Abstract Purpose Measuring quality of life in acute asthmatics is challenging, especially when asthma attacks can occur sporadically. Several questionnaires can be used to measure quality of life in this patient group; however, psychometric testing is limited on questionnaires that can be used to estimate Quality Adjusted Life years. The objective of this study is to assess the construct validity (convergent and discriminative validity) and responsiveness of the EuroQol-5-Dimensions 5-Level (EQ-5D-5L), Asthma Quality of Life Utility Index-5 Dimensions (AQL-5D) and Time Trade-Off (TTO) in acute asthma patients. Methods Data from a prospective cohort study were used to test the validity and responsiveness of the EQ-5D-5L, AQL-5D and TTO in asthma patients who were recruited from UK accident & emergency departments or hospital wards. The spearman’s rank correlation coefficient, the Kruskal–Wallis test statistic and the standardized response mean were used to test for convergent validity, discriminative validity and responsiveness, respectively. Results One hundred and twenty-one participants were included in the available case analysis. The EQ-5D-5L and AQL-5D showed moderate to strong correlations for convergent validity at baseline, week 4 and week 8. The AQL-5D and TTO showed moderate correlations at week 4 and week 8. No statistical significance was observed for discriminative validity at baseline. Both the EQ-5D-5L and the AQL-5D also showed that they were sensitive to change for the recovery responses. Conclusions The EQ-5D-5L and AQL-5D showed stronger construct validity and responsiveness compared to the TTO. Therefore, both the EQ-5D-5L and AQL-5D should be considered for use in future economic evaluations.

Abstract From five-dimensional N = 4 matter-coupled gauged supergravity, smooth time-dependent cosmological solutions, connecting a dS 5−d × H d (with d = 2, 3) spacetime at early times to a dS 5 spacetime at late times, are presented. The solutions are derived from the second-order equations of motion arising from all the gauged theories that can admit dS 5 solutions. There are eight such theories constructed from gauge groups of the form SO(1, 1) × G nc and SO ( 1 , 1 ) diag ( n ) × G nc , with n = 2, 3, where G nc is a non-compact gauge factor whose compact part must be embedded entirely in the matter symmetry group of 5D matter-coupled supergravity. Furthermore, we analyze how the cosmological solutions and their corresponding dS 5 vacua cannot arise from the first-order equations that solve the second-order field equations.

We investigated the time course of training-induced changes in the expression of GLUT-1 and GLUT-4 in human skeletal muscle. Seven healthy males trained for 2 h/day (approximately 60% pretraining VO2peak) for 31 days (31D). Muscle biopsies were obtained before training (PRE) and after 5 (5D) and 31 days (31D) of training. Training resulted in progressive increases in muscle GLUT-4 with increasing training duration (PRE<5D<31D; P<0.01). Muscle GLUT-1 content was also increased (P<0.05) after training; however, the increase was not observed until 31D (131%). Increases in muscle hexokinase (HK) activity were complete by 5D (P<0.01). Muscle malate dehydrogenase activity was not elevated after 5D of training but was increased (+35%; P<0.01) at 31D. Results from this study show that increases in both GLUT-4 and HK represent early training-induced adaptations to prolonged exercise training. As training progresses, further increases in GLUT-4, but not HK, occur in conjunction with an increase in muscle mitochondrial potential and GLUT-1.

Objective: The 5-level EuroQoL (EQ-5D-5L) is a patient-reported outcome measure frequently used in stroke research. However, it does not assess the cognitive problems many patients with stroke experience. The aim of this article is to compare the content validity, internal consistency and discriminative ability of the EQ-5D-5L with and without an additional cognitive domain (EQ-5D-5L+C), administered three months post-stroke. Design: Cross-sectional study. Setting: Six general hospitals in the Netherlands. Subjects: In all, 360 individuals with stroke three months after the event. Interventions: Not applicable. Main measures: The modified Rankin Scale and EQ-5D-5L+C were administered in telephone interviews three months post-stroke. Results: A total of 360 patients with stroke were included. Mean age was 68.8 years (standard deviation (SD) = 11.7), 143 (40%) were female, 334 (93%) had had an ischemic stroke, 165 (46%) had a National Institutes of Health Stroke Scale (NIHSS) score ⩽ 4 at presentation and the Barthel Index was 17.2 (SD = 4) four days post-stroke. Cognitive problems were reported by 199 (55%) patients three months post-stroke. Internal consistencies of the EQ-5D-5L and EQ-5D-5L+C were 0.75 and 0.77, respectively. Adding a cognitive domain resulted in a decrease of the ceiling effect from 22% to 14%. Both EQ-5D-5L and EQ-5D-5L+C showed good discriminative ability, but differences between patients with different modified Rankin Scale scores and with/without reported decrease in health and daily activities were slightly larger with the EQ-5D-5L+C compared to the EQ-5D-5L. Conclusions: The EQ-5D-5L+C, which includes a cognitive domain that is highly significant for stroke patients, showed increased content validity and good discriminative ability, without losing internal consistency.

The spectroscopic properties of several fluoride crystals ( LiYF 4, KYF 4, K 2 YF 5, KLiYF 5, CsY 2 F 7, SrF 2) doped with Nd 3+, Er 3+ or Tm 3+ are analyzed from the viewpoint of their possible applications as active media for VUV solid state lasers. It has been found that in many crystals there exists very efficient nonradiative relaxation from higher-lying 5d states to the lowest 5d level responsible for the spin-forbidden luminescence. In fact, the energy level diagram of 5d–4f transitions for these two ions represents a typical four-level laser scheme with a considerably larger Stokes shift of 5d–4f luminescence from the edge of the strong spin-allowed 4f–5d absorption than for Nd 3+, which causes smaller reabsorption of emitted VUV radiation in the crystals doped with Er 3+ or Tm 3+. Two crystals LiYF 4: Nd 3+ and SrF 2: Er 3+ were tested for laser action under pumping by a pulsed F 2 laser operating at 157 nm. However, no indications of the appearance of stimulated emission have been detected for both crystals. The possible reasons for the absence of lasing are discussed.

