Анотація:
When field-dug, balled-and-burlapped trees are planted to the landscape, several options exist relative to wire basket modification or alteration. New wire basket alteration research has been initiated due to considerable controversy that surrounds some of the alteration options, and to limited prior research specifically addressing this issue. To guide this research, and to determine what currently occurs in the tree care industry, surveys of manufacturers of wire baskets, and of landscape contractors and arborists who plant trees, were conducted. Summaries of those surveys are presented.

Анотація:
Tujuan dari penelitian ini adalah untuk pengembangan latihan imagery untuk meningkatkan lay up shoot bola basket di klub bola basket Hutama Manggala Kota Malang. Pengembangan ini menggunakan metode Research and Development (R&D) model Borg & Gall menggunakan 9 langkah. Produk divalidasi oleh 3 ahli, dengan: (1) ahli psikologi, (2) ahli bola basket, dan (3) ahli media. Penilaian validator menunjukkan hasil dari ahli media sebesar 75% (cukup valid), ahli psikologi sebesar 89,29% (sangat valid), ahli bola basket sebesar 64,29% (cukup valid). Produk media diuji coba terhadap kelompok kecil sebanyak 1 pelatih dan 4 atlet dipilih dengan acak berdasarkan kemampuan. Dalam uji coba kelompok besar sebanyak 1 pelatih dan 14 atlet. Didapatkan hasil dari uji kecil 98% (sangat valid) serta uji kelompok besar 97% (sangat valid). Berdasarkan hasil uji kelompok kecil dan besar produk video dinyatakan layak untuk digunakan.

Анотація:
In basketball, it is crucial to understand the optimal shooting pattern for each individual in terms of gender and shooting distances from the basket. However, incorporating biomechanics shooting analysis quantitatively into the learning process is not preferable. Hence, the purpose of this study is to determine the quantitative aspect of basketball shooting for biomechanics analysis using the OpenPose motion capture system. This marker-less motion capture system generated the biomechanical parameter data subjected to a significant difference test for shooting performance. A total of four players – two males and two females, divided into two levels (intermediate and novice) performed ten continuous jump shots from two different shooting distances (4.57 m and 6.40 m) from the basket. Each individual’s biomechanical parameters were tested in a statistical or independent t test to determine which significant parameter has a measurable difference in shooting performance. When the effects of different body angles on missed to scored baskets were compared, only the right elbow angle (intermediate female player), the right shoulder angle (novice male player) and the left hip angle (intermediate female player and novice male player) showed a significant difference (p < 0.05) towards shooting performance. In the comparison of the velocity of right upper limb key points, only the velocity of the right shoulder (intermediate male player) showed a significant difference (p < 0.05) toward shooting performance. In conclusion, the findings of this study suggest that shooting analysis should be done quantitatively to demonstrate a more profound and clear understanding of biomechanics when considering an improvement in shooting performance.

Safflower is a promising drought-resistant crop, its cultivation in the Southern Steppe of Ukraine as an insurance crop is an effective use of natural climatic potential in unstable weather conditions of the summer period. Due to its biological characteristics, the plant is able to withstand many adverse natural factors. Recently, crops are expanding, but oilseed yields are still low. The reason for this is the imperfection of the technology of growing safflower, taking into account the specific soil and climatic conditions of the area. The aim of our research was to establish the peculiarities of the formation of the productivity of safflower varieties depending on the methods of care for crops on herbicide-free and herbicide-based backgrounds. The research was conducted in 2017-2019 in the fields of the Institute of Oilseeds of NAAS. Sowing of safflower varieties Zhyvchyk and Dobrynya was carried out in the first decade of April with a sowing rate of 240,000 similar seeds per hectare. The system of basic tillage: classical. Options for the use of herbicides: 1. Control – without herbicides; 2. Application of Harness herbicide (2.0 l/ha) under pre-sowing cultivation. Options for care systems: 1. Control – without care; 2. Harrowing before and after germination; 3. Two inter-row tillage; 4. Post-emergence harrowing and inter-row tillage. Bookmarking of experiments and conducting research was carried out in accordance with generally accepted methods of field experiments in agriculture and crop production. According to the results of our research, the influence of agricultural cultivation techniques on the growth, development and productivity of safflower varieties Zhyvchyk and Dobrynya was established. The lowest values of the number of baskets for both varieties on herbicide and non-herbicide backgrounds were obtained in the absence of agricultural practices for the care of crops: in the variety Zhyvchyk 9.4 and 9.7 pcs., In the variety Dobrynya 8.9 and 9.1 pcs. The application of herbicides did not have a significant effect on the number of baskets on the plant, but the number of baskets increased during agricultural practices. According to studies, the number of seeds in the basket was affected by both the use of herbicides and crop care. Thus, the lowest values were obtained for both varieties on a herbicide-free background and without care (control) and were equal to 15.0 for the variety Zhyvchyk , 14.3 for the variety Dobrynya. The application of the herbicide led to an increase in the number of baskets to 16.5 pcs. in the variety Zhyvchyk and 15.7 pcs. in the Dobrynya variety. The use of agricultural methods of care contributed to an increase in the number of seeds in the basket on both herbicide and herbicide-free backgrounds. The weight of seeds from the basket varied depending on the variety, application of herbicides and agricultural practices for plant care. Its indicators in the variety Zhyvchyk on a herbicide-free background were 0.51-0.61 g, and on a herbicide 0.60-0.68 g. The application of the herbicide led to an increase in the weight of seeds in the basket by 0.4-0.9 g. Weight seeds from a basket in the variety Dobrynya on a herbicide-free background was equal to 0.60-0.69 g, and on a herbicide 0.70-0.80 g. When applying the herbicide, the weight of seeds increased by 0.10-0.11 g. Over the years of research it was found that the weight of 1000 seeds was significantly influenced by all three factors: variety, herbicide application and crop care. Depending on the variety, its values were 34.8-38.4 g for the Zhyvchyk variety and 43.3-47.2 g for the Dobrynya variety. Against the background of herbicide application, the weight of 1000 seeds increased by 2.2 g in the Zhyvchyk variety and 2.5 g in the variety Dobrynya in the absence of techniques for crop care (control). Under the influence of agricultural practices, the weight of 1,000 seeds increased. On a herbicide-free background, the lowest values were obtained during two inter-row treatments and were equal to 35.7 g in the variety Zhyvchyk and 44.5 g in the variety Dobrynya, during pre- and post-emergence harrowing – 36.7 and 45.1 g, and during post-emergence harrowing and inter-row tillage – 37.2 and 45.3 g, respectively. The weight of 1000 seeds on the herbicidal background was higher than on the non-herbicide, both for the variety Zhyvchyk and for the variety Dobrynya: during two inter-row treatments the weight of 1000 seeds was 37.2 in the variety Zhyvchyk and 46.3 g in the variety Dobrynya, at carrying out pre- and post-ladder harrowing – 37.9 and 47.2 g, and during post-ladder harrowing and inter-row tillage - 38.4 and 47.2 g, respectively. Depending on the background of herbicide application and crop care option, on average for three years of research the yield level of safflower variety Zhyvchyk was in the range of 1.08-1.54 t/ha and Dobrynya variety 1.16-1.64 t/ha. Growing safflower on a herbicidal background provided an increase in yield compared to the herbicide-free background, depending on the option of crop care: for the variety Zhyvchyk 0.13-0.19 t/ha and for the variety Dobrynya 0.17-0.24 t/ha. As a result of crop care methods, an increase in the level of safflower yield of both Zhyvchyk and Dobrynya varieties was observed on both backgrounds of herbicide application. The increase in yield was: for the variety Zhyvchyk on a background without herbicide 0.18-0.27 and on a herbicide background 0.15-0.30 t/ha; for the variety Dobrynya 0.15-0.24 and 0.15-0.30 t/ha, respectively. The highest indicators of safflower yield and fat yield per hectare for the variety Zhyvchyk 1.54 t/ha and 418 kg and for the variety Dobrynya 1.64 t/ha and 452 kg were obtained for cultivation against the background of soil herbicide for pre-sowing cultivation with post-emergence harrowing and inter-row cultivation.

