BackgroundUniversally acceptable criteria to define axial psoriatic arthritis (PsA) are lacking. Machine learning (ML) algorithms can detect patterns from large clinical datasets and identify clusters with potential therapeutic or prognostic significance in PsA patients (pts) with axial manifestations.[1]ObjectivesTo identify pt clusters based on baseline (BL) demographics and clinical indicators to support characterisation of the axial PsA phenotype within the spondyloarthritis (SpA) spectrum.MethodsPooled BL demographics and clinical data of secukinumab (SEC)-treated pts from 10 Phase-3 studies: FUTURE 1–5 (PsA), MEASURE 1–4 (ankylosing spondylitis, AS) and MAXIMISE (PsA with axial manifestations) were analysed to identify pt clusters by ML.[2]Finite mixture model methodology was applied to the pooled dataset. Longitudinal responses of SEC 300 mg vs 150 mg were explored across the identified clusters and clinical indicators tender joint counts (TJC), swollen joint counts (SJC) and enthesitis.ResultsEight distinct clusters were identified (n=3907,Figure 1 and Table 1). Pts with PsA with axial manifestations (MAXIMISE) were overrepresented in clusters 6–8. Pts in cluster 6 (mean age 48 yrs; 46% male) were overweight with pronounced psoriasis (PsO), and high articular burden in large joints. Pts in cluster 7 (mean age 47 yrs; 53% male) were less overweight with lower polyarticular joint counts (mean 11 joints) and tenderness focused on the feet, wrists and hands. Pts in cluster 8 were mostly pts with AS (mean age 43 yrs; 64% male), less overweight with oligoarthritis and high prevalence of spinal pain. Pts with PsA (FUTURE) were overrepresented in clusters 1–5. Longitudinal analysis showed significant improvements with SEC 300 mg vs 150 mg in clusters 6 and 8 for TJC, and cluster 7 for SJC.ConclusionPsA clusters obtained by ML in the pooled dataset indicate phenotypical heterogeneity of pts with PsA with axial manifestations and overlapping features across the SpA spectrum.References[1]Pournara E et al. RESUMO Foi avaliada a qualidade sanitária e físico-química das águas utilizadas na irrigação de hortaliças em cinco hortas localizadas em dois municípios paraibanos. As águas de irrigação apresentaram elevada contaminação fecal (CF: 104 UFC 100 mL-1) infectando os vegetais; esta contaminação teve como origem as descargas de esgotos domésticos nos córregos e açudes utilizados na irrigação, resultando no reúso indireto de esgotos na agricultura. As alfaces (Lactuca sativa L.) irrigadas com tais águas apresentaram concentrações de coliformes fecais entre 1,0 x 103 e 1,6 x 106 e de E. coli entre 2,5 x 102 e 2,5 x 104 NMP g-1, indicando riscos para a saúde dos consumidores, porém as alfaces provenientes dos mercados públicos apresentaram carga fecal entre 7,5 x 103 e 3,8 x 104 NMP g-1, não havendo diferenças significativas com aquelas das hortas. A elevada carga fecal do adubo orgânico de origem animal (valor médio entre 1,3 x 105 e 2,6 x 106 NMP g-1) comumente utilizada nas hortas também contribuiu na contaminação das alfaces. Do ponto de vista físico-químico, as águas de irrigação continham apreciáveis concentrações de sais, porém não apresentaram altos risco de salinização ou de sodificação do solo.

We examined methods of linking density, dispersion, and stocking information from juvenile regeneration surveys with mortality estimates to predict future yield of white spruce (Picea glauca (Moench) Voss) in boreal mixedwoods. The study focused on data from 709 stands (7–150 years) and defined a stocked plot (10 m2) as having one or more acceptable trees. In juvenile surveys, ingress of natural spruce overwhelmed the regular planting pattern, creating clumped dispersion patterns, as indicated by the Morisita index. A function was developed to describe the relationship between stocking, density, and dispersion. In mature, permanent sample plots, only 30%–40% stocking of 10 m2 plots (700 stems·ha–1) was needed to achieve full yields. Mortality rates for planted spruce varied from 0.1% to 0.8% per year for juvenile stands and from 1.7% to 3.3% per year for mature stands. For rotation-length predictions in Alberta, 0.7% per year is likely a mean mortality loss. These findings were combined to generate stocking versus time curves at a range of mortality rates. The tallest spruce measured in each juvenile survey plot had the same mortality rate regardless of absolute size, and spruce mortality was reduced when associated with aspen. These findings call into question minimum height requirements and free-to-grow criteria in regeneration standards.

Abstract Abstract 3520 IL-3, GM-CSF and G-CSF are predominant regulators for growth and differentiation of myeloid progenitors. Interestingly, they all signal via a common JAK2-STAT5 pathway in myeloid progenitor compartments. However, the specific mechanism through which JAK2-STAT5 responds differentially to early-acting and lineage restricted cytokines, particularly in leukemic and stem/progenitor cells, is largely unresolved. Aberrations in IL-3, GM-CSF and G-CSF induced signaling are frequently reported in acute myeloid leukemia (AML). microRNA (miRNA) play several crucial roles during hematopoiesis that include lineage decisions, stem cell progenitor transitions, niche control and other cell functions. Recent investigations have linked aberrant miRNA expression with AML. We hypothesized that a unique response of leukemic myeloid progenitors to IL-3, GM-CSF, and G-CSF are possibly mediated in part by distinct regulation at the miRNA level. Therefore, herein, by utilizing a unique leukemic myeloid cell line, AML-193, that responds to both early and late acting cytokines, we profiled IL-3, GM-CSF and G-CSF regulated miRNA signatures in leukemic myeloid progenitors. For miRNA profiling, AML-193 cells were initially exposed to IL-3 for 3 days followed by GM-CSF for 3 more days and subsequently to G-CSF for 3 days. We then profiled miRNA expression induced by IL-3, GM-CSF and G-CSF in AML-193 cells by treating the respective cohorts post growth factor deprivation with corresponding cytokines. Using SA Bioscience's complete Human V2.0 miRNA Genome Array platform for real-time qPCR-based miRNA expression profiling, we investigated miRNA signatures regulated by IL-3, GM-CSF and G-CSF for n=704 miRNAs in AML-193 cells. Frequencies of false positives were avoided using stringent filters. IL-3 specifically regulated 54 miRNAs and those miRNAs that were highly regulated included miR-362-3p, miR-590-3p, miR-340, miR-24-2, miR-1183 and miR-99a. GM-CSF specifically regulated miRNAs included let-7f, let-7a*, miR-195, miR-122, miR-376-c and miR-33a. G-CSF specifically regulated set included miR-21*, miR-192*, miR-32*, miR-7-1*, miR-545* and miR-37-4a*. Interestingly, 301 miRNAs were commonly regulated by IL-3, GM-CSF and G-CSF. Among the commonly regulated miRNAs, the ones that were subjected to high levels of regulation included miR-590-5p, miR-219-5p, miR-92-a1*, miR-378*, miR-548-3p, miR-29a*, miR-590-3p, miR-203, miR-363, miR-454, miR-340, miR-196a, miR-152, miR-10b, miR-24-2*, miR-10a, miR-182, miR-27a*, and miR-199a-3p. Interestingly, the commonly regulated miRNAs demonstrated a directional regulation in the order of IL-3>GM-CF>G-CSF. Analysis of the potential targets of significantly regulated miRNAs revealed important functional roles in myeloid cell development and differentiation. Conserved targets of miR-590-5p included BMPR2, PCBP2 and KLF3. Targets for miR-219-5p included Smad4 and GADD45b. Targets for miR-362-3p included Sox17, an inhibitor of the Wnt signaling pathway. Furthermore, to identify the JAK2-STAT5 responsiveness of miR-219-5p, miR-362-3p, and miR-590-5p, a STAT5 specific inhibitor (N′-((4-Oxo-4H-chromen-3-yl)methylene)nicotinohydrazide) was used prior to cytokine treatment. Interestingly, IL-3 and GM-CSF mediated induction of miR-590-5p, miR-219-5p and miR-362-3p were not significantly affected by STAT5 inhibitor. However, the G-CSF induction of miR-219-5p and miR-590-5p were significantly inhibited by the STAT5 specific inhibitor. In summary, we have discovered for the first time novel miRNA profiles regulated by IL-3, GM-CSF and G-CSF in an acute myeloid leukemia progenitor cell model. Importantly, we have established a cytokine regulated miRNome for leukemic myeloid progenitors and set the stage for future investigations in leukemic stem cells to delineate the pathological roles of dysregulated miRNAs in AML. In addition, our study displayed that G-CSF induced a subset of miRNAs that are principally regulated via the STAT5 circuit in AML. Further, these novel miRNA signatures may have therapeutic implications for targeting dysregulated miRNAs by antagomir strategy or miRNA replacement therapy, paving the way for the development of novel miRNA-based therapeutic interventions in AML. e16118 Background: Lenvatinib is an oral tyrosine kinase inhibitor that has been available for treatment of unresectable hepatocellular carcinoma (uHCC) in Japan since March 2018. We conducted a multicenter prospective observational study to evaluate the safety and efficacy of lenvatinib in clinical practice. Methods: This study conducted as a prospective study, and enrolled patients with uHCC initially receiving lenvatinib as systemic therapy who gave informed consent from July 2018 through January 2019. The observation period per patient was 12 months and e-CRF was used. Results: 713 patients were registered at 133 clinical sites. The safety analysis set and efficacy analysis set were 703 patients. Patient baseline characteristics included median age:73.0 years (25 to 94), male/ female: 564/139, bodyweight: < 60kg/≥60kg: 323/380, HBV/HCV/alcohol/NAFLD or NASH: 137/287/167/88, Child-Pugh classification A/B or higher: 624/75, BCLC stage A/B/C/D: 57/291/332/11, Vp 0/1/2/3/4: 520/31/52/60/22, with/without extrahepatic metastasis: 236/461. Previous treatment (local and systemic therapy) for hepatocellular carcinoma is with/without TACE: 513/190, median number of TACE treatment: 3.0 times (1 to 26), with/without HAIC: 74/629, with/without TKI pretreatment: 131/572. The initial dose was 8 mg (80.5%) and 12 mg (68.2%) in patients with < 60 kg and ≥60 kg, respectively. Median relative dose intensity was 64.2% and the treatment continuation rate at 3 and 12 months after administration of lenvatinib were 67.3% and 24.8%. Regarding safety, the incidence of treatment-related adverse events (TRAEs) was 84.9% (grade 3 or higher: 42.5%). Common TRAEs (incidence rate ≥10%) were decreased appetite (23.9%), malaise (21.8%), hypertension (21.3%), proteinuria (18.3%), palmar-plantar erythrodysesthesia syndrome (16.5%), hypothyroidism (15.8%), and diarrhea (13.9%). TRAEs of grade 3 or higher (incidence rate ≥2.5%) were proteinuria (6.7%), hypertension (6.4%), malaise (3.8%), decreased appetite (3.6%), platelet count decreased (3.3%), palmar-plantar erythrodysesthesia syndrome (2.7%), diarrhea (2.7%) and hepatic encephalopathy (2.6%). The incidence of TRAEs leading to discontinuation of lenvatinib was 32.9%, and main TRAEs leading to discontinuation of lenvatinib were (incidence rate ≥2%) decreased appetite (6.3%), malaise (4.7%), proteinuria (3.3%) and hepatic encephalopathy (2.4%). Regarding efficacy, the ORR and DCR was 39.5% (95% CI: 35.1, 43.9) and 78.9% (75.1, 82.5) in 494 patients who were evaluated for the best response by mRECIST. The median OS was 498.0 days (95% CI: 439.0, -) in 703 patients. Conclusions: The incidence of TRAEs leading to discontinuation of lenvatinib tended to be higher than in phase 3 REFLECT trials (19.7%), but the safety profile and effectiveness findings for lenvatinib in this study were consistent with previous reports.

