Анотація:
La infección del tracto urinario de origen bacteriano es una patología frecuente en la mujer embarazada. Constituye un importante factor de riesgo asociado con el desarrollo de sepsis neonatal. La sepsis neonatal es una infección invasiva generalmente acompañada de bacteriemia que se presenta en el neonato en el primer mes de vida. Es una de las causas principales de morbilidad y mortalidad neonatal. El objetivo de esta investigación fue mostrar la relación entre la infección en el tracto urinario en la madre embrazada y la morbilidad y mortalidad del neonato. Se realizó un estudio de casos y controles que incluyó una muestra intencional y no probabilística de 224 pacientes, dividida en dos grupos de interés: 70 casos de neonatos nacidos de mujeres diagnosticadas conla infección y 154 controles de neonatos nacidos de mujeres sanas. Se realizó un análisis bivariado aplicando la prueba Chi cuadrado y estimando el Odds Ratio con apoyo del software OpenEpi, v3. El estudio mostró que los neonatos nacidos de madres con infección del tracto urinario tienen mayor riesgo de desarrollar sepsis neonatal, y sugiere que esta patología, una vez diagnosticada, puede ser tratada con eficacia evitando consecuencias graves para la salud del recién nacido. A new blue electroluminescent conjugated polymer, poly(BP-alt-BCV), was prepared by Hornor–Emmons polycondensation of [5-carbazol-9-yl-3-(diethoxy-phosphorylmethyl)-2-(2-ethyl-hexyloxy)-benzyl]-phosphonic acid diethyl ester (PHBP) and 4,4′-biphenyldicarboxaldehyde (BPCA) in the presence of tert-BuOK in THF. UV-visible absorption maximum (λmax,UV, π-π* transition) of poly(BP-alt-BCV) was measured at 340 nm. PL maximum (λmax,PL) values of poly(BP-alt-BCV) were obtained at 406 and 430 nm, when excited at its own λmax,UV. It was found that double layer [ITO/poly(BP-alt-BCV)/Alq3 (50 Å)/Al] PLED exhibited maximum EL peak at 440 nm and blue emission (x = 0.1794, y = 0.1878), similar to the NTSC standard blue (x = 0.14, y = 0.08).

В производственных условиях исследовали возможность использования озимого и ярового ячменя, выращенных в Кабардино-Балкарской республике для пивоваренной промышленности. В качестве объектов исследований были выбраны сорта озимого ячменя — Мастер, Михайло, Козырь, Добрыня 3 и ярового — Приазовский 9, Гетьман, Виконт, Мамлюк, допущенные к использованию в Северокавказском регионе. Наибольшей зимостойкостью отличился сорт Мастер. Близким к нему показателем характеризовался сорт Михайло. Сорта Козырь и Добрыня 3 показали пониженную, на 12–14% по сравнению с 1-м сортом, зимостойкость. Наибольшие показатели по ним отмечены у сорта Приазовский 9–45,2 г при 21 зерне в колосе. Мамлюк отличается от перечисленных сортов меньшей озерненностью колоса (19 шт.) и минимальной среди них абсолютной массой зерна (41,6 г). Высокий коэффициент продуктивной кустистости отмечен у сорта Мастер, благодаря чему на 1 га насчитывалось 3,42 млн колосьев с наибольшим числом зерен в колосе — 32 шт. и их абсолютной массой 42,9 г. Остальные сорта уступили ему по числу растений на 1 м2 на 3,4–13,3%, количеству продуктивных стеблей на 4,4–17,8%, числу зерен в колосе на 3,3–31,2%, абсолютной массе зерна на 1,2–4,0%. Яровые сорта Виконт и Мамлюк обладали средней крупностью зерна, а у сортов Приазовский 9 и Гетьман она выше на 3–4%. Из предложенных озимых сортов наибольшей крупностью отличился сорт Мастер. Он же имел максимальные показатели по натуре и массе 1000 зерен, процентному содержанию крахмала и экстрактивности. Михайло и Козырь имеют близкие значения по всем перечисленным показателям и уступают Мастеру по крупности зерна на 13,0–17,2%, массе 1000 зерен на 0,9–1,9%, натурной массе — на 2,5–3,4%, содержанию крахмала — на 9,1–10,0%, экстрактивности — на 3,2–1,6%. The possibility of using winter and spring barley grown in the Kabardino-Balkarian Republic for the brewing industry was investigated under production conditions. The research objects were winter barley varieties — Master, Mikhailo, Kozyr, Dobrynya 3 and spring barley — Priazovsky 9, Getman, Viscount, Mamluk, approved for use in the North Caucasus region. The Master variety has the greatest winter hardiness. Mikhailo showed a figure close to his. Kozyr and Dobrynya 3 were characterized by reduced winter hardiness, by 12–14% compared to the first variety. The highest indicators for them were noted in the Priazovsky 9 variety — 45.2 g with 21 grains per year. Mamluk differs from the listed varieties in the smaller grain size of the ear — 19 pieces. and the minimum among them with an absolute grain weight of 41.6 g. A high coefficient of productive tillering was noted in the Master variety, due to which there were 3,42 million ears per 1 hectare