In the context of tensor network states, we for the first time reformulate the corner transfer matrix renormalization group (CTMRG) method into a variational bilevel optimization algorithm. The solution of the optimization problem corresponds to the fixed-point environment pursued in the conventional CTMRG method, from which the partition function of a classical statistical model, represented by an infinite tensor network, can be efficiently evaluated. The validity of this variational idea is demonstrated by the high-precision calculation of the residual entropy of the dimer model, and is further verified by investigating several typical phase transitions in classical spin models, where the obtained critical points and critical exponents all agree with the best known results in literature. Its extension to three-dimensional tensor networks or quantum lattice models is straightforward, as also discussed briefly.

Purpose: Chemotherapy mixing errors (CTMRs), although rare, have serious consequences. Illinois CancerCare is a large practice with multiple satellite offices. The goal of this study was to reduce the number of CTMRs using Six Sigma methods. Patients and Methods: A Six Sigma team consisting of five participants (registered nurses and pharmacy technicians [PTs]) was formed. The team had 10 hours of Six Sigma training in the DMAIC (ie, Define, Measure, Analyze, Improve, Control) process. Measurement of errors started from the time the CT order was verified by the PT to the time of CT administration by the nurse. Data collection included retrospective error tracking software, system audits, and staff surveys. Results: Root causes of CTMRs included inadequate knowledge of CT mixing protocol, inconsistencies in checking methods, and frequent changes in staffing of clinics. Initial CTMRs (n = 33,259) constituted 0.050%, with 77% of these errors affecting patients. The action plan included checklists, education, and competency testing. The postimplementation error rate (n = 33,376, annualized) over a 3-month period was reduced to 0.019%, with only 15% of errors affecting patients. Initial Sigma was calculated at 4.2; this process resulted in the improvement of Sigma to 5.2, representing a 100-fold reduction. Financial analysis demonstrated a reduction in annualized loss of revenue (administration charges and drug wastage) from $11,537.95 (Medicare Average Sales Price) before the start of the project to $1,262.40. Conclusion: The Six Sigma process is a powerful technique in the reduction of CTMRs.

Objective. In a cohort of 70 patients with childhood-onset systemic lupus erythematosus (cSLE): to determine the baseline adherence to medications and visits; to investigate the effects of cellular text messaging reminders (CTMR) on adherence to clinic visits; and to study the influence of CTMR on adherence to use of hydroxychloroquine (HCQ).Methods.CTMR were sent to 70 patients prior to clinic visits for 14 months. A subgroup of patients were evaluated for medication adherence to HCQ: 19 patients receiving CTMR prior to each scheduled HCQ dose were compared to 22 patients randomized to standard of care education about HCQ. Visit adherence was measured using administrative databases. Pharmacy refill information, self-report of adherence, and HCQ blood levels were utilized to monitor medication adherence to HCQ. Sufficient adherence to visits or HCQ was defined as estimates > 80%. Disease activity was primarily monitored with the Systemic Lupus Erythematosus Disease Activity Index.Results.At baseline, 32% of patients were sufficiently adherent to HCQ, and 81% to clinic visits. Visit adherence improved significantly by > 80% among those who were nonadherent to clinic visits at the baseline CTMR (p = 0.01). CTMR did not influence adherence to HCQ over time.Conclusion.Patients with cSLE were only modestly adherent to HCQ and clinic visits. CTMR may be effective for improving visit adherence among adolescents and young adults with cSLE, but it does not improve adherence to HCQ.

The Chinese Tianshan mountainous region (CTMR) is a typical alpine region with high topographic heterogeneity, characterized by a large altitude span, complex topography, and diverse landscapes. A significant increase in air temperature had occurred in the CTMR during the last five decades. However, the detailed, comprehensive, and systematical characteristics of climate warming, such as its temporal and spatial heterogeneity, remain unclear. In this study, the temporal and spatial heterogeneity of climate warming across the CTMR had been comprehensively analyzed based on the 10-day air temperature data gathered during 1961–2020 from 26 meteorological stations. The results revealed local cooling in the context of general warming in the CTMR. The amplitude of variation (AV) varied from −0.57 to 3.64 °C, with the average value of 1.19 °C during the last six decades. The lapse rates of the elevation-dependent warming that existed annually, and in spring, summer, and autumn are −0.5 °C/100 m, −0.5 °C/100 m, −0.7 °C/100 m, and −0.4 °C/100 m, respectively. The warming in the CTMR is characteristic of high temporal heterogeneity, as represented by the amplified warming at 10-d scale for more than half a year, and the values of AV were higher than 1.09 °C of the global warming during 2011–2020 (GWV2011–2020). Meanwhile, the amplitudes of warming differed greatly on a seasonal scale, with the rates in spring, autumn, and winter higher than that in summer. The large spatial heterogeneity of climate warming also occurred across the CTMR. The warming pole existed in the warm part, the Turpan-Hami basin (below 1000 m asl) where the air temperature itself was high. That is, the warm places were warmer across the CTMR. The cooling pole was also found in the Kuqa region (about 1000 m asl). This study could greatly improve the understanding of the spatio-temporal dynamics, patterns, and regional heterogeneity of climate warming across the CTMR and even northwest China.

