Capital charging was introduced into the National Health Service (NHS) in 1991 in order to stop capital being treated as a ‘free’ good and to encourage managers to use their assets more efficiently. This article seeks to examine the extent to which managerial thinking has been influenced. It uses as evidence interviews with NHS managers conducted in Scotland in 1994. The following uses of capital charges data are explored: capital programme; disposal programme; maintenance programme; contract pricing; and budgetary devolution. New capital programmes required more justification and capital charges were seen as relevant to estate rationalization. Less effect was found with regard to the maintenance programme, though this may have been due to a downgrading of the estates function in most Trusts. Although the capital charge costs included in contract prices affect the competitive position of providers, there was criticism of the lack of development of the purchasing function. Budgetary devolution was proceeding relatively slowly but, among those Trusts which had devolved capital charges, evidence was found that some clinicians were becoming aware of the full costs of equipment use. This article concludes, with cautious optimism, that capital charges are beginning to influence decisions and that, despite some incentives being dysfunctional, they will lead to a better managed NHS.

À ce jour, les programmes d’activité physique (AP) visant les personnes atteintes de surpoids et d’obésité s’organisent autour des recommandations érigées pour la population générale. Toutefois, ces personnes présentent des caractéristiques particulières qui nécessitent un accompagnement adapté. Afin de pouvoir amorcer un travail de recommandation, l’objectif de cet article était de répertorier les critères utilisés pour décrire et étudier les programmes d’AP à destination des personnes atteintes de surpoids et d’obésité et les suivis post-programmes. Une revue systématique des méta-analyses et revues systématiques portant sur la prise en charge du surpoids et de l’obésité adulte a permis, notamment, de mettre en évidence que peu d’éléments descriptifs des programmes sont rapportés de manière systématique, et que les programmes sont rarement suffisamment détaillés pour pouvoir calculer une dose d’AP ou permettre leur comparaison. Ces convergences, disparités ou manques dans la description des programmes d’AP, ont permis d’engager une réflexion sur l’intérêt des différents critères ainsi que sur l’intérêt de leur systématisation dans les dispositifs de prise en charge de la gestion du poids incluant l’AP, pour ce que nous espérons être une première étape vers l’élaboration de recommandations en matière de prise en charge du surpoids et de l’obésité par l’AP. Les nouveautés Objectif : répertorier les critères de description des programmes d’AP dans la prise en charge du surpoids et de l’obésité. Critères à systématiser : calcul dose d’AP, individualisation programme. Critères à développer : taux d’abandon comme indicateur d’adaptation du programme, stratégie de gestion de la fatigue.

This study examines access to International Baccalaureate schools in Australia. It is important to examine whether, as a highly regarded form of rigorous academic education, IB programmes are available to a wide range of students. We examine the location of schools in Australia that offer one or more of the IB Primary Years Programme, Middle Years Programme or Diploma Programme, their fees and admissions policies, and what types of students they enrol. The findings show that most schools in Australia that offer any of these three IB programmes are located in affluent communities of large cities, are privately-funded, charge moderate to high fees, and enrol mostly students from privileged socioeconomic backgrounds.

Public Private Partnership Programme is a cooperation between different healthcare professionals in both public and private sectors. Over the decades, many programmes have been developed which cover various diseases, including chronic conditions and surgery for cataracts, as well as vaccinations. However, these programmes are not successful because of the unsatisfactory service charges to private sector, overlapped aims and resources and wrong estimation of the needs. To become successful, programmes reform is required by rearranging resources, annual reviews, more financial support and extending the coverage of diseases. Promotions are beneficial to all stakeholders. Overall, better coordination between both sectors is the most essential factor which can lead the programme to success and maintain a better health care system in Hong Kong.

L’article décrit les programmes de prise en charge financière des psychothérapies en Australie et en Angleterre. Au début des années 2000, le gouvernement australien a implanté une réforme de santé publique visant à permettre un meilleur accès, gratuit ou peu coûteux, aux psychothérapies brèves, en soin primaire, pour les personnes souffrant de troubles mentaux courants. Cette réforme reconnaît l’importance du rôle du médecin généraliste dans la prise en charge des troubles, et la collaboration avec les professionnels de santé mentale. Deux programmes ont été mis en oeuvre : Access to Allied Psychological Services en 2003 et Better Access en 2006. En 2009, deux millions d’Australiens ont reçu onze millions de séances de psychothérapies. Les études ont montré des résultats positifs sur l’amélioration de l’accès aux soins de santé mentale primaires et sur la santé des patients. L’Angleterre a mis en place le programme Improving Access to psychological Therapies en 2007 en offrant un service de psychothérapies brèves pour les personnes souffrant de troubles mentaux courants. Le modèle de soin en étape suit les guides cliniques du National Institute for Health and Clinical Excellence, le type de professionnel (counselor ou psychothérapeute formés), le nombre de séances (4 à 13) et la méthode thérapeutique varient selon la sévérité du trouble. Un million de patients ont été traités depuis le début du programme. Les études ont aussi montré des résultats de santé positifs pour les patients du programme. En conclusion les programmes de prise en charge des psychothérapies s’avèrent coûts-efficaces.

ObjectiveTo evaluate the impact on healthcare utilisation frequencies and charges, and mortality of a programme for frequent hospital utilisers and a programme for patients requiring high acuity post-discharge care as part of an integrated healthcare model.DesignA retrospective quasi-experimental study without randomisation where patients who received post-discharge care interventions were matched 1:1 with unenrolled patients as controls.SettingThe National University Health System (NUHS) Regional Health System (RHS), which was one of six RHS in Singapore, implemented the NUHS RHS Integrated Interventions and Care Extension (NICE) programme for frequent hospital utilisers and the NUHS Transitional Care Programme (NUHS TCP) for high acuity post-discharge care. The programmes were supported by the Ministry of Health in Singapore, which is a city-state nation located in Southeast Asia with a 5.6 million population.ParticipantsLinked healthcare administrative data, for the time period of January 2013 to December 2016, were extracted for patients enrolled in NICE (n=554) or NUHS TCP (n=270) from June 2014 to December 2015, and control patients.InterventionsFor both programmes, teams conducted follow-up home visits and phone calls to monitor and manage patients’ post-discharge.Primary outcome measuresOne-year pre- and post-enrolment healthcare utilisation frequencies and charges of all-cause inpatient admissions, emergency admissions, emergency department attendances, specialist outpatient clinic (SOC) attendances, total inpatient length of stay and mortality rates were compared.ResultsPatients in NICE had lower mortality rate, but higher all-cause inpatient admission, emergency admission and emergency department attendance charges. Patients in NUHS TCP did not have lower mortality rate, but had higher emergency admission and SOC attendance charges.ConclusionsBoth NICE and NUHS TCP had no improvements in 1 year healthcare utilisation across various setting and metrics. Singular interventions might not be as impactful in effecting utilisation without an overhauling transformation and restructuring of the hospital and healthcare system.

