Il distretto estrattivo di Iscehisar, da cui proveniva il marmor phrygium (pavonazzetto), è uno dei più grandi dell'antichità romana nonché uno dei più studiati e documentati. Il primo ad interessarsi alle cave di Docimium fu J. Röder, che effettuò una ricognizione sistematica delle cave con la conseguente produzione della prima pianta topografica dell'antico sito estrattivo, allora ancora in gran parte integro anche con i rispettivi cumuli detritici. M. Waelkens rivolse la propria attenzione non solo alle cave, ma anche alla loro produzione, mentre J. C. Fant affronto soprattutto l'analisi dei corredi epigrafici dei blocchi di cava in modo da comprendere l'organizzazione delle attività estrattive di queste grandi cave imperiali. Le iscrizioni di cava sono state oggetto inoltre degli studi approfonditi di M. Christol e T. Drew-Bear, che non di rado si ponevano in forte contrasto e dissenso con quelli di Fant, e, negli ultimi anni, di P. Pensabene.

It has been ten years since the publication of Lorenzo Lazzarini's monumental volume on the quarrying, use and properties of the coloured marbles of Greece: Poikiloi Lithoi, Versiculores Maculae: I Marmi Colorati della Grecia Antica (Lazzarini 2007). The first study since Angelina Dworakowska's Quarries in Ancient Greece (Dworakowska 1975) to attempt a large-scale examination of quarrying across Greece, Lazzarini's approach is fundamentally an archaeometric one. Analysis of the evidence for quarrying in different regions is set alongside minero-petrographic and geochemical analyses of the materials extracted. Lazzarini focuses on 12 lithotypes: marmor lacedaemonium from Laconia, variously referred to as serpentino and porfido verde antico; three stone types from the Mani peninsula: rosso antico tenario, nero antico tenario and cipollino tenario; from Chios, the famous marmor chium or portasanta, breccia di Aleppo and nero antico chiota; the breccia di settebasi and semesanto of Skyros; the intensively exploited marmor carystium or cipollino verde, as well as the marmor chalcidicum or fior di pesco from Euboea; and from central and northern Greece, marmor thessalicum or verde antico and the breccia policroma della Vittoria. For each of these lithotypes, Lazzarini considers the evidence for their use and distribution, illustrated with a distribution map in each case, and provides a thorough overview of what is known about their quarries. Archaeological and geological approaches are here combined, and this is a hallmark of much recent work on the question of quarrying and stone use through Greek history.

Pemanfaatan pelayanan ANC oleh sejumlah ibu hamil di Indonesia belum sepenuhnya sesuai dengan pedoman yang ditetapkan.Tujuan dari Penelitian ini adalah untuk mengetahui hubungan faktor umur, paritas, dukungan suami/keluarga, dukungan tenaga kesehatan, media informasi, dan kondisi ibu hamil dengan pemanfaatan antenatal care di Puskesmas Madising Na Mario Kota Parepare. Jenis penelitian ini adalah observasional dengan menggunakan desain cross sectional study. Populasi dalam penelitian ini yaitu pasien ibu hamil yang memeriksakan kandungan di Puskesmas Madising Na Mario Kota Parepare sejumlah 508 ibu hamil. Penentuan sampel dengan menggunakan teknik accidental sampling sehingga jumlah sampel dalam penelitian ini adalah sebanyak 84 sampel. Hasil penelitian variabel yang menunjukkan ada hubungan dengan pemanfaatan ANC adalah variabel umur (p=0,012; r=0,273), dan variabel yang tidak ada hubungan adalah variabel paritas (p=0,095; r=0,183), variabel dukungan suami/keluarga (p=0,293; r=0,116), variabel dukungan tenaga kesehatan (p=0,605; r=0,057), variabel media informasi (p=0,964; r=0,005), dan variabel kondisi ibu hamil (p=0,861; r=0,19). Oleh karena itu, diharapkan pada penelitian selanjutnya memasukkan lebih banyak variabel mengenai faktor pemanfaatan ANC

Glass Furniture Industry is one of the growing industrial sectors in Indonesia. Providing attractive interior design and artistic value so as to bring comfort to consumers and make it their own value for art connoisseurs. Furniture not only has the function of comfort and neatness but also carries social meanings. Furniture indirectly conveys a person's economic status. This study aims to determine the impact and sales turnover on Margo Glass Gallary during the COVID-19 pandemic. This research method uses a case study approach with primary collection obtained by Margo Glass Gallery Sukoharjo through interviews. The results obtained are analyzed qualitatively. The results showed that sales and orders for furniture glass products at Margo glass Gallery decreased by 60% during the pandemic due to delays in the distribution process, due to restrictions on activities, both shipping from within the country and abroad, especially shipments abroad which were constrained in shipping. custody of goods.

CV Margo Utomo dairy farm has approximately 99 cows. Livestock activities include feeding, cage cleaning, milking and livestock bathing. High feed and water requirements resultes high amounts of waste feed and waste produced. If wastewater discharged into channel, it could cause water pollution and could reduce environment. There were two types of waste water. The wastewater that disposed directly wastewater in to channel and the wastewater that settled in basin for three days. Steps of this research included field observation, secondary and primary data collection, problem identification, clean production alternatives identification, feasibility analysis and alternative selection based on priority scale of alternative clean production actions. Feasibility analysis includes technique, environment and economy. Assessment of feasibility based on assessment indicators in the form of questionnaires. This study used NPV, IRR, B / C Ratio, and PBP. There were three alternatives based clean production, that could be applied at the CV. Margo Utomo, such as biogas, water recycle , and cleaning pool for cows. Based on these alternatives, biogas was a best priority to applied in the CV. Margo Utomo dairy farm. RiassuntoSono descritte con la stessa metodologia impiegata in altre quattro aree italiane sedi di Registro psichiatrico dei casi, le caratteristiche del bacino di utenza, la tipologia e l'organizzazione dei Servizi della Regione Liguria. Il sistema informativo psichiatrico ligure nasce ufficialmente nel marzo 1987 con una deliberazione della Giunta Regionale che conferisce l'incarico di organizzazione del sistema informativo all'lstituto di Scienze Sanitarie Applicate dell'Università di Pavia. La fonte dei dati è rappresentata dai Servizi psichiatrici della Liguria, comprendenti 20 Servizi Territoriali di Salute Mentale (SSM), 9 Servizi Psichiatrici di Diagnosi e Cura (SPDC) e 2 Presidi Socio Sanitari (PSS). Il Registro psichiatrico è gestito a livello cartaceo in tutti i Servizi della Regione; tuttavia in 12 dei Servizi di Salute Mentale, in 2 dei Servizi Psichiatrici di Diagnosi e Cura e nei Presidi Socio Sanitari si è già passati ad una gestione informatizzata del Registro. La rilevazione dei dati è iniziata dal 1988 è nel 1989 è stata fatta la prima verifica regionale sulla funzionalità del sistema. I risultati relativi all'utenza dei servizi territoriali evidenziano tassi di prevalenza annua che vanno dal 200 al 500 per 100.000 abitanti. Per quanto concerne gli interventi residenziali, SPDC e PSS sono stati analizzati separatamente confermando peraltro il dato della letteratura cioè di una eccedenza di diagnosi di psicosi soprattutto tra i maschi ricove- rati in SPDC e tra le femmine ricoverate nei PSS. La struttura per eta evidenzia che si tratta di persone di età avanzata in particolare tra le donne.

The research presented here is based on studies of the sociology of music, as well as on analyses of the dynamics of immigration. This explored some of the social dynamics and actions of a multi-ethnic orchestra founded and operating in Rome. The purpose of the study was to examine this orchestra as an example of good practice regarding intercultural integration and was the criterion by which we chose to analyse the Orchestra di Piazza Vittorio, founded in 2002 by Mario Tronco in one of Rome’s central multi-ethnic districts, the Esquiline. The question posed here is whether the creation of a multi-ethnic orchestra can act as an alternative model which, by means of the socialisation process, redefines and rediscovers the age-old relational and integrating functions of music, availing of the collective memory, identity, heritage and varieties of music, without forfeiting its own identity.

The increase in urban slum areas provokes the adaptation impact of the community towards the capability conflict and the need for shelter. Slums management by displacing residents often raises new problems that are equally complicated. Hence, another alternative treatment by maintaining household-scale green open space (RTH) to improve the quality of life of the community and the environment is necessary. The problem of slums in big cities, including in the city of Makassar has not been able to be overcome properly, one of which is the problem of slums in the Mario Sub-District, Mariso District, Makassar City where the conditions of the slums are a severe result. The concept of managing the existing slums is not only producing buildings that are habitable, but also providing green open space is one of the solutions in structuring the existing area. At present, green open space not only provides open space planted with green vegetation but has the concept of Sustainable Open Green Space or known as the Green RTH concept. In this case, the concept introduced to local residents is green open where available wastewater treatment, composter and infiltration wells and biopores. It is these facilities which are expected to be able to deal directly with the problems of clean air, wastewater and waste problems that are always present in slums. The presence of the “Sustainable” Green Open Space which explains the concept of Green RTH namely green waste and green water can minimize the slums in the region. In green waste concept, 3 communal composters are made, each composter has a volume of 440 liters for 1235.775 liters / day of waste generation, while in the green water concept, a Wastewater Treatment Plant has an area of ​​302.8 m2, 1 infiltration well , 5 biopori infiltration holes and 2 hydrants.

Konsekuensi logis dari ditetapkannya Indonesia menjadi negara agraris adalah bahwa sektor pertanian memiliki peranan penting dalam memajukan perekonomian nasional. Umumnya, sektor pertanian ini dibagi menjadi dua bagian, yaitu perkebunan besar dan produksi petani kecil. Apabila melihat fakta yang terjadi, kehidupan para petani perlu mendapat perhatian lebih karena masih jauh dari kata sejahtera. Salah satu bentuk kerjasama yang sering dilakukan para petani disebut dengan ‘maro’. Misalnya adalah yang terjadi di Desa Gunung Sari, yaitu kerjasama pengelolaan kebun kopi. Islam sebagai agama mayoritas penduduk, memiliki konsep tersendiri mengenai kerjasama antarpetani dalam pengelolaan lahannya, yakni ada yang disebut muzara’ah, mukhabarah dan musaqah.. Penelitian ini menggunakan metode kualitatif, dengan jenis penelitian berupa deskriptif analitis. Pendekatan Penelitian yang digunakan adalah case approach atau pendekatan kasus, sedangkan teknik analisis data yang didasarkan pada metode interpretasiteleologis. Hasil penelitian menunjukkan bahwa tidak ada konsep baku mengenai kerjasama ‘maro’ antarpetani kopi di Desa Gunung Sari. Meski demikian, secara garis besar konsep kerjasama ini sejalan dengan konsep musaqah yang ada dalam Fikih Islam. oleh sebab itu, di harapkan kedepan terdapat aturan baku mengenai konsep kerjasama musaqah ini, sehingga para petani kopi yang mayoritas beragama Islam dapat mempraktikkan kerjasama yang sejalan dengan syariat Islam.

