This study investigated demographic and personality variables, that counselors used in the selection of alcoholics for long term treatment and compared the completion rate for this group against completion rates reported in past studies, in order to determine if certain personality variables are associated with completion. The MMPI, Survey of Drinking Patterns and Effects, and a demographic questionnaire was administered to 355 lower class and "skid row" inpatient alcoholics from a North Tucson alcoholism treatment center. Results showed that counselors selection judgements approximated the characteristics of alcoholics who had completed long term treatment programs in past studies. Completion rates for this group studied were found to be moderately high compared to past studies. It was concluded that certain biographical and psychological variables can be used as selection criteria for determining alcoholics long term treatment completion. Implications for these findings are discussed.

The treatment of alcoholism has had a unique historical development in the United States. This study provides a chronology of how the problem of alcoholism was defined and handled during various time periods in United States history. The process that evolved resulted in an abstinence based, comprehensive, multidisciplinary approach to the treatment of alcoholism as a primary disease based on the principles of Alcoholics Anonymous. This treatment modality, that developed outside of established medicine, is currently used by the majority of treatment providers. Seven individuals who have been actively involved in alcoholism treatment were interviewed. In addition to archival research, biographies and autobiographies were examined to gain a broad perspective. Because alcoholism is both a collective and an individual problem an effort was made to include a microsociological frame of reference within a broad sociological view. Alcoholism, or inebriety, was first perceived as a legal and moral problem. By the end of the 19th century, inebriety was recognized as an illness differing from mental illness, and separate asylums were established for its treatment. Alcoholism is currently accepted and treated as a primary disease by the majority of social institutions, but the legal and moral implications remain. National Prohibition in the early part of the 20th century targeted alcohol instead of the alcoholic delaying any progress toward treatment which was made in the 19th century. The advent of Alcoholics Anonymous brought the first widely accepted hope for alcoholics. The treatment process that developed utilized the principles of Alcoholics Anonymous in a setting of shared recovery which has been difficult to quantify. In 1970 the allocation of federal funds for treatment and research brought the involvement of new disciplines creating both conflicts and possibilities. Alcoholism recovery has elucidated the connection of mind, body, and spirit.

Recovery from alcohol problems without treatment is a subject that has received little attention in the addictions literature. This study used media advertising to solicit people who had resolved a drinking problem on their own. The research used a semi-structured interview as a basis for a qualitative analysis of the phenomenon of recovery without treatment. Attention was focused on reasons offered for resolving a problem and factors involved in maintenance of that resolution. Rccovery methods used by these subjects are compared with those commonly recommended by most treatment programs, including Alcoholics Anonymous. The issues of labelling and acceptance are also discussed.

The purpose of the study was to test for theoretic trends of guilt' shame (quadratic) and defensiveness (linear) across treatment for alcoholic patients. The study sample consisted of91 participants. These included adult males who were in either outpatient or inpatient treatment for alcoholism.All participants were administered a questionnaire comprised of guilt, shame, and defensiveness measures. The questionnaires also asked self rating questions as a measure of progress and had a therapist section for progress ratings. Three null hypotheses were tested using trend analysis. Two way analyses of variances were also used to examine progress variables.ResultsIt was hypothesized that guilt and shame would demonstrate quadratic relationships across treatment and that defensiveness would demonstrate a linear relationship. None of the trend 2analyses demonstrated the expected relationships. The post-hoc two-way analyses of differences in guilt, shame, and defensiveness as a function of both weeks-in-treatment and progress measures yielded few significant results.Conclusions The results of this study do not support some of the major premises concerning treatment of the alcoholic patient. Research needs include operationally defining treatment approaches and refining concepts and measures.

The purpose of this study was to gather insights from recovering (from addictions) Adult Children of Alcoholics (ACA) at Pathway to Recovery, Inc., Indianapolis, IN, who have participated in a pilot family addictions treatment group. This research gives evidence on the group counseling support that ACA group members reported they received and/or determined they needed to receive regarding: their (1) addictions and recovery; (2) specific family of origin roles; and (3) basic ACA issues; as well as information needed to continue living a healthy, productive, drug-free life-style, and; the assistance counselors/therapists could give in this endeavor.A case study methodology was used for this research. Eight informants were purposively selected from a total of 45 ACA group members. Three interviews were conducted with each of the eight informants. Retrospective, structured, and semi-structured interviews, as described by Fraenkel and Wallen (1996) were utilized. Since an interview guide was unavailable for use in this study, one was created.The following four conclusions were drawn from the results of this study:1. The evidence collected suggests that the eight informants benefited from the three-pronged (addictions, ACA issues, specific family role) Recovery, Inc.2. The evidence (detailed in Chapter Four) indicated that many of the actions described herein assisted informants after leaving the ACA group and that the group was responsible for many of the positive changes.3. Accepting oneself was identified most often as the step needed to achieve and maintain sobriety.4. Learning about family issues and studying childhood behaviors may be painful as well as helpful.Results of this study were not generalized beyond these eight informants. However, this study contributed new information to the field of addictions and family roles research by describing the actions, behaviors, thoughts and feelings of these eight informants.

This study examined the correlation between the attrition rate of black female alcoholics in treatment and several personality and psycho!ophysiological variables. Subjects of this study were enrolled in a 28-day treatment program in a local county treatment center with any DMS III diagnosis of alcoholism. Data were collected through a secondary analysis of the records of black female alcoholic patients who entered treatment between December 1982 and December 1985. Of the three hypotheses examined, only one was significant. Results of this study indicated that those women who terminated treatment prematurely were different from those who completed treatment in terms of age and onset of drinking behavior. A Fisher's Exact Test revealed a significant relationship between the two groups regarding use of other drugs. Those women who failed to complete treatment were more likely to be multiple drug users. The remaining variables were found not to be significant.

