Анотація:
Whether left ventricular (LV) contractility changes during ventilation with positive end-expiratory pressure (PEEP) remains controversial. To assess LV inotropic state during PEEP using a load-independent index, we generated end-systolic pressure-volume relationships (ESPVRs) in eight closed-chest, chronically instrumented, anesthetized dogs undergoing 0 [zero end-expiratory pressure for the 1st time (ZEEP1)], 5 (PEEP-5), 10 (PEEP-10), and again 0 (ZEEP2) cmH2O PEEP. LV volume was calculated from three orthogonal internal diameters (sonomicrometry), and LV pressure was measured using an implanted transducer. ESPVRs at each level of PEEP were generated by transient inflation of a vena caval occluder. Despite significant decreases in cardiac output with PEEP-5 (1.81 +/- 0.38 l/min, means +/- SE; P less than 0.05) and PEEP-10 (1.70 +/- 0.46; P less than 0.01) with respect to ZEEP1 (2.12 +/- 0.41), no change was found in the slope (ZEEP1: 6.99 +/- 1.03 mmHg/ml; PEEP-5: 7.48 +/- 1.20; PEEP-10: 7.17 +/- 1.02; ZEEP2: 7.38 +/- 1.02), the volume intercept (ZEEP1: 7.4 +/- 3.4 ml; PEEP-5: 6.6 +/- 3.0; PEEP-10: 7.2 +/- 4.0; ZEEP2: 6.6 +/- 3.6), or the new index area beneath the ESPVR (ZEEP1: 304 +/- 98; PEEP-5: 329 +/- 104; PEEP-10: 310 +/- 98; ZEEP2: 343 +/- 114). We conclude that these levels of PEEP do not affect LV contractility as assessed by the ESPVR.

Background:Exposure to severe or chronic life stressors may alter immune function and high levels of subsequent distress have been implicated in autoimmune disease pathogenesis. Post-traumatic stress disorder (PTSD) is a debilitating psychiatric condition affecting 1-12% of the general population1, occurring in response to traumatic events. Growing evidence supports an association between trauma exposure and PTSD with systemic lupus erythematosus (SLE) onset2.Objectives:To cross-sectionally assess PTSD prevalence in a cohort of patients with SLE and to examine its correlation with quality of life.Methods:A 189-item anonymous questionnaire including demographics, disease features, the 9-domain Trauma and Loss Spectrum – Self Report (TALS-SR) and the 8-domain Lupus Quality of Life (Lupus QoL) was administered via web to a cohort of patients with SLE. Patients were classified as PTSD cases based on TALS-SR items corresponding to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria for PTSD.Results:Ninety-nine (95% female and 5% male) patients with a median follow-up of 16.5 years completed the questionnaire. Self-reported fatigue prevalence was 75%. Fifteen patients (15%) reportedly were on psychological and/or psychiatric support. Thirty-one patients (31%) met the DSM-5 criteria for PTSD. The average LupusQoL interdomain score was 80/100. PTSD cases reported significantly lower scores compared to non-cases in three LupusQoL domains: planning (83 vs. 100, p=0.035), body image (85 vs. 95, p=0.031), and fatigue (67 vs. 92, p=0.001). An inverse correlation between TALS-SR scores and Lupus QoL subscales was found (Table1). In particular, the degree of stress secondary to losses or upsetting events was strongly correlated to fatigue intensity (rho= -0.458, p<0.001).Conclusion:PTSD prevalence might be higher in SLE than in the general population and have a detrimental influence on quality of life. Fatigue perception might be more significantly affected by PTSD. Background and Purpose: A variety of definitions for minor stroke have been proposed. We aimed to compare the clinical characteristics and outcomes of minor stroke defined as the National Institutes of Health Stroke Scale (NIHSS) score ≤5 versus ≤3. Methods: We retrieved acute ischemic stroke patients with NIHSS score ≤5 in the CSCA study (China Stroke Center Alliance) between August 2015 and 2019. In-hospital clinical outcomes including all-cause mortality, stroke, and myocardial infarction were compared between the NIHSS score ≤5 and NIHSS score ≤3 groups using absolute standardized differences (ASD). Results: A total of 1 006 798 patients were registered in the CSCA program from 1476 hospitals, 472 352 patients had NIHSS score ≤5, of whom 356 314 patients had NIHSS score ≤3. The in-hospital composite events of death, myocardial infarction, or recurrent stroke were not significantly different between the NIHSS score ≤5 and NIHSS score ≤3 groups (5.6% [26 346/472 352] versus 5.2% [18 682/356 314]; ASD, 1.8). The in-hospital all-cause mortality (0.1% [443/472 352] versus 0.1% [255/356 314]; ASD, <0.01), recurrent ischemic stroke (5.3% [25 026/472 352] versus 5.0% [17 777/356 314]; ASD, 1.4), and hemorrhagic stroke (0.5% [2151/472 352] versus 0.4% [1475/356 314]; ASD, 1.5) were not significantly different between both the NIHSS score ≤5 and NIHSS score ≤3 groups. Conclusions: Our large-scale study identified that minor stroke using NIHSS scores ≤5 and ≤3 as the definition was comparable with each other regarding in-hospital all-cause mortality, recurrent stroke, and hemorrhagic stroke. This observation may be useful for future comparison studies and clinical trial design.

