In order to study the physiology of perception of saline stress in ‘Common Sunki’ mandarin hybrids, an experiment was realized in a greenhouse at the CCTA - UFCG, Pombal, PB, Brazil, using using a randomized block design with a 2x4 factorial , with two levels of salinity (0.3 and 4.0 dS m-1) and four ‘Common Sunki’ mandarin hybrids (1 - TSKC x CTARG–019; 2 - TSKC x CTSW–028; 3 - TSKC x CTSW–033 and 4 - TSKC x CTSW-041), with three replications and four plants per plot. The plants were grown hydroponically and 90 days after sowing, a saline water solution was applied on rootstocks and evaluations of gas exchange and chlorophyll fluorescence were carried out 24 and 48 hours after saline stress. Changes on physiologic conditions were observed in the hybrids TSKC x CTSW - 028, TSKC x CTSW - 033 and TSKC x CTSW - 041 during the first 24 hours of saline stress. Fot the hybrid TSKC x CTARG – 019, changes were observed after 48 hours od salinity stress. According to salinity tolerance, the hybrids could be classified as follows: TSKC x CTARG - 019 > TSKC x CTSW - 028 = TSKC x CTSW – 041 > TSKC x CTSW - 033.

Background: Return to play and player satisfaction have been quite high after ulnar collateral ligament reconstruction (UCLR); however, there has been little reported on how outcomes are affected by surgical technique, graft type, and tear characteristics. Purpose: To evaluate surgical techniques, graft type, and tear characteristics on Major League Baseball (MLB) performance after UCLR. Study Design: Cohort study; Level of evidence, 2. Methods: MLB pitchers who underwent primary UCLR at a single institution were included. Tear characteristics included tear location, tear grade, and acuity. Surgical technique and graft type were also collected. Pitching performance statistics, including earned run average (ERA), walks and hits per innings pitched (WHIP), innings pitched, and fastball velocity were evaluated 3 years before and after UCLR. Results: Forty-six MLB pitchers were identified as having primary UCLR. Return to play was 96%, with 82% returning to MLB play. Technique performed showed no difference in performance. As compared with pitchers with gracilis grafts, pitchers with palmaris grafts were younger ( P = .043), played longer after surgery ( P = .012), and returned to play at 100% (35 of 35) versus 82% (9 of 11, P = .010). When compared with pitchers with proximal tears, pitchers with distal tears pitched at higher velocity (93.0 vs 90.6 mph, P = .023) and had better performance before surgery (ERA, P = .003; WHIP, P = .021); however, those with proximal tears improved to match this performance and velocity after reconstruction. As compared with those having partial tears, pitchers with complete tears played longer after surgery (5.9 vs 4.0 years, P = .033), had a better ERA before injury ( P = .041), and had better WHIP ( P = .037) and strikeouts per 9 innings ( P = .025) after reconstruction. Pitchers with chronic tears had a significant improvement in postoperative ERA, from 4.49 to 3.80 ( P = .040). Conclusion: Technique performed and graft type used did not affect performance; however, pitchers with palmaris grafts returned at a higher rate than those with gracilis grafts. Distal tears occurred in pitchers with greater velocity and better performance before injury, yet pitchers with proximal tears matched this performance after reconstruction. Pitchers with complete tears played longer after reconstruction. Pitchers who had partial tears had worse performance before injury and after reconstruction, and those with chronic tears saw a significant improvement in ERA with reconstruction.

Background Tolvaptan is converted to major metabolites including three monohydroxylates (DM-4110, DM-4111 and DM-4119), an oxidate (MOP-21826) and a carboxylate (DM-4103) in humans. This study developed a simultaneous quantitative method for tolvaptan and its five major metabolites in human plasma using liquid chromatography coupled to tandem mass spectrometry. Methods Deproteinized plasma specimens using acetonitrile were separated using a 3- μm particle size octadecylsilyl column with 250 mm length and a simple linear gradient program at a flow rate of 0.3 mL/min with a total run time of 15 min. This method was applied to the determination of plasma samples collected from 20 heart failure patients treated with 3.75–15 mg tolvaptan. Results No interfering peak was found in drug-free plasma specimens. The calibration curves of tolvaptan, DM-4110, DM-4111, DM-4119, MOP-21826 and DM-4103 were linear over the concentration ranges of 3.125–1000, 0.3125–100, 1.25–400, 0.625–200, 0.125–40 and 31.25–10,000 ng/mL, respectively. Their pretreatment recovery rates and matrix factors were 94.1–113.9% and 86.9–108.0%, respectively. The intra- and inter-day accuracies and imprecisions were 91.6–106.5% and 0.9–10.9%, respectively, for all analytes. The plasma concentration ranges of tolvaptan, DM-4110, DM-4111, DM-4119, MOP-21826 and DM-4103 were 9.37–280, 1.91–16.3, 3.43–88.9, 1.43–10.4, 0.160–1.01 and 40.2–1471 ng/mL, respectively, in heart failure patients. Conclusions This validated method with acceptable analytical performance can be utilized for evaluating the pharmacokinetics of oral tolvaptan, including the determination of its major metabolites, in heart failure patients.