ENGLISH ABSTRACT: Two recent local studies of relapse among individuals who had attended inpatient alcohol rehabilitation programmes in the Western Cape found relapse rates of up to 60%. A high incidence of comorbid anxiety, low self-efficacy and avoidant coping style were principal reasons cited for relapse. The following literary review was undertaken in an effort at better understanding current findings on the comorbid relationship between alcohol abuse/dependency and anxiety, and on dysfunctional coping styles and relapse. It also reviews current literature and theory concerning the treatment of alcoholics using the Relapse Prevention (RP) model of therapy. Based on these findings, recommendations are made for the application of RP to the clinical psychology groups run for alcohol abusing/dependent inpatients at Neuro Clinic D, Stikland Hospital, Western Cape. Relapse Prevention Therapy was selected because of its integrated approach to addressing both substance abuse and the inadequate coping styles that often render people vulnerable to anxiety, depression and relapse. It was also chosen because of the more constructive, less punitive approach it takes to substance dependence/abuse and the issue of lapses and relapse. The recommendations made in this review should not, in any way, be seen as criticism of the existing programme at Neuro Clinic D. They are, essentially, the individual reflections of the author based on the four months he spent conducting clinical psychology groups in the unit and the findings of two local studies that looked at some of the reasons for relapse following treatment in this and other local facilities. The specific focus on the groups run by clinical psychologists should also not be seen as ignoring the important and valuable work done by other professionals in the unit; notably those in psychiatry, nursing, social work, occupational therapy and pastoral care. On the contrary, information gathered by these professionals is vital to the team effort of rehabilitation, and the identification of psychosocial stressors and cognitive patterns that place people at risk of relapse.
AFRIKAANSE OPSOMMING: Twee onlangse plaaslike studies van terugvalonder pasiënte wat binnepasiëntalkoholrehabilitasieprogramme in die Weskaap bygewoon het, het terugvalkoerse van tot 60% gerapporteer. 'n Hoë voorkoms van komorbiede angs, lae sin van self-vermoë en 'n vermydende streshanteringstyl was die hoofredes aangevoer vir die terugval. Die volgende literatuur-oorsig is onderneem in 'n poging tot 'n beter begrip van huidige bevindinge oor die komorbiede verhouding tussen alkoholmisbruik/afhanklikheid en angs, en oor wanfunksionele streshanteringstyle en terugval. Die oorsig beskou ook huidige literatuur en teorie aangaande die behandeling van alkoholiste deur middel van die Relapse Prevention (RP) model (Terugvalvoorkomingsmodel) van terapie. Op grond van hierdie bevindinge word aanbevelings gemaak VIr die toepassing van RP op die kliniese-sielkundegroepe aangebied VIr alkoholmisbruikende/afhanklike binnepasiënte by Neurokliniek D, Stiklandhospitaal, Weskaap. RP is gekies op grond van sy geïntegreerde benadering tot beide substansmisbruik en die onvoldoende streshanteringstyle wat dikwels mense kwesbaar maak vir angs, depressie en terugval. Die model is ook gekies as gevolg van die meer konstruktiewe, minder strafgerigte benadering tot substansafhanklikheid/misbruik en tot val en terugval. Die aanbevelings in hierdie oorsig moet in geen opsig beskou word as kritiek op die bestaande programme in Neurokliniek D nie. Hulle is, in wese, die individuele gevolgtrekkings van die skrywer gebaseer op sy vier maande ondervinding met sielkundegroepe in die eenheid en op die bevindinge van twee plaaslike studies wat ondersoek ingestel het na sommige van die redes vir terugval na behandeling in hierdie en ander plaaslike fasiliteite. Research targeting the chemically dependent woman has received little attention, even though the research indicates a convergence of male and female drinking norms. Research centered on the special issues of the chemically dependent woman also needs to be extended beyond the scope of actual alcohol and drug abuse and resulting treatment to include information on what issues women face in their recovery process. The findings would help treatment providers design more successful interventions for this population. This study used qualitative methods to investigate issues that 12 chemically dependent women, ranging in actual time in recovery from first to third year post inpatient treatment, discussed as part of a 16 week therapy group. Their recorded responses were transcribed and analyzed, using Glaser and Strauss' methods of comparative analysis, comparing the women between three groups desiginated by the divisions of first, second, and third year post inpatient treatment. The findings indicate that all share many of the same issues, however there are marked differences between the groups. All the women had difficulties with intra- and interrelationships, finding it difficult to maintain a healthy recovery in spite of the problems they confronted in experiencing reality without mind-altering substances. Many issues were influenced by the subjects' family of origin history and sex-role orientation. Conflicts in role obligations resulting in work, family, parenting, and relationship problems surfaced. All the women were aware of additional substance and compulsive dependencies that they would like to eliminate; however, avoiding relapse of their alcohol/drug addiction was the major concern for most. The findings reveal that the longer women spent time actively undertaking a concerted program of recovery, the more they experienced integration into the rest of society, and that the acquisition of life skills and resolution of the past were important factors to the success of this integration. The categories and theme issues that emerged from the analysis have implications for social work practice, policy, and further research.