(1) Background: The recent published version with five levels of response of EQ-5D-Y needs to be studied in children with chronic illness. For this, the aim of the present study was to assess and compare the psychometric properties of EQ-5D-Y-3L and EQ-5D-Y-5L in terms of feasibility, ceiling effect, redistribution properties, informativity and inconsistence responses in children with cancer. (2) Methods: A core set of self-report tools, including the Spanish version of EQ-5D-Y-3L and EQ-5D-Y-5L, were administered to children drawn from the population with cancer. EQ-5D-Y-3L and EQ-5D-Y-5L were evaluated in terms of feasibility, ceiling effects, redistribution properties and differences in absolute and relative informativity. (3) Results: A total of 73 children (9.7 ± 2.3 years old) from the population with cancer participated in the study. No missing data in the new EQ-5D-Y-5L were visualized, so the feasibility was acceptable. EQ-5D-Y-5L showed a low ceiling effect in all dimensions with relative changes from EQ-5D-Y-3L to EQ-5D-Y-5L of between 15.3% and 42.4% for the dimensions and 44.6% for the overall system. Compared to EQ-5D-Y-3L, EQ-5D-Y-5L provided a better distribution of the severity of the problem in the five levels of response. The absolute informativity (Shannon’s index) did not show statistically significant differences between EQ-5D-Y-3L and EQ-5D-Y-5L in all dimensions and the overall system. (4) Conclusions: EQ-5D-Y-5L is feasible, presenting a low ceiling effect and high discriminative power.

Background. The Medicare Health Outcomes Survey (HOS), a nationwide annual survey of Medicare beneficiaries, includes the Centers for Disease Control and Prevention’s HRQOL-4 questionnaire and Veterans RAND 12-item Health Survey (VR-12). This study compared EQ-5D scores derived from the HRQOL-4 (dEQ-5D) to SF-6D scores derived from VR-12. Methods. Data were from Medicare HOS Cohort 15 (2012 baseline; 2014 follow-up). We included participants aged 65+ ( n = 105,473). We compared score distributions, evaluated known-groups validity, assessed each index as a predictor for mortality, and estimated quality-adjusted life years (QALYs) using the dEQ-5D and SF-6D. Results. Compared to the SF-6D, the dEQ-5D had a higher mean score (0.787 v. 0.691) and larger standard deviation (0.310 v. 0.101). The decreases in estimated scores associated with chronic conditions were greater for the dEQ-5D than for the SF-6D. For example, dEQ-5D scores for persons with depression decreased 0.456 points compared to 0.141 points for the SF-6D. The dEQ-5D strongly predicted mortality, as adjusted hazard ratios for the first to fourth quintiles, relative to the fifth quintile, were 2.2, 1.7, 1.8, and 1.5, respectively, while the association between SF-6D and mortality was weaker or nonexistent (adjusted hazard ratios were 1.3, 1.1, 1.0, and 0.6, respectively). Compared to the SF-6D, QALYs estimated using the dEQ-5D were higher overall (5.6 v. 4.9 years), higher for persons with less debilitating conditions (e.g., hypertension, 5.0 v. 4.4 years), and lower for more debilitating conditions (e.g. depression, 2.5 v. 2.8 years). Conclusions. Compared to the SF-6D, the dEQ-5D was better able to measure individuals’ overall health; detect the differential impact of chronic conditions, particularly among persons in poorer health; and predict mortality. The HRQOL-4 questionnaire may be valuable for monitoring and improving health outcomes for the Medical HOS data set.

In this research, the pyrazoline pyridazine derivative 7-methyl-2-phenyl-4-(3,4,5-trimethoxyphenyl)-2H-pyrazolo[3,4-d]pyridazine (5d) was studied for its interaction with bovine serum albumin (BSA). Various spectroscopic techniques along with molecular docking analysis were utilized to understand the mechanism of interaction. The quenching of BSA fluorescence by using investigational drug 5d was the basic principle for the methodology. Spectrofluorometric methods and UV-absorption studies were conducted for exploration of the 5d and BSA binding mechanism. The fluorescence quenching mechanism involved in BSA and 5d interaction was static quenching, and a complex formation also occurred between them. Both enthalpy and entropy attained positive values suggesting involvement of hydrophobic forces in BSA and 5d interaction. The Förster distance of 2.23 nm was calculated by fluorescence resonance energy transfer (FRET). An alteration in BSA secondary structure was proven from the conformational studies of BSA-5d interaction. This binding interaction study provided a basis to comprehend the binding interaction between 5d and BSA. These results provided information about sites of BSA involved in its interaction with 5d.

The 4+1 formalism in general relativity (GR) prescribes field equations for the spacetime metric γμνx,τ which is local in the spacetime coordinates x and evolves according to an external “worldtime” τ. This formalism extends to GR the Stueckelberg Horwitz Piron (SHP) framework, developed to address the various issues known as the problem of time as they appear in electrodynamics. SHP field theories exhibit a formal 5D symmetry on (x,τ) that is strategically broken to 4+1 representations of the Lorentz group, resulting in a manifestly covariant canonical formalism describing the τ-evolution of spacetime structures as an initial value problem. Einstein equations for γμνx,τ are found by constructing a 5D pseudo-manifold (combining 4D geometry and τ-dynamics) and performing the natural foliation under broken 5D symmetry. This paper discusses weak gravitation in the 4+1 formalism, demonstrating the natural decomposition of the field equations into first-order evolution equations for the unconstrained 4D metric, and the propagation of constraints associated with the Bianchi identity.

It is shown that the 5D geodetic equations and 5D Ricci identities give us a way to create a new viewpoint on some problems of modern physics, astrophysics, and cosmology. Specifically, the application of the 5D geodetic equations in (4+1) and (3+1+1) splintered forms obtained with the help of the monad and dyad methods made it possible to introduce a new, effective generalized concept of the rest mass of the elementary particle. The latter leads one to novel connections between the general relativity and quantum field theories, and all that, including the (4+1) splitting of the 5D Ricci identities, brings about a better understanding of the magnetic monopole problem and the vital difference in the origins of the Maxwell equations and gives rise to surprising connections between them. The obtained results also provide new insight into the mechanism of the 4D universe’s expansion and its following acceleration.