ABSTRACTGovernment is about the supply and demand for many goods and services, but votes or social needs are the principal signal meant to give direction to public policy. Many neo-liberal economists contest this, arguing that charges or user-fees are more efficient for allocating services. Empirical analysis shows that in principle it would be possible to impose charges on three-quarters of the goods and services financed by government. Although some charges are imposed on a wide variety of public programmes, overall governments reject marketing what is marketable. Theoretical inconsistencies in government's use of charges indicate that they reflect political inertia. The conclusion offers an analytic paradigm for interpreting charges, since an accurate interpretation of the signals sent by charges depends upon attributes specific to a given public programme.

Plusieurs études montrent l’intérêt des programmes de remédiation cognitive et de psychoéducation dans la prise en charge des patients souffrant de schizophrénie dans le cadre de soins de réhabilitation psycho-sociale [1]. Ces programmes dispensés de façon isolée sont moins efficaces que ceux effectués dans une démarche de soins intégrative [2,3] et en impliquant l’ensemble des acteurs de soins et de réhabilitation (sanitaire, MDPH, médico-social, milieu professionnel). Depuis 2009, le centre intersectoriel Crisalid (pôle FJ5, CHI de Clermont de l’Oise, Picardie) propose un programme intégratif et personnalisé, appelé COMBIMOD (combinaison de modules de remédiation cognitive et d’éducation thérapeutique) destiné aux personnes souffrant de schizophrénie et à leurs proches habitant dans l’Oise, suivi par le sanitaire, le privé et le médico-social. Ce programme combine des modules de remédiation cognitive francophones validés à des modules d’éducation thérapeutique spécifiques construits autour des déficits cognitifs. L’objectif de ce poster est :– de décrire au travers du parcours de soins de 3 personnes souffrant de schizophrénie la mise en place du programme COMBIMOD (de l’importance de : l’entretien motivationnel, l’évaluation globale [clinique, thérapeutique, neurocognitive, cognition sociale, fonctionnelle], la restitution du bilan, la mise en place d’un programme personnalisé et intégratif) jusqu’à la réalisation des objectifs professionnels ;– montrer l’importance de l’articulation entre le sanitaire, le privé, le médico-social et le milieu professionnel.Au terme du programme et grâce à un travail d’articulation entre le sanitaire (secteur : CMP, hôpital de jour, CATTP), la MDPH, le médico-social (centre de réadaptation professionnelle et de formation, ESAT de transition) les 3 patients se sont réinsérer professionnellement en milieu ordinaire.

TT 209 was built in a wadi, a location that means it has been affected by flash floods since ancient times. The team in charge of its study and conservation has initiated a systematic programme of environmental data collection (temperature and relative humidity) in order to understand the natural conditions of the tomb and any transformations caused by archaeological work in its underground chambers, as well as to inform future conservation. These statistics can also be useful for excavation and conservation programmes in nearby tombs whose architectonic structure is similar and have also suffered from exposure to water damage.

SummaryWorldwide, there is increasing recognition that if family and reproductive health programmes are to be successful, the involvement of men is essential. As part of the problem, men also have to be seen as part of the solution. The reality is that in many countries, including Turkey, men generally do not accompany their partners to health facilities for family planning, antenatal and postnatal services and are not expected to attend the labour or birth of their child. Workplace programmes are a potential strategy for meeting the reproductive health education needs of men in industrial cities such as Istanbul. This intervention study was developed to test the feasibility and effects of expanding a special programme for expectant fathers to large workplaces in Istanbul, with the aim of improving the health of Turkish families during the pregnancy, birth and newborn periods. The findings indicate that it is possible to train workplace physicians in Istanbul to conduct regular educational programmes for expectant fathers on reproductive health, and that such programmes may have beneficial effects, especially in the areas of pregnancy nutrition, exclusive breast-feeding, and support behaviours. Considering the difficulty of getting men to attend hospital or clinic-based educational programmes in large urban areas, bringing such training programmes to men at their places of work has the potential to be an important strategy. Given that large workplaces in Turkey already have full-time physicians charged with the duty of health education for employees, this is also a feasible strategy.

Le handicap psychique, à l’origine des difficultés d’insertion ou d’autonomie des patients atteints de trouble schizophréniques recouvre, en se surajoutant aux symptômes classiques de la maladie, plusieurs dimensions comme les troubles cognitifs (troubles de l’attention, de la mémoire et de flexibilité mentale), les troubles de la communication, les troubles de la cognition sociale, le défaut d’insight,… L’engagement des soins dans le cadre de la réhabilitation psychosociale implique la mise en place d’un dispositif de soins qui fait appel aux nouvelles techniques développées ces dernières années répondant de façon spécifique aux différentes dimensions du handicap psychique. Ainsi, nous disposons en France maintenant de plusieurs programmes dont plusieurs ont été validés comme :– pour les troubles cognitifs : les programmes RECOS et CRT ;– pour les troubles de la communication : le programme IPT ;– pour les troubles de la cognition sociale : le programme ToM Remed, Mickaël's game, entraînement métacognitif ;– pour le défaut d’insight : les modules de Libermann ;– pour les famille ou les proches : le programme Profamille.Le déroulement de la prise en charge du patient nécessite une première étape d’évaluation approfondie de ses besoins, de ses ressources, de ses capacités (cognitives et autres), de son état clinique, de son autonomie au quotidien. Les soins mis en place seront choisis en fonction de cette évaluation et du projet personnel du patient (comme l’insertion dans un logement indépendant, la reprise d’étude ou d’un travail,…). Régulièrement, en fonction de l’évolution de la personne, le dispositif de soins sera réaménagé. Nous développerons l’intérêt lié à la mise en pratique de ces dispositifs dans le cadre spécifique d’une hospitalisation de jour et avec un objectif privilégié de l’intégration en ESAT.

International exchange programmes have been previously conceptualised based on the ‘opinion leader model’. It expects participants to form positive attitudes towards the host country, and to become influential back home. However, the micro-processes through which this goal can be achieved remain undertheorised. Drawing from Interaction Ritual Theory, this article argues that international exchanges consist of a chain of rituals. They strategically immerse targeted individuals into personal experiences with a shared focus on the host country, transforming the latter into a symbol charged with emotional energy and (strategically devisable) cognitive content. The symbol (now affectively and cognitively laden) is then recurrently invoked to re-circulate through the networks of the participants. A case study of a programme in which the symbol ‘Poland’ became systematically charged with the content of ‘multiculturality’ illustrates this proposition. Practical and theoretical implications (e.g. for the literature on socialisation, norm-diffusion, and face-to-face diplomacy) in international relations are discussed.

Objectives The organized population-based screening programme for abdominal aortic aneurysm in Stockholm, Sweden, started in 2010. An examination fee was initially charged, but later removed because of a policy change. We examined the effect on screening attendance of removing the fee. Methods The periods before and after removing the examination fee were compared with regard to screening attendance, overall, by municipality and by district. Results Screening attendance was 79.2% in the period with an examination fee and 79.9% in the period without an examination fee (p = 0.1787), with no significant change in screening attendance between the periods. Conclusions Although removing examination fees has been shown to have a positive impact on attendance in other screening programmes, we did not find this association in our study.

