This study examines the worsening position of jobless blue-collar workers in an increasingly meritocratic economy, and uses an innovative crossnational comparative approach to gauge how much the social safety net improves their well-being. I take pairs of unemployed autoworkers who did the same job in the same or similar firms—with the only difference being the country they live in—and compare their outcomes to measure policy effects. My analysis is based on in-depth interviews with seventy-one former autoworkers (divided among American and Canadian workers, and Detroit Three and parts factories) and thirty-six industry and community experts in Detroit, Michigan, and Windsor, Ontario, two metropolitan areas right across the river from one another. It also draws from ethnographic observation within households and the larger Detroit and Windsor areas, which allowed me to put my interviews in context and assemble a rich narrative portrait of unemployment and economic distress. Whereas one school of thought stresses the powerlessness of government in the face of globalization and related economic shifts, and another tends to see an expanded welfare state as a panacea for social ills, I stake out a view somewhere in the middle, arguing that the stronger supports in Canada help unemployed workers cope better with job retraining challenges, health problems, financial difficulties, and fragile family structures, but are limited in their ability to overcome relative inequalities: large gaps in education, family stability, and resources that exist between blue-collar workers and other segments of the labor force. I offer a theoretical and historical framework for understanding the evolution of the labor market and its consequences for less-educated workers, conceiving of the current iteration of capitalism as meritocratic in its focus on human capital as the just arbiter of status, and differentiating this meritocratic orientation from other egalitarian and fraternal approaches to policy and morality in past historical periods. Finally, I examine the meritocratic ideology that blunts political responses to rising inequality, finding that such views, long associated with white-collar professionals, have come to affect the thinking of even unionized blue-collar workers.

Esta tese, utilizando-se do mapa cognitivo Strategic Options Development and Analysis - SODA, se propôs a atender o objetivo nela especificado, ou seja, avaliar a importância do comprometimento organizacional para a implementação de um Sistema de Gestão Ambiental SGA por parte dos quinze Gerentes de uma Instituição Pública Federal, localizada no estado de São Paulo. Os dados que compuserem o mapa cognitivo, foram obtidos por meio de entrevista face a face, no período de maio a novembro de 2015, e de reuniões grupais com os referidos Gerentes, no período de dezembro de 2015 a março de 2016. A utilização do método de estruturação de problemas - Problem Structuring Methods (PSMs) - mapa cognitivo - SODA possibilitou investigar, as possíveis incertezas, complexidades e conflitos, voltados para o elemento comprometimento, provenientes da adoção de uma gestão ambiental, por intermédio da implementação de um Sistema de Gestão Ambiental SGA. O resultado da análise do mapa cognitivo demonstrou a importância do comprometimento organizacional quando da intenção de se adotar um Sistema de Gestão Ambiental. Diferentemente do entendimento de Barbieri (2007), o presente estudo coloca a importância desse comportamento não somente por parte da alta direção, mas também por parte de toda a equipe a ser envolvida nas atividades concernentes a tal sistema. Permitiu também a construção de um modelo de mensuração do comprometimento em relação ao Sistema de Gestão Ambiental SGA. O comprometimento mensurado por meio de referido instrumento, está dividido em dois componentes: afetivo que tem o comprometimento como um apego, como um envolvimento, onde ocorre a identificação com a empresa, funcionários com forte comprometimento afetivo permanecem na empresa porque querem, e normativo o comprometimento como uma obrigação em permanecer na organização, funcionários identificados com esse comportamento permanecem na empresa porque sentem que tem essa obrigação. O modelo de mensuração do comprometimento neste estudo sugerido, a ser validado em estudos longitudinais, permitirá mapear elementos de forma que possam observar tendências. Referido instrumento não foi validado neste estudo, no entanto sugere-se que estudos futuros, por meio de estudos longitudinais procedam sua validação.

Death from medical error at time of writing is the third leading cause of the death in the United States. Creating a world where patients and those who care for them are free from harm is the priority in the patient safety movement. A strong culture of prioritizing safe practices is the foundation for safe patient care; this culture can be developed and maintained by the implementation of daily safety huddles. By engaging the team in safety behaviors to achieve the goal of reducing preventable patient harm, daily safety huddles have the potential to impact the safety culture at both the unit and organizational level. Daily safety huddles are deliberate, intentional, purposed conversations in a non-punitive environment from the leader with their team about safety events, concerns, and needs so that situational awareness is created, the team has a shared mental model, and resources can be assigned to reduce the risk of potential events of harm to patients, families, and the health care team. This change project evaluated the impact of daily safety huddles on unit-level safety culture as measured by the Safety Organizing Scale (SOS) survey which is based on the principles of high reliability utilizing a pre-posttest quantitative design. Descriptive statistics were used to describe the characteristics of the inclusive of gender, race, age, experience level, and educational level. Results indicated an overall increase in mean scores from the pre-test to the post-test for all behavioral indices of safety culture with the exception of one question describing handoff communication. A statistically significant positive difference was noted between groups with p = .03 for the SOS question on discussion of mistakes and how to learn from them as a result of huddle implementation. Thus, the implementation of huddles demonstrated a clinically significant improvement in unit level safety culture and a statistically significant improvement in one domain.

From Safety Culture to Safety Orientation.

Developing a tool to measure safety in shipping

This study intend to develop a tool to measure safety orientation (SO) in shipping. SO should be considered a practical safety culture assessment instrument, indicating the degree of orientation towards safety in a group or an organisation. The scale can for example be used in benchmarking as a key performance indicator (KPI), or as an indicator in a balanced scorecard type of management tool.

The definition of the construct follows below:

“Safety orientation consists of the cultural and contextual factors that create the attitudes and behaviour that influence occupational health and safety. Organisations with a positive safety orientation are characterised by a perception of the importance of health and safety, and by confidence in the efficacy of their chosen measures to create the necessary behaviour for avoiding or limiting accidents and to continuously improve health and safety.” (Håvold, 2005).

Examples on the cultural and contextual factors (see definition) might be national culture, organizational culture, industry culture, professional culture, regulations and competition.

The research falls into two categories; to contribute to theory testing of selected hypotheses related to safety culture/safety climate and contribute to the development of a tool to measure SO in shipping.

An extensive literature search in the maritime and safety fields revealed that there has been almost no previous research dealing with safety culture/climate in the maritime context. The study therefore examines the literature dealing with safety culture and climate in areas other than shipping. Many of the factors identified by these investigations assisted the study in the development of a first culture assessment instrument to measure SO at sea.

The main study was designed to represent as broadly as possible the safety attitudes, safety climate and safety culture of seafarers employed aboard Norwegian-owned vessels.

The study was based on a quantitative research approach using two questionnaires for data gathering, generating two datasets. These datasets provide the basis upon which the analyses were conducted. The total number of seafarers who participated in the two surveys numbered more than 2800 from 27 countries. Analysis of the datasets enabled the safety orientation construct to be described comprehensively and to be tested using different quantitative methods.

Utvikling av et verktøy for å måle sikkerhet i skipsfart

Forskningen presentert i avhandlingen har som hovedintensjon å utvikle et verktøy til å måle sikkerhets orientering (SO) i skipsfart. SO er tenkt å være et praktisk instrument basert på sikkerhetskultur/klima, og som indikerer i hvor stor grad en gruppe eller organisasjon er orientert mot sikkerhet og som eksempelvis kan benyttes i benchmarking som en nøkkelindikator eller som indikator i balansert målstyring.