with the largest number of grains per ear — 32 pieces. — and their absolute weight — 42.9 g. The remaining varieties were inferior to it in the number of plants per 1 m2 by 3.4–13.3%, the number of productive stems by 4.4–17.8%, the number of grains per year by 3.3–31.2%, absolute grain weight by 1.2–4.0%. The spring varieties Viscount and Mamluk had an average grain size, while in the Priazovsky 9 and Getman varieties it was 3–4% higher. Of the proposed winter varieties, the Master variety distinguished itself by the largest size. It also had the maximum indicators in terms of nature and weight of 1000 grains, percentage of starch and extract content. Mikhailo and Kozyr have similar values for all of the above indicators and are inferior to the Master in grain size by 13.0–17.2%, weight of 1000 grains by 0.9–1.9%, natural weight — by 2.5–3.4%, starch content — by 9.1–10.0%, extract content — by 3.2–1.6%.

440 Background: Approximately 12–23% G/GEJC patients have human epidermal growth factor receptor 2 (HER2)-positive disease. Despite survival benefit from combination therapy with trastuzumab and chemotherapy, the prognosis remains unsatisfactory; more effective treatments are needed. This phase 2 study is evaluating the combination of HLX22 (an anti-HER2 monoclonal antibody targeting a different epitope than trastuzumab), trastuzumab, and XELOX chemotherapy as first-line treatment for patients with advanced/metastatic G/GEJC. Following the most recent report of 17 to 18 patients in each group at ASCO 2024 Annual Meeting, herein we present the updated efficacy and safety results with 31 patients in each group. Methods: Patients with locally advanced or metastatic HER2-positive G/GEJC and no prior systemic antitumor therapy were enrolled. Herein reported are results from Stage 2 of the study. Eligible patients were randomized to receive either HLX22 + trastuzumab + XELOX or placebo + trastuzumab + XELOX in 3-week cycles. Primary endpoints were independent radiology review committee (IRRC)-assessed progression-free survival (PFS) and objective response rate (ORR) per Response Evaluation Criteria in Solid Tumors (RECIST) v1.1. Secondary endpoints included other efficacy and safety endpoints. Results: As of June 30, 2024, 62 patients were randomized to the respective groups (31 vs 31), of whom 51 (82.3%) patients were male. Median follow-up duration was 20.3 and 24.0 months for the respective groups. The efficacy results are shown in the table. Treatment-emergent adverse events (TEAEs) were reported in 30 (96.8%) and 31 (100%) patients, and HLX22- or placebo-related TEAEs of grade 3 or higher were reported in 9 (29.0%) and 6 (19.4%) patients in the respective groups. One patient (3.2%) in the placebo + trastuzumab + XELOX group had a grade 5 HLX22-/placebo-related TEAE. Conclusions: With a manageable safety profile, the addition of HLX22 to first-line treatment with trastuzumab plus XELOX conferred survival benefit for HER2-positive G/GEJC patients. Nitriding is as an effective technique applied for many years to improve the surface hardness and wear resistance of low carbon and tool steels [1]. In the case of stainless steels, increase of surface hardness and wear resistance accompany by a drop in corrosion resistance due to the precipitation of CrN. In this respect, many attempts have been made to modify the surfaces of austenitic stainless steels to increase their surface hardness and wear resistance without scarifying the corrosion resistance [2-6]. It is finally concluded that, nitriding at temperatures lower than conventional nitriding process (which is generally about 550°C) has potentiality to produce a nitrogen expanded austenite (also known as S-phase), on the surface without formation of CrN. Due to the superb properties of the S-phase, the low temperature nitrided austenitic stainless steels exhibit very high surface hardness, a good wear resistance, and more importantly, an excellent corrosion resistance. Recently some attempts have been made to apply low temperature nitriding to martensitic stainless steels, which are widely used in the industries of medicine, food, mold and other civil areas [7-9]. In these works, where nitriding has been conducted by plasma processes, superior surface hardness, along with excellent wear and corrosion resistances have been reported for AISI 410 and AISI 420 grade martensitic stainless steels. This work focuses on low temperature gas nitriding of AISI 420 grade martensitic stainless steel in a fluidized bed reactor. In this respect the microstructures, phase compositions, hardness, wear and corrosion behaviours of the original and nitrided martensitic stainless steels have been compared.

