ABSTRACT Objectives: The aim of this research was to assess galvanic behavior of lingual orthodontic brackets coupled with representative types of orthodontic wires. Materials and Methods: Three types of lingual brackets: Incognito (INC), In-Ovation L (IOV), and STb (STB) were combined with a stainless steel (SS) and a nickel-titanium (NiTi) orthodontic archwire. All materials were initially investigated by scanning electron microscopy / x-ray energy dispersive spectroscopy (SEM/EDX) while wires were also tested by x-ray diffraction spectroscopy (XRD). All bracket-wire combinations were immersed in acidic 0.1M NaCl 0.1M lactic acid and neutral NaF 0.3% (wt) electrolyte, and the potential differences were continuously recorded for 48 hours. Results: The SEM/EDX analysis revealed that INC is a single-unit bracket made of a high gold (Au) alloy while IOV and STB are two-piece appliances in which the base and wing are made of SS alloys. The SS wire demonstrated austenite and martensite iron phase, while NiTi wire illustrated an intense austenite crystallographic structure with limited martensite. All bracket wire combinations showed potential differences below the threshold of galvanic corrosion (200 mV) except for INC and STB coupled with NiTi wire in NaF media. Conclusions: The electrochemical results indicate that all brackets tested demonstrated galvanic compatibility with SS wire, but fluoride treatment should be used cautiously with NiTi wires coupled with Au and SS brackets.

This study focused on the adsorption characteristics of hazardous heavy metals using reactive inorganic agents in synthetic wastewater. Basic properties of five reactive agents were analyzed by zeta potential, particle size, XRF and SEM. Zeta potential of five adsorbents was 45, 7, 14, -2 and 45 mV, respectively. Average particle size was in the range of 1.4 to 9㎛. By XRF analysis, main components were Na, Al, Si, S and Fe in reactive agents and no great differences in Mg, K, Ca and Ti components. As, Zn, Ni, Cu, Cr, Cd and Pb were used as target heavy metals. Adsorption capacity of Pb and Cu was in the order: TN 315Ny-T3>TN820L>TN315NY.

The transparent β-Si3N4ceramic with a whisker-like microstructure was prepared by hot-pressing at 2000 °C for 26 h, with MgSiN2as an additive. The resultant material achieves the maximum transmittance of 70 % at the wavelength of about 2.5 μm and the transmittance value keeps higher than 60 % in the range of 700-4500 nm wavelength, which is attributed to the very small amount of the intergranular amorphous phase along with high density. The present transparent β-Si3N4ceramic exhibits an indentation fracture toughness of 7.2±0.3 MPa m1/2.

AbstractSyntheses for the three members of the copper hydroxyl nitrate family – the polymorphs rouaite and gerhardtite, and likasite – are presented along with powder diffraction data and unit-cell parameters. The solubilities, determined in 0.05 M KNO3solution after equilibration at 25°C for 10 days were used to calculate activity-based solubility product constants. The Gibbs energies of formation, obtained from the solubility products, are –653.2±0.7 kJ/mol, –655.1±1.2 kJ/mol and –1506.4±1.1 kJ/mol, for rouaite, gerhardtite, and likasite (Cu3NO3(OH)5·2H2O), respectively. The values for the polymorphs rouaite and gerhardtite validate the observations of Oswald that gerhardtite is the most stable polymorph at room temperature and that the preparation of predominantly rouaite in syntheses carried out at room temperature must be due to the metastability and low rate of conversion to the more stable gerhardtite.

Optimization of factors affecting biohydrogen production from the codigestion of crude glycerol and microalgal biomass by anaerobic sludge consortium was conducted. The experiments were designed by a response surface methodology with central composite design. The factors affecting the production of hydrogen were the concentrations of crude glycerol, microalgal biomass, and inoculum. The maximum hydrogen production (655.1 mL-H2/L) was achieved with 13.83 g/L crude glycerol, 23.1 g-VS/L microalgal biomass, and 10.3% (v/v) inoculum. The hydrogenic effluents obtained under low, high, and optimal conditions were further used as substrates for methane production. Methane production rates and methane yield of 868.7 mL-CH4/L and 2.95 mL-CH4/L-h were attained with the effluent produced under optimum conditions. The use of crude glycerol and microalgal biomass as cosubstrates had an antagonistic effect on biohydrogen production and a synergistic effect on methane fermentation. The two-stage process provided a more attractive solution, with a total energy of 1.27 kJ/g-VSadded, than the one-stage process.