Sikkerhetsorientering er definert nedenfor:

Sikkerhets orientering er et resultat av kulturelle og kontekstuelle faktorer som fører til holdninger og handlinger som i sin tur påvirker yrkesmessig helse og sikkerhet. Organisasjoner med positiv sikkerhets orientering er karakterisert ved at helse og sikkerhet oppfattes som viktig, og ved tillit til at forebyggende aktiviteter vil gi ønsket handling slik at ulykker kan begrenses eller unngås og helse og sikkerhet kan forbedres kontinuerlig (Håvold, 2005).

Eksempel på kulturelle og kontekstuelle faktorer (se definisjon) kan være nasjonal kultur, organisasjons kultur, industri kultur profesjons kultur, lover og regler eller konkurranse.

Forskningen og de publiserte arbeidene som er en del av avhandlingen har både bidratt til teori ved å teste utvalgte hypoteser relatert til sikkerhets kultur/ sikkerhets klima, og bidratt til utvikling av et instrument for å måle SO i shipping.

Litteratur søk i starten på prosjektet avdekket at det var publisert så godt som ingen forskning som omhandlet sikkerhets kultur/klima i en maritim setting. Senere er det publisert noe men betydelig mindre enn for eksempel for offshore oljeproduksjon. Utviklingen av et instrument for å vurdere sikkerhetsorientering til sjøs baserer seg derfor i stor grad på forskning utført i andre bransjer enn shipping.

Hovedstudien ble designet slik at den omfatter holdninger til sikkerhet, sikkerhets klima og sikkerhets kultur til sjømenn ombord på skip eid av Norske redere.

Studien er kvantitativ, basert på analyser av to datasett innsamlet via to spørreskjemaer. I alt deltok mer enn 2800 sjømenn fra 27 land i undersøkelsen. Analyser av datamaterialet gjorde at sikkerhetsorientering begrepet kunne bli beskrevet og testet ved å benytte forskjellig kvantitative metoder.

Patient safety in healthcare has become a national objective. Healthcare organizations are striving to improve patient safety and have turned to high reliability organizations as those in which to model. One initiative taken on by healthcare is improving patient safety culture--shifting from one of a 'no harm, no foul' to a culture of learning that encourages the reporting of errors, even those in which patient harm does not occur. Lacking from the literature, however, is an understanding of how safety culture impacts outcomes. While there has been some research done in this area, and safety culture is argued to have an impact, the findings are not very diagnostic. In other words, safety culture has been studied such that an overall safety culture rating is provided and it is shown that a positive safety culture improves outcomes. However, this method does little to tell an organization what aspects of safety culture impact outcomes. Therefore, this dissertation sought to answer that question but analyzing safety culture from multiple dimensions. The results found as a part of this effort support previous work in other domains suggesting that hospital management and supervisor support does lead to improved perceptions of safety. The link between this support and outcomes, such as incidents and incident reporting, is more difficult to determine. The data suggests that employees are willing to report errors when they occur, but the low occurrence of such reportable events in healthcare precludes them from doing so. When a closer look was taken at the type of incidents that were reported, a positive relationship was found between support for patient safety and medication incidents. These results initially seem counterintuitive. To suggest a positive relationship between safety culture and medication incidents on the surface detracts from the research in other domains suggesting the opposite. It could be the case that an increase in incidents leads an organization to implement additional patient safety efforts, and therefore employees perceive a more positive safety culture. Clearly more research is needed in this area. Suggestions for future research and practical implications of this study are provided.

This research examines the effect of major changes, in the external context, on the safety culture of a UK generating company. It was focused on an organisation which was originally part of the state owned Central Electricity Generating Board and which, by the end of the research period, was a self-contained generating company, operating in a competitive market and a wholly owned subsidiary of a US utility. The research represents an attempt to identify the nature and culture of the original organisation and to identify, analyse and explain the effects of the forces of change in moulding the final organisation. The research framework employed a qualitative methodology to investigate the effects of change, supported by a safety culture questionnaire, based on factors identified in the third report of the ACSNI Human Factors Study Group; Organising for Safety, as being indicators of safety culture. An additional research objective was to assess the usefulness of the ACSNI factors as indicators of safety culture. Findings were that the original organisation was an engineering dominated technocracy with a technocentric safety culture. Values and beliefs were very strongly held and resistant to change and much of the original safety culture survived unchanged into the new organisation. The effects of very long periods of uncertainty about the future were damaging to management/worker relationships but several factors were identified which effectively insulated the organisation from any of the effects of change. The forces of change had introduced a beneficial appreciation of the crucial relationship between safety risk assessment and commercial risk assessment. Although the technical strength of the original safety culture survived, so did the essential weakness of a low level of appreciation of the human behavioural aspects of safety. This led to a limited, functionalist world view of safety culture, which assumed that cultural change was simpler to achieve than was the case and an inability to make progress in certain areas which were essentially behavioural problems.

Safety culture and safety climate are terms that are used often in the context of safety management but are not very well defined or differentiated. This research concentrates on safety climate, a summary concept of employee perceptions of safety management practices within their organisation. There is a common assumption that a positive safety climate results in better safety performance outcomes, yet there is little research evidence to support this notion.
Despite being defined as a summary concept, much of the research on safety climate has been empirically driven and has concentrated on identifying the number and content of the dimensions/factors that contribute to the safety climate. Due to a lack of theoretical input in the field, the use of proprietary instruments which are unavailable in the public domain and varied developmental histories of these tools, the findings of past research has been mixed.
This research addresses several of the limitations in the literature on safety climate by using the National Safety Council of Australia’s Safety Climate Survey. The research centres around two main issues; the dimensionality and factor structure of safety climate; and investigation of the applicability of a framework linking safety climate to safety performance.
The project was divided into two studies. Study I is based on archival data collected by the National Safety Council of Australia (NSCA) (N=215, N=127, N=90). Study II utilises data collected specifically for the purposes pf this research (N=226). The survey instrument was further developed for Study II to include scales that allow for a more complete investigation of a framework linking safety climate to safety performance.
A comprehensive validation of the NSCA safety climate survey was conducted. Structural Equation Modelling was utilised to examine various models of the factor structure of safety climate. It was found that safety climate is best understood as a second order concept comprised of several specific first order factors. The factor structure remained consistent across several samples however the strength of the individual factors varied in each sample.
Structural Equation Modelling was also used to investigate the framework of the relationship between safety climate and safety performance. Evidence in support of his model was obtained however some variations to the model were necessary to achieve this support. Reinforcement for the influence of general organisational climate in providing a context for safety climate was revealed. Given that it was not possible to include all aspects of the framework within the statistical model these findings provide a good impetus for further research in this field.

In the UK 27 million working days are lost due to work-related illness or injury; at an estimate of £13.4 billion to the economy. Over the last 30 years researchers have examined safety culture and its relationship to poor safety performance. An organisation in the high hazard construction industry wanted to understand the factors that shaped and influenced safety performance and safety culture. This thesis details a research project which addresses that aim. A multi-method, triangulated approach was adopted combining both qualitative (focus groups and interviews) and quantitative (safety climate questionnaire) methods. The results of the qualitative studies informed the development of the safety climate questionnaire that included a measure of self-reported accidents and near misses. The qualitative studies identified 6 main themes; Communication, Leadership, Employee Engagement & Involvement, Safety Prioritisation, Job Demands and Culture. Quantitative study results show, Upward Communication, Perceived Organisational Support (POS), Employee Engagement. Leader Member Exchange (LMX) and Organisational Commitment demonstrate a significant relationship with Safety Climate. Safety Climate, POS had a Significant, positive, predictive relationship with both accidents and near misses reported. Upward communication had a significant negative, predictive relationship with accidents and near misses. LMX and Organisational Commitment show a Significant, negative, predictive relationship with accidents reported only. Results can be explained in the context of social exchange relationships. Reporting behaviour is being measured, this can be conceptualised as organisational safety citizenship behaviour. The probability of increasing or reducing reporting behaviours is shaped by social exchanges such as; a) the degree that employees feel supported by the organisation, b) and their manager, c) the safety climate, d) their commitment levels e) and opportunities to raise safety concerns. Interventions should aim to develop leaders and organisational practices to be more supportive, to increase reporting behaviour and to create a more accurate picture of safety performance.

