Abstract. The values and distribution patterns of the strontium (Sr) isotope ratio 87Sr/86Sr in Earth surface materials are of use in the geological, environmental, and social sciences. Ultimately, the 87Sr/86Sr ratios of soils and everything that lives in and on them are inherited from the rocks that are the parent materials of the soil's components. In Australia, there are few large-scale surveys of 87Sr/86Sr available, and here we report on a new, low-density dataset using 112 catchment outlet (floodplain) sediment samples covering 529 000 km2 of inland southeastern Australia (South Australia, New South Wales, Victoria). The coarse (<2 mm) fraction of bottom sediment samples (depth ∼ 0.6–0.8 m) from the National Geochemical Survey of Australia were milled and fully digested before Sr separation by chromatography and 87Sr/86Sr determination by multicollector-inductively coupled plasma mass spectrometry. The results show a wide range of 87Sr/86Sr values from a minimum of 0.7089 to a maximum of 0.7511 (range 0.0422). The median 87Sr/86Sr (± median absolute deviation) is 0.7199 (± 0.0071), and the mean (± standard deviation) is 0.7220 (± 0.0106). The spatial patterns of the Sr isoscape observed are described and attributed to various geological sources and processes. Of note are the elevated (radiogenic) values (≥∼ 0.7270; top quartile) contributed by (1) the Palaeozoic sedimentary country rock and (mostly felsic) igneous intrusions of the Lachlan geological region to the east of the study area; (2) the Palaeoproterozoic metamorphic rocks of the central Broken Hill region; both these sources contribute radiogenic material mainly by fluvial processes; and (3) the Proterozoic to Palaeozoic rocks of the Kanmantoo, Adelaide, Gawler, and Painter geological regions to the west of the area; these sources contribute radiogenic material mainly by aeolian processes. Regions of low 87Sr/86Sr (≤∼ 0.7130; bottom quartile) belong mainly to (1) a few central Murray Basin catchments; (2) some Darling Basin catchments in the northeast; and (3) a few Eromanga geological region-influenced catchments in the northwest of the study area; these sources contribute unradiogenic material mainly by fluvial processes. The aim of this systematic review was to provide an update on caries prevalence in older adults aged 60 years or above around the globe. Two independent reviewers performed a systematic literature search of English publications from January 2016 to December 2020 using Pubmed, Scopus, Embase/Ovid and Web of Science. The MeSH terms used were “dental caries”, “root caries”, “DMF index”, “aged” and “aged 80 and over”. Further searches in Google Scholar retrieved eight additional publications. The epidemiological surveys reporting the prevalence of dental caries or root caries or caries experience using DMFT (decayed, missing and filled teeth) and DFR (decayed and filled root) in older adults aged 60 years or above were included. Quality of the publications was assessed using the JBI Critical Appraisal Checklist for Studies Reporting Prevalence Data. Among the 5271 identified publications, 39 articles of moderate or good quality were included. Twenty studies were conducted in Asia (China, India, Vietnam, Singapore and Turkey), ten in Europe (Ireland, Norway, Finland, Germany, Portugal, Poland, Romania and Kosovo), three in North America (USA and Mexico), one in South America (Brazil), two in Oceania (Australia) and three in Africa (Malawi, Egypt and South Africa). The prevalence of dental caries ranged from 25% (Australia) to 99% (South Africa), while the prevalence of root caries ranged from 8% (Finland) to 74% (Brazil) in community dwellers. The situation was even worse in institutionalised older adults of which the mean DMFT score varied from 6.9 (Malawi) to 29.7 (South Africa). Based on the included studies published in the last 5 years, caries is still prevalent in older adults worldwide and their prevalence varies across countries.

IntroductionBreastmilk is considered the gold standard for infant nutrition. Breast feeding is recommended as the sole source of nutrition between birth until around 6 months of age and should be continued beyond this age as complementary foods are introduced. While breast feeding initiation is generally high in developed countries, continuation of breast feeding appears to drop rapidly. This is a prospective observational study of life that aims to characterise a current picture of infant feeding practices across the first year, and motivations for feeding practices, and to identify barriers and enablers for breast feeding.Methods and analysisCaregivers with newborn singleton infants of normal birth weight are approached on the postnatal units of three hospitals in South Australia, or through targeted online advertising campaigns promoting the study. Caregivers are asked to complete surveys when their infant reaches 3, 5 and 7 weeks’, and at 3, 4, 5, 6, 9 and 12 months of age. Initially, baseline characteristics, intentions and preferences for infant milk feeds, as well as reasons for preferences are captured. Latter surveys query how infants are being fed, difficulties or barriers to breast feeding, as well as any enablers (if breast feeding). Once infants reach 5 months of age, surveys capture complementary feeding. A large opportunistic sample from the Adelaide community with a minimum of 1000 mother–infant pairs will be enrolled. The data will be analysed descriptively and using regression models.Ethics and disseminationWomen’s and Children’s Health Network Human Research Ethics Committee reviewed and approved the study (approval no HREC/19/WCHN/140, approval date: 22 November 2019). Study results will be disseminated through academic meetings, peer-reviewed journals, in-services for postnatal healthcare services, results letters for participants and social media.Trial registration numberACTRN12620000529943.

