Information regarding monitoring of operational expenditures of the Regional Public Service Agency at the Emergency Ambulance Service Unit of the DKI Jakarta Health Office. Because the finances issued or used must be accurately informed. In this case, monitoring the operational expenses of the Regional Public Service Agency at the Emergency Ambulance Service Unit of the DKI Jakarta Health Service still has many obstacles. Constraints faced are such as the technical executor. The activity of submitting an accountability letter still has a long stage, the Verifier is not there so that the process of checking the accountability letter stops, the input of reports is carried out one by one by the Spending Treasurer using Microsoft Excel. The process of monitoring the operational expenses of the Regional Public Service Agency at the Emergency Ambulance Service Unit of the DKI Jakarta Health Office takes a long time. And another obstacle is the occurrence of classifying the type of operational expenditure. In this data analysis method, using Balanced Scorecard (BSC) analysis, system design uses Visual Paradigm to create UML (Unified Modeling Language), PHP programming language, MySQL to create databases. The result of this research is that an information system for monitoring operational expenditures of the Regional Public Service Agency can create and submit a letter of accountability and verification letters are carried out computerized, the Expenditure Treasurer no longer enters the general cash books one by one, the Expenditure Treasurer no longer classifies the types of operational expenditure in recap operational expenditure.

Students in a high school biology class were introduced to the case of “Marcus” (a pseudonym), a high school football player who collapsed on the football field during a game and was rushed to the emergency room with various symptoms. Throughout the two-week, project-based unit, students worked in cooperative groups to diagnose Marcus, learning about various inherited diseases and heat-related ailments that might impact young athletes. This unit integrates ultrasound technology into the classroom as a teaching and diagnostic technique and introduces students to health science careers. Student groups work to produce a final product that is presented to a public audience (e.g., parents, teachers, coaches) to increase their awareness of the science content underlying the causes of sudden collapses in young athletes. This learning experience ended with students individually writing a letter to Marcus's family explaining his diagnosis and the related biology concepts.

ABSTRACTObjective: to relate the experience of the authors in the creation and implementation process of a theoretical-practical activity related to the management of medications. Methodology: this is a descriptive study, an experience report, developed during the supervised internship for Nursing in Emergencies from the School of Nursing at a University in the city of Salvador, Bahia, Brazil. The proposed model was divided in three phases: delivery of the medications list to be examined; daily practical activities during care to patients; and group discussion about prescription deadlines for a clinical case study. Results: the methodology developed for the teaching-learning process, allowed for the construction of knowledge based on reflection practices and on conscious and ethical attitudes in regards to the administration of medications by future nurses. Conclusion: this experience allowed for a closer look at the reality of the day-to-day of the nurse in reference to deadlines, management, and supervision of medications, as well as legal and ethical implications. Emergency Curriculum is an education unit level curriculum that is prepared and implemented by education units during an emergency by taking into account the applicable regulatory signs and the limited conditions of each educational unit during an emergency. The purpose of the study was to determine the level of implementation of the emergency curriculum at state madrasah, and to determine the process of implementing learning during the Covid-19 pandemic. This research is quantitative research. Data collection techniques using questionnaires. The objects of this research are teachers at the State Islamic School (MIN) Lima Puluh Kota. The results of the research on aspects of the implementation of the emergency curriculum, the application of learning principles, the implementation of learning activities, and the evaluation of learning showed a high category. This means that the emergency curriculum has been socialized and implemented in madrasah. as well as teachers who have implemented aspects of the curriculum in the learning process. The geographical conditions and situations where students live must be a concern in implementing distance learning using internet media, because in certain conditions the network is not capable of being used so that the learning process does not take place optimally. In the learning process, teachers still have to consider the conditions, needs and characteristics of each student (related to student conditions), accommodate different student conditions, support facilities, learning styles and learning needs. There are some students who are constrained by the network, learning tools, and the use of learning media.

Commander Zeita Merchant, PhD, is currently the Commanding Officer of the Coast Guard Marine Safety Unit Chicago and has served on active duty in the Coast Guard for more than 20 years. She was previously Special Assistant to the Vice Commandant of the Coast Guard, and has also held the positions of Executive Officer of Marine Safety Unit Texas City, Supervisor, Port of Miami Field Office, and Chief of Port Operations at US Coast Guard Sector Miami. From 2010 to 2012, Commander Merchant served as a Congressional Fellow in the US House of Representatives. She graduated with honors from Tougaloo College with a Bachelor of Science in Biology, and received her Master of Quality Systems Management from the National Graduate School in 2003, her Master of Public Administration from the Trachtenberg School in 2010, and her Doctorate in Business Administration from the National Graduate School in 2011. Commander Merchant has been honored with many professional, academic, and community service awards throughout her career, including no less than eleven medals for her service in the Coast Guard, and is a recognized authority in the field of Marine Safety, Emergency, and Environmental Management.

The study is oriented to reading - writing for the development of the linguistic skills of the elementary school students of the Bartolomé de las Casas Educational Unit of the Jama canton in 2022, due to the virtual education that has been carried out for two years. years due to the global health emergency, which has generated great changes in education, with reading and writing being affected, with elementary school students being the most affected. The descriptive methodology of field type and bibliography was applied, the scientific method was used, and the techniques used were interviews with teachers, surveys with parents and observation sheet for students with a population of 32 parents, 10 teachers and 32 students. The result was that the existing relationships between oral and written language could be described from the point of view of the components of the language of those involved.

Learning in emergency conditions cannot be carried out as usual, however, students still get education and learning services, namely through distance learning by carrying out learning activities from home. The purpose of this paper is to describe the governance of the adaptive curriculum model of the education unit in improving effective learning adapted to the conditions and needs of the school/madrasah. The method used is a qualitative method with a library research approach. The results of the adaptive curriculum development model are divided into three, namely: 1) General curriculum model (regular). 2) General curriculum model with modifications. 3) Individualized curriculum model Effective learning if teaching activities can achieve the objectives according to the initial planning. Learning is said to be effective when students can absorb the subject matter and efficiently. Emergency learning models include: Project Based Learning, online Method, Offline Method, Home Visit Method, Integrated Curriculum, Blended Learning.

Proper handling of asthmatics can maintain the patient's quality of life. Visits of asthmatic patients to the Emergency Department (ER) indicate uncontrolled asthma and a marker of risk for future asthma exacerbations. Uncontrolled asthma can increase health costs, the risk of hospitalization, and can also cause death. This research at to determine the relationship of acute asthma patients who visit the emergency department with costs of treatment. Analytical descriptive research with cross-sectional study design. The population of this study was acute asthma sufferers who went to the emergency department of RSIJ Pondok Kopi in the period August 1, 2016 – August 1, 2017, and all of the population was used as a sample of 35 people. Data collection is done by studying the documentation of medical record data. The results showed 32.4% of respondents aged 41-49 years, 62.9% were female and had a high school education level (SMA), 40% of respondents were housewives (IRT), 65.7% had acute asthma. moderate, and the most precipitating factor is respiratory tract infection. Most visits to the ER were 30 patients (85.7%) with 1 visit and not accompanied by complications. The results of the highest treatment costs ranged from Rp. 300,000 – Rp. 400,000 as many as 22 patients (62.9%) with guaranteed treatment used were non-BPJS. The costs incurred by each acute asthma patient with 1 visit to the ER are lower than 2 times or more than 2 visits. It was also found that the number of acute asthma patients with complications was greater in costs than those without complications.

The aim of this study was to compare the analytical thinking skills of 8th grade students who received emergency distance and face-to-face education in a science course and to get student ideas concerning emergency distance education. An unequal control group model from the quasi-experimental models was used in the study. The study group consisted of 39 students. 15 students who received energy transformations and environmental science unit as emergency distance education constituted the first group and 24 students who received face-to-face education constituted the second group. The data were collected with ATAT, TATDL, and SOF. In conclusion, it was determined that the cognitive success of the students who received face-to-face education based on analytical thinking improved more. The ability to adapt analytical thinking to daily life developed equally in both groups. While the students who received emergency distance education did not express a negative opinion about the activities carried out in the teaching process, they stated negative opinions about the realization of this process online, such as internet shortage, lack of communication tools, noise, occasional power cuts, inefficiency compared to face-to-face education, difficulty in understanding the lesson, difficulty in speaking in front of the screen, inability of the teacher to be effective in distance education, and inability to experiment together. However, there were also students who expressed a positive opinion that the course was productive, interesting, calm, more comfortable than face-to-face education and safer to protect from the disease. As a result, it is thought that emergency distance education cannot replace face-to-face education; however, it is an effective education process that can be applied to prevent students from being completely disconnected from school and education during periods when lessons cannot be continued due to extraordinary circumstances such as pandemic.