Our previous study found that desmethylxanthohumol (1) inhibited α-glucosidase in vitro. Recently, further investigations revealed that dehydrocyclodesmethylxanthohumol (2) and its dimer analogue rottlerone (3) exhibited more potent α-glucosidase inhibitory activity than 1. The aim of this study was to synthesize a series of rottlerone analogues and evaluate their α-glucosidase and DPP-4 dual inhibitory activity. The results showed that compounds 4d and 5d irreversibly and potently inhibited α-glucosidase (IC50 = 0.22 and 0.12 μM) and moderately inhibited DPP-4 (IC50 = 23.59 and 26.19 μM), respectively. In addition, compounds 4d and 5d significantly promoted glucose consumption, with the activity of 5d at 0.2 μM being comparable to that of metformin at a concentration of 1 mM.

The structures were solved by direct methods and refined by full-matrix least-squares procedures to R(F, I [Formula: see text] 3σ(I)) = 0.049, 0.047, 0.042, and 0.047, respectively, for CCD data for 5d, 9a, 11b, and 19. Based upon hydrophobic feedback approaches, we designed and synthesized novel sulfur-containing ERα modulators (4 and 5) as breast cancer therapeutic drug candidates. The tetrahydrothiepine derivative 5a showed the highest binding affinity toward ERα because of its high hydrophobicity, and it acted as an agonist toward MCF-7 cell proliferation. The corresponding alkylamino derivative 5d maintained high binding affinity to ERα and potently inhibited MCF-7 cell proliferation (IC50: 0.09 μM). Docking simulation studies of compound 5d with the ERα BD revealed that the large hydrophobic moiety of compound 5d fit well into the hydrophobic pocket of the ERα LBD and that the sulfur atom of compound 5d formed a sulfur–π interaction with the amino acid residue His524 of the ERα LBD. These interactions play important roles for the binding affinity of compound 5d to the ERα LBD.

Abstract A new series of urea and thiourea derivatives containing benzimidazole group as potential anticancer agents have been designed and synthesized. The structures of the synthesized compounds were characterized and confirmed by spectroscopic techniques such as 1H NMR, 13C NMR, and mass spectrometry. In vitro anticancer assay against two breast cancer (BC) cell lines, MDA-MB-231ER(−)/PR(−) and MCF-7ER(+)/PR(+), revealed that the cytotoxicity of 1-(2-(1H-benzo[d]imidazol-2-ylamino)ethyl)-3-p-tolylthiourea (7b) and 4-(1H-benzo[d]imidazol-2-yl)-N-(3-chlorophenyl)piperazine-1-carboxamide (5d) were higher in MCF-7 with IC50 values of 25.8 and 48.3 µM, respectively, as compared with MDA-MB-231 cells. Furthermore, 7b and 5d were assessed for their apoptotic potential using 4′,6-diamidino-2-phenylindole, acridine orange/ethidium bromide staining, and Caspase-3/7. After incubation with MCF-7, the compounds 7b and 5d induced apoptosis through caspase-3/7 activation. In conclusion, the compounds 7b and 5d are potential candidates for inducing apoptosis in different genotypic BC cell lines.

The treatment of non-fully substituted 4,4-difluoro-4-bora-3a,4a-diaza-s-indacene (BODIPY) with cupric nitrate leads to the introduction of a nitro group at different positions of the BODIPY core, depending on the substitution pattern. This methodology complements the treatment of fully substituted BODIPY with cupric nitrate that was previously reported. The crystal structures of 4,4-difluoro-1,3,5,7,8-pentamethyl-2-nitro-4-bora-3a,4a-diaza-s-indacene, C14H16BF2N3O2 (5a) 4,4-difluoro-3-nitro-8-phenyl-4-bora-3a,4a-diaza-s-indacene, C15H10BF2N3O2 (5b) and 3-chloro-6-ethyl-5,7,8-trimethyl-2-nitro-4,4-diphenyl-4-bora-3a,4a-diaza-s-indacene, C26H25BClN3O2 (5d) are presented. In all three structures, the fused ring system is in a very flattened `V-shape', with dihedral angles between the two outer five membered rings of 8.12 (14), 6.67 (9) and 12.30 (18) Å for 5a, 5b and 5d, respectively. In each case, the central six-membered ring is in a flattened sofa conformation. In the crystal of 5a, molecules are linked by weak C—H...O and C—H...F hydrogen bonds forming sheets parallel to (10-1). In the crystal of 5b molecules are linked by weak C—H...O and C—H...F hydrogen bonds and π–π interactions forming sheets parallel to (001). In the crystal of 5d, weak C—H...O hydrogen bonds link molecules into chains along [001]. In compound 5d, the atoms of the nitro group were refined as disordered over two sets of sites with occupancies 0.618 (12) and 0.382 (12).

(1) Introduction: There is a growing interest in health-related quality of life (HRQOL) in adolescent population. The EQ-5D-Y is a generic HRQOL instrument that allows adolescents to understand the health status of different levels of physical, mental, and social health. This study was carried out with an adolescent population in Peru. The main objective of this article is to report the normative values of the EQ-5D-Y questionnaire in Peruvian adolescents. (2) Methods: The EQ-5D-Y questionnaire was administered to Peruvian adolescent students. A total of 1229 young people participated in the survey. The EQ-5D-Y score was reflected as a function of sex and age. (3) Results: The mean utility index of the EQ-5D-Y for the total sample was 0.890; this rating was significantly better for males at (0.899) and females at (0.881). The ceiling effect was higher for adolescent males with (47.3) females (40.7). (4) Conclusions: The results of the present study provide evidence that schooled adolescents show a positive perception of HRQOL.