Electric vehicles (EVs) could be used to address the issues of environmental pollution and the depletion of non-renewable energy resources. EVs, which are energized by a battery storage system, are becoming attractive because they keep the environment clean. Furthermore, the cost of EVs is becoming cheaper. Thus, EVs will become a significant load on utility distribution system in the future. EV chargers play a significant role in the expansion of EVs. The input current of an EV charger with a high total harmonic distortion (THD) and a high ripple distortion of the output voltage can impact battery life and battery charging time. Furthermore, the high cost and large size of the chargers are considered other issues in EV development. This work presents the complete design process of a universal EV charger with a special focus on its control algorithms. In this regard, a novel control algorithm based on the integration of voltage-oriented control (VOC) and the sinusoidal pulse-width modulation (SPWM) technique is proposed to ensure effective Levels 1, 2, and 3 battery charging. A simulation of the universal EV charger was conducted and assessed in MATLAB–Simulink. Moreover, a laboratory prototype was constructed with a TMS320F28335 digital signal processor (DSP) programmed as the controller to validate its operation and performance. The findings show that the proposed charger is able to provide a controllable and constant charging voltage for a variety of EVs, with an input current of low total harmonic distortion (THD) and an almost unity power factor.

Introduction : L’épidémie à Covid-19 a bouleversé la prise en charge des maladies chroniques et a perturbé la majorité des programmes d’éducation thérapeutique (ETP) en France. Des adaptations via le numérique se sont mises en place. Objectifs : Étude exploratoire évaluant la faisabilité et la pertinence d’un web-programme d’ETP pendant le premier confinement lié à la Covid-19 ; réflexions sur l’intérêt des web-ateliers par rapport au présentiel. Méthode : Adaptation d’un programme sur la polypathologie chronique porté par une unité transversale d’éducation du patient (UTEP), pour un web-programme hebdomadaire avec 5 web-ateliers, auto-formation et organisation de l’équipe à la pratique des web-ateliers, réflexions collectives sur l’intérêt et les limites des web-ateliers. Résultats : Du 1er avril à fin mai 2020, 51 patients ont été inclus, et 32 ont terminé un cycle éducatif (minimum 3 web-ateliers). Sur en moyenne 6,1 objectifs pédagogiques convenus au départ, 92,2 % ont été totalement ou partiellement acquis. La satisfaction moyenne à l’issue de chacun des web-ateliers se situait entre 8,42 et 9,53 sur 10. Les patients ont ressenti un soutien psychologique et une possibilité de sortir de l’isolement social. Parmi les avantages et limites des web-ateliers : meilleure accessibilité (géographique, disponibilité), maintien de la dynamique de groupe à distance, limites techniques à prendre en compte. Discussion et conclusion : Le web-programme polypathologie avec 5 web-ateliers a été jugé faisable, acceptable et pertinent par les patients et intervenants. Les ateliers en distanciel paraissent répondre aux besoins des patients en période de pandémie et au-delà.

Standardisation is fundamental to ensuring that new technologies develop and grow unhindered by manufacturer-led standards. Dismissing this vital issue can have a detrimental effect on society regarding adopting new technologies, particularly when government targets and regulations are crucial for their success. We have witnessed competing global industries struggle for dominance, such as Betamax versus VHS, where each had a similar user outcome, but the confusion of differing formats slowed growth. We analyse emerging standards for electric vehicle rapid charging and investigate how standardisation challenges affect stakeholders by reviewing the existing literature on single-mode and polymodal harmonisation. By assimilating existing evidence, we then develop a new understanding of the science behind multi-model standardisation (MMS) approaches. Our literature review reveals three primary standardisation issues: (1) charge connections, (2) car to charger communication protocols, and (3) charge payment methods. We then analyse each mode type’s benefit, observing how each example contributes to the overall outcome, and suggest that their impact depends on car to charger handshake timing and intuitive user interaction. Using a structured survey of 282 respondents, we analyse end-user satisfaction for factors affecting growth in the EV sector and compare these findings with the factors identified during our literature review. We consequently articulate a programme for future research to understand EV rapid charger standardisation better, proposing recommendations for vested stakeholders that embrace sponsors in societal, technological and scientific transformation.

Electrostatic interactions with negative lipids contribute to the subcellular localization of polycationic proteins. In situ measurements using cytosolic probes of surface charge indicate that normal mitochondria are not noticeably electronegative. However, during apoptosis mitochondria accrue negative charge and acquire the ability to attract cationic proteins, including K-Ras. The marked increase in the surface charge of mitochondria occurs early in apoptosis, preceding depolarization of their inner membrane, cytochrome c release, and flipping of phosphatidylserine across the plasmalemma. Using novel biosensors, we determined that the increased electronegativity of the mitochondria coincided with and was likely attributable to increased exposure of cardiolipin, which is dianionic. Ectopic (over)expression of cardiolipin-binding proteins precluded the increase in surface charge and inhibited apoptosis, implying that mitochondrial exposure of negatively charged lipids is required for progression of programmed cell death.

At Penn State, music technology is something of a stranger in a strange land. As a programme, it began in the early twenty-first century, when the necessity of the moment was an anticipated revision to the guidelines from the National Association of Schools of Music (NASM), the North American accrediting body. Music schools were charged with ensuring that music majors were exposed to ‘relevant technologies’. It was left largely to individual institutions to interpret what this meant. At Penn State, a course was created to address this guideline, and it generated interest among students. This course then spawned a series of related courses. These courses eventually created enough of a curricular presence to warrant creating an undergraduate minor. We now expect that the minor will spawn an undergraduate major. The music technology programme's locus lies not solely within the School of Music; rather, it overlaps as an interdisciplinary area with a variety of programmes throughout the university's offerings. These overlaps are a unique feature of the programme. It is an unusual arrangement, but it is a product of its time and place. Three populations of students have coalesced, and the pedagogical challenge has been to create a curriculum that can serve all of them. The programme might be thought of as series of concentric spheres; each is centred around the same general concept structure, but with expanding breadth for different levels of student involvement.

La remédiation cognitive est une thérapie prometteuse pour cette population en demande. Une diminution des troubles cognitifs et fonctionnels des patients bipolaires suite à une thérapie de remédiation cognitive a été observée [3,4]. Cependant, les programmes proposés actuellement présentent un intérêt limité en raison d’un manque d’adaptation à la spécificité des troubles cognitifs prévalents dans les troubles bipolaires [4].ObjectifNotre objectif est d’observer sur le plan cognitif et fonctionnel, l’effet d’un programme de remédiation cognitive spécifiquement développé pour les patients souffrant d’un Trouble Bipolaire.MéthodologieNous présentons une première étude de cas d’un patient ayant bénéficié du programme ECo, actuellement en cours de validation dans le cadre d’étude contrôlée, randomisée en double aveugle. Une évaluation neuropsychologique ainsi que des échelles de fonctionnement psycho-social ont été administrées avant et après la thérapie. Le programme ECo se compose de 24 séances individuelles comprenant deux séances de psychoéducation sur les troubles cognitifs et des séances de généralisation des stratégies acquises à des situations de la vie quotidienne. Ce programme cible majoritairement les déficits cognitifs des patients souffrant d’un trouble bipolaire : mémoire et apprentissage verbal, flexibilité, planification, inhibition, raisonnement, vitesse psychomotrice et attention [2].DiscussionLes améliorations observées sur le plan cognitif et fonctionnel permettent d’émettre l’hypothèse du bénéfice d’un programme spécifiquement adapté aux difficultés des patients souffrant d’un trouble bipolaire. Notre étude de validation en cours permettra de préciser l’intérêt, la spécificité et les bénéfices de cette prise en charge.