ABSTRACT Background: Cardiac troponin elevation portends a worse prognosis in diverse patient populations. The significance of troponin elevation in patients discharged from emergency departments (EDs)without inpatient admission is notwell known. Methods: Patients without a diagnosis of acute coronary syndrome discharged fromtwo EDs between April 1, 2006, and December 31, 2007, with an abnormal cardiac troponin (troponin positive [TP]) were compared to a troponin-negative (TN) cohort matched for age, sex, and primary discharge diagnosis. Outcomes were obtained by linking with a regional ED and a provincial vital statistics database and adjusted for the following: estimated glomerular filtration rate, do-not-resuscitate status, history of coronary artery disease, Canadian Triage and Acuity Scale, and left ventricular hypertrophy on electrocardiography. The primary outcome was a composite of death or admission to hospital within 1 year. Results: Our total cohort (n 5 344) consisted of 172 TP and 172 TN patients. In the univariate analysis, TP patients had a higher rate of the primary outcome (OR 3.2, 95% CI 2.1–5.0, p &lt; 0.001) and both of its components (p &lt; 0.001). After adjusting for covariates, positive troponin remained an independent predictor of the primary outcome (OR 2.1, 95% CI 1.3–3.4, p 5 0.005) and inpatient admission (OR 2.0, 95% CI 1.2–3.4, p 5 0.006). There was no significant difference in death (OR 1.3, 95% CI 0.6–2.9, p 5 0.5) after adjustment. Conclusions: A positive troponin assay during ED stay in discharged patients is an independent marker for risk of subsequent admission. Our findings suggest that the prognostic power of an abnormal troponin extends to patients discharged from the ED.

Explosive-type strength training may alter kinetics and neuromuscular activity during stair ascent in elderly women. This may improve functional ability. Nineteen women (69.7 ± 3.4 yr) were randomly allocated to strength training (TG; twice per wk, 12 wk) or a control group (CG). Stair ascent was assessed at self-chosen (AFV), standardized (ASV), and maximal velocity (AMV) pre- and posttraining. Ground-reaction force (GRF) and EMG quantified kinetics and neuromuscular activity. After training, TG increased AMV and AFV velocity by 8% (p= .02) and 17% (p= .007), respectively (TG vs.CG; p< .05). This was accompanied by elevated rectus femoris EMG (from 21% to 48%,p< .047). At AFV, TG increased GRF first peak force 4% (p= .047), and CG increased second peak force 5% (p= .036). Muscle coactivation remained unaltered in both groups. Explosive-type strength training led to enhanced stair-climbing performance at maximal and self-chosen speed, reflecting an improved functional ability.

5-Aminosalicylic acid (5-ASA) is available in many countries in tablet, enema and suppository form. A new enteral suspension was studied in six patients with Crohn's disease affecting the upper gastrointestinal tract and in six healthy volunteers. Four grams of 5-ASA dissolved in 240 ml suspension was given via a nasoduodenal tube over 4 h. Blood was drawn at 0, l, 2, 3, 4, 5, 6, 7, 8, 12 and 24 h, and 24 h urine and stool were collected for 5-ASA and n-acetyl-5-ASA analyses. All patients and subjects tolerated the procedure without problems and no side effects were encountered. All subjects and volunteers had normal renal function before and after the study; serum creatinine and blood urea nitrogen did not change. Urine recovery of 5-ASA ranged from 0.6 to 3.4 g (controls) and 0.46 to 3.6 g (patients). Stool recovery of 5-ASA was 0.1 to 0.3 g (controls) and 0.12 to 0.54 g (patients). There was delayed absorption of 5-ASA in patients compared to controls, shown by time to peak concentrations (4 to 6 h versus 3 h). Predominantly urine excretion of the n-acetyl metabolite occurred in both groups, with less than 12% stool excretion. Continued treatment induced remission. An enteral suspension of 5-ASA may be of benefit in selected patients with proximal small bowel Crohn's disease.

Renal function as a sensitive biomarker of aging has been studied in specific pathogen-free (SPF) Fischer 344 rats (n = 211), and results are presented according to animal age (5, 8, 12, 18, 24 mo), sex, and diet (ad libitum vs. 40% calorie restriction). Plasma creatinine concentration, endogenous creatinine clearance, total protein excretion, and albumin excretion were measured. Kidney histology was evaluated by light microscopy. In both calorie-restricted and ad libitum-fed animals, kidney weight (KW) and body weight (BW) showed parallel changes with age. The KW-to-BW ratio was unaffected by age in all groups. There was no alteration in plasma creatinine concentration as a function of age or diet. In these SPF animals there was also no change in glomerular filtration rate with age. In animals fed ad libitum, albumin and protein excretion increased with age (females: 0.39 +/- 0.05 at 5 mo vs. 7.4 +/- 2.6 mg protein.24 h-1.g KW-1 at 24 mo; males: 4.1 +/- 0.6 at 5 mo vs. 15 +/- 3 mg protein.24 h-1.g KW-1 at 24 mo). The higher protein excretion rate in all males at 5 mo reflected the excretion of sex-dependent low-molecular-weight proteins that commenced with sexual maturation. Calorie restriction prevented the age-dependent increase in total protein excretion. Kidney histopathology was positively correlated with total protein and albumin excretion. Microalbuminuria preceded the development of lesions detectable by light microscopy. These observations support the concept that microalbuminuria in this model is a sensitive and early biomarker of nephropathy that can be monitored easily and noninvasively.

To investigate the morphometric and meristic variation of Atlantic salmon (Salmo salar) in North America, juveniles from 16 anadromous and 5 nonanadromous populations were collected from an area extending from Labrador to New York state. The findings from the analysis of these characters were supplemented by an examination of malate dehydrogenase variation on a subset of specimens from selected populations. Newfoundland – Labrador and Gaspé – Maritime populations were found to belong to distinct regional stocks. This conclusion was supported by the accuracy of the morphometric discriminant function and a discontinuity in Mdh-3,4(100) allele frequencies. The lack of a clinal relationship between morphometric characters, latitude, longitude, and number of degree-days above 7 °C also suggested a pattern of distinct regional stocks. Considerable overlap among populations was found for meristic characteristics, and these were considered unsuitable for stock identification purposes. While no somatic differences were found between anadromous and nonanadromous populations, there were significant differences in Mdh-3,4(100) frequencies. The congruence of morphometric and malate dehydrogenase characteristics in delineating regional stocks emphasizes the importance of a multiple character approach in solving stock identification problems.