The Navy provides inpatient treatment for diagnosed alcoholics. Replacing some of the inpatient treatment with outpatient treatment is feasible, especially when treatment matching occurs. The major objective for using outpatient treatment is to capture considerable savings while maintaining treatment effectiveness. The results indicate that the Navy should be able to implement an outpatient treatment model for early stage alcoholics and maintain a success rate comparable to the inpatient treatment model. This would reflect a savings in program costs and replacement costs

ENGLISH ABSTRACT: Alcohol is the substance that people throughout the world abuse most often. South Africa has one of the highest alcohol use level with more than 30% of the population struggling with alcohol-related problems (Telesure, 2010; Nevid et al., 2008: 297; Pisa et al., 2010:4). Statistics showed that approximately 17,5 million South Africans are affected by alcohol, but many more South Africans’ lives are completely disrupted through constant, direct daily contact with the individual abusing alcohol like a parent or life partner (Telesure, 2010). Due to this, alcohol dependency is classified as a family illness in the medical circles and by society. Alcohol dependency can tear families apart and disrupt family dynamics and –processes including rules, rituals, routines, communication, social life, finances, homeostasis and family roles (Stepping Stones, 2013). Alcohol dependency has a great impact on the functioning and existence of a family and to restore the damage alcohol dependency creates in a family, the family should play an important role in the rehabilitation process of the alcohol dependent. This study, therefore, had the goal to determine the role of a social worker in a rehabilitation centre for alcohol dependents from an ecological perspective. A combination of quantitative and qualitative research was used in this study. Furthermore, an exploratory and descriptive research design was used during the research because the available literature showed a lack of information concerning the role of the social worker in a rehabilitation centre for alcohol dependents. A purposeful sample, namely probability sampling, was used to identify participants. The researcher could, therefore, choose participants who qualified for inclusion. The data was collected using a semi-structured questionnaire during 23 individual interviews. An overview of the participants’ beliefs and opinions concerning the subject of the study was gained. The questionnaire consisted of open- and closed questions and was based on information obtained from the literature study. Data from the semi-structured questionnaire was processed and analysed and the results of this empirical study were used as basis for the conclusion and recommendations made in this study. The main findings showed that there is a need for family counselling during a rehabilitation programme for alcohol dependents since there is a definite lack of involvement of the family during the rehabilitation process for alcohol dependents. Further findings showed that rehabilitation programmes mainly provide individual counselling to alcohol dependents and minimal focus and attention is given to the family and after care services. The most important recommendation of this study is that social workers should use the ecological perspective when providing services to alcohol dependents. This perspective can ensure that the necessary services are delivered to effectively address alcohol dependents during a rehabilitation process. Recommendations also showed that social workers should involve the family more during a rehabilitation programme since the support of a family plays an important role during the rehabilitation process.
AFRIKAANSE OPSOMMING: Wêreldwyd is alkohol die middel wat deur die meeste misbruik word en Suid-Afrika het een van die hoogste alkohol inname syfers, met meer as 30% van die bevolking wat sukkel met alkohol-verwante probleme (Telesure, 2010; Nevid et al., 2008: 297; Pisa et al., 2010:4). Statistiek dui daarop dat sowat 17,5 miljoen Suid-Afrikaners deur die inname van alkohol geaffekteer word, maar baie meer Suid-Afrikaners se lewens word egter totaal ontwrig weens konstante, direkte daaglikse kontak met die individu wat alkohol misbruik soos byvoorbeeld ’n ouer of lewensmaat (Telesure, 2010). Weens hierdie spesifieke rede word alkohol-afhanklikheid in mediese kringe en in die samelewing as ’n gesinsiekte geklassifiseer. Alkohol-afhanklikheid kan gesinne uitmekaar skeur en veroorsaak dat die gesinsdinamika en –prosesse, soos byvoorbeeld die reëls, rituele, roetines, kommunikasie, sosiale lewe, finansies, homeostase en gesinsrolle, totaal ontwrig word (Stepping Stones, 2013). Alkohol-afhanklikheid het sodoende ’n geweldige impak op die funksionering en voortbestaan van ’n gesin, en om die skade wat alkohol-afhanklikheid in ’n gesin veroorsaak te herstel, behoort die gesin ’n belangrike rol te speel in die rehabilitasieproses van die alkohol-afhanklike. Hierdie studie het daarom ten doel gehad om die rol van ’n maatskaplike werker in ’n rehabilitaisiesentrum vir alkohol-afhanklikes vanuit ’n ekologiese perspektief te bepaal. ’n Kombinasie van kwantitatiewe en kwalitatiewe navorsingsbenaderings is in hierdie studie benut. Verder is daar tydens hierdie ondersoek ’n verkennende en beskrywende navorsingsonderwerp gebruik, aangesien die beskikbare literatuur ’n gebrek aan inligting rakende die rol van ’n maatskaplike werker in ’n rehabilitasiesentrum vir alkohol-afhanklikes toon. America is aging, older alcoholic numbers rise. Disagreement has long existed as to whether elderly should be treated within traditional programs or in special age-specific modalities. Few studies measure variables selected to show either need or efficacy of elderly specific programs. This study was designed to answer the question: What is the perception of certified alcohol and other drug abuse counselors regarding the best way to treat the elderly, mainstream or elderly specific? The study is based on a bias that addiction counselors have been delivering services to elderly for years in both program types and offer valuable perceptions on the question of treatment effectiveness. A random sample of 788 counselors from a nationwide population of over 25,000 was persistently solicited to participate in a survey addressing the treatment of alcoholics 55 years and older. The most common education level was a master degree; length of experience 9-15 years; age 46. The study analyzed data from the 574 returns, a 72.8% response rate, of a lengthy questionnaire previously validated by gerontology and alcoholism experts. Items selected as variables were clustered into three domains which indicated tendencies toward (a) choice of either traditional or special age-specific programming as most effective, (b) cognizance of elderly and alcohol treatment issues, and (c) willingness to counsel the elderly. On the main research question, according to domain scores, there was no statistical significance in counselor-perceived efficacy of either mainstream or elderly specific treatment of elderly alcoholics. The literature body also is ambiguous on the question. Amount of education predicted degree of gerontology knowledge. Greater gerontological acumen predicted preference (perceived greater effectiveness) for elderly specific programming. Females had more gerontologic knowledge than males. Gender predicted willingness to counsel the elderly; males were more willing than females. Counselor training should include more gerontology studies. Foci and loci for future research were identified with suggestions for treatment planning. Appendices include the questionnaire, item response frequencies, and an extensive annotated bibliography specifically devoted to elderly alcoholic treatment issues.

