of [Formula: see text]. (iii) α-Methyl-5-hydroxytryptamine (α-Me-5-HT) and 5-carboxamidotryptamine (5-CT), relatively selective 5-HT1C/5-HT2 and 5-HT1-like receptor agonists, respectively, elicited contractions in the HUA, and the responses were reduced at low [Formula: see text] but unaffected by removal of the endothelium, (iv) Responses of the HUA to high potassium (hK+) were unaffected by either changes in [Formula: see text] or removal of the endothelium, (v) The 5-HT2 receptor antagonist ketanserin at low concentration (10 nM) inhibited contractile responses to 5-HT in an apparently competitive manner. However, with 100 nM ketanserin and at low [Formula: see text], inhibition became noncompetitive. Removal of the endothelium did not influence the action of ketanserin. (vi) Regardless of [Formula: see text], the Ca2+ channel antagonist nifedipine (1 μM) significantly inhibited 5-HT- and ET-1-mediated contractions. Depletion of extracellular Ca2+ also greatly reduced 5-HT-induced contractions. We conclude that, in the HUA, endothelial cells and changes in [Formula: see text] differentially modulate contractions initiated by 5-HT agonists and ET-1, possibly via the release of an endothelium-derived contracting factor (EDCF).Key words: endothelium, contracting factors, vascular smooth muscle, oxygen tension.

RésuméDivers sous-types de récepteurs sérotoninergiques de type 5-HT1 ont été mis en évidence. Ils correspondent aux soustypes 5-HT1A (marqué sélectivement par le 8-OH-DPAT), 5-HT1B marqué par le propranolol et certains bêta-antagonistes et présent seulement chez le rat et la souris, absent chez l’homme) et 5-HT1C (marqué par la mésulergine et présent en particulier dans les plexus choroïdes). Un sous-type supplémentaire 5-HT1 non A, non B, non C a été mis en évidence; il est caractérisé par un site de liaison insensible aux concentrations de ligands inhibant sélectivement les sous-types 5-HT1A, 5-HT1B et 5-HT1C, il correspond à un système de transduction adénylcyclasique (activation ou inhibition) et apparaît enfin jouer un rôle important dans la modulation présynaptique de la libération de neurotransmetteurs - non sérotoninergiques - en particulier l’acétylcholine. Ces caractéristiques ont été mises en évidence sur des préparations membranaires de cortex de mammifères par des essais in vitro de liaison de [3H]5-HT, de mesure d’activité adénylcyclasique sur les mêmes préparations et par mesure de libération évoquée de [3H] acétylcholine à partir de préparations synaptosomales de diverses régions cérébrales de rat et de cobaye. Une analyse par autoradiographie quantitative a été réalisée chez l’homme sur des coupes fines de cortex frontal prélevées post-mortem chez des individus normaux et des individus déprimés; les résultats obtenus suggèrent une légère augmentation des sites 5-HT1 non A, non b, non c chez les individus déprimés versus les cerveaux normaux. Les antidépresseurs à faibles concentrations (10 à 100 nM) sont capables d’interagir in vitro avec le fonctionnement de ce récepteur: au niveau du site de reconnaissance en altérant les caractéristiques de liaison de la [3H]5-HT, à celui du système de transduction adénylcyclasique en inhibant l’activation induite par la 5-HT et enfin au niveau de l’effet cellulaire sérotoninergique en réversant partiellement l’effet inhibiteur du trilîuorophénylméthylpipérazine (TFMPP), un agoniste sérotoninergique, sur la libération évoquée d’acétylcholine. Cet effet est observé avec des antidépresseurs inhibiteurs d’uptake de la 5-HT mais aussi avec des inhibiteurs d’uptake de la noradrénaline et non pas avec des benzodiazépines ou des neuroleptiques. Ces résultats suggèrent I’hypothèse selon laquelle le système sérotoninergique fonctionnant à l’aide des récepteurs 5-HT1 non A, non B, non c pourrait réguler la libération de divers autres neurotransmetteurs entraînant par là une régulation du nombre des récepteurs correspondants à ceux-ci. Les antidépresseurs pourraient interagir avec cet effet modulateur sérotoninergique vraisemblablement altéré au cours de pathologies dépressives ou accompagnant celle-ci.

ObjectivesWith the present study, we aimed to investigate the association between menopausal hormone therapy (HT) and risk of colorectal cancer (CRC).SettingCohort study based on the linkage of Norwegian population-based registries.ParticipantsWe selected 466822 Norwegian women, aged 55–79, alive and residing in Norway as of 1 January 2004, and we followed them from 2004 to 2008. Each woman contributed person-years at risk as non-user, current user and/or past HT user.Outcome measuresThe outcome of interest was adenocarcinoma of the colorectal tract, overall, by anatomic site and stage at diagnosis. Incidence rate ratios (RRs) with 95% CIs were estimated by Poisson regression and were used to evaluate the association between HT and CRC incidence.ResultsDuring the median follow-up of 4.8 years, 138 655 (30%) women received HT and 3799 (0.8%) incident CRCs occurred. Current, but not past, use of HT was associated with a lower risk of CRC (RR 0.88; 95% CI 0.80 to 0.98). RRs for localised, regionally advanced and metastatic CRC were 1.13 (95% CI 0.91 to 1.41), 0.81 (95% CI 0.70 to 0.94) and 0.79 (95% CI 0.62 to 1.00), respectively. RRs for current use of oestrogen therapy (ET) were 0.91 (95% CI 0.80 to 1.04) while RR for current use of combined oestrogen–progestin therapy (EPT) was 0.85 (95% CI 0.70 to 1.03), as compared with no use of HT. The same figures for ET and EPT in oral formulations were 0.83 (95% CI 0.68 to 1.03) and 0.86 (95% CI 0.71 to 1.05), respectively.ConclusionsIn our nationwide cohort study, HT use lowered the risk of CRC, specifically the most advanced CRC.