344 Background: Patients with metastatic pancreatic cancer have a poor outcome and the radiotherapy is typically only given to patients requiring palliation. We analyzed our institutional pancreas database to compare the outcome between chemotherapy alone vs. chemoradiotherapy. Methods: From January 2000 to December 2008, 199 metastatic pancreatic cancer patients were retrospectively analyzed. 13 (6.5%) patients received chemoradiotherapy and 186 (93.5%) patients received chemotherapy alone. Chemotherapy regimens consisted of 5-fluorouracil, gemcitabine, erlotinib, or cisplatin. The follow-up time was calculated from the time of diagnosis to the date of death or the last contact. Kaplan-Meier analysis was used to calculate the overall survival (OS). Results: Median OS was 5.3 months for all patients. Median OS was 4.9 months (0.4–27.0) for patients treated with chemotherapy alone and 7.8 months (0.6–44.1) for those treated with chemoradiotherapy (p = 0.013). Univariate survival analysis of categorical variables for patients treated with chemoradiotherapy revealed that age, race, gender, location of metastatic site, T stage (T3 v. T4) or nodal stage were not significant. However, ECOG performance status (1 v. 2/3) and the dose of radiation (<35 v. >35 Gy) received were associated with improved survival (p = 0.013, p=0.049). Median OS was 12.9 months for ECOG 1 vs. 5.6 months for ECOG 2/3. Median OS was 11.1 months for patients treated with radiotherapy dose > 35 Gy vs. 5.9 months for those who received less than 35 Gy. 3/13 (23%) patients who received chemoradiotherapy lived nearly two years or more. Conclusions: Metastatic pancreatic cancer patients with good performance score may benefit from chemoradiotherapy. Long-term survival was observed in this selected group. No significant financial relationships to disclose.

Acifluorfen {5-[2-chloro-4-(trifluoromethyl)phenoxy]-2-nitrobenzoic acid} applied alone and in mixtures with propanil [N-(3,4-dichlorophenyl)propanamide] reduced rough grain yield of rice (Oryza sativaL. ‘Lebonnet’ or ‘Bond’) by 7% when applied at the late-booting or heading growth stages, averaged over several rates of application. Earlier applications did not reduce rice yield. The highest rate (0.6 kg ai/ha) of acifluorfen alone and in mixtures with propanil at 3.4 kg ai/ha reduced grain yields more than lower rates (0.1 or 0.3 kg ai/ha). Triclopyr {[(3,5,6-trichloro-2-pyridinyl)oxy] acetic acid} alone and in mixtures with propanil reduced rough grain yields of rice by 18% when applied at the late-booting growth stage, averaged over several rates of applications, but did not affect yields when applied at the early-tillering, jointing, or early-booting growth stages. Whole-grain milling yield and germination of rice seed were not affected by acifluorfen or triclopyr alone or in mixtures with propanil, regardless of rate or growth stage of application.

Background:Ocular involvement in sarcoidosis can be present in up to 80% of patients. If not treated, it can lead to significant visually complications (1-5).Objectives:Our aim was to assess the main a) epidemiology and b) clinical features of ocular sarcoidosis in a wide and unselected series from a single university hospital.Methods:Study of a large cohort (n=384) of all consecutive patients diagnosed with sarcoidosis from January 1, 1999 to December 31, 2019. Finally, 344 patients were included according to the ATS/ERS/WASOG criteria (Eur Respir J. 1999;14:735-7).Results:65 (33 men/32 women) of 344 (18.9%) patients had ocular involvement. Mean age at diagnosis was 45.6±15.9 years. The most frequent extraocular clinical clusters were respiratory (80%), osteoarticular (30.8%) and cutaneous (29.2%) (figure 1). Ocular manifestations and complications are shown in table 1. Uveitis (83.1%), orbital lesions (7.7%) and retinal vasculitis (6.2%) were the most common ocular lesions. Median Best Corrected Visual Acuity (BCVA) at diagnosis and after one year of follow-up was 0.6 [0.3-0.8] and 0.9 [0.30-1], respectively. Retinal vasculitis was associated to the worst BCVA outcome, and panuveitis to more frequent and severe complications.Conclusion:Ocular manifestations, especially uveitis, are frequent in sarcoidosis. Purpose: This study investigated within- and between-matches blood lactate (La−) responses across an international Rugby Sevens tournament (5 matches over 2 d) in male and female players. Methods: Earlobe blood samples were taken from 25 professional players around matches: before warm-up (PRE), immediately upon finishing match participation (POST), and 30 minutes postmatch (30 min). Results: POST [La−] (mean [SD], range) for males was 10.3 (3.2; 2.9–20.2) mmol·L−1 and for females was 9.1 (2.3; 3.4–14.6) mmol·L−1. Linear mixed-effects models revealed a decrease in POST [La−] after match 5, compared to match 1. Increased PRE [La−] was found before match 2 (+0.8 [0.6–1.1] mmol·L−1), match 3 (+0.8 [0.5–1.1] mmol·L−1), and match 5 (+0.6 [0.4–0.9] mmol·L−1) compared to match 1 (all P < .001). The [La−] remained elevated at 30 min, compared to PRE (+1.7 [1.4–2.0] mmol·L−1, P < .001), with ∼20% of values persisting >4 mmol·L−1. Higher POST was observed in males compared to females (+1.6 [0.1–3.2] mmol·L−1, P = .042); however, no differences between sexes were found across 30 min or PRE [La−]. No [La−] differences between positions (backs and forwards) were identified. Conclusions: Lactate concentrations above 10 mmol·L−1 are required to effectively simulate the anaerobic demands of international Rugby Sevens matches. Practitioners are advised to individualize anaerobic training prescription due to the substantial variability observed within positional groups. Additionally, improving athletes’ metabolic recovery capacity through training, nutrition, and recovery interventions may enhance physical preparation for subsequent matches within a day, where incomplete lactate clearance was observed.