Purpose This study was designed to determine (1) if a correlation existed between the clients' perceptions of their early relationships with their parents and their current tendencies toward alcohol abuse, and (2) the effects, if any, of a psycho-educational treatment approach on the clients' alcoholic behaviors. Methodology The design used in this study was a pretest/posttest control group design. The pretest/posttest instruments consisted of the LIPHE, COPE, ALCADD and counselors checklists which were administered in group form to both the experimental and control clients. The study compared two randomly formed treatment groups. The experimental group was exposed to weekly psycho-educational group experiences while the control group was provided basic therapy experiences. The groups were com posed of sixteen male clients enrolled in a public alcoholism center. Conclusions 1. There was no relationship between the child of the alcoholic's perception of his parent(s) behavior or feeling and his own alcoholic behavior. 2. Significant reduction in the alcoholic's preference for the use of denial as a coping mechanism did not necessarily result from insight into his own or that of his alcoholic parents behavior. 3. Factors indirectly related to early childhood experiences i.e. personality, individual perception, ability to cope with stress, etc. may be crucial to the development and maintenance of high levels of emotionality and tendency toward alcoholic behaviors in alcoholics. 4. There were no significant improvements in the group's behavior relative to their overall responses to day-to-day life tasks functioning. However, when the groups were compared relative to their actual and expected direction of improvement, task by task, the group exposed to the psycho-educational group experience showed significant improvement on seven of the eight life tasks as compared to two of the eight for the group exposed to a basic therapy experience as observed by the counselors.

It has been argued that individuals receiving traditional alcohol treatment do not necessarily perceive life-long abstinence from alcohol as a favorable treatment outcome, and that negative expectations associated with this abstinence goal may have an adverse effect on treatment outcome. However, "abstinence expectancies" have never been systematically explored. This study used the Theory of Planned Behavior to investigate the relationship between the abstinence outcome expectancies of alcoholics beginning treatment and subsequent alcohol consumption. The independent and combined effects of abstinence outcome expectancies, alcohol outcome expectancies, self-efficacy expectancies (to abstain from alcohol use), and the normative beliefs of individuals beginning inpatient abstinence-oriented alcohol treatment were related to level of drinking during the 90 days following treatment. One hundred ten individuals receiving inpatient alcohol treatment were recruited for the main portion of this study. A questionnaire that included belief-based measures of attitude toward alcohol and abstinence, a belief-based measure of social normative pressure to either use or abstain from alcohol, a belief-based measure of one's perceived behavioral control to abstain from alcohol, and a measure of behavioral intention to use alcohol during the 3 months following treatment was developed for use in this study. The questionnaire was administered to all subjects. During the 90-day Follow-Up period, subjects were sent brief questionnaires and asked to report any alcohol or drug use. Eighty-nine percent of the subjects provided follow-up information for the first 30 days, while 76% provided information for the entire 90 days. An analysis of the data indicated that scores obtained from the belief-based measure of perceived behavioral control and scores from the belief-based measure of attitude toward abstinence were moderately correlated with intention to abstain from alcohol, while alcohol attitude scores and subjective norm scores were uncorrelated. Contrary to expectations, scores obtained from a measure of intention to use alcohol and the measure of perceived behavioral control were minimally predictive of scores from follow-up measures of drinking. However, intention and perceived behavioral control were minimally predictive of scores from follow-up measures of drinking. However, intention and perceived behavioral control scores were somewhat more predictive of drug use for the 90-day Follow-Up period. These results were discussed in light of the Theory of Planned Behavior and the similarities between alcohol expectancies and drug expectancies.

ENGLISH ABSTRACT: The community of the Western Cape carries the burden of the serious consequences of alcohol addiction. Alcohol abuse is very common and the consequences range from severe to devastating, not just for the individual but also for the family and the community as a whole. If answers are sought within the community, the problem deepens, and it becomes apparent why the problem of alcohol abuse is not being successfully addressed. The addict refuses treatment, the community respects the autonomy of the individual, and the problem drags on. The community also has many misperceptions concerning the causes and perpetuation of the pathological drinking behaviour of the addict, and are thus not equipped with the knowledge necessary to suggest the correct interventions for this physical and psychological illness. There is also concern that a person's autonomy and human rights are so highly regarded in the community that the appropriate treatment necessary for this serious disorder of addiction does not receive the attention it deserves. This paper investigates the causes of alcoholism and the factors which reinforce a person's drinking behaviour. The autonomy of the alcoholic is challenged and examined in depth. The importance currently assigned to autonomy and individual rights is questioned, and balanced against other important moral and ethical principles of our time. Alcohol is a drug which causes physical and psychological addiction. Addiction literally means "under the control" of something. Alcohol use is a socially acceptable habit. The psychotropic (calming) effect of alcohol serves as an effective support in or escape from stress in the life of the individual. There are also "vulnerable" individuals in the community in whom a genetic predisposition increases the chance of the development of alcohol dependence. In spite of the fact that alcohol dependence is an acquired physical condition, nobody intentionally becomes addicted to alcohol. With the knowledge of the power that addiction exercises over the life of the individual, attention is now given to the autonomy of the addict, and his/her capacity for rational decision making. The significance of the decision to request treatment for the individual, his/her family and the community is balanced against competency to take the decision. Argument is developed towards the conclusion that the alcoholic is indeed not autonomous, and does not have the competency to make decisions concerning treatment. If the autonomy of the addict is thus questioned, the way in which the person is then treated by the community becomes a difficult moral dilemma. The community's responsibility of care towards the individual and the wider community are jeopardized. The ethical principles of deontology (rules), utilitarianism (the best result for the greatest number), autonomy versus beneficence, solicitude, virtue, human rights and other principles are discussed in depth. A solution is sought that will eventually be "good" for the addict and the community. The conclusion is reached that it is "good" to intervene in the life of the addict at a certain stage of addiction. Involuntary treatment is suggested as one possible way of attacking the problem of serious alcohol abuse that is threatening to overwhelm the community. Practical suggestions are offered for the renewed application of existing treatment structures and legislation to the benefit of the addict and the community.
AFRIKAANSE OPSOMMING: Die gemeenskap in die Wes-Kaap gaan gebuk onder die ernstige gevolge wat alkoholverslaafdheid meebring. Alkoholmisbruik is baie algemeen en het ernstige tot vernietigende gevolge, nie net vir die induvidu nie, maar ook vir die gesin en die gemeenskap as geheel. As daar na antwoorde vir hierdie probleem in die gemeenskap gesoek word, verdiep die probleem en kom dit duidelik aan die lig waarom die probleem van alkoholmisbruik nie suksesvol aangespreek kan word nie. Titled "Children of Addicted Parents", the purpose of this study was to evaluate the effectiveness of a psycho-educational group intervention for latency age children of alcoholics. The treatment group consisted of eleven children between the ages of eight and twelve, who have lived or who are living with an alcoholic parent or stepparent. Referrals were obtained from elementary school guidance counsellors, Family Service Agencies, Alcohol and Drug Programs and the Ministry of Social Services and Housing. The children and three therapists met one hour a week for nine weeks. The group's dual objectives include educating the children about alcoholism/drug addiction and its effect on the family as well as enhancing the childrens' inherent strengths by teaching strategies to increase self-esteem, problem solving and other coping skills. The program's objectives were carried out through brief lectures, discussion, film, art and therapeutic games. The treatment model has been evaluated using a quasi-experimental design. There was a significant change in the intensity of behaviour problems with a trend for a decrease in the number of problems, as well as a decrease in depression. In addition there was a tendency for an increase in self-esteem.