This study focuses on how relationships among constructs representing (1) consumer trust in market-related institutions (CTMRI), (2) distrust for individuals (DFI), and (3) subjective quality of life (QOL) differ across groups separated by the poverty line in a developing country (Turkey). A comparison of models across the two groups using multisample confirmatory factor analysis indicates that there is a correlation only between CTMRI and QOL for consumers below the poverty line (r = .43); there are no correlations between any of the three constructs for consumers above the poverty line. Accordingly, there is a unique relationship between QOL and CTMRI among financially constrained consumers in a developing country. Below the poverty line, consumers with lower trust in market-related institutions tend to report lower QOL, while those with higher trust in market-related institutions tend to report higher QOL.

Background: Connective tissue manipulation (CTM) has therapeutic effects on diseases with autonomic imbalance, yet its mechanisms of action are not clearly identified. Objective: The aim of this study was to investigate acute autonomic responses to CTM in healthy young women with various physical activity levels. Method: The study was designed as a nonrandomized, controlled single-center study. Healthy women aged between 18 and 25 years were assigned to a connective tissue manipulation group (CTMG) (n = 150) or a control group (CG) (n = 60). CTM was applied to the CTMG while the CG did not receive any intervention. Respiratory rate (RR), heart rate, systolic/diastolic blood pressures (SP/DP), oxygen saturation (OS) and body temperature were measured. The International Physical Activity Questionnaire short form was used to determine physical activity levels. Results: Analysis revealed significant reduction in SP and DP and an increment in RR in the CTMG (p < 0.05). RR increased and SP decreased among inactive, SP decreased among moderately active, and SP and OS decreased among highly active participants in the CTMG (p < 0.05). There were no significant alterations in the CG (p > 0.05). Conclusions: CTM has an immediate reducing effect on sympathetic activity in healthy young women independently from the physical activity level. Future studies are needed to clarify long-term effects of CTM on autonomic functions in healthy individuals.

Arid and semiarid regions are vulnerable to water deficits and salinity. Citrus plants are sensitive to saline stress and require the use of tolerant scion-rootstock combinations. Thus, this study aimed to evaluate and classify citrus scion-rootstock combinations with respect to their tolerance to salinity during seedling formation in a protected environment. An experiment was conducted in a randomized block design with a 5 x 12 x 2 factorial scheme corresponding to five levels of water salinity (0.8, 1.6, 2.4, 3.2, and 4.0 dS m-1) applied in 12 citrus rootstocks grafted with two scion varieties: ‘Tahiti’ acid lime and ‘Star Ruby’ grapefruit. The scion-rootstock combinations were evaluated for accumulated dry matter and survival index at 330 days after sowing the rootstocks. Salinity exerted different effects on the dry matter formation of scion-rootstock combinations. ‘Star Ruby’ was less sensitive to salinity, particularly when the rootstocks were the hybrids from ‘Sunki of Florida’ mandarin (TSKFL) with ‘Troyer’ citrange (CTTR) – 013 (TSKFL x CTTR – 013), common ‘Sunki’ mandarin (TSKC) with ‘Argentina’ citrange (CTARG) – 019 (TSKC x CTARG – 019), TSKC with ‘Swingle’ citrumelo (CTSW) – 031 (TSKC x CTSW – 031), and the trifoliate hybrid (HTR) - 069, as well as the varieties Volkamer lemon and Santa Cruz Rangpur lime. When grafted with ‘Tahiti’ acid lime, the rootstocks displaying the less sensitive to salinity were TSKFL x CTTR - 013 and TSKC x CTARG - 019.

We have synthesised short analogues of the marine antimicrobial peptide Turgencin A from the colonial Arctic ascidian Synoicum turgens. In this study, we focused on a central, cationic 12-residue Cys-Cys loop region within the sequence. Modified (tryptophan- and arginine-enriched) linear peptides were compared with Cys-Cys cyclic derivatives, and both linear and Cys-cyclic peptides were N-terminally acylated with octanoic acid (C8), decanoic acid (C10) or dodecanoic acid (C12). The highest antimicrobial potency was achieved by introducing dodecanoic acid to a cyclic Turgencin A analogue with low intrinsic hydrophobicity, and by introducing octanoic acid to a cyclic analogue displaying a higher intrinsic hydrophobicity. Among all tested synthetic Turgencin A lipopeptide analogues, the most promising candidates regarding both antimicrobial and haemolytic activity were C12-cTurg-1 and C8-cTurg-2. These optimized cyclic lipopeptides displayed minimum inhibitory concentrations of 4 µg/mL against Staphylococcus aureus, Escherichia coli and the fungus Rhodothorula sp. Mode of action studies on bacteria showed a rapid membrane disruption and bactericidal effect of the cyclic lipopeptides. Haemolytic activity against human erythrocytes was low, indicating favorable selective targeting of bacterial cells.