Leaders are organisational architects with an ability to influence the inputs and outputs of business performance which includes safety performance. Inputs and outputs in the context of this study include the safe behaviour of employees and the leader s role in creating a safe working environment. Studies in safety leadership have shown that transformational leadership results in high employee safety participation whereas transactional leadership results in increased safety compliance. Recent studies have focused mainly on the impact of the two leadership styles on the safety climate. This study aims to bridge the existing gap in understanding the effect of transformational, transactional leadership and safety culture on safety outcomes. Self-administered and online questionnaires were used to collect data in chemical organisations in Durban. The number of returned valid questionnaires was 250. Analysis included various correlation tests and multiple regression analysis to test the relationships between the three variables. Results revealed that transformational and transactional leadership lead to different safety outcomes. In addition, transactional leadership positively impacts on safety culture when compared with transformational leadership which has demonstrated a negative relationship.

A deficient safety culture has been implicated in a number of organisational accidents from a range of high hazard industries. Despite its implications for safety, many questions about safety culture remain unanswered. In order to contribute to the literature on this topic, this thesis set out to investigate the role of trust in safety culture. The oil and gas industry was chosen as the context for study due to the hazardous nature of its work, the industry’s focus on continuous improvement in safety performance and the interest shown by oil companies in participating in safety research. Leading models of safety culture have stressed the importance of trust in developing and maintaining patterns of safe behaviours at work. This thesis proposed a new model of safety culture based on dual attitudes about trust. This model states that explicit attitudes about trust are part of safety climate and that implicit attitudes about trust comprise some of the basic underlying assumptions that are the deepest level of safety culture. In order to test this model, this thesis developed a method to measure implicit attitudes about trust in an industrial setting. Using this method, two studies of dual attitudes about trust were conducted at different UK gas plants. In both of these studies, different patterns of results were found for measures of explicit and implicit attitudes about trust for workmates, supervisors and the plant leadership, respectively. These findings support the proposed model suggest that explicit and implicit attitudes about trust are separate constructs that may influence different types of safety behaviours.  Positive relationships were found between measures of explicit attitudes about trust and self-report items about safety behaviours like reporting incidents and challenging unsafe acts.  These findings were taken as a step toward validating the proposed model.

An organizational culture of safety affects employees' attitudes, beliefs, perceptions, and values related to safe practice as well as their behaviors and level of engagement. The purpose of this project was to determine the influence of introducing the just culture model through staff engagement in an interactive workshop. A convenience sample of acute care staff were recruited for this 1-sample pretest and posttest project design. The Agency for Healthcare Research and Quality Hospital Survey on Patient Safety Culture instrument was used to measure safety culture perceptions on 7 dimensions pre and post intervention. For the theoretical framework, Ajzen's theory of planned behavior and Kantar's empowerment theory were used. Welch's t test results showed significant improvement in perception scores overall (t = 2.7, p < 0.01), with posttest mean scores ('= 3.7) higher than pretest mean scores ('= 3.5). The dimension-specific mean posttest scores were significantly higher on 3 of the 7 dimensions including teamwork (t = 2.99, p < 0.05), feedback and communication (t = 2.14, p < 0.05), and frequency of event reporting (t = 2.31, p < 0.05). Major implications for social change include reduction of preventable errors and iatrogenic events; creating a healthcare environment that is safe, fair, transparent, and reliable; creating organizational learning through evidence-based patient safety training; and promoting the use of perception surveys to measure and improve the culture in one's organization. The project may provide a road map for just culture implementation. Future qualitative and quantitative research should explore effects of a just culture on safety reporting patterns and specific events such reducing medication errors or risk-taking behaviors.

Over the past decades, there has been a realisation that safety, within the Saudi construction industry, has been facing challenges. Even though there are good safety practices in many organisations, the industry lacks well publicised legal provisions and standards for employers to use as a means with which they can incrementally develop a safety culture in the workplace, and eventually, improve the safety performance of the industry. This research, therefore, explores and evaluates the influence of current safety culture on safety performance in the Saudi Arabian construction industry. It aims to develop a framework that could be used as a stepwise process for improving safety culture, and positively impact the safety performance in Saudi Arabian construction. The research adopted interpretivism philosophy as the epistemological position and subjectivism as the ontological assumption, which depends on people’s interpretations and their actions. In line with the philosophical stance, the research adopted a deductive reasoning by developing a theory of the issues of safety culture through research questions; and then adopted a research strategy of case study and survey to execute the research. It also adopted a mixed methodology where both qualitative and quantitative methodologies were envisaged to be relevant to the research. The survey approach included semi-structured interviews and questionnaire as techniques. For the questionnaire survey, 135 responses were received from the ranks of managers, engineers, foremen, and labourers in Saudi Arabian construction companies. Using SPSS software, and with the help of the ANOVA test for correlations, the research was able to isolate 29 factors that were associated with commitment to safety procedure, including training and practices. The highest ranked factor was on helping workers secure necessary training in order to avoid accidents and ensure safety. The results from the survey were corroborated by interview results from the three case studies. Using thematic analysis, interview data identified six main factors as components of safety culture in construction organisation in Saudi Arabia. Based on the empirical evidence from the data, the research concludes that the industry, as a whole, has a positive appetite to improve safety culture; and that there is a good realisation that safety performance can only be achieved if there is a good safety culture in the industry. The research also found that even though there is a positive outlook for safety culture, the industry struggles to operationalise it and make it pervasive industry wide. The major reason for weak operationalisation of the safety culture has been the lack of strong framework for consistent implementation of safety practices, which can integrate regulations, standards and best practices from local and international organisations that operate in the Saudi Arabian industry. The value of this research lies in the potential is has to helping stakeholders realise the highly correlation between the safety and organisational culture, and to make a connection with the overall improvements in the productivity of the industry while reducing number of accidents in their work places; hence developing a sound safety culture. The research has a two tiered recommendation, short term and long term recommendations that need to be instituted on a continuous basis. The first tier recommendations relate with the short term improvement in improving commitment to safety practice in order for employees to feel that the safety culture is relevant such as the habit of using PPEs in the workplace is recommended. The second tier recommendation relates to improving the commitment to safety procedures, proper training for construction activities and applying reward management system in the workplace. The two tier recommendations require a regulatory and standards based framework from the industry in order to localise safety culture for improved safety performance in Saudi Arabia.

Patient safety is an active field of research in medicine and the driving force behind healthcare policy and practices to ensure the delivery of safe, quality patient care. However, it is a concept in its infancy in the veterinary profession. Veterinary medical error is under reported, poorly understood and inadequately managed with consequences for patients, owners and clinicians. The aim of this thesis is to explore the causes and types of error in veterinary practice and develop solutions to improve patient safety, and by extension quality of care for veterinary patients. A mixed methodology was employed in the investigation of this aim, with data gathered by focus groups, insurance claim review and questionnaires. The findings of the study suggest that the causes of error in veterinary practice mirror those in other safety critical industries, namely individual errors and system failures. These findings led to the development of a reliable, validated safety culture survey for veterinary practice, to assess and understand the attitudes which drive safety critical behaviours of veterinary staff. This survey was then used as a pre and post training measure to assess the effectiveness of a teamwork training programme, VetTeams, as an intervention to improve safety culture, and by extension patient outcomes in veterinary practice. The outcomes of this study are a framework to inform the understanding and analysis of veterinary error, a measurement tool of veterinary safety culture and a training programme for veterinary teams which addresses the non technical skills identified as critical to preventing mistakes. The findings suggest that changing attitudes to error through an understanding of the causative factors and education in non technical skills, is essential to drive behaviour change in clinicians and enable improved delivery of clinical care.

