<em>Sustainable standards and certification can encourage coffee farmers to adopt good agricultural practices (GAP), achieving coffee production that is economically, socially and environmentally sustainable. The Common Code for Coffee Community (4C) is a standard and certification scheme currently exists in Robusta coffee production center in Lampung Province. However, sustainable standard and certification become less relevant without farmers’ participation. Farmers’ participation in standards and certification has been relatively low and studies on the issue are relatively rare. This study aims to analyze the determinants of farmer’s participation in 4C Standards and Certification. The study was conducted in West Lampung and Tanggamus Ragency, Lampung Province from February to May 2019. The total number of respondents was 120 people (4C certified farmers and non-certified farmers) surveyed with a systematic-random-sampling method. Data was analyzed using heckprobit regression. The results showed that farmer participation in 4C was determined by the selling price of coffee, farmers’ side job, farmers' preference to replace coffee with other commodities, and the farmers’ group activity. The results indicated that 4C and coffee stakeholders at national scale need to consider policies on how to improve coffee price, optimizing the farmer organizations, and the added-value of coffee production.</em>

ObjectivesEstimating mortality risk in hospitalised SARS-CoV-2+ patients may help with choosing level of care and discussions with patients. The Coronavirus Clinical Characterisation Consortium Mortality Score (4C Score) is a promising COVID-19 mortality risk model. We examined the association of risk factors with 30-day mortality in hospitalised, full-code SARS-CoV-2+ patients and investigated the discrimination and calibration of the 4C Score. This was a retrospective cohort study of SARS-CoV-2+ hospitalised patients within the RECOVER (REgistry of suspected COVID-19 in EmeRgency care) network.Setting99 emergency departments (EDs) across the USA.ParticipantsPatients ≥18 years old, positive for SARS-CoV-2 in the ED, and hospitalised.Primary outcomeDeath within 30 days of the index visit. We performed logistic regression analysis, reporting multivariable risk ratios (MVRRs) and calculated the area under the ROC curve (AUROC) and mean prediction error for the original 4C Score and after dropping the C reactive protein (CRP) component.ResultsOf 6802 hospitalised patients with COVID-19, 1149 (16.9%) died within 30 days. The 30-day mortality was increased with age 80+ years (MVRR=5.79, 95% CI 4.23 to 7.34); male sex (MVRR=1.17, 1.05 to 1.28); and nursing home/assisted living facility residence (MVRR=1.29, 1.1 to 1.48). The 4C Score had comparable discrimination in the RECOVER dataset compared with the original 4C validation dataset (AUROC: RECOVER 0.786 (95% CI 0.773 to 0.799), 4C validation 0.763 (95% CI 0.757 to 0.769). Score-specific mortalities in our sample were lower than in the 4C validation sample (mean prediction error 6.0%). Dropping the CRP component from the 4C Score did not substantially affect discrimination and 4C risk estimates were now close (mean prediction error 0.7%).ConclusionsWe independently validated 4C Score as predicting risk of 30-day mortality in hospitalised SARS-CoV-2+ patients. We recommend dropping the CRP component of the score and using our recalibrated mortality risk estimates.

Estimates of the uncertainties arising from approximations in the methods used in different nuclear data processing and neutron transport codes are usually obtained by inter-comparing calculations made using different code systems. This paper gives details of an investigation of differences between results obtained by using different codes for a single zone model of the Encapsulated Nuclear Heat Source (ENHS) benchmark core fuelled with metallic alloy of Pu, U, and Zr. The ENHS is a new lead-bismuth or lead cooled novel reactor concept for 20 effective full power years without refuelling and with very small reactivity swing. The computational tools benchmarked include MOCUP, a coupled MCNP-4C and ORIGEN2.1 utility codes with MCNP data libraries based on ENDF/B-VI evaluation; KENO-V.a/ORIGEN2.1 code system, recently developed by authors of this paper, with the ENDFB-V based 238 group library; the design-oriented procedure based on the simplified one-dimensional (1D) geometry model and SAS2H control module; and the well-established fast reactor neutronics design tools in use at Argonne National Laboratory. Calculations made for the ENHS benchmark have shown that the differences between the results obtained when using different code schemes are quite significant and should be taken into account in assessing the quality of the nuclear data library.