410 Background: Based on interim results from the phase 3 KEYNOTE-045 (NCT02256436) study comparing pembro v investigator’s choice of chemotherapy (chemo), pembro was approved for the treatment of locally advanced or metastatic UC that has progressed during or after a platinum-containing regimen. Updated results after 2 years of follow-up are presented. Methods: Eligible patients (histologically or cytologically confirmed UC, progression after platinum, ECOG PS 0-2, measurable disease per RECIST v1.1, ≤2 lines of systemic therapy) were randomly assigned 1:1 to pembro 200 mg Q3W or investigator’s choice of paclitaxel 175 mg/m2 Q3W, docetaxel 75 mg/m2 Q3W, or vinflunine 320 mg/m2 Q3W. Primary efficacy end points were OS and PFS (RECIST v1.1, blinded central review). ORR (RECIST v1.1, blinded central review) was a secondary end point. Results: As of Oct 26, 2017, among the 542 enrolled patients (pembro, 270; chemo, 272), median follow-up was 27.7 mo. Median OS was significantly longer with pembro v chemo (10.3 v 7.3 mo; HR, 0.70; P < 0.0002). OS benefit with pembro v chemo was seen in all PD-L1 expression subgroups (HR: combined positive score [CPS] < 1, 0.82; CPS ≥1, 0.58; CPS < 10, 0.75; CPS ≥10, 0.56) and was maintained regardless of age, ECOG PS, prior therapy, liver metastases, baseline hemoglobin, time from last chemo, histology, risk factor group, and choice of chemo. PFS was not different between arms (2.1 v 3.3 mo; HR, 0.96; P = 0.32). ORR was higher with pembro v chemo (21.1% v 11.0%). Median duration of response was longer with pembro (not reached [1.6+–30.3+ mo] v 4.4 mo [1.4+–29.9+ mo]), and a greater proportion of responses lasted ≥12 mo (68% v 35%) as assessed by Kaplan-Meier method. Fewer patients with pembro v chemo experienced a treatment-related adverse event of any grade (62.0% v 90.6%) and a grade ≥3 adverse event (16.5% v 50.2%). Conclusions: Results observed over 2 years of follow-up, including OS benefit and superior safety with pembro v chemo, were consistent with the interim analyses that led to the approval of pembro in locally advanced or metastatic UC that progressed during or after platinum-based chemotherapy. The syntheses and room-temperature single-crystal X-ray structural characterization of binuclear 1 : 2 adducts formed between silver(I) (pseudo-)halides, AgX, and triphenylarsine, AsPh3, for X = Cl, Br, I, SCN (1)–(4), are described. The chloride (1), obtained from 2-methylpyridine, is triclinic, P-1, a 10·410(2), b 12·716(2), c 14·196(6) Å, α 113·38(2), β 109·41(2), γ 75·08(1)°, Z = 1 (dimer); conventional R on F was 0·037 for No 3979 independent ‘observed’ (I > 3σ(I)) reflections. The bromide (2a), obtained from 2,6-dimethylpyridine, and iodide (3), obtained from a mixture of AgI/saturated KI in MeOH solutions, are isomorphous, monoclinic, P 21/c a≈ 24·2, b ≈ 13·9, c ≈ 20·2 Å, β ≈ 109·5°, Z = 4 dimers; R was 0·046 and 0·044 for No 5670 and 6039 respectively. The thiocyanate (4) has a similar cell, a 24·12(1), b 12·558(8), c 23·244(4) Å, β 110·11(3)°, Z = 4 dimers, R being 0·044 for No 7956; one of the thiocyanate ligands (which bridge in Ag-SCN-Ag mode) is disordered. A second polymorph of the bromide (2b) (from a mixture of AgBr/saturated KBr in H2O) is also monoclinic, P 21/c, a 14·121(8), b 25·577(3), c 21·968(2) Å, β 125·54(3)°, Z = 4 dimers (R was 0·047 for No 5715). Ag–As range between 2·568(1) and 2·633(1) Å throughout the series; in the isomorphous bromide and iodide, values increase slightly: 2·578(1)–2·611(1), cf. 2·601(2)-2·633(1) Å respectively. Ag–X are 2·568(2)-2·670(2) (Cl); 2·688(2)–2·715(2) (Br); 2·828(2)–2·856(1) Å (I); Ag-S, N for the ordered SCN group are 2·646(3), 2·255(6) Å. A redetermination of improved precision (R 0·035, No 6030) is reported for the triphenylphosphine/thiocyanate analogue. The far-infrared spectra of [(Ph3As)2Ag(µ-X)2Ag (AsPh3)2] show v(AgX) bands at 185, 145 (X = Cl), 145, 130, 106 (X = Br) and 121 cm-1 (X = I). The splittings and band widths reflect a decrease in the degree of distortion of the Ag(µ-X)2Ag units from a symmetrically bridged structure from X = Cl to I.

A field study determined an optimal approach for the remote sensing of a pine needle blight of unknown cause that occurs frequently in the southeastern United States. Needle spectral reflectance was measured in five pairs of slash pines (Pinuselliottii Engelm. var. elliottii), each comprising a blighted and a nonblighted tree. Reflectance of blighted needles generally was greater within the 400–700 nm wavelength range and less in the 720–850 nm range compared with nonblighted needles (p ≤ 0.050). The relative change in reflectance, or reflectance sensitivity to the blight, was greatest near 680 nm. As predicted by the reflectance sensitivity, a black and white digital image at 680 ± 5 nm revealed strong contrast of a blighted compared with a nonblighted tree; the blighted canopy was nearly white in the image, while the nonblighted canopy was dark grey. Contrasts were much less at 560 ± 5, 694 ± 3, and 700 ± 5 nm, in the panchromatic visible spectrum (410–740 nm), and at 760 ± 5 nm. Remote sensing of southeastern pine forests at 680 ± 5 nm ultimately could provide regional estimates of blight occurrence and distribution, and thus contribute to determining its cause.

Abstract A growing number of in vitro studies suggest that recombinant human stem cell factor (SCF) is capable of augmenting the proliferative capacity of human hematopoietic progenitor cells (HPC) and stem cells (HSC). We further evaluated this biologic effect by analyzing the response of bone marrow (BM) HPCs and HSCs to the administration of SCF in eight patients with locally advanced or metastatic breast cancer who were enrolled in an ongoing phase I study. SCF was administered for 14 days by daily subcutaneous injection at dosages of 10, 25, or 50 micrograms/kg/d. BM CD34+ HLA-DR+ and CD34+ HLA-DR- CD15- cells, previously shown by our laboratory to be enriched for various classes of differentiated and primitive HPCs, respectively, were quantitated in BM samples on day 0 (pretreatment) and day 15 (posttreatment). These CD34+ HLA-DR+ and CD34+ HLA-DR- CD15- cells were then isolated by cell- sorting and assayed for several classes of HPCs, including the high-- proliferative potential colony-forming cell (HPP-CFC), the burst- forming unit--megakaryocyte (BFU-MK), and the long-term BM culture-- initiating cell (LTBMC-IC). SCF administration resulted in a 3.3-fold (range, 1.4- to 18.8-fold; P = .018) increase in the absolute numbers of CD34+ cells, a 3.7-fold (range, 1.2- to 8.2-fold; P = .028) increase in the absolute numbers of CD34+ HLA-DR+ cells, and a 2.4-fold (range, 1.1- to 29.3-fold; P = .010) increase in the absolute numbers of CD34+ HLA-DR- CD15- cells. Following the infusion of SCF, a statistically significant increase in the absolute numbers of HPP-CFC (P = .018), BFU- MK (P = .046), CFU-granulocyte, erythrocyte, monocyte, megakaryocyte (CFU-GEMM: P = .043), BFU-erythrocyte (BFU-E; P = .043), CFU- granulocyte, macrophage (CFU-GM; P = .045), and CFU-megakaryocyte (CFU- MK; P = .028) per milliliter of marrow was observed. Stromal cell-free LTBMCs supplemented with SCF and interleukin-3 (IL-3), initiated with CD34+ HLA-DR- CD15- cells obtained on day 0, produced viable cells for 9.6 weeks, compared with 11.5 weeks for LTBMCs initiated with CD34+ HLA- DR- CD15- cells obtained on day 15. Cumulative cellular production by LTBMCs initiated with day 15 CD34+ HLA-DR- CD15- cells was statistically greater than that by day 0 LTBMCs (P = .031). These same cultures produced CFU-GM for 6.3 weeks (day 0) versus 9 weeks (day 15).(ABSTRACT TRUNCATED AT 400 WORDS)

Purpose: To investigate differences in neuromuscular factors between elite and nonelite players and to establish which factors underpin direction-specific unilateral jump performance. Methods: Elite (n = 23; age, 18.1 [1.0] y; body mass index, 23.1 [1.8] kg·m−2) and nonelite (n = 20; age, 22.3 [2.7] y; body mass index, 23.8 [1.8] kg·m−2) soccer players performed 3 unilateral countermovement jumps (CMJs) on a force platform in the vertical, horizontal-forward, and medial directions. Knee extension isometric maximum voluntary contraction torque was assessed using isokinetic dynamometry. Vastus lateralis fascicle length, angle of pennation, quadriceps femoris muscle volume (Mvol), and physiological cross-sectional area (PCSA) were assessed using ultrasonography. Vastus lateralis activation was assessed using electromyography. Results: Elite soccer players presented greater knee extensor isometric maximum voluntary contraction torque (365.7 [66.6] vs 320.1 [62.6] N·m; P = .045), Mvol (2853 [508] vs 2429 [232] cm3; P = .001), and PCSA (227 [42] vs 193 [25] cm2; P = .003) than nonelite. In both cohorts, unilateral vertical and unilateral medial CMJ performance correlated with Mvol and PCSA (r ≥ .310, P ≤ .043). In elite soccer players, unilateral vertical and unilateral medial CMJ performance correlated with upward phase vastus lateralis activation and angle of pennation (r ≥ .478, P ≤ .028). Unilateral horizontal-forward CMJ peak vertical power did not correlate with any measure of muscle size or activation but correlated inversely with angle of pennation (r = −.413, P = .037). Conclusions: While larger and stronger quadriceps differentiated elite from nonelite players, relationships between neuromuscular factors and unilateral jump performance were shown to be direction-specific. These findings support a notion that improving direction-specific muscular power in soccer requires improving a distinct neuromuscular profile.