Анотація:
The paper considers topical and debatable issues of consumer basket formation in modern Ukraine. The content of the concept of consumer basket and subsistence level of Ukrainians is analyzed, their interdependence is outlined. It was highlighted that terminological, systematic methods, classification method, as well as general scientific research methodology were important for solving research problems. It was found that the subsistence level is the basic state indicator of the social standard, on the basis of which the state determines social guarantees of income, as well as health care, education and housing and communal services. The attention is drawn to the fact that the causes of poverty in Ukraine are economic, because the consumer basket and the subsistence level are not related to the prices of goods and services, but primarily to the payment of utilities. It is emphasized that when compiling the consumer basket, not individual types and brands of products and goods are taken into account, but simplified (averaged) products of groups. It was stated that the set of the consumer basket is detached from the real prices in stores and the needs of the population. It is concluded that the consumer basket is a guarantee of state development in both social and economic dimensions. The higher the standard of living of the country's population, the better the economy works and the country develops. It is emphasized that when assembling a consumer basket, not individual types and brands of products and goods are taken into account, but simplified (averaged) goods of groups. It is stated that the modern set of the consumer basket is divorced from the real prices in stores and the needs of the population. It is concluded that the consumer basket is a guarantee of the development of the state in both social and economic dimensions. The higher the standard of living of the country's population, the better the economy works and the country develops.

Background. The government of Sierra Leone introduced social health insurance (SHI) scheme to provide universal health coverage to people. This study was carried out to assess the population characteristics and their implications on the benefit basket of the proposed national health insurance scheme. Methods. A cross-sectional study design was employed in six selected districts in Sierra Leone. Quantitative data were collected for this study through the use of semistructured questionnaires with a sample of 1,185 respondents. Data were analysed using descriptive and inferential statistics. Statistical analysis was run at 5% significant level using Stata 14.0 software. Results. The study found that most (83.54%) of the respondents affirmed that children below 18 years should be excluded from premium payments and as high as 71.65% also stated that pregnant women should be excluded as well. The majority, 63.69%, of the respondents want lactating mothers to be excluded from premium payments. Also, 79.87% of respondents wanted mentally challenged persons not to pay premium, while a significant proportion (84.26%) of respondents further affirmed that the aged (above 70 yrs) should also be excluded from premium payment. Most household heads (89.71%) preferred the accreditation of public health facilities. Regarding the level at which healthcare services should be covered by the scheme, 61.45% preferred the primary care services, 89% mentioned secondary care services, and 98.93% affirmed the provision of tertiary care under the scheme. As for the type of care that should be covered by the scheme, 98.66% and 99.73% affirmed outpatient and inpatient care, respectively. Conclusion. From the findings on population characteristics and their implications on the benefit basket for the proposed nation social health in Sierra Leone, most of the household heads want exemptions from paying premium for a section of the population. This provides a clear insight for policy makers into the formulation of the benefit basket.