Introducción: la nutrición parenteral domiciliaria (NPD) mejora la calidad de vida de los pacientes permitiéndoles recibir nutrición en su domicilio y facilitando su integración social y laboral.Objetivo: analizar el coste de la NPD en España.Métodos: se realizó una revisión bibliográfica de los Registros de NPD en España (años 2007-2014), elaborados por el Grupo NADYA-SENPE. Se analizó la evolución de: pacientes que requerían NPD, episodios/paciente por los que se administró NPD, duración media de los episodios, vías de acceso y tasa de complicaciones. Se estimó el consumo y coste de la NPD. Los pacientes fueron agrupados según patología: benigna o maligna. Los costes directos (€, 2015) incluidos fueron: bolsas administradas, vías de acceso y complicaciones.Resultados: el número de pacientes que recibió NPD aumentó a lo largo de los años (2007: 133 pacientes; 2014: 220 pacientes). El número medio de episodios/paciente osciló entre 1-2 episodios/año y su duración media disminuyó (2007: 323 días; 2014: 202,8 días). Las vías de acceso más utilizadas fueron los catéteres tunelizados y las complicaciones sépticas fueron las más comunes. El coste directo anual medio por paciente se estimó en 8.393,30 € y 9.261,60 € para patología benigna y maligna, respectivamente. Considerando que, en 2014, 220 pacientes requirieron NPD, el coste anual fue 1.846.524,96 € (1.389.910,55 € debidos a la fórmula de NPD) y 2.037.551,90 € (1.580.937,50 € debidos a la fórmula de NPD) para patología benigna y maligna respectivamente.Conclusiones: estos resultados sirven de base para futuros análisis económicos de la NPD y para establecer estrategias de priorización eficiente de recursos disponibles.

Purpose: To investigate the effects of a low- versus high-intensity aerobic training on biomarkers of inflammation and endothelial dysfunction in adolescents with obesity. Methods: Sixty-two adolescents with obesity [age = 15 (14) y, body mass index = 34.87 (4.22) kg·m−2] were randomized to receive either a high-intensity training (HIT, n = 31) or a low-intensity training (LIT, n = 31) for 24 weeks. All participants also received nutritional, psychological, and clinical counseling. Leptin, total and subtype leukocyte counts, tumor necrosis factor-alpha, interleukin-6, myeloperoxidase, soluble intercellular adhesion molecule-1, and soluble vascular cell adhesion molecule-1 were obtained at baseline and after 24 weeks. Results: HIT reduced neutrophils [from 4.4 (1.9) to 3.6 (1.3) µL−1 × 103; P = .01] and monocytes [from 7.2 (2.5) to 5.2 (1.8) µL−1 × 102; P < .01], but LIT increased neutrophils [from 4.5 (1.7) to 5.2 (3.3) µL−1 × 103; P = .01]. Although tumor necrosis factor-alpha increased in LIT [from 13.3 (7.5) to 17.7 (10.8) pg·mL−1; P = .01], it decreased in HIT [from 12.4 (7.5) to 11.3 (6.2) pg·mL−1; P = .01]. No changes in leukocyte counts, soluble intercellular adhesion molecule-1, soluble vascular cell adhesion molecule-1, and homeostasis assessment model for insulin resistance were observed. Conclusions: Both HIT and LIT improved the inflammatory profile. The study, however, indicated that the number of biomarkers and the magnitude of changes were higher in the HIT compared with LIT.

Foram testados, pela técnica de soroaglutinação microscópica aplicada à leptospirose, 329 cães provenientes do município de Monte Negro, RO, dos quais 156 eram da área urbana e 173 da área rural. Simultaneamente foi aplicado questionário para verificar a existência de possíveis fatores de risco pela regressão logística. Abordaram-se questões referentes a idade, sexo, dieta, tipo de criação, ambiente, contato com outras espécies e hábito de caça. Foram detectadas reações com títulos >100 em 27,3%, com 90 cães positivos ao agente, das quais 23,7% eram da área (37/156) urbana e 30,6% (53/173), da área rural (P>0,05). Os sorovares predominantes, reatores com títulos mais elevados, foram Autumnalis (22%), Pyrogenes (12%), Canicola (10%) e Shermani (7,5%). Cães com idade acima de 12 meses apresentaram maior ocorrência de anticorpos quando comparados aos cães mais jovens (P<0,05). Dentre os fatores de risco analisados, foram significativos a alimentação (dieta à base de ração comercial - odds ratio: 3,3; intervalo de confiança: 95%: 1,2 - 9,2; P=0,02) e o sexo (macho - odds ratio: 2,3; intervalo de confiança: 95%: 1,3 - 3,9; P=0,003). Ressalta-se a ocorrência de reações para sorovares mantidos na natureza por animais silvestres.

10067 Background: In childhood cancers cure rates increased up to 80% in the developed countries. On the other hand cure rates goes down 10-20% percent in countries with low resource settings. Reliable pediatric cancer data is essential for all countries. We established a nationwide pediatric cancer registry. Methods: Turkish Pediatric Oncology & Pediatric Hematology Society established a web-based database for the registry of all pediatric cancers. 11898 cases were registered between 2002-2008 from 65 centers. Various demographic data & survival endpoints were recorded & analyzed. Diseases were grouped according to the International Classification of Childhood Cancer. Results: In all 11898 cases, median age was 6 years (M/F= 6786/5112=1.32). Distribution in age groups were: 0-4 years, 42.5%; 5-9 years, 27.2%; 10-14 years, 23.4%; 15-19 years, 6.8%; >19 years, 0.1%. Only 3.8% of cases were diagnosed with clinical+radiological, the rest with histopathological data Distribution of cases in disease groups were [median age in yrs, M/F]: Leukemias (n=3777) 31.7% [5.5, 2137/1640=.31]; Lymphomas (n=2040) 17.1% [8.3, 1405/635=2.21]; CNS tumors (n=1588) 13.3% [6.9, 913/675=1.3 ]; Sympathetic tumors (n=889) 7.5% [2.1, 453/436=1.03]; Retinoblastoma (n=371) 3.1% [2, 181/190=0.95]; Renal tumors (n=655) 5.5% [3, 333/322=1.03]; Hepatic (n=166) 1.4% [1.8, 101/65=1.5]; Bone tumors (n=717) 6% [12.2, 407/310=1.3]; Soft tissue tumors (n=773) 6.5% [6.5, 442/331=1.3]; Germ cell tumors (n=531) 4.5% [5, 210/321=0.6 ]; Carcinomas and other malignant epithelial tumors (n=323) 2.7% [12, 164/159=1.03]; Others/unspecified malignant tumors (n=68) 0.6% [4.5, 40/28=1.4]. Five-year overall survival in all cases was 65%. Conclusions: This registry provides a critical information about the distribution of childhood cancer since this is the only nationwide pediatric cancer registry in Turkey. With the recent trends in non-communicable diseases at global level, registry data will be very helpful for national cancer control plans, which will also be used to compare at national and international level. This will also be a good example for many other countries with similar resources to do such projects.

Abstract Eating pattern and animal behavior of three different intensive beef fattening systems, combining cattle breed, gender, and fattening days were studied under the same commercial farming conditions. A total of 120 animals were allocated in 6 pens (two per treatment), and each pen was assigned to 1 of 3 treatments according to productive system: 1) Holstein bulls, slaughtered at 11.5 months of age (142 ± 3.3 days of age and 176 ± 5.5 kg of initial BW), 2) crossbred heifers at 10 months (141 ± 3.3 days of age and 165 ± 5.5 kg of initial BW), and, 3) crossbred Angus bulls at 12.5 months (154 ± 3.3 days of age and 192 ± 5.5 kg of initial BW). Each pen had two separate feeders for concentrate and straw, and a water trough. Eating pattern was recorded per day and animal, and animal behavior was analyzed fortnightly by scan pen sampling. Data were analyzed using a mixed-effects model with repeated measures. Most relevant interaction revealed that around 215 days of age (onset of puberty) heifers visited less frequently the feeder (7.5 vs 8.1 ± 0.23) and devoted more time to eat (7.5 vs 6.5 ± 0.22 min), decreasing eating rate (153.8 vs 177.4 ± 4.97 g/min) compared with bulls. Regardless of age, heifers tended (P &lt; 0.10) to exhibit more frequently non-agonistic (20.4 vs 13.0 ± 1.53 n/15 min), less agonistic (1.2 vs 3.8 ± 0.92 n/15 min) and sexual (1.3 vs 5.5 ± 1.65 n/15 min) behaviors, and more stereotypies (1.5 vs 0.3 ± 0.08 n/15 min) compared with bulls. In conclusion, when animals spent time performing agonistic and sexual behaviors, the time devoted to eat was reduced, identifying a gender effect. This finding could have implications in terms of feeder design, number of feeding spaces, and feeder access.