ABSTRACT The objective was to study the biochemical changes and dry matter content in lemon and mandarin hybrids under salt stress during rootstock formation. For this, a study was conducted in randomized complete block, using a 2 x 5 factorial scheme, with two salinity levels (0.3 and 4.0 dS m-1) applied in five citrus rootstock genotypes (1. TSKC x CTARG - 019; 2. LRF; 3. TSKC x (LCR x TR) - 040; 4. LCRSTC and 5. LVK), with three replicates and four plants per plot. At 90 days after sowing, saline treatments started to be applied and continued until 120 days after sowing, the moment in which the plants were collected for evaluation of biochemical characteristics and phytomass accumulation. The increase in water salinity negatively affected the biochemical components and dry matter accumulation of citrus genotypes. The genotypes TSKC x (LCR x TR) - 040, LCRSTC and LVK were the least affected by salt stress, standing out as the materials most tolerant to salinity.

ABSTRACT Using new Very Large Telescope (VLT)/XShooter spectral observations we analyse the physical properties of five z ∼ 0.3–0.4 confirmed Lyman continuum (LyC) leakers. Strong resonant Mg ii λλ2796, 2803 Å emission lines (I(λλ2796, 2803)/I(Hβ) ≃ 10–38 per cent) and non-resonant Fe ii* λλ2612, 2626 Å emission lines are observed in spectra of five and three galaxies, respectively. We find high electron densities Ne ∼ 400 cm−3, significantly higher than in typical low-z, but comparable to those measured in z ∼ 2–3 star-forming galaxies (SFGs). The galaxies have a mean value of log N/O = –1.16, close to the maximum values found for SFGs in the metallicity range of 12 + log O/H ≃ 7.7–8.1. All 11 low-z LyC emitting galaxies found by Izotov et al., including the ones considered in this study, are characterized by high equivalent width (EW) (Hβ) ∼ 200–400 Å, high ionization parameter (log(U) = –2.5 to –1.7), high average ionizing photon production efficiency ξ = 1025.54 Hz erg−1, and hard ionizing radiation. On the Baldwin–Phillips–Terlevich (BPT) diagram we find the same offset of our leakers from low-z main-sequence SFGs as that for local analogues of Lyman-break galaxies (LBGs) and extreme SFGs at z ∼ 2–3. We confirm the effectiveness of the He i emission lines diagnostics proposed by Izotov et al. in searching for LyC leaker candidates and find that their intensity ratios correspond to those in a median with low neutral hydrogen column density N(H i) = 1017–5 × 1017 cm−2 that permit leakage of LyC radiation, likely due to their density-bounded H ii regions.

Context: Hyposomatotropism in healthy aging women reflects in part physiological estrogen (estradiol [E2]) depletion associated with menopause. Objective and Design: The purpose of this study was to test the hypothesis that low concentrations of endogenous E2 after menopause continue to drive GH secretion. Setting: The study was performed at the Mayo Center for Clinical and Translational Science. Participants: The participants were 24 postmenopausal women (aged 50–77 years with body mass index of 19–32 kg/m2). Interventions: This was a randomized, double-blind, placebo-controlled, parallel-cohort treatment study with placebo (PL) (n = 14) or the antiestrogen fulvestrant (FUL) (n = 10) for 3 weeks, followed by infusion of l-arginine with saline, GHRH, ghrelin, or both peptide secretagogues. Outcomes: GH concentrations were measured over 6 hours with 10-minute sampling and mass spectrometry measures of testosterone, E2, and estrone. Results: Concentrations of testosterone, E2, estrone, SHBG, IGF-I, LH, and FSH were not influenced by antiestrogen treatment. In contrast, GH rose from 0.096 ± 0.018 (PL) to 0.23 ± 0.063 μg/L (FUL, P = .033), and IGF-I binding protein type 3 (IGFBP-3) from 3.6 ± 0.18 to 4.0 ± 2.0 mg/L (P = .041). Conversely, prolactin fell from 7.1 ± 0.69 (PL) to 5.5 ± 0.57 μg/L (FUL) (P = .05), and IGF-I binding protein type 1 (IGFBP-1) fell from 44 ± 9.4 to 27 ± 4.3 μg/L (P = .048). Moreover, FUL vs PL potentiated mean GH responses to l-arginine/saline (P = .007), l-arginine/ghrelin (P = .008), and l-arginine/GHRH + ghrelin (P = .031), but not l-arginine/GHRH. Conclusion: The potent antiestrogen, FUL, amplifies fasting and secretagogue-driven GH secretion and IGFBP-3 concentrations in postmenopausal women without altering SHBG or sex steroid levels. FUL also suppresses prolactin and IGFBP-1, without altering IGF-I. Thus, a major antiestrogen mediates 3 actions of estrogen: agonism (GH), neutral effects (sex steroids), and estrogen antagonism (prolactin and IGFBP-1).

Nitric oxide (NO) derived from neuronal NO synthase (nNOS) in the kidney inner medulla has been implicated in the regulation of arterial blood pressure. The purpose of the present study was to determine the effect of high dietary NaCl on the expression of nNOS in the rat inner medullary collecting duct (IMCD). After 3 days or 3 wk of high (4.0%)-NaCl diet in rats, urinary[Formula: see text]/[Formula: see text]excretion significantly increased. In freshly microdissected IMCD, nNOS was readily detected by immunofluorescence with polyclonal antibody, an effect that was completely blocked by neutralization of antibody with immunizing antigen. In rats fed a 4.0% NaCl diet for 3 days, IMCD nNOS mRNA, detected by RT-PCR, did not change from control values (0.3% NaCl, 19.84 ± 1.57 × 103, vs. 4.0% NaCl, 20.44 ± 3.14 × 103 cpm; P = not significant, n = 3). By Western blotting however, nNOS protein expression significantly increased (0.3% NaCl, 0.51 ± 0.12, vs. 4.0% NaCl, 0.92 ± 0.14 arbitrary units; P < 0.05, n = 5). After 3 wk of 4.0% dietary NaCl, expression of nNOS mRNA and protein in IMCD did not differ significantly from control values. In contrast to these data, renal cortical expression of nNOS mRNA and protein was significantly decreased after 4.0% NaCl diet for 3 days. High dietary NaCl had no significant effect on expression of mRNA for inducible NO synthase (iNOS) in IMCD after either 3 days or 3 wk. In summary, our data indicate that nNOS mRNA and protein are expressed in IMCD and that high dietary NaCl differentially regulates nNOS expression in IMCD and cortex. The early increase in nNOS protein in IMCD may contribute to enhanced local production of NO and thereby represent an adaptive response to salt intake.