Mesoporous metal-organic framework-5 (MOF-5), with the composition Zn4O(BDC)3, showed a high capacity for the adsorptive removal of Pb(II) from 100% aqueous media. After the adsorption process, changes in both morphology and composition were detected using a scanning electron microscope (SEM) equipped with an energy dispersive X-ray (EDX) system, Fourier transform infrared spectroscopy (FTIR), and X-ray photoelectron spectroscopy (XPS) analysis. The experimental evidence showed that Zn(II) liberation from MOF-5 structure was provoked by the water effect demonstrating that Pb(II) removal is not due to ionic exchange with Zn. A kinetic study showed that Pb(II) removal was carried out in 30 min with a behavior of pseudo-second-order kinetic model. The experimental data on Pb(II) adsorption were adequately fit by both the Langmuir and BET isotherm models with maximum adsorption capacities of 658.5 and 412.7 mg/g, respectively, at pH 5 and 45°C. The results of this work demonstrate that the use of MOF-5 has great potential for applications in environmental protection, especially regarding the removal of the lead present in industrial wastewaters and tap waters.

AIM: The study aim is to evaluate the cost-effectiveness of thrombolytic treatment in acute MI comparing with anticoagulants therapy and between each other thrombolytic (SK, r-Pa).MATERIAL AND METHODS: We used a prospective registry of all patients admitted for acute myocardial infarction in intensive care units in Tirana. The average drugs cost was calculated for the hospitalization period in Albanian money (ALL). Survival and life quality were estimated by phone contact 1 year after acute MI.RESULTS: Anticoagulant group cost is 23865.3 ALL (170.5€), SK group cost is 54148.63 ALL (386.7€), r-Pa group has a cost of 92184.90 ALL (658.5€). In the group treated with SK the hospital survival is 100%, while in the control group 88.8%. Reteplase group has a lower period of stay in hospital than SK group 13.04 days vs. 17.97 days, mean age in group treated with r-Pa is 64.29 ± 10.03 approximate with anticoagulant group mean age 64.17 ± 11.08; differ significantly with SK group mean age 56.75 ± 10.04. Survival after 1 year was 96.4% for r-Pa and 96.9% SK.CONCLUSIONS: SK and r-Pa are successful thrombolytics with high effectiveness. It is gained a higher survival with the thrombolytic treatments. Reteplase is well tolerated in older patients than SK, is easier to apply than Streptokinase, but has higher cost.

ABSTRAK Cakupan ASI eksklusif di Puskesmas Sumbersari pada tahun 2011 99%, dengan permasalah volume ASI 20 %. Tiga penyebab kurangnya volume ASI pada ibu menyusui meliputi nutrisi, masalah psikologi, dan hisapan bayi. Untuk terhindar dari masalah gangguan nutrisi, ibu menyusui memerlukan nutrisi lebih, agar jumlah air susu yang dihasilkan cukup. Kejadian ini dapat diantisipasi dengan memberikan madu, karena nutrisi madu dapat mencukupi kebutuhan nutrisi ibu menyusui. Desain penelitian yang digunakan adalah True-Eksperimen.Dimana populasi yang digunakan ibu nifas, dan sampel adalah ibu nifas hari ke-1 sampai hari ke-10 (total sampling). Penelitian ini menggunakan uji t independent. Hasil penelitian didapatkan hasil bahwa nilai nilai rata – rata volume ASI matur hari ke-10 pada ibu dengan pemberian madu 703.5 cc / 24 jam, sedangkan rata – rata volume ASI matur hari ke-10 pada ibu tanpa pemberian madu 658.5 cc / 24 jam. t hitung (3.073) ≥ t tabel (2.101) terdapat Perbedaan Pemberian Madu Dan Tidak Diberi Madu Terhadap Volume Asi MaturIbu Nifas Hari Ke Sepuluh. Disebabkan madu memiliki kandungan nutrisi (energi, karbohidrat, protein) yang memiliki fungsi sebagai tenaga untuk hipofise posterior menghasilkan hormon oksitosin dan prolaktin. Kedua hormon tersebut memiliki peran sebagai pelancar dan produksi air susu. Dan diharapkan ibu menyusui mengkonsumsi madu pada awal nifas hingga bayi tidak menyusu lagi. Abstract Dietary recommendations for managing Parkinson’s disease (PD) can be confusing for people with PD (PwPD) and their informal caregivers (ICGs) who often have the responsibility to buy and prepare foods. PwPD (63-78 years of age) and their ICGs (39-75 years of age) completed two 24-hour dietary recalls and semi-structured, dyadic interviews which were conducted to gather information about dyads’ (n=9) nutrition concerns. Calorie, macro- and micro-nutrient intake were averaged over two days and compared to the National Academy of Sciences’ dietary reference intakes. Independent t-tests and the Mann-Whitney U tests compared PwPD and ICG. Interviews were audio-recorded, transcribed verbatim, and analyzed for preliminary themes. Mean calorie intake was 1766.6±658.5 kcal/d; the majority of calories were from carbohydrates. Calorie and nutrient intake between PwPD and ICG did not differ. All participants were below recommendations for fiber (17.2±7.9g/d) and potassium (2213.3±890.0mg/d) and exceeded recommendations for sodium (2741.2±1396.9mg/d) and added-sugars (59.5 ±38.3g/d). More than half of participants (55.6%) agreed an eating plan to manage PD symptoms is important and that a nutrition consultation would be helpful. Emerging themes from qualitative interviews include: nutrition concerns related to PD symptoms, managing other conditions and PD, perceptions regarding diet quality, impact of diet on PD symptoms, and complementary medicine. In conclusion, there were no differences in dietary intake between PwPD and ICG, and participants presented with lack of adherence to recommended dietary reference intakes. Sub-optimal dietary intake and self-reported nutrition concerns strongly suggest including both nutrition professionals and ICGs when providing care for PwPD.