Research over the past 30 years suggests that sport can provide a context in which children are subjected to several forms of maltreatment. Various countries, regions, sports and individual organisations have responded to this evidence, causing a proliferation of methods used to safeguard children from harm. However, one approach that is yet to be considered is the safety culture approach. This is despite its potential to address all of the potential risk factors of child maltreatment in sport, and its evidenced ability to tackle a wide range of safety concerns in fields as far-reaching as healthcare, aviation and energy production. Based on this, the aim of this thesis was to conceptualise safety culture in sport from a child safeguarding perspective. A total of 45 interviews and 7 focus groups were conducted with participants from five organisations which differed on the grounds of size, mission and geographical location. Data was analysed using thematic analysis. Findings suggested that safety culture in sport is influenced by the presence and nature of; safety management systems, committed leadership and stakeholder engagement. Findings also suggested that these factors have an interconnected relationship, which influences the nature and strength of the safety culture. These factors and their subsequent safety culture were also found to be influenced by a multitude of internal and external contextual factors. Therefore, the findings of this thesis are used to present the Safety Culture Model for Safeguarding Children. This is the first of its kind as both a model of safety culture in sport, and a safety culture model that specifically relates to safeguarding children. Practical implications and avenues for future research are also presented.

This thesis offers a commentary on the use of an embedded approach to explore variables impacting on employee safety culture at a large manufacturing plant. A mixed method approach was adopted in order to assess the safety culture of the company. The assessment stage consisted of point-of- work observations; unstructured individual interviews, semi-structured focus groups and a safety culture survey. This afforded a detailed insight into a rich array of context-specific variables impacting on employee perceptions of safety in the company, referenced to leadership style, incident reporting, rule breaking / risk taking, time pressure, communication and reactive approach to addressing safety issues. The safety culture assessment was followed by the development and implementation of two safety culture improvement programmes (interventions). Two matched pairs of departments (two experimental and two control) were chosen in which to conduct the interventions. The first intervention comprised a replication (with enhancements) of Zohar’s (2003) safety climate improvement intervention. The results indicated that low trust towards the management and the researcher, the face validity of the intervention, negative past experiences, insufficiently transparent communication and alienation engendered a high resistance to change. Seeking to address the shortcomings of the first, the second intervention represented a more organic approach, in which the improvement programme was designed to mesh with and complement established quality management systems. An improvement in employee safety performance was observed in the first month following its introduction, however, it is also possible that this was a consequence of a lean manufacturing intervention that took place at the same time. Variables affecting the intervention success were further explored though interviews with a sample of safety experts. This resulted in the development of a six stage model for successful safety culture intervention design and implementation. The insights gained from these studies were fed back to the industrial sponsor to contribute to corporate insight and understanding into variables impacting on employee safety culture and the design of successful safety improvement programs.

In the ten year period between 2004 and 2013 the UK lost 13 operational, and one non-operational, firefighters at fires, a relatively large number in relation to previous losses. These fatalities occurred during a period in which fire fatalities of members of the public were at an all‐time low but fire and rescue services (FRS) and their staff were being accused in the media of becoming risk averse. This research was focussed on investigating the risk attitudes and safety culture in the English fire and rescue services. The research question asked how the safety culture manifests itself in the English fire and rescue service and what are its implications? A mixed approach to the research was adopted utilising both quantitative and qualitative methods. The research strategy was inductive using a multiple case study. A safety culture questionnaire was designed and then piloted in one FRS with the responses being subjected to a factor analysis the results of which indicated there were four dimensions: management, procedures, competence, and work pressures. The final version of the questionnaire was then distributed across five FRSs from which 845 were returned, of which 823 were used in a series of statistical analyses. Two independent variables were used in the analysis; the first consisted of the individual FRSs, the second consisted of three groups based on Schein’s three generic subcultures of executive, designer and operator. These were aligned with principal officers (PO), senior officers (SO), and Watch based (WB) staff respectively. The analysis of the completed questionnaires indicated that the WB group had a negative attitude towards their FRS safety culture, while the SO and PO groups had a more positive attitude with the PO the most positive. All three groups were significantly different to each other. In conducting the qualitative part of the mixed methods the researcher rode with three Watches at a station in each of three FRSs to observe the behaviour of WB staff and attending SOs at incidents and during their daily activities. During the course of the fieldwork nineteen members across all of the participating FRSs were interviewed, and twenty‐four focus groups were conducted. What was clear was that the competence based training system was not popular with WB staff who believed it to be too bureaucratic, whilst SOs and POs believed that it had not fulfilled their expectations of what it would deliver. There were also concerns expressed that the promotion system, associated with the competence‐based training, was producing managers and not the leaders required on the incident ground. It was concluded that safety culture within the FRS is associated with the systems, policies and procedures reflective of FRS management’s level of risk tolerance producing a rule-based decision‐making bureaucracy; this level of risk tolerance then influences how operational firefighters operate on the incident ground. In the world of the operational firefighter a typical incident, which by its very nature is a temporary event, is laden with uncertainty, complexity and in which all the potential risks may not yet have been identified with decision‐making being focussed on problem‐solving. FRS personnel find themselves operating in a risk climate in which they build temporary command structures, construct temporary processes and controls reflecting the incident commander’s risk appetite for the purpose of moving towards operating in a safety climate in which to resolve the incident. The combination of the FRS’s safety culture and the operational firefighter’s risk climate determine what the researcher has defined as the FRS operational culture.

