A corollary of the high proportion of people who inject drugs cycling through prisons worldwide is the accompanying high prevalence of viral hepatitis, particularly hepatitis C (HCV). Prisons have the potential to either escalate or interrupt the transmission of viral hepatitis: prisons that do not provide access to even basic prevention programs are high-risk environments for viral hepatitis transmission. In contrast, prisons can also reduce prevalence through testing and treatment programs. This chapter provides an overview of the global prevalence and incidence of HCV and hepatitis B (HBV) in prisons and the occurrence of known transmission vectors, including injecting drug use, tattooing and other body modification practices, and unprotected sexual intercourse. It discusses policy and practice options to reduce the risk behaviors associated with HCV and HBV transmission, prevent transmission through vaccination, and potentially expand viral hepatitis treatment in prisons using new directly acting anti-viral medication, and the prospect of treatment as prevention.

The most common method that US state governments use to restrict access to exemptions from school and daycare vaccine mandates is to provide exemptions only to people who object to vaccination for religious reasons. However, there are moral reasons for states not to prioritize vaccine exemptions for religious objectors. They should also offer personal belief exemptions. Furthermore, states should not try to restrict exemptions by focusing on the reasons (religious or otherwise) that people give for objecting to vaccines. It would be better to focus on the intensity of an objector’s commitment to refuse vaccines, which we can assess by making vaccination exemption application processes more burdensome to complete.

Stridor is a high-pitched sound representing partially obstructed or turbulent flow in the airway. Stridor can be part of the inspiratory or expiratory phase, depending on the origin of the flow disturbance. This chapter reviews the etiology of stridor and its subtypes. It also focuses on two important pediatric disease states where stridor is a defining characteristic: epiglottitis and croup. Epiglottitis has emerged as an uncommon cause of pediatric airway compromise, with the widespread use of Haemophilus influenza type b vaccination. Croup continues to be a significant cause of stridor, and is much more common, although less life-threatening. Current opinions on initial evaluation, anesthetic induction, intubation, and follow-up care for both diseases are discussed.

Contributors examine the basics of pseudoscience, including issues of cognitive bias; the costs of pseudoscience, with accounts of naturopathy and logical fallacies in the anti-vaccination movement; perceptions of scientific soundness; the mainstream presence of “integrative medicine,” hypnosis, and parapsychology; and the use of case studies and new media in science advocacy.

Vaccine- and vaccination-related crises require a communication response that is different from the communication strategies used to promote the benefits and importance of vaccines in general. This document presents the technical guidance needed to develop a communication plan that is appropriate for managing crises related to vaccine safety. This guidance will be useful for managers in the areas of immunization and vaccine and vaccination safety. They will also help preparedness and response teams working in safety crises to optimize their communication plans in order to regain, maintain, or strengthen trust in vaccines, vaccination, and immunization programs in general. Each chapter presents a phase (preparation, implementation, and evaluation) with suggested actions and support tools to prepare, implement, and evaluate a communication response in a crisis situation. Also, some sections can also be used to strengthen routine national communication activities such as interaction with media, message generation, spokespeople preparation among others. The current document complements the Manual for the surveillance of events supposedly attributable to vaccination or immunization (ESAVI) in the Region of the Americas. This document is published within the framework of a joint project that aims to promote communication-related to safe vaccination in the Region of the Americas and support health authorities that need to develop a communication plan to manage crises related to vaccine safety. Some of the sections in this publication are based on the guidance documents available in the WHO Regional Office for Europe’s virtual library and can be consulted on their website.

Les vaccins sauvent entre 2 et 3 millions de vies chaque année et protègent l’ensemble de la population contre plus d’une douzaine de maladies potentiellement mortelles. Grâce à la vaccination, la variole a été éradiquée en 1980 et nous sommes sur la bonne voie pour éradiquer la polio. Cependant, malgré de grands progrès dans la lutte contre la rougeole, l’une des maladies les plus contagieuses connues, une augmentation des cas a été malheureusement observée ces dernières années. Raison pour laquelle une couverture vaccinale élevée (95 % ou plus) est nécessaire en dépit du défi technique et de communication majeur que cette couverture pose au personnel de santé. Des études montrent que le fait d’informer les personnes sur la qualité, la sécurité, l’efficacité et la disponibilité des vaccins ne suffit pas à modifier les comportements en matière de vaccination et, en général, n’augmente pas la couverture vaccinale. Pour cette raison, nous devons comprendre les raisons pour lesquelles les personnes choisissent de ne pas se faire vacciner ou de ne pas faire vacciner leurs enfants, afin d’entamer un dialogue respectueux à l’aide de messages le mieux adaptés et les plus efficaces. Dans ce contexte, le principal objectif de ces lignes directrices est de fournir des outils au personnel qui travaille dans le domaine de la vaccination afin de favoriser une communication efficace entre le personnel de santé et la population, de manière à renforcer, maintenir ou rétablir la confiance dans les vaccins et les programmes de vaccination dans la Région des Amériques.

