Повний текст джерелаFive principles, and the processes required to enable them, emerged from the qualitative data: 1) use and develop direct practice skills, 2) take a human rights perspective, 3) focus on goals and desired outcomes, 4) aim for quality of service delivery, and, 5) enhance and enable the role of key stakeholders. These principles are discussed and then applied to the case studies in order to consider their potential relevance to practice within a diverse community of CTO recipients. The application of the principles identified two further findings: 1) that the principles are interdependent, and 2) the relevance of the principles varies depending on the characteristics of the consumer. The two most important findings to emerge from this thesis are that: 1) people on CTOs, their family/carers, and service providers are a diverse community of people who have a range of problems, needs and preferences in relation to either being on a CTO or supporting someone on a CTO; and 2) the implementation of CTOs is influenced by social and structural issues that need to be considered in developing any recognition or understanding about what represents good practice. Recommendations relating to each of the principles are made, along with identification of future research questions. A particular focus is whether application of the principles will enable improvements in practice on a range of measures, including reducing the use of CTOs, and the experience of coercion by consumers.

Повний текст джерелаABSTRACT This dissertation has the following specific aim: to measure outcomes of social work emergency certificates produced by a Mobile Crisis Team to determine the effectiveness of these certificates at linking clients to services. Linkage to services is a programmatic goal and is achieved by ensuring clients receive adequate crisis and follow up services as a result of Emergency Certificates. In doing so, risks of homicide, suicide and grave disability are mitigated. Under its administrative umbrella, Mobile Crisis Team operates a police-social work collaboration known as Crisis Intervention Team, and both are administered by the State of Connecticut Department of Mental Health and Addiction Services in southeastern Connecticut. The data comes from 233 consecutive cases that involved Emergency Certificate's to transport clients to the local emergency department for psychiatric evaluation and treatment. Data was gathered post-hoc by reviewing copies of each specific certificate and cross-referencing those certificates with an agency risk management report and an electronic data base that stores demographic data on all clients. Descriptive, bivariate and multivariable analysis, such as cross tabulations and binary logistic regression, were used to analyze the data in this study. Additionally, the chi-square automatic interaction detector (CHAID) was used to construct outcome trees to describe subgroups of interest. This research is a continuation of previously published research on the various permutations and outcomes of mobile crisis programs, and contributes two unique programmatic features: the use by social workers of a legally proscribed coercive tool to send people to the emergency department for assessment, and the effectiveness of police-social work collaborations compared to social work only referrals. Overall, linkage occurs in over 80% of cases, with significant predictors of linkage identified as substance abuse, Hispanic ethnicity, criminal justice involvement, suicide, affective disorders, and insurance status. Implications for program evaluation, future research and limitations of the study are also discussed

Повний текст джерелаIntroduction: With the psychiatric deinstitutionalization and consequent restructuring of the mental health services, it is increasingly emphasized the need for evaluating services from the Network for Psychosocial Care. Given the complexity involved in health systems and mental health, evaluation is considered essential for quality assurance of the services. The World Health Organization (WHO ) emphasizes the need to incorporate the assessment of treatment outcomes as a continuous and ongoing practice, using assessment data to improve quality of care. Objective: This study aims to evaluate quantitatively the structure, work process, the organization of mental health care and outcomes of mental health care provided by the Psychosocial Care Center II for alcohol and other drugs (CAPSad) from the city of Botucatu . Method: The study was conducted in a CAPSad in Botucatu . To collect the data four instruments (coordinator , worker, family and user) were administered to the participants. Those instruments were composed by \" Satisfaction Scale - SATIS - BR \" Perceived Change Scale (EMP) and the Scale of Attitudes Towards Alcohol , Alcoholism and alcoholic (EAFAAA). The instruments were also composed by sociodemographic data, service structure, human and material resources and the use of alcohol and other drugs. Descriptive analysis of the data was performed using the software Statistical Package for Social Sciences (SPSS) version 20.0 for Windows® . Results: The sample consisted of 01 coordinator, 19 workers, 25 users and 15 relatives. We obtained high levels of users and family satisfaction, and intermediate levels of satisfaction for employees with respect to the evaluated service. Regarding the perceived change, the data showed high percentages of change for both users and for family. On a global question about improvements, 100% of the families noticed an improvement in the overall life of their relatives, when comparing to the beginning of treatment, and 88% of the users noticed the occurrence of improvement in their treatment. The data showed a tendency toward negative attitudes of these workers towards alcohol, alcoholism and alcoholics because for most means of factors remained in the intermediate zone. The results highlight the need for investment in training and extension courses for professionals and the need to improve specific points regarding the service infrastructure. As this study was conducted in a single mental health service, this results cannot be generalized to other services.