Invertase, whether adsorbed on styrene-divinylbenzene copolymers or otherwise, was used for continuous sucrose hydrolysis using a cell-type membrane reactor (CTMR), coupled with an ultra (UF-100kDa), or a microfiltration (MF- pore diameter of 5 µm) membrane. In all tests, the pH (5.5), temperature (30 ºC), reaction volume (10 mL) and agitation (100 rpm) were set constant; whereas, variable parameters were: feeding rate (0.4, 0.8 and 1.6 h-1), inlet sucrose concentration (2.5, 6.5, 50 and 100 mM) and enzyme/resin ratio (1.64 mg or 3.28 mg of protein per 25, 50 or 100 mg of resin). The best result (yield of 100%, steady-state duration over 20h and specific reaction rate over 243 x 10-3 mmol/h.mE) was obtained when insoluble invertase (1.64 mg protein/100 mg resin) was used to convert 50 mM or 100 mM of sucrose solution at 0.4 h-1 using a UF-CTMR.

The scarcity of good quality water is a limiting factor for irrigated agriculture, especially in arid and semiarid regions, where water sources generally contain high concentrations of ions. In such conditions, it is essential to cultivate genotypes with economic potential and tolerance to salinity. Considering the importance of citrus and the need to identify genetic materials that adapt to saline stress, this study evaluates the salinity tolerance of 10 genotypes with rootstock potential. For the irrigation water, five levels of electrical conductivity (ECw: 0.8, 1.6, 2.4, 3.2 and 4.0 dS/m at 25 °C) were used in a randomized block design with three replications, and seed germination and growth variables, as well as physiological plant parameters, were evaluated. The germination rate of the hybrid TSKC × CTARG – 019 was the best under salt conditions, as it was the genotype with the highest tolerance to salinity in the germination stage. Salinity reduced the growth of the citrus genotypes, with the possibility of using water with EC of up to 1.6 dS/m in the rootstock formation stage. Salt stress affected the photosynthetic rate of the genotypes ‘San Diego’ citrandarin and TSKC × CTSW – 018 by reducing the stomatal conductance, restricting the diffusion of CO2 into the substomatal camera. The genotypes TSKC × CTARG – 019, TSKC × CTTR – 012 and TSKC × TRBK – 007 have the greatest potential for growth and photosynthetic apparatus efficiency when subjected to salinity.

Purpose. This study quantified the expression of substance P (SP), calcitonin gene-related peptide (CGRP),β-endorphins (β-End), and methionine-enkephalin (Met-Enk) in human dental pulp following orthodontic intrusion.Methods. Eight patients were selected according to preestablished inclusion criteria. From each patient, two premolars (indicated for extraction due to orthodontic reasons) were randomly assigned to two different groups: the asymptomatic inflammation group (EXPg), which would undergo controlled intrusive force for seven days, and the control group (CTRg), which was used to determine the basal levels of each substance. Once extracted, dental pulp tissue was prepared to determine the expression levels of both neuropeptides and endogenous opioids by radioimmunoassay (RIA).Results. All samples from the CTRg exhibited basal levels of both neuropeptides and endogenous opioids. By day seven, all patients were asymptomatic, even when all orthodontic-intrusive devices were still active. In the EXPg, the SP and CGRP exhibited statistically significant different levels. Although none of the endogenous opioids showed statistically significant differences, they all expressed increasing trends in the EXPg.Conclusions. SP and CGRP were identified in dental pulp after seven days of controlled orthodontic intrusion movement, even in the absence of pain.