This dissertation investigates compulsory care of alcohol abusers in Sweden during the years 1940 to 1981. The purposes of the dissertation are twofold: in part to determine the concrete forms which care services for alcohol abusers took during the years focused on, in part to analyze what connections existed between the development of services and conceptions of the reasons for, consequences of and possible solutions to alcohol abuse. One point of departure is that the problem was defined with respect to the interests of influential social actors, and with respect to a very particular view of what a respectable life (free of social problems) was. Among the most influential actors involved in these social services, I argue for a focus not least on so-called “street-level bureaucrats”, with direct influence on the goals and methods of institutional care. The study is thus oriented towards mapping the development of problem definitions and formulation within the praxis of compulsory care in four institutionalized care establishments for alcohol abusers. This development is contrasted to broader trends of institutional and discursive development in the definition of alcohol abuse as a social problem. The legislation regulating compulsory care has constantly been founded upon assumptions of the social damage caused by alcohol abuse. At the level of concepts or discourse the consequences of causes for and solutions to alcohol abuse were initially defined in terms of individual morality, with definitions subsequently developing so as to depart from more medicalized terminology. Towards the end of the period the problem descriptions became focused on societal dysfunctions and reforms as the respective causes of and solutions to societally problematic alcohol abuse. At the level of treatment focused upon in the dissertation, societal explanations of alcohol problems departing from societal dysfunctions as causes thereof, and societal reforms as solutions, have never been fully integrated in care services praxis. This was not the case for the simple reason that these care activities, as such, were developed to deal with individuals rather than with society. Neither did a medicalized perspective come to dominate institutionalized care during the period studied – something which can be explained not least with the fact that the perspective’s expansion was not attended by development of medical treatment methods which were convincing with respect to results of use. On one hand, concretely practiced compulsory care thus long remained dominated by problem definitions departing from inmates’ gender-specific moral qualities. On the other hand, certain elements of a more resource-oriented and societal-reformist perspective can certainly be distinguished in the development of care services, albeit on the special terms associated with service implementation in the field. In conclusion, the historical development of care services for alcohol abusers shows that alcohol abuse need not necessarily, or primarily, be seen as a problem having to do with individuals’ relationship with alcohol. Other definitions of the problem have focused upon individuals’ relationship also to working life, the family, sexual morals, the gender order, or capitalist oppression. The problem has been seen as a workers’ and poverty problem, a problem of families and violence, a medical problem, or a symptom of societal problems. Causes have been sought in the character of individuals, the ways in which they have been raised or not raised, their spiritual life, their metabolism, their genetic material, their socioeconomic environment, gender and family situation. The proposed solutions have included everything from work, organized coffee breaks, medicines, psychotherapy and democracy to piece-rate wages, no wages, collective care, or solitary confinement. Alcohol itself has been a secondary factor in the problem definitions which have let themselves be attached – either via perceived links of cause or of effect – to more overarching social issues.