The use of slurry-infiltrated fiber concrete (SIFCON) has been increasing in recent years. SIFCON is a very good alternative, especially in structural reinforcement processes. In this study, the effects of 2 different steel fibers of normal strength (3D) and high strength (5D) with different geometry and strength properties and polyolefin origin synthetic fiber are examined on the mechanical behavior and capillary water permeability properties of SIFCON. Steel fibers were used in 2 different ratios by volume 4% and 8%, while polyolefin synthetic fiber was used at 4% by volume. The bending strength and splitting tensile strength of SIFCON containing 5D steel fiber are 46.47 MPa and 18.47 MPa, respectively, 4.9 and 2.1 times higher than plain concrete. In addition, the fracture energy of SIFCON containing 5D steel fiber is 20400 N/m, and it is 358 times higher than plain concrete, 1.6 and 3.1 times higher than concrete containing the same amount of 3D fiber and polyolefin synthetic fiber, respectively. The capillary water absorption of SIFCON, which contains 4% synthetic fiber and 8% 3D steel fiber by volume, is 0.121 mm and 0.112 mm, respectively, which is higher than all other mixtures in the study. As a result of the study, higher splitting tensile strength, bending strength and fracture energy values were obtained in concretes containing 5D steel fiber, which have high tensile strength and have better adhesion to concrete due to its geometry. The use of synthetic fibers or high amounts of steel fibers increased the permeability.

Introduction: The aims of this study were to cross-culturally adapt and evaluate the validity of the Singaporean Malay and Tamil versions of the EQ-5D. Materials and Methods: The EQ5D was cross-culturally adapted and translated using an iterative process following standard guidelines. Consenting adult Malay- and Tamil-speaking subjects at a primary care facility in Singapore were interviewed using a questionnaire (including the EQ-5D, a single item assessing global health, the SF-8 and sociodemographic questions) in their respective language versions. Known-groups and convergent construct validity of the EQ-5D was investigated by testing 30 a priori hypotheses per language at attribute and overall levels. Results: Complete data were obtained for 94 Malay and 78 Indian patients (median age, 54 years and 51 years, respectively). At the attribute level, all 16 hypotheses were fulfilled with several reaching statistical significance (Malay: 4; Tamil: 5). At the overall level, 42 of 44 hypotheses related to the EQ-5D/ EQ-VAS were fulfilled (Malay: 22; Tamil: 20), with 21 reaching statistical significance (Malay: 9; Tamil: 12). Conclusion: In this study among primary care patients, the Singapore Malay and Tamil EQ-5D demonstrated satisfactory known-groups and convergent validity. It is shown, that (4+1)—and (3+1+1)—reductions of the geodetic equations in 5D theory with scalar field φ=−G55 and out of the 5D optics inevitably lead to the new concept of the Lorentz-type relativistic mass m^0 of the 5D test particle. Due to the imposition of the x5—cylindricity condition, one can obtain an integral of the 5D particle’s motion along x5; it gives its electric charge. Thus, one can obtain an exact expression for m^0. In this expression m^0 depends on the field φ through an electric charge, so there is not probably any need for a scalar charge in Nature at all. Furthermore, one can compactly express the m^0 through the mass angle χn and additionally hypothesize about the possible complex structure of m^0. It soundly leads one to the deeper understanding of the quantum properties of the matter. All of these conceptions also turn one to the idea that this mass m^0 may contribute to the recently discovered in the Universe dark matter and dark energy and also be one of the possible reasons for the Universe’s expansion. The next suggestive result in the 5D theory is connected with the (4+1)—reduction of the 5D Ricci identities. It leads one to certain connections between the 4D physicogeometrical values and permits one to obtain the first pair of Maxwell equations with the non-zero soliton-type r.h.s. and establishes the connection with the second pair of them. It subsequently leads one to the idea about the magnetic monopole’s existence in the early Universe. It is shown, that this non-zero r.h.s. vanishes together with the imposition of the x5—cylindricity condition. The peculiarity of this process permits one to hypothesize soundly about the existence of the topological second-order transition in the Universe, which leads one to the superfluid state and possibly accelerates its expansion.

Background:Sarcopenia has been defined as a loss of muscle mass and consequently of muscle function. In patients affected by osteoarthritis (OA) a more likely and accelerated development of sarcopenia has been reported. The SARC-F is a simple sarcopenia screening tool includes five assessment items: strength, assistance walking, rising from a chair, climbing stairs, and falls. SARC-F ≥ 4 is defined as sarcopenia.Objectives:The present study aimed to examine the utility of SARC-F in the patients with knee osteoarthritis.Methods:Patients with radiographic and clinic evidence of tibiofemoral OA (Kellgren-Lawrence score ≥2) were included. Sarcopenia were identified using the SARC-F scale. Patients with a total score 4 and higher than 4 were classified as having sarcopenia. Patients were assessed by The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), four-meter walking test, hand grip test, shortened version of the falls efficacy scale-international (the short FES-I) and EuroQol- 5 Dimension (EQ-5D). A multiple linear regression model was used to identify independent predictors of SARC-F.Results:A total 76 patients with median age 61 ranged 55 to 78 years old (72.4% female) were screened in this study (Table 1). Of the sample, 38.1% of patients were sarcopenic as a SARC-F (Figure 1). Through multiple regression analyses, SARC-F scores was significantly associated with four-meter walking test, hand grip, WOMAC-Function, EQ-5D and short FES-I describing between 57% and 63% of the variance (adjusted R2) (Table 2). A new series of 2-[3-(5-methyl-1-phenyl-1H-1,2,3-triazol-4-yl)-1-phenyl-1H-4-pyrazolyl]-3-aryl-1,3-thiazolan-4-one 5(a-i) have been designed, synthesized and evaluated for their in vitro antibacterial activity against Gram positive bacteria viz. Bacillus subtilis (ATCC 6633), Staphylococcus aureus (ATCC 6538p), Micrococcus luteus (IFC 12708) and Gram negative bacteria viz. Proteus vulgaris (ATCC 3851), Salmonella typhimurium (ATCC 14028), Escherichia coli (ATCC 25922) the antifungal activity against Candida albicans (ATCC 10231), Aspergillus fumigatus (HIC 6094), Trichophyton rubrum (IFO 9185), Trichophyton mentagrophytes (IFO 40996). Antibacterial evaluation indicates that compounds containing 4-methoxyphenyl 5c, 4-fluorophenyl 5d and 2,5-difluorophenyl 5h groups on thiazolidinone ring showed significant activity equal to that of standard drug. The antifungal evaluation shows that compound 5c is highly active against A. fumigatus, compound 5d and 5h were also active against C. albicans and A. fumigatus.