Purpose The purpose of this paper is to explore the application of ideas and practice from Organisation Development to the creation of a leadership development programme, based in a UK Business School. When leadership development is constructed as a personal and relational endeavour, rather than using competences to create identikit heroes, faculty need to draw less on the espoused certainties of “expert” power and adapt their teaching style and language. Design/methodology/approach This case study of an ongoing Masters programme in health leadership, draws on external evaluation data and action research. Findings The paper’s prime contribution is as a conceptual stimulus to rethink leadership development programmes based on OD. Practical implications The practical implications could be far reaching if business schools want to offer an alternative to perpetuating heroic, individualist models of leadership. Social implications As leaders take charge of their own learning, as much as their leading, faculty become powerful participants and draw on OD skills rather than those of expert knowledge. Originality/value The paper thus offers a practical example with evaluation of how leadership development can be redesigned congruent with seeing leadership as a socially constructed, relational, dynamic and context specific.

We have developed a compact and cost-effective shock tube to simulate the static and dynamic pressures of blast waves. The shock tube is open at both ends and is driven by high explosives distributed over a finite length of the tube near one end. The overall charge length is determined by the simulation time of interest, and the charge-density distribution is tailored to produce the pressure-waveform shape desired. For the shock tube to simulate a typical blast wave, the charge density must be highest at the charge front (closest to the test section) and gradually reduced towards the back. The resulting shock tube is an order of magnitude shorter than a conventional dynamic airblast simulator (DABS) in which concentrated explosives are used to drive the shock.Tailored charges designed using this method were built and tested in a simulation development programme sponsored by the U.S. Defense Nuclear Agency (DNA). The pressures measured for several charge distributions agreed very well with SRI's PUFF hydrocode computations and demonstrated the feasibility of the compact simulator under realistic test conditions.

IntroductionLa prévalence de la dépression chez les adolescents est estimée à 5 à 9 %. Elle est associée à un risque accru de suicide. La prévention semble une approche prometteuse ce qui a accéléré le développement de programmes dans ce domaine.ObjectifsÉvaluer l’implantation du programme de prévention cognitivo-comportemental “Pare-chocs”dans le système scolaire français et son efficacité sur les symptômes dépressifs et la prévention d’un épisode dépressif majeur à long terme chez les adolescents.MéthodesNous avons évalué 1968 adolescents en classe de seconde à l’aide de la Center for Epidemiological Study Depression Scale (CES-D). Les adolescents présentant un score de coupure de 26 (n = 145) ou plus ont reçu une proposition de participation à un entretien clinique puis au programme Pare-Chocs s’ils étaient sélectionnés. L’intensité de la symptomatologie dépressive était suivie par la passation régulière de la CES-D en cours (1re, 5e et 11e rencontre) et en aval de la participation au programme (8 semaine et 1 et 2 ans plus tard).RésultatsLes résultats montrent une diminution des symptômes dépressifs en fin du programme ainsi que sur les évaluations ultérieures. Pare-chocs semble adaptable au système scolaire français.ConclusionAu-delà de la diminution des symptômes dépressifs chez les adolescents, cette action semble avoir un impact au niveau collectif en sensibilisant les professionnels et en améliorant la diffusion des connaissances sur le trouble dépressif et ses moyens de prise en charge, ainsi qu’en travaillant sur la dé-stigmatisation et le dépistage d’autres troubles psychiques. La dépression reste un enjeu majeur de santé publique. Sa prévention chez les adolescents semble une approche prometteuse qui permettrait d’éviter l’apparition de la maladie à l’âge adulte.

Introduction : L’ETP fait partie intégrante de la prise en charge du patient. Une réflexion amorcée par la commission des usagers d’un CHU et conduite avec d’autres partenaires vise à favoriser la participation de tout patient hospitalisé aux programmes existants dans le cadre de son parcours de soins. L’objectif était d’explorer la pratique de l’ETP et les facteurs influents au sein du CHU du point de vue des acteurs de terrain. Méthode : Des questionnaires adressés aux coordonnateurs ou responsables (CR) et patients-experts (PE) abordaient le soutien perçu, le recrutement, les freins et leviers à l’activité des programmes. Résultats : Le soutien du CHU et du pôle est ressenti pour 72,6 % et 46,7 % des CR. Les manques de moyens humains et matériels pour les CR, la méconnaissance du programme et la difficulté à recruter pour les PE sont les premiers freins. La satisfaction des patients et la motivation de l’équipe sont les premiers leviers pour les CR et PE. L’activité éducative est jugée satisfaisante sur le plan quantitatif selon 48,3 % des CR et est proposée à tous d’après 33,3 % d’après les PE. L’envie d’arrêter le programme est venue à l’esprit de 29,5 % des CR et 20 % des PE mais ne s’est finalement pas concrétisée. Discussion : Le recrutement reste une difficulté. Le soutien des structures et l’implication de tout un service apparaissent indispensables même si la motivation de l’équipe et le retour des patients restent les moteurs pour l’engagement des acteurs de terrain. Conclusion : Une politique de valorisation avec des moyens mobilisés et la communication promotionnelle élargie sont des champs d’action à explorer. La culture éducative doit devenir véritablement un critère de qualité dans le parcours de soins du patient.

AbstractElectric charge is always present in the lower atmosphere. If droplets or aerosols become charged, their behavior changes, influencing collision, evaporation, and deposition. Artificial charge release is an unexplored potential geoengineering technique for modifying fogs, clouds, and rainfall. Central to evaluating these processes experimentally in the atmosphere is establishing an effective method for charge delivery. A small charge-delivering remotely piloted aircraft has been specially developed for this, which is electrically propelled. It carries controllable bipolar charge emitters (nominal emission current ±5 μA) beneath each wing, with optical cloud and meteorological sensors integrated into the airframe. Meteorological and droplet measurements are demonstrated to 2 km altitude by comparison with a radiosonde, including within cloud, and successful charge emission aloft verified by using programmed flight paths above an upward-facing surface electric field mill. This technological approach is readily scalable to provide nonpolluting fleets of charge-releasing aircraft, identifying and targeting droplet regions with their own sensors. Beyond geoengineering, agricultural, and biological aerosol applications, safe ionic propulsion of future electric aircraft also requires detailed investigation of charge effects on natural atmospheric droplet systems.

In this study, the Gravitational Search Algorithm (GSA) programmed on MATLAB software is applied to solve the optimal problem of determining the liner structure to enhance the penetration performance of the shaped charge warhead. The obtained results show that when using the optimal set of parameters found by the proposed approach (the internal apex angle α0 = 16°, the thickness δ = 4.9 mm), the penetration performance of the shaped charged warhead is significantly improved (the penetration depth b = 553 mm, 38% higher than the original plan).