Telah dilakukan penelitian tentang pengaruh persentase serat sabut pinang dan resin epoksi terhadap sifat fisik dan mekanik papan beton. Papan beton dibuat dengan variasi sampel serat sabut pinang yaitu 0%; 0,2%; 0,4%; 0,6%; 0,8% terhadap volume cetakan. Sifat fisik dan mekanik yang diuji meliputi densitas, porositas, daya serap air dan kuat tekan dengan sampel berukuran 5 x 5 x 1 cm, sedangkan kuat lentur dengan sampel berukuran 20 x 5 x 1 cm. Kuat tekan dan kuat lentur diuji menggunakan universal testing machine (UTM) sedangkan densitas, porositas dan daya serap air dikarakterisasi menggunakan neraca digital. Nilai kuat tekan dan kuat lentur paling tinggi didapat pada persentase serat 0,6%. Densitas yang paling rendah terdapat pada persentase serat 0,8%, sedangkan daya serap air paling tinggi terdapat pada persentase serat 0,8%. Berdasarkan hasil penelitian kuat tekan sudah mendekati standar SNI-03-3449-2002, kuat lentur sudah memenuhi standar SNI 03-3449-2002, densitas sudah memenuhi standar ISO 8335-1987, sedangkan daya serap air dan porositas belum memenuhi standar SNI 03-2105-2006.Kata kunci : papan beton, resin epoksi, kuat tekan, kuat lentur, densitas dan daya serap air.

Mitochondrial (mt) DNA deletions and low folate status, proposed characteristics of carcinogenesis, in relation to human hepatocellular carcinoma (HCC) susceptibility are not clearly understood. We hypothesised that low folate status may modify frequencies of mtDNA deletions in humans, both of which could predispose individuals to HCC development. Biochemical folate status of serum and lymphocytes, and frequencies of mtDNA deletions in lymphocytes were determined in ninety HCC cases and ninety cancer-free healthy controls, individually matched by age and sex. The data revealed that HCC patients had lower levels of serum folate (P = 0·0002), lymphocytic folate (P = 0·040) and accumulated higher frequency of lymphocytic mtDNA deletions (P < 0·0001) than the controls. In the total studied subjects, frequencies of lymphocytic mtDNA deletions were associated with hepatitic B infection (P = 0·004) and HCC incidents (P = 0·001), and were correlated with serum folate (r− 0·155;P = 0·041), lymphocyte folate (r− 0·314;P = 0·0001), levels of glutamate-oxaloacetate transaminase (GOT) (r0·206;P = 0·006), glutamate-pyruvate transaminase (GPT) (r0·163;P = 0·037) and α-fetal protein concentrations (r0·212;P = 0·005). After adjustment for age, sex, lifestyle and one-carbon metabolite factors, individuals with low blood folate ( < 11·5 nmol/l) or high mtDNA deletions (Δ threshold cycle number (Ct)>5·3) had increased risks for HCC (OR 7·7, 95 % CI 1·9, 29·9,P = 0·003; OR 5·4; 95 % CI 1·7, 16·8,P = 0·003, respectively). When combined with folate deficiency (serum folate < 14 nmol/l), the OR of HCC in individuals with high levels of lymphocytic mtDNA deletions was enhanced (OR 13·3; 95 % CI 1·45, 122;P = 0·008). Further controlling for GOT and GPT levels, however, negated those effects on HCC risk. Taken together, the data suggest that biochemical folate status and liver injuries are important modulators to lymphocytic mtDNA deletions. The mt genetic instability that results from a high rate of mtDNA deletions and/or low folate status increased the risk for HCC, which is mediated by clinical hepatic lesions.