In this study, 13% of fresh lettuce (Lactuca sativa) samples collected from markets and supermarkets in two cities of Mexico were contaminated with Salmonella spp. From those samples, amplicons of ~300 base pairs (bp) were amplified, corresponding to the expected size of the invasion (invA) and internal transcribed spacer regions of the 16S and 23S rRNA genes of Salmonella spp. Additionally, Salmonella strains were isolated and harbored plasmids ranging from ~9 to 16 kbp. From these strains, 91% were resistant to ampicillin and nitrofurantoin, whereas 55% were resistant to cephalothin and chloramphenicol. No resistance was detected to amikacin, carbenicillin, cefotaxime, gentamicin, netilmicin, norfloxacin, and sulfamethoxazole-trimethoprim. When Salmonella isolates were tested against novel bacteriocins (morricin 269, kurstacin 287, kenyacin 404, entomocin 420, and tolworthcin 524) produced by five Mexican strains of Bacillus thuringiensis, 50% were susceptible to these antimicrobial peptides. This is the first report showing that Salmonella strains isolated from lettuce are susceptible to bacteriocins produced by the most important bioinsecticide worldwide, suggesting the potential use of these antibacterial peptides as therapeutic agents or food preservatives to reduce or destroy populations of Salmonella spp.

Purpose: Prader–Willi syndrome (PWS) is a genetic neurobehavioral disorder presenting hypothalamic dysfunction and adiposity. At rest, PWS exhibits hypoventilation with hypercapnia. We characterized ventilatory responses in children with PWS during exercise. Methods: Participants were children aged 7–12 years with PWS (n = 8) and without PWS with normal weight (NW; n = 9, body mass index ≤ 85th percentile) or obesity (n = 9, body mass index ≥ 95th percentile). Participants completed three 5-minute ambulatory bouts at 3.2, 4.0, and 4.8 km/h. Oxygen uptake, carbon dioxide output, ventilation, breathing frequency, and tidal volume were recorded. Results: PWS had slightly higher oxygen uptake (L/min) at 3.2 km/h [0.65 (0.46–1.01) vs 0.49 (0.34–0.83)] and at 4.8 km/h [0.89 (0.62–1.20) vs 0.63 (0.45–0.97)] than NW. PWS had higher ventilation (L/min) at 3.2 km/h [16.2 (13.0–26.5) vs 11.5 (8.4–17.5)], at 4.0 km/h [16.4 (13.9–27.9) vs 12.7 (10.3–19.5)], and at 4.8 km/h [19.7 (17.4–31.8) vs 15.2 (9.5–21.6)] than NW. PWS had greater breathing frequency (breaths/min) at 3.2 km/h [38 (29–53) vs 29 (22–35)], at 4.0 km/h [39 (29–58) vs 29 (23–39)], and at 4.8 km/h [39 (33–58) vs 32 (23–42)], but similar tidal volume and ventilation/carbon dioxide output to NW. Conclusion: PWS did not show impaired ventilatory responses to exercise. Hyperventilation in PWS may relate to excessive neural stimulation and metabolic cost.