Campbell e il primo film, La Cosa da un altro mondo, da esso ricavato da Christian Nyby e Howard Hawks rifatto poi da John Carpenter con il semplice titolo La Cosa? E come si arriva al quadro storico-politico dell’Italia degli anni Settanta passa attraverso il filtro della fantascienza? Perché ad esempio proprio l’Italia segnata dal potere mafioso (“Cosa Nostra”) e dal terrorismo diventa lo spazio funzionale privilegiato del «paradigma della Cosa», ossia del lacaniano «misterioso oggetto alieno non morto che cade dall’universo, un oggetto non umano ma tuttavia vivente e persino in grado di avere, spesso, una propria volontà maligna» (Žižek)? Il fantasma incombente di queste oscure, talvolta concomitanti “Cose” italiane, durature, insidiose, si manifesta in film come Perché si uccide un magistrato di Damiani, in cui il protagonista, giornalista e cineasta d’inchiesta si chiama appunto “Solaris”, come il romanzo di Lem. Si manifesta in Todo modo di Elio Petri, in Identificazione di una donna di Antonioni, infine in La Cosa di Nanni Moretti e Buongiorno, notte di Marco Bellocchio, dell’impossibilità allora come oggi di intercettare la verità fattuale, oggetto non identificato per eccellenza.

Historical heritage plays a fundamental role in society. One of the primary targets for European countries is to ensure its safeguarding so that it can be enjoyed by future generations. This paper defines a procedure for the safeguarding of cultural heritage which is often in an advanced state of degradation and threatened by climate change and the increasing number of tourists. The methodology consists of three basic steps: the identification of the technical elements on which intervention is necessary, the assessment of the vulnerability level of these elements considering exogenous factors and the hierarchization of the most vulnerable elements through the application of the Analytic Hierarchy Process. The case study is the “Patio de los Leones” a world-famous site within the Alhambra and Generalife which every year attracts over 2 million tourists to Granada, Spain. The aim is to set priorities for intervention on the 124 “Macael” marble columns according to the need for restoration. The validation of the methodology and management of the data to achieve the hierarchy among the columns was done using an HBIM model. Il patrimonio storico culturale ha un ruolo fondamentale dal punto di vista sociale. Uno degli obiettivi primari per i paesi europei è quello di garantirne la salvaguardia per consentirne la fruizione alle generazioni future. In questo articolo si definisce una procedura utile alla conservazione di tale patrimonio che spesso verte in condizioni di degrado molto avanzate e che i cambiamenti climatici ed il crescente numero di turisti mettono ulteriormente a rischio. Nello specifico, la metodologia consta di tre passaggi fondamentali: l’individuazione degli elementi tecnici sui quali è necessario un intervento, la valutazione del livello di vulnerabilità di tali elementi tenendo conto dei fattori esogeni e delle caratteristiche morfologiche e di localizzazione, e la gerarchizzazione degli elementi maggiormente vulnerabili attraverso l’applicazione dell’Analytic Hierarchy Process. Il caso applicativo, è il “Patio de los Leones”, sito di fama mondiale all’interno dell’Alhambra e del Generalife, che attira a Granada, in Spagna, oltre 2 milioni di turisti ogni anno. Il fine, nel caso studio, è quello di definire le priorità d’intervento sulle 124 colonne in marmo di “Macael” che necessitano di delicati interventi di restauro. La verifica della metodologia, l’archiviazione e gestione dei dati per giungere alla gerarchizzazione degli interventi di restauro è stata fatta attraverso un modello HBIM

“Geotourism” is a particular type of “sustainable tourism” that is still in an embryonic stage, especially in Italy. The main goal is the transmission of geological knowledge to increase the awareness about geoheritage, geo-resources and geo-hazards. The geoparks represent ideal sites, with a strong educational significance for students, teachers, geo-tourists, and guides interested in geological and environmental sciences, though at different levels. With this in mind, we propose a geoitinerary through some of the most geologically interesting coastal areas in the Cilento, Vallo di Diano, and Alburni Geopark. The aim of the geoitinerary is to provide a good example of how geosites could be promoted through geotourism and used as means of divulgation of geological and environmental knowledge. The selected sites are the San Marco coast, the Licosa Cape and the Elea-Velia archaeological area. They are included in the official list of geosites and geomorphosites of the Geopark and have a relevant stratigraphic and geoarcheological value. The San Marco coast and the Licosa Cape are the “best sites” in the Geopark where Quaternary coastal deposits and morphologies are represented. The Elea-Velia site is one of the most famous archeological sites in the Geopark, which is also representative of complex human-environment interactions. Despite their high scientific significance, the sites that we have selected are not included in a specific promoting program. We have so tried to fill this gap by providing the scientific background for their geotouristic promotion that could also serve as an instrument for the increase of the local economy.

The Basilica of San Marco in Venice is a well-known masterpiece of World Heritage. It is a real multi-faceted architecture. The management of the church and its construction site is very complicated, and requires an efficient system to collect and manage different kinds of data. The BIM approach appeared to be the most suitable to collect multi-source data, to monitor activities and guarantee the well-timed operations inside the church. The purpose of this research was to build a BIM of the Basilica, considering all aspects that characterize it and that require particular care.<br><br> Many problems affected the phase of the acquisition of data, and forced the team to establish a clear working pipeline that allowed the survey simultaneously, hand in hand, with all the usual activities of the church. The fundamental principle for the organization of the whole work was the subdivision of the entire complex in smaller parts, which could be managed independently, both in the acquisition and the modelling stage. This subdivision also reflects the method used for the photogrammetric acquisition. The complexity of some elements, as capitals and statues, was acquired with different Level of Detail (LoD) using various photogrammetric acquisitions: from the most general ones to describe the space, to the most detailed one 1:1 scale renderings. In this way, different LoD point clouds correspond to different areas or details.<br><br> As evident, this pipeline allows to work in a more efficient way during the survey stage, but it involves more difficulties in the modelling stage. Because of the complexity of the church and the presence of sculptural elements represented by a mesh, from the beginning the problem of the amount of data was evident: it is nonsense to manage all models in a single file.<br><br> The challenging aspect of the research job was the precise requirement of the Procuratoria di San Marco: to obtain the 1:1 representation of all the mosaics of the Basilica. This requirement significantly increased the effort in the acquisition stage, because it was necessary to reach a submillimetre resolution in the photographic images sufficient to distinguish perfectly each single <i>tessera</i>, also in the highest domes (28 meters). Furthermore, it introduced a new problem about the management of the gigapixel - orthophotos.<br><br> The BIM approach presented in this paper tries to offer a solution to all these problems. The BIM application is based not on commercial software, but on a self-implemented system, which was previously tested on the Main Spire of Milano Cathedral. The multi-scale and multi-area approach have also been maintained in the BIM construction phase.<br><br> In the case of Basilica di San Marco, the most important requirement was the management of the orthophotos of each single element. It was necessary to give the user the possibility to recover, for each item, not only the geometric model, but also the raster representation -orthophoto- of its surface: in order to do it, the BIM model acts as a three-dimensional catalogue.

ABSTRAKDesa Andongsari telah dijadikan desa KKN tematik pengembangan Desa Wisata dan Wirausaha Sejahtera. Mayoritras profesi masyarakat sebagai petani baik itu petani maupun sebagai buruh tani. Permasalahan-permasalahan yang sering dihadapi petani dalam budidaya cabe besar yaitu adanya serangan organisme pengganggu tanaman (OPT). Efek penggunaan pestisida frekuensi tinggi menimbulkan efek samping yaitu hama yang resisten. Diperlukan teknik pengendalian yang ramah lingkungan dan ramah terhadap biaya produksi, salah satunya adalah agens hayati yang bersifat memarasit serangga hama. Mitra kegiatan pengabdian ini sendiri yaitu Desa Andongsari dan kelompok tani yang ditunjuk sebagai panitia pelaksana oleh desa adalah kelompok tani Margo Makmur I dan didampingi oleh Kelompok Wanita Tani (KWT) Larasati. Metode pelaksanaan menganut prinsip Plan Do Check Action (PDCA) meliputi kegiatan observasi, perencanaan program, pelatihan berupa praktek langsung. Penggunaan agens hayati tersebut menggunakan media padat karena berdasarkan observasi kondisi lingkungan dan teknik yang mudah digunakan lebih mengarah ke penggunaan media padat. Pelatihan produksi tersebut dibagi beberapa tahapan kecil untuk mempermudah dalam pengawasan dan evaluasi di tiap tahapan. Hasil dari kegiatan ini adalah ada peningkatan pemahaman petani terkait bahaya penggunaan pestisida dan peningkatan minat masyarakat petani terkait penggunaan agens hayati, selain itu juga ada peningkatan softskill berupa pengetahuan dan ketrampilan masyarakat dalam memproduksi agens hayati. Kata kunci: agens hayati; cendawan; desa andongsari; desa wisata; entomopatogen. ABSTRACTThe problems that are often faced by farmers in the cultivation of large chilies are attacks by plant pest. The effect of using high-frequency pesticides causes side effects (resistant pests). Control techniques that are environmentally friendly and friendly to production costs are needed, one of which is biological agents that are parasitic insect pests. The partner for this service activity itself is Andongsari Village and the farmer group appointed as the implementing committee by the village is the Margo Makmur I farmer group and accompanied by the Larasati Women Farmers Group. The implementation method adheres to the Plan Do Check Action (PDCA) principle including observation activities, program planning, training in the form of hands-on practice. The production training is divided into several small stages to facilitate monitoring and evaluation at each stage. The result of this activity is that there is an increase in farmers' understanding regarding the dangers of using pesticides and an increase in the interest of the farming community regarding the use of biological agents, besides that there is also an increase in soft skills in the form of community knowledge and skills in producing biological agents. Ruptur perineum dapat mengakibatkan dampak jangka panjang dan pendek pada ibu. Perdarahan adalah dampak jangka pendek dari ruptur perineum yang terjadi pada setiap persalinan melalui vagina. Secara historis, episiotomi diyakini mengurangi cedera perineum dan ruptur sfingter anal dengan mengendalikan arah dan tingkat kerusakan jaringan. Robekan dengan episiotomi dianggap lebih mudah untuk diperbaiki dibandingkan dengan robekan spontan perineum dan dianggap bahwa episiotomi bisa mencegah dari ruptur perineum. Evidence based case report ini bertujuan untuk mengetahui efektifitasa Tindakan episiotomy dalam mempercepat proses persalinan. Penulusuran artikel mengenai efektifitas Tindakan episiotomy dalam mempercepat proses persalinan dilakukan di Pubmed dan Google scholar. Didapatkan 1 artikel randomized-controlled trial oleh mario, dkk. yang sesuai dengan kriteria inklusi dan eksklusi dan telah dilakukan penapisan sebelumnya, kemudian ditelaah secara kritis. Setelah ditelaah secara kritis penelitian oleh mario, dkk. dinyatakan sahih, penting, dan dapat diterapkan pada pasien. Tindakan episiotomy secara selektif (tidak rutin dilakukan) dan sesuai procedure memberikan manfaat dan mempercepat proses persalinan. Selain itu juga didapatatkan 1 artikel literature review oleh mario, dkk. Dalam literature review ini didapatkan hasil Tindakan episitomy dilakukan apabila pasien memiliki risiko laserasi yang lebih tinggi, episiotomi dilakukan dengan sudut yang lebih terbuka (>40°) dan lebih awal pada periode ekspulsif. Dengan hal ini Teknik ini efektif mempercepat proses persalinan dengan meminimalisir risiko laserasi yang lebih tinggi .