Introduction This observational study investigated the incidence of distal radius fractures in children, to determine whether the rate is rising, the effect of seasonal variation on incidence and whether fracture type and rate of surgical intervention has changed, to help in determining costs for secondary care and to aid resource allocation. Materials and methods All paediatric patients(n = 6529) who sustained a distal radius fracture over an eight-year period (2007–2014) were identified. Poisson regression modelling was used to identify change in trends. Results There was no change in distal radius fracture incidence, rate of surgical intervention (P = 0.36) or fracture type (P = 0.70). Overall incidence was 337 fractures per 100,000 patient/years. The highest fracture incidence was seen in older school boys (708 per 100,000 patient/years, P < 0.005). Overall fracture rate was lower in winter (P < 0.005). Incidence is highest in summer and the main variation is related to season. Discussion These data can help to predict accurately the number of children presenting to the emergency department with wrist fractures depending on the time of year.

General anaesthesia and sedation are known to be useful adjuncts in the care of paediatric dental patients. There are several challenges that prevent patients from receiving care. Aim: To assess the treatment outcomes of paediatric dental patients seen at an emergency facility who were referred for treatment under sedation or general anaesthesia at a regional hospital in Trinidad. Methods: Records of patients seen at the Child Dental Health Unit Emergency clinic at the University of The West Indies Dental School from 2012 to 2017 were assessed. The parents of children referred for general anaesthesia or sedation at the regional hospital were then interviewed via telephone. Results: Most children (53.4%) were younger than 6 years old and the most common reasons for referral were the treatment of multiple carious teeth and behaviour management. Furthermore, 66.1% of cases did not receive treatment and had a mean waiting time of 4.7 years, and 61.7% of referred cases needed emergency care while awaiting general anaesthesia or sedation. Limited access to these services and the high cost of treatment were the main reasons for non-treatment. Conclusion: There is significant need for the timely treatment of paediatric dental patients referred for general anaesthesia or sedation. Improved availability and accessibility of these services could improve patients’ quality of life.

Introduction: Congenital Heart Disease is the most common cause of major congenital anomalies accounting 28.0%, representing a major global health problem. Prevalence of Congenital Heart Disease is 1.3 per 1000 in school children of Nepal.Material and Methods: A cross sectional prospective study was carried out in the neonatal intermediate care unit of Kanti Children’s Hospital, Nepal from Jan 2016 until Dec 2016 to see the spectrum of CHD.Results: Out of admitted 831 neonates, 85 were found to have CHD with prevalence of 102.28 per 1000 admitted neonate. Respiratory distress was the commonest symptom (51.8%) followed by cyanosis (11.8%) and reluctant to feed (10.6%) at presentation. ASD was the commonest (87.1%) cases followed by PFO 23.5%, PDA 21.2%, Complex congenital heart disease 11.8% and TOF 1.2%. Cleft lip and Cleft Palate was found in 5.9%, Down’s syndrome 3.5% of cases, polydactyly and syndactyly was detected in 2.4% newborn with CHD. The mode of delivery was spontaneous in 71.8% followed by Elective LSCS were 14.1% and Emergency LSCS were 9.4%.Conclusions: Prevalence of CHD was the 102.28 per 1000 neonates admitted in NIMCU. Atrial Septal Defect was the commonest congenital heart diseases. Cleft lip and Down’s syndrome were the most extra cardiac anomaly associated with CHD. Respiratory distress was the commonest presentation of CHD for hospital admission.

At about 3 am on Friday 16 December, a landslide occurred along the Jalan Batang kali Genting highlands road. The medical team of Hospital Selayang had been activated and responded to the site. A campsite with visitors from a local school including families and children who were hit by a landslide was reported missing and trapped in the area. Of the 92 victims, and survivors were 61, the number of deaths was 31. The search and rescue team used equipment such as excavators as well as the help of the K9 unit to help locate the victims in the landslide. The media was present at the cold zone or green zone and had 24-hour coverage of the scene to spread the necessary and accurate information to the public. Regular briefings were done by the SAR teams together with the incident commander to the media to keep the public updated on the real information about the scene.

Abstract Introduction In January 2000 fire erupted at a local college. Panicked students, many who failed to attend school-sponsored fire drills or ignored the alarms that night, found themselves scrambling to escape. Seven students were admitted for severe burns to a near-by burn center (BC), with 54 initially staged in the Emergency Department. While this dormitory fire took place more than twenty years ago, its legacy is still being felt today. Methods History shows that, with any tragedy, there are lessons to learn that lessen the impact of property destruction, injury or death. Literature review of several major fires looked at their impact promoting burn care and fire safety to correlate those lessons with this fire. Four key areas of improvement emerged: disaster preparedness, media relations, legislation, and fire prevention. Results Changes to BC disaster preparedness included formation of a more comprehensive plan, revisions to triage and transfer protocols, new guidelines for unit staffing, creation of a mid-Atlantic group of BCs that eventually morphed into the Eastern Regional Burn Disaster Consortium, and installation of a medical command center for regional disaster response. Media relations saw a collaboration with law enforcement, due to criminal investigation, and a partnership with a syndicated newspaper to document the journey of two survivors. A series of articles eventually resulted in a Pulitzer Prize-finalist book, with award winning photographs displayed at a national museum. Redesigned fire safety programs targeted high school and college students emphasizing escape plans, and clinical education included disaster drills. Two survivors became motivational speakers, sharing their personal story on campuses across the United States. New legislation mandated sprinkler installation in dormitories nationwide, and a non-profit foundation was formed to improve burn care. The anniversary of this fire is still commemorated each year with a ceremony and wreath-laying on campus. Conclusions Despite this dormitory fire being ranked as the deadliest in state history, all these years later the legacy of this landmark event remains one of triumph and resilience as its lessons still to resonate today

IntroductionThe COVID-19 pandemic forced many countries to apply restrictive measures. During the first wave Portugal went through a lockdown, and all the child and adolescents had to stay home and could only contact with the one’s they lived with for several months.ObjectivesThis study aimed to evaluate the impact of those restrictions on suicidal ideation in the pediatric population evaluated in a child and adolescent psychiatry emergency care of a tertiary referral hospital.MethodsWe conducted an exploratory retrospective study. All the data from discharge notes were collected between March 15th and June 15th of 2020 (n=59), and in the homologous period of the previous year (n=178). The referral after evaluation (primary care, child and adolescent psychiatry consultation, inpatient unit) was considered a measure of severity.ResultsThe demographic variables (sex, age) were homogeneous between the two groups (p ≥ 0,05). 17,4% (n=31) of the sample from 2019, and 16,9% (n=10) of the sample of 2020 had suicidal ideation, which was not statistically different between groups (p=1,000). The referral, after evaluation between groups were also not statistically different (p=0,186).ConclusionsEven though the proportion of patients with suicidal ideation was homogenous during the two periods, the total number of patients evaluated in the emergency room were lower during the first wave of Covid-19 pandemic. Mycoplasma pneumoniae (MP) is one of the main causes of both upper and lower respiratory infections in school-aged children, accounting for up to 40% of community-acquired pneumonia. Younger children are also affected, and extrapulmonary manifestations have been recently reported in the pediatric population. We carried out a retrospective analysis of MP-positive patients admitted to the Pediatric Emergency Unit of S. Orsola Malpighi University Hospital in Bologna, the largest tertiary pediatric referral center in the Emilia–Romagna region, Northern Italy, between 2012 and 2020. We identified 145 patients with MP infection (82 males and 63 females), 27% of which were younger than 2 years; the median age was 5 years (interquartile range 1–9). The clinical presentation partially differed between age groups. School-aged children were more likely to have a chest X-ray-confirmed pneumonia (p = 0.013), while younger children required oxygen therapy more often (p = 0.048). Seventy-four children (51%) showed extrapulmonary manifestations, mainly gastrointestinal (30%) and dermatological (14%). Neurological symptoms were more frequent in children older than 6 years (p = 0.006). The rate of other extrapulmonary manifestations did not differ significantly between age groups. This study shows that MP infection is a frequent cause of pediatric hospitalization, including of children younger than 2 years. Clinicians should be aware of the variable clinical expressions of MP, including extrapulmonary manifestations, to achieve a correct diagnosis and determine appropriate treatment.

At the present time, teaching is going through an experience at a global level, due to the Covid 19 pandemic. Students experienced a new way of learning, because of the health emergency; In this period, project-based learning was introduced as a contribution to virtual education, a methodology that was applied in the Sebastián Muñoz Educational Unit of the San Sebastián Parish of the Pichincha canton. The purpose of this work is to carry out an analysis of the contribution that this methodological strategy has had on high school students. The criterion that is intended to show is the marked acceptance that this significant learning has had, through a detailed analysis, starting from the management of the ordinary use platform Microsoft teams with which there is direct communication in the educational community. The qualitative method is proposed based on interpreting the data obtained from the objective set, using a direct survey of students and teachers. It was obtained as a result that the schoolchildren who make use of technological tools are precisely those who have efficient connectivity and that the educators who work the most applying the studied methodology are those who adequately handle the technological elements.