Yaş gruplarına göre bu sayıların dağılımı 0-4 arası 555.249, 5-9 arası 518.916, 10-14 arası 405.768, 15-18 arası 252,126 şeklindedir. Anaokulu, İlkokul, Ortaokul ve Lisede Okuyan Suriyeli Öğrenci Sayısı Millî Eğitim Bakanlığı tarafından 8 Haziran 2021 tarihinde yapılan açıklamaya göre 938 bin138 olarak (anaokulunda 35 bin 707, ilkokulda 442 bin 817, ortaokulda 348 bin 638 ve lisede 110 bin 976) belirtilmektedir. Eğitim çağında olup okula gitmeyen çocuk sayısı 432 bin 956’dur. Geçici koruma altındaki bu çocukların sosyal uyumlarının sağlanmasında en etkili araç eğitimdir ve bu bağlamda okullara, okul yöneticilerine, öğretmenlere ve diğer paydaşlara çok büyük görevler düşmektedir. Bu çalışmanın amacı okullarında ve sınıflarında geçici koruma altındaki çocukların olduğu öğretmenlere ve okul yöneticilerinin rehberlik edecek bir kaynak oluşturmaktır. Abstract Arnold (1822—88) is now mainly famous as a literary and social critic, but he was also concerned with the implications of the professionalization and regulation of writing (typically, Arnold retained copyright of this article, reprinting it in Irish Essays and Others (London: Smith, Elder and Co., 1882, 444—80)). His article is a useful collage of arguments and tropes derived from a variety of sources that date back to the five-year debate preceding the 1842 Copyright Act. In 1876, a Royal Commission was set up to examine British and international copyright law. The commission’s report, published two years later, attempted to clarify the law and close loopholes. International copyright was eventually ratified by the 1885 Berne Convention (to which America was not a signatory).

The aim of the research was to evaluate the influence of the type of the nervous system on the productive and reproductive qualities of animals. The type of nervous system (strong, weak) of cows of the Ukrainian red dairy breed was determined by the index of the nervous system, which is based on the variability of the fat content in the morning milk yield after changing the conditions of cows keeping. Under the new conditions of keeping cows with a strong type of nervous activity, the coefficients of variability of the morning milk yield and the fat content in milk practically remained at the same level (their slight increase was noted at the level of 4.5 and 3.1%). In cows of a weak type under similar conditions of keeping, a significant increase in the variability of milk yield (by 53.6%) and fat content in milk (by 266.7%) was found. In animals of both types, in the first days of summer-camp keeping, in comparison with winter-stall keeping, there was an increase in milk yield and a decrease in fat content in milk. In cows of the strong type, the fat content decreased by 0.08% and of the weak type by 0.12%. The average index of the nervous system type in animals of the strong type is 1.18, and in cows of the weak type 2.71 (P> 0.999). Before and during milking, the pulse rate in the cows of weak type was higher by 8.0 and 10.8 beats / min. (P> 0.999), compared with the strong type. The animals of strong type of the nervous system, in comparison with the weak type, had a smaller thickness of the concha and skin, a higher pulse and respiratory rate, and a lower rectal body temperature. In cows of a strong type of the nervous system in postnatal ontogenesis, the formation of productive traits took place more rapidly and they better adapted to the conditions of the external environment. This is evidenced by their high coefficient of relative decline in the growth at the age of one (85.0) than in animals with a weak type of nervous system (78.5). The advantage of cows with a strong type of nervous activity over a weak one in milk yield was 348 kg (P>0.95) and in milk fat 8.0 kg. The service period in animals with a strong type of nervous activity was 6.3 days shorter. The animals with a strong type of nervous activity during the first lactation had a higher adaptation index compared with animals of a weak type. The difference for this indicator is 0.49 (or 3.7%). They also had a higher productivity index for the first (3.1%) and second (4.0%) lactation. Thus, the typological features of the nervous activity of cows affect the formation of the most important functions of the body, which can be used in breeding work to improve the herd.