Abstract Introduction Obstructive sleep apnea (OSA) is characterized by sleep fragmentation and intermittent hypoxia. Both are known causes of sleepiness and poor sustained attention, i.e., lapses in vigilance, which can be measured using the psychomotor vigilance test (PVT). Crucial to vigilance, the thalamus plays a key role by regulating sensory information essential for sustaining attention. However, to date, the relationship between thalamic activity underlying PVT and its relationship to sleepiness in OSA has not been investigated systematically. Methods A total of 5 newly diagnosed OSA subjects (mean AHI4% = 51±24, 5M, age 42±3 yrs., ESS [Epworth Sleepiness Scale] 11 ± 3.4), 3 healthy controls (1M/2F, age 35±2 yrs., ESS 3±3) and 9 PAP treated OSA subjects (diagnostic AHI4% = 15±12, 6M/3F, age 57±10 yrs., ESS 8±5) who demonstrated &gt;4 hours nightly PAP adherence were studied. Participants performed a PVT during fMRI after an in-lab nocturnal polysomnography. Each fMRI session consisted of 4 runs: two PVT runs interleaved with two control runs. Analyses in AFNI were restricted to the bilateral thalamus with percent signal change used as the primary metric of thalamic activity. Results There was a dose response relationship of ESS to thalamic activity during PVT such that subjects with lowest levels of sleepiness (ESS) exhibited the greatest thalamic activity: untreated OSA (3±2%), CPAP treated OSA subjects (7.2±1.9%), healthy controls (21.8±1.1%); χ2(3) = 7.13, p&lt; 0.05. CPAP treated OSA subjects with persistent sleepiness (ESS≥10) exhibited lower thalamic activity than those without persistent sleepiness (ESS&lt; 10), however it did not reach statistical significance (5.3±3.4% vs. 6.9±2.9%). Conclusion Based on the collected data, the thalamic activity underlying PVT shows a dose response relationship with level of sleepiness. The lower degree of thalamic activity in CPAP treated patients with persistent sleepiness as compared to those without suggesting a level of irreversible injury to the thalamus due to OSA. Object In this paper, the authors' goal was to report the outcome of patients with unruptured intracranial aneurysms undergoing endovascular treatment under conscious sedation (local anesthesia). Methods Between November 5, 2001, and February 5, 2009, the authors treated 340 patients with 358 unruptured aneurysms by using neurointerventional procedures at Millard Fillmore Gates Hospital (Buffalo, New York). The data were retrospectively reviewed for periprocedural safety and long-term follow-up. Results A total of 496 procedures were performed under local anesthesia. Of those, 370 procedures (74.6%) were completed successfully. In 82 procedures (16.5%), an associated medical or technical event occurred. Forty-four procedures (8.9%) were aborted. Rates of overall procedure-related morbidity and mortality were 1.2% (6 of 496) and 0.6% (3 of 496), respectively. The average hospital stay was 1.5 ± 2.5 days. Long-term follow-up was available in 261 (82.1%) of 318 patients whose procedures were performed with local anesthesia. Of those, 246 patients (94.3%) had a good outcome (modified Rankin Scale score ≤ 2), 6 patients (2.3%) had an unfavorable outcome, not related to the procedure, and 9 patients (3.4%) had a poor outcome (modified Rankin Scale score > 2) as a result of the intervention. Conclusions Interventional treatment under conscious sedation (local anesthesia) can be effectively performed in most patients with unruptured intracranial aneurysms and is associated with a short hospital stay and low morbidity and mortality.

AbstractThirty sows were allocated at their first farrowing to five litter size treatments. Litter sizes of six, eight, 10,12 and 14 were established by cross-fostering within 48 h post partum. Milk yield (Y, kg/day) of sows was linearly related to litter size (L) and was described by the equations; Y = 5·98 (s.e. 0·48) + 0·689 (s.e. 0·046)L, R2 = 0·99; residual s.d. = 0·29; P < 0·001 and Y = 8·20 (s.e. 0·46) + 0·324 (s.e. 0·044)1, R2 = 0·95; residual s.d. = 0·28; P < 0·002 for early (day 10 to day 14) and late (day 24 to day 28) lactation, respectively. The composition of milk from sows suckling various litter sizes remained relatively stable but suckling frequency in early lactation increased linearly in response to increased litter size. Litter size significantly affected the average growth rate of individual piglets; piglet growth rate from birth to weaning at 28 days of age decreased from 283 g/day to 202 giday in response to increasing litter size from six to 14. The relationships between milk yield, average piglet growth rate and litter size indicate that the number of functional glands is the major factor influencing milk yield of sows. Increasing the number of functional glands by increasing litter size more than compensates for any decrease in milk output from individual glands.

Regulation of active Na+transport across fetal distal lung epithelial cells (FDLE) by corticosterone (CST), corticotropin-releasing hormone (CRH), and oxygen tension may be crucial for postnatal adaptation. FDLE isolated from 19-day rat fetuses (term: 22 days) were grown on permeable supports to confluent monolayers (duration 3 days) in 2.5, 5, 12, or 20% O2 with 5% CO2-balance N2 and mounted in Ussing chambers for measurement of short-circuit currents ( I sc). FDLE monolayers grown in 20% O2 had significantly higher levels of total I sc and of their amiloride-sensitive ( I amil) and ouabain-sensitive ( I ouab) components than hypoxic cells. Values (μA/cm2 ± SE) for 2.5–5% O2 and 20% O2 were, respectively, I sc5.3 ± 0.2 vs. 8.4 ± 0.3 ( P < 0.001), I amil 3.4 ± 0.2 vs. 4.3 ± 0.2 ( P < 0.01), and I ouab 3.4 ± 0.6 vs. 9.1 ± 0.6 ( P < 0.001). Addition of CST but not CRH to the culture medium at any O2concentration increased I amil. FDLE cells grown at 5% O2 expressed significantly lower levels of α-, β-, and γ-epithelial Na+ channel (ENaC), and of the α1-Na+-K+-ATPase, as determined by Western blotting. We conclude that higher O2concentrations increased total vectorial Na+ transport, and the function of Na+-K+-ATPase and apical amiloride-sensitive Na+ conductance, whereas CST only increased ENaC function.

Polyamideimide resins were prepared using three monomeric reactants: 5-norbornene-2,3'dicarboxylic anhydride (NA), 4-aminobenzoic acid, and 3,4'-oxydianiline (3,4'-ODA). The characteristics of the resins were analyzed by FTIR and elemental analysis. The PAI / Copper composites were prepared at 320°C and a pressure of 30Kg/cm2 (2.9Mpa). The heat resistance, chemical resistance, dielectric constant and dissipation factor were studied. The experimental results show that the peel strength of the composites with no coupling agent was 1.69Kgf/cm (1657N/m) and with 0.6% LICA97 coupling agent was 2.27Kgf/cm (2225N/m) After 24h at 300°C, the dielectric constants of PAI.1 (0.6% LICA97) were 2.46 and 2.47 (1MHz), and their dissipation factors were 0.0101 and 0.0110. After immersian in 10% H2SO4 solution, the peel strength retentions of the laminates were greater than 90%.