Object In instrumentation of the upper cervical spine, placement of pedicle screws into C-2 is generally safe, although there is the potential for injury to the vertebral arteries. Owing to this risk, translaminar screws into C-2 have been used. The aim of this study was to compare the stability of the in vitro cadaveric spine using C-2 laminar compared with C-2 pedicle screws in C2–3 instrumentation. Methods Eight fresh frozen human cadaveric cervical spines (C1–6) were potted at C1–2 and C5–6. Pure moments in increments of 0.3 Nm to a maximum of 1.5 Nm were applied in flexion, extension, right and left lateral bending, and right and left axial rotation. Each specimen was tested sequentially in three modes: 1) intact; 2) C2 pedicle screw–C3 lateral mass fixation; and 3) C2 laminar screw–C3 lateral mass fixation. The sequence of fixation testing was randomized. Motion was tracked with reflective markers attached to C-2 and C-3. Results Spinal levels with instrumentation showed significantly less motion than the intact spine in all directions and with all loads greater than 0.3 Nm (p < 0.05). Although there was no significant difference between C2 pedicle screw–C3 lateral mass fixation and C2 laminar screw–C3 lateral mass fixation, generally the former type of fixation was associated with less motion than the latter. Conclusions When pedicle screws in C-2 are contraindicated or inappropriate, laminar screws in C-2 offer a safe and acceptable option for posterior instrumentation.

Context: Successful elite sprint to long-distance runners are known to have shorter ground-contact time (GCT) than their less successful counterparts. Purpose: To investigate whether augmented feedback (aF) about GCT can reduce the time on ground (TOG) per minute in long-distance runners and, if so, whether this reduction improves running performance. Methods: Thirty well-trained runners were allocated to 3 groups. The intervention group (IG) received visual aF about their GCT during 8 high-intensity interval sessions in the 4-wk training period and were instructed to minimize GCT. The 1st control group (CG1) trained with the IG but was not given any feedback. The 2nd control group (CG2) followed their own training routine. Data were obtained pre- and postintervention for all 3 groups. The dependent variable was TOG per minute, computed from step frequency and GCT. Results: The IG significantly reduced TOG (P = .043, −1.7%, 90%CL −3.1;−0.3) and improved their mean 10 × 400-m performance time (P < .001, −1.5%, 90%CL −1.9;−1.1). In contrast, the 2 control groups revealed unchanged values, indicating that normal high-intensity training and an individualized routine without aF were not able to reduce TOG. The fact that CG1 received the same instructions and participated in the same training sessions as the IG underlined that aF was crucial to reduce TOG. Conclusions: The provision of aF about GCT seems to be a promising approach that should be considered during training practice of well-trained runners.

Abstract Differential thermal scanning of the new lipid-like bilayer material (CH2)10(NH3)2CdCl4 showed two structural phase transitions, with onset temperatures at T2 = (359 ± 2) K and T1 = (415± 1) K. Permittivity measurements were performed between room temperature and 450 K at 60-100 kHz. A step-like rise in permittivity at T2, associated with an order-disorder transition, is attributed to chain melting. Two anomalies at (413 ± 1) K and (430 ± 3) K, showing thermal hysteresis of -8 and 10 K, respectively, in dicate first order transitions which are associated with crystalline phase change. The AC conductivity follows an Arrhenius-type relation with the activation energy ΔE varying with the frequency / according to the relation ΔE = ΔE0 [1 -exp (f0/f)] where ΔE0,f0 and a are 0.95 eV, 52 Hz and 0.11, respectively. The frequency dependent conductivity follows the power law σ= σdc +Bωx with 0.3 <s< 1.5 for hopping conduction of hydrogen and/or chloride ions in the high temperature range, and localized hopping and/or orientational motion predominating temperatures lower than 413 K. Variations of B and s with temperature are discussed. PACS No. 76, 77

Background: There are few published reports on the safety and efficacy of stereoelectroencephalography (SEEG) in the presurgical evaluation of pediatric drug-resistant epilepsy. Our objective was to describe institutional experience with pediatric SEEG in terms of (1) insertional complications, (2) identification of the epileptogenic zone and (3) seizure outcome following SEEG-tailored resections. Methods: Retrospective review of 29 patients pediatric drug resistant epilepsy patients who underwent presurgical SEEG between 2005 – 2018. Results: 29 pediatric SEEG patients (15 male; 12.4 ± 4.6 years old) were included in this study with mean follow-up of 6.0 ± 4.1 years. SEEG-related complications occurred in 1/29 (3%)—neurogenic pulmonary edema. A total of 190 multi-contact electrodes (mean of 7.0 ± 2.5per patient) were implanted across 30 insertions which captured 437 electrographic seizures (mean 17.5 ± 27.6 per patient). The most common rationale for SEEG was normal MRI with surface EEG that failed to identify the EZ (16/29; 55%). SEEG-tailored resections were performed in 24/29 (83%). Engel I outcome was achieved following resections in 19/24 cases (79%) with 5.9 ± 4.0 years of post-operative follow-up. Conclusions: Stereoelectroencephalography in presurgical evaluation of pediatric drug-resistant epilepsy is a safe and effective way to identify the epileptogenic zone permitting SEEG-tailored resection.

Soil and water salinity cause physiological disorders in sensitive plants, such as altered gas exchange in citrus genotypes. However, it is possible to analyse these effects and to identify genotypes tolerant to salt stress. An experiment was carried out in order to evaluate the tolerance of citrus genotypes considering the irrigation with saline water during rootstock formation. The study took place under greenhouse conditions in Pombal county, Paraiba, Brazil. A randomised block design was used, with treatments arranged in a factorial scheme (5 x 8). The two factors were: [i] five salinity levels of irrigation water (0.8, 1.6, 2.4, 3.2 and 4.0 dS m-1) and [ii] eight genotypes of citrus rootstocks: 1. ‘Rangpur Santa Cruz’ lime (LCRSTC); 2. common ‘Sunki’ mandarin (TSKC) x ‘Swingle’ citrumelo (CTSW) - 028; 3. TSKC x CTSW - 033; 4. TSKC x CTSW - 041; 5. ‘Volkamer’ lemon (LVK) x ‘Rangpur’ lime (LCR) - 038; 6. ‘Sunki of Florida’ mandarin (TSKFL); 7. TSKC and 8. ‘Florida’ rough lemon (LRF). Gas exchange was evaluated at 15, 30 and 60 days after the beginning of the saline water application. High water salinity reduces gas exchange in citrus genotypes, which was more evident at 15 days from the beginning of stress. The genotypes ‘Santa Cruz Rangpur’ lime, TSKC x CTSW - 041, LVK x LCR - 038 and ‘Florida’ rough lemon exhibit a satisfactory physiological behaviour during the first 30 days of exposure to the stress, showing moderate tolerance to salt stress. The genotypes TSKC x CTSW - 033 and common ‘Sunki’ mandarin are the most sensitive to salinity.