The Basilica of San Marco is a singular case in the field of Cultural heritage, as it constitutes a construction site always active for the maintenance and preservation of the basilica itself. The continuous intervention of conservation, due to the particular environmental conditions of Venice and the opening to the public, together with the complexity of the building itself, imposes the need to identify an optimized management system. For this reason, in 2013 the Procurator of St. Marco Basilica commissioned the construction of a 3D model of the Basilica to be used for the creation of a BIM. The model must meet the required precision of the scale of 1:50, and should also include, in addition to the geometric description, a description of the mosaic and marble surfaces of the basilica through high resolution orthophoto which are essential for the restoration of the mosaics. <br><br> The complexity of the church and the large and continuous flow of public led to work in non-optimal conditions especially for the acquisition stage. The basilica has certain peculiarities that led to some important choices, for example the use of photogrammetry instead of laser scanning technique. The same technique was preferred also by the need to realize, in addition to the geometric model, high-resolution orthophotos of marbles and mosaic surfaces. <br><br> The modelling of the basilica has highlighted a number of problems related to the building features. The basilica, indeed, is the result of the juxtaposition of elements (capitals, columns) with different origin, which therefore cannot be standardized in special libraries. Moreover, especially in the extensive mosaic areas, there are not edges that characterize the architecture, but only beveled surfaces. This has resulted in a change also in the modeling paradigm with the need to identify alternative systems even for the construction of simple elements. Therefore, we decided to model using NURBS since it is the method that allows greater adherence to reality and, at the same time, a model with acceptable dimensions in terms of navigation and usability. Only the most sculptural pieces such as capitals and statues have been replaced by mesh models. <br><br> As we were interested in different elaborations, the results of this work are a geometric NURBS model for the subsequent insertion into BIM environment, with the possibility of extracting two-dimensional drawings such as plans and sections, a mesh model with low resolution textures for online navigation and high resolution orthophotos. <br><br> The survey work and modeling has been almost completed for the interior of the basilica whereas only the external remains to be acquired and modeled. For the management of the entire work it was decided to use the data organization system already used by the Politecnico di Milano for the Duomo di Milano. The BIM3DSURVEY system will be essential not only to manage the finished model, but it has been very useful also during the construction phase because it allows us to divide the entire work into more work units. However, this system required some changes to meet the peculiarity of this BIM, in particular the need to append and link the orthophotos to real surfaces, in order to use the model also as a spatial index for the images.

Questo lavoro analizza il decreto legislativo del 4 marzo 2014, n. 38 sulle cure transfrontaliere. Le norme del decreto sembrano garantire il diritto alla salute, perché stabiliscono che non esiste l’obbligo di ottenere una autorizzazione da parte dell’amministrazione, prima di poter espatriare al fine di ottenere le cure all’estero. Esse, però, al contempo, svantaggiano i meno abbienti, perché approntano un sistema di assistenza indiretta e precisano che gli amministrati possono ottenere il rimborso delle spese sostenute solo nei limiti dei costi che la prestazione avrebbe avuto se fosse stata eseguita nel territorio di provenienza. Inoltre, quando introducono la eccezionale necessità di una previa autorizzazione, attribuiscono alla p.A. un potere fortemente discrezionale. Infine, non chiariscono alcuni aspetti relativi ai procedimenti amministrativi da attivare per ottenere il rimborso e non regolano in modo adeguato i rapporti con il regolamento CE n. 883/2004. Esiste davvero un diritto alle cure oltre lo Stato? ---------- This work analyzes the Italian Legislative Decree of 4 March 2014, n. 38, about the cross-border healthcare. The provisions seem to guarantee the right to health, because they state that the patients have not to obtain an authorization from the administration, before being able to get treatment abroad. At the same time, they disadvantage the poor, because they establish a system of indirect assistance and they specify that individuals may obtain only the reimbursement of the costs that the service would have incurred if it had been performed in the territory of origin. In addition, when introducing the exceptional need for prior authorization, they confer to the public Administration a highly discretionary power. Finally, they aren’t clear when they regulate the aspects of the administrative procedures. Is there really a right to health care over the state?

EN The study investigates the use of codes of ethics in teaching English as a Foreign Language (EFL) to students enrolled in a first-level master’s course in Marketing Management in an Italian public university. According to Bhatia (2002), genre-specific documents such as codes of ethics have the potential to “sensitise and prepare students to meet the communicative demands of disciplinary communication” (p. 13). Within the theoretical framework of genre analysis and its possible implications for language teaching, this paper focuses on learners’ perceptions and achievements in EFL learning using corporate codes of ethics, presented to students both theoretically and on a practical level. A pre-questionnaire, pre- and post-test, and abridged post-treatment questionnaire were used to collect data. The goal of the five-week study was to demonstrate that genre pedagogy caters to learners’ needs, raises rhetorical and genre awareness, and fosters language awareness at a micro-textual, lexico-syntactical level. Se espera que los códigos revelen su potencial de “sensibilizar y preparar a los alumnos a fin de poder estar a las alturas de las exigencias comunicativas en comunicación disciplinaria” (Bhatia, 2002, p.13 [traducción nuestra]). Dentro del marco teórico del análisis de género y sus posibles implicaciones para la enseñanza de idiomas, este trabajo se enfoca en las percepciones y resultados de los estudiantes de inglés LE, a quienes se les han presentado códigos de ética empresarial desde un punto de vista tanto teórico como práctico. Para obtener los datos, se han utilizado una encuesta antes del tratamiento, una prueba tanto antes como después y una versión reducida de la encuesta post tratamiento. El objetivo de este estudio de cinco semanas es demostrar que la pedagogía de los géneros se vincula con las necesidades de los principiantes, porque, por un lado, solicita su conocimiento retórico y de género, y, por otro, favorece, a nivel microtextual, su conocimiento del léxico y de la sintaxis de la lengua extranjera. Per raccogliere i dati sono stati impiegati un questionario preliminare, un pre-test, un post-test e un questionario ridotto finale. Questo studio, durato cinque settimane, si è proposto di dimostrare che la pedagogia dei generi testuali va incontro ai bisogni dei discenti, perché, da una parte, sollecita la loro consapevolezza retorica e dei generi testuali, dall’altra favorisce, a livello microtestuale, la loro consapevolezza del lessico e della sintassi della lingua straniera. This article is the second in a three-part series on the theory and applications of a music care framework. Music is increasingly being recognized in health care as an effective psychosocial and rehabilitative intervention. Currently, there is little standardization as to how music may best be integrated into health care settings. It is the absence of standardization that prompted the authors to identify new possibilities for integrating music in health care. The purpose of this study was to explore how music could be optimized in complex rehabilitation and continuing care environments, using one such facility in Ontario, Canada, as an example. Data collection focused on the feasibility of incorporating music in care delivery by surveying stakeholders regarding the potential for music in the facility, and collecting specific ideas for the integration of music within the space. Participants’ perspectives were collected using 4 methods: design charrettes, a musical café, an electronic questionnaire and ‘idea boxes’. Data revealed participants’ perceived values and assumptions about the importance of music in care. The researchers utilized a conceptual framework of music care, which was designed to help clarify various dimensions of music in care, assist in the mapping of existing music care initiatives, and identify opportunities to optimize the use of music in care. Ad oggi c’é una leggera standardizzazione su come la musica puó essere meglio integrata per obbiettivi di cura individuale e in altri ambiti di cura. É proprio questa assenza di standardizzazione che ha portato gli autori a creare un quadro concettuale di cura con la musica, cosi i diversi scopi delle pratiche che integrano la musica possono essere distinti gli uni dagli altri, rendendo quindi possible l’ identificazione di nuove possibilitá per utilizzare la musica come cura.Mentre il primo studio ha esaminato l’ottimizzazione della cura con la musica in strutture di assistenza a lungo termine canadesi [1], lo scopo di questo studio é quello di esplorare come la musica possa essere ottimizzata in complessi ambienti di cura continua, utilizzando un impianto in Ontario, Canada, come sito di esplorazione.Questo studio ha esaminato la fattibilitá di incorporare la cura con la musica all’interno di questa struttura, sorvegliando come I membri della comunitá valutano il potenziale della musica nella struttura e su idée specifiche che potrebbero avere per ottimizzare l’uso della musica nello spazio. Music care is a developing approach to care that allows the therapeutic principles of music to inform caring practices in both formal healthcare settings and community or home-based contexts, and to create an integral role in developing more relational and person-centered cultures in caregiving. A significant part of the music care approach is a conceptual framework describing 10 domains of delivery. This article is the third in a three-part series on the theory and applications of a music care framework. Music is increasingly being recognized in health care communities as an effective psychosocial and rehabilitative intervention, increasing many aspects of quality of life. Currently, there is little standardization as to how music may best be integrated into individual care goals and care settings, though a growing body of literature supports the important impact of music in health care. It is this absence of standardization that has led the authors to develop a music care conceptual framework, so the varying scopes of practice that integrate music can be distinguished from one another and new possibilities for optimizing music in care can be identified. While the first study in this series examined how music care is understood in Canadian long term care facilities (1), the purpose of the second study explored how music could be optimized in complex continuing care environments, using one such facility in Ontario, Canada, as an exploration site (2). The 10 Domains of Music Care presented in this paper can be used as both a research tool and a practical, actionable tool for healthcare providers, managers, and decision makers. This paper is an attempt to learn more about the inflow of Serbian silver into the Venetian mint based on three account statements sent from Venice to the Kabuzic (Caboga) brothers in Ragusa (Dubrovnik) and on the sets of contemporary data published and discussed by Alan M. Stahl. A reference to the Venetian mint occurs in 1431 in a letter of Christophore Alberto, a citizen of Ragusa, but it is only the fourth page of an extensive account statement drawn by the Venetian Nicolo Grioni in 1435 that refers almost entirely to the mint?s mode of operation. It contains the name of ser Aluvisse, whose responsibility was to add an alloy to silver bullion, and of ser Rafael Barisson maser ala zecha, a mint master. An account statement presented by Marco di Stai to the Kabuzic brothers in 1435 mentions the name of the same mint master and refers to some steps in the operation of the mint but without specifying their sequence. Relying on the data contained in the accounting books of Guglielmo Condulmer, a Venetian merchant, Alan M. Stahl has singled out some fifteen or so names of the persons whose supplies of silver to the mint exceeded 70 kg each, notably Marin di Gradi (165 kg), a member of a well-known noble family of Ragusa. Apart from him, eight more Ragusans may be identified, four from the ranks of nobility and four from the citizen class, who supplied a total of 891 kg of silver to the mint. EN The study is based on a training project completed in Italy as part of a qualifying course for high school teachers of L2 English. The project moves around the framework that Rod Ellis proposed in 2010 on the relationship between Second Language Acquisition and Language Pedagogy and it argues the need for student teachers (STs) to take the role of classroom researchers during their learning process and carry out critical observations through a solid theoretical lens, such as the 10 principles for instructed language learning by Ellis (2005a, 2005b). Three case studies are analyzed to explore how STs observed classroom activities armed with the knowledge of the principles. Data is based on feedback they gave during seminar discussions and remarks made in written reports. Moreover, a follow-up survey of informants after five years’ in-service practice questions whether and how this experience influenced the quality and degree of their understanding of teaching and learning. Considering these results, suggestions are made for future implementations of this kind of project. Este estudio sostiene que los estudiantes necesitan recibir una lente teórica sólida para la observación crítica, como los 10 principios para el aprendizaje de idiomas instruido de Ellis (2005a, b), y asumir el papel de investigadores dentro del aula durante su proceso de aprendizaje. La análisis de tres estudios de caso explora cómo los estudiantes observaron las actividades del aula armados con el conocimiento de los principios. Los datos se basan en los comentarios que dieron durante los debates del seminario y en las observaciones realizadas en los informes escritos. Además, una encuesta de seguimiento de los informantes después de cinco años de docencia en servicio cuestiona si esta experiencia influyó, y de qué manera, en la calidad y el grado de su comprensión de la enseñanza y el aprendizaje. L’analisi esplora il processo di formazione dei tirocinanti in tre casi studio e le modalità in cui essi si sono mossi nell’osservazione delle lezioni di lingua, armati della conoscenza dei principi di Ellis. I dati includono le riflessioni dei tirocinanti durante le discussioni seminariali e nelle loro relazioni di fine percorso. Inoltre, dopo cinque anni d’insegnamento di ruolo, quegli stessi tirocinanti sono stati intervistati per ricevere informazioni su se e come questa esperienza abbia influenzato la qualità e il livello della loro consapevolezza dei processi di insegnamento e apprendimento. Sulla base dei risultati, vengono avanzate alcune proposte per lo sviluppo di questo tipo di progetti. O Desenvolvimento Infantil (DI) tem sido alvo de investimento em pesquisas e práticas voltadas ao cuidado na primeira infância. Atualmente o Brasil tem incorporado políticas públicas, leis e diretrizes que asseguram os direitos na infância, considerando a importância de práticas intersetoriais para o cuidado integral à criança com vistas à garantia do pleno desenvolvimento de seu potencial. Nesse contexto, observa-se uma lacuna no que se refere à atuação da Terapia Ocupacional na promoção da atenção integral ao DI, o que aponta para a necessidade de produzir conhecimentos que fundamentam tais práticas. O presente estudo tem por objetivo descrever os estudos que têm sido desenvolvidos pelo Grupo de Pesquisa “Intervenção da Terapia Ocupacional na atenção integral à criança”, vinculado ao Laboratório de Atividade e Desenvolvimento do Departamento de Terapia Ocupacional da Universidade Federal de São Carlos (LAD/DTO/UFSCar) e analisar as contribuições para a produção de conhecimento em Terapia Ocupacional. São descritas cinco pesquisas, sendo duas de doutorado e três de mestrado, desenvolvidas sob o referencial do Modelo de Construção de Capacidades Centrado na Família e do Desenvolvimento Ocupacional. Abstract Child Development has been the target of investment in researches and practices focused on early childhood care. Currently, Brazil has incorporated public policies, laws and guidelines that ensure the children's rights, considering the importance of intersectoral practices for the child's integral care, ensuring the full development of their potential. In this context, there is a gap regarding the performance of Occupational Therapy in the promotion of integral care for child development, which points to the need of producing knowledge that supports such practices. The present study aims to describe the studies that have been developed by the Research Group "Intervention of Occupational Therapy in the integral attention to the child", linked to the Laboratory of Activity and Development of the Department of Occupational Therapy of the Federal University of São Carlos (LAD/DTO/UFSCar) and analyze the contributions to the production of knowledge in Occupational Therapy. <p>This article aims to critically examine the entire methodology of very large scale (1:1) surveying and documentation of mosaic surfaces. The term ‘survey’ should be read in its broadest and most complete and sense, including the phases of measurement and data processing as well as management and use of these data for the purposes of preservation and maintenance. The case study presented here took place at St Mark’s Basilica (<em>Basilica di San Marco</em>), in Venice, where mosaic flooring, wall and vault decorations have been surveyed on two separate occasions. These two experiences shared a common goal (a full-scale survey of the mosaic decorations) but differed in terms of the methodologies used, chiefly due to the technological developments of recent years. All this, therefore, lends itself to a methodological reflection and critique of the ways in which surveying technology has evolved over time. It enables to conduct surveys that would, just a few years ago, have been inconceivable due to their size and complexity. This article describes in detail current surveying processes, which includes the use of a multi-scale “image-based” approach, “re-topology” methods such as non-uniform rational B-spline (NURBS) and a tailor-made Building Information Modeling (BIM) system. This system allows the direct use of a three-dimensional (3D) model of the Basilica within the maintenance process of the monument itself with the options to georeferencing information, extract basic metric data and catalogue all its mosaics.</p><p><strong>Highlights:</strong></p><ul><li><p>Modern digital photogrammetric techniques enable the acquisition of very complex objects, not only in terms of form but also in terms of material.</p></li><li><p>To obtain high resolution orthophotos, it is necessary to accurately take care of all the stages of the process: photographic acquisition, surveying, modelling and orthographic reprojection.</p></li><li><p>High resolution images and detailed 3D models can benefit from a complex BIM system for the management of all data.</p></li></ul>