Beauty care and the need for cosmetics have become the lifestyle of modern women, especially in big cities. Public awareness to look beautiful makes modern women competing to take care of themselves to be more beautiful. The body care industry in Indonesia continues to grow. The growth has reached 6% and is predicted to continue to grow along with the high concern of Indonesian women in caring for their skin. To win the competition, companies need to know the market and consumer situation. One strategy that can be applied by the company is to use a promotional or advertising strategy. This research was conducted at the Ariana Audy beauty clinic in 2022 with the aim of identifying customer buying patterns which will then be used as reference material in the development of promotional menus for products and services offered by the beauty clinic. The approach used to design components on the promotional menu is Market Basket Analysis by applying the fp-growth algorithm using rapid miner software. Market basket analysis is focused on finding relationships between products based on customer purchases. The market basket analysis conducted resulted in 5 association rules that define consumer purchasing patterns for products and services provided by the Ariana Audy clinic. Through the 5 association rules formed, 3 promotional menus were produced, namely menu 1 consisting of baby skin crystal and oxy blue cream, menu 2 consisting of brightening cream products and sunscreen brightening, and menu 3 consisting of oxy jet peel and photodynamic therapy.

Agency for Health Technology Assessment and Tariff System in health care system in Poland The Agency for Health Technology Assessment and Tariff System is national institution supervised by Minister of Health and funded by government. Agency is an advisory body in the reimbursement process. Agency has an impact on content of benefit basket, not only by evaluating new technologies but also re-evaluating benefit basket. Main goal of Agency is to provide patients access to the most effective and safe technologies. The most important Agency statements concern medical technologies and its modifications, qualification, removal in reference to guaranteed medical services. The Minister of Health, taking decisions about which health technology should be financed by public found, takes into account: recommendations and opinions of President of Agency, opinions and statements of the Transparency Council and the Council for Tariffs Affairs. The Agency’s task is also: setting tariffs for health care services; evaluating diagnostic and therapeutic guidelines prepared by medical societies; preparing opinions about national and local government health care programs.

ImportancePhysicians across the US spend substantial time working in the electronic health record (EHR), with primary care physicians (PCPs) spending the most time. The association between EHR time and ambulatory care quality outcomes is unclear.ObjectiveTo characterize measures of EHR use and ambulatory care quality performance among PCPs.Design, Setting, and ParticipantsA cross-sectional study of PCPs with longitudinal patient panels using a single EHR vendor was conducted at Brigham and Women’s Hospital and Massachusetts General Hospital during calendar year 2021.ExposuresIndependent variables included PCPs demographic and practice characteristics and EHR time measures (PCP-level mean of daily total EHR time, after-hours time, time from 5:30 pm to 7:00 am and time on weekends, and daily EHR time on notes, sending and receiving patient, staff, results, prescription, or system messages [in-basket], and clinical review).Main Outcomes and MeasuresOutcome variables were ambulatory quality measures (year-end, PCP panel–level achievement of targets for hemoglobin A1c level control, lipid management, hypertension control, diabetes screening, and breast cancer screening).ResultsThe sample included 291 physicians (174 [59.8%] women). Median panel size was 829 (IQR, 476-1157) patients and mean (SD) clinical full-time equivalent was 0.54 (0.27). The PCPs spent a mean (SD) of 145.9 (64.6) daily minutes on the EHR. There were significant associations between EHR time and panel-level achievement of hemoglobin A1c control, hypertension control, and breast cancer screening targets. In adjusted analyses, each additional 15 minutes of total daily EHR time was associated with 0.58 (95% CI, 0.32-0.84) percentage point greater panel-level hemoglobin A1c control, 0.52 (95% CI, 0.33-0.71) percentage point greater hypertension control, and 0.28 (95% CI, 0.05-0.52) higher breast cancer screening rates. Each daily additional 15 minutes of in-basket time was associated with 2.26 (95% CI, 1.05-3.48) greater panel-wide hemoglobin A1c control, 1.65 (95% CI, 0.83-2.47) percentage point greater hypertension control, and 1.26 (95% CI, 0.51-2.02) percentage point higher breast cancer screening rates. Associations were largely concentrated among PCPs with 0.5 clinical full-time equivalent or less. There were no associations between EHR use metrics and diabetes screening or lipid management in patients with cardiovascular disease.Conclusions and RelevanceThis cross-sectional study found an association between EHR time and some measures of ambulatory care quality. Although increased EHR time is associated with burnout, it may represent a level of thoroughness or communication that enhances certain outcomes. It may be useful for future studies to characterize payment models, workflows, and technologies that enable high-quality ambulatory care delivery while minimizing EHR burden.

Market Basket Analysis (MBA) merupakan salah satu teknik penemuan aturan asosiasi dalam data mining. MBA memanfaatkan data transaksi pada suatu toko untuk menentukan strategi penjualan. Konsep utama analisis ini adalah menentukan barang yang dibeli secara bersamaan oleh konsumen. Penentuan asosiasi dalam MBA berdasarkan kriteria minimum support dan confidence. Penelitian ini menggunakan algoritma apriori untuk data transaksi 212 Mart Soebrantas Pekanbaru periode Januari-Desember 2020. Algoritma apriori merupakan algoritma yang efisien untuk menentukan kandidat aturan asosiasi pada data dengan jumlah besar. Aturan asosiasi yang akan dibangkitkan adalah aturan asosiasi antar kelompok item dan asosiasi antar item. Berdasarkan hasil analisis ditemukan aturan asosiasi antar kelompok yang terbaik berdasarkan nilai lift tertinggi yaitu asosiasi antara clothing care dan body care dengan support 6,1% dan confidence 45,88 %. Aturan asosiasi terbaik untuk item yaitu asosiasi Lemonilo Mie Instan Ayam Bawang 7 dan Lemonilo Mie Instan Kari Ayam dengan support 0,17% dan confidence 42,11%.