A convenient synthesis of novel1,4-disubstituted 1,2,3-triazoles (4a-j & 5a-j) is reported via copper (I) - catalyzed one pot [3+2] cycloaddition of various alkyl halides, sodium azide with (prop-2-yn-1-yl)thiomorpholine and 4-(prop-2-yn-1-yl)thiomorpholine 1,1-dioxide. All the synthesized compounds were investigated for their antimicrobial activity. Compounds 4a, 4b, 4c, 4g, 5a and 5j against S.epidermidis, 4a, 5a and 5d against P. aeroginosa, 4a, 4b and 4g against K.pneumoniae, 4b, 5a and 5d against S.aureus and 5b, 5e and 5j against B.Subtilis have shown excellent antibacterial activity compared to the standard drugs Penicillin and Streptomycin. Compounds 4c, 4e, 4f, 4j, 5c, 5d, 5g and 5j have registered moderate antifungal activity as compared with standard drug Ampothericin-B.

ObjectivesTo determine the response process validity, feasibility of completion, acceptability and preferences for three patient-reported outcome measures that could be used in economic evaluation—the EQ-5D-5L, ICECAP-A and ICECAP-O—in people requiring kidney care.DesignParticipants were asked to ‘think-aloud’ while completing the EQ-5D-5L, ICECAP-A and ICECAP-O, followed by a semistructured interview. Five raters identified errors or struggles in completing the measures from the think-aloud component of the transcripts. Patient preferences for measures were extracted from the semistructured interview.SettingEligible patients were identified through a large UK secondary care renal centre.ParticipantsIn total, 30 participants were included in the study, consisting of patients attending renal outpatients for chronic kidney disease (n=18), with a functioning kidney transplant (n=6) and receiving haemodialysis (n=6).ResultsParticipants had few errors and struggles in completing the EQ-5D-5L (11% error rate, 3% struggle rate), ICECAP-A (2% error rate, 2% struggle rate) and ICECAP-O (4% error rate, 3% struggle rate). The main errors with the EQ-5D-5L were judgements that did not comply with the ‘your health today’ instruction. Comprehension errors were most prominent on ICECAP-O. Judgement errors were the only errors reported on ICECAP-A. Although the EQ-5D-5L had slightly more errors and struggles, it was the measure most preferred, with participants able to make a clearer link with EQ-5D-5L and their health condition.ConclusionsThe EQ-5D-5L, ICECAP-A and ICECAP-O are feasible for people requiring kidney care to complete and can be included in studies conducting economic evaluations of kidney care interventions. Further research is required to assess how health (eg, EQ-5D) and capability (eg, ICECAP) measures can be included in an economic evaluation simultaneously, as well as what ICECAP measure(s) to include when patient groups straddle the age ranges for ICECAP-A (18 years and older) and ICECAP-O (65 years and older).

The double phosphate KLuP2O7 doped with Pr3+ ions was prepared by solid-state reaction. The material obtained was characterized by powder X-ray diffraction. The luminescence spectroscopy and the excited state dynamics of this material were studied upon excitation with VUV synchrotron radiation. The 5d-4 f emission of Pr3+ upon both direct and band gap excitation was detected and assigned. The decay kinetics of the Pr3+ 5d-4 f emission is characterized by a decay time of about 20 ns and is nearly temperature independent within the range 8 - 300 K. Both dynamics and energy transfer peculiarities revealed in the study suggested good potentials for application of KLuP2O7:Pr3+ as a fast scintillator material.

On the basis of this theory, we construct a 4D physical space–time by choosing a certain supersurface equation F(xA) = 0 (A runs over 0 ~ 4). It is shown that, the 4D two-fluid FRW cosmological model can be obtained from 5D flat space–time by specifying F(xA) = 0 as x4-x0 = 0.

Aim The purpose of this prospective multicenter study was to test the predictive value of cortical comminution and intra-articular involvement on function and quality of life in distal radius fractures (DRFs) using the Buttazzoni's classification system. Patients and Methods We studied 406 patients between skeletal maturity and 74 years of age with DRF. Fractures with acceptable radiographic alignment were immobilized with a cast. Fractures with nonacceptable radiographic alignment underwent closed reduction and 4 to 6 weeks cast immobilization. Radiographs were obtained after reduction and at 10 to 14 days. Redisplaced fractures were offered surgical treatment. One-year follow-up included grip strength, range of motion (ROM), quickDASH, EQ-5D (including visual analog scale [VAS] for health status), and VAS pain. Results We found no statistically significant differences in QuickDASH, EQ-5D questionnaire, EQ-5D health status VAS, and VAS pain among the Buttazzoni classes. However, initial displacement was associated with worse quickDASH score, worse EQ-5D score, reduced grip strength, and reduced ROM. Dorsal comminution was associated with worse quickDASH score, reduced flexion, and reduced pronation–supination ability. Volar comminution predicted loss of extension, while intra-articular involvement was associated with reduced flexion–extension arc and worse EQ-5D score. There was a significant difference in ROM between noncomminuted and comminuted fracture classes. Conclusion Initial fracture position, type of comminution, and intra-articular involvement influenced the clinical outcome in DRF.