Les anàlisis posteriors a l'ocurrència dels accidents nuclears més importants (Three Mile Island, Chernobyl i Fukushima) han posat de manifest que les causes dels incidents de sistemes tecnològics complexos no solament estan vinculades a aspectes tècnics, sinó que factors aparentment més imperceptibles, com el comportament humà o la cultura organitzacional, juguen un paper important en el seu desenvolupament (Rasmussen, 1997). En la indústria nuclear espanyola, l'estudi d'incidents com l'incendi en l'edifici de les turbines de la central nuclear (C. N.) de Vandellòs I (1989), la corrosió dels sistemes de refrigeració en la C.N. de Vandellòs II (2004) i l'alliberament de material radioactiu en la C.N. d'Ascó I (2008), també ha evidenciat la influència del component humà i organitzatiu en l'operació segura de les instal·lacions. En aquest context, la present tesi doctoral té el propòsit investigador de tractar d'identificar trets característics de la cultura de les organitzacions nuclears espanyoles a partir de l'anàlisi dels resultats de les avaluacions externes de la cultura organitzativa i de seguretat realitzades al sector. El coneixement d'aquests trets culturals pot ajudar a comprendre el comportament humà en un context on la seguretat ha de ser un dels fins de l'organització. Per a això, es proposen tres objectius específics plasmats en els tres estudis empírics que conformen la tesi: 1) Descriure la cultura organitzativa del sector nuclear espanyol i identificar la possible existència de diferents subcultures dins de la indústria (Badia, Navajas i Losilla, 2020). 2) Descriure alguns dels factors rellevants per al desenvolupament de les característiques de cultura de seguretat sobre la base de tres teories científiques sobre organitzacions complexes (Badia, Navajas i Losilla, 2021). 3) Identificar, en un estudi de cas, els factors que determinen el comportament notificador del personal en el sistema de reporti de problemes (Navajas i Badia, 2020). Aquesta tesi aporta coneixement sobre alguns dels patrons normatius i trets culturals de la indústria nuclear espanyola que impacten en la seguretat de les instal·lacions. Aquest coneixement pot ser utilitzat pel sector per a la millora de l'acompliment i l'operació segura de les plantes. En aquest sentit, la similitud cultural entre centrals nuclears podria ser considerada per les diferents organitzacions per a treballar col·lectivament a optimitzar el seu funcionament. Compartir actuacions en l'àmbit dels factors humans i organitzatius i buscar estratègies per a gestionar la variabilitat cultural no sols podria ser un factor clau per a la seguretat, sinó que els permetria fer front conjuntament als desafiaments als quals s'enfronta la indústria amb l'abandó progressiu de l'energia nuclear. Los análisis posteriores a la ocurrencia de los accidentes nucleares más importantes (Three Mile Island, Chernobyl y Fukushima) han puesto de manifiesto que las causas de los incidentes de sistemas tecnológicos complejos no solamente están vinculadas a aspectos técnicos, sino que factores aparentemente más imperceptibles, como el comportamiento humano o la cultura organizacional, juegan un papel importante en su desarrollo (Rasmussen, 1997). En la industria nuclear española, el estudio de incidentes como el incendio en el edificio de las turbinas de la central nuclear (C. N.) de Vandellòs I (1989), la corrosión de los sistemas de refrigeración en la C.N. de Vandellòs II (2004) y la liberación de material radiactivo en la C.N. de Ascó I (2008), también ha evidenciado la influencia del componente humano y organizativo en la operación segura de las instalaciones. En este contexto, la presente tesis doctoral tiene el propósito investigador de tratar de identificar rasgos característicos de la cultura de las organizaciones nucleares españolas a partir del análisis de los resultados de las evaluaciones externas de la cultura organizativa y de seguridad realizadas al sector. El conocimiento de dichos rasgos culturales puede ayudar a comprender el comportamiento humano en un contexto donde la seguridad debe ser uno de los fines de la organización. Para ello, se proponen tres objetivos específicos plasmados en los tres estudios empíricos que conforman la tesis: 1) Describir la cultura organizativa del sector nuclear español e identificar la posible existencia de diferentes subculturas dentro de la industria (Badia, Navajas y Losilla, 2020). 2) Describir algunos de los factores relevantes para el desarrollo de las características de cultura de seguridad en base a tres teorías científicas sobre organizaciones complejas (Badia, Navajas y Losilla, 2021). 3) Identificar, en un estudio de caso, los factores que determinan el comportamiento notificador del personal en el sistema de reporte de problemas (Navajas y Badia, 2020). Esta tesis aporta conocimiento sobre algunos de los patrones normativos y rasgos culturales de la industria nuclear española que impactan en la seguridad de las instalaciones. Este conocimiento puede ser utilizado por el sector para la mejora del desempeño y la operación segura de las plantas. En este sentido, la similitud cultural entre centrales nucleares podría ser considerada por las distintas organizaciones para trabajar colectivamente en optimizar su funcionamiento. Compartir actuaciones en el ámbito de los factores humanos y organizativos y buscar estrategias para gestionar la variabilidad cultural no sólo podría ser un factor clave para la seguridad, sino que les permitiría hacer frente conjuntamente a los desafíos a los que se enfrenta la industria con el abandono progresivo de la energía nuclear. Asimismo, esta tesis abre nuevas líneas de investigación orientadas a profundizar en algunos de los hallazgos obtenidos como son la gestión de las diferencias culturales entre colectivos por parte de las organizaciones o el estudio de la influencia en la seguridad de las diferentes interacciones culturales.
Post-accident analyses of the most important nuclear accidents (Three Mile Island, Chernobyl and Fukushima) have revealed that the causes of incidents involving complex technological systems are not only linked to technical aspects, but also to seemingly more imperceptible factors, such as human behavior or organizational culture which play an important role in their development (Rasmussen, 1997). In the Spanish nuclear industry, the study of incidents such as the fire in the turbine building at Vandellòs I NPP (1989), the corrosion of the cooling systems at Vandellòs II NPP (2004) and the release of radioactive material at Ascó I NPP (2008), has also evidenced the influence of the human and organizational component in the safe operation of the facilities. In this context, the purpose of this doctoral thesis is to try to identify characteristic features of the culture of Spanish nuclear organizations based on the analysis of the results of the external evaluations of the organizational and safety culture carried out in the sector. Knowledge of these cultural traits can help to understand human behavior in a context where safety must be one of the organization's goals. To this end, three specific objectives are proposed in the three empirical studies that make up the thesis: 1) To describe the organizational culture of the Spanish nuclear industry and identify the possible existence of different subcultures within the industry (Badia, Navajas and Losilla, 2020). 2) To describe some of the relevant factors for the development of safety culture characteristics based on three theories on complex organizations (Badia, Navajas and Losilla, 2021). 3) To identify, in a case study, the factors that determine the reporting behavior of personnel in the problem reporting system (Navajas and Badia, 2020). This thesis provides knowledge on some of the normative patterns and cultural traits of the Spanish nuclear industry that impact on the safety of the facilities. This knowledge can be used by the industry for the improvement of plant performance and safe operation. In this sense, the cultural similarity between nuclear power plants could be considered by the different organizations to work collectively to optimize their operation. Sharing actions in the field of human and organizational factors and looking for strategies to manage cultural variability could not only be a be a key factor for safety, but would also allow them to jointly address the challenges they face with the progressive withdrawal of nuclear power energy. Likewise, this thesis opens new lines of research aimed at deepening some of the findings obtained, such as the management of cultural differences between groups by organizations or the study of the influence of different cultural interactions on safety.

Safety culture is a new safety “buzz word” and is promoted as a phenomenon that could assist organisations in improving safety performance in their endeavour to safety excellence. It is however, a relatively new concept and research in this field was completed during the past two decades only. Safety culture is a multi-dimensional, dynamic and complex concept. An improved understanding of the safety culture concept is required to enable organisations to determine the value of pursuing safety culture enhancement in their quest for superior safety performance.

Safety Management Systems (SMS) are becoming the industry standard for safety management throughout the aviation industry. As the Federal Aviation Administration (FAA) continues to mandate SMS for different segments, the assessment of an organization’s safety culture becomes more important. An SMS can facilitate the development of a strong aviation safety culture. This study describes basic principles and components of an SMS and how safety culture and SMS are integrated. Studies focusing on safety culture assessment were identified for other industries as well as for different areas of the aviation industry. The purpose of this study was to examine the relationship between an organization’s safety culture and SMS implementation in collegiate flight schools. The research study was designed to determine (a) relationship between SMS implementation and safety culture, (b) the relationship between safety promotion and safety culture, and (c) the relationship between management commitment and safety culture. The study population consisted of 453 individuals at 13 collegiate flight schools. Data were gathered through an online survey to safety officers at collegiate flight schools within the University Aviation Association to determine the level of SMS implementation; and the Collegiate Aviation Program Safety Culture Survey (CAPSCUS) was used to measure the safety culture at those collegiate flight schools. The results indicated that a relationship existed between SMS implementation and safety culture, safety promotion and safety culture, management commitment and safety culture. The relationship for all three was more prominent within the Formal Safety Program major scale of the CAPSCUS. It is recommended that collegiate flight schools examine their existing level of SMS, management commitment, and their safety promotion and assess safety culture within their institution. ENGLISH ABSTRACT: Manufacturing plants should strive towards achieving and maintaining good safety records. It is however difficult for plants to identify specific safety problem areas that can be improved on. One way to identify specific safety areas that needs improvement is by conducting a safety culture survey. A manufacturing plant within South Africa realised the need to improve on its safety performance. However, it was not clear which aspects of its work and safety related practices were at risk. Management therefore decided to obtain an assessment of the safety culture profile of the plant. The assessment was done by way of a plant-wide safety culture survey. The safety culture questionnaire was developed, as part of this research, through an in-house consultation process. This process resulted in identifying 16 different safety-related themes. With the help of literature, these themes were further explored to design the questionnaire. The identified safety themes were measured during the survey and results obtained for each of the plant’s identified safety practices. The survey also provided an overall mean score of the plant’s safety culture, providing management with a better understanding of where they stand in their safety improvement journey. The plant’s equipment, materials and tools; overall rules and regulations; environment, health and safety suggestions; rewards and reinforcement; and management involvement practices was identified as practices that require an immediate response. The plant’s incident reporting and investigation, discipline and training safety practices were identified as less urgent risks.

