SUMMARYIn 1994 and 2002, respectively, the World Health Organisation proposed that treatment for hookworm and schistosomiasis could be provided during pregnancy. It was hoped that this might have benefits for maternal anaemia, fetal growth and perinatal mortality; a beneficial effect on the infant response to immunisation was also hypothesised. Three trials have now been conducted. Two have examined the effects of benzimidazoles; one (the Entebbe Mother and Baby Study) the effects of albendazole and praziquantel. All three were conducted in settings of high prevalence but low intensity helminth infection. Results suggest that, in such settings and given adequate provision of haematinics, the benefit of routine anthelminthics during pregnancy for maternal anaemia may be small; none of the other expected benefits has yet been demonstrated. The Entebbe Mother and Baby Study found a significant adverse effect of albendazole on the incidence of infantile eczema in the whole study population, and of praziquantel on the incidence of eczema among infants of mothers with Schistosoma mansoni. Further studies are required in settings that differ in helminth species and infection intensities. Further research is required to determine whether increased rates of infantile eczema translate to long-term susceptibility to allergy, and to explore the underlying mechanisms of these effects. The risks and benefits of routine anthelminthic treatment in antenatal clinics may need to be reconsidered.

In recent years, horse breeding has been gradually reviving. Pedigree stud farms for breeding different horse breeds are developing, and the role of work horses in farms and the private sector has significantly increased. Nevertheless, helminthiases are a serious problem in the development of horse breeding. To determine the infection of horses with helminths, 53 animals were subjected to scatological research: 11 animals at the age from 1 to 2.5 years, 9 animals from 3 to 5 years, 20 animals from 6 to 10 years, and 13 animals from 11 years to 21 years. One day of inactivity of horses infected with gastrointestinal strongylates will cost the farm 72,000 Rubles. Considering that the animals must be on the farm for another 10 days after deworming until the animals are completely free of the invasion, the possible damage to the farm from inactivity of work horses increases to 720,000 Rubles. Economic efficiency per 1 Ruble of costs for deworming horses with an anthelmintic drug containing albendazole amounted to 6.01 Rubles, that containing eprinomectin – 3.9 Rubles, and that containing praziquantel + ivermectin – 5.8 Rubles.

The study investigated persistence of urinary schistosomiasis and geohelminthiasis during MDA of anthelminthics. Information on supply and utilization of the anthelminthics from 2014 to 2017 were obtained from Ebonyi State Ministry of Health, Abakaliki, and from Ezza North LGA headquarters respectively. Urine and stool specimens were collected from 200 pupils from selected schools in Ezza North LGA. The specimens were examined using standard parasitological techniques. A total of 389,064 and 725,443 tablets of praziquantel and albendazole respectively were supplied to the study-area from 2014 to 2017. The percentage utilization of the regimen was 57.03% and 63.88% respectively. Age-dependent prevalence of schistosomiasis among the pupils indicated a total of 10.5%. Age group 9-11 years had least prevalence (6.15%) in comparison with others, with significant difference (p=0.044). Though the males were infected more than the females (11.96% vs 9.26%), no significant difference (p=0.535) was observed in prevalence of S. haematobium infections. Gender-related prevalence of soil-transmitted helminthiasis indicated no significant difference (p=0.224). Infection prevalence with respect to taxa of geohelminths indicated that hookworms had higher prevalence (24.50%) than A. lumbricoides (5%) and Trichuris trichiura (2.5%). Prevalence of the helminths were still high, despite appreciable levels of supply and utilization of the regimens. Surveillance for drug resistance is recommended. BACKGROUND: A big proportion of populations in sub-Saharan Africa still battle with poverty and intense exposure to infections including chronic worm infections. Studies of cognitive effects of worms have not been consistent regarding which functions are affected and whether deworming is beneficial. However, executive functions could be particularly susceptible to impairment. The home environment is reported to affect child development. However, its unique contribution in Ugandan children exposed to various factors has not been estimated. This PhD research therefore aimed to [I] examine effects of maternal and childhood worm infections and their treatment on executive function and other developmental outcomes in infancy and preschool-age children; and [2] investigate the independent effects of the home environment on child mental performance, taking into account the confounding effects of other socio-demographic exposures. In the same sample, [3] relationships between executive functions were examined in order to understand how they may be disrupted by worms and other adverse factors. METHODS: This research was conducted within the ongoing Entebbe Mother and Baby Study (EMABS) in which 2500 pregnant women were randomised to receiving a single dose of albendazole or placebo and praziquantel or placebo in a 2x2 design. 983 infants born to these women were assessed at age 15 months on executive function, and other abilities to measure the effect of maternal worm infections and their treatment on these functions. The infants were then randomised to quarterly doses of albendazole or placebo from age 15 months and were assessed again at age five years (N=870) on measures of executive function, general ability and motor function to examine effects of childhood worm infections and their treatment on performance on these functions. Relevant health and socio-demographic date were collected and used in the analyses. 200 children were assessed on an extended battery of executive function measures and their scores were used to explore the structure and relationships between executive functions . Home environments of 163 children were examined using an adapted version of the Home Observation for Measurement of the Environment (HOME). RESULTS: Maternal Mansonella perstans and Strongyloides stercoralis infection were negatively associated with infants' performance on the A not B task (a measure of executive function) and language respectively but anthelminthic treatment did not show significant effects overall. In childhood, worms were negatively associated with performance on measures of working memory (Verbal Fluency, Counting Span, and Running Memory), attention (Picture Search), and general ability (Block Design). However, treatment with quarterly albendazole did not have significant effects on performance on measures of cognitive functioning. The home environment contributed to cognitive performance above and beyond the effect of other factors. Low-to-moderate inter-construct correlations were observed among executive functions and three components were extracted from the four domains. Planning was most related to inhibition with working memory also contributing to the best fit model. CONCLUSION: Both infant and childhood developmental outcomes suggest that executive functions may particularly be susceptible to impairment by worm infections but treatment may not reverse the effects. Furthermore, the results show that the home environment affects the child independently of SES, parental education and other global measures. Correlations and factor loadings indicate unity among the executive functions, particularly between planning and a combination of inhibition and working memory,

