Анотація:

Introduction: Currently, numerous ablation techniques are available for atrial fibrillation (AF), in addition to manual radio frequency ablation. The aim of this prospective, non-randomized concurrent controlled trial was to compare the mid-term efficacy and procedural outcomes of persistent AF (PerAF) using cryoballoon (CB) and robotic magnetic navigation (RMN). Methods: Two hundred PerAF patients were assigned, in a 1:1 ratio, to undergo catheter ablation using RMN (RMN group) or CB (CB group). The primary endpoint was freedom from AF recurrence following a 3-month period after the index ablation. The secondary endpoint was peri-procedural outcomes, including the total procedure time, left atrial procedure time, fluoroscopy time, and fluoroscopy dose. The Two-step cluster analysis was used to determine the efficacy of RMN and CB between the different groups. The Cox proportional hazard model and restricted cubic spline were used to determine predictors for AF recurrence. Results: At the mean follow-up of 28.1 ± 9.7 months, the primary endpoint was achieved in 71 PerAF patients in the RMN group and in 62 PerAF patients in the CB group (71% vs. 62%, p = 0.158). Compared with CB, RMN-guided ablation led to a longer procedure time (p < 0.001), but with less radiation (p < 0.001). Cluster analysis returned two clusters of patients and RMN was favorable for one cluster (p = 0.037), in which more patients presented with diabetes mellitus and smaller left atria. Conclusions: For patients with PerAF, CB is generally equivalent to RMN-guided ablation with regard to overall efficacy. RMN-guided ablation could be favorable in specific patient populations presenting with diabetes mellitus and smaller left atria.

Анотація:

Este trabalho teve por objetivo estabelecer as variações na atividade alelopática da substância química titonina, em função da acetilação de sua molécula. Bioensaios de germinação (25 ºC de temperatura constante e fotoperíodo de 12 horas) e de desenvolvimento da radícula e do hipocótilo (25 ºC de temperatura constante e fotoperíodo de 24 horas) foram desenvolvidos. Como planta receptora, utilizou-se a planta daninha Mimosa pudica (malícia). Análise de espectros RMN ¹H e 13C e técnicas de RMN bidimensionais foram realizadas na molécula acetilada. O processo de acetilação produziu a molécula 3'-acetil-7,4'-dimetoxiflavona, que diferiu da molécula original, identificada como 3'-hidroxi-7,4'-dimetoxiflavona. A estrutura da titonina-Ac foi confirmada pelos espectros de RMN ¹H, 13C, DEPT, COSY e HETCOR. A titonina foi acetilada com anidrido acético em piridina. A análise comparativa da atividade alelopática das duas substâncias revelou que titonina-Ac apresentou maior potencial para inibir tanto a germinação das sementes como o desenvolvimento da radícula e do hipocótilo da planta daninha malícia. A intensidade dos efeitos alelopáticos das duas substâncias esteve positivamente associada à concentração. O conjunto das informações obtidas permite sugerir a possibilidade de se aumentar a atividade alelopática de uma substância química sem comprometer suas peculiaridades biológicas desejáveis à natureza e aos interesses da sociedade.

Анотація:

Ciclopaladados diméricos do tipo [Pd(dmba)(m-X)]2 (dmba = N,N-dimetilbenzilamina; X = Cl, N3, NCO) reagem com tiouréia (tu), à temperatura ambiente, resultando espécies mononucleares do tipo [Pd(dmba)(X)(tu)]. Todos os compostos foram caracterizados por análise elementar, espectroscopias no IV e de RMN de ¹H e com base nessas técnicas as estruturas, desses compostos, foram propostas. Os espectros de absorção no infravermelho mostraram que a coordenação da tiouréia, ao paládio, ocorre através do átomo de enxofre. O anel ciclometalado foi confirmado através da espectroscopia de RMN de ¹H.

