The optical interface between a light source and a detector is important. In most practical realizations the aim should be to collect the maximum light possible. Lens systems seldom do this efficiently, especially where the light source is diffuse. Underlying any attempt at concentrating or guiding light is subject to a fundamental limitation referred to as étendue (phase space cannot be squeezed). Light collection from small volume scintillators of high refractive index may approach 50 %, while collection from large-area scintillators is typically less than a few per cent. Incorporation of wavelength-shifting light guides and fibres leads to enhanced performance. Efficiency measurements by the author in terms of photoelectrons per keV are presented for selected configurations. Optical recycling derived from total internal reflection provides enhancement in effective quantum efficiency by a factor of up to 10. Concepts such as escape cones, adiabatic light guides, and trapped light are covered in detail.

One of the criticisms against international arbitration is its occasional failure to live up to its potential for the expeditious resolution of disputes. Indeed, there is a tension at the core of modern international arbitration. On the one hand, parties seek speedy, cost-efficient, and final dispute resolution; while on the other hand, the absence of an appeal mechanism for arbitral awards generates a perceived need for exhaustive analysis of every fact and conceivable argument and for the retention of the ‘best’ (and therefore the busiest) lawyers and arbitrators, creating an attendant risk of ever-longer and more expensive arbitration proceedings. This chapter discusses the SIAC's rules designed to achieve expedition and cost-efficiency in all cases. These include Rule 5.1 on circumstances in which the expedited procedure may be appropriate, and Rule 5.2 covering the SIAC expedited procedure.

Exogenous ketone supplements rapidly elevate blood ketones in a dose-dependent manner regardless of dietary intake, making them a practical method of inducing therapeutic ketosis for medical use. It is thought that ketone supplementation could be used as a stand-alone therapy, or as a way to further augment the therapeutic efficacy of the ketogenic diet. Ketone supplementation could increase treatment compliance by allowing many patients to maintain a more normal lifestyle with a less restrictive diet. The therapeutic effects of ketone supplementation are likely mediated in part by a stabilization of blood glucose and insulin levels, an increase in metabolic efficiency, and an inhibition of oxidative stress and inflammation. Ketone supplements may also serve as an effective preventative medicine due to their potential ability to protect and enhance mitochondrial health and function. Indeed, preliminary evidence suggests there are a number of conditions for which exogenous ketone supplementation may be beneficial.

The clinical mycological laboratory strives to provide an accurate and rapid diagnosis of suspected fungal disease, or exclude its possibility. The most common and most traditional methods for demonstrating fungi in tissue and body fluids are microscopy and culture. The techniques discussed in this chapter are not exhaustive but have proved to be efficient for the visualization and recovery of fungi from clinical samples. In general, these methods do not require expensive or specialist equipment. The clinician’s presumptive diagnosis may help with the selection of the most appropriate specimen and aid the laboratory in the method of processing and selection of the primary culture media. However, all too often there is scant clinical information provided to the laboratory, and indeed a fungal infection may initially not have been suspected.

While bottom-up visual processing is important, the brain integrates this information with top-down, generative expectations from very early on in the visual processing hierarchy. Indeed, our brain should not be viewed as a classification system, but rather as a generative system, which perceives something by integrating sensory evidence with the available, learned, predictive knowledge about that thing. The involved generative models continuously produce expectations over time, across space, and from abstracted encodings to more concrete encodings. Bayesian information processing is the key to understand how information integration must work computationally – at least in approximation – also in the brain. Bayesian networks in the form of graphical models allow the modularization of information and the factorization of interactions, which can strongly improve the efficiency of generative models. The resulting generative models essentially produce state estimations in the form of probability densities, which are very well-suited to integrate multiple sources of information, including top-down and bottom-up ones. A hierarchical neural visual processing architecture illustrates this point even further. Finally, some well-known visual illusions are shown and the perceptions are explained by means of generative, information integrating, perceptual processes, which in all cases combine top-down prior knowledge and expectations about objects and environments with the available, bottom-up visual information.

The chapter describes the historical process as well as the analytical and empirical factors that, at the end of the twentieth century, led to the dominance, in advanced economies, of a central bank model based on an independent institution devoted to price stability as its overriding objective. The central bank pre-crisis model was elegant, performing, and efficient. However, it could not easily accommodate the pursuit of a traditionally important central bank objective: financial stability. Indeed, since central banks have, in essence, just one tool, that is, the interest rate, the pursuit of a financial stability objective in addition to a price stability objective could create dilemma situations. In the two decades between the mid-1980s and the mid-2000s, the economies of advanced economies were very stable, and this period was thus identified as Great Moderation. However, subsequent experience showed that, in this period, the crisis was incubating.