This research aims to analyze comparative advantage, competitive advantage, the impact of government policies on input output of certified coffee farming, and analyze the sensitivity of changes in input and outputprices to the comparative advantage and competitive advantage of coffee farming in Sumberejo Subdistrict Tanggamus District. The samples of 25 farmers were taken simply randomly from 133 coffee farmers who applied 4C coffee certification. Data were analyzed using PAM (Policy Analysis Matrix). The results showed that 4C certification coffee farming in Sumberejo Subdistrict of Tanggamus District had a competitive advantage with value of Private Cost Ratio of 0.53 and comparative advantage with of Domestic Cost Ratio of 0.38. Comparative advantage was sensitive to 20.31 percent decline in production volume and 40 percent decline in farm output price. Increase in input price due to revocation of subsidies reduced competitive and comparative advantages. Competitive and comparative advantages were not sensitive to an increase in input price.Key words: certified, coffee, comparative, competitive, PAM

Both nuclear fusion and fission devices have to face the issue of the cooling of some critical components, and in particular the cooling of the superconducting coils and of the core fuel assemblies, respectively. In order to investigate these issues during the design phase of new reactors, e.g. new tokamak experiments and the forthcoming Generation IV reactors, new dedicated models and reliable tools should be developed. In this work, two separate tools, for fusion and fission devices, respectively, are developed relying on the same quasi-3D thermal-hydraulic model, i.e. the 1D advection flow of the coolant in a diffusive (solid) bundle along the channel axis and the 2D coupling between the channels. The already available 4C code for the analysis of thermal-hydraulic transients in the superconducting coils of fusion devices is firstly validated against experimental data, then benchmarked against other tools and finally its predictive capability are checked performing the simulations before the experiment. The 4C code is thus proved to be able to simulate transients spanning from very short to week-long timescales with the very same model. The code is then applied to the analysis of operational transients in the ITER TF coils. Whenever the minimum temperature margin requirements are not satisfied, different possible mitigation strategies are investigated. Concerning the fission devices, a new multi-physics simulation tool (FRENETIC) is developed for the quasi-3D analysis of a lead-cooled fast reactor core with the closed hexagonal fuel element configuration, as currently proposed within the framework of the European project LEADER. The code couples a neutronic and a thermal-hydraulic analysis of the full reactor core, each performed in separate modules. A first validation of the thermal-hydraulic module of this code is presented both in the single assembly and, even if preliminarly, in the multi-assembly configuration. In the latter case, a benchmark against another qualified tool (RELAP5-3D©) is also carried out. Finally the coupled code is tested in some simple transients in order to assess at least qualitatively the effectiveness of the coupling and the representativity of the multi-physics results.

While since mid-seventies it has been clearly shown that relativistic effects play a crucial role for the complete understanding of the chemistry of molecules especially when heavy elements are involved, still nowadays in most of the case the relativistic effects are introduced via approximate methods. The main motivation behind the introduction of such approximation, respect to the natural and most rigorous component (4c) formalism derived from the Dirac equation, is the computational burden. In the present paper we are proposing a review of the BERTHA code that, together with the recently introduced Python bindings, represents the state of the art for full 4c calculations both in term of performances as well as in terms of code usability.

This paper is focused on the preliminary analysis of results obtained on the first cores of the first phase of the BASALA (Boiling water reactor Advanced core physics Study Aimed at mox fuel LAttice) programme, aimed at studying the neutronic parameters in ABWR core in hot conditions, currently under investigation in the French EOLE critical facility, within the framework of a cooperation between NUPEC, CEA and COGEMA. The first “on-line” analysis of the results has been made, using a new preliminary design and safety scheme based on the French APOLLO-2 code in its 2.4 qualified version and associated CEA-93 V4 (JEF-2.2) Library, that will enable the Experimental Physics Division (SPEx) to perform future core designs. It describes the scheme adopted and the results obtained in various cases, going to the critical size determination to the reactivity worth of the perturbed configurations (voided, overmoderated, and poisoned with Gd2O3-UO2 pins). A preliminary study on the experimental results on the MISTRAL-4 is resumed, and the comparison of APOLLO-2 versus MCNP-4C calculations on these cores is made. The results obtained show very good agreements between the two codes, and versus the experiment. This work opens the way to the future full analysis of the experimental results of the qualifying teams with completely validated schemes, based on the new 2.5 version of the APOLLO-2 code.