Анотація:

As iniciativas para consolidação de um arranjo institucional no contexto de implementação do Estatuto da Metrópole (EM) tiveram pouca eficácia desde a criação da Região Metropolitana de Natal (RMN). Desse modo, o presente estudo investiga as mudanças institucionais que o Fórum de Imersão Local, desenvolvido pela Rede de Pesquisa Observatório das Metrópoles (OM) - Núcleo Natal, promoveu ao viabilizar o debate acerca do tema. O objetivo do artigo é analisar como o Fórum de Imersão Local contribuiu para o debate acerca da gestão metropolitana na RMN e para a configuração do seu arranjo institucional calcado no EM. Metodologicamente, foram analisados os resultados dos quatro fóruns de imersão em municípios da Região Metropolitana, contando-se com a presença de gestores públicos e de representantes da sociedade civil. Os resultados do estudo demonstram que a atuação do OM Núcleo Natal refletiu positivamente na atuação governamental. No primeiro momento, a instituição de Planos Plurianuais (PPA’s) com ações metropolitanas por municípios da RMN é resultado de um esforço conjunto com o município de Natal, tendo em vista a ausência de coordenação por parte do governo do estado. Posteriormente, com a realização dos Fóruns de Imersão Local e articulação com o governo eleito em 2018, percebe-se a realização de atividades com a pauta metropolitana, como uma nova reativação do Conselho de Desenvolvimento Metropolitano de Natal (CDMN) e eventos para debater o tema.

Objective. The objective of this study was to investigate the impact of left atrial (LA) size for the ablation of atrial fibrillation (AF) using remote magnetic navigation (RMN). Methods. A total of 165 patients with AF who underwent catheter ablation using RMN were included. The patients were divided into two groups based on LA diameter. Eighty-three patients had small LA (diameter <40 mm; Group A), and 82 patients had a large LA (diameter ≥40 mm; Group B). Results. During mapping and ablation, X-ray time (37.0 (99.0) s vs. 12 (30.1) s, P<0.001) and X-ray dose (1.4 (2.7) gy·cm2 vs. 0.7 (2.1) gy·cm2, P=0.013) were significantly higher in Group A. No serious complications occurred in any of the patients. There was no statistical difference in the rate of first anatomical attempt of pulmonary vein isolation between the two groups (71.1% vs. 57.3%, P=0.065). However, compared with Group B, the rate of sinus rhythm was higher (77.1% vs. 58.5%, P<0.001) during the follow-up period. More patients in Group A required a sheath adjustment (47/83 vs. 21/82, P<0.001), presumably due to less magnets positioned outside of the sheath. In vitro experiments with the RMN catheter demonstrated that only one magnet exposed created the sheath affects which influenced the flexibility of the catheter. Conclusions. AF ablation using RMN is safe and effective in both small and large LA patients. Patients with small LA may pose a greater difficulty when using RMN which may be attributed to the fewer magnets beyond the sheath. As a result, the exposure of radiation was increased. This study found that having at least two magnets of the catheter positioned outside of the sheath can ensure an appropriate flexibility of the catheter.

BACKGROUND: The ChAdOx1 nCoV-19 vaccine against COVID-19 is a two-dose vaccine spread 3 to 4 weeks apart. This study aims to ascertain the antibody response to each dose with respect to age, previous infection status etc. METHODS: Baseline total COVID-19 antibody level was ascertained using Siemens SARS‑CoV‑2 Total Antibody assay in consenting health care workers before the first dose of vaccination. Adverse effects were noted in each individual and were monitored weekly for the total antibody titre following both doses. Descriptive statistical tests were used to analyse the changes in antibodies levels weekly after both doses. Association of previous COVID infection and age with antibody levels was assessed. RESULTS: Median (range) of age of the 30 study participants was 31.5 years.23% of the participants had a history of previous COVID-19 infection. Mild adverse events following immunisation were reported by 87% participants after first dose whereas only in 7% after second dose. Median baseline antibody titres were significantly higher among those with previous COVID infection as compared to previously uninfected individuals. Antibody titres increased consistently after first dose and showed a declining trend following the second dose in all participants and showed no significant association with previous COVID-19 infection or age. CONCLUSIONS: Antibody titre response was similar amongst the various age groups. Higher response in the previously infected individuals following first dose may make them ideal candidates for a single dose vaccine regimen. Individuals showing lower levels of neutralising antibodies can be ideal candidates for a booster dose.