The first article reveals the expressiveness of the “Turkic” utterances of Afanasy Nikitin in The Journey Beyond Three Seas according to the list of the Russian State Archive of Ancient Acts (RSAAA), f. 181, no. 371 of the first quarter of the 16th century. The second article characterizes the distorted, fantastic earthly worlds depicted in the Tale of the Twelve Dreams of King Shahaisha according to the list of the Russian National Library (RNL), Kir.-Beloz., no. 22/1099 of the 1470s; in the Conversation of Three Saints according to the list of the Russian State Library (RSL), Troitsk., no. 778 of the beginning of the 16th century; in the collection of proverbs and sayings according to the list of the RSAAA, Ministry of Foreign Affairs, Moscow Main Archive (MMA), no. 250–455 of the late 17th century; in The Tale of Ersh Ershovich according to the list of Pushkin House, 1.27.105 of the late 17th — early 18th centuries; in the Bird Council according to the list of the RNL, 0.XVII.17 the mid-18th century; in the Medicine Book. The fourth article shows that some compilers of collections of the 17th century appreciated the visual arts of works, mostly very old (оn the example of collections of the RSL, Tikhonravov, no. 460, 384, 18, 340, 231). In two Appendices to the article are published the descriptions of the composition of the collection no. 231 and the text of the parable about the dispute of parts of the human body. In two Ap- pendices to the article, it is said about the everyday depiction of the collection of proverbs and sayings according to the list of the RSAAA, MMA, no. 250–455 of the late 17th century and on the expressiveness of articles in the miniature collection of the RSL, Bolshakov, no. 325. The fifth article points to the mocking meaning of proverbs and sayings about criminals in the same collection of the RSAAA, MMA, no. 250–455. Finally, the sixth article draws attention to the evolution of the literary work of Archpriest Avvakum from brief mentions of events to detailed stories about them (оn the material of Vita, petitions, Book of Interpretations, Book of Accusations, Write-off about the creation of man, The Lamentable Word about the death of noblewoman F. Morozova). We must warn you that the pictorial and expressive meaning of the examples and phrases quot- ed from the texts of the monuments is not thoroughly proved in this work, but is only stated. Otherwise, each example would require an independent essay on certain literary means, and the theme and composition of the work would be completely different.

Abstract Skarn deposits are one of the most common deposit types in China. The 386 skarns summarized in this review contain ~8.9 million tonnes (Mt) Sn (87% of China’s Sn resources), 6.6 Mt W (71%), 42 Mt Cu (32%), 81 Mt Zn-Pb (25%), 5.4 Mt Mo (17%), 1,871 tonnes (t) Au (11%), 42,212 t Ag (10%), and ~8,500 Mt Fe ore (~9%; major source of high-grade Fe ore). Some of the largest Sn, W, Mo, and Zn-Pb skarns are world-class. The abundance of skarns in China is related to a unique tectonic evolution that resulted in extensive hydrous magmas and widespread belts of carbonate country rocks. The landmass of China is composed of multiple blocks, some with Archean basements, and oceanic terranes that have amalgamated and rifted apart several times. Subduction and collisional events generated abundant hydrous fertile magmas. The events include subduction along the Rodinian margins, closures of the Proto-Tethys, Paleo-Asian, Paleo-Tethys, and Neo-Tethys Oceans, and subduction of the Paleo-Pacific plate. Extensive carbonate platforms developed on the passive margins of the cratonic blocks during multiple periods from Neoarchean to Holocene also facilitated skarn formation. There are 231 Ca skarns replacing limestone, 15 Ca skarns replacing igneous rocks, siliciclastic sedimentary rocks, or metamorphic silicate rocks, 113 Ca-Mg skarns replacing dolomitic limestone or interlayered dolomite and limestone, and 28 Mg skarns replacing dolomite in China. The Ca and Ca-Mg skarns host all types of metals, as do Mg skarns, except for major Cu and W mineralization. Boron mineralization only occurs in Mg skarns. The skarns typically include a high-temperature prograde stage, iron oxide-rich higher-temperature retrograde stage, sulfide-rich lower-temperature retrograde stage, and a latest barren carbonate stage. The zoning of garnet/pyroxene ratios depends on the redox state of both the causative magma and the wall rocks. In an oxidized magma-reduced wall-rock skarn system, such as is typical of Cu skarns in China, the garnet/pyroxene ratio decreases, and garnet color becomes