Helminth infections are major health problems, especially in developing countries with warm and humid environments. Limited synthetic drugs are available for treating helminth infections, and some of them showed resistance to helminths. Phytoconstituents can be used to develop cheap and effective anthelminthic drugs. Borassus flabellifer has been used in traditional medicine, in addition to consuming as food. This study was designed to evaluate the comparative in vitro anthelmintic activities of aqueous and methanol extracts of root, fruit pulp, and sap of Borassus flabellifer using two different concentrations (50 and 100 mg/mL). Adult earthworms, Eisenia fetida was used to evaluate the in-vitro anthelmintic activity. In-vitro anthelmintic activity was evaluated by determining times of paralysis and the death of worms. The anthelminthic activity of extracts was compared with mebendazole, and earthworm saline was used as a control. ANOVA followed by Dunnett’s multiple comparison test was used to compare the activities of extracts with mebendazole. All extracts showed concentrations dependent activity. All extracts except aqueous extract of roots of Borassus flabellifer showed statistically significant in-vitro anthelminthic activity. Further studies are needed to isolate and evaluate the active components from the extracts of Borassus flabellifer.

This study was conducted to determine the nutritional status of primary school children with the mid-day meal program. A cross-sectional study was conducted in 05 primary schools with the mid-day meal program of Sundarganj Upazila of Gaibandha district from January 01 to December 31, 2017. A total of 357 primary school children of class IV and class V were selected using a systematic sampling technique. A semi-structured questionnaire was used to collect data from the school children of mid-day meal program schools. The anthropometric measurements such as weight, height and MUAC were measured for each child individually. Then weight and height were used to calculate the Z score to interpret nutritional status. The mean age of children was 10.83±1.03 years. The mean weight and mean height of children were 32.40±7.21 kg and 141.22±8.52 cm respectively, and the mean MUAC was 19.65±2.31 cm. It was revealed that 91.3% of children were normal by Height for Age (HAZ) and 89.1% were normal by Weight for Age (WAZ), and the remaining children were stunted and underweight respectively. 78.8% of children used anthelminthic drugs regularly and among them, 74.3% were normal in Weight for Age Z score. Socioeconomic status, dietary habit, physical activity and anthelminthic use by the child were found to be determinants of their nutritional status.