Abstract According to [1]-[3], the healthcare systems are suffering from severe personnel shortages with a lack of medical care workers, operating room nurses, and surgical technicians. This deficiency will become worse in the upcoming decades. Automatization of highly standardized procedures could be a promising approach to address this issue. We designed a new concept for the automated handover and return of surgical instruments to the surgeon in an operating theater scenario to relieve operating room personnel from their workload. In this setup, the surgical instruments are delivered pre-sorted in an upright position by the sterilization unit in an instrument basket or drum. A single-use, doublelayered, brushy conveyor belt system mounted on a robot arm acts as the connector between pick-position (instrument basket) and individually pre-definable hand-over-position. The system is placed next to the table inside the operation room (OR). Based on voice commands, the conveyor intake moves over to the selected instrument and delivers it to the surgeon in a fast and short motion. After use, the surgeon can feed the instrument into the intake at the handover position and place it back in the instrument basket. The robot arm allows precise maneuvering of the intake and output position. The concept was realized as a minimal-viable product and will be evaluated for further improvement.

Metode market basket analys dapat digunakan untuk menganalisis keterkaitan produk yang dibeli konsumen sekaligus untuk mengetahui kebiasaan membeli konsumen. Penelitian berhasil menemukan kombinasi produk yang saling terkait yang memiliki nilai support dan confidence tertinggi pada kombinasi produk Basic Food, Home Care, Instant Food yang memiliki nilai support 14,1% dan confidence 100% serta produk kombinasi. Kombinasi pembelian produk berupa susu, snack, ice cream memiliki nilai support 14,1% dan confidence 100%.

Studi meta analisis dengan literature review dari 50 artikel yang diterbitkan di jurnal internasional yang berbeda terkait dengan ilmu olahraga dari Q4 ke atas dengan pengambilan sampel secara acak. Data dianalisis menurut epidemiologi cedera olahraga pada bola basket. Hasil Total, 50 artikel dipilih. Sebanyak 1.8275 cedera yang diamati, sebagian besar terjadi pada tungkai bawah 718 (3,92%) cedera lutut, 12030 (93,18%) cedera pergelangan kaki (ankle) dan 1.094, (5,96%) cedera lainnya. META ANALYSE STUDY OF THE COMMON INJURIES OF BASKETBALL PLAYERS AbstractsThe aims of this literature review was to identify all musculoskeletal and joints injuries, their common causes, and their treatments in order to pay attention to the basketball teachers, to take care for their students and athletes according to the trauma, but also a warning for the coach trainers of the different disciplines that require a lot of knee and ankle joints, and to show to the researchers the angles which are not yet solicit by the research. Meta analyse study with literature review of 50 articles published in different international journal related to sport science from Q4 and above by random sampling. Data were analysed according to the general epidemiology of sports injuries in basketball. Results In total, 50 articles were selected. A total of 18275 injuries were observed, most of them occurred in the lower limbs 718 (3,92%) Knee injuries,12030(93,18%) ankle injuries and 1094, (5,96%) others injuries. Children, adolescents ( athlete), and elders received head injuries more often compared with the other age.

8591 Background: Relapsed SCLC is a difficult-to-treat disease. A considerable number of SCLC patients are elderly with associated fragility and numerous comorbidities. Accelerated approval from the US FDA of lurbinectedin (Zepzelca) 3.2 mg/m2 q3wk as second-line therapy in metastatic small cell lung cancer (SCLC) was based on results from a phase 2 basket trial (NCT02454972) showing overall response rate of 35.2% and duration of response of 5.3 months. The ATLANTIS trial (NCT02566993) assessed the combination of lurbinectedin 2mg/m2/ doxorubicin (DOX) 40 mg/m2 as second-line treatment for SCLC vs. a control arm of topotecan/CAV. Methods: This post hoc analysis explores the efficacy and safety of lurbinectedin in relapsed SCLC patients ≥65 years included in both phase 2 basket (26 patients treated with lurbinectedin) and ATLANTIS (121 patients treated with lurbinectedin/DOX and 118 patients treated in the control arm) trials. This analysis did not include patients from the phase 2 basket trial with chemotherapy-free interval (CTFI) < 30 days, as these patients were excluded in the ATLANTIS trial. Results: Median age was similar (72.5 in lurbinectedin basket vs. 69 in ATLANTIS lurbinectedin/DOX vs. 69 in ATLANTIS control arm), most were males (65.4% vs. 57.9% vs. 60.2%), with ECOG PS 1 (65.4% vs. 68.6% vs. 68.6%) and CNS involvement (7.7% vs. 10.7% vs. 15.3%). Median CTFI (days) was also similar (105.5 vs. 123 vs. 120). Median number of cycles was 4 vs. 6 vs. 4. Main efficacy and safety results are shown in the table. Treatment-related adverse events (AEs) were reported in 92.3%, 92.6% and 94.1% of patients, respectively (grade ≥3 in 50.0%, 59.5% and 81.4%). Conclusions: In elderly patients, lurbinectedin seems to be superior to the standard of care in terms of both efficacy (higher response rate and longer duration of response and overall survival) and safety (less associated hematological AEs), which reinforces its role as a treatment option in relapsed SCLC patients ≥65 years. Abstract Background The period immediately following discharge is highly stressful for many individuals with schizophrenia spectrum illnesses as they transition from protracted inpatient stays to community settings with minimal support. In this period the risk of hospital re-admission is at its highest, many do not engage with community supports and clinical resources, and the risk of suicide is greatest. The evidence base for brief, effective interventions to support such transitions is to date underdeveloped. Methods This randomized controlled trial examined the effectiveness of a Peer Support intervention that combines components of Cognitive Adaptation Training (CAT) and the Welcome Basket Program. CAT is an evidence-based intervention that provides environmental supports to help people with schizophrenia compensate for the cognitive impacts of the illness. The Welcome Basket Program is a peer intervention approach that helps bridge discharge through the provision of a small individualized ‘basket’ of staple supplies and comfort items along with the facilitation of engagement with community resources. The intervention involves peer supports engaging patients in the days preceding discharge to assess goals and needs followed by weekly visits post-discharge for 1 month providing Welcome Basket and CAT supports. The study also collected pilot data examining the outcomes of an abbreviated, two contact version of the intervention. Inpatients with a schizophrenia spectrum diagnosis were randomized with a 2:2:1 ratio to treatment as usual, the full intervention, and the abbreviated intervention. Along with feasibility assessments, outcome metrics included re-hospitalization, symptomatology, quality of life, and community functioning. Assessments at baseline, 1-month post-discharge, and 6 months post-discharge facilitated the examination of relative effectiveness and sustainment of gains. Results The trial was successfully implemented with data collected from 106 participants at baseline, 82 at post-intervention, and 74 at 6-month follow up. Overall, the interventions and the study design appeared feasibility with attrition primarily due to the high acuity nature of a population recruited largely through an early psychosis inpatient unit (mean age 34.6 years). Preliminary analysis suggests limited effects on community functioning though completed analyses of other metrics are pending and may provide insight into the possible mechanisms of action of this intervention should it prove to be effective. Discussion This study was designed to assess the development and dissemination of a cost-effective method for mitigating relapse risk and promoting community involvement and engagement in care. The effort to better support successful care transitions through approaches such as this is a priority area for service systems globally and contributes to the literature on peer support. While widely implemented, models of peer support have seldom been examined for effectiveness in clinical trials.