lighter away from the intrusion. In a reduced intrusion-reduced wall-rock skarn system, such as a cassiterite- and sulfide-rich Sn skarn, the skarn is dominated by pyroxene with minor to no garnet. Manganese-rich skarn minerals may be abundant in distal skarns. Metal associations and endowment are largely controlled by the magma redox state and degree of fractionation and, in general, can be grouped into four categories. Within each category there is spatial zonation. The first category of deposits is associated with reduced and highly fractionated magma. They comprise (1) greisen with Sn ± W in intrusions, grading outward to (2) Sn ± Cu ± Fe at the contact zone, and farther out to (3) Sn (distal) and Zn-Pb (more distal) in veins, mantos, and chimneys. The second category is associated with oxidized and poorly to moderately fractionated magma. Ores include minor porphyry-style Mo and/or porphyry-style Cu mineralization ± Cu skarns replacing xenoliths or roof pendants inside intrusions, zoned outward to major zones of Cu and/or Fe ± Au ± Mo mineralization at the contact with and in adjacent country rocks, and farther out to local Cu (distal) + Zn-Pb (more distal) in veins, mantos, and chimneys. Oxidized and highly fractionated magma is associated with porphyry Mo or greisen W inside an intrusion, outward to Mo and/or W ± Fe ± Cu skarns at the contact zone, and farther to Mo or W ± Cu in distal veins, mantos, and chimneys. The final category is associated with reduced and poorly to moderately fractionated magma. No major skarns of this type have been recognized in China, but outside China there are many examples of such intrusions related to Au-only skarns at the contact zone. Reduced Zn-Au skarns in China are inferred to be distal parts of such systems. Tungsten and Sn do not occur together as commonly as was previously thought. The distal part of a skarn ore system may transition to carbonate replacement deposits. Distal stratabound mantos and crosscutting veins/chimneys may contain not only Zn-Pb but also major Sn, W, Cu, Mo, and Au mineralization. The Zn-Pb mineralization may be part of either an oxidized system (e.g., Cu, Mo, Fe) or a reduced system (e.g., Sn). In China, distal Zn-Pb is more commonly related to reduced magmas. Gold and W may also be related to both oxidized and reduced magmas, although in China they are more typically related to oxidized magma. There are numerous examples of distal mantos/chimneys that continuously transition to proximal skarns at intrusion-wall-rock contact zones, and this relationship strongly supports the magmatic affiliation of such deposits and suggests that distal skarns/carbonate replacement deposits systems should be explored to find more proximal mineralization. Carbonate xenoliths or roof pendants may host the majority of mineralization in some deposits. In contact zones, skarns are better developed where the intrusion shape is complicated. The above two skarn positions imply that there may be multiple skarn bodies below drill interceptions of intrusive rocks. Many of the largest skarns for all commodities in China are related to small or subsurface intrusions (except for Sn skarns), have multiple mineralization centers, are young (<~160 Ma), and have the full system from causative intrusion(s) to distal skarns or carbonate replacement extensions discovered. Chinese skarn deposits fall in several age groups: ~830, ~480 to 420, ~383 to 371, ~324 to 314, ~263 to 210, ~200 to 83, ~80 to 72, and ~65 to 15 Ma. They are typically associated with convergent plate boundaries, mostly in subduction settings but also in collisional settings. Seven major skarn metallogenic belts are recognized based on skarn geographic location and geodynamic background. In subduction settings, skarns may form in a belt up to 4,000 km long and 1,000 km inland, with skarns continuously forming for up to 120 m.y., e.g., the eastern China belt. In most other belts, skarns form in 5- to 20-m.y. episodes similar to the situation in South America. In collisional settings, skarns may form up to 50 m.y. after an ocean closure, and the distance to the collisional/accretionary boundary may extend to ~150 km inland. The size of collision-related skarns may be as large as the largest skarns related to oceanic crust subduction. Older suture zones may be favorable sites for younger mineralization, for example, the Triassic Paleo-Tethys suture between the North and South China blocks for the younger and largest skarn cluster of the Middle-Lower Yangtze belt in the eastern China belt, and the Triassic sutures in southwestern China for Cretaceous to Tertiary mineralization.