This Final Project is an exploration of the lived experience of dynamics of the personal will of substance-abusing individuals going through the process of recovery in AA 12-Step self-help groups. The study also investigates the dynamic of the personal will interfacing with the will of a perceived higher power. The research context is within the paradigm of Alcoholics Anonymous self-help groups and their 12-step program. Most research in this field is related to predicting or measuring outcome of treatment in 12-step facilities and understanding the motivation for treatment among individuals with Substance Use Disorder. Few studies are found on the subject of the lived experience of the personal will prior to recovery, during recovery and into late recovery in the context of AA and the 12-step program. Twelve-step groups are available worldwide, in some countries it is the only type of free self-help group available within which recovery processes take place. There is a need for an enhanced understanding of the psychological mechanisms involved with the dynamics of the personal will in the process of recovery in the field of 12-step facilitation and in the field of addiction treatment generally. I chose qualitative research methods and the data were collected through semi-structured interviews. The participants were eight active members of AA with a sobriety time range of 2-17 years, with a mean length of 7.4 years. The analysis and description of the data were conducted using Interpretative Phenomenological Analysis. The results reveal the Will Complex in Substance Dependence (WCSD) of the addicted personality as multilayered, compensatory selves of destructive intents that correspond to the undifferentiated term ‘self-will’ in the AA literature. In the continuum of recovery, the participants unanimously name the appearance of a qualitatively different and constructive aspect of the personal will emerging into conscious awareness at the point of ‘hitting bottom’, as an authentic intent or Core Will. These aspects of the personal will are described in all eight cases to exist in parallel, constituting internal conflict negotiated until the point of surrender to a perceived higher power. From the point of surrender, the core will is described to be in correspondence with the experienced higher will, whilst the aspects of the will WCSD recede or withdraw. The dynamics of the will are concomitant with a shift in belief system and consequently in intents. These paradoxical findings are discussed and interpreted through the theoretical lens of transpersonal psychology. Building on existing knowledge, this study can contribute to deciphering the psychological mechanisms behind the fluctuations of the personal will of substance-dependent individuals in treatment, which in turn can be used to enhance therapeutic interventions in the treatment of Substance Use Disorder. Suggestions for future research include a focus on further exploration into the proposed dynamic and the therapeutic effect of increased consciousness of one’s internal dynamics and their effect on choice alternatives. The conditions leading to the emergence of the ‘core will’ might also be explored to understand how we can facilitate the development of this will aspect to achieve constructive intentionality. The clinical significance of the study is potentially broad as the underlying psychological dynamic of the will of the substance-addicted individual can be incorporated into a variety of treatment approaches from different philosophical standings. This study is also significant and a helpful tool for psychotherapists working in rehabilitation facilities or in private practice with substance-dependent clients. For transpersonal psychotherapists this investigation will offer new knowledge, theory and competence to the complex field of Substance Use Disorder treatment.

The purpose of this qualitative study was to determine from the treated students' perspective what teacher behaviors facilitate recovery and what teacher behaviors inhibit recovery. Study participants included 16 high school students who had completed treatment for alcohol and drug dependency in grades 9-12, and 112 teachers. Students and teachers were interviewed to better explain the phenomenon from their perspectives. Data gathered from student interviews, student journal recordings, observer notes, and teacher interviews were used as corroborating data. The constant comparative method was used to analyze data (Glaser & Strauss, 1967).Five major themes emerged from the data analysis explaining the effects of teacher behavior on students who had completed treatment for drug and alcohol dependency. The data were segmented into themes using a bi-polar structure. These themes were: (1) student performance; (2) student communication; (3) student relations: security; (4) student: affective; and, (5) authority: student behavior. These themes yielded student identified causal conditions, intervening conditions, and action/reaction strategies that either inhibited or facilitated recovery.The data showed students did not have neutral feelings; they had either negative or positive feelings about their personal interactions with teachers or their observations of teachers' interaction with other teachers and/or other students. Teacher behaviors which resulted in negative student behavior consequences in each of the five major theme categories were not helpful to students and inhibited their recovery. Teacher behaviors which resulted in positive student behavior consequences in each of the five major theme categories helped students and facilitated their recovery.

The aim of this research was to develop guidelines for after-care services for rehabilitated alcoholics, in order to render a more effective support service towards the rehabilitated alcoholic on his way to full recovery. The necessity of the research project originated on account of a lack of knowledge among social workers on the need of the rehabilitated alcoholic towards after-care service and the specific nature that after-care services have to contain. A literature overview of after-care services was given to the alcoholic, as a background for the empirical study of the project. The literature study showed that after-care services as an integral part of the total rehabilitation of the alcoholic is conceptualized. Further out of the literature came to light that social workers have a resistance against the delivering of after-care services and that they experience feelings of discourage regarding the alcoholic. The empirical study confirmed that social workers experience resistance against giving after-care services to rehabilitated alcoholics. Further they don't consider this as part of their task and they give a low priority towards the rendering of after-care services. In spite of this, the necessity of after-care services is being realized by the social workers. Alcoholics who are standing on the point of being discharged from the rehabilitation center have a need for after-care services to help them adapt in the community. They especially have a need for individual contact with a professional person and do not want to be part of only a self-help group. Among the alcoholics who are already functioning in the community after treatment, the particular need of the female alcoholic came forward. This group feels that their family and spouse do not truly have an idea of their needs. Further there was a need among the rehabilitated alcoholics to know who will be responsible for their after-care services. This group would have wanted to be prepared on possible risky situations and ways of handling these situations. The research showed that there is a shortcoming in the practice concerning the rendering of aftercare service. Guidelines were formulated from which more effective after-care services could be given to rehabilitated alcoholics.

Alcoholics Anonymous (AA) continues to be the most accepted approach for the treatment of addictions in the United States. However, due to recent evidence questioning the effectiveness of AA, the need for alternative approaches to the treatment of addictions has become clear. The following research addresses the efficacy of one such alternative, Rational Recovery (RR). Node-Link Mapping (NLM), a graphic communication technique which uses links and nodes as building blocs to facilitate and enhance communication of information as well as awareness in a counseling environment, was implemented to enhance the recovery process. Three groups of ten (10), chemically dependent, adjudicated subjects were exposed to three different treatment approaches at an outpatient counseling center. The Experimental group received RR with NLM, the Comparison group was exposed only to RR, and the Control group continued in treatment according to the protocol of the counseling agency. All subjects were given the Substance Abuse Subtle Screening Inventory-2 (SASSI-2) as a measurement of symptoms associated with chemical dependency. The subjects were also administered the Rotter Internal-External Locus of Control Scale (Rotter I-E Scale) to determine locus of control prior to treatment and any change after treatment.