The gamma-ray dose calculation is essential for the radiation shielding of pressurized water reactor (PWR) spent fuels. Homogenization modeling of fuel pin lattices for typical PWR spent fuel pins is regularly applied on the radiation protection calculation of gamma-ray dose in an air medium. However, depending on the size of the homogenized lattice and the location of the detectors, under-estimation or over-estimation of the gamma-ray dose due to the homogenization modeling can be obtained with respect to the detailed heterogeneous model. In previous published results from MCNP-4A and 4C calculations on gamma-ray dose from spent PWR fuel pins, very different homogeneous to heterogeneous (Hom/Het) ratios were reported. In this study these Hom/Het ratios have been re-evaluated and benchmarked by using the TRIPOLI-4 Monte Carlo transport code. The new TRIPOLI-4 mesh tally capabilities have also been applied to calculate the radial and axial gamma-ray dose distribution. With the recently upgraded TRIPOLI-4 display tool, the dose rate maps and the isodose rate curves around a spent PWR fuel assembly have been established.

Code validation problems involve calculation of experiments and a comparison experiment-calculation. Experimental data and physical properties of these systems are used to determine the range of applicability of the validation. Once a sequence-code of calculations has been validated, it has to be underlined that the comparison experimental-calculated results involving “complex systems” or “complex experimental measures” permits also a bi-lateral cross-check between the calculation scheme and the experimental procedures. The results of the testing and the validation effort related to the collection of information and measured data and the comparison between code results with experimental data coming from a “low-level waste” repository are presented in this paper. The Baita-Bihor repository, sited into former disused uranium mine in Transylvania, has been considered as the source of experimental data. The study was developed through the following steps: a) collection and processing of measured data (radioactivity content and dose rate), from the cemented containers of the Baita-Bihor repository; b) decay gamma source calculation by the ANITA-2000 code package (the input data for the calculations are the measured isotope activities for each container); c) decay gamma transport calculation by the SCALENEA-1 shielding Sn sequence approach (Nitawl-Xsdrnpm-Xsdose modules of the Scale 4.4a code system, using the Vitenea-J library, based on FENDL/E-2 data) to obtain dose rates on the surfaces and at various points outside the containers; d) comparison experimental-calculated dose rates, taking into account also the measurement uncertainties. The new version of the ANITA-2000 activation code package used makes possible to assess the behaviour of irradiated materials independently from the knowledge of the irradiation scenario but using only data on the isotope radioactive material composition. Radioactive waste disposed of at Baita Bihor repository consists of worn reactor parts, resins and filters, packing materials, mop heads, protective clothing, temporary floor coverings and tools, the sources normally generated during the day-to-day operation of research reactors, the remediation-treatment stations and the medicine and biological activities. The low and intermediate wastes are prepared for shipping and disposal in the treatment stations by confining them in a cement matrix inside 220 litre metallic drums. Each container consists of an iron cladding filled by concrete Portland. Radioisotope composition and radioactivity distributions inside the drum are measured. The gamma spectroscopy has been used for. The calibration technique was based on the assumption of a uniform distribution of the source activity in the drum and also of a uniform sample matrix. Dose rate measurements are done continuously, circularly, in the central plan on the surface of the drum and 1 m from the surface, in the air. A “stuffing factor” model has been adopted to simulate, for the calculation, the spatial distribution of the gamma sources in the concrete region. In order to guarantee a complete Quality Assurance for codes and procedures, a simulation of the radioactive containers to evaluate the dose rates was done also by using the Monte Carlo MCNP-4C code. Its calculation results are in a very good agreement with those obtained by the Sn approach (discrepancies are around 2%, using the spherical approximation).