Drawing on the case of Ohio black ministers and their voter-mobilization efforts leading up to President Barack Obama’s 2012 reelection, this book addresses when, how, and why black ministers engage in broad-based mobilization in the twenty-first-century United States. We learn they can indeed mobilize, and they can do so effectively and efficiently. Black religious leaders across Ohio mobilized the black vote in 2012. Their efforts, it seems, paid off. For the first time, we saw a greater proportion of the black electorate vote than the white electorate. The question begs: If black religious leaders could collectively mobilize on a broad scale for the black vote, why have they not done so for black lives? We propose four factors affecting contemporary black ministers’ engagement in mobilization: post-civil-rights-era racism, changes in the field of organizing, nostalgic reverence for the civil rights movement, and the structure of the black minister network. In many ways, the very thing that sustains the black religious leader community also constrains black ministers’ capacity to mobilize in today’s world.

What is more, it not only provides guidance to lawyers in such situations but also helps them understand crises better so that they can respond more effectively, efficiently, appropriately, and creatively. This volume strives to improve crisis and emergency responses by shining a light on the role of the law and the lawyers who help resolve crises. General anaesthesia (GA) is most often indicated for category 1 (immediate threat to life of mother or baby) caesarean delivery (CD) or when neuraxial anaesthesia has failed or is contraindicated. Secure intravenous access is essential. Jugular venous cannulation (with ultrasound guidance) is required if peripheral access is inadequate. A World Health Organization surgical safety checklist must be used. The shoulders and upper back should be ramped. Left lateral table tilt or other means of uterine displacement are essential to minimize aortocaval compression, and a head-up position is recommended to improve the efficiency of preoxygenation and reduce the likelihood of gastric contents reaching the oropharynx. Cricoid pressure is controversial. In the United Kingdom, thiopental remains the induction agent of choice, although there is scant evidence upon which to avoid propofol. In pre-eclampsia, it is essential to obtund the pressor response to laryngoscopy with remifentanil or alfentanil. Rocuronium is an acceptable alternative to succinylcholine for neuromuscular blockade. Sugammadex offers the possibility of swifter reversal of rocuronium than spontaneous recovery from succinylcholine. Management of difficult tracheal intubation is focused on ‘oxygenation without aspiration’ and prevention of airway trauma. The Classic™ laryngeal mask airway is the most commonly used rescue airway in the United Kingdom. There is a large set of data from fasted women of low body mass index who have undergone elective CD safely with a Proseal™ or Supreme™ laryngeal mask airway. Sevoflurane is the most popular volatile agent for maintenance of GA. The role of electroencephalography-based depth of anaesthesia monitors at CD remains to be established. Intraoperative end-tidal carbon dioxide tension should be maintained below 4.0 kPa.

Steadily rising federal debt—largely fueled by rising healthcare costs and an aging population that will boost spending on Social Security, Medicare, and Medicaid—will make it harder to grow the nation’s economy, boost living standards, respond to wars or recessions, address social needs, and maintain the US role as a global leader. At the same time, an increasingly fractured society has left many people behind and let critical investments lag, even as overall prosperity has grown. How and when US citizens address these challenges will help determine the future they build for themselves and their children. This book proposes a remedy with three core elements: controlling entitlement spending in ways that preserve and enhance the programs’ anti-poverty and social insurance roles; betting on the future by stipulating major new public investments in human and physical capital; and raising and reforming taxes to pay for government services fairly and efficiently. Together, these changes would control federal borrowing, strengthen the economy, increase opportunity, reduce inequality, and build better lives for current and future generations. There is no need to kill popular programs or starve government. Indeed, a primary goal of fiscal reform is to maintain and enhance the vital functions that government provides. The country needs to act responsibly, pay for the government it wants, and shape that government in ways that serve it best.

