This chapter describes the events surrounding normal and abnormal labour and delivery with particular relevance to the anaesthetist. The first two sections explain the course of a normal labour, delivery, and third stage. Subsequently attention is paid to obstructed labour, delivery, and prolonged third stage. Since induction of labour has become common practice in many pathological conditions, several methods of induction and their complications are then discussed. Historical origins of the field are reviewed and the meaning of a developmental psychopathology perspective is described. Definitional principles and conceptual issues of the field are discussed, and these parameters include (1) the mutual interplay between normality and psychopathology; (2) a multiple-levels-of-analysis and multidisciplinary approach; (3) developmental pathways to psychopathology and resilient functioning; (4) translational research; and (5) prevention and intervention. Examples are derived from research conducted with individuals at high risk for developing psychopathology and those suffering from mental disorders. Future directions and challenges for translating research into practice and policy are proffered. Advances in the knowledge base in developmental psychopathology not only have benefitted the scientific understanding of the relation between normal and abnormal development, but also have contributed to reducing the individual and societal burden of mental illness.

This chapter on primary muscle diseases explains how analysis of compound muscle action potential (CMAP) amplitude, abnormal spontaneous activity on needle electromyography (EMG), and motor unit action potentials (MUAP) characteristics may be used to give an indication of pathophysiological processes, and goes on to describe the combination and distribution of abnormalities that may be expected in the more commonly encountered myopathies. The conditions considered in detail are inflammatory myopathy (including myositis), critical illness myopathy, disorders with myotonia, inherited myopathy (including muscular dystrophy), and endocrine, metabolic and toxic disorders. Each of these has a characteristic combination of CMAP, spontaneous EMG, and MUAP findings, but the systematic approach to clinical neurophysiology as a way of understanding muscle pathophysiology can be used to investigate the myriad of rare myopathies that may be encountered in clinical practice.

As a concept of international law, the right to self-determination is widely renowned for its lack of clarity. Broadly speaking, one can differentiate between a liberal and a nationalist tradition. In modern international law, the balance between these two opposing traditions is sought in an attempt to contain or ‘domesticate’ the nationalist conception by limiting it to ‘abnormal’ situations, that is to colonialism in the sense of ‘alien subjugation, domination and exploitation’. Essentially, this distinction between ‘normal’ and ‘abnormal’ situations has since, the distinction was made, been the heart of the matter in the legal discourse on the right to self-determination, with the important qualification regarding the need to preserve existing borders. This book situates Russia’s approach to the right to self-determination in that discourse by way of a regional comparison vis-à-vis a ‘Western’ or European perspective, and a temporal comparison with the former Soviet doctrine of international law. Against the background of the Soviet Union’s role in the evolution of the right to self-determination, the bulk of the book analyses Russia’s relevant state practice in the post-Soviet space through the prisms of sovereignty, secession, and annexation, illustrated by a total of seven case studies on the conflicts over Abkhazia, Chechnya, Crimea, Nagorno-Karabakh, South Ossetia, Tatarstan, and Transnistria. Complemented by a review of the Russian scholarship on the right to self-determination, it is suggested that Russia’s approach may be best understood not only in terms of power politics disguised as legal rhetoric, but can be seen as evidence of traits of a regional (re-)fragmentation of international law.

The central nervous system (CNS) in relation to dentistry is the topic of this chapter. Nerve conduction is described, then the physiology of facial and dental pain and processing of afferent pain nerve impulses in the cerebral cortex. We discuss abnormal sensations of allodynia and paraesthesia. Pain control with non-steroidal anti-inflammatory drugs or paracetamol is explained. The function of the cranial nerves and the autonomic nervous system are described. We explain the nerve pathways involved in salivation, lachrymation, and taste sensation. We propose some techniques for treating the nervous patient, e.g. modelling, systematic desensitization, and feedback. Effective local anaesthesia is essential in gaining the cooperation of nervous patients. The major types of local anaesthetics are compared. The techniques for inferior alveolar and superior alveolar nerve blocks are described as are drugs commonly used in dental sedation. There are final sections on drug problems encountered in dental practice and on dementia.