OBJECTIVE: To describe a case of a child with altered mental status following the topical administration of doxepin. CASE SUMMARY: A five-year-old Hispanic girl was brought to the emergency department because she was difficult to arouse at school. She had recently developed a generalized eczematous rash for which she was prescribed doxepin hydrochloride 5% cream. An entire tube (30 g) of doxepin cream was applied in the 24 hours prior to presentation. The patient was responsive only to noxious stimuli, with no focal neurologic abnormalities. She was decontaminated and observed in a pediatric intensive care unit. By 18 hours after presentation, she had fully recovered and was discharged. CONCLUSIONS: Topical doxepin, available as a 5% cream, is indicated for the treatment of pruritus secondary to eczematous dermatoses in adults. Diminished skin integrity and the application of a massive quantity of doxepin 5% cream to a large body surface area contributed to the toxicity in this child. Since the safety and efficacy of doxepin cream has not been established in children younger than 12 years, it should be used with caution in this population.

I hope you have all settled well into your new jobs and a very warm welcome to those that have recently joined the ‘family’ of acute medicine. I would first of all like to thank Ruth Johnson for all her hard work as trainee representative over the last 18 months and wish her all the best as she ventures into consultant territory: her replacement will be announced later in the autumn. July was a busy month, dominated by our Acute Medicine Awareness Week, during which AMUs across the UK undertook events to raise the profile of the speciality and the important work they were doing locally. Barnsley completed a 25 mile virtual marathon, Crosshouse Hospital made £350 in a cake sale, Salford Royal staff walked around every acute medical unit in Greater Manchester, North Staffordshire staff ran a half marathon and there was more cake on sale in Kings College and Leicester Royal. The AMU staff at Southampton raised over £400 with their cake sale and cycle challenge, during which they were joined by the Trust Chief Executive for a ‘virtual’ 120 miles on an exercise bike situated outside the hospital entrance. The highlight, however was the contribution of Dr Nigel Lane, an acute medicine trainee from Southmead Hospital in North Bristol, who put together an outstanding weekly programme of events. This included a visit from the Chief Executive of the trust, visit from local GPs to the unit, daily MDT teaching, daily ‘messages of the day’ located on the trust website and lots of screensavers, banners and information scattered throughout the hospital. I am delighted to announce that Nigel has received the SAM awareness week prize. This involves the opportunity to join the European School of Internal Medicine and attend the winter EFIM school camp in Latvia. Nigel will also be joining us as one of the speakers in the trainee session at SAM Brighton. He will be speaking on “Preparing for your PYA”. There will also be talks in the trainee session on “Keeping your e-portfolio updated”, “Choosing your specialist skill” and “Preparing for your consultant job”. The session will be aimed at both junior and senior trainees. The trainee that has produced the best poster at Brighton will also have a chance to win a place to attend the summer EFIM school camp. The day before the conference starts there will be a SCE revision session. I attended last year and found it extremely helpful! Looking forward to seeing you all in Brighton. Abstract Burns are a devastating public health problem that result in 10 million disability-adjusted life-years lost in low- and middle-income countries. Adequate first aid for burn injuries reduces morbidity and mortality. The rate of proper first aid practices in other countries is 12% to 22%.1,2 A 5-year retrospective audit was performed on the database of the Burn Unit in Hospital Universiti Sains Malaysia for 2012–2016; this involved 485 patients from the east coast of Malaysia. The mean age of the patients is 17.3 years old. The audit on first aid practices for burn injury showed poor practice. Out of 485 burned patients, 261 patients (53.8%) claimed that they practiced first aid. However, only 24 out of 485 patients (5%) practiced the correct first aid technique where they run their burn wound under cool water for more than 20 minutes. Two hundred and twenty-two patients had not received any first aid. Two patients did not respond to the question on the first aid usage after burn injury. The mean age of patients who practiced first aid was 15.6 years old. Out of the 261 patients who practiced first aid, 167 (64%) run their wound under tap water for different durations. Others practiced traditional remedies such as the application of “Minyak Gamat” (6.5%), soy sauce (5.5%), other ointments (3.6%), milk (1.8%), and eggs (0.7%), as well as honey, butter, and cooking oil (0.4% each). First aid practices for burn injuries in the population of east coast Malaysia are still inadequate. The knowledge and awareness of school children and the general Malaysian population must be enhanced.

The research aims to evaluate the implementation of curriculum implementation during the Covid-19 pandemic at Peninggilan 3 Public Elementary School through Distance Learning (PJJ) seen from the evaluation of context, input, process, and product, as well as Letters Ministerial Circular Number 4 of 2020 concerning the Implementation of Educational Policies in the Emergency Period for the Spread of Corona Virus Disease (Covid-19). The method used in this research is descriptive qualitative and CIPP evaluation (Context, Input, Process, and product). The implementation of the learning curriculum during the Covid-19 pandemic was developed referring to the national curriculum whose implementation was carried out online through google classroom, google meet, zoom meeting, and whatsapp group described in the Education Unit Level Curriculum (KTSP). Difficulties and obstacles in implementing the learning curriculum for teachers during the Covid-19 pandemic can be overcome by participating in training in the field of Information and Technology, such as: making video media, models, and learning methods that are appropriate to the Covid-19 pandemic, so that teachers can make their own learning videos according to the student's character and eliminate student boredom in the PJJ process. PJJ clearly has positive and negative impacts on students, therefore assistance and parental participation are needed, especially in mastering scientific disciplines, assertiveness in controlling and using children's gadgets at home, as well as good communication between teachers and parents.

Introduction: The social determinants of health (SDoH) can play a significant role in a person’s overall wellbeing. This is especially true for adults with mental illness and mental health disorders. In this study, we describe the SDoH of patients presenting to an academic, inner-city emergency department (ED) with an acute mental health complaint (AMHC). Methods: We prospectively identified and enrolled a convenience sample of patients presenting to an ED with an annual census of 85,000 visits. Participants provided informed written consent, and completed a questionnaire package containing questions related to demographics and SDoH. As well, participants were asked to complete four mental health, quality of life, and recovery validated patient-reported outcome measures. Results: A total 108 participants were enrolled in this study, of which 65% were male, aged 37.5 years (IQR 26.7-50.3), 56% Caucasian, and 22% Aboriginal. Depression was the primary diagnosis reported by 55% of participants, with 58.9% meeting the PhQ-9 cutoff for moderate-severe depression. The highest level of educational achievement for 44% of participants was high school or less, with 75% reporting being unemployed. Almost half (45%) reported engaging in less than two hours of structured activity each week. Thirty eight percent of participants reported living in their own apartment, with 25% reporting being homeless and 17% living in a single-room housing unit. The majority of participants (56%) sampled were not satisfied with their housing, and 67% were actively looking for new housing. Sixty percent of participants reported smoking cigarettes daily and 40% reported weekly cannabis use. A total of 11% of the sample reported that they did not have access to clean drinking water; 35% worried that their food would run out, and 47% reported cutting the size of meals due to a lack of money. Conclusion: This study lends evidence towards the circumstances in which patients presenting to the ED with an AMHC live and work. A considerable proportion of patients reported homelessness or being marginally housed, lack access to clean drinking water and sufficient food, and high rates of unemployment. Mitigating the effects of harmful social determinants is critical for optimal health of this population. Future work is needed to clarify the role of the ED in the surveillance, screening, and intervention of SDoH for this vulnerable patient group.

Background:The implementation of an FLS in the Spanish public health system is not an easy task since there are no official plans for the incorporation of personnel dedicated to the unitObjectives:To expose the consolidation and improvement of an FLS after its implementation as well as the problems that have arisen over time.Methods:The health program for secondary fracture prevention was implemented in 2012. Initially worked with the same staff assigned to the Rheumatology service, since 2016 we have a part-time support nurse. Patients are identified from the emergency registry and, more recently, from patients admitted for hip fracture and treated in a monographic osteoporosis clinic. The baseline visit consists of consultation with the nurse, DXA and bone metabolism analytics. Falling patients are referred to a fall prevention school. Most patients are referred to their primary care physician to start a treatment.Results:Of the 2,416 patients attended the baseline visit, 30% were forearm fractures, 27% hip, 20% humerus, 10% spine and 11% other fractures. In comparison to 2012, in 2019 the monthly average of patients has doubled, increased the number of hip and spine fractures, and increased the percentage of captured patients (Table). ABSTRACT Objective The aim of this study is to determine the complaints of patients and their relatives, to provide necessary measures and to find solutions. Material and Methods It was retrospectively analyzed and conducted as a single-center study. The complaints of patients and their relatives who applied to the Patient Rights Unit, Ministry of Health Communication Center, Prime Ministry Communication Center, Presidential Communication Center from the hospital medical records between 2018 and 2019 were examined. Data were analyzed on IBM SPSS Statistics 22.0 package program. Results 1385 applications were examined. Most of the complainants were men (66.1%). The most frequent complainants were between the ages of 20-40 (56.3%), high-school graduates (56.3%), in the self- employed/artisan group (24.2%). The most of the complaints were made application to the Patient Rights Unit (58.7%). Complaints were most common for physicians who in charge of primary care of the patient (49.5%) and least common for consultant physician (0.2%). The most common complaints were about poor attitude (26.9%) and long waiting time (21.7%). Most of the complaints were made in 2019 (n:835), July (n:239) and on Friday (n:326), and also most of the complaints were made against the Department of Pediatrics (n:190 (13.7%)) and Emergency Medicine (n:129 (9.3%). When the complaint-patient ratio was examined, it was perceived that the most complaints were from the laboratory and pathology departments. Conclusion It is concluded that patient rights units are very crucial and they have an important role in evaluating the quality of institutions and determining the existing deficiencies as well as increasing patient satisfaction.

