Nowadays, taking good care of the right patient at the right time in the right place according to the latest guidelines established by professional societies is the main concern of each health care provider, aiming to improve populations’ state of health. From the example of the innovative and accelerated care pathway “Oneday diagnosis” for hepatobiliary and pancreatic diseases taking place in Strasbourg university hospital and the comparative study to ordinary pathway with respectively 330 vs 152 patients, we demonstrated its efficiency and sustainability. We confirmed the possible replication elsewhere and for other pathologies. Furthermore, the application of the VBHC concept through standardized questionnaires for liver and pancreatic cancers would allow to collect outcomes that matter to patients PROMs and clinical outcomes CROMs with the objective of reaching relevant and excellent care

An analysis of state policy on community facilities. With the unprecedented demand for new secondary school building that emerged in 1960, public institutions had to react. The right to build schools lay with the local authorities, but most of them in fact relinquished that right to the state. Once the state became the contracting authority for all these projects, and therefore a developer on a large scale, it devised a very specific approach called " the industrialized system ", to optimize the use of human and financial resources by rationalizing and standardizing construction procedures and techniques. The system operated as a "cludb", with overall control at the centre (the national education ministry's division for community facilities -the "direction des equipements") and implementation at local level (the responsibility of prefects and each "departement"-level division for community facilities"-direction departementale de l'equipement). The national authorities, as leaders of this club, promoted a technocracy and institutional expertise in which "technique" prevailed over "concept" and the rational outweighed creativity and imagination. The resulting "functionalism" led to a monotony of architectural form and urban design. Because it enjoyed the consensus of the parties involved, the system was to last for twenty years

Numerous software systems must execute constantly without being interrupted. On the other hand, they also have to be regularly adapted to changes in their operational domains and to the evolution of users requirements. Dynamic software update is the process that consists in replacing a running program with a newer version without interrupting the program operation. This thesis divides the dynamic evolution problem in two sub-problems. The essential difficulty is to ensure that the resulting executions will be globally coherent across time, such that updated processes actually perform their task without producing any error. The accidental difficulty is caused by the impossibility to define and understand dynamic software updates in the existing programming languages. To address these problems, a language model is presented. It gives the ability to express the dynamic evolutions of programs and to foresee the behaviors produced by dynamically updated processes. This language model introduce a code entity - the stage - associated with a high level control operator - reach - that allows programmers to express the temporal stucture of programs. The program decomposition in stages offers a high level view of the possible executions chronologies and gives programmers control points from where the entire future of computations can be redefined

In the context of significant mutations of employment and employment statuses, transformations of family structures and the appearance of new socio - economic risks, the objective of this thesis is to study trajectories out of poverty and the factors which can affect them, in particular the economic, social and territorial en vironment of individuals and households living in urban areas. This research also aims to explain the role the households’ individual and family trajectories play in theses processes. It endeavours to renew the question of routes out of poverty by using a multidimensional, dynamic and contextualised approach in reference to a local territory. Quantitative and qualitative methods are mobilised in a complementary way. The diversity of experiences of poverty is highlighted. The thesis shows that getting out of poverty depends on a combination of factors, and in particular, on a return to stable work, and fa mily and institutional support. However, the existence of barriers to employment and the breakdown of households’ family support explain the professional pre carity and economic instability in which they find themselves. Four obstacles to getting out of poverty were identified : single parenthood, illness, little or no participation in productive activity and social isolation. The practices developed by househ olds to cope with difficulties in access to consumption and to maintain or improve their economic situation were also studied

One of the problems raised by the practice of psychiatry in Morocco is that of interpenetration in a single theoretical corpus consisting of clinical experience and an ethnological view of mental disorders. This twin approach constitutes the methodological basis of the present research and thesis. While working as a psychologist at the psychiatric hospital in Rabat-Sale, the author decided to investigate the various therapy techniques used in "traditional" psychiatry, a subject with which he had not previously been familar. His first step was to isolate certain pathognomic signs specific to possession as a psychic disorder with a view to distinguishing possession from hysteria, a nevrosis which is badly defined and inappropriately diagnosed in psychiatry. In the second part of this study he applied the ethnological information which he had acquired while treating hospitalized patients in psychology department at Rabat-Sale

Movie stars came into being at the end of the 1900s and the beginning of the 1910s, but it is notuntil the end of the First World War that the star system became a reality in France, thanks to the creation of the first fan magazines. Regarding the economic, social and cultural aspects of the phenomenon, which covers every area of the cinema (from production to reception), this thesis studies how the French star system was established between 1919 and the Fall of France in June1940. At the beginning of the period the American movies and their stars dominate in France; instark contrast, the difficulties of French cinema are related to the weakness of its stars – its“vedettes” as the French say. Twenty years later, French movie stars have become unquestionably popular and they play a key part in the film industry, although they are involved in a system very different from the Hollywood one, which still fascinates French people. In the middle of this period, there occurred the transition to sound, an evolution whose impact on thestar system this thesis tries to determine precisely. Thus, this research in cultural history compares the way the French system and its rivals are seen, and it analyses both discourses and practices in order to explain the representations and the issues related to the movie star system in France during the period