Abstract The appetite for cutting-edge cancer research, across medical institutions, scientific researchers, and health care providers, is increasing based on the promise of true breakthroughs and cures with new therapeutics available for investigation. At the same time, the barriers for advancing clinical research are impacting how quickly drug development efforts are conducted. For example, we know now that under a microscope, patients with the same type of cancer and histology might look the same; however, the reality is that most cancers are driven by genomic, transcriptional, and epigenetic changes that make each patient unique. Additionally, the immunologic reaction to different tumor types is distinct among patients. The challenge for researchers developing new therapies today is vastly different than it was in the era of cytotoxics. Today, we must identify a sufficient number of patients harboring a rare mutation or other characteristic and match this to the right therapeutic option. This summary provides a guide to help inform the scientific cancer community about the benefits and challenges of conducting umbrella or basket trials (master trials), and to create a roadmap to help make this new and evolving form of clinical trial design as effective as possible.

Abstract:This paper develops an account of the normative basis of priority setting in health care as combining the values which a given society holds for the common good of its members, with the universal provided by a principle of common humanity. We discuss national differences in health basket in Europe and argue that health care decision-making in complex social and moral frameworks is best thought of as anchored in such a principle by drawing on the philosophy of need. We show that health care needs are ethically ‘thick’ needs whose psychological and social construction can best be understood in terms of David Wiggins's notion of vital need: a person's need is vital when failure to meet it leads to their harm and suffering. The moral dimension of priority setting which operates across different societies’ health care systems is located in the demands both of and on any society to avoid harm to its members.

Healthy food is becoming increasingly expensive, and families on low incomes face a difficult financial struggle to afford healthy food. When food costs are considered, families on low incomes often face circumstances of poverty. Housing, utilities, health care, and transport are somewhat fixed in cost; however food is more flexible in cost and therefore is often compromised with less healthy, cheaper food, presenting an opportunity for families on low incomes to cut costs. Using a “Healthy Food Basket” methodology, this study costed a week’s supply of healthy food for a range of family types. It found that low-income families would have to spend approximately 30% of household income on eating healthily, whereas high-income households needed to spend about 10%. The differential is explained by the cost of the food basket relative to household income (i.e., affordability). It is argued that families that spend more than 30% of household income on food could be experiencing “food stress.” Moreover the high cost of healthy foods leaves low-income households vulnerable to diet-related health problems because they often have to rely on cheaper foods which are high in fat, sugar, and salt.

603 Background: Standard treatment of oligometastatic pancreatic ductal adenocarcinoma (PDAC) consists of multi-agent chemotherapy, although outcomes remain poor. We tested the hypothesis that the addition of comprehensive metastasis-directed therapy (MDT) to standard-of-care systemic therapy would improve progression-free survival (PFS) compared with systemic therapy alone. Methods: The External Beam Radiation to Eliminate Nominal Metastatic Disease (EXTEND) trial is a multicenter phase II basket trial randomizing patients with ≤ 5 solid tumor metastases 1:1 to MDT plus systemic therapy versus systemic therapy alone. Each histology-specific basket was pre-specified and independently powered for the primary endpoint of PFS; here we present results of the pancreatic cancer basket. Progression was defined by radiographic criteria (RECIST v1.1), clinical events, or death. Patients were stratified at randomization by number of metastases (1-2 vs 3-5), number of prior lines of systemic therapy (0-1 vs 2+), and cancer antigen 19-9 (CA-19-9) level (<90U/ml vs ≥90U/ml). Exploratory endpoints included systemic immune response measures. The cutoff date for the primary analysis was September 1, 2023. ClinicalTrials.gov registration: NCT03599765. Results: Between March 2019 and February 2023, 41 patients with oligometastatic PDAC were randomized and 40 were eligible for PFS analysis (MDT arm: 19 patients; control arm: 21 patients). Baseline characteristics were well-balanced between arms. Thirty-one patients experienced progression or death; median follow-up time for censored patients was 9.9 months. The MDT arm had significantly longer PFS compared to the systemic therapy arm (median PFS 10.3 versus 2.5 months; log-rank p=0.006). After stratification, the estimated PFS hazard ratio was 0.43 (95% confidence interval: 0.20, 0.94; p=0.03) for MDT compared with systemic therapy. There were no grade 3 or greater adverse events with at least possible attribution to MDT, and only one grade 2 adverse event possibly related to treatment in the MDT arm occurred. Increased systemic T-cell stimulatory cytokines, CD8+ T-cell activation, CD8+ T-cell proliferation, and T-cell receptor clonal expansion following treatment were associated with the MDT arm compared with the control arm (all p<0.05). Conclusions: Among patients with oligometastatic PDAC, the combination of MDT plus systemic therapy was associated with improved PFS compared with systemic therapy alone. Further exploration in phase III trials is warranted, as is investigation into the potential therapeutic implications of systemic immune activation associated with MDT receipt. The everyday use of the bicycle is becoming increasingly common. People noticed the need to take care of both their health and the environment. The geographical location of Poland encourages cycling, which is the reason new kilometres of cycling routes are being built every year. This is associated with the rising interest in buying a bicycle. A conceptual tricycle with an additional electric drive powered by solar panels is presented in this article. The photovoltaic panel is installed on the basket roof. The electric motor may be used during an uphill ride and when the driver has run out of energy for pedalling. In the crank mechanism, there is a sensor which starts the motor when a small foot pressing force on the pedal is detected. The tricycle has an all-purpose structure, owing to which the seat mounted in the basket can be easily replaced with a double seat, a shopping bin or a child's safety seat. The tricycle may be used by both private persons and companies involved in tourist transport. The cost of the structural details of the tricycle suggested in this article does vary considerably from standard electric bikes, however, that solution has several important advantages, which may be decisive in the purchase of this solution.