Background: Patients with poorly controlled diabetes mellitus (DM) often visit the emergency department (ED) for management of hyperglycemic episodes, including diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS). It has been previously reported that risk factors for readmission to the intensive care unit (ICU) in DKA include older age, female sex and the presence of significant comorbidity including sepsis. However, there are no ED-based studies on this topic, particularly in a Canadian setting, and data on outcomes such as recurrent ED visits, hospital or ICU admission after discharge in these patients is lacking. Objectives: The primary objective of this study is to derive and internally validate a clinical risk tool for prognosis of patients presenting with hyperglycemic emergencies to identify those at higher risk of adverse outcomes within 30 days of initial ED presentation. Methods: This will be a multicentre prospective cohort study of eligible consecutive adult patients with an ED diagnosis of hyperglycemia, DKA or HHS. We will include all visits of adult (≥18 years) ED patients with either a known or unknown history of DM and a diagnosis of hyperglycemia (blood glucose &gt;11.0 mmol/L), DKA or HHS. We will include patients with co-morbid diagnoses in addition to hyperglycemia. We will exclude patients: a) with advanced care directives for resuscitation involving refusal of treatment, and b) who are initially assessed at a peripheral hospital and transferred to our sites for ongoing management. Research assistants will then contact the enrolled participants via telephone for follow-up regarding clinical outcomes, including repeat visits to see a health care provider, changes in diabetic medications, and time taken off of work or school. Participants will be followed to determine if they have further ED visits, admissions or ICU admissions after their ED visit for hyperglycemia. Data on missed patients or those who refused consent will be collected to assess for selection/enrolment bias. Statistical considerations: The primary outcome will be an unplanned return ED visit for hyperglycemia within 30 days of initial presentation. Secondary outcomes will include unplanned admission to hospital or ICU for hyperglycemia, or death within 30 days of the index ED visit. Additionally, we hope to characterize patient-important and health-care system outcomes such as time taken off work or school and follow-up visits to see a healthcare provider. We will conduct descriptive statistics on investigations, treatments, disposition and patient-important outcomes. We will perform an initial univariate logistic regression, followed by a multivariate analysis to identify predictor variables associated with adverse events such as recurrent ED visits, and admission to hospital or ICU for hyperglycemia within 30 days. We will include individual patients who have multiple recurrent visits to the ED during the study period and statistically weight for these using generalized estimating equations (GEE), which are used to develop regression models for correlated data that arise from repeated measures of the same individuals over time. Finally, a clinical risk tool will be derived by rounding the beta co-efficients. Internal validation will be conducted using bootstrapping techniques. Importance: ED visits for hyperglycemia significantly affect both the healthcare system overall and the individual patient. The results of this project will assist clinicians to better identify these patients and enable them to intervene either medically or educationally to prevent subsequent visits to the ED. As a result, patients will have improved care, better blood glucose control, and be identified for closer follow-up with a family physician or diabetes specialist. Furthermore, by aiming to reduce the number of recurrent visits, this project may reduce ED utilization and the associated healthcare costs with frequent visits and admissions for hyperglycemia.

Thromboelastography (TEG) is usually used to monitor coagulation disorder clinically. It is unclear whether TEG has association with urosepsis and sepsis-induced coagulopathy (SIC). The purpose of this study was to investigate the clinical significance of TEG parameters in urosepsis. 90 patients who were admitted to the Emergency Ward and Emergency Intensive Care Unit (EICU) of Ren Ji Hospital affiliated to Shanghai Jiao Tong University School of Medicine due to urinary infection from February 2014 to February 2022 were retrospectively studied. Urosepsis patients and non-sepsis patients were separately investigated according to the final discharge diagnosis and Sepsis 3.0. At the same time, patients with urosepsis were further divided into groups of SIC and non-SIC based on the definition of SIC. The data of clinical features, laboratory biomarkers, and TEG parameters were collected and analyzed. There were significant differences in white blood cell count, C-reactive protein (CRP), platelet count, procalcitonin (PCT), fibrinogen (FIB), international normalized ratio (INR), prothrombin time (PT), D-dimer, and incidence of urinary tract obstruction between the urosepsis group and non-sepsis group ( P < 0.05 ). In the comparison with non-sepsis group, K value was significantly lower (P = 0.006), while α-angle ( P = 0.003 ) and clot index (CI) ( P = 0.048 ) were significantly higher in urosepsis group. The area under the K value curve excluding urosepsis was 0.667. The areas under CI and α-angle curves for diagnosing urosepsis were 0.682 and 0.621, respectively. The patients in SIC group had significantly higher K value, lower α-angle, and maximum amplitude (MA) than those in non-SIC group ( P < 0.05 ). Coagulopathy is prone to occur in patients with urosepsis. TEG is helpful for assessment of hypercoagulable state in urosepsis and prediction of hypocoagulability in SIC patients implying the dynamic process of DIC.

Results There were 150 questionnaire distributed out of which 120 responded. Mean Age was 23.33 and most of the respondents were female with majority having education qualification equivalent to intermediate level. Only 49.20 said it to be a different specialty and 72.5% said anaesthesiologist work differently in the theatre where as 70% knew anaesthesiologist did something in the post-operative period too. Conclusion Anesthesiologists have duty to visit patient’s pre operatively and post operatively. The role inside the theatre and expanding role outside the theatre is poorly known. The awareness about the role of anesthesiologist in operation theatre, in intensive care unit, acute and chronic pain management and emergency care areas should be highlighted to all the staffs. The Covid-19 pandemic has changed many aspects of life, one of which is education. The transformation and acceleration of education is a challenge that must be faced by the government and educational institutions. This article aims to find out how education policies during the COVID-19 pandemic are in accordance with Circular No. 4 of 2020 concerning the Implementation of Education Policies in the Emergency Period for the Spread of Coronavirus Disease (covid-19) and various teaching media that can be used to support online teaching and learning activities. /on line. This study uses relevant literature review research methods and surveys of students and teachers in one school using a google form which is processed and interpreted as a result. The data taken in this study came from various sources and research results related to the investigated case, then the data was processed using qualitative descriptive analysis techniques, which is a way of processing data that is formulated in the form of words or sentences and data collection techniques using observation techniques. , interviews and documentation. Based on research, various teaching media are used such as youtube, educational platforms such as whatsapp, google classroom, google meet, zoom cloud meeting, the teacherscorner. net and others. The results of this study indicate that the use of teaching media and educational platforms may be an alternative solution in learning, for the sake of continuity of learning in every educational unit during the Covid-19 Pandemic.

The policy, the Minister of Education and Culture of the Republic of Indonesia immediately made a derivative policy in the field of education through the Circular of the Minister of Education and Culture No. 3 of 2020 concerning Prevention of COVID-19 in the Education unit, all education from elementary school to tertiary level in Indonesia to take decisive steps on the appeal of the government to carry out learning activities based on E-Learning. For the sake of implementing E-Learning, efforts such as application-based learning began to be applied. Background: The admission rate for bronchial asthma has increased dramatically all over the world. This increase in admission influences the children’s quality of life in addition to health care cost. Objective of the study was to identify the risk factors for readmission because of acute asthmatic attacks.Methods: Case control study applied on asthmatic children admitted because of acute asthma attacks attending emergency room, paediatric intensive care unit and inpatient wards of Alexandria university children's hospital, Alexandria, Egypt in the period from September 2016 to July 2017. The study group was subdivided into 2 groups; group (A) readmitted within one year from first admission and group (B) firstly admitted.Results: The mean age in group (A) and (B) was 9.09±3.98 and 8.65±4.01 respectively. Males were more than females in both groups, and no sex differentiation effects on readmission. The duration of the disease in readmitted group ranged from 1-8 years, it was a risk factor for acute asthma readmission. Viral infection, exercise and dust were risk factors for acute asthma exacerbation readmission. Disease severity was found to have a higher percentage of hospitalization; cases with severe and moderate bronchial asthma compared to mild cases. Readmitted patients had more sleep disturbance and lack of school attendance. Readmitted patients were less adherent to treatment.Conclusions: Duration of the disease, viral infections, common cold, dust and exercise are risk factors for acute asthma readmission. Severity of the disease and adherence to medications affect acute asthma readmission.

