D generator which will also perform the necessary processing of the said data. A basis of image data will be established. Повний текст джерелаThis thesis embraces a social science research perspective to examine uses of the app SMSlivräddare (eng. SMSlifesaving), now Heartrunner, dedicated to alert volunteers nearby to assist people suspected to suffer from a cardiac arrest outside hospital. This case study of the uses of the health and medical app juxtaposes the public portrayals of the app, its prospective users, their agencies and use practices with the volunteer users’ own accounts. The analysis explores dimensions of the app’s and its users’ agencies as delegated by the technology’s portrayals and perceived by its users. It renders visible also possibly obscured aspects of the volunteer users’ agencies and practices at the time of the technology’s implementation in the two first regions, before its subsequent adoption in other Swedish regions and in Denmark. A medical research perspective has so far dominated the studies of lifesaving apps. Such research evaluates the patients’ health outcomes resulting from the app use by the volunteers and concentrates on the examination of the efficiency aspects of the app, such as how many users arrived and how many engaged in resuscitating the patients. At the same time, it contributes to the promissory discourses and instrumental approaches applied to understand the meanings and uses of health and medical apps. In contrast, building on the discourse and thematic analysis of the qualitative research material, this thesis seeks to highlight the users’ perspectives in their co-constructing of the SMSlifesaving technology through their app use practices; it embraces a socio-material theoretical approach and critically explores the users’ agencies as delegated by the discourses of the project developers, managers and evaluators of the medical technology and as negotiated by the users in their daily practices. This thesis, first, investigates the public portrayals of the app, its users and their agencies published online, in the user-recruiting practices, and in a medical research publication evaluating the SMSlifesaving technology. Next, it examines how the volunteers’ accounts describe the rationales of their entry into their SMSlifesaving app use practices, the social context embedding their entry and the meanings which they ascribe to their practices. Third, the study investigates how the volunteers’ accounts in juxtaposition to the online portrayals of the SMSlifesaving technology represent the volunteers’ app use before their receptions of the app’s notifications which inform them about cardiac-arrest cases nearby, at the time of reception of such notifications, and following acceptance of such notifications.Contributing to the field of critical social research on health and medical apps, the thesis identifies that both the SMSlifesaving app users and the technologies they co-construct have agencies. It illustrates the users’ agencies delegated and negotiated; the latter when they overcome the app everyday dependencies and judge the app-mediated volunteer work importance versus their paid work and private life commitments, develop dutiful engagement with the app and re-define the app’s medical promises for the patients and their families

Повний текст джерелаThe research addressed in this thesis is one of the first studies to address the Emergency Medical Service in France, known as SAMU (which stands for the French acronym of Urgent Medical Aid Services). It is funded by the French National Research Agency and aims at improving the organizational processes of the Val-de-Marne department SAMU system in order to meet the population’s needs under limited resources. For this purpose, we develop a discrete event simulation model in order to assess the current performance of this complex system, as well as to investigate the effects of potential process changes that would lead to enhanced operational efficiency, in terms of response time performance (i.e. the period between the receipt of a call and the first arrival of a rescue team at the scene), which is a critical aspect for SAMU providers. This model was used as a decision-support tool for comparing the relative benefits of several scenarios mainly related to the needed resource levels and static location of rescue teams throughout the Val-de-Marne area and their assignment to incoming calls. Sensivity analyses were also performed by changing values of some input parameters such as arrival rates of calls, travel times and service times. Finally, we used two simulation optimization approaches to analyze the impact of rescue teams assignment to bases by considering temporal fluctuations of travel times and number of resources during a day. Experimental results of the two approaches were analyzed and compared regarding computational times and response time distribution