Context. Detecting ecological impacts of invasive species can be extremely difficult. Even major population declines may be undetectable without extensive long-term data if the affected taxon is rare and/or difficult to census, and exhibits stochastic variation in abundance as a result of other factors. Our data suggest such a situation in an iconic Australian reptile species, the bluetongue lizard. Originally restricted to Central and South America, cane toads (Bufo marinus) are rapidly spreading through tropical Australia. Most native predators have no evolutionary history of exposure to the toads’ distinctive chemical defences (bufadienolides), and many varanid lizards, elapid snakes, crocodiles and marsupials have been killed when they have attempted to consume toads. Aims. Scincid lizards have not been considered vulnerable to toad invasion; however, one lineage (the bluetongues, genus Tiliqua) consists of large omnivores that may be affected. Our field and laboratory research aimed to elucidate this concern. Methods. Nightly surveys for bluetongue lizards (Tiliqua scincoides intermedia) and cane toads were conducted along two adjacent roadways on the Adelaide River floodplain of the Northern Territory. Scent discrimination trials in the laboratory assessed lizard responses to chemical cues from three food types (native frogs, cane toads and ‘preferred foods’) by counting tongue-flicks and biting elicited by cotton swabs. A subset of lizards was presented with live toads. Key results. Numbers of bluetongues encountered during standardised field surveys in the Darwin region declined soon after toads arrived, and we have not recorded a single lizard for the last 20 months. In the laboratory, foraging responses of bluetongues were as intense to cane-toad scent as to the scent of native frogs, and many of the lizards we tested attempted to consume toads, and were poisoned as a result. Conclusions and implications. The population decline of bluetongues in this region appears to have been the direct result of fatal ingestion of toxic cane toads. Our studies thus add a scincid lizard species to the list of native Australian predators imperilled by cane-toad invasion, and point to the difficulty of detecting invader impact even for an iconic species in a system subject to detailed survey work.

Access to appropriate health and social care is challenging for vulnerable populations. We used a ‘pop-up’ delivery model to bring community-based services in contact with communities with poor access to health and social care. Our aim was to examine whether pop-up events improve access to essential health and social support services for selected vulnerable communities and increase collaboration between community-based health and social services. Set in south-eastern Melbourne, two pop-up events were held, one with people at risk of homelessness attending a community lunch and the other with South Sudanese women helping at-risk youth. Providers represented 20 dental, housing, justice, employment and mental health services. We made structured observations of each event and held semi-structured interviews with consumers and providers. Pre-post surveys of managers assessed acceptability and perceived impact. We reached 100 community participants who had multiple needs, particularly for dentistry. Following the events, participants reported increased knowledge of services and access pathways, community members spoke of increased trust and partnerships between service providers were fostered. The pop-up model can increase provider collaboration and provide new options for vulnerable populations to access needed services. ‘Bringing the service to the person’ is a compelling alternative to asking consumers to negotiate complex access pathways.