Aims:To compare efficacy and medical costs of typital and atypical antipsychotics in treating schizophrenic patients.Methods:250 in patients in general hospital and psychiatric hospital were randomly allocated to one of the two groups, one treated with typital antipsychotics as chlorpromazine, haloperidol and so on with a mean daily dose of 500∼800 mg in chlorpromazine equivalents, and the other with atypical agents in cluding clozapine (mean daily dose of 400∼800mg ), risperidone of 6 mg, or olanzapine of 20 mg.Results:The total costs of the treatment were (3157±253) Yuan RMB for patients treated with typical agents in general hospital, and (3673±294) Yuan RMB for patients treated with typical agents in psychiatric hospital respectively. the costs for medicine were (774±903) Yuan RMB, (1061±903) Yuan RMB respectively. the costs in atypical group were(1120±390) Yuan RMB and (1637±325) Yuan RMB, and (108±157) Yuan RMB and (240±317) Yuan RMB respectively. the duration of patients treated with typical agents in hospital is longer than that with atypical agents.Conclusions:It indicates that atypical antipsychotics have similar efficacy to typical agents in treating schizophrenic patients with less costs.

Анотація:

A Maytenus ilicifolia, conhecida como espinheira-santa, é uma planta popular de uso diverso, como chá é empregada no combate a problemas de doenças gástricas e da diabetes, por exemplo. As plantas possuem em sua constituição polímeros naturais como polissacarídeos, politerpenoides, celulose e fibras. O estudo dos constituintes poliméricos nesse tipo de material tem importância fundamental tanto na identificação, ou seja, qualificação quanto na adulteração da planta. É sabido que podem ser encontrados diversos produtos rotulados como sendo originário da mesma planta, entretanto, muitas das vezes a origem da planta é similar, mas não a mesma, o que pode gerar algum desconforto ou não atividade biológica dos chás quando da ingestão destes. Na literatura não são encontrados muitos trabalhos que envolvam a caracterização comparativa dos constituintes poliméricos, como polissacarídeos e fibras presentes em amostras de origens distintas, para comprovação de adulteração destes produtos. Uma técnica que pode ser promissora para este tipo de estudo é a Ressonância Magnética Nuclear (RMN) em estado sólido, já que permite analisar a amostra in natura não sendo um método destrutivo ou invasivo. Esta espectroscopia analisa amostras em campos magnéticos de diferentes potências ou forças. Neste trabalho é apresentado um estudo de caracterização dos constituintes poliméricos majoritários presentes na M. ilicifolia. Três amostras foram estudadas: uma amostra denominada controle e duas comerciais. O estudo envolveu a determinação dos tempos de relaxação spin-rede (T1) do ¹H utilizando a técnica de campo cíclico rápido (FFC), numa gama de frequências que variou de 100 kHz a 10 MHz. Assim, o principal objetivo deste trabalho foi estudar a possibilidade de utilização da RMN de baixo campo magnético na elaboração de um método expedito de análise que permita realizar a caracterização estrutural, verificação de autenticidade e/ou avaliação de eventual adulteração nesse tipo de amostra. Pelos resultados obtidos foi possível diferenciar as duas amostras comerciais por comparação com a amostra controle.

Анотація:

The experiment was conducted on two years old guava at Research Field, Department of Horticulture, SHUATS, Allahabad, during the year 2016-2017. It was laid out in Randomized Block Design (R.B.D) with 4 replications and 11 treatments. The treatments included different levels of inorganic fertilizer and vermicompost. The maximum increase in plant height (155.71 cm), number of branches (25.23), canopy spread E-W direction (101.88 cm) and N-S direction (104.88 cm), was obtained with the application of T10 [50% RDN through chemical fertilizer +50% RDN through vermicompost (100:50:50 g NPK+1kg vermicompost)]. Maximum number of flowers/plant (18.25), fruit set (48.42%), fruit length (7.57 cm), fruit diameter (7.78 cm), volume of fruit (118cm3) and specific gravity (1.14 g/ml), were recorded with the application of T10 (50% RDN through chemical fertilizer +50% RDN through vermicompost), which was superior as compared to other treatments taken under present study. The maximum average fruit weight (128g) and number of fruits per plant (8.53) as well as significant highest yield per plant (959.14g) were obtained with the application of 50% RDN through chemical fertilizer+50% RDN through vermicompost (T10). The maximum total soluble solids (12.040Brix), ascorbic acid (180.89mg/100 g pulp) and minimum titratable acidity (0.51%) were recorded with the soil placement of 50% RDN through chemical fertilizer +50% RDN through vermicompost (T10), which was superior and economically viable as compare to other treatments with a maximum cost benefit (C:B) ratio of (1: 1.04).