Background. For people with hypersensitivity, the problem of allergenic and pyrogenic environmental load is of great importance. Material and methods. Hostel and apartment dust in Moscow was analyzed for bacteria and fungi by microbiological and molecular methods. Results. Aerobic and facultative anaerobic bacteria prevailed, numbers varying from 4.8×104 to 2.0×109 MPN/g of dust in hostels (Me=4.8×106 MPN/g) and from 3.0×104 to 1.0×109 of dust MPN/g in apartments (Me=3.8×106 MPN/g). Gram-positive bacteria were revealed in 100% of apartments and in 80% of hostels, Gram-negative, in 47% of apartments, and in 73% of hostels. 9 yeast species were isolated: Filobasidium wieringae, F.magnum, Papiliotrema flavescens, Vishniacozyma victoriae, Rhodotorula mucilaginosa, Debaryomyces hansenii, Candida parapsilosis, C.tropicalis, Meyerozyma guilliermondii. R.mucilaginosa dominated (34.5% and 33.4%). Candida spp. were more frequent in hostels, while Filobasidium spp., in apartments. Yeast cell number varied from 3.5×103 to 1.3×106 CFU/g of dust in hostels (Me=3.2×104 CFU/g), and from 2.3×103 to 2.5×106 CFU/g of dust in apartments (Me=3.3×104 CFU/g). 56 mycelial fungi species were revealed. Penicillium chrysogenum (100%), Aspergillus niger (100%), Rhizopus stolonifer (100%), A.ochraceus (80%), Mucor plumbeus (67%), P cyclopium (60%) were the most frequent in hostels. P.cyclopium (60%), P.chrysogenum (60%) were the most frequent in apartments. Mold concentration varied from 1.8×105 to 7.5×106 CFU/g of dust in hostels (Me=5.2×105 CFU/g) and from 3.3×103 to 2.3×105 CFU/g of dust in apartments (Me=4.0×104 CFU/g). Micromycetes complexes were similar in different hostels but differed in apartments. Discussion. The species diversity and CFU counts of microorganisms do not give the true values of the concentration of allergenic and pyrogenic compounds. Conclusion. We propose complex assessment involving identification of both allergenic and pyrogenic compounds and producers.

Introdução: A língua é uma das regiões da cavidade oral mais atingida por neoplasias malignas. Em grande parte dos casos o diagnóstico de câncer bucal é realizado tardiamente, colaborando para os altos índices de morbilidade e mortalidade dessa doença. Objetivo: Demonstrar o perfil epidemiológico e a distribuição da prevalência dos casos de câncer de língua em fumantes e ex–fumantes nos estados do nordeste brasileiro. Métodos: Trata-se de um estudo epidemiológico descritivo, transversal, realizado a partir dos dados fornecidos pelo Instituto Nacional do Câncer através dos Registros Hospitalares de Câncer, nos últimos 5 anos disponíveis, de 2013 a 2017. Todos os casos foram provenientes do Sistema Único de Saúde, separados segundo sexo e histórico de tabagismo. Os dados coletados foram analisados descritivamente. Resultados: A região do nordeste brasileiro registrou 1793 casos de câncer de língua. Os estados de Sergipe, Maranhão, Piauí apresentaram, respectivamente, os seguintes números de neoplasia na língua, 103 (5,7%), 55 (3,1%), e 2 (2,6%). Já o estado da Paraíba apresentou um total de 132 (7,4%) e Alagoas 136 (7,5%). Os estados de Pernambuco com 651(36,3%), Bahia com 489(27,3%), Rio Grande do Norte e Ceará com 173(9,6%) e 229(12,8%) respectivamente. A análise dos dados mostrou que 72,4% dos indivíduos são homens e que o hábito de parar de fumar não impediu o aparecimento da doença. Conclusão: Existe o predomínio da referida lesão no sexo masculino, cor parda e nos indivíduos tabagistas. O estado de Pernambuco apresentou o maior número de casos de câncer de língua na região nordeste e o estado do Piauí a menor prevalência da doença.

Background:BAFF, APRIL and BAFFR are genes that encode cytokines with a key role in the development and survival of B-lymphocytes [1-4]: The B cell-activating factor (BAFF, also known as BLyS), a proliferation-inducing ligand (APRIL) and BAFF receptor (BAFF-R), respectively. Previous genetic studies have revealed that the BAFF-APRIL-BAFFR pathway is implicated in the genetic predisposition to several immune-mediated diseases [5].Objectives:To determine whether the BAFF-APRIL-BAFFR pathway represents a novel genetic risk factor for the pathogenesis of Immunoglobulin-A vasculitis (IgAV), an inflammatory disease in which IgA deposits and B-lymphocytes are crucial [6, 7].Methods:A functional BAFF polymorphism (rs374039502) and two tag variants within APRIL (rs11552708 and rs6608) and BAFFR (rs7290134 and rs77874543) were genotyped in 386 Caucasian IgAV patients (the largest series of Caucasian patients with IgAV ever assessed for genetic studies) and 806 sex and ethnically matched healthy controls by TaqMan assays.Results:No statistically significant differences in the genotype and allele frequencies between patients with IgAV and healthy controls were observed when each genetic variant of BAFF APRIL and BAFFR was analyzed independently (Table 1). Likewise, no statistically significant differences in genotype and allele frequencies of BAFF APRIL or BAFFR were found when patients with IgAV were stratified according to the age at disease onset or to the presence/absence of gastrointestinal or renal manifestations. Abstract Background Serum Beta-2 microglobulin (B2M) has been proposed as a potential prognostic factor for Non Hodgkin Lymphomas, including diffuse large B cell lymphoma (DLBCL). However, its prognostic value in DLBCL is not well known in rituximab era. We aimed to investigate its role as a prognostic factor in patients with DLBCL treated with R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) Method Between January 2004 and April 2014, 940 patients with DLBCL, newly diagnosed and treated with R-CHOP, were identified in a single center lymphoma registry. Baseline serum B2M was available in 834 of these patients. Progression-free survival (PFS) and Overall survival (OS) was compared according to the levels of B2M using log-rank test. Result Median B2M value was 2.1 (range, 0.67-29.6 mg/L) and baseline value was elevated (> 2.5 mg/L) in 269 patients (32%). Elevated serum B2M was significantly associated with poor prognostic factors, as summarizedin table 1. In univariate analysis, elevated B2MG was significantly associated with poor PFS (hazard ratio [HR]=3.0, 95% confidence interval [CI]: 2.4-3.9; P <0.001) and OS (HR=3.5, 95% CI: 2.6-4.6; P <0.001). Each elements of IPI, high intermediate or high risk group of IPI, accompanying B symptoms, and bone marrow (BM) involvement were associated with shorter PFS and OS, also. We performed multivariate analyses to establish the prognostic role of B2M with confounding variables inlucidng sex, existence of B symptoms, Han's classification, BM involvement, bulky disease, risk groups according to IPI, revised IPI and National Comprehensive Cancer Network (NCCN)-IPI using three different models to avoid multicollinearity problem, each including IPI, revised IPI and NCCN-IPI. With adjustment for IPI, elevated B2M was demonstrated as a significant prognostic factor for PFS (HR=1.9, 95% CI: 1.3-2.6; p<0.001) and OS (HR=2.1, 95% CI: 1.4-3.0; p<0.001). After adjustment for revised IPI and NCCN-IPI with above confounding factors, elevated B2M still maintained statistical significance in PFS and OS, respectively (Table 2). The relevance of prognostic value of serum B2M in different risk groups by IPI was shown in subgroup analysis. Serum B2M was significantly associated with PFS and OS in both lower-risk group (IPI, 0-2) and higher-risk group (IPI, 3-5) (Figure 1) Conclusion In conclusion, elevated serum B2M seems to be an independent prognostic factor in patients with DLBCL who were treated with R-CHOP, which warrants further validation. Abstract. Particulate organic matter (POM) derived from permafrost soils and transported by the Lena River represents a quantitatively important terrestrial carbon pool exported to Laptev Sea sediments (next to POM derived from coastal erosion). Its fate in a future warming Arctic, i.e. its remobilization and remineralization after permafrost thawing as well as its transport pathways to and sequestration in marine sediments is currently under debate. We present the first radiocarbon (14C) data set of surface water POM within the Lena Delta sampled in summers 2009–2010 and spring 2011 (n = 30 samples). The bulk Δ14C concentrations varied from −55 to −391‰ translating into 14C ages of 395 to 3920 yr BP. We further estimated the fraction of phytoplankton-derived POM to our samples based on (1) particulate organic carbon to particulate nitrogen ratios (POC:PN) and (2) on the stable carbon isotope (Δ13C) composition of our samples. Assuming that this phytoplankton POM has a modern 14C signature we inferred the 14C concentrations of the soil-derived POM fractions. The results ranged from −258 to −768‰ (i.e. 2340 to 11 700 14C yr BP) for the POC:PN-based scenario and from −191 to −704‰ (i.e. 1640 to 9720 14C yrs BP). Despite the limitations of our approach, the estimated Δ14C concentrations of the soil-derived POM fractions seem to reflect the heterogeneous 14C signal of the Lena River catchment soils covering a range from Holocene to Pleistocene ages. We therefore propose a typical isotopic signature of riverine soil-derived POM with a Δ13C of −26.6 ± 1.1‰ deduced from our data of Lena Delta soils and published values, and a Δ14C concentration of −362 ± 123‰ deduced from our Δ13C-based estimates. These data can help to improve the dual-carbon-isotope simulations used to quantify contributions from riverine soil POM, Pleistocene ice complex POM from coastal erosion, and marine POM in Siberian shelf sediments.

Forest tree biomass is hypothesized to increase in a CO2-enriched atmosphere if mechanisms exist to ensure acquisition of limiting nutrients in forest soils. Investment of additional photosynthate produced at elevated CO2 into mycorrhizal proliferation and root growth may provide one such mechanism. To test this hypothesis, mycorrhizal density and seedling biomass were measured in shortleaf pine (Pinusechinata Mill.) and white oak (Quercusalba L.) grown in unfertilized forest soil in controlled-environment chambers at 360 μL L−1 and 700 μL L−1 CO2. Mycorrhizal density was greater at elevated CO2 in both species after 6 weeks of exposure; in white oak, the increased density persisted for 24 weeks. Root dry weight was increased 76% in P. echinata and 91% in Q. alba at 700 μL L−1 CO2; total seedling dry weight was increased by 66 and 56%, respectively. It is hypothesized that increased photosynthesis at elevated CO2 offsets the carbon requirement for mycorrhizal establishment on shortleaf pine. Greater mycorrhizal density and enhanced 1st-year root growth in both species may facilitate future nutrient acquisition, supporting further biomass increases in an enriched CO2 atmosphere.