Cardiovascular disease (CVD) is one of the leading causes of premature death in the world.1 Recent advances in medical technology have led to lower mortality and better relative recovery in CVD patients. Today, various invasive procedures are used in cardiovascular interventions such as percutaneous coronary interventions (PCIs), which have led to improvements in the outcomes of CVD patients. However, despite the benefits of cardiovascular interventions, patients need to change their lifestyles. Therefore, patients with CVD need rehabilitation after cardiovascular interventions for complete recovery, which is often overlooked.2 Meanwhile, patients' perception of recovery after cardiovascular interventions is one of the key factors in predicting lifestyle changes in CVD patients. These patients should have a full awareness of a healthy lifestyle about physical activity, alcohol and tobacco use, and diet, as well as a proper perception of recovery after cardiovascular interventions.3 Hence, the results of a randomized controlled trial in the UK that assessed the impact of educational and rehabilitation programs on patients 'perceptions after myocardial infarction (MI) showed that educational and rehabilitation programs can lead to positive changes in patients' perceptions of recovery after cardiovascular interventions and lifestyle after MI, especially after three months of intervention. Also, patients in the intervention group reported fewer symptoms of angina compared to patients in the control group. Overall, educational and rehabilitation programs were effective in changing the perception of CVD patients and improving the functional outcome of patients after MI.4 In contrast, the results of another study in the Netherlands that evaluated the effect of a lifestyle intervention on CVD risk indicators in patients with familial hypercholesterolemia showed that there was no significant improvement between the intervention and control groups (usual care).5 The importance of this issue becomes clearer when these patients undergo invasive cardiovascular interventions. Despite the benefits of cardiovascular interventions, patients need to change their lifestyles. On the other hand, there is a link between patients' perceptions of recovery after cardiovascular interventions and lifestyle changes. Therefore, there is a need for serious attention to patients' perception of recovery after cardiovascular interventions. Obviously, patients' perceptions of recovery after cardiovascular interventions can be an important predictor of their lifestyle changes. However, to our knowledge, studies in this field are very limited. Therefore, it is suggested that researchers in future studies pay special attention to the perception of CVD patients of recovery after cardiovascular interventions as an important predictor of lifestyle changes in them after cardiovascular interventions. Abstract Introduction. Breast-conserving surgery (BCS) followed by postoperative radiotherapy (RT) still represents the standard of care for early breast cancer (BC) patients. Hypofractionated schedules in maximum 15 fractions are currently accepted as the gold standard for external beam whole and partial breast irradiation (PBI). PBI for selected early BC patients allowed a shorter overall treatment duration and an improved patient compliance as compared to old-fashioned RT schedules. Preoperative PBI, due to the advantage of treating a well-defined volume, has been gaining attention in this multidisciplinary scenario. It avoids local treatment delay and might allow tumour downstaging with increased rates of BCS and improved cosmetic outcomes. Since local recurrence might be driven by biological mechanisms of radioresistance rather than geographical miss, higher dose per fraction may overcome repair mechanisms allowing tumoral cells to escape from conventional RT damage. We report the results of the phase II ROCK trial (NCT03520894), enrolling early BC patients treated with preoperative robotic radiosurgery (prRS), in terms of acute and early late toxicity, disease control, and cosmesis. Material and methods. The study recruited between August 2018 and September 2021 at the Radiation Oncology Unit of the University of Florence (Florence, Italy). Eligible patients were women aged 50+ years, with histologically proven invasive early BC, HR+/HER2- disease, without lymph vascular invasion, tumour size up to 25 mm suitable for BCS. Exclusion criteria were clinical node positive disease, multiple foci tumours, and patients with breast lesion limiting within 5 mm from the skin surface. The study aimed to prospectively assess the safety and feasibility of a single Cyberknife® (Accuray Incorporated, Sunnyvale, CA, USA) 21Gy-fraction prRS in preoperative setting, and to identify predictive factors for outcomes based on biologic and clinical features. The primary endpoint was the acute skin toxicity (from the end of prRS to surgery) according to the RTOG and the EORTC scales. Secondary endpoints were the rate of early late skin and non-skin toxicity as measured 90 days from the end of prRS, the rate of pathological complete response (pCR) according to Chevalier score. Cosmetic outcomes were prospectively scored every 6-month using the BCCT.core software. Results. From August 2018 to September 2021, a total of 70 patients were recruited and enrolled. Of those, 41 were excluded due to tumour biology exclusion criteria and 7 due to multiple foci breast disease evidenced at basal MRI. Therefore, 22 patients were successfully treated with pRS. Median age at diagnosis was 68 years (range 50-86), median tumour size was 14 mm (range 7.5-25). Required target dosimetric parameters were met in all patients, as well as normal tissue constraints. Patients received surgery after a median time of 29 days from biopsy, without any delay or postoperative complication. Overall, three G1 adverse events (13.6%) were recorded within 7 days from prRS (1 erythema, 2 breast pain). Three events (13.6%) were recorded between 7 and 30 days from prRS, one G2 breast oedema and two G1 breast pain. No acute toxicity greater than G2 was recorded. Five patients experienced early late G1 toxicity (1 breast pain, 4 breast induration). One patient reported G2 breast induration. No early late toxicity greater than G2 was observed. At a median follow up of 18 months (range 6-29.8), cosmetic results were scored excellent/good and fair in 14 and 5 patients, respectively, while 3 patients experienced a poor cosmetic outcome. Overall, pCR after surgery was reported in 2 patients (9%). Two patients received postoperative whole breast irradiation, according to histopathological results. Conclusions. ROCK trial showed that a single 21 Gy dose prRS represents a feasible technique for selected patients affected by early BC, showing a good safety profile and a promising effectiveness. Abstract Abstract 1921 Poster Board I-944 Background: Janus kinase 1 (JAK1) plays a critical role in lymphocyte proliferation and differentiation. Somatic JAK1 mutations are found in 18% of adult precursor T acute lymphoblastic leukemias (T-ALL). Some of the mutations were shown to induce the phosphorylation of JAK1 and STAT5 and lead to cytokine-independent proliferation. These data suggest that dysregulation of JAK1 can be involved in the development or progression of T-ALL (Flex et al. J Exp Med. 2008;205:751-758). Adult T-cell leukemia/lymphoma (ATLL) is a type of T-cell neoplasm, and the activation of JAK/STAT is sometimes observed in the tumor cells. Therefore, we investigated JAK1 mutations in ATLL patients. Patients and methods: Twenty Japanese ATLL patients whose percentage of peripheral abnormal lymphocytes was greater than 30% total cell count were sequentially enrolled into the study from 2000 to 2007. Diagnosis of ATLL was made on the basis of clinical features and laboratory characteristics. All cases tested positive for the serum anti-HTLV-1 antibody. The diagnosis was confirmed by observing monoclonal insertion of the HTLV-1 viral genome into leukemia cells by Southern blot hybridization. Peripheral blood mononuclear cells (PBMCs) were isolated and cryopreserved at -80°C. These PBMCs were thawed and genomic DNA was isolated using standard protocol. The entire coding sequence of the JAK1 gene (exons 2 through 25) was amplified by the polymerase chain reaction (PCR) method. The sequence of PCR primers were kindly provided by Dr. Marco Tartaglia (Istituto Superiore di Sanità, Roma, PhD). The nucleotide sequences were determined by fluorescent dye chemistry sequencing and analyzed by sequencing analysis software. By referencing the assembled sequence in the Ensembl genome database, the presence of homozygous mutations was first checked and then candidates for heterozygous mutations or single nucleotide polypeptides (SNPs) on each allele were screened by comparing the ratio of different bases calculated with the height of the peaks seen from sequencing to the reference genome when the ratio was between 0.15 and 1.0. Result: The percentage of abnormal lymphocytes ranged from 30-90%, and the mean value was 55.4%. The mean value of WBC and lymphocyte number was 40.5×109/L and 33.4×109/L, respectively. The mean value of LDH, Ca2+ or sIL-2R was 609 IU/L, 11.4 mg/dL, or 54748 U/mL, respectively. According to Shimoyama criteria (Shimoyama et al. Br J Haematol. 1991;79:428-437), 19 cases were diagnosed as acute-type ATLL, and one case was diagnosed as chronic-type ATLL. The surface markers of all but one abnormal PBMC were CD3+CD4+CD8-CD25+. In that one exception, loss of CD4 expression was observed. We examined the entire coding sequence of the JAK1 gene in 20 ATLL patients and identified no nonsynonymous or nonsense mutations and five types of silent substitutions in 12 cases. All silent substitutions were synonymous SNPs, as determined from referencing the base sequence in the Ensembl genome database. In the ATLL patients examined, the genotype frequency (%) is c546-AA/AG/GG, 97.5/2.5/0; c1590-CC/CT/TT, 97.5/2.5/0; c2049-CC/CT/TT, 50/50/0; c2097-CC/CG/GG, 95/5/0; c2199-AA/AG/GG, 60/40/0. There is no statistical difference in genotype frequency pattern of these SNPs, between the Japanese ATLL patients examined and the general Asian population on the Ensembl database. Conclusion: Mutations in the coding region of JAK1 do not associate with either activation of the JAK/STAT pathway or leukemogenesis in ATLL. We only examined the coding region of JAK1, and the regulatory region of JAK1 remains to be investigated. Further investigation including downstream signaling molecules and inhibitory molecules in the JAK/STAT signaling pathway is necessary to clarify the mechanism contributing to the leukemogenesis of ATLL. Abstract BACKGROUND: Young breast cancer (BC) survivors are at risk of infertility. Ovarian stimulation for fertility preservation before (neo)adjuvant chemotherapy is standard of care. Research efforts have shown no negative prognostic effect of pregnancy following BC therapy, also among BRCA carriers. Currently, poor evidence is available on the safety to undergo ART following BC treatment, with no data in carriers of germline BRCA pathogenic variants. To provide evidence on the safety of fertility treatments in this specific population, we assessed the outcomes of a cohort of BRCA-mutated BC survivors who had a pregnancy after prior BC history by comparing the group of patients who underwent ART to achieve pregnancy to the group with spontaneous pregnancy. METHODS: We conducted a multicenter retrospective cohort study across 30 centers worldwide including women diagnosed at ≤ 40 years with stage I-III BC, between January 2000 and December 2012, bearing germline BRCA1/2 pathogenic variants. Survivors with a pregnancy (any outcome) after BC, with no disease-free survival (DFS) event before pregnancy, were assigned to the ART and non-ART group if their pregnancy was achieved through ART or spontaneously, respectively. ART procedures included ovulation induction, ovarian stimulation for in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI), and embryo transfer under hormonal replacement therapy (HRT). RESULTS: Of 1,424 patients registered in the study, 168 with a pregnancy after BC were included in the present analysis. A total of 22 patients were included in the ART group and 146 in the non-ART group. Before BC diagnosis, 18.2% patients in the ART group had at least one child, compared to 38.4% in the non-ART group (P=0.030). Patients had a median age at BC diagnosis of 33.0 vs 30.2 years old in the ART group and in the non-ART group, respectively (P=0.004); 45.4% and 17.1% had grade 1-2 tumors, respectively (P=0.008), and 59.1% vs 31.5% had hormone receptor-positive tumors, respectively (P=0.016). Both cohorts had similar tumor size and nodal stage characteristics. Type and duration of endocrine therapy were comparable between groups. The type of ART was not specified in 5 survivors (22.7%). Ovulation induction was used in 1 patient (4.5%), ovarian stimulation in 7 patients (31.8%), embryo transfer under HRT following oocyte donation in 5 patients (22.7%), and embryo transfer under HRT following oocyte and/or embryo cryopreservation for fertility preservation in 4 patients (18.2%). Median age at conception among survivors was 39.7 years in the ART group versus 35.4 years in the non-ART group (P&lt;0.001). Overall, no differences in obstetrical outcomes were observed between groups, although there were more delivery complications in the ART group vs the non-ART group (22.1% vs 4.1%, respectively, P=0.011). Median follow-up from pregnancy was 3.4 years (range: 0.8-8.6) for patients in the ART group vs 5.0 years (range: 0.8-17.6) in the non-ART group (P=0.009). In the ART group, 2 patients (9.1%) experienced a DFS event (both were loco-regional recurrences) as compared to 40 patients (27.4%) in the non-ART group (P=0.182). No patients died in the ART group compared to 10 patients (6.9%) in the non-ART group. CONCLUSIONS: To our knowledge, this is the first study assessing the safety of ART in BC survivors bearing germline BRCA pathogenic variants. Even though the exposed cohort was small, results showed that the use of ART does not appear to increase the relapse risk at short-term follow-up. Further reproductive studies in BRCA-mutated BC patients are warranted. Abstract :The Covid-19 pandemic has slowed the pace of economic growth in Indonesia. As a result, several business sectors in the community experienced a decline, so the state issued a legal policy to take steps to resolve it. Legal politics as a form of government policy in an effort to restore the national economy is a necessity. This article uses a type of normative juridical research method, with a statutory and conceptual approach, legal materials (primary, secondary, tertiary) are analyzed using the content analysis method. This article shows and emphasizes the urgency of the government's legal politics in efforts to recover the national economy due to the Covid-19 pandemic. In the process of its formation, it certainly requires a comprehensive, integrated and targeted policy set. The 1945 Constitution Article 22 paragraphs (1-3) provides space for the government to stipulate Perpu No. 1 of 2020 becomes Law No. 2 of 2020 as a legal umbrella in an effort to maintain the resilience of all elements of the nation from all threats that endanger the safety of the people of the nation and state. The role of the government's legal policy in national economic recovery has a central position in anticipating a fragile "system" and management. The national economic recovery policy cannot be separated from the legal politics policy itself, of course by taking into account the principles of prudence, good faith and full integrity and remains based on the principles of good governance, accountability, and transparency.Keyword: Legal Politics, Economic Recovery, Covid-19, Economic Constitution.Abstrak :Pandemi Covid-19 memperlambat laju pertumbuhan ekonomi di Indonesia. Akibatnya beberapa sektor usaha bisnis di masyarakat mengalami penurunan, sehingga negara mengeluarkan kebijakan hukum untuk mengambil langkah penyelesaian. Politik hukum sebagai bentuk kebijakan pemerintah dalam upaya pemulihan ekonomi nasional menjadi keniscayaan. Artikel ini menggunakan jenis metode penelitian yuridis normatif, dengan pendekatan perundang-undangan dan konseptual, bahan hukum (primer, sekunder, tersier) dianalisis menggunakan metode content analysis. Artikel ini menunjukan dan menekankan urgensi politik hukum pemerintah dalam upaya pemulihan ekonomi nasional akibat pandemi Covid-19. Dalam proses pembentukannya tentu membutuhkan perangkat kebijakan yang komprehensif, terintegrasi dan tepat sasaran. Konstitusi UUD 1945 Pasal 22 ayat (1-3) memberikan ruang bagi pemerintah menetapkan Perpu No. 1 Tahun 2020 menjadi UU No. 2 Tahun 2020 sebagai payung hukum dalam upaya dan menjaga ketahanan seluruh elemen bangsa dari segala ancaman yang membahayakan keselamatan masyarakat bangsa dan negara. Peran kebijakan hukum pemerintah dalam pemulihan ekonomi nasional, memiliki kedudukan sentral guna mengantisipasi “sistem” dan manajeman yang rapuh. Apresentação A presente edição da revista Rónai – Revista de Estudos Clássicos e Tradutórios da Universidade Federal de Juiz de Fora reúne contribuições de diversas naturezas. Ao total publicamos onze artigos, seis traduções e uma resenha.No que diz respeito aos artigos, a área de Estudos Clássicos conta com cinco deles. O primeiro, de autoria de Bruna Fernanda Abreu, tem como título “Referências ao teatro nas Filípicas de Cícero e suas implicações para o ethos de Marco Antônio: a atriz de mimo Volumnia Cytheris”. Esse estudo trata da estratégia difamatória de Cícero, nas Filípicas, contra Marco Antônio. Ao analisar trechos dos quatorze discursos que compõem essa obra de Cícero, a autora buscou verificar como as referências à atriz de mimo Volumnia Cytheris aparecem nos discursos e de que modo colaboram para a construção da imagem de Marco Antônio como um personagem cômico ele próprio.Em “Retrospectiva do mito: o Minotauro de ontem e de hoje”, Amanda Naves Berchez e Wellington Ferreira Lima, apresentam, embasados em conceitos benjaminianos, uma reflexão pontual sobre as reescritas do mito do Minotauro em suas variantes antigas. Para observar essas reescritas em autores antigos, como Plutarco e Apolodoro, e em autores modernos, como Jorge L. Borges e Verônica Stigger, os autores buscaram refletir sobre os fenômenos da transmissibilidade e da citação e, ainda, acerca das novas leituras do mito do Minotauro e possíveis significações em novos contextos.Em seguida, apresentamos o artigo “O drama Epeu, de Eurípides”, de Wilson Alves Ribeiro Junior. Nesse texto faz-se uma avaliação dos elementos do mito de Epeu que Eurípides pode ter utilizado para o enredo do drama homônimo. Além disso, o autor apresenta uma tradução, acompanhada de breve comentário, da única possível fonte desse misterioso drama, perdido possivelmente antes de ser preservado na Biblioteca de Alexandria. E, por fim, discute-se a possibilidade de Epeu se tratar de um dos poucos dramas satíricos criados por Eurípides durante a segunda metade do século V a.C.O quarto artigo deste volume é o texto “Tradução do Texto Grego do Horário como Unidade Autônoma do Testamento de Adão”, de autoria de Milton Luiz Torres, Ana Maria Moura Schäffer e Deleon Ferreira de Lima.. Nele, os autores apresentam uma proposta de tradução do texto grego do Horário para o português, que serviu como base para discutir sua autoria. Além da tradução, comentou-se o gnosticismo de Nag Hammadi e fez-se uma comparação breve com a literatura apocalíptica judaica do século I A.D.Em seguida, apresentamos o texto de Matheus Trevizam, intitulado “Os desafios da tradução de textos ‘agronômicos’ latinos”. Neste trabalho, o autor discute alguns dos desafios envolvidos na tradução de textos agrários romanos, relacionados à dificuldade de transpor, para a língua de chegada, as especificidades da linguagem, léxico e estilo dos “agrônomos”. O modo de análise envolveu exame do léxico e do estilo em obras como o De agri cultura, de Catão, e o De re rustica, de Varrão, por vezes em cotejo com os mesmos aspectos encontrados nas Geórgicas.No que diz respeito à área de tradução, os artigos selecionados para o presente número refletem em ampla medida a diversidade das pesquisas e produções em termos de estudos e práticas tradutórias no Brasil. Revela-se aqui não só a fertilidade e amplitude do campo tradutório, mas também – e mais profundamente – a sua configuração enquanto espaço de interação e movimento. A tradução se mostra como um locus privilegiado de estabelecimento de contatos interculturais e transposições de fronteiras.Dentre esses contatos e transposições, alguns foram particularmente contemplados por nossos autores. Primeiramente, vemos sobretudo um profundo questionamento das interações entre teoria e prática. Não por acaso a obra de Henri Meschonnic foi contemplada de maneira mais detida em dois artigos que dialogam em profundidade com a ideia de uma poética do traduzir (enquanto série de princípios e enquanto tendências práticas). O texto de Mateus Roman Pamboukian, “Amanhãs entrelaçados: três traduções de Henri Meschonnic”, inscreve-se de maneira clara nessa encruzilhada, abordando diferentes aspectos da obra literária, tradutória e teórica do autor para propor, por sua vez uma discussão de práticas tradutórias próprias.De maneira similar, o artigo-ensaio de Maria Sílvia Cintra Martins, intitulado “A tradução como procedimento poético de repetição”, parte de uma pesquisa que entrelaça a perspectiva de Meschonnic com a tradução de Lavoura Arcaica, para construir uma perspectiva sui generis do ato de traduzir. Teoria produtora de expressividade e fértil, portanto: aberta a novas contribuições e desenvolvimentos, em constante contato com a poesia, a narrativa e o ensaio.Outra barreira transposta nos estudos aqui reunidos diz respeito às barreiras temporais. Nesse sentido três artigos se destacam. Primeiramente, a análise de Gabriel Contatori e Érico Nogueira (“A recepção e reelaboração de alguns preceitos das poéticas clássicas no Arte nuevo de hacer comedias en este tiempo, de Lope de Vega”) acerca da recepção/transposição/reelaboração de preceitos clássicos na obra de Lope de Vega. Tal perspectiva mostra que a ideia de trazer e deslocar, presentes no verbo traduzir, possuem diversas configurações e se aplicam a processos de recepção complexos.Ponto de vista similar, ainda que mais pontualmente ligado ao estudo de traduções no sentido restrito do termo, é apresentado em duas contribuições ligadas ao século XIX brasileiro. Num estudo acerca dos manuscritos da “A tradução em prosa de Dom Pedro II da tragédia Prometeu acorrentado de Ésquilo”, Ricardo dos Santos traz à tona um momento importante da história da tradução no Brasil, cujo interesse tem sido crescente.Já Juliana Aparecida Gimenes aborda, em seu texto “Olhos nos olhos: Capitu e Conceição, mulheres machadianas em tradução para o espanhol”, importantes temas como reescrita e reimaginação no que toca às personagens femininas machadianas e aos complexos procedimentos de enigma e insinuação que evocam.Por fim, cabe ressaltar a fronteira espacial que se dissolve em nossas duas últimas contribuições. João Paulo Ribeiro, além de propor um texto que entrelaça objetividade e subjetividade, relata, em “Tradução e Xamanismo: Em busca de Vidas Secas pelos caminhos de Ñapirikuli e as Amarunai”, um interessante percurso teórico-mitológico de tradução inspirado em sua tentativa de estabelecer laços entre Vidas Secas, de Graciliano Ramos, e o nheengatu. Seu texto evoca, em linguagem ritualística e ensaística, por vezes enigmática, os ecos do xamanismo no ato de tradução.Quanto às contribuições na seção de tradução, contamos, primeiramente, com “Pítica 1 de Píndaro”. Nesta submissão, os autores Ricardo Tieri de Brito e Christian Werner apresentam uma tradução para o português da Pítica 1 de Píndaro, acompanhada de uma pequena introdução e notas.Na sequência, em “A história de Egialeu (Efesíacas, V.1): tradução e comentário”, Adriane da Silva Duarte apresenta a tradução dessa narrativa intercalada em Efesíacas ou Ântia e Habrócomes, romance de Xenofonte de Éfeso (II. d.C.). A tradução da passagem é antecedida por uma breve exposição sobre autor e obra e sua discussão.Em meio às traduções de textos antigos, temos uma contribuição que versa sobre um texto em língua moderna sobre o ensino da língua latina. De autoria de Leni Ribeiro Leite e Ariane Ribeiro Santana, a tradução de “Latim ativo: falando, escrevendo, ouvindo a língua” apresenta o artigo, originalmente publicado em 2012, de Milena Minkova e Terence Tunberg. Ali os autores apresentam brevemente sua compreensão sobre a razão do uso do latim na prática pedagógica desta língua clássica não apenas de forma escrita e lida, mas também no que diz respeito à produção oral da língua.O texto a seguir, cujo título é “Proposta de tradução do Discurso em agradecimento ao povo romano, de Cícero”, é de autoria de Gilson Charles dos Santos. Nele, o autor apresenta uma tradução do Discurso em agradecimento ao povo romano, de Cícero e, ainda, breve introdução ao tema tratado pelo orador romano em tal texto.De autoria de Paulo Eduardo de Barros Veiga, o texto “O mito de Orfeu e Eurídice no Livro IV das Geórgicas de Virgílio: tradução e notas” traz uma tradução em prosa dos versos de número 453 a 527 do Livro IV das Geórgicas de Virgílio, em que o mito de Orfeu e Eurídice é narrrado.Livia Lakomi fecha a seção de tradução deste número com “Uma tradução de ‘Nada de não ficção: Pl2658.E8’, de Emily Goedde”, fazendo-nos transitar entre China, Estados Unidos e Brasil – ao mesmo tempo que aborda o gênero híbrido da não-ficção – numa contribuição acompanhada de comentário e entrevista com a autora do texto-fonte.Por fim, apresentamos a resenha, em língua inglesa, de Rodrigo Pinto de Brito ao livro Arcesilao di Pitane: l’origine del platonismo neoaccademico. Analisi e fonti. Philosophie hellénistique et romaine de Simone Vezzoli, lançado em 2016.O conjunto de textos desta edição alia, dessa forma, perspectiva acadêmica e ensaística, bem como práticas de tradução e de crítica. Resumo: A pesquisa propõe seguir a via simbólica do pensamento de Nicolau de Cusa (1401-1464) e pensá-la como um caminho para o ensino-aprendizagem da filosofia. Para tanto, toma como impulso a sugestão que o Cardeal de Cusa deixa na sua obra De coniecturis (Parte I, n. 4): “É oportuno atrair, em certo sentido guiando-os pela mão, os mais jovens, privados da luz da experiência, à manifestação do que se oculta, de tal maneira que possam elevar-se gradualmente ao que é mais ignorado”. A “condução” cusana será pela via dos símbolos e a pesquisa se concentrará, principalmente, sobre duas obras em que a “condução pela mão” aparece: o Livro I do De docta ignorantia e partes do De visione dei. Tanto na primeira quanto na segunda obra o “procedimento” para se conhecer o “mais ignorado” é basicamente o mesmo, embora os símbolos utilizados na “condução” sejam diferentes: a matemática na primeira obra e na segunda um quadro pintado ou o ícone de Deus. Inicialmente, a pesquisa entende que cabe ao professor-filósofo “atrair” e “guiar pela mão” os mais jovens para a filosofia. Depois, apresenta a perspectiva de Nicolau de Cusa sobre o “conduzir pela mão” confrontando-o com as críticas de Jacques Rancière, em O mestre ignorante, a “condução” socrática. Em seguida, busca explicitar o conceito de “douta ignorância” em Nicolau de Cusa para depois explicitar a “condução pela mão” em A douta ignorância e em A visão de Deus. Por fim, embora considere que talvez Nicolau não seja “o mestre ignorante” de Rancière, considera também que a partir da filosofia cusana, pode-se pensar um caminho para o ensino da filosofia que não seja mera transmissão, mas um exercício de aprendizagem por parte do aluno. Abstract: The enquiry proposes to follow the symbolic path of the thought of Nicholas of Cusa (1401-1464) and to think of it as a way for the teaching and learning of philosophy. For this purpose it takes impulse in the suggestion made by the Cardinal of Cusa in his work De coniecturis (Part I, n. 4): “It is useful to attract, in a sense leading them by hand, the younger, deprived from the light of experience, to the manifestation of what is hidden, in such a way that they can gradually rise toward what is most ignored”. The Cusanus' “lead” shall be through the path of the symbols, and the research will focus, mainly, on two works in which the “leading by hand” theme appears: Book I of De docta ignorantia and some parts of De visione dei. Both in the former and the latter the “procedure” to cognize the “most ignored” is basically tge same, although the symbols deployed in the “leading” are different: mathematics in the former and, in the latter, a painted picture or God's icon. Initially, the enquiry assumes the understanding that it is up to the teacher-philosopher to “attract” and “guide by the hand” the younger to philosophy. Later on, it presents Nicholas of Cusa's perspective on the “leading by hand” in confrontation with the criticisms issued by Jacques Rancière, in The Ignorant Schoolmaster, to the Socratic “lead”. Then, the concept of “learned ignorance” in Nicholas of Cusa is to be made explicit in order to make thenceforth explicit the doctrine of “leading by hand” in On Learned Ignorance and The Vision of God. Finally, although considering that perhaps Nicholas is not Rancière's “learned schoolmaster”, it is also comsidered that, departing from the philosophy of the Cusanus, a way of teaching philosophy can be thought that is not sheer transmission, but rather an exercise of learning by the student. King Matthias Corvinus of Hungary (1458‒1490), son of the “Scourge of the Turks,” John Hunyadi, was a foremost patron of early Renaissance art. He was only fourteen years old in 1470 when he was elected king, and his patronage naturally took some time and maturity to develop, notably through his relations with the Neapolitan Aragon dynasty. In December 1476, he married Beatrice, daughter of Ferdinand of Aragon, who brought to Buda a love of books and music she had inherited from her grandfather, Alphonse of Aragon.I studied the work of Beatrice’s brother John of Aragon (Giovanni d’Aragona), previously known mainly from Thomas Haffner’s monograph on his library (1997), from the viewpoint of his influence on Matthias’s art patronage. John was born in Naples on June 25, 1456, the third son of Ferdinand I of Aragon. His father, crowned king by Pope Pius II in 1458 following the death of Alphonse of Aragon, intended from the outset that he should pursue a church career. Ferdinand’s children, Alphonse (heir to the throne), Beatrice, and John were educated by outstanding humanist teachers, including Antonio Beccadelli (Il Panormita) and Pietro Ranzano. Through his father and the kingdom’s good relations with the papacy, John acquired many benefices, and when Pope Sixtus IV (1471‒1484) created him cardinal at the age of twenty-one, on December 10, 1477, he made a dazzling entrance to Rome. John was — together with Marco Barbo, Oliviero Carafa, and Francesco Gonzaga — one of the principal contemporary patrons of the College of Cardinals.On April 19, 1479, Sixtus IV appointed John legatus a latere, to support Matthias’s planned crusade against the Ottomans. On August 31, he departed Rome with two eminent humanists, Raffaele Maffei (also known as Volaterranus), encyclopedist and scriptor apostolicus of the Roman Curia, and Felice Feliciano, collector of ancient Roman inscriptions. John made stops in Ferrara, and Milan, and entered Buda — according to Matthias’s historian Antonio Bonfini — with great pomp. During his eight months in Hungary, he accompanied Matthias and Beatrice to Visegrád, Tata, and the Carthusian monastery of Lövöld and probably exerted a significant influence on the royal couple, particularly in the collecting of books. Matthias appointed his brother-in-law archbishop of Esztergom, the highest clerical office in Hungary, with an annual income of thirty thousand ducats.Leaving Hungary in July 1480, John returned to Rome via Venice and Florence, where, as reported by Ercole d’Este’s ambassador to Florence, Lorenzo de’ Medici showed him the most valuable works of art in his palace, and he visited San Marco and its library and the nearby Medici sculpture garden.In September 1483, Sixtus IV again appointed John legate, this time to Germany and Hungary. He took with him the Veronese physician Francesco Fontana and stayed in Buda and Esztergom between October 1483 and June 1484. The royal couple presented him with silver church vessels, a gold chalice, vestments, and a miter.John’s patronage focused on book collecting and building. He spent six thousand ducats annually on the former. Among his acquisitions were contemporary architectural treatises by Leon Battista Alberti and Filarete, which he borrowed for copying from Lorenzo’s library. They were also featured in Matthias Corvinus’s library, perhaps reflecting John’s influence. Around 1480, during his stay in Buda (approximately 1478‒1480), the excellent miniaturist, Francesco Rosselli made the first few large-format luxury codices for Matthias and Beatrice. Both Queen Beatrice and John of Aragon played a part of this by bringing with them the Aragon family’s love of books, and perhaps also a few codices. The Paduan illuminator Gaspare da Padova (active 1466‒1517), who introduced the all’antica style to Neapolitan book painting, was employed in Rome by John as well as by Francesco Gonzaga, and John’s example encouraged Matthias and Beatrice commission all’antica codices. He may also have influenced the choice of subject matter: John collected only ancient and late classical manuscripts up to 1483 and mainly theological and scholastic books thereafter; Matthias’s collection followed a similar course in which theological and scholastic works proliferated after 1485. Anthony Hobson has detected a link between Queen Beatrice’s Psalterium and the Livius codex copied for John of Aragon: both were bound by Felice Feliciano, who came to Hungary with the Cardinal. Feliciano’s probable involvement with the Erlangen Bible (in the final period of his work, probably in Buda) may therefore be an important outcome of the art-patronage connections between John and the king of Hungary.John further shared with Matthias a passion for building. He built palaces for himself in the monasteries of Montevergine and Montecassino, of which he was abbot, and made additions to the cathedral of Sant’Agata dei Goti and the villa La Conigliera in Naples. Antonio Bonfini, in his history of Hungary, highlights Matthias’s interest, which had a great impact on contemporaries; but only fragments of his monumental constructions survive.We see another link between John and Matthias in the famous goldsmith of Milan, Cristoforo Foppa (Caradosso, c. 1452‒1526/1527). Caradosso set up his workshop in John’s palace in Rome, where he began but — because of his patron’s death in autumn 1485 — was unable to finish a famous silver salt cellar that he later tried to sell. John may also have prompted Matthias to invite Caradosso to spend several months in Buda, where he made silver tableware.Further items in the metalware category are our patrons’ seal matrices. My research has uncovered two kinds of seal belonging to Giovanni d’Aragona. One, dating from 1473, is held in the archives of the Benedictine Abbey of Montecassino. It is a round seal with the arms of the House of Aragon at the centre. After being created cardinal in late 1477, he had two types of his seal. The first, simple contained only his coat of arm (MNL OL, DL 18166). The second elaborate seal matrix made in the early Renaissance style, of which seals survive in the Archivio Apostolico Vaticano (Fondo Veneto I 5752, 30 September 1479) and one or two documents in the Esztergom Primatial Archive (Cathedral Chapter Archive, Lad. 53., Fasc. 3., nr.16., 15 June 1484). At the centre of the mandorla-shaped field, sitting on a throne with balustered arm-rest and tympanum above, is the Virgin Mary (Madonna lactans type), with two supporting figures whose identification requires further research. The legend on the seal is fragmentary: (SIGILL?)VM ……….DON IOANNIS CARDINALIS (D’?) ARAGONIA; beneath it is the cardinal’s coat of arms in the form of a horse’s head (testa di cavallo) crowned with a hat. It may date from the time of Caradosso’s first presumed stay in Rome (1475‒1479), suggesting him as the maker of the matrix, a hypothesis for which as yet no further evidence is known to me. The seals of King Matthias have been thoroughly studied, and the form and use of each type have been almost fully established.John of Aragon was buried in Rome, in his titular church, in the Dominican Basilica of Santa Sabina. Johannes Burckard described the funeral procession from the palace to the Aventine in his Liber notarum. Matthias died in 1490 in his new residence, the Vienna Burg, and his body was taken in grand procession to Buda and subsequently to the basilica of Fehérvár, the traditional place of burial of Hungarian kings. Background:Knowledge of changes over time in baseline characteristics and tumor necrosis factor inhibitor (TNFi) response in bionaïve psoriatic arthritis (PsA) patients treated in routine care is limited.Objectives:To investigate secular trends in baseline characteristics and retention, remission and response rates in PsA patients initiating a first TNFi.Methods:Prospectively collected data on bionaïve PsA patients starting TNFi in routine care from 15 European countries were pooled. According to year of TNFi initiation, three groups were defined a priori based on bDMARD availability: Group A (1999–2008), Group B (2009–2014) and Group C (2015–2018).Retention rates (Kaplan-Meier), crude and LUNDEX adjusted1 remission (Disease Activity Score (DAS28) <2.6, 28-joint Disease Activity index for PsA (DAPSA28) ≤4, Clinical Disease Activity Index (CDAI) ≤2.8) and ACR50 response rates were assessed at 6, 12 and 24 months. No statistical comparisons were made.Results:A total of 17453 PsA patients were included (4069, 7551 and 5833 in groups A, B and C).Patients in group A were older and had longer disease duration compared to B and C. Retention rates at 6, 12 and 24 months were highest in group A (88%/77%/64%) but differed little between B (83%/69%/55%) and C (84%/70%/56%).Baseline disease activity was higher in group A than in B and C (DAS28: 4.6/4.3/4.0, DAPSA28: 29.9/25.7/24.0, CDAI: 21.8/20.0/18.6), and this persisted at 6 and 12 months. Crude and LUNDEX adjusted remission rates at 6 and 12 months tended to be lowest in group A, although crude/LUNDEX adjusted ACR50 response rates at all time points were highest in group A. Анотація:
Se estudió el impacto psicológico frene al COVID – 19 en los internos de enfermería de prácticas pre – profesionales en relación a las variables sociodemográficas y los niveles de ansiedad, depresión y miedo. Para el análisis se consideró a 150 internos prácticos de la carrera de enfermería, para la evaluación de este estudio se utilizaron las escalas de STAI, IDER, FCV19S y K10. Los resultados exponen que existe mayor nivel de ansiedad en los internos que declararon no tener equipos de bioseguridad necesarios. Los internos que rotan en el área hospitalaria presentan mayores niveles de ansiedad, miedo y malestar psicológico compara con los internos del área comunitaria. Sin embargo, los niveles de depresión fueron ligeramente mayores en los internos de área comunitaria. No hubo diferencia significativa de los estados emocionales entre mujeres y varones. The global pandemic of Covid-19 has caused enormous damage to scientific research by dramatically reducing the functionality of the institutions responsible for it and at the same time polarizing many lines of research toward solving the pandemic problems. This situation has similarities with what happened during the Second World War. In that case, the post-war recovery was intense, also bringing many changes in the methodology and motivations of scientific research. In this paper I analyzed, as a model of post-war rebirth, that of the Institute of Pharmacology of the University of Milan that became from a small laboratory a research and teaching institution based on human and scientific concepts that were innovative at the time. Its birth started in the years 1946-47, was favored by the collaboration between private and public institutions (the De Marchi Foundation, the University of Milan and the Municipality of Milan), was build by the scientists as a high-level, broad-based international study center, and formed the basis of a possible example of a university campus. The methodological and conceptual development of a new pharmacology and its growth in researchers and scientific purposes are outlined. The birth from this Institute of the Mario Negri Research Institute, of the School of Pharmacy of the University of Milan, of the Cellular and Molecular Pharmacology Center of the CNR and of the Lombardy Biomedical Library System is briefly described. The different reactions to a global damage brought to science and research in the post-war and post-pandemic periods are discussed.

