Повний текст джерелаCardiorespiratory fitness is the primary element of a good physical condition. It regroups several systems: cardiovascular, pulmonary, muscular and mechanical, all of which are affected in the presence of severe obesity. The interpretation of the cardiorespiratory fitness among severe obese is still misunderstood. Up to now, the only effective treatment to counter severe obesity is bariatric surgery. Following a bariatric surgery, several parameters influencing cardiorespiratory fitness are affected. In the scientific literature, the heterogeneity of the evaluation methods to characterize cardiorespiratory fitness does not allow us to take position on its effect. In addition, there is no available information regarding the physical activity intervention following bariatric surgery. The general objective of this dissertation was to evaluate the impact of a structured and supervised training program on the cardiorespiratory fitness inpatients having been subject to bariatric surgery from the direct measure of their consumption of oxygen at the peak of effort (VO2peak). We have demonstrated that with severely obese individuals the major determinant of VO2peak was the pulmonary ventilation at maximum effort. Following the bariatric surgery, we were able to demonstrate the positive impact of a physical activity intervention on patients’ VO2peak. This effect seems to be caused by a better management of pulmonary ventilation, possibly linked to the maintained muscle mass in the intervention group. The results obtain in the context of this dissertation demonstrate the importance of the taking in charge in physical activity for individuals having been subject to bariatric surgery. A thorough analysis of the current literature, combined to our results, allows us to produce verified hypotheses regarding the ideal prescription of training following bariatric surgery. More research will be required in order to better understand the physiological parameters involved, such as the evaluation of physic-histochemical properties of muscle mass.

However, thé return to daily physical and leisure activities, recommended by most international guidelines, can be difficult for obèse youths, due to low self-esteem, difficulties in integrating groups and poor physical fitness. Overweight combined with sedentarity progressively modify cardio-respiratory exercise tolérance, rendering difficult thé return to exercise and contributing to non-compliance with prescriptions for physical activity. To meet thé growing needs of professionals in thé field of physical activity responsible for thé management of thèse subjects, thé main objective of this doctoral thesis was to validate clinical tools exploring cardiorespiratory capacity in obèse adolescent.A 12 minute walk/run test, an adapted 20m shuttle walk test (starting speed 4 km.b.-1,incréments : 0.5 km.h-1.miir1) and a submaximal exercise test by cycle ergometer with 4 stageswere proposed to calculate indices of cardiorespiratory fitness (D12, Vmax, PMA).Time-varying spectral analysis of heart rate variability was studied to estimate intensitiessubmaximal exercise near ventilatory thresholds.-The "Childhood Obesity Perceived Exertion Scale" (COPE-10) was used to quantify thé intensity of exercise as perceived by thé subjects during exercise.Thèse non-invasive tools, inexpensive, useful in thé évaluation and follow-up of cardiorespiratory fitness in obèse adolescents, can easily be used to develop and personalise adapted re-training programmes in thèse overweight teenagers

Повний текст джерелаThe cognitive decline that occurs during aging would be reduced by a high level of cardiorespiratory fitness. In this thesis, the underlying neurophysiological correlates of this prophylactic effect of cardiorespiratory fitness have been explored by near-infrared spectroscopy, which allows non-invasive measurement of cerebral hemodynamic activity. The objectives are firstly, to investigate the effects of task complexity, age and cardiorespiratory fitness level on the evolution of different indicators of cognitive performance, subjective perception of task difficulty and cerebral hemodynamic activity. Secondly, to examine the functional relationships that might exist between these different indicators. Throughout three experimental studies using different tasks involving executive control processes, whose level of complexity was manipulated, we measured these different indicators in young adults and seniors with high and low levels of VO2max. The results obtained showed that hemodynamic activity, cognitive performance and subjective perception of difficulty increase significantly as a function of the complexity of the cognitive tasks. This demonstrates the sensitivity of the different indicators to the manipulation of cognitive complexity. Furthermore, advancing in age induces a reduction in cognitive performance and hemodynamic activity but not in the subjective perception of difficulty. Also, our results indicated that cardiorespiratory fitness is linked to an increase in executive performance as well as in hemodynamic activity, only in seniors and for some tasks; the benefits being limited to the updating of working memory. Finally, the overall results have shown a possible link between hemodynamic activity and cognitive performance selective to young adults and seniors with a high level of VO2max when the experimental task is particularly complex; thus when the cognitive load is high. In sum, this work provides support for the understanding of the neurophysiological mechanisms underlying the prophylactic effects of cardiorespiratory fitness on the decline of the updating of working memory during normal aging