Recent developments in reinforcement learning for non-Markovianproblems witness a surge in history-based methods, among which weare particularly interested in two frameworks, PhiMDP and MC-AIXI-CTW. PhiMDP attempts to reduce the general RL problem, where the environment's states and dynamics are both unknown, toan MDP, while MC-AIXI-CTW incrementally learns a mixture of contexttrees as its environment model. The main idea of PhiMDP is toconnect generic reinforcement learning with classical reinforcementlearning. The first implementation of PhiMDP relies on astochastic search procedure for finding a tree that minimizes acertain cost function. This does not guarantee finding theminimizing tree, or even a good one, given limited search time. As aconsequence it appears that the approach has difficulties with largedomains. MC-AIXI-CTW is attractive in that it can incrementally andanalytically compute the internal model through interactions withthe environment. Unfortunately, it is computationally demanding dueto requiring heavy planning simulations at every single time step.We devise a novel approach called CTMRL, which analytically andefficiently finds the cost-minimizing tree. Instead of thecontext-tree weighting method that MC-AIXI-CTW is based on, we usethe closely related context-tree maximizing algorithm that selectsjust one single tree. This approach falls under the PhiMDPframework, which allows the replacement of the costly planningcomponent of MC-AIXI-CTW with simple Q-Learning. Our empiricalinvestigation show that CTMRL finds policies of quality as good as MC-AIXI-CTW's on sixdomains including a challenging Pacman domain, but in an order ofmagnitude less time.

<p class="Abstract">Mobile Ad-hoc Networks (MANETs) involved in many applications, whether smart or traditional and for both civilian and military uses, and that because of their special features, where it does not depend on any infrastructure during its working, as well as the nodes in MANETs have a freedom of movement with the ability to self-configure, in addition, to working as a router or client at the same time. Moreover, MANETs considered as an infrastructure less network, so the cost of this type of networks is less in comparison to other traditional networks. On the other hand, the routing considered one of most important challenges in MANETs due to the perpetual motion and randomness of the nodes that can causing a continuous change of the network topology and thus to all paths between nodes, where finding valid paths between the nodes is the core task of routing protocols. Recently, it has been argued that the traditional layered architecture is ineffective to deal with receiving signal strength related problems. In an effort to improve the performance of MANETs, there has been increased in protocols that rely on cross-layer interaction between different layers. In this paper, a Cross-layer design among Network, MAC and Physical layers based on Threshold Multipath Routing Protocol (CTMRP) is proposed. The CTMRP is designed for decision maker based on threshold value of average paths signal for efficient transmission of the Text, image, audio and video as well as sending the data via multiple paths, which mitigate the negatives effects causes from forcing the nodes to send the data via single. The Route Discovery Delay, Number of RREQ Messages, Number of RREP Messages, End-to-End Delay, Packet Delivery Ratio (PDR), and Throughput were selected as the main performance evaluation metrics. The results show that the proposed algorithm has better performance and lead to increase stability of transmission link.</p>

ABSTRACT The objective was to study the biochemical changes and dry matter content in lemon and mandarin hybrids under salt stress during rootstock formation. For this, a study was conducted in randomized complete block, using a 2 x 5 factorial scheme, with two salinity levels (0.3 and 4.0 dS m-1) applied in five citrus rootstock genotypes (1. TSKC x CTARG - 019; 2. LRF; 3. TSKC x (LCR x TR) - 040; 4. LCRSTC and 5. LVK), with three replicates and four plants per plot. At 90 days after sowing, saline treatments started to be applied and continued until 120 days after sowing, the moment in which the plants were collected for evaluation of biochemical characteristics and phytomass accumulation. The increase in water salinity negatively affected the biochemical components and dry matter accumulation of citrus genotypes. The genotypes TSKC x (LCR x TR) - 040, LCRSTC and LVK were the least affected by salt stress, standing out as the materials most tolerant to salinity.