<p class="abstrak2">This study was carried out to evaluate the sires and dams genetically for milk production and mastitis traits in Egyptian 12 herds of Holstein cattle using Best Linear Unbiased Prediction via MTDFREML program. The data was obtained from a commercial farm called Dena, located in Cairo-Alex Desert Road (80 Km), Menofia, Egypt. Data included 4791 cows, 4227 dams and 248 sires that represented the period from 2007 to 2014. Estimating breeding values for milk production traits as cumulative milk yield at 90 days (90-DM), cumulative milk yield at 180 days (180-DM), cumulative milk yield at 270 days (270-DM), cumulative milk yield at 305 days (305-DM), and number of mastitis infection around the season of lactation (MAST). The averages of the 90-DM, 180-DM, 270-DM, 305-DM and MAST were 3026.3±655.1 kg, 5873.3±1081.1 kg, 7891.1±2692.2 kg, 9611.2±1897.9 kg, and 0.712±1.2 time/parity, respectively. Estimates of heritability for the previous traits were 0.11±0.016, 0.15±0.014, 0.18±0.012, 0.22±0.015, and 0.09±0.029, respectively; genetic variance were 47206.2 kg, 175300.6 kg, 1304654.4 kg, 792411.6 kg and 0.12 time/parity, respectively; and phenotypic variance were 429147.6 kg, 1168670.6 kg, 7248079.9 kg, 3601870.9 kg, and 1.35 time/parity, respectively. The EBV values as average, SD, (Min: Max) for sires were 0.0±0.179 (-0.4: 0.66) for MAST, 0.0±86.176 (-263.1: 245.4) for 90-DM, 0.0±227.523 (-600.3: 800.3) for 180-DM, 0.0±413.48 (-323.3: 1277.7) for 270-DM and 0.0±440.26 (-1280.9: 1565.1) for 305-DM. Also, The EBVs for dams were 0.0±0.055 (-0.14: 0.45) for MAST, 0.033±26.24 (-142.8: 103.0) for 90-DM, 0.074±76.81 (-360.2: 289.6) for 180-DM, -0.045±139.66 (-591.9: 529.2) for 270-DM and 0.266±154.1 (-666.3: 617.6) for 305-DM. These results provide that the selection of sires and dams will improve the traits of milk production and mastitis in this herd because of the wide differences in genetic potential among sires and dams.</p>

Background Healthcare spending in the emergency department (ED) setting has received intense focus from policymakers in the United States (U.S.). Relatively few studies have systematically evaluated ED spending over time or disaggregated ED spending by policy-relevant groups, including health condition, age, sex, and payer to inform these discussions. This study’s objective is to estimate ED spending trends in the U.S. from 2006 to 2016, by age, sex, payer, and across 154 health conditions and assess ED spending per visit over time. Methods and findings This observational study utilized the National Emergency Department Sample, a nationally representative sample of hospital-based ED visits in the U.S. to measure healthcare spending for ED care. All spending estimates were adjusted for inflation and presented in 2016 U.S. Dollars. Overall ED spending was $79.2 billion (CI, $79.2 billion-$79.2 billion) in 2006 and grew to $136.6 billion (CI, $136.6 billion-$136.6 billion) in 2016, representing a population-adjusted annualized rate of change of 4.4% (CI, 4.4%-4.5%) as compared to total healthcare spending (1.4% [CI, 1.4%-1.4%]) during that same ten-year period. The percentage of U.S. health spending attributable to the ED has increased from 3.9% (CI, 3.9%-3.9%) in 2006 to 5.0% (CI, 5.0%-5.0%) in 2016. Nearly equal parts of ED spending in 2016 was paid by private payers (49.3% [CI, 49.3%-49.3%]) and public payers (46.9% [CI, 46.9%-46.9%]), with the remainder attributable to out-of-pocket spending (3.9% [CI, 3.9%-3.9%]). In terms of key groups, the majority of ED spending was allocated among females (versus males) and treat-and-release patients (versus those hospitalized); those between age 20–44 accounted for a plurality of ED spending. Road injuries, falls, and urinary diseases witnessed the highest levels of ED spending, accounting for 14.1% (CI, 13.1%-15.1%) of total ED spending in 2016. ED spending per visit also increased over time from $660.0 (CI, $655.1-$665.2) in 2006 to $943.2 (CI, $934.3-$951.6) in 2016, or at an annualized rate of 3.4% (CI, 3.3%-3.4%). Conclusions Though ED spending accounts for a relatively small portion of total health system spending in the U.S., ED spending is sizable and growing. Understanding which diseases are driving this spending is helpful for informing value-based reforms that can impact overall health care costs.