Energy Dispersion X-Ray Fluorescence Technique (EDXRF) was employed to study the effects of the fumagina disease on the elementary chemical composition of the leaves. The experimental set up consisted of a Mo X-ray tube (Ksub<FONT FACE=Symbol>µ</FONT> = 17.44 keV) with Zr filter and a Si (Li) detector. The measurements were performed with the infected and healthy leaves of citric plants. The elements Ti, Mn, Fe, Cu and Zn were quantified, with an average DL of 69, 12, 8, 4 and 4 µg.g-1 respectively. The obtained concentration for Fe varied from 44 to 192 µg.g-1 in healthy leaves and from 363 to 704 µg.g-1 in infected leaves with fumagina .

Background. The 6-minute walk test (6-MWT) is used to assess patients with chronic heart failure (CHF). The prognostic significance of the 6-MWT distance during long-term followup (>5 years) is unclear.Methods. 1,667 patients (median [inter-quartile range, IQR]) (age 72 [65–77]; 75% males) with heart failure due to left ventricular systolic impairment undertook a 6-MWT as part of their baseline assessment and were followed up for 5 years.Results. At 5 years’ followup, those patients who died (n= 959) were older at baseline and had a higher log NT pro-BNP than those who survived to 5 years (n= 708). 6-MWT distance was lower in those who died [163 (153) m versus 269 (160) m;P< 0.0001]. Median 6-MWT distance was 300 (150–376) m, and quartile ranges were <46 m, 46–240 m, 241–360 m, and >360 m. 6-MWT distance was a predictor of all-cause mortality (HR 0.97; 95% CI 0.96-0.97; Chi-square = 184.1;P< 0.0001). Independent predictors of all-cause mortality were decreasing 6-MWT distance, increasing age, increasing NYHA classification, increasing log NT pro-BNP, decreasing diastolic blood pressure, decreasing sodium, and increasing urea.Conclusion. The 6-MWT is an important independent predictor of all-cause mortality following long-term followup in patients with CHF.

Objective The aim of the current study was to determine the incidence of organ function preservation failure (OFPF) in patients with head and neck squamous cell carcinoma (HNSCC) treated by (chemo)radiotherapy and to identify its risk factors. Study Design Retrospective cohort analysis. Setting Tertiary cancer care center. Subjects and Methods A single-center retrospective cohort analysis was done (n = 703) in which OFPF after (chemo)radiotherapy was assessed. OFPF was defined as local failure or pure functional failure in the absence of local failure because of major surgical intervention (total laryngectomy, commando resection, permanent tracheostomy) or feeding tube dependence >2 years. Results OFPF occurred in 153 patients (21.8%). Reasons for OFPF were local failure in 103 patients (14.6%) and functional failure in 50 patients (7.2%). Evidence of functional failure included need for total laryngectomy (n = 9, 1.3%), commando resection (n = 2, 0.3%), permanent tracheostomy (n = 16, 2.3%), and/or long-term feeding tube for functional reasons (n = 23, 3.3%). In a Cox proportional hazards model, OFPF was worse for patients with T4 tumors (hazard ratio [HR] <0.5 and P < .001 for all other stages), for laryngeal vs oropharyngeal cancer (HR, 1.83; 95% confidence interval [CI], 1.20-2.79, P = .005, hypopharyngeal not significant), and for smokers (HR, 1.68; 95% CI, 1.10-2.56, P = .015). Exploratory multivariate analysis by tumor site showed that T4 tumor and pretreatment tracheostomy were the strongest predictive factors for OFPF in laryngeal and hypopharyngeal carcinoma while T4 tumor and smoking were predictive for poor OFPF in oropharyngeal carcinoma. Conclusion This work shows a detrimental effect of smoking on functional outcomes after (chemo-)radiotherapy for HNSCC. Moreover, T4 tumor, laryngeal subsite, and pretreatment tracheostomy are strong predictors of OFPF.

ObjectivesTo assess how biological disease-modifying antirheumatic drugs (bDMARDs), glucocorticoids and disease activity affect risk ofStaphylococcus aureusbacteraemia (SAB) in patients with rheumatoid arthritis (RA).MethodsIn a nationwide cohort of patients with RA from the DANBIO registry, we conducted a nested case–control study including first-time microbiologically verified SAB cases from 2010 to 2018 and incidence density matched controls (1:4 by sex, age). We interlinked Danish registries and identified antirheumatic treatments, RA-specific clinical characteristics, comorbidities and socioeconomic status. The relative risk of SAB was assessed by adjusted ORs with 95% CIs and number needed to harm (NNH) reflected the absolute risk.ResultsAmong 30 479 patients, we identified 180 SAB cases (incidence rate: 106.7/100 000 person-years) and matched 720 controls (57% women, median age 73 years, IQR: 65–80). Risk of SAB was increased in current (OR 1.8 (95% CI 1.1 to 3.2)) and former bDMARD users (OR 2.5 (95% CI 0.9 to 7.0)), and in current users of oral glucocorticoids ≤7.5 prednisolone-equivalent mg/day (OR 2.2 (95% CI 1.3 to 4.0) and >7.5 mg/day (OR 9.5 (95% CI 3.9 to 22.7)) (non-use as reference). ORs for moderate/high disease activity compared with remission were 1.6 (95% CI 0.8 to 3.3)/1.5 (95% CI 0.6 to 4.3). Risk was increased in patients with longstanding RA (>10 years vs ≤3 years, OR=2.4 (95% CI 1.1 to 5.3)). The NNH was 1172(95% CI 426 to 9374) for current use of bDMARDs and 110(95% CI 43 to 323) for glucocorticoids >7.5 mg/day.ConclusionWe identified a dose-dependent increased risk of SAB in patients with RA currently using oral glucocorticoids. Daily use of >7.5 mg appeared to be a clinically relevant risk factor, whereas the absolute risk was low for bDMARDs. No clear impact of disease activity was found.

Excessive mortality from spawning through the first year of life is likely responsible for recruitment failures in many lake trout (Salvelinus namaycush) populations. We evaluated relationships between interstitial predator species and lake trout egg deposition in three regions of the Great Lakes. The introduced crayfish Orconectes rusticus dominated the nearshore zone in northern Lake Michigan, whereas other sites in Lake Michigan and Parry Sound were dominated by native Orconectes propinquus. In Lake Champlain, sculpin (Cottus spp.) were the most common interstitial predator, yet densities were comparable with those in Lake Michigan. Predator densities in Lake Michigan (mean ± standard error = 15.5 ± 0.4 predators·m–2) were twice those in Lake Champlain or Parry Sound (6 ± 1 predators·m–2). Densities of eggs were higher in Lake Champlain (2994.1 ± 398.3 eggs·m–2) and Parry Sound (454.7 ± 36.3) than in Lake Michigan (7.5 ± 1.3; Kruskal-Wallis, P ≤ 0.05). Ratios of eggs to predator provide simple measures of the effectiveness of lake trout rehabilitation efforts. The average ratio calculated from 10 successful spawning locations is 190 eggs·predator–1·m–2. Ratios may help establish target egg densities for restoration efforts and the concentration of spawning adults required to allow survival to the fry stage.

Abstract Background: The use of recovery tests has been proposed to disclose interferences from anti-thyroglobulin antibodies (TgAbs) in thyroglobulin (Tg) assays. We studied the value of a recovery test in Tg measurement by a new commercial IRMA. Methods: Blood samples were collected from 153 patients with untreated Graves disease. Tg and TgAbs were measured by IRMA and RIA, respectively (Dynotest Tg-plus and Dynotest anti-Tgn; Brahms Diagnostica). The recoveries of added amounts of Tg were calculated for each serum. Results: TgAbs were detected in 72 of the 153 patients (47%). The recovery test results for the 81 TgAb-negative sera (median, 101%; range, 80–115%) were identical to the results for the 91 controls (median, 102%; range, 80–124%). By contrast, significantly lower recovery test results were observed for the 72 TgAb-positive sera (median, 79%; range, 60–103%; Z = −8.363; P &lt;0.0001). In the 34 of the 72 TgAb-positive sera with a normal recovery test, Tg concentrations were significantly lower (median Tg, 13.6 μg/L; range, 1.1–360 μg/L) than those measured in the TgAb-negative sera (median, 107 μg/L; range, 1.2–700 μg/L; Z = −3.797; P &lt;0.0001). Conclusions: Tg values were decreased in TgAb-positive sera even when the results of the recovery tests were normal. This test should not be used alone to determine the validity of a serum Tg measurement in Graves disease.

Objectives:This study evaluated the prevalence of dental trauma for patients attending the emergency dental clinic at the University of Alberta Hospital between 2006-2009. Patients’ examination and treatment charts were reviewed.Methods:Total number of patients’ charts was 1893.The prevalence of different types of trauma was 6.4 % of the total cases (117 patients). Trauma cases were identified according to Ellis classification and as modified by Hollandet al.,1988.Results:Logistic statistical model showed that 21.7% were Ellis class I trauma, 16.7% were Ellis class II trauma, and 6.7% were Ellis class III. In addition, 11.7 % presented with avulsion, 7.5 % presented with dentoalveolar fracture and 7.5% presented with sublaxation. Also, 17.55 % presented with tooth displacement within the alveolar bone, 3.3 % presented with crown fracture with no pulp involvement, 4.16 % presented with crown fracture with pulp involvement and 3.3 % presented with root fracture. In conclusion, the general prevalence of dentoalveolar trauma in patients attending the emergency clinic at the University of Alberta is less than other reported percentages in Canada or other countries.