This chapter discusses key components of cardiovascular physiology applicable to clinical practice in the field of anaesthesiology. From theory development to ground-breaking innovations, the history of cardiac and vascular anatomy, as well as physiology, is presented. Utilizing knowledge of structure and function, parameters created have allowed adequate patient clinical assessment and guided interventions. A review of concepts reveals the impact of multiple physiological variables on a patient’s haemodynamic state and the need for more accurate and efficient measurements. In particular, it is noted that a more reliable index of ventricular contractility is the end-systolic elastance rather than the ejection fraction. Constant direct preload assessment has not yet been achieved but continues to be determined through surrogate variables, and continuous cardiac output monitoring for oxygen delivery, although advancing, has limitations. Considering the effect of compound factors perioperatively, especially heart failure, modifies the goals and interventions of anaesthetists to achieve improved outcomes. Therefore, medical management prior to surgery and complete assessment through history, physical examination, and diagnostic tests are a priority. This chapter also details the expectations following volume expansion to augment haemodynamics during surgery, the concept of functional haemodynamic monitoring, and limitations to the parameters applied in assessing fluid responsiveness. Challenging the accuracy of conventional indices to predict volume status led to the use of goal-directed therapy, reducing morbidity and minimizing length of hospital stay. The mainstay of this chapter is to reinforce the relevance of advances in haemodynamic monitoring and homeostasis optimization by anaesthetists during surgery, using fundamental concepts of cardiovascular physiology.