Повний текст джерелаPhysical activity promotion in pediatric oncology is an essential aspect of patient care. Indeed, physical activity significantly improves the functional capacity, musculoskeletal system, inflammatory system, immune system, cardiovascular system, as well as the psychological health of children with cancer and childhood cancer survivors. Physical activity in pediatric oncology is central to this thesis, which was part of an international joint supervision approach, answering several questions posed by children with cancer and childhood cancer survivors, in particular those with acute lymphoblastic leukemia (ALL). Among other things, "How does cancer and its treatments affect my health in the short, medium and long term?" And "How can physical activity help me?". To answer these questions, this thesis focuses on physiological, epidemiological, genetic, electrophysiological and psychological parameters. There are five works carried out within this thesis. They specify the effects of cancer treatments on the physical and psychological health of children with cancer and childhood ALL survivors. The first study in this thesis aims to explore the physiological differences between childhood cancer survivors (i.e., ALL survivors) and the healthy Canadian population. For these two populations, physical activity data, as well as cardiorespiratory fitness data, were compared in order to provide a better understanding of the daily challenges faced by childhood ALL survivors. The second study in this thesis provides answers to childhood ALL survivors’ physical deconditioning. This study is the first in the exercise and oncology field to explore the genetic association between childhood ALL survivors’ cardiorespiratory fitness and their trainability genes. These findings have an important impact on clinicians, patients, survivors and their families since they could allow, in the near future, to offer better follow-up to ALL patients, in line with their genetic profile and their cardiorespiratory fitness. The third study in this thesis explores the harmful long-term effects of cancer treatments on the electrophysiological parameters of ALL survivors. This study aims to better understand the physical deconditioning of these survivors, especially in regard to their autonomic nervous system which undergoes significant changes. These parameters, closely linked to the sympathetic and parasympathetic nervous system, are of paramount importance in the safe practice of physical activity. Finally, the last two studies are closely linked and follow up on the alarming observation made in ALL survivors. In this sense, these studies also stem from the interest of proposing a physical activity program in pediatric oncology. This work focuses on measures of the theory of planned behavior, self-esteem and perceived physical condition of children with cancer who are undergoing treatment. In this sense, the fourth article of this thesis documents the effects of cancer diagnosis on the psychological parameters mentioned above and shows the importance of providing children with cancer with physical activity support as soon as they are diagnosed. The fifth article can be considered as a follow-up to the fourth article since it proposes a supervised physical activity program in pediatric oncology. This latest study shows the positive effects of six weeks of physical activity on the physiological and psychological parameters of children with cancer. It shows the importance of family support to improve behavior linked to physical activity in children and alerts clinicians to the real and pressing need to provide children with cancer with support in physical activity as soon as they are diagnosed

Повний текст джерелаThe present thesis studied the effect of the mode of recovery on the improvement of the performance following intermittent exercises. Healthy and voluntary group of subjects aged 25,66 ± 2,39 years, weight 69,6 ± 8,6Kg and height of 176,7 ± 5,2cm took part in the experiment. The protocol consisted on 2 series of 4 repetitions of 30S at 120% of the MAP on ergocycle. Exercise repetitions are separated by 30S of passive recovery. Two periods of 4min - recovery were carried out between the 2 exercise series and the second series and exercise time limit (Tlim) assessment. During these last exercises, the recovery mode was randomly changed (passive: PR, active: AR or dynamic stretching: SR). The variables assesses were the VO2, MAP, the heart rate (HR), the blood lactate concentration [La] and Tlim as well as the RMS and the MPF from the electromyographic signals. The results show that lowest value of lactate was obtained with RS. The VO2 found during RS ranged between the value obtained during RA and RP. Concerning the FC, not difference was observed between RS and RA. The RMS values found were significantly higher than those obtained during Tlim following RP and RA. The MPF values were significantly lower compared at RA and RP attesting more important fatigue during SR. It is concluded that dynamic stretching recovery (SR) allowed respectively more improvement of Tlim (P<0. 01) than active recovery (AR, P<0. 05) in comparison to passive recovery