Background: Chronic obstructive pulmonary disease (COPD) is a common preventable and treatable chronic respiratory diseases. The BODE, ADO, DOSE are the composite indices taking into consideration for prognostication. In addition, COPD assessment test (CAT) score is associated with quality of life, taken for assessing symptoms severity and for personalizing treatments. Objective: To investigate the CAT score in Thai COPD patients according to their BODE, ADO, and DOSE in the influence of clinical parameters and outcomes. Methods: Medical records were reviewed between January 2016 and December 2016. BODE, ADO and DOSE was calculated based on their individual components. Results: Total 125 consecutive COPD patients were recruited. There was difference between CAT score among the GOLD stages classified (P = .02). There was significant difference of CAT between 4 BODE quartiles in cohort (P = .001).Thai COPD patients had the higher ADO index exhibit the higher CAT score than those with the lower ADO index (P = .001). There was no difference in CAT score between COPD with the higher and the lower DOSE index (P = .05). Conclusions: The higher CAT score found in Thai COPD patients with the higher composite indices of BODE and ADO. However, there was no difference in the DOSE index.

In the midst of the global pandemic of COVID-19 and its significant morbidity and mortality reported across the world due to severe acute respiratory distress syndrome (SARS), it has always been posing a new set of complications each passing day. As we are still in the process of understanding about the complications related to COVID-19, we are encountered with complications related to immunization for COVID-19. We are reporting a case of facial onset Guillain-Barré syndrome (GBS) in the patient who received first dose of COVISHIELD vaccine a couple of weeks prior to the onset of his illness.

OBJETIVO: Avaliar a incidência e as características das lesões tipo SCIWORA nos indivíduos até os 16 anos, da área de um hospital central entre 1989 e 2009, após traumatismo cervical. MÉTODOS: Estudo retrospectivo de consulta processual. RESULTADOS: Nove (10,5%) de 86 crianças apresentavam achados clínicos e radiológicos compatíveis com o diagnóstico de SCIWORA. A média de idades foi de 10,7 anos. A causa mais frequente foi o acidente de trânsito. Seis indivíduos eram classificáveis como Frankel D. Os restantes três casos eram Frankel C. Em três doentes a RMN mostrou imagem de lesão. Sete efetuaram metilprednisolona endovenosa e todos mantiveram imobilização com colar cervical até a primeira consulta de seguimento, às 2 semanas. Na alta, os seis doentes que apresentavam Frankel D à entrada melhoraram para um grau E. Dos doentes com Frankel C à entrada, um melhorou até Frankel D e os restantes dois mantiveram-se inalterados em Frankel C. CONCLUSÃO: Em um hospital de referência traumatológica, SCIWORA representa cerca de 10% das lesões cervicais pediátricas. Os défices neurológicos à entrada e a RMN têm valor prognóstico de recuperação. A corticoterapia em dose elevada não está formalmente indicada e não é consensual o tempo de utilização de imobilização ou a sua indicação em todos os SCIWORA.