410 Background: We report prognostic evaluation of BL weight-associated parameters (body-mass index [BMI], body surface area [BSA] and weight) in pts with mPDAC after progression following gem-based therapy (NAPOLI-1 trial; NCT01494506). Pts received nal-IRI+5-FU/LV, nal-IRI monotherapy or 5-FU/LV in NAPOLI-1, an international, randomised, phase 3 trial; nal-IRI+5-FU/LV treatment resulted in a 45% increased median OS vs. 5-FU/LV (unstratified HR = 0.67; p = 0.012). Methods: This exploratory subgroup analysis compares outcomes by BL BMI, BSA and weight, using primary survival analysis data from the ITT population for all treatment arms combined (n = 417) and the nal-IRI+5-FU/LV arm on its own (n = 117). Results: OS and PFS were not significantly different between BL BMI, BSA and weight median subgroups in the entire NAPOLI-1 ITT population (HR range 1.06–1.15; log-rank p-value range 0.21–0.60; Table) and in the nal-IRI+5-FU/LV arm (HR range 0.94–1.19; log-rank p-value range 0.43–1.00). Conclusions: This post-hoc subgroup analysis did not detect any prognostic impact on treatment outcome by BL BMI, BSA and weight for mPDAC pts progressed following gem-based therapy. This observation rules out a treatment-independent effect. No evidence of a predictive effect on nal-IRI+5-FU/LV efficacy was found. Clinical trial information: NCT01494506. [Table: see text]

Despite human endometrium undergoing more than 400 cycles of regeneration, differentiation and shedding during a woman's reproductive years, and that in non-menstruating species (eg rodents) there are cycles of endometrial growth and apoptosis, endometrial stem/progenitor cells have only recently been identified. Since there are no specific stem cell markers, initial studies using functional approaches identified candidate epithelial and stromal endometrial stem/progenitor cells as colony forming cells/units (CFU) (1). Further evaluation of key stem cell properties of individual CFU demonstrated that rare EpCAM+ epithelial cells and EpCAM- stromal cells underwent self renewal by serial subcloning >3 times and underwent >30 population doublings in culture. Clonally-derived epithelial cells differentiated into cytokeratin+ gland-like structures. Single stromal cells were multipotent as they differentiated into 4 mesodermal lineages; myogenic, adipogenic, osteoblastic and chondrogenic, suggesting that human endometrium contains a rare population of epithelial progenitor cells and mesenchymal stem cells (MSC) (2). Transplantation of freshly isolated human endometrial cells into immunocompromised mice reconstructed endometrial tissue that responded to estrogen and progesterone (3). Endometrial MSC can be prospectively isolated by co-expression of CD146 and PDGFRβ (4), but not Stro-1, a bone marrow MSC marker (5). Currently there are no known markers of endometrial epithelial progenitor cells. Endometrial cancer tissue harbours a small subpopulation of clonogenic, self-renewing, tumour-initiating cells, producing tumours that recapitulate parent tumours in histoarchitecture and differentiation markers (ERα, PR, cytokeratin, vimentin) when xenografted into mice, suggesting they are cancer stem cells. Candidate epithelial and stromal stem/progenitor cells have been identified in mouse endometrium as label retaining cells (LRC) in the luminal epithelium and perivascular cells at the endometrial-myometrial junction, respectively (6). It is likely that endometrial stem/progenitor cells play key roles in the development of gynecological diseases associated with abnormal endometrial proliferation such as endometriosis and endometrial cancer (7).

512 Background: Pazopanib is a standard treatment for metastatic renal cell carcinoma (mRCC) and 800 mg/day is considered the optimal dose for mRCC patients (pts). However, some pts require a dose reduction due to toxicity. It remains unclear whether reduced-dose pazopanib is as effective as the standard dose in achieving a response. Methods: We retrospectively evaluated treatment duration, objective response rate (ORR), progression-free survival (PFS), and discontinuation rate in consecutive pts with mRCC treated with first-line pazopanib between 2011 and 2016 at the Istituto Nazionale Tumori of Milano, Italy. Three patient groups were compared: group 1 received the standard starting dose of 800 mg/day continously, group 2 received a dose reduced to 400 or 600 mg/day after starting with 800 mg/day, and group 3 received a reduced starting dose of 400 or 600 mg/day because of ECOG performance status = 2-3 and/or comorbidities. Results: We included 69 pts, with 34 in group 1, 19 in group 2, and 16 in group 3. Median age at diagnosis was 62 years, and 64% were male. Overall 13% and 87% of pts were classified as Heng good and intermediate-risk respectively. In 10 and 9 pts of the group 2, the dose was reduced to 600 and 400 mg/day respectively while 12 and 4 pts in the group 3 received a reduced initial dose of 400 and 600 mg/day respectively. After a median follow-up of 13.9 months (range 0.3-43.8), 27 (39.1%) pts showed progressive disease (PD) and 3 (4.3%) pts were dead. Incidence rate of PD or death was 2.5 (95% CI: 0.6-4.4; Hazard ratio [HR]:1) per 100 person-months in group 1; 4.0 (95% CI: 0-11.4; HR: 1.45) per 100 person-months in group 2 and 3.3 (95% CI: 0-6.8; HR: 1.19) per 100 person-months in group 3. Rates of discontinuation due to PD were 28% in group 1, 42% in group 2, and 44% in group 3. ORR was 44%, 11% and 19% in group 1, group 2, and group 3 respectively. Conclusions: Our results suggest that mRCC pts receiving a lower dose of first-line pazopanib might not have a meaningful PFS advantage compared with those receiving standard dose. These data highlight the importance of management of the treatment-related side effects that may eventually lead to optimal drug exposure.