Motivational Interviewing (MI) is an evidence-based, directive, client-centered therapy designed to develop discrepancy and resolve ambivalence by eliciting and reinforcing client Change Talk. However, the exact link between the process engaged in during MI and outcome is only starting to be uncovered. The present thesis has replicated and expanded on the current knowledge of the relationship between Therapist and Client Behaviours during a MI-based intervention (Motivational Enhancement Therapy; MET) and outcome, and has provided support for the emergent theory of the inner workings of MI. This was achieved by coding 106 audiotaped MET sessions primarily by the methods outlined in the Motivational Interviewing Skill Code Version 2.0. Data was drawn from 28 participants who received 3-4 sessions of MET within the context of a randomised controlled trial for mild-moderate alcohol dependence at the Community Alcohol and Drug Service of Christchurch. Therapist and Client Behaviours were analysed within sessions (categorised into Early, Mid, or End Intervals) and across sessions, and compared with whether the client had drank within national drinking guidelines during the 6-months after MET (Controlled Drinkers). In terms of Client Behaviours during MET it was found that Uncontrolled Drinkers (compared with Controlled Drinkers) uttered a significantly higher frequency of Sustain Talk, lower Ability Language strength (over all MET and during End Intervals), and lower Commitment Language strength (during Session 2 and 4, and change over MET). Giving Information was the only Therapist Behaviour where significant differences were observed over all MET, with a higher frequency given to the Uncontrolled Drinkers. However, during End Intervals within MET Sessions, Controlled Drinkers received a significantly higher frequency of Advise without Permission and a lower frequency of Emphasise Control statements. In most instances MI-Consistent Therapist Behaviours were associated with higher strength of Ability and Commitment Language, and a lower frequency of Sustain Talk. MI-Inconsistent Therapist Behaviour, Direct, was associated with lower Client Language strength. Limitations to these results include small sample, limited ability to make inferences about causality, coder biases, and uneven reliability. However, this exploratory study was unique in investigating the relationship between Therapist Behaviours and the strength of Client Language, and in examining these factors within and across multiple sessions, and has produced a number of potentially valuable findings that warrant further investigation.

Alkoholvården i Sverige kan vara utformad på många olika och skilda sätt. Det är ett område där det finns ett flertal olika åsikter kring vilken behandling som är lämpligast eller effektivast. Uppsatsens syfte är att titta närmare på hur diskursen kring alkoholvård kan se ut i Sverige idag genom närmare granskning av de två tidskrifterna Socionomen och Alkohol & Narkotika. Två stora tidningar inom praktiker av socialt arbete. För att uppnå syftet utgår uppsatsen från följande frågeställningar:-Vilka behandlingsmetoder för alkoholmissbruk diskuteras i tidskrifterna?-Hur problematiserar tidskrifterna kring olika behandlingsformer?-Hur lyfts för respektive nackdelar för behandlingsformerna upp?-Vad väljer tidskrifterna att lyfta fram i debatten kring alkoholvården? Som metod används kritisk diskursanalys för att synliggöra diskurserna i tidskrifterna. Analysen av tidskrifterna grundar sig på en analysmodell efter diskursanalytikern Fairclough och används främst för att synliggöra hur tidskrifterna förhåller sig till uppsatsens diskursordning. Diskursordningen som används är baserad på aktuell litteratur kring alkoholmissbruksbehandling och innefattar tre diskurser: Psykologisk, psykosocial och farmakologisk.Resultaten i uppsatsen visar att den psykosociala diskursen är dominerande i de båda tidskrifterna men att även den farmakologiska diskursen nämns. De behandlingsmetoder som tas upp är i de båda tidskrifterna kognitiv beteendeterapi (KBT) och motiverande samtal. Vidare visa uppsatsen att Socionomen efterfrågar en mer strukturerad, organiserad och evidensbaserad alkoholbehandling i allmänhet. Alkohol & Narkotika å andra sidan ifrågasätter den formella vårdapparaten kring alkoholmissbruk och beskriver den som otillgänglig och bristfällig. Som alternativ för att högkonsumenter av alkohol ska få tidig hjälp tar tidskriften upp olika självhjälps metoder grundade på KBT och motiverade samtal.
In Sweden, the treatment of alcoholism can be shaped in many various and separate ways. The subject of area contains various opinions regarding what treatment would be the most correct or effective. The aim in this essay is to look closer at the discourse of how the treatment of alcoholism may work in Sweden today. This will be done by closer examine the magazines, Socionomen and Alkohol & Narkotika, that are large papers within the practice of social work. To achieve the aim, this essay will start out from the following questions of issue:- What methods of treatment are discussed in the two magazines?- How do the magazines questions the various treatments? - How are the advantages and disadvantages in the different treatments highlighted?- What chooses the magazines to highlight in the debate of alcohol treatment?As the method, to make the discourses in the magazines visible, a critical analyze of discourse is being used. The analyze of the magazines is based on a model of analyzing by the discourse analyzer Fairclough, and is mainly used to make the magazines relations to the essay’s system of discourse visible. The system of discourse that is being used is based on actual literature in the area of treatment of alcohol addiction and contains three different discourses: psychological, psychosocial and pharmacological.The conclusion of this essay shows that the psychosocial discourse is the dominating discourse in both of the magazines but that the pharmacological discourse also is mentioned. The methods of treatment being raised in the magazines are cognitive behavioral therapy and motivational interviewing. Further more the essay shows that the paper Socionomen generally asks for a more structured, organized and evidence based treatment of alcoholism. In the other hand, the magazine Alkohol & Narkotika questions the formal care unit’s treatment of alcohol abuse and describes it as inaccessible and insufficient. As an alternative this magazine highlights the methods of self- help based on cognitive behavioral therapy and motivational interviewing so that those that are high consumers of alcohol will get help at an earlier stage of their abuse.