Abstract Context Information regarding the impact of parental multiple endocrine neoplasia type 1 (MEN 1) on neonatal outcomes is limited to case reports. Objective To determine the impact of parental MEN 1 on neonatal outcomes. Methods Retrospective cohort analysis of the Tasman 1 MEN 1 kindred stratified by whether birth occurred before (“historical cohort”) or after (“contemporary cohort”) prospective screening commenced. The historical cohort included kindred members born between 1825 and 1984 (n = 341 children with a MEN 1 positive (MEN 1+) parent and n = 314 children with MEN 1 negative (MEN 1–) parents). The contemporary cohort included neonates (n = 52) of MEN 1+ women (n = 21) managed at a tertiary referral hospital between 1985 and 2018. Results Historical cohort: compared with MEN 1– parents, children of MEN 1+ parents were more likely to die postpartum (HR 4.6, P = .046 at 6 months of age). Excess mortality at 15 years of age was observed for children of MEN 1+ mothers (HR 8.50, P = .002) and fathers (HR 3.82, P = .03). Contemporary cohort: neonates of MEN 1+ mothers were more likely to have low birth weight (28.9% vs 6.7%, P = .01), be admitted to a higher care nursery (40.4% vs 17%, P = .02), and require a longer median postnatal stay (5 vs 4 days, P = .009) than the Australian average. Isolated antenatal hypercalcemia did not significantly alter neonatal outcomes. Conclusion Children with a MEN 1+ parent are disproportionately vulnerable postpartum. Neonates of MEN 1+ mothers remain vulnerable despite contemporary care. The excess risk was not fully explained by maternal MEN 1 or antenatal hypercalcemia.

ABSTRACT The effect of dopamine (1 μg/kg per min) on corticosteroid response to ACTH (0·1, 1 and 10 ng/kg per min) was compared with that of a placebo in sodium-replete (150 mmol/day) and -deplete (10 mmol/day) normal man. Dopamine had no effect on aldosterone, cortisol or corticosterone responses in either dietary phase, but increased deoxycorticosterone (897·0 ± 126·4 (s.e.m.) vs 590·0 ±84·3 pmol/l, normal Na+; 1264·2 ±84·3 vs 764·5 ±84·3 pmol/l, low Na+) and deoxycortisol (6·033 ± 0·583 vs 5·048±0·680 nmol/l, normal Na+; 5·112 ± 0·600 vs 4·130± 0·367 nmol/l, low Na+) levels during ACTH administration (all P <0·01). Deoxycorticosterone and corticosterone responses to ACTH were greater during sodium depletion than repletion (both P <0·01). Dopamine therefore increased 11-deoxycorticosteroid concentrations during ACTH-stimulated steroidogenesis. MADB106 rat mammary tumor growth rate was the same in folate replete and supplemented rats, but retarded in the low folate groups. The tumor growth inhibitions in low folate, replete and high folate rats treated with cyclophosphamide were: 53%, 98%, and 97% (P = .048); with 5-fluorouracil (5-FU): 46%, 49%, and 66%; and with doxorubicin: 25%, 55%, and 61%. Significant differences in survival were observed for cyclophosphamide (P = .0084) and 5-FU (P = .025) related to dietary folate content. Thus, folate deficiency impedes tumor growth rate, but supplementation does not accelerate it in folate replete animals. Correction of folate deficiency approximately doubles the efficacy of cyclophosphamide in rats with much less host toxicity. Folate repletion improves survival in 5-FU–treated animals. These studies indicate that nutritional folate status has an important influence on the efficacy and toxicity of some commonly used cancer chemotherapeutic drugs.

Abstract Background: The composition and diversity of editorial board members in oncology journals are critical indicators of the field's inclusivity and global perspective. Despite a historical trend of female underrepresentation on editorial boards of peer-reviewed medical journals, the extent of diversity within oncology's various subspecialties remains less understood. This study aims to elucidate the landscape of gender and economic diversity among editorial boards in three major oncology subspecialties: thoracic, gastrointestinal (GI), and breast oncology. Methods: 5 top oncology journals in thoracic, GI, and breast oncology were selected based on 2022 Journal Citation Reports impact factors (Clarivate). Editorial board members, identified from each journal's website, were assessed for economic diversity using the World Bank's 2024 high-income country classification and gender based on academic profile data. Chi-square tests evaluated the gender and economic diversity differences. Results: A total of 762 editor positions from 15 journals were analyzed across the 3 subspecialties. In thoracic oncology, 64.6% were men, 33.5% women, with 7.3% from non-high-income countries. In GI oncology, 84.5% were men, 10.5% women, with 11.1% from non-high-income countries. In breast oncology, 59.3% were men, 39.7% women, with 2.8% from non-high-income countries. Significant disparities in gender and geographical origin were noted (χ² &lt; 0.01). A majority of GI oncology journal editors are based in Asia (66.1%), while in thoracic and breast oncology, most editors are located in North America and Europe. Among 17 editors-in-chief, 88.2% were men, all from high-income countries. Conclusion: The study highlights a significant gender imbalance and a predominance of editors from high-income countries in oncology journals. This imbalance underscores the need for greater inclusivity and diversity in the editorial boards of prominent oncology journals. The contributions of three major gluconeogenic regulators, glucagon (10(-7) M), alpha-adrenergic agonist phenylephrine (10(-5) M), and beta-agonist isoproterenol (10(-5) M) to hepatic glucose synthesis in liver slices from Fischer 344 rats were examined in relation to age and endurance training. Young (4 mo), middle-aged (12 mo), and old (22 mo) male Fischer 344 rats (n = 66) were divided into trained or sedentary groups. Trained animals were run 10 wk on a treadmill at 75% of maximal capacity, 1 h/day, 5 days/wk. Animals were killed at rest, and sections of liver were removed and sliced in a tissue microtome. Slices were incubated in L-[U-14C]lactic acid, Ringer solution, and one of the aforementioned gluconeogenic regulators. Rates of lactate incorporation into glucose and glycogen were significantly greater in young compared with old animals for all three regulators in both trained and untrained animals. Training elicited a 35, 52, and 63% improvement in lactate incorporation into glucose compared with untrained when the livers of young (16.9 +/- 1.2 vs. 10.9 +/- 1.1 mumol.g protein-1.min-1), middle-aged (12.8 +/- 1.3 vs. 6.1 +/- 1.2 mumol.g protein-1.min-1), and old (11.2 +/- 1.1 vs. 4.1 +/- 0.6 mumol.g protein-1.min-1) animals, respectively, were incubated in glucagon. Rates with phenylephrine followed a similar pattern to that with glucagon across age and training, but absolute rates were significantly lower. No training effect in gluconeogenic rate was found when liver was incubated in the presence of isoproterenol. It is concluded that the gluconeogenic capacity of liver declines with age regardless of the gluconeogenic regulator and that training was able to partially offset age-related declines in glucagon-stimulated and alpha-receptor-mediated gluconeogenesis.