As we write these words, the COVID-19 pandemic has become part of our lives in a much more controlled way. For instance, some of our habits have changed and we are able to resume our activities in the way of a “new normal,” returning to social contact with family, friends, and colleagues. In returning to a life without the constraint of the virus at such a high level, the academy tries to resume its rituals, including scholarly events. Email boxes and physical boards at universities are once again filling up with calls for submission of abstracts for congresses, seminars, and workshops. As these events are happening again, academia seems to be reflecting on the pros and cons of onsite scientific events. While acknowledging the importance of such scientific events and their potential for strengthening scholarly communities and collaborations, many academics have begun questioning the real impact of being physically present. This questioning seems to be based on several factors. On the one hand, it is clear that universities have been increasingly devaluing academics’ presence in congresses (unless by invitation). They allocate less funding for these activities, especially for those academics who wish to attend an event without presentation. With no presentation, institutions devalue attendance in performance appraisal processes. Increasingly, academic institutions value publications (indexed, despite some positive movement seeking to counter the tyranny of the “publish or perish” motto), and an academics ability to raise funding. Yet, not all congresses are associated with publication processes in indexed journals or proceedings. Books of abstracts (once edited by any congress) are almost extinct, namely because of their devaluation by institutes of higher learning (and funding entities). On the other hand, the massive and necessary use of online scientific events in 2020 and 2021 allowed us to realize that it is possible, efficient, and effective to hold these events in a format different from the traditional one. The internet offers versatility and more and more congresses are now offered online or in hybrid formats. These formats allow academics to overcome financial and physical complications caused by in-person scholarly events. Academics can request less funding and, at the same time, mitigate concerns of acceptance without presentations, covering classes while away, or having to supplement university sponsorship with personal funds. At some universities, funding comes after attendance regardless of availability of those funds and academics are asked to pay registration fee, plane tickets, and lodging with the expectation of being reimbursed upon return. This is particularly challenging given the present economic situation around the globe. At the same time, while physically at the event and away from families, work continues to accumulate for academics. They then must wade through this excess upon returning home, adding to an already excessive workload. This makes maintaining a work-life balance challenging. We at New Trends in Qualitative Research (NTQR) believe it is particularly relevant to discuss this topic within the context of the release of NTQR Volume 11. NTQR is an indexed journal associated with international scientific events in the field of qualitative research - Congreso Ibero-Americano en Investigación Cualitativa (CIAIQ) and the World Conference on Qualitative Research (WCQR). Specifically, the volume that we edit here aggregates works that, having been originally presented at WCQR2022 (held in an online format), went through a double-blind review process. This volume, annually edited (as WCQR is an annual event), allows us, as editors, to condense a diverse set of qualitative research work, focusing on different topics, and with different methodological designs. And, our concern as editors has always been to assure the quality of the published works, namely through a careful review and editing process. We do not know if we are ready to give up our physical presence at scientific events. But, with opportunities such as online presentations and online publishing venues, we may now be much more judicious in this presence. We may now take time to ponder the relevance of investing in attending a scientific event, and selecting (hand-drawn) two or three events per year, at most. WCQR has a strong emphasis in the building of a scientific community (in this case, bonded by the interest in qualitative research), reconciling physical and online presence, and is associated with quality journals. Just as the economy or the production of material wealth is unimaginable without a man, so human existence and development are impossible without the wealth created in the economy. Shortly, both the goal and the means of achieving and realization of the economy are still the human resources. People have long ago noticed that it was the economy that created livelihoods, and the delays in their production led to the catastrophic events such as hunger, poverty, civil wars, social upheavals, revolutions, moral degeneration, and more. Therefore, the special interest of people in understanding the regulatory framework of the functioning of the economy has existed and exists in all historical epochs [A. Sisvadze. Economic theory. Part One. 2006y. p. 22]. The system of economic disciplines studies economy or economic activities of a society. All of them are based on science, which is currently called economic theory in the post-socialist space (the science of economics, the principles of economics or modern economics), and in most countries of the world - predominantly in the Greek-Latin manner - economics. The title of the present book is also Modern Economics. Economics (economic theory) is the science that studies the efficient use of limited resources to produce and distribute goods and services in order to satisfy as much as possible the unlimited needs and demands of the society. More simply, economics is the science of choice and how society manages its limited resources. This current book is the second complete updated edition of the challenges of the modern global economy in the context of the coronary crisis, taking into account some of the priority directions of the country's development. Its purpose is to help students and interested readers gain a thorough knowledge of economics and show them how this knowledge can be applied pragmatically (professionally) in professional activities or in everyday life. Given that the behavior of the economy reflects the behavior of the people who make up the economy, after clarifying the essence of the economy, we move on to the analysis of the four principles of individual decision-making. Furtermore, the book describes how people make independent decisions. The key to making an individual decision is that people have to choose from alternative options, that the value of any action is measured by the value of what must be given or what must be given up to get something, that the rational, smart people make decisions based on the comparison of the marginal costs and marginal returns (benefits), and that people behave accordingly to stimuli. Afterwards, the need for human interaction is then analyzed and substantiated. If a person is isolated, he will have to take care of his own food, clothes, shoes, his own house and so on. In the case of such a closed economy and universalization of labor, firstly, its productivity will be low and, secondly, it will be able to consume only what it produces. It is clear that human productivity will be higher and more profitable as a result of labor specialization and the opportunity to trade with others. Indeed, trade allows each person to specialize, to engage in the activities that are most successful, be it agriculture, sewing or construction, and to buy more diverse goods and services from others at a relatively lower price. The key to such human interactions is that trade is mutually beneficial; That markets are usually the good means of coordination between people and that the government can improve the results of market functioning if the market reveals weakness or the results of market functioning are not fair. Moroever, it also shows how the economy works as a whole. In particular, it is argued that productivity is a key determinant of living standards, that an increase in the money supply is a major source of inflation, and that one of the main impediments to avoiding inflation is the existence of an alternative between inflation and unemployment in the short term, that the inflation decrease causes the temporary decline in unemployement and vice versa. The Understanding creatively of all above mentioned issues, we think, will help the reader to develop market economy-appropriate thinking and rational economic-commercial-financial behaviors, to be more competitive in the domestic and international labor markets, and thus to ensure both their own prosperity and the functioning of the country's economy. How he/she copes with the tasks, it is up to the individual reader to decide. Because for this period the economic resources, especially non-productive, non-recoverable ones (oil, natural gas, coal, etc.) are becoming increasingly limited. According to the World Energy Council, there are currently 43 years of gas and oil reserves left in the world (see “New Commersant 2007 # 2, p. 16). Under such conditions, sustainable growth of real gross domestic product (GDP) and maximum satisfaction of uncertain needs should be achieved not through the use of more land, labor and capital (extensification), but through more efficient use of available resources (intensification) or innovative economy. And economics, as it was said, is the science of finding the ways about the more effective usage of the limited resources. At the same time, with the sustainable growth and development of the economy, the present needs must be met in a way that does not deprive future generations of the opportunity to meet their needs; 2. Why is the 21st century the age of knowledge? Because in a modern economy, it is not land (natural resources), labor and capital that is crucial, but knowledge. Modern production, its factors and products are not time-consuming and capital-intensive, but science-intensive, knowledge-intensive. The good example of this is a Japanese enterprise (firm) where the production process is going on but people are almost invisible, also, the result of such production (Japanese product) is a miniature or a sample of how to get the maximum result at the lowest cost; 3. Why is the 21st century the age of information? Because the efficient functioning of the modern economy, the effective organization of the material and personal factors of production largely depend on the right governance decision. The right governance decision requires prompt and accurate information. Gone are the days when the main means of transport was a sailing ship, the main form of data processing was pencil and paper, and the main means of transmitting information was sending letters through a postman on horseback. By the modern transport infrastructure (highways, railways, ships, regular domestic and international flights, oil and gas pipelines, etc.), the movement of goods, services and labor resoucres has been significantly accelerated, while through the modern means of communication (mobile phone, internet, other) the information is spreading rapidly globally, which seems to have "shrunk" the world and made it a single large country.