Data collected from commercial embryo transfer programs in 63 farms in China during June 2002 to December 2005 was analyzed to examine the effects of various factors (biopsy, freezing, sample size, embryo development and quality, in vitro culture, and recipient quality) on pregnancy rates of in vivo-biopsied embryos. Embryos were flushed from superovulated dairy cattle and subjected to a biopsy for sexing determination using protocols and sexing kits supplied by AB Technology Ltd. Fresh embryos were implanted on the same day or frozen with AG freeze medium (AB Technology Ltd., Pullman, WA, USA) for later transfer. Recipients were synchronized with CIDA + PG protocols. Embryos were cultured in 6-well dishes containing 1.3 mL of holding medium (AB Technology Ltd.) in each well at room temperature (20–25�C) for examination of embryo survival in vitro. The chi-square test was used in statistic analysis. The implantation of fresh embryos after biopsy did not affect pregnancy rates (49.6%, 257/518) compared to that of non-biopsied fresh and frozen–thawed embryo groups (52.9%, 47/140 and 46.6%, 177/380, respectively). However, for biopsied embryos subjected to frozen and thawed procedures before implantation, particularly for those subjected to the removal of a larger biopsy, a reduced pregnancy rate was observed (41.8%, 297/710; P &lt; 0.01). Pregnancy rates among biopsied embryos at 3 different development stages (morula-early blastocyst, blastocyst, and expanded blastocyst) were not different. Similar results were found between embryo groups of grade 1 and 2. A significant decrease in pregnancy rate (0/10) was observed with embryos held in vitro for a longer period of time (&gt;5 h), suggesting detrimental effects of in vitro conditions on embryo survival. The highest pregnancy rate (68.0%) was observed in recipients synchronized for the first time before being implanted with biopsied embryos. Significant decreases in such rates were found in recipients synchronized for the second or third times or those with an abortion history at the first or second synchronization-implantation treatment (P &lt; 0.01). Better pregnancy rates (45.6%, 41/90; 46.1%, 76/165; and 45.5%, 5/11) were obtained for recipients implanted with biopsied embryos at Days 7.5, 8.0, and 8.5 post-heat detection, respectively, compared to 16% at Day 7 (3/18, P &lt; 0.05). It is concluded that mechanical treatment (cutting) does not reduce the survival of biopsied embryos; however, cryopreservation reduces their ability to survive in vivo. The analyses also suggest that holding embryos in vitro should not be longer than 5 h unless more favorable in vitro conditions can be provided. To achieve better results of implantation of biopsied embryos, embryo transfer should be performed during 7.5–8.5 days post-estrus, and the healthy recipients synchronized for the first time should be used.

Co-continuous degradable polymer-ceramic composites were produced via in-situ polymerization of (D,L-lactic)acid monomer within a porous β-tricalcium phosphate matrix. The mechanical properties of both the composite and the unfilled porous ceramic were investigated with compressive testing. The average stress to failure increased from 1.3±0.1 MPa for the unfilled ceramic matrix to 82±2 MPa for the composite. The Young’s modulus increased from approximately 20 MPa to 700±42 MPa. A combination of X-ray micro-tomography and mechanical testing provided insight into the failure mechanisms of the composite. Stress may be deflected by crack bridging around the polymer phase leading to debonding of the polymer along the crack lines.

It has a long-lasting prolonged effect. Green pear cuttings K-1, K-2, OHF 333, Piro II had the greatest rootability when using the plant growth regulator kornevin (30.0 mg/l) (from 85.0 to 87.7%). Without the use of plant growth regulators, the forms K-1, K-2, OHF 333, Piro II had high rooting rates of green pear cuttings (from 70.5 to 75.9%). The highest growth height when using the plant growth regulator kornevin (30.0 mg/l) was possessed by the rootstock forms of the Caucasian pear, 4-26, 4-39, K-1, K-2, OHF 333, Piro II, this indicator varied from 12.5 to 14.0 cm. The largest diameter of the conditional root neck (from 1.3 to 1.6 cm) had the forms of pears Caucasian, 4-26, 4-39, K-1, K-2, OHF 333, Piro II and varieties ‘Pervomayskaya’, ‘Yakovlevskaya’, ‘Avgustovskaya Rosa’, ‘Irista’, ‘Chudesnitsa’, ‘Fevral'skiy Suvenir’. The largest number of roots (from 7.5 to 8.5 pcs.) were demonstrated by the forms of the Caucasian pear, 4-26, 4-39, K-1, K-2, OHF 333, Piro II. The largest root length (from 9.0 to 9.8 cm) was possessed by the rootstocks of the Caucasian pear, 4-26, 4-39, K-1, K-2, OHF 333, Piro II and varieties ‘Nika’, ‘Avgustovskaya Rosa’, ‘Irista’, ‘Fevral'skiy Suvenir’. Without the use of plant growth regulators, the largest plant height (from 10.0 to 12.0 cm) was possessed by the forms of Caucasian pear, 4-26, 4-39, K-1, K-2, OHF 333, Piro II. The largest number of roots (from 5.0 to 6.6 pcs.) had pear shapes 4-39, K-2, OHF 333, Piro II and varieties ‘Svetlyanka (k)’, ‘Nika’, ‘Pervomayskaya’. The largest root length without the use of plant growth regulators was characterized by the forms of the Caucasian pear, 4-26, 4-39, K-1, K-2, OHF 333, Piro II and varieties ‘Nika’, ‘Fevral'skiy Suvenir’, this indicator varied from 7.0 to 7.8 cm.

Aim. To investigate the utility of fundus autofluorescence (FAF) and optical coherence tomography (OCT) in the evaluation of cystoid macular edema (CME) following cataract surgery.Materials and Methods. Forty eyes of 29 patients undergone phacoemulsification, with posterior chamber intraocular lens implantation surgery. Central macular thickness (CMT) of the patients was evaluated using OCT and FAF preoperatively and postoperative 1st, 30th, 60th, 90th, and 180th days.Results. CME was detected in three eyes (7.5%) of two patients using OCT. Hyperautofluorescence (HAF) was detected in two of these three eyes and resolved with treatment. In the remaining 37 eyes without CME, there was a significant increase in visual acuity when compared to preoperative values () Mean macular thicknesses (MMT) of the eyes without CME were 174 ± 20 μm preoperatively and 179 ± 22 μm at day 1, 178 ± 19 μm at 1st month, and 168 ± 10 μm at 6th month postoperatively. In the eyes with CME, the MMTs, measured with OCT were 189 ± 23 μm preoperatively and 432 ± 361 on day 1, 343 ± 123 μm at 1st month, 345 ± 196 at 2nd month, and 200 ± 36 μm at 6th month postoperatively.Conclusion. We found a moderate increase in CMT in the first 3 months postoperatively, in the eyes without CME which did not cause visual disturbances. FAF is a noninvasive, rapid method for the evaluation and follow-up of CME following cataract surgery.

The molecular mechanisms of lung injury and fibrosis are incompletely understood. MicroRNAs (miRNAs) are crucial biological regulators that act by suppressing their target genes and are involved in a variety of pathophysiological processes. To gain insight into miRNAs in the regulation of lung fibrosis, total RNA was isolated from mouse lungs harvested at different days after bleomycin treatment, and miRNA array with 1,810 miRNA probes was performed thereafter. MiRNAs expressed in lungs with bleomycin treatment at different time points were compared with miRNAs expressed in lungs without bleomycin treatment, resulting in 161 miRNAs differentially expressed. Furthermore, miRNA expression patterns regulated in initial and late periods after bleomycin were identified. Target genes were predicted in silico for differentially expressed miRNAs, including let-7f, let-7g, miR-196b, miR-16, miR-195, miR-25, miR-144, miR-351, miR-153, miR-468, miR-449b, miR-361, miR-700, miR-704, miR-717, miR-10a, miR-211, miR-34a, miR-367, and miR-21. Target genes were then cross-referenced to the molecular pathways, suggesting that the differentially expressed miRNAs regulate apoptosis, Wnt, Toll-like receptor, and TGF-β signaling. Our study demonstrated a relative abundance of miRNA levels in bleomycin-induced lung fibrosis. The miRNAs and their potential target genes identified may contribute to the understanding of the complex transcriptional program of lung fibrosis.

Objective: The objective of this study was to assess the characteristicsof publications by members of the Canadian UrologicalAssociation (CUA) over a 10-year period.Methods: All publications by active CUA members during the periodsJanuary 1993-December 1994 and January 2003-December2004 were reviewed.Results: Of the 487 active members in 1993-1994, 130 (26.7%)were authors a total of 649 times in 641 publications. Externalfunding was acknowledged in 195 (30.4%). There were 131observational studies (20.4%), 127 review articles (19.8%), 58case reports (9.0%), 37 case series (5.8%), and 21 randomizedcontrolled trials (RCTs) (3.3%). Of the 454 active member in2003-2004, 139 (30.6%) were authors a total of 748 times in 705publications. External funding was acknowledged in 237 (33.6%).There were 153 observational studies (21.7%), 124 review articles(17.6%), 52 case reports (7.4%), 49 case series (7.0%), and 46RCTs (6.5%). There were significantly more RCTs and clinical trialsin 2003-2004. The most common journal was The Journal ofUrology in both eras. There were significantly more publicationsin The Canadian Journal of Urology, the British Journal of UrologyInternational and the Journal of Endourology in 2003-2004. Therewere significantly more publications acknowledging industry fundingand more publications citing more than 1 CUA member in2003-2004. The aim of this study was to increase bovine pregnancy rate by intra-vaginal biostimulation penis like device (PLD). A total of 336 heifers/cows were AI, in which PLD was used in 193 animals. Animals were grouped as A (only AI) and B (PLD after AI). The overall pregnancy rate was 70.0%. Pregnancy rate in group B was significantly (P=0.004) higher than that of group A. Cows of >7.5 years old is showed lower pregnancy rate in group A, but higher in group B. The heifers were showed more responsive to PLD than others parity. The parity 5 or more in group B was responded to PLD and showed higher pregnancy rate than that of group A. It was found that age was significantly positive correlation parity whereas significantly negative correlation with breed. ObjectivesTo describe demographic, clinical, radiological and laboratory characteristics, as well as outcomes, of patients admitted for COVID-19 in a secondary hospital.Design and settingRetrospective case series of sequentially hospitalised patients with confirmed SARS-CoV-2, at Infanta Leonor University Hospital (ILUH) in Madrid, Spain.ParticipantsAll patients attended at ILUH testing positive to reverse transcriptase-PCR on nasopharyngeal swabs and diagnosed with COVID-19 between 1 March 2020 and 28 May 2020.ResultsA total of 1549 COVID-19 cases were included (median age 69 years (IQR 55.0–81.0), 57.5% men). 78.2% had at least one underlying comorbidity, the most frequent was hypertension (55.8%). Most frequent symptoms at presentation were fever (75.3%), cough (65.7%) and dyspnoea (58.1%). 81 (5.8%) patients were admitted to the intensive care unit (ICU) (median age 62 years (IQR 51–71); 74.1% men; median length of stay 9 days (IQR 5–19)) 82.7% of them needed invasive ventilation support. 1393 patients had an outcome at the end of the study period (case fatality ratio: 21.2% (296/1393)). The independent factors associated with fatality (OR; 95% CI): age (1.07; 1.06 to 1.09), male sex (2.86; 1.85 to 4.50), neurological disease (1.93; 1.19 to 3.13), chronic kidney disease (2.83; 1.40 to 5.71) and neoplasia (4.29; 2.40 to 7.67). The percentage of hospital beds occupied with COVID-19 almost doubled (702/361), with the number of patients in ICU quadrupling its capacity (32/8). Median length of stay was 9 days (IQR 6–14).ConclusionsThis study provides clinical characteristics, complications and outcomes of patients with COVID-19 admitted to a European secondary hospital. Fatal outcomes were similar to those reported by hospitals with a higher level of complexity.