L’autore parte da un breve resoconto del suo coinvolgimento nella questione, concentrandosi sugli incontri dei filosofi morali/teologi, medici, giuristi e infermieri che presiedette nel 1986 per discutere l’argomento in profondità, dopo la dichiarazione della Pontificia Accademia delle Scienze del 1985 che affermava che il trattamento delle persone in uno stato di incoscienza permanente non è obbligatorio ma che tutta l’assistenza, compresa la nutrizione, dovrebbe essere loro profusa. Questi incontri portarono l’autore stesso e alcuni altri a cambiare le loro precedenti opinioni, secondo cui non era obbligatoria la nutrizione, e a scrivere un documento, pubblicato nella rivista Issues in Law and Medicine nel 1987 e firmato da più di 90 studiosi, in cui vengono fornite le ragioni per cui tale nutrizione è moralmente necessaria in modo ordinario. Egli analizza quindi i punti di vista delle diverse fonti sull’argomento, compresi i vescovi cattolici e gli studiosi, dai primi anni ‘80 fino al pronunciamento di Giovanni Paolo II del 20 marzo 2004. In particolare egli presenta l’argomento assai influente presentato da Kevin O’Rourke, O.P., che afferma che un’adeguata lettura del discorso del 1957 di Papa Pio XII sosterrebbe l’opinione secondo cui tale somministrazione di cibo/acqua è straordinaria perché non permette al soggetto in stato di incoscienza permanente di perseguire il fine spirituale della vita. L’autore mostra che questa interpretazione di Pio XII è grossolanamente imprecisa. L’autore, quindi, riassume il discorso di Giovanni Paolo II del 20 marzo affermando che la nutrizione di tali persone è una questione di assistenza ordinaria ed è obbligatoria. Egli poi esamina l’accoglienza estremamente ostile che questo discorso ha avuto da parte di molti teologi, che hanno affermato che esso non è compatibile con l’insegnamento della tradizione Cattolica, o che impone gravi pesi non necessari a coloro che forniscono le cure, o che non era ben ragionato, ecc. L’autore conclude con la difesa del discorso di Giovanni Paolo II, rispondendo alle obiezioni sollevategli contro. ---------- The Author begins with a brief account of his own involvement in the question, focusing on the meeting of moral philosophers/theologians, doctors, lawyers and nurses he chaired in 1986 to discuss the issue in depth after the 1985 declaration by the Pontifical Academy of Sciences that treatment of permanently unconscious person is not required but that all care, including feeding, should be lavished on them. These meeting led him and some others to change their carlier views that considered such feeding not obligatory and to prepare a paper, published in Issues in Law and Medicine in 1987 and signed by more than 90 scholars, proving the reasons why such feeding is ordinarily morally required. He then reviews the views of different sources, including both Catholic bishops and scholars, over this issue from early 1980s until John Paul II’s statement of March 20, 2004. He in particular presents the highly influential argument advanced by Kevin O’Rourke, O.P., claiming that a proper reading of a 1957 address by Pope Pius XII support the view that such provision of food/hydration is extraordinary because it does not enable the permanently unconscious to pursue the spiritual goal of life and shows that this interpretation of Pius XII is grossly inaccurate. He then summarizes John Paul II’s March 20 address affirming that feeding such persons is ordinarily a matter of ordinary care and is obligatory. He next surveys the extremely hostile reception this address received from may theologians, who declared that it was not compatible with traditional Catholic teaching, that it imposed unnecessarily grave burdens on care givers, that it was not well reasoned etc. He concludes by defending John Paul II’s address and responding to objections leveled against it.