Burn injuries cover a broad category of injuries to the health and life of victims. They can cause severe general illness caused by local damage to the body affecting all organs and systems of the body. Globally, more than 300,000 people die from fires each year alone, and the death toll from thermal, electrical and chemical burns is much higher [23]. Burning is one of the most serious traumas in the infant body, leaving lasting consequences in the life of the injured child and one of the most common causes of hospitalization of children [10]. According to statistics from the Red Cross Burning Unit of South Africa, 650-900 victims of different ages have been admitted to a specialized children's hospital in just one year [19]. It is stated that children under 5 are the most endangered age group, and 50% of all burns occur in children under 2 years. The main cause of burns in them is the burning of hot liquids. The fire causes only 13% of the burns but causes 83% of the deaths [23]. The purpose of this study is to investigate the need to increase parental awareness of the risk and severity of burns in adolescents. The sociological survey was conducted in the period May-June 2019, through its own anonymous poll among 91 parents of kindergarten children in Sofia. The survey found that a significant proportion (75.82%) of the respondents estimated the severity of the impact of trauma on the health and life of the victims and in the event of a burn incident, about half would seek assistance from the Emergency Medical Center and only 5.05 % of them to specialized structures for the treatment of burns. A significant proportion (95.60%) of the respondents confirm the need to provide information on the possible risks, and 95.60% of them expect this to reduce incidents in the social and school environment. Raising the awareness of the population regarding the risk factors for burning and carrying out health education on the topic among adolescents will influence the incidence of incidents in the social, industrial and school environment.

IntroductionMost mental disorders begin during adolescence and early adulthood (18–24-years-old), highlighting the importance of understanding the onset and progression of mental disorders among youths. Although progress has been made, namely by creating youth mental health transition services, gaps continue to exist. Locally, our recently constituted young adults unit aimed to minimize discontinuities in care.Objectives/aimsTo characterize a population of young adult psychiatric outpatients, regarding socio-demographic and clinic variables.MethodsSocio-demographic and clinic characterization of young adult psychiatric outpatients observed during 1st January 2015–30th July 2016.ResultsTwo hundred and fifty-five outpatients were observed: 64.3% females and 35.7% males, average age 20.56-years-old (median 20). Most lived in urban areas (59.4%), with their parents (27.8%), were students (80.4%), attending secondary school (36.1%). A total of, 27.5% were referenced by an emergency department, and adjustment disorders (ICD-10 F43.2) were the most frequent diagnosis (21.6%). Regarding suicidal behaviours and self-harm, 9.8% did self-cutting. A vast majority did not have previous psychiatric hospitalizations–only 5.9% outpatients had at least one. In total, 39.8% were medicated with antidepressants (1/3 of which in association with other drugs), and about 38.8% received cognitive-behavioural interventions. About 2/3 of patients (66.3%) remained in care and only about 1/6 (15.7%) were discharged.ConclusionsOur typical youth psychiatric outpatient was of female gender, student, living with its parents. Adjustment disorders were the most frequent diagnosis, and antidepressants were the main psychopharmacologic option, often combined with other psychotropic drugs. Abstract Introduction: Obesity and type 2 diabetes mellitus (T2DM) are growing health concerns. A ten-fold increase of T2DM was noted in the Hong Kong paediatric population from 1997 to 2007. T2DM is often asymptomatic at presentation, but complications can emerge rapidly, especially in youngsters. Experience in Japan, Korea and Taiwan suggests that urine glucose screening is a practical and non-invasive screening tool for identification of T2DM. The Hong Kong Student Health Service (SHS) offers yearly health checks for students and is a good platform for screening of T2DM since attendance rate is over 90% for primary school students and over 70% for secondary school students. Method: In 2005, SHS and the Hong Kong Paediatric Society formulated a protocol on urine glucose screening for early diagnosis of T2DM in students with obesity. Students between the ages of 10–18 years old with age- and sex- specific body mass index (BMI) &gt;97th percentile were recruited. Those screened positive for glycosuria were referred to paediatric departments for workup under a standardized protocol, whilst those who screened positive for both glucose and ketones were referred to the emergency departments. Students enrolled from school year 2005–2006 to 2017–2018 were included. Demographic data, clinical presentation, investigatory results and co-morbidities were captured using a structured reply letter. Results: A total of 219,276 eligible students attended SHS in the years specified and 216,528 students (99%) completed urine glucose screening. 381 (0.18%) students were tested positive for urine glucose; 18 (4.7%) had concomitant urine ketones. In total 120 students had T2DM, 41 had pre-diabetes [impaired fasting glucose and /or impaired glucose tolerance] and 126 turned out normal. 43 students defaulted the referrals and 51 students had known diabetes. 21 students (17.5%) were started on insulin therapy upon diagnosis. A significant proportion of students with T2DM had co-morbidities including raised alanine amino-transferase (57%), hypercholesterolaemia (59%), and hypertension (13%). Five students (4.2%) had microalbuminuria at presentation. Of those with ketonuria, two students had serum glucose of over 20mmol/L and required fluid resuscitation ± insulin infusion in high dependency unit. Conclusion: Our pick up rate for T2DM from students with obesity aged 10–18 years using urine glucose is 0.05% (120/216,528). According to the Hong Kong Childhood Diabetes Registry, the crude incidence of T2DM for this age group was 6.16 /100,000/year over the study period, which equates to 506 new cases of T2DM. Thus 24% of the new T2DM cases were diagnosed by this program and many had associated co-morbidities at diagnosis. Our study shows that urine glucose testing is an inexpensive and simple test that allows for early diagnosis and treatment of T2DM in the primary care setting in this at risk population.

A local man sits on the pavement in Patan Durbar Square at one am. A taxi driver calls a cell phone number I handed to him that belongs to the guesthouse I am staying at. Both of them are strangers to me, but go out of the way to make sure I reach home safe. It struck a chord, nice warm people and very hospitable. The next morning begins with meeting Mr. Shakya, the person in charge of visitors at Patan hospital, touring the hospital, including the outpatient department, emergency room, wards, auditorium, and medical school. I’m greeted with Namaste, and introduced as “tyo America ma doctor ho”. This is followed by a most important visit to the cafeteria. We go over the menu. I am told the most popular options and also advice to not order ‘jawanoko jhol” (for nursing females only, helps let down), I am told this will be explained soon in private. Curiosity grew. This hospital tour has surprised me because what I had in mind was a hospital with few rooms with barely any equipment. On the contrary, Patan hospital is fairly well equipped and organized with a CT scanner, multiple wards, and the medical school has an extensive public health department. Meanwhile I am offered a tea and biscuit everywhere. My first few hours of touring showed me that this is going to be a fun experience. This is followed by a stop at the Intensive Care Unit (ICU) where I will be on rotation for the next few weeks. Another cup of tea and biscuit follows, fourth of the day already. Day one starts in ICU with meeting medical officers, staff, patients and nurses. Talking to patients helps me learn their perspective. It is quick mixing up with medical officers through lunch strolls, breaks. Our lunch discussions are very interesting, sharing our knowledge and experiences about our culture and educational system. What struck me then is I read and hear so much about what visitors have to say and how extraordinarily helpful this experience has been in shaping their career.1 But I wonder what it’s like for the local officers, what’s the impact on the medical officers, residents and students. Is it helpful? Do they feel they learn something, positive or negative impact? Or is it a source of brain drain?