Sedum jinglanii, a new species of Crassulaceae from Mount Danxia in Guangdong, China, is described and illustrated. Phylogenetic analysis based on the internal transcribed spacer (ITS) region of nrDNA suggests that the new species belongs to S. sect. Sedum sensu Fu and Ohba (2001) in the “Flora of China”, and is sister to a clade comprising S. alfredi and S. emarginatum with high support values (SH-aLRT = 84, UFBS = 95) but is distantly related to S. baileyi. The new species is morphologically similar to S. alfredi but it can be distinguished from the latter in its opposite leaves (vs. alternate leaves), its usually wider leaves (0.4–1.2 cm vs. 0.2–0.6 cm), its usually shorter petals (3.4–4.5 mm vs. 4–6 mm), its shorter nectar scales (0.4–0.5 mm vs. 0.5–1 mm), its shorter carpels (1.5–2.6 mm vs. 4–5 mm), and its shorter styles (0.6–0.9 mm vs. 1–2 mm). The new species can be easily distinguished from S. emarginatum which both have opposite leaves by its short, erect or ascending rhizome (vs. long and prostrate rhizome in the latter), shorter petals (3.4–4.5 mm vs. 6–8 mm) and shorter carpels (1.5–2.6 mm vs. 4–5 mm). It can also be easily distinguished from S. baileyi by its short, erect or ascending rhizome (vs. long and prostrate rhizome) and its shorter style (0.6–0.9 mm vs. 1–1.5 mm).

Abstract Treatment intensifications in SCD with HU, TP or SCT are applied in order to reduce SCD related complications but their comparative effects have still to be described. We report our experience concerning the annual check-up performed in SCD pediatric patients. Patients and Methods: Among our cohort of 397 SS/Sb0 pediatric SCD patients, 157 of them were intensified with HU (n= 86), TP (n=104) or SCT (n=36) and some of them received successively HU, TP and SCT. HU was proposed to patients &gt; 3 years of age and having experienced more than 3 VOC/ACS/year or &lt; 7g/dl severe anemia. TP defined as &gt; 4 months program was applied in patients with cerebral vasculopathy defined by an history of stroke or abnormal TCD (&gt; 200 cm/sec). TP was also proposed in patients with HU-failure and in patients with frequent VOC, less than 3 years old. SCT was proposed in patients with an indication of treatment intensification and an available HLA identical sibling donor. Annual check-up were performed in our day-care unit. We analysed 1261 check-ups performed and recorded since 1992 in 341 SS/Sb0 patients (sex: 164 F, 177 M). Median age was 8.8 ± 5.1 years. Mean number of annual check-ups per patient was 3.7 ± 2.8 (range 1 to 13): 816 were performed in non intensified patients, 196 in HU, 123 in TP and 126 in transplanted patients. Categories of age were distinguished: &lt; 2 y of age (n=110), 2–5y (n=244), 5–10y (n=415), 10–15y (n=317) and 15–20y (n=175). Results: Respective follow-up were 4.4 y ± 3.3 in HU, 2.6 y ± 2.6 inTP and 5.8 y ± 4.7 in SCT patients. Comparison with non intensified patients showed that weight was significantly higher in SCT patients &gt; 15 y of age (p=0.001), spleen size was significantly higher in (2–5y) young patients treated with HU (p=0.005) or TP (0.001) and in 5–10 y old patients on HU (p=0.046) but no difference was observed after the age of 10 y. O2 saturation was significantly improved after SCT (p&lt;0.001) (98.8 ± 1.0 vs 97.1 ± 2.6) and was unchanged on HU and TP. Cardiac pulsations were significantly (p&lt;0.001) decreased after all type of intensification. Biological data are shown (table1and 2). Conclusion : Treatment intensifications (TP, HU, SC) reduced the decrease of weight observed with aging in SCD patients and significantly reduced anemia using different mechanisms. SCT was the most effective to correct anemia, supress hemolysis and decrease leucocytosis. Introdução: Nosso trabalho visou observar a freqüência das etiologias na população de pacientes com síndrome parkinsoniana (SP). Casuística e método: Foram analisados retrospectivamente prontuários de pacientes com SP atendidos no Hospital das Clínicas da Universidade Estadual de Campinas entre Janeiro de 1999 e Dezembro de 2001, avaliando-se quadro clínico e exames neuroimagem. Resultados: Dos 177 prontuários analisados, 151 (85,3%) apresentavam diagnóstico de doença de Parkinson (DP), 6 (3,4%) parkinsonismo vascular (PV), 7 (4,0%) parkinsonismo induzido por drogas (PID), 6 (3,4%) Parkinson-plus (PP), 1 (0,6%) deficiência de pantetonato-quinase, 1 (0,6%) ataxia espinocerebelar forma 3, e 5 (2,8%) SP indeterminada (PI). A idade de início da doença foi de 54 ± 12 anos na DP, 75 ± 9 no PID, 60 ± 14 no PV e 53 ± 9 no PP. Conclusão: A DP foi a forma de SP mais comum em nosso ambulatório, seguido por PID, PV e PP. Os pacientes com PID tiveram idade de início significativamente maior em relação à DP.