Buffering agents have not been comprehensively profiled in terms of their capacity to influence water retention prior to exercise. The purpose of this investigation was to profile the fluid retention characteristics of sodium bicarbonate (BIC) and sodium citrate (CIT) to determine the efficacy of these buffering mediums as hyperhydrating agents. Nineteen volunteers (13 males and six females; age = 28.3 ± 4.9 years) completed three trials (randomized and cross-over design). For each trial, a baseline measurement of body mass, capillary blood, and urine was collected prior to ingestion of their respective condition (control condition [CON] = 25 ml/kg artificially sweetened water; BIC condition = CON + 7.5 g/L of sodium in the form of BIC; CIT condition = CON + 7.5 g/L of sodium in the form of CIT). The fluid loads were consumed in four equal aliquots (0, 20, 40 and 60 min; fluid intake was 1.972 ± 361 ml [CON]; 1.977 ± 360 ml [BIC]; 1.953 ± 352 ml [CIT]). Samples were recorded at 20 (body mass and urine) and 60 min (blood) intervals for 180 min. Blood buffering capacity (HCO3−) was elevated (p < .001) in both BIC (32.1 ± 2.2 mmol/L) and CIT (28.9 ± 3.8 mmol/L) at 180 min compared with CON (25.1 ± 1.8 mmol/L). Plasma volume expansion was greater (p < .001) in both BIC (8.1 ± 1.3%) and CIT (5.9 ± 1.8%) compared with CON (−1.1 ± 1.4%); whereas, total urine production was lower in BIC and CIT at 180 min (BIC vs. CON, mean difference of 370 ± 85 ml; p < .001; CIT vs. CON, mean difference of 239 ± 102 ml; p = .05). There were no increases observed in body mass (p = .9). Under resting conditions, these data suggest BIC and CIT induce a greater plasma hypervolemic response as compared with water alone.

Solid Oxide Cells (SOC) are the kind of electrochemical devices that provide reversible, dual mode operation, where electricity is generated in a fuel cell mode and fuel is produced in an electrolysis mode. Our current work encompasses the design, fabrication, and performance analysis of a micro-tubular reversible SOC that is prepared through a single dip-coating technique with multiple dips using conventional materials. Electrochemical impedance and current-voltage responses were monitored from 700 to 800 °C. Maximum power densities of the cell achieved at 800, 750, and 700 °C, was 690, 546, and 418 mW cm−2, respectively. The reversible, dual mode operation of the SOC was evaluated by operating the cell using 50% H2O/H2 and ambient air. Accordingly, when the SOC was operated in the electrolysis mode at 1.3 V (the thermo-neutral voltage for steam electrolysis), current densities of −311, −487 and −684 mA cm−2 at 700, 750 and 800 °C, respectively, were observed. Hydrogen production rate was determined based on the current developed in the cell during the electrolysis operation. The stability of the cell was further evaluated by performing multiple transitions between fuel cell mode and electrolysis mode at 700 °C for a period of 500 h. In the stability test, the cell current decreased from 353 mA cm−2 to 243 mA cm−2 in the fuel cell mode operation at 0.7 V, while the same decreased from −250 mA cm−2 to −115 mA cm−2 in the electrolysis operation at 1.3 V.

Beach variants of popular sports like soccer and handball have grown in participation over the last decade. However, the characterization of the workload demands in beach sports remains limited compared to their indoor equivalents. This systematic review aimed to: (1) characterize internal and external loads during beach invasion sports match-play; (2) identify technologies and metrics used for monitoring; (3) compare the demands of indoor sports; and (4) explore differences by competition level, age, sex, and beach sport. Fifteen studies ultimately met the inclusion criteria. The locomotive volumes averaged 929 ± 269 m (average) and 16.5 ± 3.3 km/h (peak) alongside 368 ± 103 accelerations and 8 ± 4 jumps per session. The impacts approached 700 per session. The heart rates reached 166–192 beats per minute (maximal) eliciting 60–95% intensity. The player load was 12.5 ± 2.9 to 125 ± 30 units. Males showed 10–15% higher external but equivalent internal loads versus females. Earlier studies relied solely on a time–motion analysis, while recent works integrate electronic performance and tracking systems, enabling a more holistic quantification. However, substantial metric intensity zone variability persists. Beach sports entail intermittent high-intensity activity with a lower-intensity recovery. Unstable surface likely explains the heightened internal strain despite moderately lower running volumes than indoor sports. The continued integration of technology together with the standardization of workload intensity zones is needed to inform a beach-specific training prescription.

Изучены классный состав и экстерьер Бай-Талского типа яка породы сарлык, разводимого в ГУП «Бай-Тал» Бай-Тайгинского района Республики Тыва. В данном хозяйстве количество быков-производителей классов «элита» и «элита-рекорд» составило 94,6%, якоматок, отвечающих требованиям I класса и выше, — 89,3%, в том числе классов «элита» и «элита-рекорд» — 49,2%. Якоматки (76,6%) имели возраст 4 года и старше, яки-производители — 5—7 лет. Основная масть яков — черная или черно-пестрая, 15% животных имеют коричневый окрас c отметинами белого цвета. Живая масса производителей и якоматок в исследуемом хозяйстве выше, чем у их сверстников в КФК «Чинге-Шивилиг», на 4,9% (P<0,001) и 1,6% соответственно. Производители и матки ГУП «Бай-Тал» превосходили сверстников по показателям высоты в холке на 9 и 5 см (7,5 и 4,2%); по косой длине туловища — на 5,5 и 4,2 см (4,3 и 3,5%); по глубине груди — на 3,0 и 5,2 (4,3 и 8,5%), ширине груди — на 2,0 и 3,3 см (4,4 и 8,5%), обхвату груди — на 13,0 и 8 см (7,1 и 4,9%) при P<0,01—0,001. Показатель грудного индекса был больше на 0,76 и 0,1%, тазогрудного — на 1,24 и 0,91% сответственно. In this farm, the number of sires of the "elite" and "eliterecord" classes amounted to 94.6%, yak females meeting the requirements of class I and above — 89.3%, including the "elite" and "elite-record" classes - 49.2%. Yak females (76.6%) were 4 years old and older, sire yaks were 5—7 years old. The main color of yaks is black or black-and-white, 15% of animals are brown with white markings. The live weight of sires and yak females in the study farm is higher than that of their peers in Chinge-Shivilig, by 4.9% (P<0.001) and 1.6%, respectively. The sires and queens of Bai-Tal State Unitary Enterprise surpassed their peers in terms of height at the withers by 9 and 5 cm (7.5 and 4.2%); along the oblique length of the body — by 5.5 and 4.2 cm (4.3 and 3.5%); chest depth — by 3.0 and 5.2 (4.3 and 8.5%), chest width — by 2.0 and 3.3 cm (4.4 and 8.5%), chest circumference — by 13.0 and 8 cm (7.1 and 4.9%) at P<0.01—0.001. The thoracic index was higher by 0.76 and 0.1%, the thoracic index — by 1.24 and 0.91%, respectively. The advantage of yaks of the Bai-Tal type is confirmed by the downfall index, which characterizes the best development of body weight, the value of which is higher by 3.1 and 1.3%.

Abstract BACKGROUND: Despite increased risk of comorbidities, overweight may be associated with improved outcome in patients with metastatic cancer. Conversely, tumor cachexia has been identified as a negative predictor of outcome in patients with brain metastasis (BM) from lung cancer. Here we evaluate the association of abnormal body mass index (BMI) and weight change with outcome in patients with BM from different primary tumors. METHODS: Patients with a diagnosis of BM diagnosed and treated at the University Hospital Zurich (n=703) were assessed for associations of BMI, weight change, comorbidities and survival. RESULTS: Compared with patients with normal BMI of 18.5–24.9 kg/m2 who experienced a median overall survival (OS) of 9 months (95% confidence interval (CI) 7.5–10.5), OS was inferior in patients with BMI&lt; 18.5 kg/m2 (OS 6 months, 95% CI 1.6–10.3, p=0.04), but superior in patients with BMI &gt;25 kg/m2 (OS 13 months, 95% CI 11.0–15.0; p=0.033). For patients with documented weight course (n=173 of 703), we report a median relative weight loss of 5% within the first 6 months of BM diagnosis (95% CI 3.3–6.5). Reduction above the median was associated with an unfavorable outcome in this subgroup (weight loss ≧5% 22.0 months, 95% CI 19.2–24.8; weight loss &lt; 5% 14.0 months, 95% CI 11.9–16.). CONCLUSIONS: Despite being associated with a worse cardiovascular risk profile, high BMI is associated with preferable and underweight with poor outcome in BM patients. Conversely, weight loss above median may be a predictor of poor outcome. Future studies need to address the question whether vigorous treatment of tumor cachexia, e.g. by specific nutrition management, might improve outcome of BM patients. In contrast, regimens that are associated with weight loss such as ketogenic diet may be detrimental.

Background. Few large prospective studies have evaluated burden and outcomes of malaria in children with sickle cell anaemia (SCA) in malaria endemic countries. We evaluated the incidence, complications and outcome of malaria in three cohorts of Ugandan children with SCA in three areas with differing malaria transmission. Methods. Cohort 1 were SCA aged 3-9 years, enrolled in the hydroxyurea therapy for neurological and cognitive protection in Uganda (BRAINSAFE II) trial at Mulago Hospital, a low transmission area, while cohort 2 and 3 aged 6 months to 15 years, were enrolled in the dihydroartemisinin-piperaquine (DP) or sulphadoxine-pyrimethamine (SP) for the chemoprevention of malaria in SCA (CHEMCHA) trial at Jinja Hospital (moderate transmission) and Kitgum Hospital (high transmission). In cohort 1, all received SP for malaria chemoprophylaxis and hydroxyurea at 20-30mg/kg/day, while cohorts 2 and 3, received either DP or SP for malaria chemoprevention, and a proportion (10%) received hydroxyurea in the study. Results. A total of 706 participants were enrolled: 267, 249 and 190 at Mulago, Jinja and Kitgum respectively. The mean age was 6.5 (SD 3.4) years; 5.1 (1.7), 7.9 (4.3) and 6.8 (3.5) years for Mulago, Jinja, and Kitgum respectively. Incidence of malaria was 13 (95% CI 6, 20) per 100 child years in low, 38 (95% CI 26, 51) in moderate and 104 (95% CI 64, 144) in the high transmission area, p&lt;0.001. Adjusting for hydroxyurea treatment, incidence of malaria was 33 (95%CI 14, 52) per 100 child year on hydroxyurea and 51 (95%CI 40, 62) per 100 child years without hydroxyurea, p=0.001. The most common SCA-related complications were anaemia (39.6%, 133/336), and pain (27.7%, 93/336) among children with malaria. Over a period of 1042.8 person years, there were 115 admissions for malaria, with an overall incidence of 11 per 100 child year (95% CI 9, 13), and this differed across sites, p&lt;0.001. Overall, 1.5% (11/706) children died, and 18.2% (2/11) deaths were directly related to malaria infection. Conclusion. Malaria incidence remains high among Ugandan children with SCA, and differs by transmission patterns. Prevention strategies should be strengthened, especially in high transmission areas.