Abstract Introduction: The standard of treatment for SMM is watchful waiting. PVX-410 (OncoPep, Inc.) is being developed for the treatment of SMM to stop progression to active MM. Methods: PVX-410 consists of 4 HLA-A2 restricted, synthetic 9-mer peptides from unique regions of 3 multiple myeloma (MM)-associated antigens (XBP1 US184-192; XBP1 SP367-375; CD138260-268; and CS1239-247) emulsified in Montanide® ISA-720 VG (Seppic, Inc.). Adults with SMM at high risk of progression to active MM (ie, ≥2 of the following risk factors: serum monoclonal [M]-protein ≥3 g/dL; bone marrow clonal plasma cells >10%; and/or abnormal serum free light chain ratio [0.26-1.65]) and HLA-A2+were eligible. The primary objective of this study phase was to determine the tolerability of PVX-410 as a single agent. Immune response to PVX-410 and change in M protein were also assessed. This ongoing study is similarly evaluating PVX-410 co-administered with lenalidomide (Celgene Corp.). Patients received 6 doses of PVX-410 subcutaneously plus 0.5 mL (1 mg) Hiltonol®(poly-ICLC, Oncovir, Inc.) intramuscularly over 10 weeks. Patients initially received PVX-410 0.4 mg (3 patients, 0.1 mg/peptide / 0.4 mg total dose; low-dose) with escalation to 0.8 mg (9 patients, 0.2 mg/peptide / 0.8 mg total dose; target-dose). Patients were followed for 12 months post-treatment. Blood for immune response evaluation is collected at Week 0 (Baseline; pre-dose), 2, 4, and 8 during treatment and at Months 1, 3, 6, 9, and 12 post-treatment; disease response is assessed at the same timepoints, except Weeks 0 and 2. Immune response to PVX-410 was determined by interferon-gamma (IFN-γ) enzyme-linked immunosorbent spot (ELISPOT) and multimer assays. Samples were tested for IFN-γ production in response to peptide stimulation using ELISPOT and analyzed according to the distribution free sampling (DFR) method (Moodie et al, 2010). A positive (+) response was defined as 1) DFR+ at the timepoint (p-value ≤0.05); and 2) >2-fold increase from Baseline. A result also was considered + if the Baseline value was negative and the post-dose value was DFR+. For determination of peptide-specific memory CD8+ T cell responses, a flow cytometry-based major histocompatibility complex (MHC) multimer assay was performed using tetramers specific for HLA-A2 and each of the 4 peptides. A + response was defined as a value ≥2 X the negative control; ≥Baseline; or, if the negative control or Baseline value was 0.0 or 0.1, the patient result was ≥0.03. Results: Twelve patients were enrolled (3 low-dose; 9 target-dose). Ten subjects were male and all were white. Median age was 56 years. Median time since diagnosis was 0.8 years (range 0.3 to 7.3 years); time since diagnosis was <1 year for 8 patients. All patients had at least 2, and 6 had all 3 high-risk factors. Results are available for all 12 patients through at least 1 month post-treatment. All 12 patients had a positive immune response to at least 1 peptide, as measured by IFN-γ ELISPOT: XBP1 US184-192 (9/12); XBP1 SP367-375 (8/12); CD138260-268 (5/12); and CS1239-247(2/9). DFR-positivity to 1, 2, 3, and all 4 peptides was seen in 5, 3, 3, and 1 patient, respectively. The multimer assay was performed for 9 patients, of whom 7 had vaccine-induced, peptide-specific memory CD8+ T cells: XBP1 US184-192 (7/9); XBP1 SP367-375 (4/9); CD138260-268 (2/9); and CS1239-247(1/9). Five subjects, 2 of 3 in the low-dose and 3 of 9 in the target-dose cohorts, experienced progression to active disease within 9 months post-treatment. Seven patients had stable disease at last follow-up (median follow up of 9 months). PVX-410 was well-tolerated. All adverse events (AEs) were ≤Grade 2 and non-serious. Study vaccine-related AEs generally occurred within the first 2 days post-dose and consisted of systemic symptoms and local reactions commonly seen with vaccines (eg, fever, chills, fatigue, nausea, and other flu-like symptoms / localized erythema, induration, pain, rash, and pruritus). Conclusions: Six doses of PVX-410 were well tolerated in 12 patients with SMM. All 12 patients showed an immune response to the vaccine, with 4 having an immune response to ≥3 of 4 peptides. Based on these promising findings, PVX-410 is being investigated in combination with 3 cycles of fixed-dose lenalidomide. Given its immunomodulatory properties, it is hypothesized that co-administration of lenalidomide will enhance the T cell-mediated immune response induced by PVX-410. AbstractThe mullein bug, Campylomma verbasci (Meyer), was successfully established on eggplant cv. “Black Beauty” using the twospotted spider mite, Tetranychus urticae Koch, as a source of prey for early instars. Colony-raised teneral females lived 17.4±0.8 days (mean±SE) at 25°C, and their progeny averaged 6.7±0.6 nymphs per 48-h oviposition period (range 0–36). Fecundity peaked during days 6–10 and declined steadily beyond day 16. Over their lifespans, females produced eggs that hatched into 37.5±8.2 nymphs (range 2–184). Eggs from a 24-h oviposition period required from 7.2±0.1 days at 27°C to 13.0±0.2 days at 20°C for 100% hatch. An estimated developmental threshold of 9.5°C (r2 = 0.50) was obtained using the equation: rate of development = −0.075 + 0.008 (temperature). The frequency distribution of hatch at each of four constant temperatures was unimodal and approximated a normal distribution. Nymphal development at 22°C required 2.8±0.2, 3.8±0.2, 4.0±0.3, 3.9±0.2, and 6.0±0.3 days for instars 1–5, respectively.

IntroductionDBS can improve motor deficit in Parkinson’s disease (PD) patients. Existing devices have limitations due to electrode positioning errors, fallible manual programming and delivery of continuous ‘open-loop’ stimulation despite fluctuating patient state. This results in partial efficacy, adverse effects and increased cost. One solution is to use an electrical feedback signal or ‘biomarker’ recorded from DBS electrodes. The most widely studied signal has been spontaneous local field potentials (LFPs), particularly beta band (13–30 Hz) and high frequency oscillations (HFO) (200–400 Hz). Here, we report a novel biomarker in the form of a large amplitude, evoked potential, with a characteristic oscillatory decay, termed evoked resonant neural activity (ERNA).1MethodsLFPs and ERNA were recorded in 14 patients with PD (28 hemispheres) undergoing STN DBS surgery. The four contacts in each electrode array were ranked according to ERNA amplitude, beta power, HFO power and proximity to the anatomically ideal stimulation location. At least 3 months after surgery, motor scores (UPDRS III, reaction time) were evaluated off-DBS and during stimulation delivered through each electrode contact in a randomised order.ResultsERNA amplitude, beta power and contact proximity to the anatomically ideal stimulation location predicted magnitude of therapeutic response to DBS. However, after exclusion of covariance, ERNA amplitude remained the only significant predictor of DBS response.ConclusionERNA is a readily recordable, large amplitude signal that accurately correlates with motor response to DBS. It holds significant potential as a biomarker for guiding electrode implantation, ideal contact selection, automated parameter fitting and delivery of closed-loop DBS.ReferenceSinclair NC, McDermott HJ, Bulluss KJ, Fallon JB, Perera T, Xu SS, et al. Background: There exists little information in the relevant literature regarding tunnel enlargement after posterior cruciate ligament (PCL) reconstruction (PCLR). Purpose: To sequentially evaluate tunnel enlargement and radiographic posterior laxity through double-bundle PCLR using autologous hamstring tendon grafts. Study Design: Case series; Level of evidence, 4. Methods: We prospectively analyzed 13 patients who underwent double-bundle PCLR for an isolated PCL injury. Three-dimensional computed tomography images were obtained at 3 weeks, 6 months, and 1 year postoperatively, and the tunnel enlargement was calculated by sequentially comparing the cross-sectional areas of the bone tunnels. We also sequentially measured radiographic posterior laxity. The correlation between the tunnel enlargement ratio and the postoperative increase in posterior laxity was evaluated. Results: The cross-sectional area at the aperture in each tunnel significantly increased from 3 weeks to 6 months ( P < .003), but it did not continue doing so thereafter. The 6-month tunnel enlargement ratios of the femoral anterolateral tunnel, the femoral posteromedial tunnel, the tibial anterolateral tunnel, and the tibial posteromedial tunnel were 31.6% ± 23.5%, 90.3% ± 54.7%, 30.5% ± 26.8%, and 49.6% ± 37.0%, respectively, while the corresponding ratios at 1 year were 28.1% ± 19.8%, 83.1% ± 56.9%, 26.8% ± 32.8%, and 47.6% ± 39.0%, respectively. The posterior laxity was 9.0 ± 4.0 mm, −1.5 ± 2.3 mm, 3.4 ± 2.0 mm, and 3.9 ± 1.9 mm, preoperatively, immediately after surgery, 6 months and 1 year postoperatively, respectively. From the immediate postoperative period, the posterior laxity significantly increased at 6 months postoperatively ( P < .001), but it did not thereafter. The postoperative increase in posterior laxity had a significant positive correlation with the anterolateral tunnel enlargement ratio in both femoral and tibial tunnels at 6 months (ρ = 0.571-0.699; P = .011-.041) and 1 year (ρ = 0.582-0.615; P = .033-.037). Conclusion: Tunnel enlargement after PCLR mainly occurred within 6 months, with no progression thereafter. The anterolateral tunnel enlargement positively correlated with postoperative increase in posterior laxity.