Повний текст джерелаThis study is about the evaluation of a Center of Psychosocial Care (CAPS), a service supposed to work according to the patterns divulged by the Brazilian Psychiatric Reform which, currently, in its assistance trend, is marked by the implantation of services intended to replace the psychiatric hospital. The evaluation proposal is centered in the micro space and in the day-to-day of the service and is performed by means of the participation of the team, the users and family members. It is about a qualitative evaluation, by means of which one searches to grasp the dynamics of the service, the way how the actors interact and the directions that they construct in relation to the practice itself; an evaluation that can also device, allowing, by means of a participation process, that interest groups broaden the possibility of intervening in the reality of the service and that they may become subjects, considering that in traditional methodologies, they are excluded. The Evaluation of Fourth Generation, developed by Egon G. Guba and Yvona S. Lincoln, together with the Paidéia Method, developed by Gastão Wagner de Sousa Campos, guided the theoretical-methodological process of the research. The instruments for the collection of data were interviews with the team, users and family members as well as observation. The issues that emerged over the evaluation process were grouped in three central themes: dimensions of the work object, the team, practices and the working means. It has been noticed that, even though the service seems to be a replacement for the attendance in the psychiatric hospital, both external factors, related to the local policies of mental health, and internal factors, related to the day-to-day of the service, prevents it from becoming effective.

Повний текст джерелаThe redirection of the care model in Brazil raises the need to discuss the reinvention of practices in tune with the scenery of changes. The practices must now be developed in order to overcome the isolation of bars, walls and keys, the deletion of the subject, the suppression of symptoms and the personalities of individuals in psychological distress. Given this, the present study aims to evaluate best practices in mental health in psychosocial care in the context of a city in Southern Brazil. To do so, it is organized as a constructivist evaluation research, with a responsive approach, as it develops a critical consensus among stakeholdres. It is based on the Fourth Generation Evaluation, proposed by Guba and Lincoln (2011), which emerges as a methodological option and articulates with the theoretical framework of \"Best Practice in Community Mental Health\" proposed by Thornicroft and Tansella (2010). This one is presented in order to identify best practices in mental health based on the articulation of three indispensable and inseparable pillars: ethics, as a fundamental principle that should guide the planning, assistance and assessment of services; evidence, which must base the interventions and services; and experience, as an evidence of what is being produced in the local and regional experience of organizing network services. As a result of the process, Welcoming practices, Singular Therapeutic Project and Transfer of Care emerged for discussion. The Welcoming becomes concrete in the actions of the Porta Aberta project, and it means qualified hearing at the moment the need arises, besides representing the offer of quality care based on the relationship process that strengthens the bonds and the construction of life projects. The Singular Therapeutic Project is presented as best practice, as it is able to give individualized responses to the needs of the subject in order to create, as a result, a person who is able to walk through life. It is set in a shared manner among staff, user and family, according to the therapeutic goals for each subject, and it uses the reference technician (ou professional), the mini teams, therapeutic workshops, therapeutic groups, assemblies, in other words, any offers and proposals which allow the subject to resume the engagement with the spaces of the city, on the way to production of life. Regarding the Transfer of Care, it enables the person who lives the process not only to have access to a network of services but, above all, to seek their emancipation through its support. To do so, it invests in practices that encourage the subjects\' autonomy, using instruments such as the Therapeutic Monitoring, Patient Discharge Support Group, spaces for decisions and discussions, motivation for searching and solving problems, recognition activities of city areas , transport, culture and leisure. In addition, it invests in actions with the Basic Health Care, based on the matrix support with processes of continuing education with the teams and mapping of the mental health situation in the territories. At the end of this study, the conclusion was that the psychosocial care model showed capability of producing best practices in mental health, and these have changed the lives of individuals in psychological distress, and have helped in the process of conquering social spaces again