Introduction. According to modern concepts one of the key links in the pathogenesis of urolithiasis is metabolic lithogenic disturbances. The study of the complex effect of many factors on the metabolism of urolithiasis patient is the basis of modern scientific research. We studied the frequency of various chemical urinary stones occurrence depending on various levels of uricuria. Materials and methods. Data from of 708 urolithiasis patients (303 men and 405 women) were analized. The results of blood and urine biochemical analysis and chemical composition of urinary stone were studied. The degree of uricuria was ranked by 10 intervals: from 0.4 to 14.8 mmol/day to assess the occurrence of different stones at various levels of uricuria. Results. The incidence of calculi consisting of uric acid also increases with increasing levels of uric acid in the urine. An increase in the level of uricuria above 3.11 mmol/day is observed to increase calcium-oxalate stones occurrence. Decrease in the prevalence of carbonatapatite and struvite stones observed at an increase of urine uric acid excretion. At high levels of uric acid excretion, we found uric acid and calcium oxalate stones most often. Conclusion. Control over the level of urinary acid excretion in urine is important in case of calcium-oxalate and uric acid urolithiasis.

An investigation of the effect of particle size on the performance of vegetable oil recovery by solvent extraction is reported. Experiments were conducted using soxhlet extractor, groundnuts and n-hexane. Samples were grouped into mean particle sizes of 0.25, 0.75, 1.3, 3.3, and 7.5 mm using standard sieves. The effect of particle size was studied for extraction time intervals of 1, 2, 3, 4, 5 and 8 hours. The oil yield, oil recovered per kg solvent used, kg solvent lost per unit time, and the rate of extraction (kg oil recovered per hour) decreased with increasing particle size. Meanwhile, the percent of solvent recovered, the ratio of oilrecovered to the total volatile matter driven off and the kg solvent lost per kg oil recovered, increased with increasing particle size. Based on the normalization of averaged extraction-parameters, a mean particle size of 3.3 mm was observed to be the optimum size.

RESUMO A utilização de plantas com fins medicinais é uma das mais antigas formas de prática medicinal da humanidade. Entretanto, o conceito de “natural” contribui para o pensamento popular e errôneo de que Plantas Medicinais (PM) são sinônimos de produtos seguros, o qual pode ocasionar no desenvolvimento de efeitos adversos ou interações medicamentosas. Neste contexto, foi realizado levantamento etnofarmacológico das Plantas Medicinais (PM) utilizadas no município de Lagarto, SE, Brasil, com ênfase de seu uso por pacientes oncológicos. Um total de 706 moradores foram entrevistados. Foram citadas 80 plantas, das quais 57 foram identificadas em nosso laboratório. O uso de plantas medicinais (MP) para fins terapêuticos foi relatada por 336 (47,65%) entrevistados. As MPs mais utilizados foram: Erva-Cidreira (Lippiaalba (Mill) N. E. Brown - 103, 30,8%), Boldo (Plectranthus barbatus Andr. - 53, 15,7%), e Capim-Santo (Cymbopogon citratus (D.C.) Stapf - 49, 14,6%). Dos entrevistados que relataram o uso de MPs, metade (360, 50,95%) comunicaram ao médico, mas não receberam orientações específicas. As doenças crônicas mais comuns identificadas foram: hipertensão arterial (144, 20,34%), câncer (55, 7,81%) e diabetes (41, 5,89%). Dos pacientes com câncer, cerca de 40% (22) relataram utilizar PMs concomitante com quimioterapia, dado alarmante, visto a possibilidade de interações medicamentosas entre PMs e antineoplásicos. Diante desses dados, foi observado o uso de PM pela população de Lagarto, SE, e por pacientes oncológicos dessa região, porém, sem os mesmos receberem orientações de um profissional qualificado.

BackgroundAlthough temporal artery biopsy (TAB) is the gold standard in the diagnosis of giant cell arteritis (GCA), the specific histopathological findings and their relationship with clinical diagnosis have not been well discussed. In recent 2022 ACR/EULAR GCA classification criteria, the presence of giant cells, predominantly mononuclear cell infiltration, and fragmentation in the internal elastic lamina have been defined for the interpretation of definitive vasculitis. [1]ObjectivesTo re-evaluate TAB histopathological findings of patients with GCA recruited from 4 tertiary centres in Ankara according to the2022 ACR/EULAR Classification criteria and compare frequencies of these items with DCVAS study [2].MethodsTAB reports of patients with GCA from 4 tertiary centres between March 2004 and February 2022 were retrospectively reviewed according to the histopathological items defined in the updated criteria. TAB reports were classified by the local physicians as normal, nondiagnostic, consistent with vasculitis but not definite, and definite vasculitis.ResultsTotally, TAB histopathologic reports of 88 patients with GCA were reviewed. The mean age of the patients at the time of TAB was 69.7(±8.5) years and 46(52.2%) were female. Twenty-eight reports without microscopic details were excluded and reports of 60 patients were reanalysed. Of the 60 patients, 46 (76.6%) met one of the defined histopathological findings of updated criteria (Figure 1). Abstract Aim We investigated the effect of alogliptin and gliclazide on endothelial progenitor cells (EPCs) in type 2 diabetes. Methods Eighty patients with type 2 diabetes and HbA1c between 7.5% and 8.5% were randomized to receive either alogliptin (25 mg/daily) or gliclazide extended-release (30 mg/daily for HbA1c 7.5-8.0% and 60 mg/daily for HbA1c 8.0-8.5%) in combination with metformin for 4 months. At baseline and 4 months, clinical and laboratory parameters of EPCs were determined. Results After 4 months of treatment, alogliptin and gliclazide resulted in a similar significant reduction in HbA1c (%) (8.0±0.3 vs. 7.1±0.2, and 8.0±0.3 vs. 7.0±0.2, respectively; P<0.05) and a similar and significant increase in EPC count (cells/106 WBC) (CD45−CD133+KDR+ : 2.2±1.2 vs. 3.7±1.6, CD45−CD34+KDR+: 3.3±1.8 vs. 4.9±1.8; P<0.05 for alogliptin; CD45−CD133+KDR+: 2.3±1.3 vs. 3.6±1.5, CD45−CD34+KDR+: 3.1±1.3 vs. 4.6±1.7; P<0.05 for gliclazide). Conclusions Both alogliptin and gliclazide demonstrated a beneficial effect in increasing EPCs in poorly controlled type 2 diabetes. As alogliptin and gliclazide exhibit different mechanisms of action, the observed increase in EPCs seems to be due to their glucose-lowering effect.

Источником анализа послужили 15 списков славянского Синаксаря, содержащие тексты на сентябрь–февраль. На основе сплошного сравнения их состава и языковых данных были выделены четыре редакции. В болгарскую редакцию вошли прологи болгарского и сербского происхождения (Григ. 23, БАН 72, Рум. 319, САНУ 53, Пог. 58, Хлуд. 189, Рс 705, Хлуд. 191, Срем. 323), текст которых с небольшими вариациями совпадает с известным церковнославянским переводом Синаксаря. Сербская редакция представлена четырьмя сербскими списками (Пог. 463А, Увар. 70, ХАЗУ III.с.6 и Wuk. 38). Несмотря на наличие точных текстовых параллелей к Соф. 1324, их агиографическим статьям присущи иные содержательные и языковые особенности. В сербской книжной среде происходило не только тиражирование, но и редактирование Синаксаря, поскольку список Wuk. 37 отражает сербскую сокращенную редакцию. Pre-implantation embryos secrete extracellular vesicles (EV) most likely to communicate with the surroundings. The objective of this study was to determine the distribution (size and concentration) of EV secreted by bovine pre-implantation embryos with different developmental competence. The IVF bovine embryos were produced from oocytes recovered from slaughterhouse ovaries. Presumptive zygotes were in vitro cultured (IVC) in groups in 4-well plates (30 zygotes per 500-µL well) using SOFaa medium at 39°C under 5% CO2, 5% O2, and 90% N2 until the morula stage (Day 5 post IVF). Morulae were cultured individually in 96 well at 39°C under until blastulation time (Day 6.5–7.5) in EV-free SOF medium. Culture medium was collected only from embryos that developed to the blastocyst stage that were classified in a group of early (Day 6.5) or late (Day 7.5) blastulation. Blastocysts were kept in culture until Day 11 to assess embryo developmental competence, considering embryo size (>350 µm) and total cell count (>500 blastomeres). For EV analysis, 4 groups were organised a posteriori: G1: Day 6.5-competent; G2: Day 6.5-not competent; G3: Day 7.5-competent; G4: Day 7.5-not competent. The EV in culture media were analysed using a nanoparticle tracking analysis (Nanosight NS300). Statistical analysis was performed using the InfoStat program (Buenos Aires, Argentina). Differences were considered significant at P < 0.05. Early blastulation rate (Day 6.5) was 40.3% (112/278), whereas late blastulation rate (Day 7.5) was 20.5% (57/278), showing a significant difference (P < 0.0001). Embryos derived from Day 6.5 blastocysts have a higher probability (39.3%: 44/112) of posthatching development [until Day 11; Day 7.5, 10.5% (6/57); P = 0.0001]. At Day 11, competent embryos (G1) derived from Day 6.5 blastocysts have a higher diameter and total cell number (447 µm; 688 cells) than those derived from Day 7.5 blastocysts (G3; 405 µm, 598 cells; P < 0.05 for both parameters). It was possible to detect EV from collected medium of individual embryos independent of their competence. Neither the EV size nor the EV concentration was statistically different between Day 6.5 and Day 7.5 blastocysts (without considering their further competence; 2.9 × 108, 147 nm; and 3.0 × 108, 149 nm, respectively). However, independent of the day of blastulation, competent embryos had a significantly lower concentration of EV (2.7 × 108 v. 3.3 × 108; P = 0.03). Moreover, competent embryos from early and late blastocysts (G1 and G3) tend to produce a lower amount of EV (G1: 2.8 × 108; G2: 3 × 108; G3: 2.6 × 108; G4: 3.5 × 108; P = 0.05). Furthermore, EV concentration was statistically different between G3 and G4 (P = 0.002). No differences in EV size were observed among groups (G1: 145 nm; G2: 148 nm; G3: 146 nm; G4: 151 nm). Our results provide an initial approach to study the EV secreted by individual pre-implantation embryos to assess their competence. From these results, we can conclude that blastulation time affects the future development of bovine embryos and a model based on blastulation time and EV secretion could be a simple noninvasive tool to improve embryo selection.