Background:Clostridioides difficile infection (CDI) is one of the most common laboratory-identified (LabID) healthcare-associated events reported to the National Healthcare Safety Network (NHSN). CDI prevention remains a national priority, and efforts to reduce infection burden and improve antibiotic stewardship continue to expand across the healthcare spectrum. Beginning in 2013, the Centers for Medicare and Medicaid Services (CMS) required acute-care hospitals participating in CMS’ Inpatient Quality Reporting program to report CDI LabID data to NHSN and, in 2015, extended this reporting requirement to emergency departments (ED) and 24-hour observation units. To assess national progress, we evaluated changes in hospital onset CDI (HO-CDI) incidence during 2010–2018. Methods: Cases of HO-CDI were reported to NHSN by hospitals using the NHSN’s LabID criteria. Generalized linear mixed-effects modeling was used to assess trends of HO-CDI by treating the hospital as a random intercept to account for the correlation of the repeated responses over time. The data were summarized at the quarterly level, the main effect was time, and the covariates of interest were the following: CDI test type, inpatient community-onset (CO) infection rate, hospital type, average length of stay, medical school affiliation, number of beds, number of ICU beds, number of infection control professionals, presence of an ED or observation unit , and an indicator for 2015 to account for CDI protocol changes that required hospitals to conduct surveillance in both inpatient and ED or observation unit setting. Results: During 2010–2013, the number of hospitals reporting CDI increased and then stabilized after 2013 (Table 1). Crude HO-CDI rates decreased over time, except for an increase in 2015 and steeper reduction thereafter. (Table 2). During 2010–2014, the adjusted quarterly rate of change was −0.45% (95% CI, −0.57% to −0.33%; P < .0001). The rate of reduction was smaller in 2010–2014 compared to those of 2015–2018 (−2.82%; 95% CI, −3.10% to −2.54%; P < .0001). Compared to 2014, the adjusted rate in 2015 increased by 79.14% (95% CI, 72.42%–86.11%; P < .0001). Conclusions: The number of hospitals reporting CDI LabID data grew substantially in 2013 as a result of the CMS requirement for reporting. Adjusted HO-CDI rates decreased over time, with a rate hike in the year of 2015 and a rapid decrease thereafter. The increase in 2015 may be explained by changes in the NHSN CDI surveillance protocol and better test type classification in later years. Abstract Background The American Academy of Ophthalmology, in conjunction with the Accreditation Council of Graduate Medical Education (ACGME), has mandated the creation of a linked ophthalmology internship year for all programs by 2021. This mandate provides a unique opportunity to tailor a curriculum specific to the ophthalmology trainee. This study reports the results of a national survey of ophthalmology residents and fellows regarding the relevance and applicability of extramural content to their ophthalmology training. Methods Using the online portal SurveyMonkey, an anonymous survey was sent to 119 ACGME-accredited ophthalmology residencies. It consisted of nine questions formulated to measure the perceived applicability and relevance of other specialties to ophthalmology training. Responses were summarized with descriptive statistics. Outcomes were compared by one-way analysis of variance with Tukey's post hoc analysis for between group differences and Benjamini–Hochberg for multiple comparisons. Results Ninety-four respondents (residents and fellows) completed the survey (January–February 2020); 52.1% completed an internal medicine internship, 42.6% transitional, and 5.3% general surgery. Overall, prior internship experience was rated favorably. Otolaryngology and radiology were deemed the most relevant to ophthalmology training. Intensive care unit (ICU) was considered the least beneficial, followed by general surgery, primary care, and pediatrics. Neuroradiology was overwhelmingly preferred over general radiology. The majority of respondents reported minimal exposure to oral and maxillofacial surgery (OMFS) in medical school/internship. When choosing a reason for inclusion in the new curriculum, respondents reported a high level of surgical overlap with otolaryngology, OMFS, and plastic surgery; clinical overlap with neurology; and ophthalmic consultation with emergency medicine. Conclusion Several specialties—notably otolaryngology, radiology, and neurology—were considered highly relevant but lacking in preresidency exposure. The preference for neuroradiology was almost unanimous. In contrast, general surgery and ICU were found to be minimally relevant. Overall, ophthalmology residents reported a high level of satisfaction with their internship experience, an important consideration when building a curriculum that will appeal to potential applicants. The findings of this survey provide a framework to build a curricular plan based on current resident and fellow experience.

The Coronavirus Disease (COVID-19) pandemic has contributed to a co-occurring psychiatric epidemic. Children and adolescents have been particularly impacted, with disruptions in continuity of learning and healthcare. Psychological stressors such as fear of infection, boredom, decreased socialization, supply shortages, and incomplete information have contributed to low mood, irritability, insomnia, and emotional exhaustion. Our study contributes to this research by quantifying the devastating psychiatric toll that the COVID isolation period and virtual schooling had on the mental health of children. The Loyola University Medical Center (LUMC) Consultation-Liaison Psychiatry Service (CLP) (Maywood, IL) designed a study to identify and analyze data regarding the total number of CLP consult requests, patient presentations to the emergency department (ED), and admissions to the Pediatrics floor and Pediatric Intensive Care Unit (PICU) during the COVID pandemic. IRB approval was obtained for this retrospective chart review study. ED visits and Pediatric floor/PICU admissions of all pediatric patients (6-18 years old), specifically those related to mental health, were identified using ICD-9 diagnostic coding for the dates of March 2019 through February 2022. The total number of admissions and encounter diagnoses were analyzed and compared between 3 pandemic periods: Pre-pandemic phase (March 2019-February 2020), acute pandemic phase: isolation/virtual schooling (March 2020-February 2021), and chronic pandemic phase: post-isolation/virtual schooling (March 2021-February 2022). During the chronic pandemic phase, ED visits and Pediatric floor/PICU admissions of all pediatric patients were identified via manual chart review of existing CLP patient lists. The total number of admissions, along with diagnoses during encounters, were compared between the 3 periods and psychiatric admissions were further subcategorized into intentional drug overdoses. In the pre-pandemic phase, there were 39,304 total encounters for children (6-18 years), with 11,189 ED-only visits, and 3,043 inpatient admissions. In the acute pandemic phase, total encounters fell to 15,779, with 4,631 ED-only visits, and 2,578 inpatient admissions. Pre-pandemic, 0.55% of all pediatric encounters required psychiatry consults compared to 1.32% after the onset of the pandemic. During the pre-pandemic phase, there were 27 PICU admissions for psychiatric-related conditions (i.e. intentional ingestion) or 4.52% of all PICU admissions. During the acute pandemic period, there were 57 PICU admissions for psychiatric-related conditions or 11.19% of all PICU admissions. Specifically, there was a rise in intentional ingestion cases from 25 prior to the pandemic to 44 after the onset of the pandemic. During the chronic pandemic period, there were 43 PICU admissions for psychiatric-related conditions. Of those 43 admissions, 33 were intentional drug overdoses. Total encounters during this phase were comparable to the pre-pandemic phase (39,304 encounters pre-pandemic vs. 32,544 encounters in chronic pandemic phase).At this large academic medical center, the pandemic has had a clear impact on total pediatric encounters, with a rise in the number of PICU admissions for psychiatric-related conditions. Findings from this study can be used to inform public policy and develop guidelines in preparation for future pandemics. Safeguards should be put in place to address the significant effects that social isolation and virtual schooling have on the mental health of children, including but not limited to widespread therapy sessions incorporated into the virtual school day and socially distanced in-person activities when safe.

BACKGROUND: Nurses’ perception of medical device–related pressure injuries (MDRPIs) may affect their performance, but there is a lack of studies in this area. PURPOSE: The current study aimed to examine intensive care unit (ICU), cardiac care unit (CCU) and emergency department nurses’ perception of proper prevention of MDRPIs and care for individuals with such injuries. METHODS: This descriptive study was conducted in 4 general hospitals in Iran in 2021. All nurses (N = 310) working in ICUs, CCUs and emergency departments of these facilities were invited to complete a researcher-made demographic checklist and an 11-item questionnaire to assess attitudes toward MDRPIs. The questionnaire item responses were scored from 1 (strongly agree) to 5 (strongly disagree) with the total score for the 11 items ranging from 11 to 55. A score of 11 to 25 was categorized as indicating a negative attitude toward proper prevention of MDRPIs and care for such patients; a score of 26 to 40 indicated a neutral attitude, and a score >40 indicated a positive attitude. RESULTS: A total of 260 nurses fulfilled the data collection tool. The response rate was 83.8%. The mean total score of attitude toward MDRPIs was 41.7. No significant relationship was observed between the total score of nurses’ attitudes and their demographic variables. Of the 260 participants, 159 stated they had not received any trainings on MDRPIs at nursing schools during their education, 212 stated they had not participated in any scientific workshops on MDRPIs, and 167 described their knowledge about the prevention and care of MDRPIs as insufficient. CONCLUSION: Among ICU, CCU, and emergency nurses in Iran, most had a positive attitude toward the prevention and care of MDRPIs, but steps should be taken to offer more opportunities for nurses to increase their knowledge in this area.

This study aimed to identify problems, describe, analyze, and evaluate the use of e-commerce to increase the competitiveness of innovative and entrepreneurial products produced by students of State Vocational High School 3 South Tangerang, Banten province. This research had three main stages, namely the preparation stage, the implementation stage, and the evaluation stage. The research location was the State Vocational High School 3 South Tangerang, Banten province, Indonesia, due to the school being outstanding in the field of Motorcycle and Animation expertise. The resource persons and respondents in this study were teaching teachers and students in classes 10-12 majoring in culinary, automotive, and animation. Implementing e-commerce activities at the production unit of State Vocational High School 3 South Tangerang, has increased the image of the production unit of State Vocational High School 3 South Tangerang. This study was a descriptive qualitative with the questionnaire, interview, and practice method. This study revealed that the application of e-commerce using the Shopee seller marketplace provided significant benefits and impacts for the production unit of State Vocational High School 3 South Tangerang. When the production unit implemented e-commerce, sales turnover increases rapidly, and the number of consumers, as a means of promotion, ease of transaction did not need to meet directly with buyers and can expand the business. However, there were also several obstacles, like weak human resources, intense competition, and the emergence of plagiarism of ideas and products.