Background: Dyspnea is considered as one of the most frequent and distressing symptoms in patients with advanced cancer, and systemic administration of morphine has been reported to have beneficial effect on this complaint. Aim and design: The effect of red morphine drops (RMD) given orally and kept in the mouth as long as possible before swallowing to alleviate dyspnea was compared to the effect of morphine given subcutaneously (SCM) in a double-blind, double-dummy trial.Participants: Twenty consecutive terminally ill patients with primary lung cancer or lung metastases admitted to Sankt Lukas Hospice were included in the study. All patients had dyspnea at rest, and they received either 1/12 of their 24-hour morphine dose as RMD or 60 percent of this dose as SCM. Perceived breathlessness was evaluated on a Visual Analogue Scale (VAS) during a 60 minutes observation time, and pulse rate, respiratory rate and oxygen saturation were assessed as well.Results: Compared to baseline both RMD and SCM showed a small, but statistical significant effect on VAS (mean decrease of 1.1 [RMD] and 1.7 [SCM]) and pulse rate (mean decrease of 4 per minute [RMD] and 6 per minute [SCM]), the effect being significantly larger after SCM.Conclusions: The study confirms the beneficial effect of morphine on dyspnea in terminally ill patients, and we conclude that RMD is an attractive alternative to injected morphine, when self-administration is preferable.

Aim: To investigate the integrated use of chemical fertilizer and potential tasar silkworm (Antheraea mylitta) excreta as fertilizer on the growth, yield and quality of rice.. Methodology: A pot experiment was designed in Completely Randomized Design with four replicates with nine treatments, where Terminalia arjuna leaf litter, tasar silkworm excreta and recommended dose of chemical nitrogen fertilizer at 25%, 50% or 100% alone or in combinations was applied in transplanted rice plants. The observations were recorded for different growth parameters at 30, 60 and 90 days interval, while yield traits were recorded at the time of crop harvest. Results: Leaf litter and tasar silkworm (Antheraea mylitta Drury) excreta on growth, yield and quality of rice revealed that application of 100% recommended dose of nitrogen (RDN) from tasar silkworm excreta recorded significantly higher plant height, panicle length, number of grains panicle-1 and 1000 grain weight at rice harvest. While, chlorophyll index, grain yield, number of effective tillers per hill, biological and protein yield of rice recorded significantly higher in 75% recommended dose of nitrogen through chemical nitrogen fertilizer + 25% recommended dose of nitrogen from tasar silkworm excreta. Nitrogen and protein content of grain recorded statistically higher in 50% recommended dose of nitrogen through inorganic fertilizer + 50% RDN from tasar silkworm excreta. Grain yield was significantly correlated with nitrogen uptake and protein yield, plant height, effective tillers per hill, 1000 grain weight, straw and biological yield. Interpretation: Integrated use of 75% RDN through chemical nitrogen fertilizer with 25% RDN from tasar silkworm excreta is suitable for higher production that may reduce the external input of chemical nitrogen fertilizers and promote zero waste management.

A obtenção de óleos essenciais de espécies nativas no Brasil ainda é pequena, apesar das suas florestas apresentarem grande potencial para a exploração desses produtos. No caso do Cerrado, até agora apenas algumas espécies foram estudadas quanto à presença de óleos essenciais, sendo necessárias mais pesquisas para verificar o potencial desse importante Bioma. Objetivou-se, nesta pesquisa, avaliar o potencial de plantas arbóreas e arbustivas do Cerrado para a produção de óleos essenciais, determinando o rendimento e a composição química preliminar de óleos de algumas espécies do Bioma. Foram selecionadas em campo as espécies Baccharis sp. (assa-peixe), Blepharocalyx salicifolius (maria preta), Psidium myrsinites (araçá do cerrado) e Protium ovatum (almecega). Essa seleção foi feita com base em estudos prévios sobre famílias botânicas produtoras de óleos essenciais, junto com a constatação da presença de odores nas folhas maceradas das plantas. A extração dos óleos foi feita com um minidestilador por arraste de vapor utilizando as folhas das plantas no processo. Para analisar quimicamente cada óleo, primeiramente, foi realizada a separação das fases orgânica e aquosa dos hidrolatos com o solvente acetato de etila. Posteriormente, foram feitas análises cromatográficas (cromatografia em camada delgada, CCD) e espectroscópicas de Ressonância Magnética Nuclear (RMN) de ¹H e ¹³C e Infravermelho (IV). Foram obtidos rendimentos de 0,06% para Baccharis sp., 0,12% para Blepharocalyx salicifolius, 0,10% para Protium ovatum e 0,13% para Psidium myrsinites. Nas análises espectroscópicas foi observado que esses óleos são misturas complexas de hidrocarbonetos saturados (grupos alifáticos) e insaturados (olefinas ou alquenos) e oxigenados (aldeídos, cetonas, ésteres, dentre outros). As espécies Psidium myrsinites e Blepharocalyx salicifolius apresentaram sinais característicos de carbonilas de cetonas. A primeira apresentou o componente linalol, enquanto a segunda apresentou forte odor característico. Pelos resultados mostrou-se o potencial das plantas do Cerrado como produtoras de óleos essenciais. Com a análise química, observou-se que esses óleos essenciais são constituídos por complexas misturas que devem ser melhor estudadas para avaliar, detalhadamente, a composição de cada óleo.