Endoscopic retrograde cholangiopancreatography (ERCP) became the standard method of choice for the management of patients with a variety of benign and malignant pancreaticobiliary disorders. A growing range of ERCP accessories has been developed to support the increasing demands and complexity of therapeutic ERCP. Various accessories are needed from selective cannulation to the removal of bile duct stones which involves endoscopic sphincterotomy or endoscopic papillary (large) balloon dilation with a balloon or basket-assisted stone extraction. Detailed knowledge and correct usage of accessories are essential to ensure optimal patient care and safety. This review describes current accessories that are available to use during diagnostic and therapeutic ERCP.

The case-law of the Court of Justice (ecj ) on patient mobility was recently challenged by a ruling that a patient could go to Germany for treatment when facilities in Romanian hospitals were inadequate. Given the reported impact of austerity measures in the field of health care this raises the question; what is the impact of the ecj ’s ruling on how Member States can manage expenditure and limit outflows of patients and how should such measures be legally evaluated? The objective of this article is to analyse potential impact on health systems in the context of increasing pressure on public financing for health. While the ecj mainly referred to the requirement of treatment in due time, we also analyse possible austerity reductions of the basket of care against the background of eu law (i.e., ecj case-law, patient mobility directive, Charter of Fundamental rights and social security regulation).

The Agency for Health Technology Assessment and Tariff System is national institution supervised by Minister of Health. Agency is an advisory body in the reimbursement process. One of the Agency’s roles is setting a tariff for publicly funded healthcare services. Determining the cost of medical services is one of the most important conditions for valuating these tariffs. As in most European countries, the system of financing in-patient care in Poland is based on the Diagnosis Related Group system (DRG). DRG systems were created to increase efficiency in the use of available resources, and consequently to improve the quality of medical care. In order to compare the method and level of financing healthcare services with their equivalent abroad, the Agency analyzes information from other countries, especially those with GDP per capita similar to that of Poland. The Agency searches for patient group classification methods, payment methods, the shape of services and all factors affecting the valuation of services. The article describes the Polish inpatient services system against the background of solutions functioning in the world. The pricing process was presented, with particular emphasis on the process of obtaining data on foreign tariffs and their impact on the construction and pricing of healthcare services in Poland.

With the launch of the National Cancer Institute’s Precision Medicine Initiative in 2015, there has been a shift to trial designs that tailor health care solutions to individual patients by using a screening platform and by moving away from the one-trial/one-biomarker-at-a-time approach. To make precision medicine a reality, it is critical to identify and validate potential biomarkers to help select patients who will truly benefit from a targeted therapy. In this article, we discuss five trial designs: enrichment, umbrella, basket, subgroup, and window of opportunity. For each trial design, we describe the design characteristics, use ongoing or completed trials as case studies, provide any recent advances to the trial design, and discuss advantages and disadvantages of each design.

1636 Background: Oncology patients often encounter preventable acute issues prompting the need for proactive monitoring and management. To manage disease and therapy complications, Ochsner Health developed Chemotherapy Care Companion (CCC) for remote monitoring of treatment patients. This abstract highlights the feasibility of digital remote monitoring (DRM) in managing oncologic patient care, while increasing patient satisfaction and reducing patient events and healthcare costs. Methods: Starting in January 2021, patients initiating an oral or intravenous cancer treatment with an active Ochsner portal account and smartphone became eligible. Providers can manually enroll patients via an order set and receive enrollment reminders within the Epic electronic medical record (EMR). Patients receive a digital WiFi-enabled monitoring scale, blood pressure cuff, and ear thermometer, at no cost. Patients are reminded via phone alerts to take vitals at 9am and 3pm, along with a daily questionnaire. The data is transmitted wirelessly to the EMR, where abnormal vital signs or questionnaire responses trigger escalations sent to an Epic in-basket pool. During clinic hours, advanced practice providers (APPs) review and intervene, utilizing embedded urgent care slots for same-day access. In August 2022, a patient satisfaction survey comprising of nine questions was distributed to all enrolled patients. Results: A retrospective analysis was conducted from September 2022 to August 2023 including 722 participants, 7,360 escalations and 668 interventions. Patient compliance reached 62%, defined as submitting at least 50% of daily vitals. The most common interventions were related to hypertension, hypotension, weight changes, and questionnaire responses. Regarding questionnaire responses, diarrhea, pain, fever, and dizziness were the mostly frequently triaged. Data analytics revealed that CCC led to a 5.9% decrease in emergency room visits, a 41.7% decrease in the rate of inpatient hospital admissions, and a 32.3% improvement in the inpatient length of stay compared to non-enrolled patients. Extrapolating this data across 5,500 oncology patients within our hospital system, CCC could prevent 172 ER visits, 914 inpatient hospital admissions and 2,006 hospital days. The survey (131 responses) revealed high satisfaction with CCC: over 80% felt a sense of security, 90% were overall satisfied, and more than 90% would recommend CCC. Conclusions: Findings highlight the value of integrating DRM in cancer care, showcasing its ability to reduce healthcare costs and optimize resource utilization while enhancing patient care and satisfaction. Moreover, the study underscores the high patient satisfaction and engagement with DRM, bolstering its feasibility and acceptance in oncology care. Further investigation is warranted to explore the financial benefits for hospitals, insurance companies, and pharmaceutical companies.