Comprehending the ongoing (re)building process of the substitutive psychosocial model to the nursing home care model for people suffering from mental disorders, it is worthwhile to know the healthcare dynamic of a substitutive service, in order to identify any issues within the team, which could interfere with the quality of care they provide. The aim of this study was to identify the working process of professionals at a Type III Psychosocial Care Center (PCC). This is an interventional study using a qualitative approach, that is based on the problem-based method, known as Arco de Maguerez. It consists of five stages: reality observation, key points, theorizing, problem-solving hypothesis, and application to reality. The development of the study was based on lightweight technology sources mediated by group technique. In the first stage, strong and weak points of the service were identified, followed by a discussion of issues related to the patient, family, teamwork, territory, and management. From this, the strengths and weaknesses of the staff and service were discussed. Following this, the participants emphasized the importance for discussion on the difficulty of integration and coordination of the Psychosocial Care Network. In the next step, problems regarding key points in question were expressed and this guided the active search in the literature for theorizing, which would guide in developing possible solution options. Therefore, permanent health education to perform the matricial and approaches to disclose the PCC territory were debated, and subsequently strategies were developed and applied to reality. Through the research process, the strengths of the team that would help them to overcome their shortcomings were identified. While some restraining forces that precluded this movement were also identified. In this context, the awareness-making process and the participants’ willingness to embrace change were evident throughout the study. This highlights the acceptance of the proposal of the interventional research that is based on the problematization of the service. It is evident that the spaces validity for the exchange of practices and knowledge, reflection-action-reflection, are imperative to the production of knowledge, which supports professionals for the qualification of care in mental health.
Entendendo o processo contínuo de (re)construção do modelo psicossocial substitutivo ao modelo de assistência asilar a pessoas com sofrimento ou transtornos mentais, considera-se fundamental conhecer a dinâmica assistencial de um serviço substitutivo, com vistas a delinear junto à equipe situações problemas que interferem na qualidade do cuidado que oferecem. A pesquisa teve o objetivo de conhecer os processos de trabalho dos profissionais de um Centro de Atenção Psicossocial tipo III. Estudo do tipo de intervenção, de abordagem qualitativa, direcionada pela metodologia problematizadora do Arco de Maguerez, que compreende cinco etapas sucessivas: Observação da Realidade, Pontos-chave, Teorização, Hipóteses de Soluções e Aplicação à Realidade. Seu desenvolvimento foi norteado utilizando-se essencialmente recursos das tecnologias leves, mediadas por técnicas grupais. Na etapa inicial, foram elencados pontos frágeis e fortes do serviço, seguidos da discussão de questões relativas ao usuário, família, equipe, território e gestão. A partir desse momento discutiram-se os entraves e pontos propulsores da equipe e do serviço. Na sequência, os participantes elegeram como ponto prioritário de discussão a dificuldade de integração e articulação da Rede de Atenção Psicossocial. A etapa seguinte privilegiou-se a problematização dos pontoschaves em questão, que orientaram a busca ativa na literatura para a teorização que orientaria as possíveis hipóteses de soluções. Assim, a educação permanente em saúde para realizar o matriciamento e formas de divulgação do CAPS no território foi debatida e, posteriormente, estratégias de ações foram aplicadas à realidade. No caminhar do processo de pesquisa, constataram-se forças propulsoras da equipe para o enfrentamento de suas dificuldades e também algumas forças restritivas que impediam este movimento. Neste contexto, os processos de tomada de consciência e de mudança dos participantes ficaram evidentes ao longo do estudo, salientandose a aceitação da proposta da pesquisa intervencionista, fundamentada na problematização do cotidiano do serviço. É perceptível a validez dos espaços de trocas de práticas e saberes, reflexão-ação-reflexão, para a produção de conhecimento que subsidiem os profissionais para a qualificação do cuidado em Saúde Mental.