UKBM is a small learning unit based on basic competency-based textbooks. The preparation is done sequentially from the easiest to the most difficult to help students learn independently and achieve learning mastery. The research aims to (1) determine the characteristics of the UKBM compiled, (2) describe the preparation and development of UKBM, (3) analyze the development of UKBM-based assessment components, and (4) find obstacles, problems, and solutions for teachers in developing UKBM-based assessment components for Biology High School in Malang City. This research is qualitative descriptive research at State High School in Malang City. The data is primary data from interviews with 5 Biology teachers and secondary data from UKBM scores and documents. The instruments used were interviews, observation, and documentation guidelines. The characteristics of UKBM Biology at State High School in Malang City are in accordance with the characteristics of UKBM guidelines. The preparation and development by the Principal who represents the school in training, then developed by teachers and MGMP that adjusted in each school. Obstacles to implementing UKBM are (1) the emergence of heterogeneous groups in the classroom based on the ability to accelerate UKBM implementation and (2) requiring team teaching and extra effort given that student learning acceleration is different and learning infrastructure adjustment to learning progress and needs.

Abstract Background Air pollution is a major environmental risk to health. Exposure to xenobiotic heavy metals such as lead and cadmium—constituents of air pollution such as particulate matter and nitrogen and sulfur oxides, organic solvents, and other environmental pollutants—could be component causes for schizophrenia and other psychotic disorders. Long-term exposure to ambient air pollution could be an independent risk factor for mental health disorders. Recent studies analysing the association between daily levels of air pollutants and hospital admissions for mental disorders showed significant results for different pollutants considered both for admissions for generic mental disorders and for specific diagnoses such as schizophrenia, depression and substance abuse. Aim of the present study is to investigate the associations between daily levels of air pollutants (particulate matter, ozone, carbon monoxide, nitrogen dioxide) and daily admissions for mental disorders to the emergency department of two general hospitals in Umbria region (Italy) Methods We collected data about daily admissions to psychiatric emergency services of two general hospitals, daily levels of respirable particular matter (PM10 and PM2.5), ozone (O3), carbon monoxide (CO), nitrogen dioxide (NO2), and meteorological data (daily average temperature, pressure, humidity, and maximum and average wind speed) for the time period 1 January 2015 until 31 December 2016 (731 days). We used air monitoring data for the different types of air pollutants which were averaged across up to 16 monitor stations dispersed in the region. We assessed the impact of an increase in air pollutants on the number of daily admissions using a time-series econometric framework. Results A total of 1860 cases of emergency department admissions for mental disorders were recorded over the study period. We observe a statistically significant impact of O3 levels on daily admissions. The estimated coefficient of O3 is statistically significant at the 1 percent level. According to our results, an increase of one µg/ m3 of O3 concentration (relative to the average concentration of the last 20 days) results in 0.013 more admissions to the hospitals. All other pollutants are not significantly associated with the number of daily admissions. Discussion We observed a statistically significant association of daily ozone levels and daily number of admissions to psychiatric emergency services. Ozone is a component of photochemical smog and a powerful oxidant, and is considered as one of the most important air pollutants. Ozone may have a relevant interference with CNS physiology, and its exposure may be linked to brain disease and contribute to inflammation and oxidative stress. Ozone exposure may be considered a potential environmental risk factor for impaired mental health. Epidemiological studies found evidence that ozone exposure may affect autism spectrum disorders, lead to motor disorders and cognitive impairment, and have an influence on the incidence of depression and suicide. In recent studies ozone air levels showed an association with increased risk of an emergency department visit for depression. This is one of the first studies worldwide investigating the association between daily concentration of air pollutants and the daily number of visits to a psychiatric emergency unit. Our results add to previous literature on existing evidence for air pollution to have a role in the cause or worsening of mental distress and psychiatric disorders. Contrary to our hypothesis, we did not find a positive association between most of the air pollutants considered in our study. This is in contrast with previous literature on this subject. This could be explained by the methodological limitations of our study.

The article considers the principles of the emergence of virtual assets (cryptocurrencies) and the impact on these events of economic, socio-political relations through technological features. The paper considers the influence of the Austrian School of Economics and its achievements on the emergence and development of modern virtual assets, namely they considered the impact on the choice of available currencies: 1) as the purchase of goods and services, 2) as a reserve for future needs; 3) as deferred payments; 4) as a unit of account. According to Hayek's theoretical framework, market forces will determine the relative values of different competing currencies. As a result, exchange rates between competing currencies will float freely. The ideas of scientists such as Hayek and Mises and the ideas of the Austrian school of economics formed the basis of such a political trend as cryptoanarchism. Cryptoanarchism can be described as the realization of cyberspace and the manifestation of anarchism, a philosophy whose main goal is to respect human freedoms in cyberspace, regardless of status. Abstract BACKGROUND: Attention deficit hyperactivity disorder is considered the most common childhoodneurobehavioral disorder worldwide with well documented adverse consequences in adolescence and adulthood, yet 60-80% of cases go undiagnosed. Routinescreening for the condition is not practiced in most pediatric outpatient services andlittle information exists on factors associated with the condition in developingcountries. OBJECTIVES: This was a questionnaire based cross-sectional survey whose primary objective was to determine prevalence of attention deficit hyperactivity disorder (ADHD) symptoms in children aged 6-12 years attending the Accidents and Emergency unit of a tertiary care hospital in Nairobi. Secondary objectives were to (i) ascertain if physical injury and poor academic performance were associated with ADHD, (ii) compare diagnostic utility of parent-filled Vanderbilt Assessment Scale (VAS) against Statistical Manual of Mental Disorders-IV (DSM-IV) as the gold reference and (iii) establish if there exists an association between ADHD symptoms cluster and comorbid conditions. DESIGN/METHODS: The study was undertaken at the paediatric accidents and emergency (A&E) section of the Aga Khan University Hospital (AKUHN) between March and June 2012. AKUHN is a private, not for profit, tertiary health care facility based in Nairobi, Kenya. Paediatrics A&E offers a 24-hour service provided by paediatric residents and senior house officers under the supervision of paediatric registrars. Children aged 6-12 years were enrolled provided guardians demonstrated ability to read and write in English. A written signed informed consent was also required from the primary care provider. Children on methylphenidate, antidepressants or behavioral therapy and those with neurological disorders, hearing and visual impairments or need for emergency care were excluded. Those who consented were clinically evaluated and treated for the ailments that brought them to hospital prior to completion of the self-administered study questionnaire. Sample size was estimated at 240 based on estimated ADHD prevalence of 6% reported by Kashala et al from a neighboring country with similar socio-economic setting as Kenya. Study approval was obtained from the Aga Khan University Hospital Scientific and Ethical Review Committees. Enrolling of children was done after written consent from parents or primary guardians as required by the institutional review board for children under the age of 18 years. It was made clear that recruitment was entirely voluntary and that refusal to participate would not in any way compromise provision of care. Study records were secured in a locked cabinet to safeguard confidentiality. Study was carried out using a two-stage ascertainment procedure. Children were evaluated for eligibility after registration at the reception between 9am to 8pm during week days. A maximum of 10 participants were recruited on any given day to minimize burden in the department and to hopefully capture a wider spectrum of medical conditions. Details about the study were explained to the parents by the principal investigator or the research assistant after patients had been seen by the clinician for the presenting problem. Information necessary for DSM-IV classification was obtained from parents who also completed VAS form. Care providers of study children were requested to complete the risk assessment form with assistance provided as needed. It contained questions about school performance such as repetition of class and average end of term marks which was categorized as; below 25%, 25-50%, 50-75% or above 75%. A grade above 50% was considered as acceptable performance. Only injuries for which medical treatment was sought were considered for inclusion and categorized into burns,fractures and open wounds. Information on causes of injuries was classified under falls, fight, car accident and others. Completion of an assessment form took approximately 15 minutes after which questionnaire was scored and tabulated before providing feedback to parents. Data were entered in Microsoft Excel® and analysis done using STATA®Version 11 (StataCorp). Prevalence of ADHD symptoms was calculated using the number of positive cases as numerator and study population as denominator. Chi square or Fischer’s exact test were used as appropriate to compare categorical variables with P-value below 0.05 considered significant. Wilcoxon test was used for ordinal data. Odds ratios (OR) were used to determine association between ADHD symptoms and categorical variables and 95% confidence interval (CI) to determine precision around individual estimates. RESULTS: Prevalence of cluster of symptoms consistent with ADHD was 6.3% (95% CI; 3.72-10.33) in 240 children studied. Those affected were more likely to repeat classes than the asymptomatic (OR 20.2; 95%CI 4.02-100.43). Additionally, 67% of the symptomatic had previously experienced burns and 37% post-traumatic open wounds. The odds of having an injury in the symptomatic was 2.9 (95%CI; 1.01-8.42) compared to the asymptomatic. Using DSM-IV as the reference, VAS had a low sensitivity of 66.7% (95%; CI 39.03-87.12) but specificity of 99.0% (95%CI; 96.1-99.2). Its positive predictive value was 83.0% (95%CI; 50.4-97.3) and the negative predictive value 98.0% (CI 95.1-99.1). Positive and negative likelihood ratios were 75(95%CI; 18.3-311.2) and 0.3 (95%; CI 0.21-0.73) respectively. Oppositional defiant disorder symptoms, anxiety, depression and conduct problems were not significantly associated with ADHD cluster of symptoms. CONCLUSION: A relatively high prevalence of symptoms associated with ADHD was found inchildren visiting the Paediatric Accidents and Emergency department. Symptomaticchildren had also experienced more poor school performance. These findings makea strong case for introduction of a policy on routine screening for ADHD in pediatricoutpatient service in a similar setting. Positive history of injury, especially burns, and poor academic performance is associated with symptoms of ADHD which should trigger need forfurther evaluation for ADHD and appropriate referral. Even though easier toadminister than DSM-IV, Vanderbilt assessment scale has low sensitivity hence itwould not be appropriate for use in ADHD screening. However, in view of its highspecificity and ease of administration, it could be used as an alternative confirmatorytest to determine who among clinically symptomatic patients would require referral to a psychiatrist for further evaluation and management.