14105 Background: Cytotoxic chemotherapy has been widely used in patients (pts) with advanced or metastatic gastric cancer, but the reported high response rates in phase II settings have not been confirmed by randomized trials. Moreover, the therapeutic impact on survival has been modest. The purpose of this study was to evaluate Irofulven, a DNA interacting acylfulvene analog, as first line therapy for pts with recurrent or metastatic gastric cancer. Methods: The main eligibility criteria were: Histologically or cytologically confirmed disease; age > 18 yrs; ECOG ≤ 2; measurable disease; no prior chemotherapy for recurrent or metastatic disease; life expectancy > 3 months. Irofulven was given at 0.45 mg/kg i.v. over 30-min infusion (max. 50 mg), on Day 1 and 8, every 3 weeks. The primary endpoint was treatment response and toxicity; the secondary end-point was survival. A 2-stage phase II design was used. In the first stage, the target enrollment was 20, if ≤ 2 responses were observed, the study would be stopped and the treatment concluded to be ineffective; if ≥ 6 responses were observed, the study drug would be concluded to be active. Otherwise, another 15 pts would be entered into the second stage. Survival was constructed using the Kaplan-Meier method. All patients entering the trial were included in the survival analysis. Results: 23 pts were entered into the first stage. The median no. of cycles per pt was 2 (range: 1–6). 2 pts (9%) had ≥ 1 week delay in administration of subsequent cycle of chemotherapy. For the day 8 chemotherapy, 7 pts (30%) required dose reductions; 5 (22%) had dose omitted. 22% and 17% pts developed grade 3/4 anemia and neutropenia respectively. There was no grade 4 thrombocytopenia or neutropenic fever. Of the 20 evaluable pts, no responses was observed, 3 pts had stable disease after 2 cycles of treatment which was not confirmed by a further assessment. Median overall survival was 6.05 months (95% CI 4.55–9.39). Conclusions: Irofulven was tolerated at the described dose but showed no evidence of antitumor activity in patients with advanced gastric cancer. Recent data obtained in the authors' laboratories concerning the behaviour of mobile genetic elements of Drosophila melanogaster are reviewed. It was found that the mobile element jockey represents the typical LINE element. It is efficiently transcribed in D. melanogaster cells in flies and in culture. Transcription is initiated from the +1 nucleotide of jockey and depends on an internal promoter. This is the first case of an internal promoter being used by RNA polymerase II. Several events which take place during the transposition bursts in ctMR2 family of strains were described. Among them are the removal of mobile dispersed genetics (mdg) elements (with solo long terminal repeat (LTR) remaining at the site of excision), complete removal of an mdg element, and reinsertion of the same mdg to the same place either in the presence or in absence of solo LTR sequence. Finally, the formation of deletions was observed. A 462-bp deletion destroying the white locus can be further repaired (w+ reversion). Thus, transposition bursts include many different genetic events. A novel system of prolonged genome destabilization was described. It depends on mobilization of a new mobile element called Stalker. After certain crosses Stalker actively moves for dozens of generations giving rise to large numbers of insertion mutations. Several novel genes were detected using mobilized Stalker. They include a modifier of mdg4 and six enhancers of yellow mutations.Key words: Drosophila melanogaster, mobile elements, transcription.

The enzyme-catalyzed hydrolysis of carboxy-methyl cellulose (CMC) was performed in three different types of reactors; in a batch stirred-tank reactor (BSTR) operating at atmospheric pressure, in a high-pressure batch stirred-tank reactor (HP BSTR) and in a high-pressure continuous tubular-membrane reactor (HP CTMR). In the high-pressure reactors aqueous SC CO2 was used as the reaction medium. The aim of our research was optimization of the reaction parameters for reaction performance. All the reactions were catalyzed by cellulase from Humicola insolens. Glucose production in the high-pressure batch stirred-tank reactor was faster than in the BSTR at atmospheric pressure. The optimal temperature for the reaction performed in the BSTR at atmospheric pressure was 30?C, while the optimal temperature for the reaction performed in SC CO2 was 32?C. The influence of the application of tubular ceramic membranes in the high-pressure reaction system was studied on the model reaction of CMC hydrolysis at atmospheric pressure and in SC CO2. The reaction was catalyzed by cellulase from Humicola insolens covalently linked to the surface of the ceramic membrane. The hydrolysis of CMC in SC CO2 and at atmospheric pressure was performed for a long time period. The reaction carried out in SC CO2 was more productive than the reaction performed at atmospheric pressure.

259 Background: Belinostat is a novel histone deactylase inhibitor which demonstrates preclinical activity in HCC. We report the results of a phase I/II study on belinostat in patients (pts) with unresectable HCC. Methods: Major eligibility criteria included histologically confirmed HCC not amenable to curative treatment; PS ≤ 2; adequate organ function; prior systemic therapy was allowed. In the phase I portion, belinostat was given i.v. on D1-5 every 3 weeks with dose levels of 600, 900, 1200 and 1400 mg/m2/day. In the phase II portion, belinostat was tested at the MTD. Primary endpoint was PFS and secondary endpoints were RR according to RECIST and OS. CT assessment was done every 8 weeks. Results: Phase I portion: a total of 18 pts were accrued; no DLTs were observed at 1400mg/m2/day for 5 days, and this dose was selected for phase II development. Phase II portion: 42 pts were accrued; Median age = 57.5 years; 41 had Child’s A function, and 24 pt had ECOG 0. Sixteen (38%) had previous systemic therapy, and 21 (50%) had previous transarterial therapy. Median follow-up was 20.0 months. The PR and SD rate was 2.4% (1/42) and 45.2% (19/42). Median PFS was 2.64 months (95%C.I. 1.55-3.17) and OS was 6.60 months (95%C.I. 4.53-11.60). Grade 3 or higher toxicities (>5% rate) were abdominal pain (9.5%), (9.5%) hyperbilirubinemia (9.5%), raised ALT (9.5%); anemia (7.1%) and vomiting (7.1%). Conclusions: Belinostat demonstrates disease stabilization in a predominantly pretreated population of pts with unresectable HCC with an acceptable safety profile. Further randomized studies are warranted.Supported in part by N01-CM-62205.