This study aims to verify the readiness to begin a treatment and the effect of a treatment (Minnesota Style) done to alcoholics in a center for treatment in Brazil for those who are drug dependents. In this survey, 25 alcoholic patients were treated. The patients ranged from severe to moderate level of addiction, aged from 23 to 60 years old, male and female who came to be treated at the center for treatment of alcohol dependents. The procedure consisted of first contact with the subjects for an explanation of the aims of the research and the filling in of their informed consent to it. With the aim of identifying the level of readiness to change / motivation of the subjects in the begining, middle, at the end and after the treatment and to evaluate the rate of recovering, the interviews were done in stages. The first one was performed on the very first day of the treatment and consisted of demographic data and the application of the instruments Alcohol Dependence Data Questionaire (SADD) and The Stages Readiness and Treatment Eagerness Scale (SOCRATES). The second interview was done 23 days after the begining of the treatment and the third one at the end. The fourth interview was done a month after the end of the treatment, which was considered the follow-up. Those interviews consisted of the SOCRATES scale. The main results of this study showed that the majority of the participants were predominantly men (80%), white men (60%), married or living in concubinage (60%), having studied only up to the first 4 years of elementary school (60%), without an occupation (60%), with a family income from 2 to 5 minimum wages (40%) and having a serious exposure to the disease (76%). There has been a correlation among the SOCRATES scale factors of ambivalence and acknowledgement, acknowledgement and ambivalence, action and acknowledgement and acknowledgement and action in the entrance to the treatment. Comparing the factors of the scale in the different stages of the treatment, there has been no significant difference, thus having no reduction of the ambivalence. The results made it possible to conclude that there must be a valuation of the patients in a long term basis after the end of the programme so as to have a better notion of their gradual development on the stages, and researches which aim to verify the specific efficacy of the patterns of treatment so as to encourage the motivation for a change and to lower the ambivalence of the patients.
O objetivo desta pesquisa foi verificar a prontid?o para iniciar o tratamento e o efeito de um tratamento (Modelo Minnesota) em alcoolistas em um centro de tratamento para depend?ncia qu?mica, no Brasil. Participaram dessa pesquisa, 25 pacientes alcoolistas, de grau moderado e severo, com idade entre 23 a 60 anos, de ambos os sexos que deram entrada no centro de tratamento de depend?ncia qu?mica. O procedimento realizado constou de contato inicial com os sujeitos para a explana??o dos objetivos da pesquisa e preenchimento do consentimento informado. Com o objetivo de identificar o grau de prontid?o para mudan?a / motiva??o dos sujeitos no in?cio, meio, t?rmino e ap?s o tratamento e avaliar os ?ndices de recupera??o, as entrevistas foram feitas em etapas. A primeira entrevista foi realizada no primeiro dia do tratamento dos sujeitos e constou de dados demogr?ficos, e aplica??o dos Instrumentos Alcohol Dependence Data Questionaire (SADD) e The Stages Readiness and Treatment Eagerness Scale (SOCRATES). A segunda entrevista foi realizada vinte e tr?s dias ap?s o in?cio do tratamento, a terceira no t?rmino do tratamento e a quarta um m?s ap?s o t?rmino do tratamento, que foi considerado o seguimento. Essas entrevistas constaram da Escala SOCRATES. Os principais resultados deste estudo mostraram que os participantes eram predominantemente homens (80%), brancos (60%), casados ou amasiados (60%), do 1? grau (60%), sem ocupa??o (60%), com renda familiar entre 2 a 5 sal?rios m?nimos (40%) e com comprometimento grave da doen?a (76%). Darbe nagrinėjama alkoholio vartojimo problemų turinčių asmenų gyvenimo kokybės kitimas atsižvelgiant į tai, kokius gydimo metodus jie pasirenka šiai problemai spręsti. Didžiausias dėmesys skiriamas AA grupių įtakai gyvenimo kokybei ir jos kitimui. Ši problematika Lietuvoje mažai tyrinėta, tačiau dėl alkoholio vartojimo masto Lietuvoje ir pasaulyje yra pakankamai aktuali. Darbe buvo iškeltas tikslas ištirti AA grupių įtaką alkoholio vartojimo problemų turinčių asmenų gyvenimo kokybės kitimui. Darbo tikslui pasiekti analizuota alkoholio vartojimo problemų turinčių asmenų lankiusių ir nelankiusių anoniminių alkoholikų grupes gyvenimo kokybė, jos kitimas ir vertybių bei gydymo metodo įtaka jai. Manoma, kad alkoholio vartojimo problemų turinčių asmenų, lankiusių anoniminių alkoholikų grupes, gyvenimo kokybė pagerėjo labiau, palyginus su tais kurie tų grupių nelankė arba buvo viena-du kartus ir daugiau nelankė. Darbo tikslui pasiekti naudota PSO gyvenimo kokybės klausimynas (WHOQOL-BREF; angl. The World Health Organization Quality of Life – Bref, 1996), gyvenimo vertybių įvertinimas pagal Goštautą A, Javtoką Z, Žagminą K. (2001) ir gydymo metodo įvertinimas sudarytas pačios darbo autorės. This Paper deals with changing quality of life of the people with alcohol problems, depending on what treatment methods they choose to tackle this problem. The main focus is made on the impact of AA groups to quality of life and the changes thereof. This problem has been little researched in Lithuania; however, due to the consumed alcohol level in Lithuania and world widely this issue is sufficiently relevant. The objective of the Paper is to examine the impact of AA groups to the change of the quality of life of people who have alcohol-consuming problems. For the purpose of the Objective the quality of life and of the people with alcohol-consuming problems who attended AA groups and who not attended AA groups and the change of their quality of life were analysed, as well as the values and the impact of treatment methods on it were explored. It is estimated that the quality of life of the people with alcohol-consuming problems, who attended alcoholics anonymous groups, improved more in comparison with those who did not attend these groups or those visited them once or twice and did not attend them constantly.