Per Cure Palliative si intende l’insieme degli interventi terapeutici, diagnostici e assistenziali rivolti sia alla persona malata, sia al suo nucleo familiare, finalizzati alla cura attiva e totale dei pazienti la cui malattia di base, caratterizzata da un’inarrestabile evoluzione e da una prognosi infausta, non risponde più a trattamenti specifici. La Legge n. 38 del 15 marzo 2010 tutela il diritto del cittadino ad accedere alle Cure Palliative e alla terapia del dolore nel nostro Paese. Come specificato nell’art.1 della Legge 38/2010 è tutelato e garantito, in particolare, il diritto all’accesso alle Cure Palliative e alla terapia del dolore da parte del malato, al fine di assicurare il rispetto della dignità e dell’autonomia della persona umana, il bisogno di salute, l’equità nell’accesso all’assistenza, la qualità delle cure e la loro appropriatezza. L’approccio delle Cure Palliative è finalizzato non solo al controllo dei sintomi, ma soprattutto alla difesa e al recupero della migliore qualità di vita possibile attuando interventi mirati a coinvolgere anche la sfera psicologica, sociale e spirituale. La natura stessa di tale disciplina impone che l’attenzione venga rivolta alla persona nella sua totalità più che alla sola malattia. In tale contesto si inserisce il bisogno di significato, inteso come ricerca del senso della propria vita, che è universale e che, nella condizione di malattia cronica evolutiva assume un’importanza peculiare, andando di pari passo con la ricerca del senso della sofferenza. La ricerca di significato nella maggior parte dei casi non viene tracciata nella comune pratica clinica. La Logoterapia, fondata da Viktor Frankl, è un orientamento psicoterapeutico volto ad aiutare l’uomo a ritrovare il senso della vita. Obiettivo della logoterapia e del logocounseling è quello di cercare di ridare una speranza di vita, un motivo di vita a chi a causa di un evento critico ha paura di aver perso la propria identità. Il nostro contributo si propone di esaminare alcuni aspetti della ricerca di significato, in particolare: la “neurochimica” del significato come aspetto della psico-neuro-endocrino-immunologia, le conversazioni di fine vita come strumento di accesso alla logoterapia, il sofferto percorso di ricerca di significato affrontato nel Libro di Giobbe. ---------- Palliative Care are all therapeutic interventions, diagnostic and care services, aimed at both the sick person and to the family, for the active and total treatment of patients whose underlying disease, characterized by a relentless progress and a poor prognosis, does not respond to specific treatments. The Law n. 38 of 15 March 2010 protects the right of Italian citizens to have access to Palliative Care and pain management. As specified in Article 1 of Law 38/2010 it is protected and guaranteed, in particular, access to Palliative Care and pain therapy for the patient, in order to ensure the respect of human dignity and the autonomy of the human person, the need for health, equity in access to care, the quality of care and their appropriateness. The approach of Palliative Care is aimed not only at controlling the symptoms, but also the defense and recovery of the best possible quality of life by implementing targeted interventions which involve also the psychological, social and spiritual dimension of life. The nature of this discipline requires that the attention must be paid to the person rather than the disease itself. In this contest, there is the need for meaning, understood as a search for meaning of life, which is universal and that, in the condition of rapid evolution illnesses assumes a special importance including the search for the meaning of suffering. Despite its importance, the search for meaning in most cases is not examined in common clinical practice. Logotherapy, founded by Viktor Frankl, is a psychotherapeutic approach aimed at helping man to rediscover the meaning of life. The aim of logotherapy and logocounseling is to try to restore hope of life to those who are afraid of losing their identity because of a critical event. The purpose of our contribution is to examine some aspects of the search for meaning, in particular: the “neurochemistry” of meaning as an aspect of psycho-neuro-endocrino-immunology, the end of life conversations as means of access to logotherapy, the suffered search for meaning addressed in the Book of Job.