Abstract. We undertook a comparison of human growth hormone (hGH) binding and metabolic responses in rat adipocytes of epididymal, subcutaneous, and retroperitoneal origin to determine whether the site of fat depot biopsy might affect the response to hGH stimulation. The results showed highest specific binding in epididymal (3.6%), followed by subcutaneous (2.3%) and retroperitoneal adipocytes (1.5%); half-maximal binding was achieved at 14–18 ng/ml hGH for the three sites. Scatchard analysis of the binding data from each site was linear; there was no significant difference in binding affinities (2.1 to 3.3 × 109, m−1), but the number of binding sites was statiticially higher in epididymal (9.8 × 103) as compared to subcutaneous (7.5 × 103, P < 0.05) and retroperitoneal cells (3.3 × 103, P < 0.01). Stimulation with 5 to 2500 ng pituitary hGH produced a dose-related increase in glucose incorporation, with the largest increase in epididymal fat cells (31%, P <0.05) followed by subcutaneous cells (18%, P < 0.05); no significant increase was seen with retroperitoneal cells. Biosynthetic hGH produced a similar pattern of glucose incorporation in the three sites. Addition of hGH antibodies blocked the glucose incorporation in epididymal adipocytes using both pituitary-derived and biosynthetic hGH. It seems clear that this insulin-like effect is caused by hGH, not an insulin-like impurity. We conclude that the number of binding sites, perhaps related to adipose cell size, differs in adipose tissue from different locations and this influences the metabolic response to hGH stimulation.

Background: Robotic prostatectomy techniques are evolving rapidly as the procedure gains popularity and continues to be compared to the gold standard of open retropubic radical prostatectomy. Our objective is to report the operative technique and outcomes of 700 consecutive robotic radical prostatectomies performed by a single surgeon at Mount Sinai Medical Center between May 2007 and October 2008. Surgical Procedure: Key aspects of our technique include 1) dissection of the bladder neck first; 2) minimal to no use of cautery from posterior bladder neck dissection onward; 3) leaving endopelvic fascia intact until after neurovascular bundles dissected; 4) preservation of a wide margin of endopelvic fascia; 5) posterior dissection and nerve-sparing in a medial to lateral direction; 6) cold transection of the dorsal venous complex without prior ligation; and 7) posterior bladder neck reconstruction. Results: Mean OR time from skin incision to skin closure was 124 minutes [48-266]; mean robotic time was 88 minutes [36-190]. Mean EBL was 69.3ml [5-400]. Mean and median length of stay was 1 day. Overall complication rate was 3.3% with no mortalities and no conversions to open or laparoscopic approaches. The overall positive margin rate (PMR) was 11.9%. PMR was 1.4% for pT2a, 0% for pT2b, 8.3% for pT2c, 39.7% for pT3a, and 56.7% for pT3b. Biochemical recurrence rate at one year was 1.7%. Continence rate by 12 months was 94%. Potency rate by 12 months was 83%. Conclusions: Both perioperative and postoperative outcomes of our series of robotic prostatectomies performed by a single surgeon at Mount Sinai Medical Center demonstrate the superb outcomes that can be achieved through this modality of treatment.

El propósito del estudio fue evaluar la respuesta superovulatoria y embrionaria en alpacas con gonadotropina coriónica equina (eCG) según el número de folículos reclutados >3 mm en la onda folicular (OF). Se seleccionaron 26 alpacas del CIP “Chuquibambilla”, Universidad Nacional del Altiplano, Puno, Perú, para conformar dos grupos experimentales: GE01 (n=15 alpacas) con menos de tres folículos >3 mm y GE02 (n=11 alpacas) con más de cuatro folículos >3 mm de diámetro al reclutamiento folicular pos-inducción de la ovulación de un folículo >7 mm con 0.0042 mg de acetato de buserelina (AB). Ambos grupos fueron evaluados por ecografía cada dos días hasta inicios de la tercera onda folicular. Posteriormente, hembras con folículos >7 mm fueron inducidas a la ovulación con AB (día 0), se verificó la ovulación a las 36 h y se administró 700 UI de eCG. Al día 6.5 se hizo el empadre y 7.5 días después se colectaron los embriones. La respuesta superovulatoria fue similar en GE01 y GE02 con 4.4 ± 4.0 y 4.7 ± 3.7 folículos entre >3 y <7 mm; 7.5 ± 3.7 y 10.1 ± 6.3 folículos >7 mm; y 7.2 ± 3.3 y 8.6 ± 5.0 cuerpos lúteos, respectivamente. Además, GE01 y GE02 presentaron 1.6 ± 0.5 y 2.8 ± 1.3 folículos anovulatorios luteinizados y 2.0 ± 1.0 y 2.0 ± 2.2 folículos anovulatorios, respectivamente. En 80% de alpacas en GE01 se colectaron 2.5 ± 1.4 embriones y en 70% de alpacas de GE02 se colectaron 2.3 ± 1.1 embriones. Así mismo, se colectaron 25 embriones transferibles y 5 no transferibles en GE01 (n=15) y 12 embriones transferibles y 4 no transferibles en GE02 (n=10). Los resultados sugieren que no hay diferencia en la respuesta ovárica y colección embrionaria en alpacas superestimuladas con eCG a pesar de una marcada diferencia de folículos emergentes ≥3 mm en la primera OF y que no se hizo sostenible en la segunda y tercera OF.

68 Background: Previous retrospective analyses have revealed higher response rates and a trend towards longer progression-free survival (PFS) and overall survival (OS) in response to immune checkpoint inhibitors (ICIs) in overweight patients. There are few reports concerning the association between the overweight state and the efficacy of ICIs specifically in non-small cell lung cancer (NSCLC) patients. We investigated the association between the body mass index (BMI) and the efficacy of ICIs in patients with NSCLC. Methods: Patients with advanced NSCLC who received ICI therapy (nivolumab or pembrolizumab) at the National Cancer Center Hospital from January 2016 to December 2018 were included in this retrospective cohort study. Based on their BMI, the patients were categorized into the overweight (A) group (BMI ≥25) and the non-overweight (B) group (BMI < 25). The PFS was compared between the two groups as the primary outcome. Results: Data of a total of 323 patients (median age, 63 years) were analyzed; 87 (26.9%)/43 (13.3%)/193 (59.8%) patients received pembrolizumab as 1st line therapy, pembrolizumab as 2nd line therapy, and nivolumab, respectively. Tumor proportion score was ≥50% in 58.4% (139/238) patients. The ECOG-PS was ≥2 in 37 patients (11.5%). The median body weight was 58.1, and the median BMI was 21.4. Of the 323 patients, 46 (14.2%) were categorized into group A (overweight) and 277 (85.8%) into group B (non-overweight). The median PFS and OS in two groups were as follows: A, 6.9 m/B, 5.6 m (HR 0.84, 95% CI [0.57-1.25], p = 0.38), and A, 22.3 m/B, 15.4 m (HR 0.90, 95% CI [0.56-1.43], p = 0.64), respectively. In accordance with the ICI regimen that the patients received, the PFS was A, 7.9 m/B, 7.8 m (HR 0.86, 95%CI [0.37-2.04], p = 0.74) in the patients who received pembrolizumab as 1st line therapy, A, 7.5 m/B, 5.3 m (HR 0.60, 95% CI [0.18-2.00], p = 0.40) in the patients who received pembrolizumab as 2nd line therapy, and A, 6.5 m/B, 4.4 m (HR 0.84, 95% CI [0.52-1.36], p = 0.48) in the patients who received nivolumab. Conclusions: Overweight NSCLC patients treated with ICIs showed a trend (non-statistically significant) towards a longer PFS as compared to non-overweight patients.

ObjectivesIn Malawi, a recent infection testing algorithm (RITA) is used to characterise infections of persons newly diagnosed with HIV as recent or long term. This paper shares results from recent HIV infection surveillance and describes distribution and predictors.SettingData from 155 health facilities in 11 districts in Malawi were pooled from September 2019 to March 2020.ParticipantsEligible participants were ≥13 years, and newly diagnosed with HIV. Clients had RITA recent infections if the rapid test for recent infection (RTRI) test result was recent and viral load (VL) ≥1000 copies/mL; if VL was <1000 copies/mL the RTRI result was reclassified as long-term. Results were stratified by age, sex, pregnancy/breastfeeding status and district.Results13 838 persons consented to RTRI testing and 12 703 had valid RTRI test results and VL results after excluding clients not newly HIV-positive, RTRI negative or missing data (n=1135). A total of 12 365 of the 12 703 were included in the analysis after excluding those whose RTRI results were reclassified as long term (n=338/784 or 43.1%). The remainder, 446/12 703 or 3.5%, met the definition of RITA recent infection. The highest percentage of recent infections was among breastfeeding women (crude OR (COR) 3.2; 95% CI 2.0 to 5.0), young people aged 15–24 years (COR 1.6; 95% CI 1.3 to 1.9) and persons who reported a negative HIV test within the past 12 months (COR 3.3; 95% CI 2.6 to 4.2). Factors associated with recent infection in multivariable analysis included being a non-pregnant female (adjusted OR (AOR) 1.4; 95% CI 1.2 to 1.8), a breastfeeding female (AOR 2.2; 95% CI 1.4 to 3.5), aged 15–24 years (AOR 1.6; 95% CI 1.3 to 1.9) and residents of Machinga (AOR 2.0; 95% CI 1.2 to 3.5) and Mzimba (AOR 2.4; 95% CI 1.3 to 4.5) districts.ConclusionsMalawi’s recent HIV infection surveillance system demonstrated high uptake and identified sub-populations of new HIV diagnoses with a higher percentage of recent infections.