The judgment of the General Court of the European Union (Sixth Chamber), Case T 696/15 of 9 February 2017, against EUIPO examines, among the procedural aspects of the European trade mark regime, non-registration of a wine product in the event of non-compliance with the essential, in particular absolute, requirements for the acquisition of a mark in the territory of the European Union. In examining the various provisions at European level which provide for the granting or denial of trademarks and for the purpose of ensuring that geographical indications are fully effective in the European trade mark registration proceedings, this is to prevent holders from incurring any the grounds for annulment, Regulation Nº 207/2009 on the Community trade mark (hereinafter referred to as the CTMR), 7. 1, (j), points out that they are excluded from the register ‘wine marks containing or consisting of a geographical indication identifying wines, or marks of spirit drinks containing or consisting of a geographical indication identifying the spirits, where such wines or drinks do not have such origin’. The inadmissibility of the application for a word mark which contains a geographical indication which does not belong to it and also coincides with other appellations of origin is, inter alia, the central argument of the judicial issue under consideration. The legal question arose in 2014, when a Spanish mercantile company (Bodegas Vega Sicilia) decided to register the word mark “Tempos Vega Sicilia” before the EUIPO which, however, denied the registration of the same because it the term “Sicilia” is part of the protected designations of origin for wines. Hence, the mercantile company decided to appeal in 2015 to the EU General Court, which rejected the appeal by ruling of 9 February 2017, also rejecting, as the EUIPO did at the time, the arguments of the applicant relating to the existence of a family of registered trade marks prior to the registration of the DO or the potential popularity of said family of trademarks and guaranteeing maximum protection of protected geographical denominations and, therefore, denying registration when, when included in a trademark, it was not limited to the product of the said geographical area.

One of the causes of infertility in females is tubal pathology as a result of pelvic inflammatory disease (PID) caused by Chlamydia trachomatis (Ct). Diagnosis and identification of patients is hampered by the lack of rapid, easy, sensitive and specific methods. The introduction of Ct-specific enzyme-linked immunosorbent assay (ELISA) tests, based upon synthetic peptides may subsequently increase the sensitivity and specificity for the detection of tubal factor infertility caused by Ct. In order to determine the value of these tests for serological diagnosis of Ct infections, we evaluated several commercially available assays (C. trachomatis enzyme immunoassay (EIA), Labsystems (CtL); SeroCT, Savyon (CtS); pELISA, Medac (CtMp); and a reference assay rELISA, Medac (CtMr)) in two study populations. The first group consisted of 134 female patients with infertility problems. Tubal factor infertility was observed in 85 of these patients (63%). A higher % positivity was found for Ct-specific IgG for the CtL, CtS and CtMp, 41% vs 10%, 57% vs 18% and 55% vs 25% respectively as compared to patients with infertility due to other problems. A similar trend was observed for Ct-specific IgA. The specificity of Ct-specific IgA and IgG in this patient group varied between 92 to 98% and 76 to 90%, respectively. The second group consisted of 107 consecutive gynaecology patients with fertility problems or suspected PID. In this particular patient group, the specificity of the peptide based tests were around 80% and 90% for Ct-specific IgA and 75% and 85% for Ct-specific IgG, respectively. The negative predictive values exceeded 90%, while the positive predictive values varied from 30% to 47% for Ct-specific IgA and was around 30% for Ct-specific IgG. Testing Ct-specific IgG had no additional value above Ct-specific IgA alone. We conclude that the new synthetic peptide-based EIA tests are able to detect species-specific antibodies, which are correlated to (active) infection, and that in particular IgA may be useful in the serodiagnosis of tubal factor infertility caused by C. trachomatis, and will contribute in simplifying the work-up in patients with infertility.