Background: The use of cytokine-blocking agents has been proposed to modulate the inflammatory response in patients with COVID-19. Tocilizumab and anakinra were included in the local protocol as an optional treatment in critically ill patients with acute respiratory distress syndrome (ARDS) by SARS-CoV-2 infection. This cohort study evaluated the effects of therapy with cytokine blocking agents on in-hospital mortality in COVID-19 patients requiring mechanical ventilation and admitted to intensive care unit.Methods: The association between therapy with tocilizumab or anakinra and in-hospital mortality was assessed in consecutive adult COVID-19 patients admitted to our ICU with moderate to severe ARDS. The association was evaluated by comparing patients who received to those who did not receive tocilizumab or anakinra and by using different multivariable Cox models adjusted for variables related to poor outcome, for the propensity to be treated with tocilizumab or anakinra and after patient matching.Results: Sixty-six patients who received immunotherapy (49 tocilizumab, 17 anakinra) and 28 patients who did not receive immunotherapy were included. The in-hospital crude mortality was 30,3% in treated patients and 50% in nontreated (OR 0.77, 95% CI 0.56-1.05, p=0.069). The adjusted Cox model showed an association between therapy with immunotherapy and in-hospital mortality (HR 0.40, 95% CI 0.19-0.83, p=0.015). This article is simultaneously a call for, a study of, and a contribution to the preservation of scientific heritage. Focusing on the heritage of a disciplinary niche like that of limnology, we take the specific heritage of the Istituto Italiano di Idrobiologia ‘Dr. Marco De Marchi’ (Istituto) as a case study to illustrate the present challenges and possibilities for the conservation and study of limnological scientific heritage. In particular, we highlight two different forms that this heritage takes at the Istituto: its ongoing scientific publication – the current Journal of Limnology, and its collection of scientific instruments – the Crypta Baldi. Showing both the limitations and potential of these collections, we offer insights into the history of limnology through the lens of the Istituto. Furthermore, we bring these insights to bear on broader reflections about the importance of preserving the scientific heritage that shaped and continues to shape our world, and holds the promise of helping us not to lose sight of the past as well as the future of a scientific field like limnology.