AbstractSome salicylaldehyde-derived Baylis-Hillman adducts and 3-(chloromethyl)coumarins have been synthesised and evaluated in vitro for their antisickling activities. The compounds were screened for inhibitory and reversal activity against mutated haemoglobin (HbSS) in red blood cells at four different concentrations (4 mg/mL, 2 mg/mL, 1 mg/mL and 0.5 mg/mL) as a measure of their antisickling potentials. Among the synthesized compounds, 6-chloro-3-(chloromethyl)coumarin 4d showed the highest inhibitory activity (83.75±1.90%), followed by 6-chlorocoumarin-3-methylsulfinic acid 5d (80.90 ±0.91%) and the least was tert-butyl-3-hydroxy- 3-(2-hydroxyphenyl)-2-methylenepropanoate 3a (33.33±1.86%). The results obtained from the reversal antisickling experiment showed that the percentage of sickle cells able to revert to the normal biconcave shape was dose dependent. Compound 5d had the highest reversal activity (66.49±1.39%) followed by 6-bromo-3- (chloromethyl)coumarin 4c (59.66±2.95) and 4d (55.50±1.95%) at 4 mg/mL. Compound 4c had higher reversal activity than the standard p-hydroxybenzoic acid at 2 mg/mL, 1 mg/mL and at 0.5 mg/mL. The 3-substituted coumarins 4a-d, and 5d had higher inhibitory antisickling activities than their Baylis-Hillman precursors 3a-d. Effect of 4a-d and 5d on the rate of polymerization of sickle cell heamoglobin was further studied spectrophotomerically using hemolysate of HbSS. The considerable inhibitory and reversal activities of these compounds make them good candidates for further antisickling studies.

5D models with one 3D brane and one infinite extra dimension are studied. Matter is confined to the brane, gravity extends to the bulk. Models with positive and negative tension of the brane are studied. Cosmological solutions on the brane are obtained by solving the generalized Friedmann equation. As the input in cosmological solutions we use the present-time observational cosmological parameters. We find constraints on dimensionless combinations of scales of 5D models which follow from the requirement that the models reproduce the data on production of 4 He in primordial nucleosynthesis.

e19141 Background: Patient-Reported Outcome Measurement (PROM) is the way to collect not only Health-Related Quality of Life (HRQoL) but also symptoms. EUROQOL-5D is a PROM tool explored widely in patients with cancer, although is not specific for cancer patients. The primary end-point of this project is to analyse the significance of impairments in EUROQOL-5D domains in overall survival of patients with hematological malignances, and the utility to incorporate as a PROM tool in a daily clinical practice. Methods: Patients with hematologic malignancies attended at Fundacion Jimenez Diaz University Hospital were assessed with EUROQOL-5D prior to receive any therapy. Self-reported health status stated as better, equal or worse was recorded at the time of assessment. Other variables such as age, sex, diagnosis and ECOG was collected to analyze their impact in survival. Results: From January 1, 2017 to December 31, 2019, 390 consecutive patients were included. Median age was 72 years (range 18-92), 194 (49.7%) were female, with a diagnosis of lymphoma in 257 (65.9%) and myeloma in 70 (17.9%), and ECOG 0-1 in 78.2% of patients. With a median follow-up of 10.5 months (range 0-32 months), we registered 72 events for overall survival. In the univariate analysis, age > 76 years (AUC 0.739, Likelyhood Ratio 3.321, p < 0.0001), ECOG 2-4 (OR 11.161, 95% CI 5.276-23.610, p < 0.0001), impairment in EUROQOL-5D domains such us mobility (OR 2.896, 95%CI 1.406-5.967, p = 0.003), self-care (OR 3.959, 95%CI 1.732-9.050, p = 0.001) and usual activities (OR 3.190, 95%CI 1.573-6.467, p = 0.001), but not pain/disconfort nor anxiety/depression were identified as prognostic factors for shorter survival. Self-reported health status (OR 3.975, 95%CI 1.845-8.565, p < 0.0001) was also a prognostic factor for survival. Impairments in EUROQOL-5D were also prognostic in patients with ECOG 0-1 (p < 0.001). In the multivariate analysis, only ECOG 2-4 and self-reported health status remained with statistical significance. Conclusions: EUROQOL-5D is a valid tool to be incorporated as a PROM tool in pacients with hematological malignancies. Impairments in mobility, self-care and usual activities will identify a poorer prognosis group that would need closer monitoring.

IntroductionInjuries are among the 10 leading causes of deaths worldwide. In recent years, the quality and reporting of injury mortality has improved but little or no data are available on the morbidity burden and impact of non-fatal injuries in India. This study evaluates health recovery status postinjury, identifying predictors of recovery in North India.MethodsProspective cohort study recruiting patients from one tertiary-level and two secondary-level hospitals in North India between April and June 2014 hospitalised due to any injury. Health-related quality of life was assessed at baseline and at 1-month, 2-month, 4-month and 12-month postinjury using the EuroQol five-dimensional (EQ-5D-5L) questionnaire. Multivariable linear regressions with generalised estimating equations were used to examine the relationship between sociodemographic and injury-related factors with the EQ-5D-5L single utility score and the visual analogue scale (VAS) score.ResultsA total of 2416 eligible patients aged ≥18 years were enrolled in the study. Of these, 2150 (74%) completed baseline and all four follow-up EQ-5D-5L questionnaires. Almost 7% (n=172) patients died by the first follow-up and the overall mortality at 12 months was 9% (n=176). Both EQ-5D-5L utility and VAS scores dropped significantly at 1-month postinjury but gradually improved at 2, 4 and 12 months. Severe injuries, defined as those requiring a hospital stay of ≥7 days, were associated with lower utility scores at 1-month, 2-month and 4-month follow-ups (p<0.001).ConclusionThis is the first study to examine health outcomes following injuries in India. The findings highlight the need to understand the social, psychological and biological factors influencing recovery outcomes. High mortality following discharge emphasises the need to invest in secondary and tertiary injury prevention in India.