The purpose of this study was to investigate the safety culture and safety behaviors of metropolitan professional firefighters. A validated and reliable safety culture survey was used to assess the safety culture of a metropolitan fire department. A safety behavioral checklist was created and used to assess the safety behaviors of professional fire fighters. The sample for the study included 156 firefighters from a metropolitan fire department in North Carolina. A Pearson correlation was used to determine if there was a significant relationship between safety culture and safety behaviors. ANOVA and t-tests were used to determine if significant differences existed in safety culture and/or safety behavior on selected demographic factors. Data analysis revealed a significant correlation between safety culture and safety behavior. Results indicated the more positive safety culture is viewed, the more likely the firefighter is to practice safe behaviors. Findings also indicate that demographic factors such as education, marital status, work experience, and dependent status have no significance on how firefighters view safety culture and on the safety behaviors of firefighters. Data analysis did indicate a marginal significance in safety culture by participants who reported being moderately or severely injured ‘on the job’.

Objective:Patient safety in health care organization has received much attention following the Institute of Medicine report “To Err Is Human: Building a Safer Health System” (IOM, 1999). The patient safety movement was rooted in the mid 1970s and 1980s, when medical malpractice claims were both high in number and severity (Johnson & Hudson, 2004). In Taiwan, the healthcare organization authorities, government and media initiated some attention to patient safety since 2002. Those several medical adverse events forced healthcare organizations and clinical manger started some movement including the recognition of healthcare risk management as a profession, adoption by health care leaders of continuous quality improvement principles, and advancement of the role of the hospital setting. Background:Patient safety culture as the product of individual and group values, attitudes, perceptions, competencies and patterns of behavior that determine the commitment to, and the style and proficiency of, an organization’s safety management. An organization with a ‘safety culture’ is open and fair with staff when incidents happen, learning from mistakes and, rather than blaming individuals, looking at what went wrong in the system. Through those actitives couls avoid unnecessary harm of patient and cost of healthcare organizations. The previous studies lacked systematic research on patient safety culture, therefore, it is important for health care organizations to assess their culture regarding patient safety in order to improve patient safety within the health care process. Methodology:The authors measured organizational culture (bureaucratic, supportive and innovative culture), patient safety culture and performance from 788 hospital workers among 42 hospitals in Taiwan. Multilevel analysis was applied to explore the relationship between organizational culture (group level) and patient safety culture and patient safety performance (individual level). In this research, the authors used SPSS 14.0 for Windows, Amos 7.0 and HLM 6.06 tools to estimate and evaluate the relationship among those variables. Conclusion: Overall, organizational culture plays an important role in patient safety activities. Safety performance of hospital staffs are influenced by the prevailing cultural norms in their organizations and work groups, building a safety culture for healthcare system which might understand the how organizations shaping their culture and how its work in safety issue. Constructed patient priority from management commitment to leadership is necessary, learning from and holding an open mind about mistake. For academic and management implications, research on patient safety should consider safety perception and consequence of hospital staffs, and furthermore consider those aspects in patient side. These factors are important for understanding the barriers and the possibilities embedded in patient safety.

This research aims at identifying the correlations among safety culture, safety Behavior, and safety performance, as well as finding and explaining the relational dimensions and factors that affects the above three aspects. Most of the existing reference about this topic (the correlation among safety culture, safety Behavior, and safety performance) were conducted by literature review, few of them has been identified by actual data. The SEM (structure equation modeling) and factor analysis are applied by issuing questionnaires to drivers of Taiwan Railway. A total of 536 effective samples were obtained. Results show safety culture has certain effect upon the awareness in safety Behavior. Safety culture and safety Behavior influence personal awareness in safety performance. Contrary to previous literatures, this study uses case study to analyze hypothesis and results from other researchers, and ensures the mutual effects among safety culture, safety Behavior, and safety performance.

Organizational safety culture implies the recognition of safety value and ways for an airline operation and is a key factor affecting an airline safety performance. Consequently, a comprehensive airline organizational safety culture inventory which can efficiently diagnose airline safety status as well as discover culture difference and potential risk so as to enhance regular operation is a necessary tool for airline safety management. This research starts with the identification of regular operation and duties for all departments in an airline. On this basis, the organization structure of an airline is divided into three levels: management, operation and staff. Moreover, airline organizational safety culture is decomposed as three dimensions - perception, internal environment and behavior - according to literature review. Thereafter, the airline organization safety culture inventory is constructed by combing the aforementioned three organization level and three safety culture dimensions in addition to their corresponding measurements. This inventory was tested with a domestic airline and the main findings are as follows: (1)The developed inventory has good reliability, content validity and conceptual validity; (2)Results of the path analyses have shown that superiors and subordinates of management and operation levels have significant positive effect in each dimension, except those of superiors’ perception to behavior and subordinates’ perception to internal environment; (3)Superiors and subordinates of staff level had no significant effect in each dimensions, except those of superiors’ perception to internal environment and subordinates’ internal environment to behavior ; (4)Gender and education background had no significant difference in each dimension, but age, marital status, and work experience had shown significant difference in some dimensions; (5)The dimension scores of management level was obviously higher than other levels and some of the dimension scores of superiors was higher than subordinates’. The analysis results show that the constructed airline organization safety inventory is practicable and helpful for understanding the features of an airline organizational safety culture as well as discovering the deficiency of regular operation of an airline.

This study constructs the basic safety leadership, safety behavior and safety culture, to analyze the performance of safety-related factors, and these factors are aspects of the behavior of organizations and individuals involved in research to investigate the high-risk job of Taipower distribution line maintenance , affecting the security dimensions of the relationship between the organization, research methods and the use of literature questionnaire for statistical analysis, the object belongs to the Taiwan Power Company, represented by the Taichung District Office. Total sample survey released 350 samples for surveying, 315 valid samples, to descriptive statistics, factor analysis and multiple regression analysis, to verify the correlation between each dimension. Study found that safety leadership and safety behavior on organizational safety performance dimensions, which can produce very significant positive correlation, and safety leadership and safety behavior on the safety culture dimensions also has a positive effect on high strength, and "safety leadership", "safety behaviors" and "safety culture" between "safety performance" also showed a positive correlation. Also among the facets of "manager control" variables, "the Organization for Security operator, "the worker into mutual aid and "security organizations, Taiwan Power Company is currently an urgent need to further strengthen the Department, and the study of the overall structure of fit is appropriate.