Pregnancy-associated breast cancer (PABC) is defined as a breast cancer diagnosed during pregnancy, lactation, or in the first postpartum year. PABC is a rare complication that occurs in approximately 0.01% to 0.03% of all pregnancies. The difficulty in diagnosis worsens the prognosis. D.G., 29-year-old, female, noted a mass in her right breast in June 2020. One month later with 13+4 weeks’ gestation, she presented to the obstetrics emergency with recurrent episodes of lower back pain. She was released home with pain relief and was instructed to realize a mammography due to the presence of a 4-cm mass on physical examination of the right breast. Patient returned 12 days later with severe low back pain, a BIRADS 4C mammography, and multiple liver lesions in total abdomen ultrasound. Core-needle biopsy demonstrated a stage II invasive ductal carcinoma with hormone receptors positive and human epidermal growth factor receptor 2 positive. There is involvement of the axilla and intramammary lymph nodes. Magnetic resonance imaging of the lower back and sacroiliac joint was performed and found multiple lesions suspected of metastasis in the inferior thoracic vertebrae, lumbar vertebrae, sacrum, ilium, and femurs. Computed tomography (CT) of the thorax identified a 2.3×1.8 cm irregular lesion in the right breast compatible with the primary neoplasm. Chemotherapy was initiated till she was 31 weeks’ gestation. After childbirth, she reinitiates chemotherapy. Three months later, the patient has convulsive episodes. Cranial CT was done and found multiple lesions compatible with brain metastasis, so she initiated brain radiotherapy. PABC can present itself as a challenging situation with nonspecific symptoms and at an advanced stage. Therefore, it is important to have the PABC in our list of differential diagnoses in this patient.

Rosai-Dorfman disease (RDD) is a rare histiocytic disorder that was first described by Destombes in 1965 and subsequently classified by Rosai and Dorfman in 1969 as a distinct clinical and pathological condition. It is a self-limiting disease process characterized by a non-Langerhans cell benign proliferation that usually involves the lymph nodes, but can affect extranodal sites. Classic RDD most commonly presents as massive bilateral painless cervical lymphadenopathy, with associated fever and loss of weight in children and young adults. Despite the predilection for the head and neck lymph nodes, RDD may present extranodal case in 43% of cases and in rare instances (around 23% of extranodal cases) only extranodal involvement can be described. The more common extranodal sites include the skin, nasal cavity, bone, orbital tissue, and the central nervous system. RDD involving the breast tissue is extremely rare, with fewer than 50 reported cases and it is important to recognize because it can mimic malignancy. Patients with involvement of the breast can present with painless palpable mass and ill-defined sensation or abnormal mammogram. The radiologic presentation of RDD lesions can have a notable diversity, but commonly have an appearance that is indistinguishable from breast carcinoma on mammogram and US. Therefore, the main diagnostic modality is histopathologic evaluation, with the features of marked dilatation of sinuses due to accumulation of activated histiocytes that demonstrate variable degrees of emperipolesis, along with the immunohistochemical characteristics like S100+, CD68+, and CD1a−. Management of RDD depends on the symptoms and it can be conservative with observation, as long as some cases have spontaneous regression, especially in the classic nodal disease. Surgical excision may be indicated in unifocal extranodal disease, especially for symptomatic cases. A 14-year-old female adolescent visited a breast surgeon with a self-detected painless palpable mass in the left breast for 3 months, without systemic symptoms or significant family history. On examination, there was a firm 3.5-cm mass in the upper inner quadrant of the left breast. The mammogram revealed a focal distortion on axillary tail in the left breast, designated as BIRADS 4. US demonstrated two masses in the left breast, including a 3.4×2.6×1.5 cm hypoechoic, irregular mass at 11:00 and a 1×0.9×0.7 cm hypoechoic, irregular mass at 11:00, both near the infraclavicular region, accompanied by a 2.8×1 cm atypical lymph node in the left axillary region, designated BIRADS 4C. US-guided core biopsies of both masses were obtained, which showed a nonspecific chronic inflammatory process. A new core biopsy was performed but once more with nonspecific histopathological findings, as well as negative results from culture examinations. Following these indeterminate findings, the patient underwent excisional biopsy, which histopathologic conclusion finally elucidated the extranodal RDD diagnosis.