Il existe plusieurs types de leucémies, toutefois nous nous sommes intéressés à la leucémie lymphoblastique aiguë (LLA) pédiatrique. L’anomalie se trouve au niveau des cellules souches lymphoïdes, et se développe de manière soudaine, en quelques jours ou quelques semaines. De nombreuses conséquences néfastes sont connues au niveau cardiorespiratoire, musculaire, de l'énergie et de l’activité physique d’enfants en fin de chimiothérapie. L’objectif premier de l’étude était de mesurer les capacités cardiorespiratoires et musculaires réelles d’enfants atteints d’une LLA à trois temps de la chimiothérapie : l’induction (T1), la consolidation 2 (T2) et l’entretien (T3). Un deuxième objectif était de tester la faisabilité d’une épreuve d’effort maximal cardiorespiratoire pendant ou après l’induction. Le protocole de l’étude comprenait une épreuve d’effort physique sur ergocycle pour évaluer la consommation maximale d’oxygène (V ̇ O2max) un questionnaire sur la qualité de vie et un sur l’activité physique. La force musculaire était évaluée par un audit des dossiers (N=16) de physiothérapie d’anciens patients et des participants à l’étude. Une patiente a complété le test V ̇ O2max aux T1 et T2, et s’est améliorée de 54,5% entre les deux tests, mais restait nettement sous les valeurs normales. Pour l’ensemble des données de force musculaire : force de préhension, moment de force des extenseurs du genou et des fléchisseurs dorsaux de la cheville, les valeurs étaient inférieures à celles d’enfants sains, et avaient diminuées entre l’induction et la consolidation 2. Les résultats de ce mémoire démontrent que certains enfants atteints d’une LLA présentent un déconditionnement cardiorespiratoire et musculaire dès l’induction. Elles démontrent également la faisabilité d’un exercice cardiorespiratoire soutenu après la phase d’induction. Ces données importantes serviront aux professionnels de la santé dans la mise en place d’interventions en activité physique adaptée aux capacités d’enfants atteints d’une LLA.
Leukemia is a blood cancer and can be expressed in many forms, but the main focus of this thesis will be on acute lymphoblastic leukemia (ALL) in the pediatric population. The anomaly develops in the lymphoid stem cells and the disease develops rapidly, within a few days to a few weeks. Leukemia and chemotherapy have been shown to have many deleterious on the children cardiorespiratory and muscular status, energy level and engagement in physical activity mainly reported for children in the last phase of chemotherapy. The first aim of our work was to collect data on their cardiorespiratory and muscular status during three phases of the chemotherapy: induction (T1), consolidation 2 (T2) and maintenance (T3). The second aim was to test the feasibility of maximal oxygen consumption test ((V ̇ O2max) during or post induction. The protocol comprised a (V ̇ O2max) test on ergocycle, a quality of life and physical activity questionnaires. Data on muscular strength was collected through auditing the physical therapy files (n=16) of past and current patients. One participant completed the T1 and T2 (V ̇ O2max) testing and showed a 54.5% increase from T1 to T2. Despite this improvement, her score at T2 was still poor compared to her expected value. Data collected on muscular strength were: grip strength, moments of force of the knee extensors and the ankle dorsiflexors. They all showed lower values than expected compared to normative value at T1 and a decline from T1 to T2. The results outlined in this thesis show that children with ALL may present with decreased cardiorespiratory and muscular fitness with an onset as early as induction chemotherapy. Nonetheless, the results showed that high intensity cardiorespiratory exercise was feasible post induction. The data are of importance as they will help health care professionals build physical activity interventions adapted to the physical capacities of children with ALL.

Повний текст джерелаMany studies have reported that regular physical activity and cardiorespiratory fitness were associated with cognitive performance and more selectively with executive functions. Among numerous physiological mechanisms that may underlie the association between them, prefrontal cortex oxygenation seems to play a major role. However, the specific impact of prefrontal oxygenation on the link between physical activity and cognition is influenced by several factors, such as gender, age, or cardiovascular health. Therefore, the aim of this thesis was to investigate the impact of physical activity and cardiorespiratory fitness on executive functions across the adults' age span in healthy males, as well as the influence of prefrontal cortex oxygenation and cardiovascular health. To obtain those objectives, we conducted a review of the effect of physical activity on the brain in older adults and four cross-sectional studies. From our review, we highlighted that: • In older adults, higher fitness level is associated with better performance in several executive function tests. Even though the limited number of studies available makes it difficult to draw definitive conclusions.• Better cardiovascular fitness in older adults is associated with improve arterial stiffness, higher vascular reactivity, and greater amplitude of cerebral oxygenation during exercise or cognitive tasks.• Greater gray matter volume and white matter integrity were related to the cardiorespiratory fitness but less consistently related to physical activity.• At least 12 weeks of an aerobic exercise program are required to give advantageous effects to the brainAnd our experimental works show that:In young males:• The active individuals performed better in executive tasks than their inactive counterparts and had a larger change in prefrontal cortex oxygenation during the most complex conditions of Stroop task.• High cardiorespiratory fitness was related to a better performance in dual-task and greater oxygenation on both sides of the prefrontal cortex.In older males:• Cardiorespiratory fitness is not related to Stroop task performance nor prefrontal cortex oxygenation in overall older males.• Higher cardiorespiratory fitness was related to a better performance and greater right prefrontal cortex oxygenation during a Stroop task in 61+ years old group but not in 55-60 years old group, suggesting the importance of age-group classification to evaluate the effect of cardiorespiratory fitness on executive function in older male subjects.• Older males have stronger relationships between several vascular parameters and Stroop task performance than young malesThis work shows the relationship between physical activity and cardiorespiratory fitness on executive functions in young and older males. The potential neurophysiological mechanisms that underlie that relationship, especially prefrontal oxygenation and vascular health, are presented

Повний текст джерелаBackground: Older adults constitute one of the most rapidly growing population groups. Consequently, avoiding an inactive lifestyle and encouraging regular physical activity remains one of the main measures that should be promoted for older adults. The main objective of our study was to determine the effects of a new lightweight protocol including recovery bouts called: “The Intermittent Aerobic Training Program with Recovery bouts” (IATP-R - PEP’C-R in French) for seniors over 70 on maximal cardiorespiratory and endurance parameters. The secondary objectives were to determine the effects of IATP-R on vascular function and cognitive performances. Results: Our results showed that this new “IATP-R” training protocol permits to achieve a significant improvement in both endurance parameters and maximal cardio-respiratory parameters. In addition, the IATP-R permits to improve the vascular function and cognitive performances. Conclusion: The results of this study could be useful to implement this new IATP-R protocol for healthy older adults over 70. This would also be within the framework of prescribing training programs