Il contributo esamina il “Caso n. 12.361 Gretel Artavia Urilla et Al. vs. Costa Rica” sul quale si attende una pronuncia della Corte interamericana dei diritti umani. La vicenda ha origine dalla sentenza della Corte costituzionale costaricana (del 15 marzo 2000 n. 2000-02306) che aveva annullato, per ragioni di forma e di sostanza, il Decreto Ejecutivo n. 24029-S1 (del 3 febbraio 1995) sulla procreazione artificiale umana. La vicenda prosegue davanti alla Commissione interamericana chiamata in causa da una “Petición” che accusa la Repubblica del Costa Rica di aver violato i diritti di alcune coppie in attesa di realizzare il loro “progetto parentale”. Il divieto di fecondazione artificiale confliggerebbe, in sintesi, con il diritto alla privacy e alla vita familiare, con il diritto di fondare una famiglia con il principio di uguaglianza contenuti nella Convenzione americana dei diritti umani (“Patto di San Josè”). Al termine di un lungo percorso e di un ampio dibattito, la Commissione ha ritenuto che tali diritti fossero stati violati e ha rimesso il caso alla Corte interamericana dei diritti dell’uomo. Con riferimento a questa nuova fase, nell’articolo si dà conto del “Escrito de Amici Curiae” presentato alla Corte dal Movimento per la vita italiano, dall’Istituto di Bioetica, dall’Asociación Crece Familia-CreceFam, dal Coordinamento di Human Life International e da Bioethics Defend Found. Nell’“Escrito” si afferma che il divieto del Costa Rica non viola la Convenzione americana sui diritti umani che afferma: “Ogni persona ha diritto al rispetto della propria vita. Tale diritto è protetto dalla legge e, in generale, è tutelato a partire dal momento del concepimento. Nessuno può essere privato arbitrariamente della vita (art. 4/1). Nel parere, inoltre, si avanzano argomenti di ordine scientifico e giuridico a sostegno del divieto di procreazione artificiale, in nome del riconoscimento della dignità umana e del conseguente diritto alla vita dell’essere umano nella fase più giovane della sua esistenza. Questo diritto, primo fra tutti, è già ampiamente accolto nella Convenzione americana sui diritti dell’uomo sottoscritta e ratificata dalla Repubblica del Costa Rica. ---------- The article deals with the “Case n. 12.361 Gretel Artavia Urilla et Al. vs. Costa Rica” which the Inter-American Court of Human Rights is going to decide. This case has its roots in the Supreme Court of Costa Rica’s decision (n. 2000-02306, March 15, 2000) which annulled the Decree n. 24029-S1 (February, 3, 1995) on human artificial procreation because of both formal and substantial aspects. Indeed, the Supreme Court of the Costa Rica considered that in vitro fertilization constituted a threat against human life before birth. Afterwards, the Inter-American Commission on Human Rights received a “Petición” which charged Costa Rica with a violation of the rights of some couples who wanted to achieve parenthood by medically assisted procreation. In short, according to Petitioner, the ban on in vitro fertilization violated the right to privacy and family life, the right to raise a family and equality before the law and equal protection established in the American Convention on human rights (“Pact of Saint José”). At the end of a long iter and an extended debate, the Inter-American Commission on Human Rights submitted the case to the jurisdiction of the Inter-American Court of Human Rights claiming the violation of said rights and asking the Court to rule and declare the international responsability of the Costa Rican Republic. Regarding this new stage, the article relates the “Escrito de Amici Curiae” sent to the Inter-American Commission on Human Rights by the Italian Movement for the Life, the Institute of Bioethics of teh Catholic University of the Sacred Heart, Asociación Crece Familia-CreceFam, Human Life International and the Bioethics Defense Fund. This “Escrito” argues that Costa Rica’s ban does not violate the American Convention on Human Rights which says that “Every person has the right to have his life respected. This right shall be protected by law and, in general, from the moment of conception. No one shall be arbitrarily deprived of his life” (article 4/1). Besides the “Escrito” presents scientific and legal arguments corroborating the ban on artificial human procreation in the light of modern idea of human rights, recognition of human dignity and the right to life of human beings in the youngest stages of their lives. This right, the primary or first right, is already widely recognized in the American Convention on Human Rights, signed and ratified by Costa Rica’s Republic.

ObjectiveOur knowledge on the long-term consequences of COVID-19 is still scarce despite the clinical relevance of persisting syndrome. The aim of this study was to analyze patient-reported outcomes, including assessment by specific questionnaires of health impairment and symptoms.MethodsThis is a prospective, observational and multicenter cohort study coordinated by Fondazione IRCSS Ca' Granda Ospedale Maggiore Policlinico di Milano and Istituto di Ricerche Farmacologiche Mario Negri IRCCS including eight hospitals located in North and Central Italy. A telephone interview to assess rehospitalization, access to health care resources, general health status subjective evaluation, and symptoms was performed at 12 months after the discharge in patients admitted to hospital because of COVID-19 from February 2020 to the end of May 2020.ResultsAmong the 776 patients discharged alive, 44 (5.7%) died, 456 subjects (58.8%) completed the questionnaire and 276 (35.6%) were not reachable or refused to join the telephone interview. The mean age of the study population was 59.4 years (SD 14.1), 69.8% of individuals needed oxygen support during hospitalization and 10.4% were admitted to ICU. Overall, 91.7% of participants reported at least one symptom/sequela at 12 months. Exertional dyspnea (71.7%), fatigue (54.6%), and gastrointestinal symptoms (32.8%) were the most reported ones. Health issues after discharge including hospitalization or access to emergency room were described by 19.4% of subjects. Female and presence of comorbidities were independent predictors of whealth impairment and presence of ≥2 symptoms/sequelae after 12 months from hospitalization for COVID-19.ConclusionsPatient-reported symptoms and sequelae, principally dyspnea and fatigue, are found in most individuals even 12 months from COVID-19 hospitalization. Long-term follow-up based on patient-centered outcome can contribute to plan tailored interventions.