The purpose of this study is to discuss the influence of aircrajt maintenance safety culture on safety performance. The methods adopted in this research were literary analysis and questionnaire. The subjects were aircraft maintenance staffs, servicing in the Army Aviation, who stationed in north, middle and south of Taiwan. By using the purposive sampling methed, 420 observed data were collected. After removing the invalid questionnaires, 373 ralid samples remained, SPSS and AMOS(SEM) statistical software packages were used for data analysis. In this study, the reliability of all dimensions are above 0.7 in terms of the Cronbach αvalues which means the significance levels are reached. As to the overall fitness, χ2/df=2.215, GFI=0.982, CFI=0.993,NFI=0.888,IFI=0.993,RMSEA=0.057,indicate that the model has a good explanatory power. The conclusions of the study are as follows: 1. Safety culture on path analysis of safety performance:the safety culture will directly affect the safety performance through safety behavior. 2. Sfaety climate on safety performance path analysis:the meditaing effect of safety behavior achieves a significatnt level, To enhamce aircraft maintenance staffs’ safety performance efficiently, aircraft maintenance units shoule concern staffs’ safety behavior other than personality. 3. In the interference analysis of personality for safety behavior and safety performance:the P value achieved significant difference which means personality interferes with sfety behavior and safety performance. 4. The assessment model of personality for safety performance:thorough multi-group analysis, safety culture has significant positive impact on ssfety performance. As a result, the safety performance of aircraft maintenance staffs will be affected by marital status,ramks and job content. 5. The potential variables in this reaserch model include safety culture, safety climate, safety behavior, safety performance and personality which provid multi-facet discussion about how maintenance safety culture influences safety performance. The reaserch model is on of this study’s academic contributions.

The research topic of safety culture and safety performance becomes more and more important in recent years, but the application that using structural equation model(SEM) to carry out systematic model fitness of safety culture and safety performance model is still scarce. In this study, the questionnaire based on the modified International Atomic Energy Agency (IAEA) safety culture model that we developed and safety performance model proposed by Teh-Sheng Su et al. (2006, 2007, 2008), has good reliability and validity. Eight global factors of safety culture and four global factors of safety performance were identified via factor analysis. To further achieve the best fit of the safety culture and safety performance model we built a series of SEM competition models using Amos 17.0. Using confirmatory factor analysis (CFA) confirmed the safety culture and safety performance correlated model, and results showed that the second order multi-factor model and/or the third order single-factor model of safety culture model, as well as the first order multi-factor model and/or the second order single-factor model of safety performance, are the desirable models where both of the measured variables and latent variables are in good reliability. Finally, model analysis and fitness of the integrated safety culture and safety performance model showed good reliability, validity and stability.

With providing high-quality health care services as the fundamental purpose, medical institutions should regard safeguarding patient safety as the core task. In recent years, the Chinese government has formulated relevant policies to promote patient safety. Medical institutions at all levels have formulated corresponding systems and measures in accordance with the policy requirements and the features of hospitals. However, due to the long-term impact of traditional harsh culture on the medical environment, many of the views and behaviors have not been fundamentally changed, leading to many problems in patient safety performance. As a group who first discover and handle patient safety problems, nurses can identify the causes of safety problems in a relatively accurate way. Therefore, it is necessary to conduct an in-depth study on the relationship between patient safety culture and safety performance in nursing.
Tendo como propósito providenciar serviços de saúde de alta qualidade, as instituições médicas devem perspetivar a proteção da segurança do paciente como uma tarefa central. Nos últimos anos, o governo chinês elaborou políticas relevantes para promover a segurança do paciente e as instituições médicas de todos os tipos também têm desenvolvido várias atividades de segurança do paciente. Contudo, dado que o clima médico atual é afetado pela cultura tradicional de culpa-e-acusação, muitas das atitudes e comportamentos não foram mudados na essência, originando vários problemas de desempenho na segurança dos pacientes. The objective of this research is to study the influence of safety disposition tendency on the safety and health attitude for the workers of a government-owned enterprise. Reviewing relevant literature has been carried for understanding the theory that underlines the worker’s safety and health attitude so that the theoretical basis for conducting this study can be established. Questionnaires are distributed to the workers of a government-run enterprise for their experience to carry out the empirical study; experts in the selected field are also interviewed for their profession opinions. The results are expressed in percentage of data distribution; the average and standard deviation of the results are calculated for understanding the safety disposition tendency of the workers surveyed. Additionally, statistical analyses such as student t-test and one-way ANOVA are applied for testing the various presumptions in order to confirm the results on safety disposition tendency. The results of questionnaire survey and experts’ opinions collected during their interview are also used for studying the correlation between and weighted factor of safety disposition tendency, and safety and health attitude. The conclusions will be useful references for enterprises to implement the most effective method for improving the employees’ safety and health attitude.

The retail business has been negatively affected due to the increasing customer concerns about food safety and the recent events related to microbiological and chemical contamination of food products, such as the melamine in infant formula and the multiple cases of produce pathogen contamination. It has been shown that a scientific-based food safety system, such as, Hazard Analysis and Critical Control Points (HACCP), help reduce the likelihood of food safety incidents. Nevertheless, companies with these kinds of systems have too experienced public food safety issues. Food safety professionals have created instruments to measure food safety based on lagging indicators, such as pathogen presence or food safety incident reduction. Though, they have not created metrics based on leading indicators to measure the behavior driven by the culture of employees who handle the food. The employees who handle food are influenced by cultural values and behave in a company based on the company's cultural influence; hence, food safety should also be measured in cultural terms as a leading indicator. In order to measure food safety culture of a retail company and understand differences among groups, the researcher used case study methodology to select a USA based retailer. The researcher described the culture of the retailer, as well as the leadership styles. Based on these descriptions, the food safety culture of the company and the employee constructs were defined and piloted for construct validity and construct reliability. Once validated, a food safety culture survey instrument was implemented in the operations area of this retailer. The results showed that the employees agreed or strongly agreed that food safety was part of the corporate culture. One of the factors influencing the food safety culture was service to the customer. The results showed that there were differences in food safety culture across the different groups, but with negligible or small effect size. One of the main contributions of this study was the development of a metric to measure food safety culture in the retail industry. It also suggests that besides food safety audit scores and incident reduction, food safety needs to be measured in terms of the corporate culture of the retailer. In an effort to improve food safety around the world, organizations should consider that culture plays an important role.

Background: Patient safety issues have received widespread attention all over the world. Measuring patient safety culture is an important part to manage and improve patient safety. Objective: The purpose of this research is to test the validation of the Hospital Safety on Patient Safety Culture (HSOPSC) scale in the Chinese healthcare setting. Method: This study used a sample of 2465 from a medical group. The following analysis methods were employed to understand the validity of the HSOPSC scale and the patient safety culture issues in the medical group: Exploratory Factor analysis (EFA) was performed to examine the applicability of the factor structure of the original questionnaire to the HM Hospital Group data. Two-sample T test and One-way ANOVA for discrimination according to variable conditions and Pearson's correlation also were conducted to understand the patient safety issues in Chinese hospitals. Result: This study indicates towards a 7-factor model instead of the 10-dimenion HSOPSC scale. This study also identifies some cultural issues of using SPOPSC in Chinese hospital setting which will compromise the applicability of this instrument. Conclusion: Cautions should be taken when using SPOPSC in Chinese hospitals. In addition, measuring patient safety culture in Chinese hospital per se is challenging due to the national culture.
Antecedentes: As questões de segurança do paciente têm recebido atenção generalizada em todo o mundo. Medir a cultura de segurança do paciente é uma parte importante para gerenciar e melhorar a segurança do paciente. Objetivo: O objetivo desta pesquisa é testar a validação da escala Hospital Safety on Patient Safety Culture (HSOPSC) no ambiente de saúde chinês. Método: Este estudo usou uma amostra de 2.465 de um grupo médico. Os seguintes métodos de análise foram empregados para entender a validade da escala HSOPSC e as questões da cultura de segurança do paciente no grupo médico: A análise fatorial exploratória (EFA) foi realizada para examinar a aplicabilidade da estrutura fatorial do questionário original ao HM Hospital Group dados. O teste T de duas amostras e a ANOVA de uma via para discriminação de acordo com as condições variáveis e a correlação de Pearson também foram realizados para compreender as questões de segurança do paciente em hospitais chineses. Resultado: Este estudo indica um modelo de 7 fatores em vez da escala HSOPSC de 10 dimensões. Este estudo também identifica algumas questões culturais do uso do SPOPSC em ambiente hospitalar chinês que irão comprometer a aplicabilidade deste instrumento. Conclusão: Cuidados devem ser tomados ao usar HSOPSC em hospitais chineses. Além disso, medir a cultura de segurança do paciente no hospital chinês per se é um desafio devido à cultura nacional.