RÉSUMÉLes programmes d'intervention rapide ont été institués dans le but d'amoindrir les coûts des centres hospitaliers en diminuant le nombre d'admissions non nécessaires dans ces établissements grâce à la prompte coordination de soins à domicile. Aucune évaluation économique formelle de l'impact de cette réorientation des services n'a été réalisée. Ce document expose un essai contrôlé qui a été réalisé dans le but de comparer les coûts et les conséquences entre des patients comparables du service des urgences soignés selon le programme d'intervention rapide par rapport aux services de départements habituels. Les patients ont été évalués au moment de leur admission et dix jours après avoir reçu leur congé de l'urgence spécifiquement pour leur état de santé, pour la charge du dispensateur de soins, pour la satisfaction des soins reçus et pour les coûts. Le programme d'intervention rapide s'est avéré un choix acceptable par rapport aux services de l'urgence pour un faible pourcentage de patients et ne s'est pas avéré plus cher pour l'ensemble des coûts bien qu'il y ait eu un réacheminement des dépenses de l'hôpital vers les soins à domicile. À cause de l'échantillonnage restreint, la capacité de détecter les différences importantes dans l'état de santé, sur le plan clinique, s'est établie à 60 pour cent.

Le programme d'assainissement des eaux de Québec (PAEQ), mis en place à la fin des années 1970, s'est d'abord attaqué au problème de la pollution ponctuelle d'origine urbaine. Plusieurs stations de traitement des eaux usées municipales ont été construites dans le cadre de ce programme réduisant de façon importante les charges de polluants d'origine urbaine. La question demeure toutefois de savoir dans quelle mesure les charges urbaines rejetées avant et l'après l'instauration de ce programme peuvent entraîner des dépassements de différents critères de l'eau. La présente étude a pour objectif d'examiner cette problématique pour le bassin versant de la rivière Chaudière en utilisant le système de modélisation intégrée GIBSI (Gestion Intégrée par Bassin versant à l'aide d'un Système Informatisé). Deux scénarios d'assainissement urbain ont été examinés, l'un représentatif de la période avant la mise en place du programme, le début des années 1980, et un autre représentatif de la période plus récente. Deux chroniques météorologiques ont été utilisées (années 1983 et 1994). L'estimation des probabilités de dépassement de différents critères de l'eau montre une nette réduction de ces probabilités après mise en place du programme. Pour la DBO5 et l'azote total, les charges urbaines rejetées actuellement n'entraînent pas de dépassement des critères de qualité de l'eau pour les deux années retenues. Toutefois, les résultats montrent que les charges en phosphore total d'origine urbaine peuvent à elles seules entraîner des probabilités de dépassement importantes lors d'étiage.

Les approches centrées sur l’autorégulation ainsi que sur l’acceptation des émotions et des sensations désagréables inspirées des programme MBSR et MBCT, représentent une voie intéressante dans la prise en charge des troubles du comportement alimentaire dans lesquels le contrôle et l’évitement sont au premier plan. Ces programmes permettent de travailler les aspects obsessionnels (comme les ruminations) et compulsifs (sur l’alimentation, la réactivité avec perte de contrôle) associés à ces troubles alimentaires. La clinique des maladies mentales et de l’encéphale (CMME), hôpital Sainte-Anne a mis en place un protocole « MBCT Boulimie » en huit séances. Le programme MBCT a été modifié, notamment au niveau des outils cognitifs et de la durée des pratiques de méditation pour des patients présentant un TCA. La recherche s’est réalisée sur vingt-quatre sujets de l’hôpital de jour de la CMME et avait pour objet d’évaluer l’impact du programme MBCT dans une population de sujets souffrant de boulimie et de Binge Eating Disorder.Outils d’évaluationBMI, EDI-2, EAT, BDI-13, BITE, STAI Ya/Yb, le Ruminative Response Scale for Eating Disorder (RRSED), l’Acceptance and Action Questionnaire-II (AAQ-II), le Five Facets Mindfulness Questionnaire (FFMQ), le Mindfulness Attention Awareness Scale (MAAS), le Kentucky Inventory of Mindfulness Skills (KIMS), le Body Shape Questionnaire (BSQ), l’Impulsive Behavior Scale : Urgency, Premeditation, Perseverance, and Sensation Seeking (UPPS), le Cambridge Exeter Repetitive Thinking Scale (CERTS) et le Three Factor Eating Questionnaire (TFEQ). Le recours aux benzodiazépines à visée hypnotique est fréquent dans la prise en charge des plaintes d’insomnie chronique, notamment en médecine générale. L’ANSM estime que 4,2 millions de français a utilisé une benzodiazépine à visée hypnotique en France en 2012 et souligne l’augmentation de leur utilisation dans cette indication chez les femmes de plus de 65 ans (18 % des femmes de cette tranche d’âge). La littérature internationale actuelle est à charge : troubles cognitifs, somnolence, dépendance, chutes… Oubliant les effets thérapeutiques recherchés de ce type de traitement en tant qu’« antidouleur psychique » puissant. Si on peut s’interroger sur l’intérêt des benzodiazépines dans le long terme pour la prise en charge des troubles du sommeil, notre expérience montre qu’elles constituent une alternative de court terme dans le cadre d’une souffrance psychique certaine. Dès lors, quelle place peuvent-elles avoir dans la prise en charge de l’insomnie ? Quelles sont les préconisations actuelles de leur utilisation ? Quelles alternatives peut-on proposer pour améliorer la prise en charge des troubles du sommeil et éviter leur recours abusif ?

La cognition sociale, définie par Penn et al. [3] comme : « la faculté de comprendre soi-même et autrui dans le monde social » est déficitaire chez la plupart des personnes souffrant de schizophrénie [2]. Cette composante est pourtant d’une importance fondamentale pour le fonctionnement social, professionnel et interpersonnel [1]. Dans la schizophrénie, 5 processus du domaine de la cognition sociale sont régulièrement altérés : la théorie de l’esprit, la perception sociale, les connaissances sociales, le style attributionnel et les processus émotionnels. Dans le champ de la remédiation cognitive, plusieurs équipes ont développé des programmes afin de cibler les déficits de cognition sociale. On peut décomposer ces interventions en fonction de leur base théorique. Certaines procédures, qualifiées de « larges », reposent sur l’idée que les compétences neurocognitives renforcent les aptitudes relationnelles des patients, d’autres programmes « ciblés », plus restrictifs, permettent d’améliorer efficacement une composante de la cognition sociale, enfin, plus récemment, des interventions « globales », tenant compte de l’ensemble des processus de la cognition sociale altérés dans la schizophrénie ont été développés [4]. Le programme RC2S (Remédiation Cognitive de la Cognition Sociale) mis au point à Lyon est ainsi le premier programme en langue française entrant dans le champ des interventions globales. Cette intervention est individualisée et repose en partie sur l’outil informatique par le biais de situations de réalité virtuelle. Ce type de technologie offre en effet la possibilité de construire des environnements réalistes en 3D où toutes les composantes d’une interaction sociale réelle peuvent être mises en jeu. Des études de cas uniques multiples sont aujourd’hui en cours afin d’évaluer la pertinence de cet outil pour la prise en charge des déficits de cognition sociale chez les personnes souffrant de schizophrénie ou de troubles associés.