Objective: We evaluated the "extended" long-term (5 years) functional outcomes in obstetric brachial plexus injury (OBPI) patients, who underwent triangle tilt surgery between February 2005 and January 2008. Methods: Twenty two children (9 girls and 13 boys, mean age at surgery was 5.8 years; ranging 2.1-11.8 years old), who initially presented with medial rotation contracture and scapula deformity secondary to obstetric brachial plexus injury were included in this study. Functional movements were evaluated pre-operatively, and 5 years following triangle tilt surgery by modified Mallet scale. Results: Here, we report long-term (5 years) follow-up of triangle tilt surgery for 22 OBPI patients. Upper extremity functional movements such as, external rotation (2.5±0.6 to 4.1±0.8, p<0.0001), hand-to-spine (2.6±0.6 to 3.4±1.1, p<0.005), hand-to-neck (2.7±0.7 to 4.3±0.7, p<0.0001), hand-to-mouth (2.3±0.9 (92º±33) to 4.2±0.5 (21º±16), p<0.0001), and supination (2.6±1.1 (-8.2º ±51) to 4.1±0.7 (61±32)) were significantly improved (p<0.0001), and maintained over the extended long-term (5 years). Total modified Mallet functional score was also shown to improve from 14.1±2.7 to 20.3±2.5. Conclusions: The triangle tilt surgery improved all shoulder functions significantly, and maintained over the extended long-term (5 years) in these patients.

5-Phenyl-1,3,4-oxadiazole-2-thiol (POT) and the systems Ni(II)-POT-acetate buffer, Co(II)-POT-acetate buffer were investigated by differential-pulse cathodic stripping voltammetry (DPCSV). The results of the voltammetric measurements show that POT gives a DPCSV signal at about -0.2 V vs Ag|AgCl. The presence of both POT and nickel(II) ions gives rise to a new peak at about -0.6 V; in the presence of cobalt(II) ions a new peak appears at -1.1 V. Based on published results obtained for other sulfur compounds in the presence of Ni(II) or Co(II) ions, the peak at -0.6 V is ascribed to catalytic reduction of Ni(II), and the peak at -1.1 V to hydrogen catalytic discharge. Optimum conditions have been found for the determination of POT by DPCSV. A linear dependence has been found only for the catalytic Ni(II) peak in the region 10-8-10-7 M POT. The calculated limit of detection and limit of quantification are 3.4 × 10-9 and 1.1 × 10-8 mol l-1, respectively.

Purpose: To evaluate pulsatility and movement along the ascending thoracic aorta using dynamic electrocardiographically-gated 64-slice cine computed tomographic angiography (CTA). Methods: Diameter and area change and center of mass (COM) movement of the ascending thoracic aorta was determined per cardiac cycle in 15 patients at surgically relevant anatomical levels: (A) 5 mm distal to the coronary arteries, (B) 5 mm proximal to the innominate artery, and (C) halfway up the ascending aorta. Additionally, COM movement was determined 1 cm (level P) and 2 cm (level Q) distal from the origins of the innominate, left carotid, and left subclavian arteries. Eight gated datasets covering the cardiac cycle were used to reconstruct images at each level perpendicular to the aortic lumen. The distance between important anatomical landmarks was determined. Results: All levels showed significant cardiac cycle—induced diameter and area changes (p<0.001), with the largest pulsatility 5 mm distal to the coronary arteries. Mean maximum diameter changes were (A) 17.4%±4.8% (range 7.5%–27.5%), (B) 13.9%±3.5% (range 10.6%–25.0%), and (C) 12.9%±3.4% (8.3%–19.6%). Mean area changes were (A) 12.7%±5.5% (range 4.3%–21.8%), (B) 7.5%±2.0% (range 4.1%–11.0%), and (C) 5.6%±2.2% (range 1.9%–11.4%). Mean maximum COM movements were (A) 6.1±2.0 mm (range 2.7–9.0), (B) 2.3±1.1 mm (range 1.1–5.6), and (C) 3.6±1.5 mm (range 1.4–6.5). Mean COM movements of the innominate, left carotid, and left subclavian arteries, respectively, were (P) 1.960.7 mm (range 0.9–3.7), 2.4±0.6 mm (range 1.4–3.3), and 1.9±0.6 mm (range 0.8–2.8), and (Q) 1.8±0.7 mm (range 0.8–3.5), 1.8±0.6 mm (range 0.8–2.7), 1.9±0.6 mm (range 1.1–3.4). Conclusion: The dynamics of the ascending thoracic aorta and the arch vessels are impressive, showing a wide range of 3-dimensional motions. Future ascending arch branched and fenestrated thoracic endograft designs must consider this active local environment, as it may have implications for durability, sealing, and ultimate clinical success.