SUD remains a significant public health problem with limited evidence-based treatment options available for many individuals. This chapter provides a review of integrative treatment approaches for stimulant use disorder, including current literature on evidence-based behavioral interventions, pharmacotherapy, and complementary and alternative treatments (CAM) for both cocaine and amphetamine use. Current epidemiology, physical and mental health effects, and public health impacts, of these disorders is also discussed. The chapter provides an overview of current mainstream treatments for these disorders and challenges that have been faced in developing effective treatment modalities. With the relative paucity of available traditional treatments, the chapter seeks to explore the evidence for the use of such CAM treatments as acupuncture, exercise, yoga, transmagnetic cranial stimulation (TMS), meditation, and N-acetylcysteine (NAC).

The larval stage of the parasite Taenia solium can encyst in the central nervous system causing neurocysticercosis, which is the main cause of acquired epilepsy in the countries in which the parasite is endemic. Endemic areas are those with the presence (or likely presence) of the full life cycle of Taenia solium. The parasite is most prevalent in poor and vulnerable communities in which pigs roam free, open defecation is practiced, basic sanitation is deficient, and health education is absent or limited. Several tools are available for the control of Taenia solium. Preventive chemotherapy for Taenia solium taeniasis, which is directed at the adult tapeworm, is one of them. Other tools focus on pig management, pig vaccination and treatment, sanitation and hygiene, and community education. Three potential drugs—niclosamide, praziquantel, and albendazole—have been considered for use for preventive chemotherapy in Taenia solium taeniasis control programs through mass drug administration or targeted chemotherapy. In this Guideline, we provide recommendations for preventive chemotherapy in Taenia solium-endemic areas using niclosamide, praziquantel, or albendazole, including at which dose and in which population groups. The development of this Guideline is based on the latest standard World Health Organization methods for guideline development, including the use of systematic search strategies, synthesis, quality assessment of the available evidence to support the recommendations, and participation of experts and stakeholders in the Guideline Development Group and External Review Group. The recommendations are intended for a wide audience, including policymakers and their expert advisers, and technical and program staff at governmental institutions and organizations involved in the planning, implementation, monitoring, and evaluation of preventive chemotherapy programs for the control of Taenia solium.

Salmonellosis remains the second most common form of bacterial food-poisoning in the UK and in most of the developed economies. Although the number of isolations per annum has declined since 2000, over 10,000 laboratory-confirmed cases are recognised each year in England and Wales, and over 150,000 in Europe. Most of infections are associated with contaminated food, particularly of poultry origin, but also may originate from cattle and pigs, and to a lesser extent, sheep. The most common serovars from cases of human infection is Enteritidis, followed by Typhimurium. Contact with pets, particularly reptiles and amphibians is becoming an increasing problem and infections can be severe, particularly in children. Accurate and reproducible methods of identification and subtyping are crucial for meaningful epidemiological investigations, and traditional phenotypic methods of typing are now being supplemented by DNA- based methods such as pulsed-field gel electrophoresis, variable number of tandem repeats analysis, and multilocus sequence typing. The use of such methods in combination with phenotypic methods has been invaluable for outbreak control at the international level. The occurrence of resistance to antimicrobial drugs is an increasing problem, particularly in relation to the development of resistance to antimicrobials regarded as ‘critically-important’ for last resort therapy in humans. Control measures such as vaccination of poultry flocks appear to have had a substantial impact on the number of infections with Salmonella Enteritidis. Nevertheless good hygiene practices in both catering establishments and the home remain essential for the control of infections at the local level.