Emergence of novel variants of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) underscores the need for next-generation vaccines able to elicit broad and durable immunity. Here we report the evaluation of a ferritin nanoparticle vaccine displaying the receptor-binding domain of the SARS-CoV-2 spike protein (RFN) adjuvanted with Army Liposomal Formulation QS-21 (ALFQ). RFN vaccination of macaques using a two-dose regimen resulted in robust, predominantly Th1 CD4+ T cell responses and reciprocal peak mean serum neutralizing antibody titers of 14,000 to 21,000. Rapid control of viral replication was achieved in the upper and lower airways of animals after high-dose SARS-CoV-2 respiratory challenge, with undetectable replication within 4 d in seven of eight animals receiving 50 µg of RFN. Cross-neutralization activity against SARS-CoV-2 variant B.1.351 decreased only approximately twofold relative to WA1/2020. In addition, neutralizing, effector antibody and cellular responses targeted the heterotypic SARS-CoV-1, highlighting the broad immunogenicity of RFN-ALFQ for SARS-CoV−like Sarbecovirus vaccine development.

ABSTRACT The pharmacokinetics, safety, and tolerability of posaconazole, an investigational triazole antifungal, were evaluated following the administration of rising single and multiple oral doses. A total of 103 healthy adults were enrolled in two phase I trials. Each study had a double-blind, placebo-controlled, parallel-group design with a rising single-dose (RSD) or rising multiple-dose (RMD) scheme. In the RSD study, subjects received single doses of posaconazole oral tablets (50 to 1,200 mg) or placebo. In the RMD study, subjects received posaconazole oral tablets (50 to 400 mg) or placebo twice daily for 14 days. By using model-independent methods, the area under the plasma concentration-time curve and the maximum concentration in plasma were determined and used to assess dose proportionality. In the RSD study, the levels of posaconazole in plasma increased proportionally between the 50- and 800-mg dose range, with saturation of absorption occurring above 800 mg. Dose proportionality was also observed in the RMD study. In both studies, the apparent volume of distribution was large (range, 343 to 1,341 liters) and the terminal-phase half-life was long (range, 25 to 31 h). Posaconazole was well tolerated at all dose levels, and the adverse events were not dose dependent. No clinically significant changes in clinical laboratory test values or electrocardiograms were observed. Following the administration of single and twice-daily rising doses, the level of posaconazole exposure increased in a dose-proportional manner. The long elimination-phase half-life of posaconazole supports once- or twice-daily dosing in clinical trials; however, additional studies are required to determine if further division of the dose will enhance exposure.