L’efficacité de la psychoéducation des patients et des familles a été mise en évidence par des essais cliniques contrôlés et plusieurs méta-analyses. La psychoéducation s’apparente à l’éducation thérapeutique (ETP). Ce terme, qui a été retenu par la Haute Autorité de santé, désigne un processus continu d’apprentissage intégré à la démarche de soins, comprenant des activités organisées de sensibilisation, d’information, d’apprentissage, et d’aide psychologique et sociale. Elle est destinée à aider les patients et leurs familles à mieux comprendre leur maladie et leur traitement. Différents décrets et arrêtés fixent les modalités d’autorisation des programmes d’éducation thérapeutique du patient par l’Agence régionale de santé et les compétences requises pour les dispenser. Les auteurs de cette communication présentent un programme d’ETP dans le domaine de la schizophrénie, développé par le CHS d’Ainay-le-Château et agréé par l’ARS Auvergne. Le CHS d’Ainay, spécialisé en Accueil familial thérapeutique développe des actions de réhabilitation psychosociale. L’accueil familial thérapeutique repose sur une synergie entre le travail des accueillants familiaux qui apportent une ouverture vers la réinsertion sociale et celui d’une équipe pluridisciplinaire formée aux techniques de réhabilitation. Le projet thérapeutique de l’établissement s’appuie sur un programme appelé EDUcation thérapeutique et accueil Familial (EDUFA), qui comporte différents modules : remédiation cognitive, maladie et médicaments, tabacologie et équilibre alimentaire. Ces modules correspondent à des enjeux majeurs de la prise en charge de la schizophrénie et des comorbidités qui lui sont associées.

This study developed a hydrocode to numerically simulate both conical and linear-shaped charges using an Eulerian multi-material and multi-phase flow model. Elasto-plastic solids and the detonation of a high explosive charge were modeled using a Johnson–Cook material model and the programmed burn model, respectively. Further, the plasticity of the solids was calculated using a radial return mapping algorithm. The model was solved using a high-resolution computational fluid dynamics (CFD) technique on Cartesian grids. Material interfaces were tracked using the level-set method, and the boundary conditions were imposed using the ghost fluid method. The developed hydrocode was validated using high-speed impact problems. Consequently, the developed hydrocode was used to successfully simulate the evolution and penetration of metal jets in shaped charges after a detonation.

La prise en charge du sujet alcoolodépendant, utilisant les techniques de thérapie cognitivo-comportementales (TCC) a suivi l’évolution de ces techniques. Historiquement, les TCC ont évolué selon trois vagues. La première vague est comportementale et s’inspire directement des théories de l’apprentissage. On cherche dans ce cas à aider le patient à modifier son comportement. La deuxième vague des TCC a été centrée sur les cognitions. Le but du travail psychothérapique était d’aider le patient à favoriser des pensées alternatives lors d’une situation à risque. Depuis les années 2000, est née une troisième vague. Celle-ci se centre sur les relations entre la cognition et l’émotion [4]. L’hypothèse est que les troubles psychiques résulteraient d’une suppression erronée d’information émotionnelle. Par conséquent, la thérapie de la mindfulness – de la pleine conscience –, c’est à dire de l’expérience vécue pleinement de l’ici et maintenant, s’intéresse au contexte des expériences psychologiques. Les premiers programmes de thérapie de la pleine conscience ont été développés pour le sujet souffrant de maladies ou de douleurs chroniques pour améliorer leur qualité de vie [5]. Pour le sujet alcoolodépendant, un programme intégrant la pratique de la mindfulness à la prévention de la rechute (Mindfulness-Based Relapse Therapy: MBRT) [3,6,7]. Ce programme thérapeutique, que nous décrirons, s’appuie sur huit séances hebdomadaires. Nous avons aussi utilisé le programme classique de la mindfulness chez le sujet alcoolodépendant. La thérapie de la pleine conscience nous paraît intéressante à la fois dans la gestion du craving, mais aussi dans la gestion des émotions. Enfin, nous chercherons aussi, à partir des lectures des Stoïciens, d’Augustin (De Trinitate) [2], et d’Arendt (La vie de l’esprit) [1] à expliquer comment la pleine conscience est acceptation, acceptation d’un ordre qui ne dépend pas de nous, mais acceptation constitutive de notre liberté. L’acceptation est donc éthique.

Si les outils numériques et particulièrement les tablettes tactiles ont été rapidement investies pour la prise en charge des troubles du spectre autistique et d’autres formes de troubles du développement, leur utilisation raisonnée devant la multitude des applications ne va pas sans difficulté. Construit en cohérence avec les recommandations HAS et les programmes scolaires de l’éducation nationale, le programme LearnEnjoy développé en France est un support particulièrement bien structuré pour guider cliniciens et éducateurs dans leur travail. En s’appuyant sur la construction et le fonctionnement de cet outil, la session développera sa thématique en décrivant l’environnement théorique, le contenu des applications, l’utilisation dans le cadre de recherches en psychologie du développement, et le suivi dans plusieurs institutions de la mise en œuvre de ce programme. Olivier Bourgueil, psychologue spécialisé en analyse fonctionnelle du comportement (ABA) montrera comment le contenu des activités a été construit en prenant en compte les spécificités cognitives, attentionnelles et motivationnelles des enfants avec TSA. Les applications sont autant un espace pour le travail des compétences de l’enfant (ou de l’adulte) qu’un guide pour les accompagnants, parents, éducateurs et rééducateurs. Sylvain Moutier, professeur de psychologie cognitive, décrira un protocole de recherche en cours sur les troubles des fonctions exécutives utilisant ce support numérique, recherche visant à mieux connaître les particularités développementales de certains handicaps cognitifs et à améliorer les remédiations en fonction des difficultés observées. Introduction : L’Éducation Thérapeutique du Patient fait partie intégrante de la prise en charge du patient. Pourtant, un programme dédié à la polyarthrite rhumatoïde rencontre des difficultés de recrutement. Objectif : L’objectif principal de l’étude était d’analyser le recrutement et secondairement d’identifier des facteurs influençant la décision des patients. Méthodes : L’analyse réalisée sur 6 mois a recueilli le nombre de patients hospitalisés dans les différents secteurs du département de rhumatologie, le nombre de patients informés, leur décision, les données démographiques, professionnelles, géographiques, médicales, le statut professionnel des informateurs. Résultats : 10,2 % de la file active ont été informés et 7,4 % inclus au programme avec une capacité d’information variable selon les secteurs et les professions des informateurs. 27,5 % des patients informés ont refusé de participer. La situation géographique et le secteur hospitalier dans lequel l’information est donnée pourraient avoir un impact sur la décision du patient. Discussion : Les résultats montrent une carence dans l’information. Le manque d’implication des professionnels de santé interroge sur les freins à l’application des textes législatifs. Il apparaît essentiel de les sensibiliser à diffuser l’offre éducative en étant attentif à la qualité du discours et aux conditions favorables dans lesquelles l’information est donnée. Conclusion : Le déploiement de l’ETP faisant évoluer les professionnels de santé et leur formation, les programmes disponibles toujours plus nombreux, permettront que tout patient puisse avoir connaissance de l’offre éducative qui le concerne et choisir d’en bénéficier ou pas.