Повний текст джерелаPedagogical agents, non-player characters, virtual advisors or assistants, virtual agents are more and more present in our daily life. Some attract our attention, seem to show us interest, to be able to communicate and to express their emotional states. These agents have been the subject of numerous investigations in various fields of research such as computerscience, psychology or cognitive sciences. The work of this PhD thesis focuses on the expressive virtual agents and their impact on the user empathy induction. The evolution of computer graphics techniques now makes it possible to create virtual agents that are visually and behaviorally realistic. The expressiveness of agents is an important issue for human-computer interactions. However, it is still rare for virtual agents to be equipped with facial expressions, thus limiting their ability toinduce empathy in the user.Our work follows up on the perspectives opened by researchers in the field concerning the transcription of emotions on a virtual agent, and contributes to extend the knowledge concerning interactions with agents, in particular the impact of their expressiveness on the implementation of an empathetic situation. To carry out this work, we conducted two experiments. The first one deals with the recognition of basic emotions on a virtual agent designed using advanced modeling techniques. This study also allowed us to evaluate the relevance of human expressiveness factors on the agent, notably the presence of expression wrinkles and the variation of pupil size according to the emotional state, to facilitate the perception of the agent’s emotions. Our second experiment focuses on the impact of the virtual agent’s facial expressiveness on the user’s empathy. Depending on the context, the results show that user’s perspective taking, the cognitive component of empathy, is greater when the realistic virtual agent presents emotional facial expressions compared to an agent who has no facial expressions. Finally, we studied the impact of the agents’ expressiveness on the user’s engagementand social presence. This study opens perspectives on a potential correlation between the notions of empathy, social presence and engagement

It does succeed since it gets enriched bye the principles of dignity, safety and precaution (first part) implying ethical, then jurisprudential obligations whether they are civil or administrative then at the origin of new subjective rights for the patient. The public order so strengthened turns out to be both politically correct as it is deprived of moral or religious connotations and legally effective as it is able to reconciling the general, local but also peculiar, universal, current or yet to come interests, to which the society attaches importance. This new and unstabilised potentialisation forces to wonder about the emergence of a management public order (second part), softer, more consensual, operationnal in the sector of medical and biomedical activity, which coherence, efficiency and legitimacy come less from protection than from the search of a consensus. The public order, then better accepted, or even expeted, goes in the direction wanted by the gouvernance, helped bye the sanitary democracy, sort of partnership between the tree actors, the patients, the professionals of health and the public authorities

Повний текст джерелаCloud computing has emerged as a successful paradigm allowing individuals and companies to store and process large amounts of data without a need to purchase and maintain their own networks and computer systems. In healthcare for example, different initiatives aim at sharing medical images and Personal Health Records (PHR) in between health professionals or hospitals with the help of the cloud. In such an environment, data security (confidentiality, integrity and traceability) is a major issue. In this context that these thesis works, it concerns in particular the securing of Content Based Image Retrieval (CBIR) techniques and machine learning (ML) which are at the heart of diagnostic decision support systems. These techniques make it possible to find similar images to an image not yet interpreted. The goal is to define approaches that can exploit secure externalized data and enable a cloud to provide a diagnostic support. Several mechanisms allow the processing of encrypted data, but most are dependent on interactions between different entities (the user, the cloud or a trusted third party) and must be combined judiciously so as to not leak information. During these three years of thesis, we initially focused on securing an outsourced CBIR system under the constraint of no interaction between the users and the service provider (cloud). In a second step, we have developed a secure machine learning approach based on multilayer perceptron (MLP), whose learning phase can be outsourced in a secure way, the challenge being to ensure the convergence of the MLP. All the data and parameters of the model are encrypted using homomorphic encryption. Because these systems need to use information from multiple sources, each of which outsources its encrypted data under its own key, we are interested in the problem of sharing encrypted data. A problem known by the "Proxy Re-Encryption" (PRE) schemes. In this context, we have proposed the first PRE scheme that allows both the sharing and the processing of encrypted data. We also worked on watermarking scheme over encrypted data in order to trace and verify the integrity of data in this shared environment. The embedded message is accessible whether or not the image is encrypted and provides several services

Medical images are carrying crucial information for diagnostic, teaching and research, moste of the time this information being very difficult to describe using only text. In this context, we are proposing architecture for the annotation of medical images included in online health documents. Our system extracts specific medical information (modality, anatomical regions ...) from several sources and combines this information to annotate the images. This annotation is necessary to be able to search for images inside documents. We implemented two distinct approaches (each based on a different type of information) : one oriented towards the image content, and a second oriented on the image context (image-related textual regions)