AbstractResearch was conducted from 2015 to 2017 to investigate the potential for 2,4-D and multiple herbicide resistance in a waterhemp [Amaranthus tuberculatus(Moq.) J. D. Sauer] population from Missouri (designated MO-Ren). In the field, visual control of the MO-Ren population with 0.56 to 4.48 kg 2,4-D ha−1ranged from 26% to 77% in 2015 and from 15% to 55% in 2016. The MO-Ren population was highly resistant to chlorimuron, with visual control never exceeding 7% either year. Estimates of the 2,4-D dose required to provide 50% visual control (I50) of the MO-Ren population were 1.44 kg ha−1compared with only 0.47 kg 2,4-D ha−1for the susceptible population. Based on comparisons to a susceptible population in dose–response experiments, the MO-Ren population was approximately 3-fold resistant to 2,4-D, and 7-, 7-, 22-, and 14-fold resistant to atrazine, fomesafen, glyphosate, and mesotrione, respectively. Dicamba and glufosinate were the only two herbicides that provided effective control of the MO-Ren population in these experiments. Examinations of multiple herbicide resistance at the individual plant level revealed that 16% of the plants of the MO-Ren population contained genes stacked for six-way herbicide resistance, and only 1% of plants were classified as resistant to a single herbicide (glyphosate). Results from these experiments confirm that the MO-RenA. tuberculatuspopulation is resistant to 2,4-D, atrazine, chlorimuron, fomesafen, glyphosate, and mesotrione, making this population the third 2,4-D–resistantA. tuberculatuspopulation identified in the United States, and the first population resistant to six different herbicidal modes of action.

Bone metastasis is a severe complication of malignant tumors. Management of multiple bone metastases remains difficult and prognosis is generally unfavorable, radiotherapy being often the therapeutic option. The main goals of the palliative irradiation of bone metastases are the reduction of pain intensity and the decreasing the analgesic dose used. Overall survival of patients with oligometastatic disease for hormone dependent cancers like breast and prostate may exceed three years and for these groups of patients the reduction of the associated tardive toxicity after irradiation is essential for preserving the quality of life. We present the case of a patient diagnosed in March 2011 with prostate cancer, multiple bone metastases in November 2012. In December 2012, palliative radiotherapy was administrated in a total dose of 21Gy/ fractions, weekly in the lumbosacral region. Seven years after the first presented an increase in pain intensity in the left ischio-pubian branch and pubic symphysis and the bone scintigraphy reveal the progression of the metastatic disease. Palliative re-irradiation was administrated in total dose of 20Gy/5fractions in painful region. Re-irradiation for bone metastasis remains a therapeutic option with the potential to reduce the pain. Image guided radiotherapy can reduce the risk of late toxicity, especially for long-term survivors.

This study investigated the effects of incorporating a mixture of fructooligosaccharide (FOS) and resistant maltodextrin (RMD) at a ratio of 1:2 on body fat accumulation and fecal bacterial parameters in rats. Our results indicated that high dietary fat consumption might effectively (p < 0.05) increase body fat, but consequently inducing a significantly (p < 0.05) higher growth of C. perfringens and retarded growth (p < 0.05) of the Bifidobacterium spp. in the large intestine. As compared with the high fat control, an incorporation of the FOS and RMD mixture at a high dose (0.97 and 1.94 g/kg body weight, respectively) could result in a significant (p < 0.05) reduction in feed efficiency (−16%), total visceral fat (−17.4%), non-visceral fat levels (−20.3%), and total body fat (−19.2%). Furthermore, feeding the FOS and RMD mixture at a high dose was capable to counter the above undesirable impacts by reducing the C. perfringens count (−14.8%) and increasing the total Bifidobacterium count (134.4%) and total fecal short chain fatty acids (195.4%). A supplementation of adequate amount of FOS and RMD might confer a concreted solution to the obesity and deteriorated fecal bacteria profiles due to high fat consumption.

Três esteroides (espinasterol, espinasterona e glicopiranosil espinasterol) foram isolados do caule de Moutabea guianensis, planta de origem amazônica. Suas estruturas foram determinadas a partir das análises de RMN e por comparação com dados espectroscópicos da literatura. Foram avaliadas as atividades alelopáticas das três substâncias em duas plantas invasoras de pastagens comuns da região amazônica: Mimosa pudica (malícia) e Senna obtusifolia (mata-pasto). A substância glicopiranosil espinasterol foi mais ativa no ensaio de desenvolvimento da radícula, inibindo a espécie Senna obtusifolia em 75%; o espinasterol inibiu em 22% o desenvolvimento do hipocótilo de Senna obtusifolia; e no bioensaio de germinação de sementes as substâncias espinasterol e espinasterona proporcionaram 10% de inibição em Mimosa pudica. A partir desses resultados, observou-se que a diferença de substituintes observada na posição-3 dos esteroides é capaz de alterar a atividade alelopática. O grupo glicosil na posição-3 elevou a atividade alelopática de forma mais expressiva no bioensaio de desenvolvimento de radícula da espécie malícia, quando comparado aos esteroides hidroxilado e carbonilado. Este é o primeiro estudo químico e alelopático do gênero Moutabea.