ABSTRACTSchool meals today are provided on a compromise basis; some pupils are charged and others are not. The compromise thus rejects two types of universalistic principles: allocating private benefits by the market and provision of merit goods collectively free of charge. The article examines, empirically and conceptually, the changing meaning of school meals; the demand and supply of school meals and the allocation of public subsidies to non-needy children; conflicting goals of expert nutritionists, egalitarians, redistributionists, efficient managers, and parents and children. The article concludes by considering political constraints on rationalising the programme in the name of universalistic principles. These constraints include the inheritance of commitments from previous governments, collective responsibility of spending and Treasury ministers; intra-party divisions between frontbench and backbench MPs; and low political salience of charging as an issue.

Korean Institute of Medical Education and Evaluation (KIMEE) evaluates the 'Basic Medical ScienceCurriculum' in medical schools in Korea. The KIMEE recommends standards, which are structured to 6 domain with a total of 97sub-domain in the medical education. 6 domains are operating system, basic medical curriculum, student, faculty, facilities&equipments, and post-graduation. KIMEE has developed 97 global standards for appraisal of basic medical education. Récemment, le concept de Serious game (jeu sérieux) [1] a fait son apparition dans les programmes d’éducation thérapeutique [4]. BIPOLIFE®[3], programme d’information ludoéducatif sur le trouble bipolaire accessible sur Internet, a été développé afin de s’intégrer dans la panoplie psychoéducative. BIPOLIFE® consiste à faire évoluer son avatar au quotidien. Ce jeu est basé sur 3 cibles : les règles hygiénodiététiques, l’observance médicamenteuse, le recours au médecin/psychiatre.ObjectifMise en place d’une étude pilote évaluant l’acceptabilité de BIPOLIFE® et son effet sur la connaissance de la maladie auprès de 63 patients bipolaires sur 3 semaines. Les patients ont été évalués avec l’échelle de connaissance du trouble bipolaire et une échelle d’acceptabilité de BIPOLIFE®.RésultatsBIPOLIFE® semble être mieux accepté par des jeunes (p = 0,011) et plus efficace sur des hommes (p = 0,043) ayant une moins bonne connaissance de la maladie initiale (p ≤ 0,001). La majorité des patients a rapporté en avoir tiré bénéfice sur leur vie quotidienne, leur connaissance ou leur gestion de la maladie. Les patients ont déclaré que BIPOLIFE® est à conseiller à des personnes souffrant de TB (32 %), à leur proche entourage (32 %) ou au grand public (16 %). ConclusionNotre étude montre que BIPOLIFE® est déjà un outil pertinent dans la prise en charge des patients bipolaires. Des études complémentaires sont nécessaires pour évaluer précisément sa place dans les programmes de psychoéducation.

Three senior Chief Executives of acute hospital trusts in the UK recently visited the Northern Territory (NT)and South Australia (SA) to study remote and rural health care in general - and Aboriginal health in particular. As with all other aspects of Aboriginal life, the subject of health status is highly charged and generates heightened emotions and intense political debate across the country but particularly in the NT and SA where many of the remote indigenous people live. Every "mainstream" Australian has an opinion on the trials and tribulations of the indigenous people.The field study was part of the NHS Leadership Centre's Senior Chief Executives' Development Programme. Itcomprised a longitudinal journey across the continent from Darwin (NT) through to Alice Springs to Tanundain the Barossa Valley and then on to Adelaide following the route of the 2,500 kilometre Stuart Highway. Itinvolved visiting rural health services, and meetings with Aboriginal leaders, academics, health practitioners and senior officials of the SA government.A specific research topic was to understand how practitioners working in extreme environments, and delivering long-term programmes of care, can maintain their morale and motivation.

Après que des études contrôlées effectuées à l'étranger aient démontré que des cours d'auto prise en charge permettent d'améliorer l'état général de patients chroniques d'âge avancé de façon durable à un prix abordable, un programme similaire appelé ALLFIT a été établi en Suisse pour les patients âgés atteints de maladies chroniques cardiaques, pulmonaires, rhumatismales ou de diabète sucré. Une analyse des besoins révèle qu'environ 7% des patients âgés pourraient bénéficier d'un tel programme qui consiste en une évaluation individuelle des buts personnels grâce aux conseils d'une infirmière, un entraînement hebdomadaire de la résistance, de la force et de l'équilibre ainsi que la participation à huit cours d'auto prise en charge en groupe. 85 des 100 premiers participants à ce programme l'ont terminé. Personne n'a abandonné en raison d'attentes non satisfaites. 99 patients ont pu définir les buts individuels et 80 les ont atteints au moins partiellement en trois mois. 21% ont pu améliorer leur capacité à marcher sur terrain plat et 25% à monter les escaliers. Ceci signifie qu'en Suisse aussi la prise en charge par le malade lui-même peut être utilisée pour améliorer l'indépendance des personnes âgées.

Breast cancer is one of the leading forms of cancers in women worldwide. In Poland, it accounts for approx. 20% of all cancers diagnosed, with approximately 11,000 new cases and 5000 deaths from this disease annually. To prevent unfavourable statistics, Poland introduced free breast cancer screening programmes, available to women aged 50–69. Over a million women take advantage of this programme each year. The aim of the research was to assess the quality of consent women give prior to mammography screening and address the question of whether this quality is sufficient to make an informed choice. The study was conducted on a representative group of 600 Polish women over 50 years old (475 of them had undergone mammography screening), who agreed to take part in the study. Using the computer-assisted interview technology (CATI) method, all women were asked about their perception of breast cancer and screening and those who had undergone mammography were quizzed about the consent process. They will form the focus of this research. The validated tool contained items on both the benefits and risks of screening. The results indicate that the quality of informed consent was insufficient. A discrepancy was observed in the awareness between the benefits and risks of mammography screening. The main motivations to undergo screening were: prophylactic purposes and the free-of-charge nature of this health service. Population-based screening programmes for breast cancer should be reconsidered in terms of information policy, and the quality of informed consent should be increased.

Vivre au quotidien avec un trouble bipolaire ou vivre avec un patient atteint d’un trouble bipolaire génère un stress familial intense [1]. Les recommandations de la Haute Autorité de santé (HAS) et le développement parallèle de l’éducation thérapeutique du patient proposent 3 axes prioritaires de prise en charge des aidants familiaux : l’information sur le trouble, l’aide dans la gestion du trouble au quotidien et le soutien psychologique. Dans ce contexte, les programmes de psychoéducation familiale constituent un dispositif pertinent pour répondre à la question complexe de la place des familles dans la prise en charge : comment inclure les aidants dans le soin tout en leur garantissant de ne pas devenir des soignants [2] ? Des nombreuses études ont pointé le bénéfice d’une telle prise en charge sur l’évolution de la pathologie [3,4]. Ainsi dans le cas des troubles bipolaires, la psychoéducation a un impact sur le nombre d’épisodes dépressifs et maniaques, sur leur durée et leur intensité. À l’heure actuelle peu de travaux nous renseignent sur les mécanismes qui permettent une telle amélioration et en particulier sur les processus favorisant les modifications de stratégies des personnes dans la gestion du trouble bipolaires. Un des leviers essentiel à évaluer concerne la représentation de la maladie. En effet, l’éducation thérapeutique place d’emblée la personne au cœur du dispositif en s’appuyant sur ses connaissances et sa représentation de la maladie et ne peut se réduire à l’instruction. Notre objectif est donc d’évaluer le poids de l’évolution des représentations sur les stratégies d’adaptation des patients et des familles confrontées au trouble bipolaire. Cette étude a été effectuée auprès de patients ayant reçu un diagnostic de trouble bipolaires et de leur famille pris en charge dans un programme de psychoéducation de 12 séances (sur 6 mois). Nos résultats offrent des pistes de compréhension des processus en jeu au cours d’une psychoéducation multifamiliale.