Повний текст джерелаDespite psychoactive substances have been consumed for thousands of years, in recent decades, the issue of their abuse and dependence has been configured as a public health demand, against a backdrop marked by rapid socioeconomic transition that has been occurring in several countries. The object of this study is the evaluation of practices offered at Psychosocial Care Centers for Users of Alcohol and Other Drugs (CAPSad) based on the point of view of the families of their users. Methods: evaluative research, by quantitative method, to analyze data collected at CAPSad located in the city of São Paulo, using the Satisfaction Scale (Satis-BR) and the Perceived Change Scale (EMP), already validated and previously used in evaluation studies, will guide the focus of analysis. In addition to the scales, data on the service structure, sociodemographic data, access, and characterization of the family\'s participation in the user´s treatment, were analyzed, with the lead of the multiple linear regression model, in which it revealed a change perceived as a predictive variable higher level of satisfaction with the services and the negative correlation of these variables with perceived change in rates: living in the eastern and northern areas of the city and considering a monthly income between 2 and 4 minimum salaries. The fact that no nuclear kinship, as being an uncle or nephew, with the user, has had a positive association with the perceived change. Further studies are needed with this type of service to better explore these dimensions related to quality of care in this theme, helping to assess the impact of new policies and Psychosocial Care Network.

Повний текст джерелаThe University of Zululand Community Psychology Centre (CPC) is a joint project of the Department of Psychology, Educational Psychology and Industrial Psychology. It was established in response firstly to the University's need to provide relevant training for its post-graduate psychology students and secondly to meet the need within the Zululand community for affordable psychosocial and psycho-educational services. The Centre works in partnership with other local Zululand Mental Health centres. The Centre is accredited with the Professional Board for Psychologists of the Health Professions Council of South Africa (HPCSA). This board ensures that institutions providing psychological services observe ethical codes of conduct and good practice (HPCSA, 2002). The present research was motivated by the fact that the CPC functioning has never been evaluated in terms of service delivery and relevance. The desire to know whether the CPC serves the purposes for which it was established, and to identify areas that need improvement, is a strong motivation. This helps to identify its viability, effectiveness, and value for the University of Zululand and the community served by the University. The aims of the study were to evaluate the CPC in terms of clients' perceptions with regard to effectiveness of the centre in meeting their needs and improving their psychological well-being. An accidental or convenient sample was used. An invitation to volunteer was extended to clients who happened to come for the CPC services. Volunteers completed a questionnaire^ which included a biographical inventory needs analysis questionnaire and RyfTs Scale (1995) of psychological well-being. Participants were pre and post-tested on the variable of psychological well being. The study concluded that the CPC still serves the purpose for which it was established. It is still relevant to the demand of the community. The results revealed that pre and post testing was associated with a significant improvement in total psychological well-being as with special reference to personal growth and positive relations with others.