This issue of the Journal uses the metaphor of raranga te kete aronui (weaving the basket of the pursuit of knowledge) as it continues the conversation of developing areas of open, flexible, and distance learning (OFDL). It comes at a time when the COVID-19 pandemic has disrupted traditional face-to-face teaching in classrooms on a global scale, and the value of open, flexible, and distance learning has become starkly apparent. This issue has five articles which contribute to the ongoing knowledge presented in this issue. There are two articles based on educational theory: Higgins explores the theoretical history of OFDL, and the work of Nichols et al. is based on Mezirow’s 10 stages of transformative learning. Three research articles provide insight into a range of specific areas: Sime et al. use networked learning theory to investigate the design of a social exploration MOOC, Irons and Hartnett present exploratory research into the teaching of computational thinking in junior classrooms in New Zealand, and Nyugen describes the design and development of qualifications in early childhood education and care to meet a demand for New Zealand home-based early childhood educators to be qualified. These are all contributions to our weaving and filling of te kete aronui.

Budget standards are specified baskets of goods and services which, when priced, can represent predefined living standards. ‘Low cost but acceptable’ (LCA) is a minimum income standard, adequate to provide warmth and shelter, a healthy and palatable diet, social necessities, social integration, avoidance of chronic stress and the maintenance of good health (physical, mental and social) in a context of free access to good-quality health care, good-quality education and social justice. The LCA food budget standard identifies a basket of foods and corresponding menus which provides (for a given household composition) a palatable diet that is consistent with prevailing cultural norms, and that satisfies existing criteria for health in relation to dietary reference values, food-based dietary guidelines and safe levels of alcohol consumption. Two previous studies that explored the relationship between diet and food expenditure in low-income households suggested that the amount spent on food was a good predictor of dietary adequacy, growth and health in children. The current paper will focus on diet and measures of deprivation in 250 low-income households in London. Households were screened for material deprivation (e.g. no car, no fixed line telephone, in receipt of Income Support) using a doorstep questionnaire. Diet was assessed using four 24 h recalls based on the ‘triple pass’ method. Expenditure on food and other aspects of household circumstances were assessed by face-to-face interview. Food expenditure in these households was characterized in relation to food budget standards. Further analyses explored the relationships between food expenditure and dietary adequacy, growth in children and measures of deprivation.

There are many players for health care delivery systems like public and private providers. In between, another service delivery system exists and survived at large scale in rural India. One of them is called Rural Medical Practitioner or quacks or Jhola Chaap (unqualified service provider) or unethical practitioner. The term RMP is still not clear. In the lay men language, RMP stands for registered medical practitioner but only graduate in MBBS are registered by the Medical Council of India and has a valid licence to practices allopathic medicine in public health. In the medical world. In many developing countries like India, informal or rural medical practitioner provides the large-scale services to the poor and villagers. These practitioners cater a large population especially in rural India. In Rajasthan, the coverage of population by Government Public Health Services is not sufficient. This study carried out with interviewing 117 unqualified health care providers in rural area of Rajasthan. The current paper discusses about academic background, training, experience, daily patient load and practice of informal health care providers altering the unavailable health care professional for a long time. As resulted, a total of 95 (81.20%) informal health care providers have their own clinic where they attend the patients and provide services 24 x 7 without charging consultation fee, go for home visits to see the patient and refer the patients in critical situation to Government health facilities, etc. The strong presence of these so-called rural practitioners in the basket of private health care system put the rural health in very critical situations.

Low-threshold support services are provided within the basket of services of German long-term care insurance as a part of respite care to support family carers and people with dementia. This study investigates various stakeholders’ (providers, coordinators, volunteers, family carers and people with dementia) perspectives on low-threshold support service regarding its organisation and conceptualisation as well as how stakeholders and users value low-threshold support service using a qualitative approach. Twelve guided interviews and group discussions were conducted with 31 participants. Organisation and conceptualisation are characterised by the lowness of the service thresholds, which is perceived to be quick and simple forms of support with no user requirements. Multiple barriers such as the challenging behaviour of people with dementia and their initial refusal as well as their holding low-threshold support service in low esteem can hinder the utilisation of these services. Low-threshold support service within the scope of the long-term care insurance law can be separated into two types: low-cost (non-professional) services and high-cost services with comprehensive training for ‘employed’ volunteers (professional). Both types are constantly developing within the landscape of the German long-term care system, and low-threshold support service appears to be adapted to diverse needs. Therefore, it is important to avoid replacing non-professional services with professional services.