The installation of solar panels in rural community Malaysia are still preliminarily explored due to the excessive cost and limited access to the technology. This limitation has also led to dissemination delays especially in public-funded buildings such as mosques, public halls, hospitals and schools. Meanwhile, the existing subsidy of electricity supply rate to the society has led the community to become highly dependent and demanded on the power supply from Tenaga Nasional Berhad (TNB). Therefore, the increase of electricity tariff since 2014, has subsequently resulted in the increasing charge of monthly electricity bills. Without awareness, the electricity usage and charges will keep on increasing and may affect the monthly expenses especially to the public and private institutions. This situation may become worst to the self-funded institutions in rural areas including the ‘tahfiz’, international and conventional religious schools. The concern of this excessive charges on electricity tariff may influence these institutions to outsource funds such as through collecting donations from the community in which are not sustainably sufficient. Therefore, this research investigates an alternative supply of electricity that may reduce the cost and will benefit the institutions through bill-savings. The objective is to determine whether solar panel installation through a community-service programme can help the institutions to not only becoming electricity-sufficient but could also save the energy by generating power from solar energy. A case study was conducted at the ‘tahfiz’ school in Perak Tengah to determine the effectiveness of how this institution could reduce electricity bills besides establishing the green energy initiative. Remarkably, through the installation of a 2 kW solar power system, this research has discovered that the institutions have successfully saved 70% of the existing monthly electricity expenses. Therefore, this finding acknowledged solar energy through community-service as sufficient green initiative programme towards achieving sustainable living.

19 Background: Sustainable development goals and voluntary global NCD targets 2025 plan to achieve 80% coverage by essential NCD medicines and technologies and drug therapy and counselling coverage to the tune of 50%. This can only be achieved by making coverage wider, going to periphery and urgently creating workforce capable of doing such an activity at scale. District cancer care model (Pendharkar model) tried to involve these principles and shown a scalable acceptance by the health system. Methods: To expand the access administratively, most peripheral but medically most suitable, district hospitals, were used to create nodal cancer units. To achieve the scale, all districts of a state were targeted. For creating an alternate workforce existing medical officers and nurses were utilised. Results: Over a period of 5 years seven states of India, having a population of more than 250 million, have agreed to join the programme.176 districts have created nodal cancer units with physician and two nurses in-charge of the programme. They delivery variety of care including counselling, chemotherapy, supportive, palliative, follow up care. They serve as nodal units for public education driving government programmes including public education. At least 300-500 patients avail the services on a daily basis, including more than 200 chemotherapy procedures in a day. On world cancer day educational activities happened in all these districts simultaneously creating massive impact. Conclusions: This unique innovative model has proven that health system changes can be brought at very large scale using existing government possibilities. Available infrastructure, human resource, minimal financial changes with administrative reforms can lead to impactful change in access to cancer care at scale.

Background: One of the most debilitating effects of advancing age is the progressive decline in aerobic exercise capacity. To prevent this decrease, exercise prescription has been proved to be effective in compensating for the age-associated decline in maximal oxygen capacity.
Methods: 27 healthy sedentary elderly men (10) and women (17) aged 67±7 yr, were involved in a tailored 6-week intermittent exercise programme, with 30 min cycling per session, 3 times a week. One session alternated 4 min at a workload corresponding to the ventilatory threshold (called “bases”) and 1 min to a charge corresponding to 90% of the maximum tolerated power of the subjects (called “peaks”). This was repeated 6 times during one session. To determine these charges, all subjects underwent a 1-minute-step maximal incremental exercise test to find out their maximal tolerated power, peak oxygen consumption (VO2peak), maximal heart rate, and maximal lactate. A second maximal incremental exercise test was performed after the programme, to compare the different parameters.
Results: For the whole group, maximal tolerated power increased from 113±8 watts to 13±9 watts (P < 0.001), VO2 peak increased from 19.8±0.9 ml·kg-1·min-1 to 22.2 ±0.9 ml·kg-1·min-1 (P=0.002), maximal heart rate (143±4 vs 144±4 beats·min-1) and maximal lactate (5.4±0.3 vs 5.7±0.3 mmol·L-1) did not change. VO2 at ventilatory threshold increased from 950±57 ml·min-1 to 1095±69 ml·min-1 (P=0.007), and the corresponding power increased from 65±5 watts to 82±6 watts (P < 0.001).
Conclusions: Even after a short time training programme (6 weeks), we observed a significant improvement in aerobic capacity, and especially in sub-maximal workloads, which represented, for these subjects, their daily-life capacity.

Background and aims Learning from error can have a negative impact on the staff involved in the error (‘second victim phenomenon’1). We created a project, based on the principles of the Learning from Excellence project,2 to learn from excellence and correct the imbalance of negative to positive feedback in the context of hospital practice. Methods and results Using a questionnaire, we surveyed staff on existing feedback mechanisms and morale. We then introduced a system where staff recorded and commented on examples of excellence in practice. Recipients and their supervisors received copies of these reports and the feedback was analysed and discussed with senior staff (consultant, senior charge nurse, managers). We re-audited the staff two months after starting this project and noted improvements in staff morale and in positive reporting. Conclusions This project has improved the process of giving and learning from positive feedback and had a significant impact on staff morale. We can also demonstrate an example of improved clinical practice (from feedback received) and will now attempt to measure clinical outcomes with a new prospective study. Finally, we hope to set up a regional programme of reporting excellence in South-East Scotland.

In Chapter 14 of Atomic and Molecular Processes, Bates (1962) outlines a procedure for calculating ion-atom cross sections in the plane-wave Born approximation (pwBa). The procedure involves integration over the product of elastic scattering factors or generalized oscillator strengths for excitation or ionization from both projectile and target. We have programmed this procedure to use our large database of excitation and ionization generalized oscillator strengths (GOS). The program calculates both cross sections (CS) and stopping power (SP) on a subshell basis. The calculations are done in the center of mass system where the distinction between projectile and target is lost. Thus, the SP in the laboratory frames of both target and projectile are symmetrical in nuclear and net charges. The traditional simple modeling of SP, using scaled proton SP and an effective projectile charge, is unsymmetrical, and therefore dubious as a guide for extrapolating to ion-ion SP. At high projectile energy, the SP curves, as a function of increasing projectile charge, approach the scaled protonic result from above, indicating that lowering the average charge raises the SP, in contradiction to the traditional picture that the projectile SP increases with increasing effective charge (assuming there is an underlying physical reality relating the effective and average charge). Comparison with experimental SP data (mostly from 30 years ago) shows generally poor agreement for Li ion projectiles in the 1–10 MeV range.