Background:Rheumatic diseases such as rheumatoid arthritis (RA) and spondyloarthritis (SA) may occur among women of childbearing age. Adverse events during pregnancy including disease flare, preterm delivery, and neonatal or fetal death have been reported. Objectives:Our aim was to assess the impact of rheumatic diseases on the course of pregnancy. Methods:We conducted a cross-sectional study involving patients with RA (2010 ACR/EULAR criteria) and SA (2009 ASAS criteria) (study group) as well as healthy controls (control group) matched by age and gender. All women included had at least one pregnancy. Data were collected through telephonic interviews. We used the Student t test to compare the study group and the control group. Results:We enrolled 57 patients (30 RA and 27 SA) and 57 controls. The mean age in the study and the control groups were respectively 43.2 ± 8.2 years [26-48] and 37.5 ± 6 years. The mean chronic inflammatory rheumatisms duration was 13.81 ± 6.2 years. A history of primary sterility was found in 3 patients (1 RA and 2 SA) and none in the control group. The mean age during the first pregnancy was significantly higher in the study group (28 ± 6.2 years versus 24 ± 7 years in the control group) (p=0.01). Eight patients (5 RA and 3 SA) had a history of spontaneous miscarriage. A terminated pregnancy was noted in 3 patients (1 RA and 2 SA). Complications during pregnancy in the study group were gestational diabetes (0.3%), premature delivery (0.3), premature rupture of membranes (0.3%), abortion threat (0.3%) and pre-eclampsia (6%). Pregnancy was more associated with complications in the study group (p=0.05). Cesarean section was more used in the study group (28 patients versus 9 in the control group; p= 0.00). The main indications of cesarean section in the study group were macrosomia (11 patients), scar uterus (6 patients), sacroiliitis (4 SA patients), twin pregnancy (2 patients), and undetermined reasons (6 patients). Fetal presentation in the study group was the seat presentation (3%), top presentation (4%), face (2%), and forehead presentation (0.3%) with no significant difference with the control group. Complications of childbirth in the study group were hemorrhage of delivery (10.3%), cord widening (6.6%), perinatal asphyxia (4.9%), and dystocia (1.9%). However, no statistically significant differences were found between the two groups in the prevalence of complications of childbirth. During pregnancy, 5 patients were on salazopyrine, 2 on corticosteroids and, 1 on non-steroidal anti-inflammatories.Conclusion:Our study showed that pregnancies with rheumatic diseases were at increased risk of having maternal complications and adverse neonatal outcomes.Disclosure of Interests:None declared

AbstractAcute mountain sickness (AMS) can occur upon rapid ascent from low to high altitude. This study examined the association between central adiposity and the development of AMS in young adults during a high-altitude hike. Total and regional body fat were measured at sea level using dual-energy X-ray absorptiometry. Within 24 h of arriving in Cusco, Peru (3 400 meters) participants embarked on a 14-mile hike across the Andes Mountain range in southern Peru. Symptoms of AMS were assessed using the Lake Louise score at 24 h (3 400 meters), 29 h (4 100 meters), 34 h (3 800 meters) and 53 h (2 900 meters). 14 participants (mean age 21±2 years; women: 11, men: 3) completed the study. The number of participants exhibiting at least mild AMS increased from 6 (54%) at 3 400 meters to 9 (64%) at 3 800 meters. A higher AMS score at 4 100 meters was associated with greater android (r=0.72, p<0.01), trunk (r=0.73, p<0.01) and total body (r=0.71, p<0.01) fat, but not with total body fat % (r=0.39, p=0.16). Our findings suggest that central obesity, but not total body fat per se, may be an important factor in the development of AMS.

Nanocrystalline and porous barium titanate (BaTiO3) nanofibers with diameter 200-400 nm were synthesized via electrospinning and followed calcinations. The morphology and microstructure of the nanofibers were characterized using field emission scanning electron microscope, X-ray diffractometer and transmission electron microscope, respectively. And the electrical and humidity sensing properties of the nanofibers were also measured. The results reveal that the BaTiO3 nanofibers have a conductivity of about 0.3 S/cm, and show an ultrafast response time (~0.7 s) and a recovery time (~0.4 s) to humidity at room temperature. In addition, the sensing mechanism was also discussed briefly based on its nanocrystalline and porous microstructure of the electrospun material.

Purpose The purpose of this paper is to investigate how the 401(k) auto-enrollment rate influences the size of elected contribution rates in defined contribution plans for new, young enrollees. Design/methodology/approach The authors survey 324 undergraduate students at a mid-sized Midwestern university, and compare the elected contribution rates for two groups who were randomly given two default rates: 3 and 15 percent. Findings The results indicate widespread evidence of the anchoring and adjusting heuristic in regards to the provided auto-enrollment rate, as the 3 percent default rate group selects a contribution rate of approximately 2 percent less than the group that was provided with the 15 percent default rate. The results also provide support to the benefits of financial education: those who were taking or had already taken a college-level finance course provide higher contribution rates by about 1.7 percent overall. Additionally, individuals with the lowest critical thinking skills elect approximately 2 percent less in annual contributions overall than those who demonstrate higher critical thinking skills. Originality/value Interestingly, all groups seem to be susceptible to the anchoring and adjustment heuristic, as the default rate plays a significant role in the elected contribution rate, regardless of an individual’s financial sophistication or critical thinking skill level. The authors hope that these findings prompt benefit plan administrators and policy-makers to reconsider default rates in their retirement plans that would allow for maximum savings and participation rates. The findings also speak in favor of developing programs that would assist enrollees with financial education and critical thinking skills that would yield better retirement savings decisions when asked to make their employee benefit selections.

7050 Background: Treatment of older pts with AML is often complicated by comorbidities and pts with comorbidities are often underrepresented in clinical trials. The HCT-CI, which was developed in pts receiving allogeneic SCT, has been applied to pts receiving induction therapy for AML in an effort to assist in therapeutic and investigational decisions (Kantarjian 2006; Etienne 2007; Giles 2007). HCT-CI scores have been shown to be predictive of early death and survival in pts ≥ 60 years receiving induction therapy for AML, with early death rates of 3%, 11%, and 29% for pts with HCT-CI scores of 0, 1–2, and ≥ 3, respectively (Giles 2007). Methods: 140 pts age ≥ 60 with poor risk de novo AML from two phase II studies were scored for comorbidity by HCT-CI. In these studies, poor risk was defined by the presence of at least one risk factor: age ≥ 70, ECOG PS = 2, unfavorable cytogenetics, or cardiac, pulmonary, or hepatic dysfunction. All pts received induction therapy with 600 mg/m2 laromustine as a single 30–60 min infusion. A second induction cycle could be administered to pts with PR or hematologic improvement. Pts with CR or CRp were able to receive consolidation with laromustine 400 mg/m2 (Study CLI-033) or araC 400 mg/m2/day CIV for 5 days (Study CLI-043). Results: HCT-CI score was 0 in 7 (5%) pts, 1–2 in 19 (14%), and ≥ 3 in 114 (81%). The median HCT-CI score was 5 (range 0–13). The most common comorbidities were cardiac (42%), severe pulmonary (39%), infection (35%), and arrhythmia (31%). 52 (37%) of all pts achieved a CR/CRp; 39/114 (34%) of pts with HCT-CI score ≥ 3 achieved a CR/CRp. Deaths within 30 days of first induction were observed in 0/7 pts with HCT-CI score 0, 4/19 (21%) with score 1–2, and 16/114 (14%) with score ≥ 3. Overall survival at 12 months was 43%, 14%, and 21% for pts with HCT-CI scores of 0, 1–2, and ≥ 3, respectively. Conclusions: The majority (81%) of these older poor risk AML pts treated with laromustine had a HCT-CI score ≥ 3, confirming the poor risk nature of this patient group. The induction death rate for pts treated with laromustine and with HCT-CI score ≥ 3 was lower than that reported for a group of pts with HCT-CI score ≥ 3 treated with standard induction chemotherapy (14% vs 29%; Giles 2007). The results of a study of the effect of partial substitution of Sr2+ ions by Ba2+ ions on the thermophysical and magnetocaloric properties of manganite La0.7Sr0.3-xBaxMnO3 (x = 0; 0.02; 0.05 and 0.10) in the temperature range 100-400 K and in a magnetic field up to 1.8 T. It is shown that such a substitution leads to a significant decrease in TC. In the behaviour of the thermal diffusivity (T) and thermal conductivity (T) near TС, minima were found that are associated with both the scattering of phonons by local distortions of the crystal lattice and by spin fluctuations. The absolute values of thermal conductivity decrease with an increase in the disorder parameter. The magnetocaloric effect exhibits a weak dependence on the substitution level (x).