Анотація:

Aim: This study investigated the safety and clinical outcomes of expanded allogeneic human adipose-derived mesenchymal progenitor cells injected into patients with symptomatic, bilateral knee osteoarthritis. Design: In this single-site, randomized, double-blind, dose-ranging, Phase I study, patients were randomized to three treatment groups (low dose, 1 × 107 cells; medium dose, 2 × 107 cells; high dose, 5 × 107 cells). All patients received two bilateral intra-articular injections: week 0 (baseline) and week 3. The primary end point was adverse events within 48 weeks. Secondary end points were measured with Western Ontario and McMaster Universities Osteoarthritis index, visual analog scale, short form-36 at weeks 12, 24 and 48. Quantitative MRI measurements of cartilage volume were compared from baseline and week 48. Results: A total of 22 subjects were enrolled of which 19 (86%) completed the study. Adverse events were transient, including mild to moderate pain and swelling of injection site. Improvements from baseline were measured in the secondary end points. MRI assessments showed slight improvements in the low-dose group. Conclusion: Safety and improvements in pain and function after intra-articular injections of allogeneic human adipose-derived mesenchymal progenitor cells into arthritic patients was demonstrated.

Renal medullary carcinoma (RMC) is a rare but aggressive malignancy affecting young individuals with sickle cell trait. Renal medullary carcinoma commonly presents with advanced or metastatic disease and is associated with a rapidly progressive clinical course and an extremely short overall survival measured in weeks to few months. Due to the rarity of RMC, there is no proven effective therapy and patients are often treated with platinum-based chemotherapy. We report near-complete radiological and pathological response in a patient treated with dose-dense MVAC (methotrexate, vinblastine, doxorubicin, and cisplatin) chemotherapy. The patient underwent consolidation nephrectomy and retroperitoneal lymph node dissection and had a 16-month progression-free survival, one of the longest reported in patients with RMC.

Анотація:

Background: Combined recombinant human erythropoietin (rHuEPO) and iron therapy significantly reduced postoperative blood loss (PBL) and allogeneic blood transfusion (ABT) in elderly hip fracture (HF) patients. However, the minimum effective rHuEPO dosage and route had not been studied before. Objective: To evaluate the efficacy of single low dose preoperative intravenous rHuEPO on PBL and ABT in elderly HF. Methods: A randomized controlled trial (RCT) in 32 elderly HF underwent surgical intervention was conducted. The patients were randomly assigned to receive a single dose of 10 000 IU rHuEPO (EPO group, n = 16) or placebo (control group, n = 16) on admission. All patients were given 200-mg iron sucrose intravenously for 3 days after admission. Perioperative data, outcome related to PBL, ABT, rHuEPO adverse effects, and functional outcome during 1-year period were collected and analyzed. Results: There was no significant difference in demographic data, postoperative complication, and functional outcome between both groups (P < .05). Total hemoglobin loss (THL) and the number of patients receiving ABT in EPO group (2.1 ± 1.0 g/dL and 12 patients) did not significant differ from those in control group (2.2 ± 0.8 g/dL and 10 patients) (P = .81 and P = .44, respectively). However, EPO group demonstrated a nonsignificant greater in hemoglobin recovery (P = .07) and increase in reticulocyte count (P = .10). Conclusion: Combined single low dose preoperative intravenous rHuEPO with iron therapy does not significantly reduce PBL and ABT in elderly HF compared to who received intravenous iron therapy alone. However, this adjunct rHuEPO may hasten the hemoglobin recovery and helpful for the patients’ outcome.