15081 Background: Hepatocellular carcinoma (HCC) is a common cause of cancer morbidity and mortality. It is a highly aggressive tumor with 90% presenting with unresectable disease, resulting in a median survival of 3–6 months. Inhibitors of histone deacetylase (HDAC) have been demonstrated in HCC cell lines and xenografts to induce apoptosis and tumor regression, and have anti-proliferative, anti-metastatic and anti-invasive effects. Belinostat (N-hydroxy-3-[phenylsulphamoylphenyl] acryl amide) is a novel, low molecular weight, HDAC inhibitor. The Cancer Therapeutic Research Group (CTRG) is conducting a phase I/II study of belinostat as the first line therapy for pts with unresectable HCC. The phase I study aims to determine dose limiting toxicity (DLT) and maximum tolerated dose (MTD). Methods: Patient eligibilities include histologically or cytologically confirmed unresectable HCC, ECOG = 2, adequate hematologic, renal and hepatic functions. The dose of belinostat was started at 600 mg/m2/day i.v. on day 1–5 every 3 weeks; with increment of 300 mg/m2/day up to 1200 mg/m2/day. Dose limiting toxicities (DLT) are defined as grade 4 hematological toxicity or grade 3 or 4 nonhematological toxicity during cycle 1 (according to NCI CTC v3), or treatment delay >2 weeks. The MTD is defined as the dose below which ≥ 2 of 3 or ≥ 2 of 6 patients experiencing DLT. After determination of the MTD, 3 additional patients will be treated at this dose to further define toxicity. Results: From June-December 2006, 12 pts were entered; 3 were treated at level I (600 mg/m2/day), 3 at level 2 (900 mg/m2/day), and 6 at level 3 (1200 mg/m2/day). There were no DLTs on level 1, 2 or 3. Therefore, the MTD of belinostat was not reached. Overall toxicities were WBC gr. 0/1/2/3/4: 12/0/0/0/0, ANC gr. 0/1/2/3/4: 12/0/0/0/0; Hb gr. 0/1/2/3/4: 4/7/0/1/0; platelet gr. 0/1/2/3/4: 8/4/0/0/0; lethargy gr. 0/1/2/3: 12/0/0/0/0, phlebitis gr. 0/1/2/3: 10/0/2/0; Cough gr. 0/1/2/3: 9/0/3/0; Nausea gr. 0/1/2/3: 7/3/2/0 and vomiting gr. 0/1/2/3: 9/3/0/0. Conclusions: At the maximum dose of 1200 mg/m2/day (level 3), MTD has not been reached. Belinostat is very well tolerated. Phase II study will be started at dose level 1200 mg/m2/day. Objectives The study aimed to evaluate the cutaneous trunci muscle reflex (CTMR) in healthy cats using methods performed by the clinician and the cat’s owner at home. Methods A total of 55 healthy cats without neurological abnormalities were included in this study. CTMR evaluation was performed sequentially in each cat using three methods by a clinician: method A, pinch skin with a straight 14 cm Crile haemostat forceps; method B, displace fur with the tip of a pen or haemostat forceps; and method C, poke skin with the tip of a straightened paper clip. The normal response rates for each method were obtained and compared. A ‘CTMR performance score’ was assigned for each cat, reflecting the presence of a normal CTMR response using one or more of the three methods. An ‘owner performance score’ was also obtained, reflecting the response of the CTMR when performed at home by the cat owner. The two scores were compared as paired data for each cat. Results The CTMR was elicited normally in 17 (31%), 27 (49%) and 16 (29%) cats using methods A, B and C, respectively. Method B delivered a significantly higher percentage of normal responses. When comparing the ‘CTMR performance score’ and ‘owner performance score’, the percentage of normal responses was 60% and 100%, respectively, which was significantly different. Conclusions and relevance The overall normal response rate of the CTMR in healthy feline subjects was low when performed by a clinician, regardless of the method applied. Conversely, a high percentage of normal responses was obtained by cat owners performing CTMR at home, potentially indicating the impact of stress on the CTMR performance.