The aim of this research is to study some functional properties and the antioxidant activity of cherry gum, collected from Serghaya and Suwayda in Syria, and to compare these features with those of Arabic gum. The values of the hydroxyl groups for the Arabic gum, Serghaya and Suwayda cherry gums were 757.1, 655.1 and 564.3 mg KOH/gm, respectively. The solubility of exudate gums ranged from 53.53 to 86.53% and was arranged as follows: Arabic gum>Serghaya cherry gum >Suwayda cherry gum. Gum solubility increased with rising the temperature. Water and oil holding capacities of cherry gums were significantly higher (p<0.05) than those of Arabic gum, while their emulsifying capacity was significantly lower than that of Arabic gum. The antioxidant activity of the gum was evaluated using the DPPH scavenging activity method and reducing power method. The inhibition percentages of DPPH were 51.52%, 23.23% and 21.19% for Serghaya, Suwayda and Arabic gums, respectively. The antioxidant activities of Serghaya and Suwayda gums using the reducing power method were 753.12 and 312.93 meq ascorbic acid/100gm, whereas Arabic gum didnot exhibit any Fe+3 reducing activity. All of gum types showed similar FT-IR spectra.

Abstract Background and Aims Modified PET (use of 3.86% glucose dialysate instead of 2.27%) and temporal drainage at one hour allows a more complete study of peritoneal membrane transport, as it provides information about sodium sieving, free water transport (FWT) and ultrafiltration failure (UFF). Longitudinal data using this PET are sporadic. Aims of our study were a) longitudinal follow-up of the membrane function using modified PET b) evaluation of UFF and association with clinical and membrane characteristics. Method We analyzed all modified PET tests performed in our unit during the last 9 years. Patients who underwent the first test during the first year of treatment and then once a year were included. We estimated classic peritoneal transport parameters like D/P creatinine, D/D0 glucose and ultrafiltration (UF) at 4 hours. Moreover we calculated sodium seiving expressed as Dip/DPNa= (Dialysate sodium time 0/plasma sodium)-(Dialysate sodium time 1 hour/plasma sodium)], sodium removal (in one hour) and FWT. We collected clinical and lab data of the patients too. Results A total of 219 PETs were performed in 85 PD patients, (median age 64 years (range 18-45). The baseline membrane characteristics of the patients were: DP creatinine=0.74±0.11, D/D0glucose=0.28±0.08, DipDPNa=0.05±0.04, sodium removal=29,804 ±26,894 (mmol/L), FWT=214.9±194.7 ml and UF =655.1±265.5 ml. On multivariate regression analysis D/P creatinine, FWT and sodium removal were significant independent covariates accounting for 50% of the variability of ultrafiltration . The patients were followed-up for a median of 36 months (range 2.4-143).For the repeated measurements, the analysis showed that the observed values of D/Pcre,D/D0, Dip/DPNa, FWT and UF remained rather stable across the different time measurements. During the study, 23 out of 85 pts developed peritoneal UF failure (UFF). Analysis was performed using univariate random effects logistic time-series models and only D/D0 (OR=0.84, P=0.001), D/P Cre (OR=1.15, p&lt;0.001), urine volume (OR=1.26, p=0.027) and sodium removal (OR=0.99, p=0.003) were significantly associated with relative risk of UFF development. Conclusion Surprisingly, our patients present a stable membrane function over time. Possible explanations could be the low peritonitis rate and strict use of hypertonic glucose dialysates. Ultrafiltration failure was correlated with higher D/P creatinine and lower D/D0 glucose values, higher baseline urine volume and lower sodium sieving.