This study aimed to investigate the influence of safety management and safety leadership on safety performance as well as to examine the mediating effect of safety culture in the Steel Corporation C. This study conducted the questionnaire survey and collected data from approximately 700 workers at the steel rolling mill. Using a convenient sampling technique, a total of 560 questionnaires were delivered and 412 valid responses returned with a response rate of 73.6%. All the research hypotheses in this study were completely or partially accepted. The results of regression analysis revealed that the factors of ‘organizational system building’ in safety management, ‘initiating structure leadership’ in safety leasership, and ‘organizational shared beliefs and practice’ in safety culture were the main influence on safety performance. This study also confirmed that safety culture have significantly positive and partial mediating effect on the relationships among safety management, safety leadership and safety performance. This study suggests managers of the Steel Corporation C to develop the capability of initiating structure leadership and to enhance the constructing of organizational system and implementation of safety audits. Minewhile, it is also important to cultivate the workers’ value and beliefs of safety culture in order to pursuit safety performance improvements.

This research takes employees of tourist and hotel industries as the object of study. According to the literature development, 11 dimensions for the safety culture scale are included, and there are 57 topic items as the research tool. The method of evaluation adopts five-point scale of Likert. 450 questionnaires were distributed and 317 effective samples were retrieved (70.44%). SPSS 14.0 is used to analyze. The research findings show that the entire safety culture status is presented by a radar chart and the percentage score of the entire safety culture dimension reaches 80.92. The percentage scores of safety culture in position types are higher and the employees in managerial hierarchy have more safety culture consciousness than non-managerial positions. The result of percentage scores in different organizational characteristics show that the level of safety culture consciousness for (Hotel B) winning many awards for health and safety environment is better than other hotels. Through t test, ANOVA analysis and post hoc test, we find out that the background variables for employees of tourist and hotel industries in safety culture consciousness are significantly different. According to the research findings, tourist and hotel industries should emphasize and upgrade the safety culture consciousness for female employees, young working groups (age below 25), unmarried persons, the persons who have less years of service and the employees of non-managerial hierarchy. As for relevant variables, the variables of “age” and “years of service” have an obvious positive impact on the entire safety culture. The variables of “gender”, “education background”, “marriage status”, “and post of job” and “experiences of occupational accidents” have an obvious negative influence on the entire safety culture. At last, this research builds up the norms of safety culture in tourist and hotel industries according to this safety culture scale and the results will be offered to the field of practitioners as the safety culture analysis and the tool for improving the intervention point. In current stage, enhancing health activities, prize and penalty for safety and upgrading the safety communications shall be the priorities to promote the safety consciousness of employees and make a safety culture with high quality, create a safe and healthy working environment, encourage employees’ morale to establish a good business reputation and increase the market advantage.

In this study, an instrument for measuring hotel safety culture was developed and empirically cross-validated. The study considered how Total Safety Culture, Reciprocal Safety Culture Model and Industrial Development Bureau could be integrated to develop the framework by using content validity and modified Delphi method. Survey responses from 418 5-star hotel staffs and directors were used to verify the instrument for hotel safety culture via exploratory and confirmatory factor analysis, and 13 standards, 5 dimensions were identified. The 13 standards, in terms of Safety Commitment, Safety supervision, Rewards and sanctions, Safety environment, Safety obedience, Safety training, Risk management, Safety regulations, Safety knowledge, Safety perception and attitude, Safety communication, Safety behavior and Safety involvement/participation.

The goal of this research is to discuss the influence of safety culture on safety performance of naval vessels and to understand the different situations of different personal background parameters versus the awareness of safety culture. This research proposes four perspectives of safety culture of naval vessels: “ the awareness of risks”, “ the promises of safety”, “ the attitudes toward safety” and “ the training of safety” and another two perspectives of the efficiency of safety: “ the score of the supervision of sailing safety” and “ the surveys and statistics of Marine Casualty.” The research has been processed with questionnaires.The objects of the research are the present officers and petty officers who work on naval vessels. There are totally 800 questionnaires among which 743 are effective. The result of the research shows that in the aspect of personal background parameters versus safety culture perspectives, except the “ parameter of classes” and “working department”, all other parameters show positive relations along with the ages and the experiences of the objects. In terms of safety culture , except “the training of safety”, all other perspectives present obvious positive related to “the score of the supervision of the safety of sailing.”In addition, in all the perspectives of safety culture versus the efficiency of safety of “the surveys and statistics of Marine Casualty ”, all of them reach significant negative related.

In Taiwan, the construction industry has for years topped the others in terms of industrial and occupational accidents. While there are studies that investigate the effects of safety culture or safety climate in jobsites on safety performance, there are no existing publications that examine the effects of leadership behaviors and safety culture on safety performance in Taiwan’s construction industry. In this study, related literature was referred to for the development of rating scales and expert interviews were conducted. 54 factors (in three dimensions: leadership behavior, safety culture and safety performance) were extracted from 72 factors in question items and underwent first exploratory factor analysis and then confirmatory factor analysis. Finally, a structural equation model was created to measure the interactions between the three dimensions and the relationships between key factors in these dimensions. Analytical results show that these dimensions all have good fits according to the confirmatory factor analysis. The structural equation model reveals the following. The relationships between consideration leadership and proactive and passive performance differ from the corresponding hypotheses, so these hypotheses are not supported. The relationships between initiating structure leadership and proactive and passive performance also differ from the corresponding hypotheses, so these hypotheses are not supported. These results mean that the relationships between leadership behaviors and safety performance differ from the corresponding hypotheses, so these hypotheses are not supported. Safety culture has significant positive direct effects on both proactive and passive performance and these results are consistent with the corresponding hypotheses, so these hypotheses are supported. Both consideration and initiating structure leaderships have significant positive direct effects on safety culture, demonstrating that the effects of leadership behaviors on safety culture are consistent with the corresponding hypotheses, so these hypotheses are supported. Regarding influential effects, both consideration and initiating structure leaderships can have impacts on proactive and passive performance via safety culture, indicating that they can indirectly influence safety performance through safety culture. Regarding mediation effects, it is proved that leadership behaviors can significantly influence safety performance via the mediation of safety culture. This study also shows that the mediation is complete mediation. This study also finds that initiating structure leadership has greater positive effects on safety culture, which also has influence on safety performance. These connections can undoubtedly contribute to improvement of safety performance. When adopting initiating structure leadership, (senior) managers in construction companies, besides achieving safety performance targets through establishment of safety management, management systems and job assignment, can also increase expedient and flexible practices to facilitate the achievement of safety performance targets. When adopting consideration leadership, these managers, besides achieving safety performance targets by focusing on communication and allowing employee autonomy, also need to always supervise and impose proper regulation on their employees to ensure continuous good safety performance. Therefore, shaping of effective leadership behaviors in construction companies can create positive safety culture. Moreover, actions such as safety education and training as well as creation of a comprehensive safety reporting system also help the development of safe organizational systems. In this way, it is possible to facilitate improvement of safety performance and in turn reduce occupational accidents.