: Worldwide 75 million women need post abortion care (PAC) services each year following safe or unsafe induced abortions and miscarriages. Majority of them do not wish to conceive in near future and are receptive towards using contraception. Contraceptive counselling and basket of choices should be made available to all women undergoing abortion and her right to decline or postpone this care should be respected while recognizing that each woman has a unique unmet need of contraception.: To compare the acceptance and continuation of various contraceptive choices offered after first and second trimester abortion. : A retrospective study was conducted at a tertiary care hospital in which women who underwent first and second trimester abortion over a period of one year were interviewed regarding their acceptance and continuation of the contraceptive method till six months post abortion.: A total of 378 women underwent first and second trimester abortions over one year, of which 61.3% had resulted from failure of contraception. Women were grouped into Group A for those undergoing first trimester and Group B for women undergoing second trimester abortion for study purpose. Average age of women in group A was 28.9 years and in group B was 26.5 years. Mean period of gestation of pregnancy in group A was 7.5weeks and in group B was 19.5 weeks. In group A 96% parous women had conceived as a result of failure of contraception of which 90% were using natural method (Coitus Interruptus). Group B majority (76.3%) women were nulliparous and early parous (P1, P2) and indication for termination of pregnancy in them were gross congenital anamolies (GCA) in fetus while in rest of multiparous were due to failure of contraception. With group A 53.1% nulliparous women did not opt for any contraception whereas 48.6% primiparous and second parous opted for long acting reversible contraception(LARC) –Intrauterine contraceptive device(IUCD) in contrast to 66% multiparous opting for permanent sterilization at the time of abortion. In group B 45.3% nulliparous and early parous opted for depot medroxy progesterone acetate (DMPA) and 64.2% multigravidas underwent permanent sterilization. : Post abortion care must include counselling and choice of contraceptive basket. Women with future reproductive plans preferred LARC after first trimester abortion whereas DMPA was most acceptable method of contraception after second trimester abortion. Women were most satisfied with LARC and permanent methods compared to methods that required daily intake of pills. There is need of effective contraceptive choices according to the reproductive plans of the women to reduce the burden, morbidity and mortality associated with abortions.

The need to improve living and working conditions in the European Union, but also the idea of ensuring economic and social progress within it, required that the minimum income of each worker could be determined transparently and predictably by each member state, in accordance with social reality and in accordance with national legislation. Directive 2.041/2022 on the adequate minimum wage in the EU, states that from the moment of its entry into force (October 2022), member states have two years to transpose the provisions into domestic legislation. Not all EU countries will have the same adequate minimum income. Each state will establish its level according to social and economic conditions, purchasing power, productivity level and the evolution of macroeconomic indicators at the national level, but having as a guide precisely the provisions of the European directive and the principles it includes. Thus, countries that already have a minimum wage per economy are committed to changing its level according to a formula that ensures a decent living, is consistent with the inflation rate and can cover the minimum shopping basket for various necessary goods and services. Member States are recommended to update their minimum wages at least every two years, and in countries using an automatic indexation mechanism, at least every four years. In order to correctly calculate this income, the directive recommends that member states take as a starting point the value of the consumption basket (as provided by national statistical reports), or set this income at 60% of the median salary or at 50% of the salary gross average per economy. For example, a consumption basket for a decent living can be composed of costs for: food, clothing and footwear, housing insurance, furnishing and maintenance, household products and personal hygiene, services, education and culture, health care, recreation and vacation, savings fund. Romania (with just over 600 euros) ranks among the last EU countries, in 20th place out of 22, just ahead of Hungary and Bulgaria, on a scale of this indicator between 2,387 euros in Luxembourg and 399 euros in Bulgaria. The EU directive will generate greater transparency, certainty, stability and predictability on the labor market, and in times of crisis it will become an anchor for vulnerable citizens from EU countries, implicitly also for those from Romania. Abstract Objective The COVID-19 pandemic changed clinician electronic health record (EHR) work in a multitude of ways. To evaluate how, we measure ambulatory clinician EHR use in the United States throughout the COVID-19 pandemic. Materials and Methods We use EHR meta-data from ambulatory care clinicians in 366 health systems using the Epic EHR system in the United States from December 2019 to December 2020. We used descriptive statistics for clinician EHR use including active-use time across clinical activities, time after-hours, and messages received. Multivariable regression to evaluate total and after-hours EHR work adjusting for daily volume and organizational characteristics, and to evaluate the association between messages and EHR time. Results Clinician time spent in the EHR per day dropped at the onset of the pandemic but had recovered to higher than prepandemic levels by July 2020. Time spent actively working in the EHR after-hours showed similar trends. These differences persisted in multivariable models. In-Basket messages received increased compared with prepandemic levels, with the largest increase coming from messages from patients, which increased to 157% of the prepandemic average. Each additional patient message was associated with a 2.32-min increase in EHR time per day (P &lt; .001). Discussion Clinicians spent more total and after-hours time in the EHR in the latter half of 2020 compared with the prepandemic period. This was partially driven by increased time in Clinical Review and In-Basket messaging. Conclusions Reimbursement models and workflows for the post-COVID era should account for these demands on clinician time that occur outside the traditional visit.

ABSTRACT Objective: To analyze the influence of social incentives for adherence to tuberculosis (TB) treatment. Method: Qualitative study, in which 26 primary health care professionals of São Paulo were interviewed in 2015.Their testimonies were submitted to the speech analysis technique. The theoretical reference was the social determination of the health-disease process. Ethical procedures were observed. Results: TB is related to precarious living conditions. Incentives such as the basic food basket and transportation stipends are relevant for patients’ adherence to treatment, as well as to the create bonds between the patient and the health team. Final considerations: The incentives strengthened adherence to TB treatment. However, interventions in the context of public measures must transcend the remedial dimension and be guided towards the transformation of the TB situation, which means supporting processes that modify living conditions.