Purpose The aim of this study was to evaluate sensitivity and specificity of left ventricular (LV) pressure–strain loop (PSL) area, which reflects regional myocardial work and metabolic demand, in predicting subtle myocardial abnormalities in KD patients with coronaries aneurisms. Methods A total of 88 patients (59 male, age 8.95 ± 4.95 years) were included in our study. Among the children admitted in our institution with a diagnosis of KD during the study time frame, 42 patients (KDg) (29 male) with coronary artery dilatation (Z-score &gt;2.5) were selected. These cases were compared with 46 (30 male) age-matched controls (CTRg). Classical echocardiographic parameters of LV systolic function were normal in both groups, while global longitudinal strain (GLS) was decreased in 6 KD patients. Global work index (GWI) was calculated as the area of the LV PSL. From GWI, it was estimated also Global Constructive Work (GCW), Global Wasted Work (GWW) and Global Work Efficiency (GWE). We also made a subgroup analysis between KD patients with normal GLS (&gt; -19 %) and control patients. Results Despite normal LV systolic function by routine echocardiography, compared to controls, KD patients had lower GWI (1448 ± 382 mmHg% in KDg vs 1751 ± 263] mmHg% in CTRg, p = 0.00003), GCW (1904 ± 390 mmHg% in KDg vs 2174 ± 292 mmHg% in CTRg, p= 0.0004) and GWE (94 ± 5 % in KDg vs 96 ± 2 % in CTRg, p= 0.01). There was not significant difference in GWW between the KDg and CTRg. When KD patients with normal GLS were analysed separately, they preserved a significant difference in GWI, GCW and GWE in comparison with controls (GWI: 1490 ± 347 mmHg% in KDg vs 1751 ± 263 mmHg% in CTRg, p = 0.0002; GCW: 1972 ± 321 mmHg% in KDg vs 2174 ± 292 mmHg% in CTRg, p = 0.004; GWE: 95 ± 3 in KDg vs 96 ± 2 % in CTRg, p= 0.04). No association was found between GWI, GCW, GWW, GWE and number or dimension of the involved aneurysmatic coronaries. Conclusions The estimation of myocardial work by PSL is a novel tool for the evaluation of patients with KD. GWI, GCW and GWE were significantly reduced in KD patients with dilated coronaries. In KD patients with normal GLS, estimation of GWI, GCW and GWE may be a sensitive indicator of myocardial dysfunction and an adjuvant criterion to avoid delayed diagnosis of KD. Abstract P316 Figure. Abstract Funding Acknowledgements Type of funding sources: None. Background Blood speckle imaging (BSI) is a new speckle-tracking-based technique for the evaluation of blood flow. Ventricular flow vortices have been studied in sinus rhythm in normal and dysfunctional hearts, however, data are lacking in patients with Kent accessory pathway (KAP) and short atrio-ventricular (AV) conduction. Purpose We aimed to evaluate the characteristics of left ventricle blood vortices in children with KAP. Methods Nineteen paediatric patients (age 7 ± 2.9 years) were included in the study: 13 patients with manifest KAP (KAP group) and 6 age and sex -matched controls with normal AV conduction (CTR group). A thorough echocardiographic evaluation with 2D, color doppler and BSI was performed in all the included patients. BSI was recorded in apical 3-chamber view with a 6S-D probe. Vortices characteristic were analysed during filling phase of the left ventricle. We focused on the anterior vortex generated by mitral valve, which persisted longer during the cardiac cycle and is assumed to contribute to optimizing cardiac function. A standard 12-lead ECG was also recorded for each child in KAP group to esteem KAP localization. Results According to Arruda criteria for KAP localization, all patients in KAP group manifested the accessory pathway in the septal region. All patients in CTR group presented one major anterior vortex during filling phase, while in KAP group 10 patients out of 13 (p= 0.009) lacked of this main anterior vortex, showing instead fragmented different vortices. There were no differences in term of left ventricle function (KAPg 59.8 ± 4.02% vs CTRg 59.0 ± 2.5%, p= 0.6) and global longitudinal strain (KAPg -18.6 ± 1.0% vs CTRg -19.6 ± 3.1%, p= 0.5) between the two groups. Conclusion In our preliminary analysis, in patients with septal KAP, blood vortices adapted their diastolic traces to the septal dyssynchrony showing a fragmented pattern. We speculate this fragmentation may contribute to impair the performance of the left ventricle. Abstract Figure. BSI in control vs patient with WPW

Abstract Aims Wolff–Parkinson–White syndrome (WPW) is a clinical condition characterized by pre-excitation on electrocardiogram (ECG) and symptoms of arrhythmias. It has been described that premature ventricular activation induces septal wall motion abnormalities and ventricular dyssynchrony, causing detrimental effects on cardiac performance. The new speckle-tracking-based technique, blood speckle imaging (BSI), has been previously used to evaluate blood flow characteristics in normal and dysfunctional hearts in patients with normal atrio-ventricular (AV) conduction. We aimed to study the features of left ventricle blood vortices in patients with WPW and short AV conduction by using BSI. Methods and results Nineteen paediatric patients (age 7 ± 2.9 years) were included in the study: 13 patients with manifest WPW (WPW group) and 6 age and sex -matched controls with normal AV conduction (CTR group). A complete echocardiographic evaluation with 2D, color Doppler, and BSI was performed in all the included patients. BSI was recorded in apical 3-chamber view with a 6S-D probe. Vortices characteristics were analysed during the filling phase of the left ventricle. We focused on the anterior vortex generated by the mitral valve, which persisted longer during the cardiac cycle and is assumed to contribute to optimizing cardiac function. For each child in the WPW group, Arruda criteria were used to esteem accessory pathway localization from a 12-lead surface ECG. All patients in the WPW group manifested the accessory pathway in the septal region. All patients in the CTR group presented one major anterior vortex along the septal wall during the filling phase (Figure A), while in the WPW group, 10 patients out of 13 (P = 0.009) lacked this main anterior vortex, showing instead fragmented vortices (Figure B). Interestingly, the main differences in blood vortices pattern were showed in the last part of filling phase, when blood flow could be affected by premature activation of the septal wall. There were no differences in terms of left ventricle function (WPWg 59.8 ± 4.02% vs. CTRg 59.0 ± 2.5%, P = 0.6) and global longitudinal strain (WPWg −18.6 ± 1.0% vs. CTRg −19.6 ± 3.1%, P = 0.5) between the two groups. 136 Figure A and B Conclusions In our preliminary study, patients with WPW showed a fragmented pattern of diastolic blood vortices that adapted to septal dyssynchrony. We speculate this fragmentation may contribute to impair the performance of the left ventricle.