Background: The EuroQol Group 5-Dimension (EQ-5D) questionnaire is frequently used as an outcome measure of general patient-reported health-related quality of life (HRQoL). We evaluated the internal and external responsiveness of EQ-5D (specifically EQ-5Dindex score) in patients with a surgically treated distal radius fracture. Methods: Within the context of a randomized controlled trial (RCT), 132 patients with a surgically treated distal radius fracture filled out EQ-5D and Patient-Rated Wrist Evaluation (PRWE-Swe) at baseline (preinjury state), and at 3 and 12 months. Internal responsiveness was evaluated by calculating mean change score and standardized response mean (SRM) of the EQ-5Dindex scores. External responsiveness was evaluated with PRWE-Swe as the external criterion. PRWE-Swe was used to define 4 subgroups of patients with different clinical outcomes. The ability of EQ-5Dindex change score to discriminate between these subgroups was analyzed with logistic regression, receiver operating characteristic (ROC) curves, and area under the ROC curves (AUROCs). Correlation analysis was made with Spearman’s ρ test. Results: The mean change in EQ-5Dindex score from baseline to the 3-month follow-up was −16.1 and from the 3- to the 12-month follow-up was 7.6. The corresponding SRMs were large (0.93) and small to moderate (0.47). Analysis of external responsiveness displayed odds ratios >1 and AUROCs between 0.70 and 0.76 in all 4 subgroups. The proportion of patients correctly classified into the 4 subgroups by the EQ-5Dindex change scores was 78% to 94%. Spearman’s ρ was 0.35. Conclusion: EQ-5D displayed an overall acceptable to good responsiveness in patients with a distal radius fracture. It may thus be used as a measure of HRQoL in this patient group.

Background The International Burden of Migraine Study (IBMS) showed chronic migraine (CM) was associated with a higher disease burden than episodic migraine (EM). However, in this study Asians with CM were underrepresented. Objectives We investigated if CM and EM differed in headache-related disability, health-related quality of life (HRQoL) and health care resource utilization in Taiwan. Methods This study recruited patients with EM and CM from two headache clinics in Taiwan. Diagnosis was made by physicians based on Silberstein-Lipton criteria. Participants completed a questionnaire including sociodemographics, Migraine Disability Assessment (MIDAS), EuroQol five-dimensions (EQ-5D), Migraine-Specific Quality of Life v2.1 (MSQ), Patient Health Questionnaire-4 (PHQ-4), productivity and health care resource utilization. Results A total of 331 patients (EM, n = 164 (49.5%); CM, n = 167 (50.5%)) completed the study. CM patients reported a significantly higher MIDAS score, lower generic (EQ-5D visual analogue scale score and EQ-5D index score) and migraine-specific (all three domains of MSQ) HRQoL, higher levels of anxiety and depression (PHQ-4 ≥ 6) and greater health care resource utilization and productivity loss than those with EM. Positive correlations were found between these instruments and levels of anxiety and depression. Conclusion Compared to EM, CM was significantly associated with higher disability, lower HRQoL and greater health care resource utilization and productivity loss.

Object Outcome studies for spine surgery rely on patient-reported outcomes (PROs) to assess treatment effects. Commonly used health-related quality-of-life questionnaires include the following scales: back pain and leg pain visual analog scale (BP-VAS and LP-VAS); the Oswestry Disability Index (ODI); and the EuroQol-5D health survey (EQ-5D). A shortcoming of these questionnaires is that their numerical scores lack a direct meaning or clinical significance. Because of this, the concept of the minimum clinically important difference (MCID) has been put forth as a measure for the critical threshold needed to achieve treatment effectiveness. By this measure, treatment effects reaching the MCID threshold value imply clinical significance and justification for implementation into clinical practice. Methods In 45 consecutive patients undergoing transforaminal lumbar interbody fusion (TLIF) for low-grade degenerative lumbar spondylolisthesis-associated back and leg pain, PRO questionnaires measuring BP-VAS, LPVAS, ODI, and EQ-5D were administered preoperatively and at 2 years postoperatively, and 2-year change scores were calculated. Four established anchor-based MCID calculation methods were used to calculate MCID, as follows: 1) average change; 2) minimum detectable change (MDC); 3) change difference; and 4) receiver operating characteristic curve analysis for two separate anchors (the health transition index [HTI] of the 36-Item Short Form Health Survey [SF-36], and the satisfaction index). Results All patients were available at the 2-year follow-up. The 2-year improvements in BP-VAS, LP-VAS, ODI, and EQ-5D scores were 4.3 ± 2.9, 3.8 ± 3.4, 19.5 ± 11.3, and 0.43 ± 0.44, respectively (mean ± SD). The 4 MCID calculation methods generated a range of MCID values for each of the PROs (BP-VAS, 2.1–5.3; LP-VAS, 2.1–4.7; ODI, 11–22.9; and EQ-5D, 0.15–0.54). The mean area under the curve (AUC) for the receiver operating characteristic curve from the 4 PRO-specific calculations was greater for the HTI versus satisfaction anchor (HTI [AUC 0.73] vs satisfaction [AUC 0.69]), suggesting HTI as a more accurate anchor. Conclusions The TLIF-specific MCID is highly variable based on calculation technique. The MDC approach with the SF-36 HTI anchor appears to be most appropriate for calculating MCID because it provided a threshold above the 95% CI of the unimproved cohort (greater than the measurement error), was closest to the mean change score reported by improved and satisfied patients, and was least affected by the choice of anchor. Based on the MDC method with HTI anchor, MCID scores following TLIF are 2.1 points for BP-VAS, 2.8 points for LP-VAS, 14.9 points for ODI, and 0.46 quality-adjusted life years for EQ-5D.

The Toddler and Infant (TANDI) dimensions of Health-Related Quality of Life assess ‘age appropriate’ behaviour and measurement could be extended to older children. A sample of 203 children 3–4 years of age was recruited, and their caregivers completed the TANDI, Pediatric Quality of Life Inventory (PedsQL) and EQ-5D-Y Proxy. Spearman and Pearson’s correlation coefficients, and Kruskal–Wallis H-test were used to explore the feasibility, known-group validity, discriminate validity and concurrent validity of the TANDI. Children with a health condition (n = 142) had a lower ceiling effect (p = 0.010) and more unique health profiles (p < 0.001) than the healthy group (n = 61). The TANDI discriminated between those with and without a health condition. In children with a health condition, the TANDI discriminated between clinician rated severity of the health condition. The TANDI had moderate to strong correlations with similar PedsQL and EQ-5D-Y items and scores. The TANDI is valid for children aged 3–4 years and is recommended for children with a health condition, whereas the PedsQL may be better for healthy children. The TANDI is recommended for studies with young children whereas the EQ-5D-Y Proxy is recommended for a sample including older children or for longitudinal studies with preschoolers. Further work on the TANDI is recommended to establish test-retest reliability and responsiveness.