Defl2 is an expert system designed for the medium term analysis of a company. A preliminary study has demonstrated that it is necessary to put aside the obligation to settle all propositions on the basis of true or false. The original aspect of this work consists in a particular approach of artificial reasoning in the area of company management. This involves the use of the theory of fuzzy sub-groups and the theory of the possibilities concerning the representation of knowledge and the application of gradual reasoning. The rules, which include deductive knowledge, are written to make use of linguistic analysis, and process imprecise and uncertain information. The meta-rules include procedural knowledge and ensure the piloting of the system, by controlling the comparison of fuzzy data; the optimal use of all the rules regrouped into specific modules is achieved by means of repetition

Today's communications networks are widely used to connect the resources, enabling the remote data transmission, reducing the cabling complexity, minimizing costs and providing easy maintenance. Because of these advantages, the networks have been introduced in automatic systems during recent decades and new industrial network protocols were also developed for the remote control. The systems controlled by networks, the term "Networked Control Systems (NCS)" are automatic traditional systems where the actuators, the sensors, the controllers and other components are distributed around a communication network that can be shared or not with other applications. The data of both control and diagnostic are exchanged between system components (sensor, controller and actuator) via the shared network. This new architecture of control system introduces new problems in terms of variable delay affecting the transmission, the packet loss, etc. With the aim to maintain good performance of diagnostic module face of possible changes introduced by the network, it is interesting to introduce new approaches. We have reported results for the optimal estimation techniques based on Kalman filter, thus creating a report as complete as possible, treating the residual generation and fault isolation in NCS. Our contribution consists, firstly, to develop a state space model of a system controlled via a network. Secondly, we propose a residual generator based on the delay models induced by the network. Finally, we develop a isolation filter in order to directly identify in the actuators faults in a SCR

The main interrogation of this thesis is: "How to describe cognitive mechanisms of the physician's medical diagnosis in the complex situation of an emergency department? ". To answer this question, we test a model suggested by Raufaste (2001) called "the model of the three values of rationality" describing the cognitive mechanisms during the diagnosis. The first empirical study is a based on diagnostic errors that occurred in Toulouse hospitals. This test appears conclusive because it shows the existence of these three values. The two last studies carry out more detailed investigation of one among five values, "flexibility". This value is defined as the capacity of the subject to change its representation taking into account new information. We show that information can thus be transformed, modified and adjusted according to its coherence with other information of the representation but also according to the nature of the activity which the physician achieves

Episodic memory makes it possible to mentally travel back in our own past; it breaks the law of theunidirectionality of time, and allows us to mentally relive past events. The main goal of this work wasto investigate whether episodic memory and the subjective feeling of reliving a past event (i.e.,autonoetic consciousness) arise from the fluency of the reconstruction process of the event. Thisreconstruction would involve the process of egocentric spatial updating with self-motion. Thishypothesis was first developed in Gomez and colleagues' model (Gomez, Rousset, & Baciu, 2009) thatsuggests that egocentric updating re-instances an egocentric spatial point of view on the rememberedevent. The present work brings additional behavioral evidences to this model by assessing both theconception of attributional episodic memory and the functional link between online egocentricupdating with self-motion and episodic memory. A first set of studies showed that enhancingartificially the fluency of the egocentric updating process, during the recognition phase, increasedautonoetic consciousness. Moreover, such phenomenon only happened when recognition involved areconstruction process. A second set of studies showed that performing an online egocentric updatingwith self-motion interfered with remembering. Although the updating of its own position though selfmotionis automatic, it specifically slows down source recall. Finally, a last study showed adissociation between preservations of and deficits of egocentric spatial updating abilities in patientssuffering from Alzheimer's disease. The results of these studies are discussed in the context of Gomezand colleagues' model, and in terms of embodied and situated cognition

The ACCES psychotherapy brings to light a form of thought known as ‘original thought’ which would seem to have its source prior to any personal history. We advance the hypothesis of the existence of a process of universal thought, and drawing on the record of sessions with clients we may describe its diverse properties. It would seem to be analogic, outside time, and fractal. The content analysis of utterances of a client (according to the associative logic of the Wordmapper programme) allows the identification of the passage from fractal thought to causal thought. We show that this thought seems to be a major vital energy, beyond any physical energy, prior to matter and creative, notably of awareness of one’s self. To offer an explanatory framework we propose the model of Energetic Identity Consciousness and thus find links between this reflexion founded in a field of psychology with a number of theories from physics and life sciences. We have reconceptualised the question of death in relation to the awareness of the self