Purpose:To investigate the effect of 3 swim-pacing profiles on subsequent performance during a sprint-distance triathlon (SDT). Methods:Nine competitive/trained male triathletes completed 5 experimental sessions including a graded running exhaustion test, a 750-m swim time trial (STT), and 3 SDTs. The swim times of the 3 SDTs were matched, but pacing was manipulated to induce positive (ie, speed gradually decreasing from 92% to 73% STT), negative (ie, speed gradually increasing from 73% to 92% STT), or even pacing (constant 82.5% STT). The remaining disciplines were completed at a self-selected maximal pace. Speed over the entire triathlon, power output during the cycle discipline, rating of perceived exertion (RPE) for each discipline, and heart rate during the cycle and run were determined. Results:Faster cycle and overall triathlon times were achieved with positive swim pacing (30.5 ± 1.8 and 65.9 ± 4.0 min, respectively), as compared with the even (31.4 ± 1.0 min, P = .018 and 67.7 ± 3.9 min, P = .034, effect size [ES] = 0.46, respectively) and negative (31.8 ± 1.6 min, P = .011 and 67.3 ± 3.7 min, P = .041, ES = 0.36, respectively) pacing. Positive swim pacing elicited a lower RPE (9 ± 2) than negative swim pacing (11 ± 2, P = .014). No differences were observed in the other measured variables. Conclusions:A positive swim pacing may improve overall SDT performance and should be considered by both elite and age-group athletes during racing.

Abstract Background and study aims Per-oral endoscopic myotomy (POEM) has emerged as an efficacious treatment modality for the management of achalasia cardia (AC) and non-achalasia spastic esophageal motility disorders. Initial results are encouraging. We analyzed the safety and efficacy of POEM in a large cohort of patients with AC. Patients and methods The data from patients who underwent POEM (from January 2013 to June 2016) was prospectively collected and analyzed. Clinical success was defined as Eckardt score ≤ 3 after POEM procedure. Objective parameters including high-resolution manometry (HRM) and timed barium swallow (TBS) were analyzed and compared before and after the procedure. Gastroesophageal reflux was analyzed using 24-hour pH impedance study and esophagogastro-duodenoscopy. Results A total of 408 patients (mean age 40 years, range 4 – 77 years) underwent POEM during the specified period. POEM could be successfully completed in 396 (97 %) patients. Clinical success rates at 1, 2 and 3 years were 94 %, 91 % and 90 %, respectively. Mean Eckardt score was 7.07 ± 1.6 prior to POEM and 1.27 ± 1.06 after POEM (P = 0.001) at 1 year. Significant improvement in esophageal emptying on TBE (> 50 %) was documented in 93.8 % patients who completed 1-year follow up. Pre-procedure and post-procedure mean lower esophageal sphincter pressure was 45 ± 16.5 mmHg and 15.6 ± 6.1 mmHg, respectively (P = 0.001). Technical and clinical success were comparable in naïve vs prior treated cases (97.3 % vs 96.8 %, P = 0.795) (95.7 % vs 92.6 %, P = 0.275). GERD was documented in 28.3 % patients with 24-hour pH-impedance study and erosive esophagitis was seen in 18.5 % of patients who underwent POEM. Conclusions POEM is safe, effective and has a durable response in patients with achalasia cardia. Prior treatment does not influence the outcomes of POEM.

2 Background: The CA184-043 phase 3 study did not reach statistical significance for its primary endpoint of OS (HR=0.85, p=0.053). However, antitumor activity was observed in other efficacy endpoints, including progression-free survival. Prespecified subset analyses were performed to understand if any prognostic features may identify mCRPC patients (pts) more likely to benefit from Ipi treatment. Methods: 799 pts were randomized to receive a single dose of radiotherapy (RT) followed by either Ipi (N=399) or Pbo (N=400). Prespecified subset analyses based on Kaplan-Meier/Cox methodology were performed using known prognostic factors for OS in mCRPC. Results: Prespecified subset analyses suggested that Ipi may be more active in pts with favorable prognostic factors, including no visceral disease, alkaline phosphatase <1.5 ULN, and hemoglobin ≥11 g/dL (Table). The safety profile in this study was consistent with previous reports of Ipi. Conclusions: Based on these subset analyses, Ipi added to RT appears to have greater activity than RT alone in pts with a favorable prognostic profile. These results support continued investigation of Ipi in the ongoing CA184-095 study in chemotherapy-naive mCRPC pts. Clinical trial information: NCT00861614. [Table: see text]

Objective. To clarify the frequency of insulin resistance (IR) in patients with systemic lupus erythematosus (SLE), traditional and associated with rheumatic disease risk factors for its development, to assess the possibility of using the Finnish Type 2 Diabetes Risk Assessment Score (FINDRISC) questionnaire to detect IR.Material and methods. The cross-sectional study included 49 patients with SLE (46 women, 3 men) without diabetes mellitus and hyperglycemia, observed at the V.A. Nasonova Research Institute of Rheumatology in 2019–2020. The median age of the patients was 40 [33; 48] years, the duration of the disease was 3.0 [0.7; 8.0] years. Glucocorticoids (GC) were received by 41 (84%) patients, hydroxychloroquine – by 38 (78%), immunosuppressive drugs – by 10 (20%), biological agents – by 5 (10%). The glucose and fasting immunoreactive insulin levels were examined, and the Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) index was calculated in all patients. HOMA-IR value ≥2.77 corresponded to IR. Traditional risk factors for type 2 diabetes and the risk of its development in the next 10 years in patients with SLE were assessed using the Russian version of the FINDRISC questionnaire.Results. The median HOMA-IR level in SLE patients was 1.7 [1.2; 2.5]. IR was detected in 10 (20%) of 49 patients with SLE. Patients with and without IR were comparable in terms of sex, age, duration and activity of SLE, therapy, and type 2 diabetes traditional risk factors. BMI, WC and insulin levels were higher in patients with IR. HOMA-IR correlated with body mass index (BMI) (r=0.6; p<0.001), waist circumference (WC) (r=0.5; p<0.001), risk categories for developing type 2 diabetes according to FINDRISС (r=0.3; p=0.03), SLEDAI-2K index (r=–0.4; p<0.01), C3 complement levels in serum (r=0.3; p=0.04) and the duration of GC therapy (r=0.3; p=0.03).Conclusion. IR was diagnosed in 20% of SLE patients without a history of diabetes and with normal fasting glucose in venous blood. The lower SLE activity, the longer GC intake in patient, the higher the HOMA-IR index was detected in him. However, the IR appearance was reliably associated only with an increasing BMI and WC. The use of the FINDRISC questionnaire, which allows to stratify respondents in the general population by the risk of developing type 2 diabetes mellitus, did not help to identify SLE patients with IR.