Повний текст джерелаNISAA was established in 1994. It is one of the first organisations dealing with women abuse in Lenasia and the surrounding areas. The organisation has great potential in becoming a recognised institution in South Africa. Women who are in distress, have no where to go and are left abandoned have a shelter with NISAA. In the short period of its existence NISAA has strived hard to uplift and assist women. Counselling and treatment is offered on a daily basis by professionals and trained personnel. However, many needs still have to be met and research has shown us that not nearly enough is being done for those who are being abused, be it wife abuse or child abuse. The processs of research is presented in five stages and covers, the problem statement, the literature survey, research methodology, analysis, conclusions and recommendation. The survey of the literature is very important in this study. South Africa has become epitomised by a rapidly changing social order, swiftly unfolding political development and an unprecedented fluidity in the thinking of future opinions. The attitudes and perceptions of domestic violence and violence in general needs to be changed. The chapter on research methodology stresses the importance of feminist organisations. Feminist organisations' role is vital in any study on woman abuse, as research on woman abuse began with these organisations. Methodology in this study also focuses on the qualitative nature of battering, research and the relevant themes surrounding battering. These themes include woman abuse, intervention. treatment and shelters. Other important aspects discussed is coding of data, research design, sampling and interviewing procedures. The analysis and results shows that NISAA has made a significant contribution to battering of women. From the results of this study, NISAA has become an important structure and service in the South African society. The services NISAA should expand on include, a safe and positive crisis help, as well as promoting the rights of all victims. Important to the growth of NISAA is alleviating the funding problem. Other relevant findings discussed are attitudes and perceptions of women abuse and services that encourage and discourage the use of NISAA. These findings compliments. Gishen's (1993) study on the Power Organisation. Recommendations made to NISAA are based on the findings of this research. This research hopefully will expose abuse, help women to achieve recognition in its fight against abuse and in its quest for the empowerment of women. The research was also intended as an educative process. This study entails the physical and psychological abuse of women which has become a global problem. The problem needs to be addressed, understood, analysed and treated. An effective solution must be found.

Повний текст джерелаAssertive Community Treatment (ACT) represents an effective treatment for individuals with severe mental illness. Though studies estimate that as many as half of all people with severe mental illness are parents, little is known about consumers receiving ACT services who are parents. Thus, the purpose of the present study was to 1) estimate the prevalence of parent ACT consumers, 2) identify current ACT team policies and practices for treating parent consumers, and 3) examine the perspective of parent consumers served by ACT teams. Quantitative and qualitative data were collected and analyzed via two separate studies. In study 1, eighty-two ACT providers from 76 teams across the United States and Canada were surveyed to determine the prevalence of parent ACT consumers, ACT team policies for identifying the parental status of consumers, treatment services available for parent consumers, and provider attitudes about parent consumers. Providers estimated roughly 21.6% of ACT consumers were parents. Less than half of providers (46.3%) reported formally asking about parental status during intake and only 20.7% providers belonged to ACT teams that provide special programs/services designed for parent consumers. The majority of providers (75.6%) reported negative or mixed attitudes about parents with severe mental illness. In study 2, seventeen parents with severe mental illness being served by ACT teams were interviewed about parenting, the relationship between parenting and severe mental illness, parenting needs, and suggestions for improved treatment services for parents. All parents were able to identify at least one positive aspect of parenting and most parents (76.5%) also identified negative aspects of parenting. Loss of custody emerged as a significant parenting problem, with the majority of parents (88.2%) experiencing loss of custody at least once. Given the difficulties of being a parent and having to manage a severe mental illness, parents expressed interest in several parent-focused treatment services, including family therapy, parenting skills, communication skills training, resources/finances for children, and social support groups with peers. When asked about overall satisfaction with ACT services, most participants with adult children (87.5%) reported having no unmet parent-related needs and high satisfaction (4.63 out of 5) with ACT services, whereas parents with young children (77.8%) reported having numerous unmet parenting needs and low satisfaction (3.78 out of 5) with ACT services. Thus, the age of participants’ children was a significant factor, indicating that the ACT treatment model may not be adequately serving parents of young, dependent children. Overall, findings suggest the need for more attention and focus on parent consumers, including formal identification of consumers’ parental status and improved parent-related treatment services and support
