Повний текст джерелаIn the overseas departments, the prevalence of diabetes is high and has been for several years, a situation considered worrying by the national and regional authorities. In Martinique, the health observatory estimates the number of people affected by diabetes at around 40 000, for a population of 368 783 inhabitants in 2018 according to INSEE. The dietary transition reflects a shift towards a rich country lifestyle. Therapeutic patient education (TPE), which has been legislated in France since 2009 and aims to help people with a chronic disease to live with it on a daily basis, represents an important issue for Martinique. Inscribed in regional cultures and languages, our doctoral research seeks to understand the conditions for the development of therapeutic patient education (TPE) in a Creole context by questioning, in particular, the cultural competence of caregivers and other actors. Inspired by the "learning nests" deployed in Reunion Island and in several African countries, our interventional research questions what happens during learning situations proposed in the homes of diabetics in Martinique. Conducted in their ordinary context, the interviews and TVE workshops focus on the diabetic patients' and their carers' dietary balance. The initial results show the need to adapt food and nutrition education messages to the ordinary and cultural context of these people. They can thus better understand the information, transform it into knowledge and then use it in their daily practice to balance their diabetes. To this end, the cultural competence of the educator or trainer is necessarily called upon and mobilised. The adaptation to the ordinary context of people with diabetes can now be considered as a perspective for the deployment of a "STEP" learning situation - (Ecological, Territorialized, Adapted, Personalized and Efficient) with a view to the development of TVE in this territory

BECAUSE OF THE ASSOCIATION OF SOME GENETIC POLYMORPHISMS AND PATHOlOGIES, WE WONDERED IF SOME GENETIC POLYMORPHISMS COULD PREDICT COMPLICATIONS IN SICKLE CELL PATIENTS AND TYPE 2 DIABETIC WE INVESTIGATED POLYMORPHISM OF GENES INVOLVED IN THE IMMUNE SYSTEM (MYELOPEROXYDASE, HLA, TNF ALPHA, IFN GAMMA, IL6 AND IL10) AS MODULATORS OF INFECTIOUS COMPLICATIONS IN SCD. WE ALSO TRIED TO FIND AN ASSOCIATION BETWEEN FABP2 POLYMORPHISM, METABOLIC SYNDROME AND CARDIOVASCULAR COMPLICATIONS IN T2D SUBJECTS. OUR RESULTS REVEALED AN ASSOCIATION BETWEEN MYELOPEROXIDASE POLYMORPHISM AND INFECTIOUS COMPLICATIONS IN SICKLE CELL PATIENTS JUST AS IFNr POLYMORPHISM. ON THE OTHER HAND, HLA-DRB1*11 COULD HAVE A PROTECTIVE EFFECT AGAINST INFECTIONS. MOREOVER, OUR RESULTS HIGHLIGHTED A PREDOMINANT COMBINATION OF DYSLlPIDEMIA AND OBESITY IN T2D PATIENTS CARRYING THE THREONINE VARIANT OF FABP2

Повний текст джерелаWe have performed an ethnobotanical survey in Martinique on the common name “bwa kaka”. During this survey, we observed that the younger generation interviewed knew more about the uses of medicinal plants than the older generation. Moreover, the suggested uses for this plant were significantly different and depended on the gender of the person interviewed. Men indicated that “bwa kaka” was mainly used for diseases of the digestive system whereas women indicated that the main use was linked to magicoreligious rites. The ethnobotany surveys published up until now in Martinique did not investigate this result based on gender. The use of “bwa kaka”, which was a plant in great demand in the past, seems to have declined nowadays. This could be explained by its link to magico-religious rites, a practice which has practically disappeared in modern Martinique society. Six species are locally called “bwa kaka”. We have documented the therapeutic uses associated with this plant. However, we have chosen to evaluate the therapeutic potential of Solanum triste for a use that has only been associated with this species, namely the treatment of urinary lithiasis. This plant species was first used in a phytochemical study which made it possible to identify several compounds such as phenolic acids, flavonoids and glycoalkaloids by LC-DAD-ESI-MS2. The complete structure of the predominant glycoalkaloid has been obtained, i.e. 22S, 25S 3-beta-spirosol-5-ene -O- α -L-rhamnopyranosyl (1→2)beta-Dxylopyranosyl-O(1→3)alpha-L-rhamnopyranosyl O(1→4) glucopyranose. In order to evaluate the antilithiasis capacity of an aqueous extract of S. triste, three in vitro tests were performed: the evaluation of its anti-oxydant capacity, its action on the crystallization of oxalate, and on urinary stones already formed. The most significant result is the in vitro preventive capacity of extracts of S. triste on urinary lithiasis, a result which has never been previously demonstrated for this species and which needs to be confirmed in subsequent experiments