Herein we report the design, synthesis, computational, and experimental evaluation of the antimicrobial activity of fourteen new 3-amino-5-(indol-3-yl) methylene-4-oxo-2-thioxothiazolidine derivatives. The structures were designed, and their antimicrobial activity and toxicity were predicted in silico. All synthesized compounds exhibited antibacterial activity against eight Gram-positive and Gram-negative bacteria. Their activity exceeded those of ampicillin and (for the majority of compounds) streptomycin. The most sensitive bacterium was S. aureus (American Type Culture Collection ATCC 6538), while L. monocytogenes (NCTC 7973) was the most resistant. The best antibacterial activity was observed for compound 5d (Z)-N-(5-((1H-indol-3-yl)methylene)-4-oxo-2-thioxothiazolidin-3-yl)-4-hydroxybenzamide (Minimal inhibitory concentration, MIC at 37.9–113.8 μM, and Minimal bactericidal concentration MBC at 57.8–118.3 μM). Three most active compounds 5d, 5g, and 5k being evaluated against three resistant strains, Methicillin resistant Staphilococcus aureus (MRSA), P. aeruginosa, and E. coli, were more potent against MRSA than ampicillin (MIC at 248–372 μM, MBC at 372–1240 μM). At the same time, streptomycin (MIC at 43–172 μM, MBC at 86–344 μM) did not show bactericidal activity at all. The compound 5d was also more active than ampicillin towards resistant P. aeruginosa strain. Antifungal activity of all compounds exceeded those of the reference antifungal agents bifonazole (MIC at 480–640 μM, and MFC at 640–800 μM) and ketoconazole (MIC 285–475 μM and MFC 380–950 μM). The best activity was exhibited by compound 5g. The most sensitive fungal was T. viride (IAM 5061), while A. fumigatus (human isolate) was the most resistant. Low cytotoxicity against HEK-293 human embryonic kidney cell line and reasonable selectivity indices were shown for the most active compounds 5d, 5g, 5k, 7c using thiazolyl blue tetrazolium bromide MTT assay. The docking studies indicated a probable involvement of E. coli Mur B inhibition in the antibacterial action, while CYP51 inhibition is likely responsible for the antifungal activity of the tested compounds.

В качестве примеров выведены $\mathcal N=4$ суперсимметричное эффективное действие в $4D$, $\mathcal N=4$ суперсимметричной теории Янга-Миллса, $\mathcal N=2$ суперсимметричное эффективное действие в $5D$, $\mathcal N=2$ суперсимметричной теории Янга-Миллса и $\mathcal N=(1,1)$ суперсимметричное эффективное действие в $6D$, $\mathcal N=(1,1)$ суперсимметричной теории Янга-Миллса. Они обладают соответственно явными $4D$, $\mathcal N=2$ суперсимметриями, $5D$, $\mathcal N=1$ суперсимметриями и $6D$, $\mathcal N=(1,0)$ суперсимметриями вне массовой поверхности. Pentaphenylcyclopentadiene (1) reacts with acyl chlorides RC(O)Cl (R = CH3, C3H7, C5H11, C7H15) with formation of the corresponding [(cyclopentadiene pentayl)pentakis] (1,4-phenylene) alkanones C5H(C6H4C(O)R-4)5 (2a, 2c, 2d and 2e). 2 a reacts with KOCl in water/dioxane to yield C5Cl(C6H4C(O)OH-4)5 (3), which after reaction with C2H5OH and C5H11OH gives the pentaesters C5Cl(C6H4C(O)OR-4)5 (4b and 4d), and after reaction with (CH3)3SnH the corresponding [(cyclopentadiene pentayl)pentakis] benzoic acid pentaalkyl esters C5H(C6H4C(O)OR-4)5 (5b and 5d). Reaction of 2a, 2c, 2d, 2e and 5b with NaNH2 results in the formation of the sodium salts Na[C5(C6H4C(O)R-4)5] (6a, 6c, 6d and 6e) and Na[C5(C6H4C(O)OC2H5-4)5] (7). 2a, 2c, 2d, 2e, 5b and 5d react with TlOC2H5 to give the thallium(I) derivatives Tl[C5(C6H4C(O)R-4)5] (8a, 8c, 8d, 8e) and Tl[C5(C6H4C(O)OR-4)5] (9b and 9d). BackgroundTo completely comprehend the burden of an illness and the success of its treatment, it is essential to analyze the patients’ perspective on health-related quality of life. Because of this, the Primary Sjögren’s Syndrome-Quality of Life questionnaire (PSS-QoL) was developed, which evaluates the disease’s physical and psychosocial effects in PSS patients.ObjectivesIn the literature, there is no Turkish questionnaire evaluating quality of life in patients with PSS. Therefore, the aim of this study was to translate and evaluate cross-culturally adaptation the PSS-QoL into the Turkish language and investigate its convergent validity and reliability in Turkish-speaking population with PSS.MethodsPatients completed PSS-QoL questionnaire, the EULAR Sjögren’s Syndrome Patient Reported Index (ESSPRI) and Euro-QoL 5D (EQ-5D). To test the convergent validity of PSS-QoL, correlation between PSS-QoL and EQ-5D and ESSPRI were assessed with Pearson correlation test. In order to investigate the internal consistency and test-retest reliability, Cronbach’s alpha and Intraclass Correlation Coefficient (ICC) values interpreted, respectively.ResultsSeventy-nine patients with PSS (53.30 + 11.98 years, 74F/5M) were enrolled in the study. Good and moderate correlations were found between the PSS-QoL and ESSPRI (r: 0.818, p<0.001) and EQ-5D-pain/ discomfort (r:0.589, p<0.001). Correlations of subscales of PSS-QoL with subscales of ESSPRI and EQ-5D ranged from fair to strong (Table 1).