AbstractIntroduction:Latex allergy first was recognized early in the 20th Century, but was not a matter of concern until the last decade of that Century. The reported incidence of latex allergy in different occupations varies considerably. It has been documented in dental workers, operating theater staff, anesthetists, and laboratory technicians. However, little data specifically related to those involved in patient care in the emergency prehospital setting are available.Methods:A questionnaire was distributed to a sample of both volunteer and salaried first responders from St. John Ambulance Australia in South Australia and Western Australia, and the South Australian AmbulanceService. The first responders were surveyed to: (1) determine the incidence of latex allergy; (2) consider possible factors associated with its development; (3) compare characteristics of the surveyed groups; and (4) reinforce the development of an educational program.The study tool had predetermined statistical qualities. Data were collated and processed using standard statistical procedures. Surveys were collected anonymously.Results:Of the 2,716 forms distributed, 1,099 were returned, resulting in an overall response rate of 40.5%. Atopy was identified in 14.9% of participants, hand dermatitis in 9.4%, and latex allergy in 6.4%. In the group of full-time ambulance officers, there was a significantly higher incidence of hand dermatitis and latex allergy. There also was a significant relationship between latex allergies and both dermatitis and glove usage (as measured by frequency and duration).Conclusion:In a group of first responders assessed by an anonymous, voluntary questionnaire, the subset of full-time, salaried ambulance officers was identified as having a higher incidence of hand dermatitis and latex allergy than their volunteer co-workers. These results require further assessment to substantiate the frequency of latex allergy and determine the predisposing factors. All personnel must learn about hand care. Non-powdered, natural rubber latex gloves should be supported for general use in this setting.

The Orthopaedic Unit of the Repatriation General Hospital (RGH) in Adelaide, South Australia has implemented a quality care management system for patients with arthritis of the hip and knee. The system not only optimises conservative management but ensures that joint replacement surgery is undertaken in an appropriate and timely manner. This new service model addresses identified barriers to service access and provides a comprehensive, coordinated strategy for patient management. Over 4 years the model has reduced waiting times for initial outpatient assessment from 8 to 3 months and surgery from 18 to 8 months, while decreasing length of stay from 6.3 to 5.3 days for hips and 5.8 to 5.3 days for knees. The service reforms have been accompanied by positive feedback from patients and referring general practitioners in relation to the improved coordination of care and enhanced efficiency in service delivery. What is known about the topic? Several important initiatives both overseas and within Australia have contributed significantly to the development of this model of care. These include the UK National Health Service ‘18 weeks’ Project, the Western Canada Waiting List Project, the New Zealand priority criteria project, the Queensland Health Orthopaedic Physiotherapy Screening Clinic, and most importantly the Melbourne Health–University of Melbourne Orthopaedic Waiting List Project where a wide range of models were explored across Victorian hospitals from 2005 and the Multi-Attribute Prioritisation Tool (MAPT) was developed, validated and tested. This project became the Osteoarthritis Hip and Knee Service (OAHKS) and was operationalised in the Victorian healthcare system from 2012. These initiatives examined and addressed various aspects of management systems for patients with arthritis of the hip and knee in their particular setting. What does this paper add? The development of this system is an extension of what is already known and is the first to encompass a comprehensive and coordinated strategy across all stages of the care management pathway for this patient group. Their management extends from the initial referral to development and implementation of a management plan, including surgery if assessed as necessary and organisation of long-term post operative follow up as required. By detailing the elements, key processes and measurable outcomes of the service redesign this paper provides a model for other institutions to implement a similar initiative. What are the implications for practitioners? An important aspect of the design process was practitioner acceptance and engagement and the ability to improve their capacity to deliver services within an efficient and effective model. Intrinsic to the model’s development was assessment of practitioner satisfaction. Data obtained including practitioner surveys indicated an increased level of both satisfaction with the redesigned management service, and confidence in it to deliver its intended improvements.

John Davies Thomas (1844–1893) described a two-ounce drop-bottle for chloroform in 1872 while he was a resident medical officer at University College Hospital, London. After working as a ship’s surgeon, he settled in Australia. In May 1875, Thomas presented a paper on the mortality from ether and chloroform at a meeting of the Medical Society of Victoria in Melbourne, Victoria. Surveys conducted in Europe and North America had established that the mortality from chloroform was eight to ten times higher than that from ether. At that time, chloroform was the most widely administered anaesthetic in Australia. Thomas’ paper was published in The Australian Medical Journal and reprinted by the Medical Society of Victoria for distribution to hospitals in the Colony of Victoria. Later that year, Thomas moved to Adelaide, South Australia, where he may have been influential at the Adelaide Hospital in ensuring that ether was administered more often than chloroform. It does not appear that Thomas’ papers on anaesthesia had a significant effect on the conduct of anaesthesia in Victoria or New South Wales.

Caries experience was related to dementias severity and not to specific dementia diagnoses. Coronal and root caries experience was higher in dementia participants with moderate-severe dementia, the socio-economically disadvantaged, more functionally dependent, taking neuroleptic medications with high anticholinergic adverse effects, with eating and swallowing problems, were not attending the dentist, who needed assistance and were behaviourally difficult during oral hygiene care and whose carers were burdened.