Within the framework of the discussions are the external evaluations as a way of qualifying the schools and the education systems in the country. It is based on the epistemological perspective on education. It is an exploratory and descriptive research, of qualitative approach, according to socio-cultural studies, evaluation of public policies; Analysis and description of actions of education. The aim is to verify the extent of public policies to promote dignity in social and ethnic minorities, at national level, in addition to their participation in the emancipation of the individual and the promotion of the quality of life and human dignity.

They aim to scrutinize the historical order and evolution of the state of social protection and its emergence, as well as its importance not only in Brazil but also in the world within the field of public policies. The discussions about the quality of education in Brazil raise questions about teacher training, financing, physical structure of the building, pedagogical practice, socioeconomic profile of the student and school management - elements considered determinants of the quality of education in a school unit. Within the framework of the discussions are the external evaluations as a way of qualifying the schools and the education systems in the country. The application of tests that assess the academic performance of students from public and private schools in Brazil became a constant from the 1990s, after the State reform and with a new political agenda for the educational area, in view of the reconfiguration of the economy and the valuation of criteria such as efficiency, effectiveness, productivity and competences for Brazilian education. The guiding problem of work is pubic and social policies as an instrument of the Welfare State. It is based on the hypothesis of the need for public policies at the federal, state and municipal levels that relate to the quality of education and to what is the citizen's right or need in its basic aspects. It is a study described in documentary review and literature. It is hoped to contribute between the relationship of the state of social protection and education and reaffirm its importance for the social and political development of citizens and citizenship.

To describe the condition of the teachers in Rokan Hilir Regency according to the type of education unit and the level of education in a more accurate and clear manner and to find out the extent to which social status, system pressure, limited facilities and the work environment influence the aspirations of public school teachers in Rokan Hilir Regency to improve their professionalism. The population in this study were public primary school teachers who served in Rokan Hilir Regency, Riau, Indonesia. The primary data was collected by circulating questionnaires with closed questions and equipped with several open questions to obtain opinions that are free from the respondents. The findings are the aspirations to increase professionalism are in between middle and low level, such aspirations indicate that there is a tendency for ritualism in the form of obedience to ‘institutional means’. No previous study explain the tendency of the emergence of ritualism symptoms of teachers in school by measuring their aspiration level to improve professionalism

<p><strong>Abstract.</strong> Since movement between countries has become easier for people, preventing the spread of various infectious diseases occurring around the world has become an international public health challenge. The use of “disease maps” in infectious disease prevention can encourage a range of studies on patterns of occurrence and modes of transmission to create thematic visual guides charting the geographical spread of diseases, and infectious disease research agencies around the world are conducting epidemiological surveillance for tracking and analyzing the occurrence of infectious diseases. In the case of Japan, influenza is a recurring and widespread infectious disease for which surveillance programs involving mapping are ongoing. With the development of Information-Communication Technology (ICT), surveillance information is easily accessible on the Web to both medical professions and ordinary citizens, as are online Geographical Information Systems (GIS) utilizing these data. Adapting to these transformations in the environment of information delivery requires that disease maps reflect an accurate grasp of local trends and real-time information delivery.</p><p> Most studies of disease mapping have focused on the technical dimensions of the utilization of disease maps and WebGIS, and very little research has evaluated the use of disease maps and the quality of surveillance information on a regional scale. Accordingly, the aim of this study is to examine the current status of utilization of disease maps in Japan and issues for health crisis management, with a focus on local influenza surveillance.</p><p> We conducted a survey of the websites of specialized agencies and local governments in Japan and we engaged in semi-structured interviews with officials from nine agencies that deliver epidemiological information using disease maps. The website survey assessed the quality of information provided by 82 public health institutes, 552 public health centers, 1,042 medical associations, and 1,977 local governments based on an index of 15 items measuring spatial scale along with map forms and usage. Interview items encompassed the “history of construction and management of the local surveillance system,” “users and utilization of local surveillance,” “impact of introduction and relationship with other surveillance measures,” “new developments and enhancements.”</p><p> From the results of website survey, we found that only 332 agencies and local governments delivered original information on infectious disease jurisdictional districts, namely 116 public health centers, 108 medical associations, and 51 local governments. The spatial scale of surveillance generally corresponded to the jurisdictions of agencies and local governments; however, medical associations are provided at various levels, such as the county and city medical association level, municipal district level, public school district level, chome and aza (block) level, school facility level, and hospital and clinic levels. Among a total of 56 agencies and local governments conducting visualization, only few three were found to be using WebGIS.</p><p> The semi-structured interviews revealed that the construction of local surveillance systems was necessary due to limitation of existing nationwide surveillance in rapid information delivery. Specialized agencies and local governments operating regional surveillance pointed out three problems of current nationwide infectious disease surveillance. First, the number of patients reported is limited because a few sentinel medical institutions report this kind of information. Second, use of the FAX information delivery system results in a time lag of two weeks for data aggregation and delivery. Third, because the minimum spatial unit is the public health center level, detailed information at the level under jurisdiction is not available. Such challenges arose amidst the crisis caused by the influenza A/H1N1pdm09 pandemic in 2009. In the aftermath of the failures of the 2009 epidemiological response, agencies and local governments cooperated with local government departments and the board of education to establish a system to collect information from all medical institutions and school facilities in the jurisdiction. Through visualizing such data as a disease map and sharing progress online, local surveillance has been effective in supporting medical consultations among local doctors, infection prevention at school facilities, and emergency response for night-time medical treatment, among other interventions.</p><p> Local surveillance is informed by data in the jurisdiction of each specialized agency and local government; however, disease maps utilizing these information are maintained by 56 agencies and local governments. To construct the information delivery system on the local scale, we recommend the promotion of cooperation between agencies and local government departments and the strengthening of interactions between medical professionals, and mapping and GIS experts. In addition, it is evident that disease maps are effective for medical consultation support and infection control measures in familiar areas. From this perspective, local-scale disease maps have potential to be utilized as a communication tool for the sharing of risk management information between medical profession and residents.</p>

Abstract This summer I worked in the Cruikshank lab at the Boston University School of Medicine. The Cruikshank research team determined that mannose lipoarabinomannan, (ManLAM), a cell wall component of M. tuberculosis can decrease S1P sensitivity of activated T cells as well as to alter production of selected cytokines. These alterations likely affect the normal immune response by changing T cell homing patterns and decreasing production of response cytokines thereby increasing the mTB’s chances of survival and optimal infection. This research inspired me to create a high school biology unit. This unit is designed to link the study of the immune system with evolution through a closer examination of mTB, particularly, its two main forms: latent TB infection and active TB disease. The unit includes teacher presentations and demonstrations, an internet activity entitled TB in the Media, and laboratory activities. Students are first introduced to the basic functions of the immune system through its defense against Mycobacterium tuberculosis infection. Then, students work through a simulated ELISA to see its importance in diagnosing disease. Finally, we turn to the emergence of multi-drug resistant mTB as students carry out a laboratory exercise on bacterial transformation. Students present a summary of their learning through the creation of a pamphlet/poster where they investigate a specific disease and describe its symptoms, treatments, suggested causes and immune system interactions.