Pyelonephritis is a suppurative infection of the kidney most commonly due to bacterial infection that may be either acute or chronic. Acute pyelonephritis is usefully subdivided into uncomplicated and complicated. The term complicated pyelonephritis is associated with an abnormal urinary tract, obstruction, stones, immunocompromise, diabetes, pregnancy, or in practical terms any pyelonephritis resulting in severe illness. The distinction is important principally in terms of type, duration, and place of treatment.The clinical spectrum of acute pyelonephritis ranges from relatively mild illness to pyonephrosis and emphysematous pyelonephritis with a fulminant course and high mortality.The term chronic pyelonephritis has been used to describe kidneys damaged from multiple, recurrent, or partially treated episodes of infection, but for many of these examples ‘reflux nephropathy’ is a better description as the role of infection is not always clear. However chronic or very recurrent infections may be associated with diabetes, abnormal urinary tracts, and any cause of obstructed drainage. Chronic pyelonephritis is also an accurate description for the progressive ongoing destruction of the kidney that occurs in xanthogranulomatous pyelonephritis or malakoplakia.

Radiation considerations are an integral part of the practice of nuclear cardiac imaging. Concern regarding radiation has increased in recent years, reflected in statements by many professional societies, and likely attributable both to rapid growth in use of nuclear cardiology as well as high doses received by some nuclear cardiology patients. The fundamental principles of medical radiological protection are justification (ensuring that the right test is performed for the right patient at the right time), optimization (ensuring that the test is performed in the right manner), and dose limitation, which while applicable to healthcare workers is not operative regarding patients. Three "As" facilitate and serve as an organizing principle for justification: awareness, appropriateness, and audit. Awareness incorporates knowledge of the benefits and risks of testing involving radiation and effective communication of these to the patient. Appropriateness in nuclear cardiology can be assessed using the American College of Cardiology's appropriateness criteria. Methods that have been demonstrated to improve appropriateness include using a collaborative learning model, a point-of-order decision support tool, and a multifaceted intervention including threatened loss of insurance coverage. A variety of strategies should be considered for optimization to ensure patient-centered imaging. These including strategic selection of both the protocol, e.g. selecting a stress-first protocol and performing stress-only imaging in patients without a high pre-test probability of abnormal findings on stress imaging, or using PET, and also the administered activity, e.g. by using weight-based dosing and/or software- or hardware-based advances in camera technology. Special considerations are required for pregnant, nursing, and pediatric patients.

Activation procedures are commonly employed to increase the diagnostic yield of electroencephalography (EEG) in patients with suspected epilepsy. This chapter reviews the effects and utility of hyperventilation, intermittent photic stimulation, and color/pattern stimulation on the EEG in patients with epilepsy and other neurological disorders. In theory, the greater the number of different activation methods used in EEG evaluation of an epilepsy patient, the greater the chance of obtaining abnormal findings. However, the specificity of these findings for epilepsy is uncertain. Furthermore, from a practical point of view, desirable activations are those methods that can be carried out easily and systematically, in a short time frame, with affordable equipment, without undesirable side effects for patients, and with reliable and predictive results. At this time, hyperventilation and intermittent photic stimulation are the most widely used activation methods and have an extensive body of literature supporting them.

Although mutant mouse models are unable to capture the subjective experiences and full complexity of psychiatric disorders, they have nonetheless provided important insights into the pathophysiology of various endophenotypes observed in neuropsychiatric illnesses. These mouse models enable the detailed dissection of cellular, molecular and circuit mechanisms that may underlie the abnormal behaviors observed in OCD, using methods that cannot be applied to human patients for ethical, practical and technical reasons. Mouse models exhibiting repetitive OCD-like behaviors, especially grooming, include the Sapap3-null (Sapap3-/-), Slitrk5-null (Slitrk5-/-), Hoxb8-null (Hoxb8-/-), and Shank3b-null (Shank3b-/-) mouse models. Evidence from these mice implicates a defect in cortico-striatal glutamatergic transmission in compulsive behavior. Moreover, studies using these mice have shown that this impaired cortico-striatal transmission is due to the aberrant expression of silent synapses. These mouse models support the hypothesis that defective glutamatergic neurotransmission in the cortico-striatal-thalamo-cortical circuit underlies OCD.