Повний текст джерелаThe thesis, conducted as part of the E-care project, proposes a methodological process to facilitate the analysis and design of medical remote monitoring systems for early detection of signs of any complications. The proposed methodology is based on a multi-agent system using several types of ontologies associated with an expert system. The multi-agent system is suitable for medical monitoring with a distributed architecture to keep some autonomy and responsiveness of habitats. The process identifies the generic and specific aspects of each system. The designed architectures take into account all the patient data such as: patient profile, medical history, drug treatments, physiological and behavioral data, as well as data relating to patient's environment and his lifestyle. These architectures should be open to be adapted to new data sources.This methodology was applied to E-care project in order to define its information system. This information system is composed of two types of ontologies (problem ontology and several domain ontologies) and an expert system for the detection of risk situations. The problem ontology was built to manage the system including users and their tasks. Three domain ontologies have been built to represent, disease, drugs and cardiovascular risk factors. The expert system uses inference rules, which are defined in collaboration with medical experts using their knowledge and some medical guidelines. This methodology also defined the system architecture, which consists of four autonomous agents types namely: medical sensors to collect physiological measurements. The gateway collects data from sensors and transmits them from the patients' homes to the server. The server processes data and gives access to them. Finally the database secures storage of patient data.As part of the E-care project, an experiment was conducted to validate the various system components. This experiment has gone through two phases, the first was held at the University Hospital of Strasbourg, and the second is in the patients' homes

Повний текст джерелаStroke is a sudden and brutal pathology that can lead to serious sequelae. The damage to major neurological functions caused by stroke explains these severe sequelae. In the long term, a disability of very varied nature and severity most often persists, often accompanied by significant depressive syndromes and emotional and sometimes behavioural disorders. In France, stroke is the third leading cause of death among men (13,003 deaths) and the first among women (18,343 deaths) in 2013. The management of stroke patients consists of several sequences: the pre-hospital phase, with an urgent need for transfer to a stroke unit (UNV), then acute hospitalization, with emergency therapeutic management and, finally, direct return home. During the pre-hospital phase, the notion of urgency is essential: the sooner the patient is treated, the more likely he is to reduce the risk of serious sequelae. Indeed, a victim loses 2 million neurons every minute during the acute phase of stroke. Two treatments, which may be complementary, exist: thrombolysis and thrombectomy. They are carried out in the treatment reference structures, the stroke unit (UNV). Areas are not equal in terms of stroke management, either in terms of accessibility or in terms of the spatial distribution of stroke occurrences. The geography of health is thus an essential discipline to address these issues. Geographic tools allow to target populations at risk of delay, but also at higher risk of occurrence. This research work thus pursues different complementary objectives. First, this work aims to describe the patient's accessibility to treatment, regardless of where he is treated, from the moment he calls the SAMU centre 15, depending on the type of vectors of care (SAMU, fire brigade), traffic conditions and therapeutic strategy. In this way, populations at risk of delayed care, and possibly lack of access to treatment, can be identified. In addition, the second objective is to describe the spatial distribution of stroke occurrences and characterize models of spatial distribution of stroke. This distribution is then analyzed using different territorial variables (socio-economic, facilities, environmental) to understand the causes of the over-representation of stroke in some areas. In order to conduct a global characterization of the explanatory causes of stroke occurrence, time series analyses were also conducted to determine, among other things, whether a type of time at risk of stroke exists. The analyses conducted throughout this work were based on data from the thrombolytic stroke registry of a French emergency network called Réseau des Urgences de la Vallée du Rhône (RESUVal), as well as data from the STROKE 69 cohort in the Rhone. In addition, the evaluation of accessibility in the department was able to benefit from the use of the SDMIS database.This work is intended to be very operational, making it possible to propose ways to improve stroke management in the region: in terms of the organization and distribution of structures that can treat patients, but also in terms of public policies to raise awareness among populations at risk of having a stroke