ЦЕЛЬ: оценить влияние рукавной гастропластики на менструальный цикл и уровень половых гормонов у пациенток репродуктивного возраста с ожирением МАТЕРИАЛЫ И МЕТОДЫ: проведено наблюдательное проспективное исследование 32 пациенток с индексом массы тела (ИМТ) ≥ 35 кг/м2 в возрасте от 19 до 38 лет с различными нарушениями менстру- ального цикла (НМЦ). Пациенткам выполняли рукавную гастропластику (РГ) по стандартной методике. Исходно и через 1 год после операции оценивали ИМТ, на 3-5 день менструального цикла при его наличии определяли уровень гонадотропных гормонов: фолликулостимулирующего (ФСГ) и лютеинизирующего (ЛГ), уровень общего эстрадиола, свободного тестостерона и пролактина, а также антимюллерова гормона (АМГ) как маркера овариального резерва. Уровень прогестерона определяли на 21-23 день менструаль- ного цикла (при его наличии). Индекс инсулинорезистентности НОМА-IR рассчитывали по стандартной формуле, за инсулинорезистентность принимали значение более 2,7. РЕЗУЛЬТАТЫ: медиана возраста пациенток составила 29 (22 – 35) лет, ИМТ исходно - 38,8 кг/м2 (36,2 – 40,2). Структура МНЦ была представлена следующим образом: олигоменорея выявлялась у 28,1% па- циенток (n = 9), аномальные маточные кровотечения у 53,1% пациенток, (n = 17), аменорея на протяжении как минимум 6 месяцев до оперативного вмешательства – у 18,8% пациенток (n = 6). Через 1 год после РГ у всех пациенток ИМТ снизился до 27,3 кг/м2 (24,6 – 29,1), р < 0,001. НОМА-IR был ниже исходных значений: 2,8 (1,6 – 3,6). У 22 женщин произошла нормализация менструального цикла; через 1 год после операции у 4 женщин сохранялись аномальные маточные кровотечения, у 3 – олигомено- рея, у 3 – аменорея. Описанные пациентки были старше, однако различия в возрасте не были статистически значимы. Уровень ФСГ через 1 год после РГ повысился и составил 11,6 мМЕ/мл (8,2 – 15,6), р = 0,042; уровень ЛГ снизился по сравнению с исходными значениями до 6,4 мМЕ/мл (3,2 – 9,1), р = 0,048. Уровень общего эстрадиола снизился до 320 пг/мл (265 – 373), однако различия статистически не значимы. Свободный тестостерон был ниже исходных значений – 3,2 пг/мл (2,5 – 4,8), р = 0,034. ВЫВОДЫ: выполнение РГ у пациенток с ожирением приводит к снижению инсулинорезистентности, нормализации менструальной функции у большинства пациенток, что сопровождается повышением уровня АМГ, изменением соотношения гонадотропных гормонов и уменьшением уровня свободного тестостерона, а также повышением уровня прогестерона.

A method is presented for simulating underwater blast waves for unconfined laboratory experiments. High speed water entry of projectiles can create ballistic waves that resemble underwater blast waves. This paper provides several practical examples of launching projectiles downward into a 1000L container of water, a method accessible in many laboratories. Resulting ballistic waves are measured with a piezoelectric pressure transducer. Results are reported for four projectiles: 1) A 4.5 mm diameter steel sphere fired from a pump “bb gun” impacting at 199 m/s. 2) A 7.82 mm diameter hardened lead sphere fired from a 30/30 rifle impacting at 345 m/s. 3) A 5.59 mm diameter lead hollow point bullet fired from a .22 LR rifle impacting at 389 m/s. 4) A 9 mm diameter jacketed hollow point pistol bullet fired from a service caliber pistol impacting at 397 m/s. In all cases presented here, the ballistic waves generated in high speed water entry might find experimental use as simulated underwater blast waves, depending on experimental requirements. Peak blast magnitude ranged from 400 kPa to 2500 kPa, and positive pulse duration ranged from 0.2 ms to 0.6 ms. Wave shape varied in quality.

Introdução: A Covid-19 é uma doença infecciosa declarada como emergência da saúde pública em 2020. Gestantes representam um grupo com vulnerabilidade única em razão de suas alterações fisiológicas, susceptibilidade a infecções e comprometimento de funções imunológicas. Relato de Caso: P.S.S., 25 anos, branca, residente do Rio de Janeiro, gestante de 34 semanas e 3 dias, hipertensa em uso de metildopa, índice de massa corpórea (IMC) 42 kg/m2. Admitida na Unidade Materno-Fetal do Hospital Federal dos Servidores do Estado (UMF-HFSE) com queixa de febre há 7 dias e tosse produtiva. Na avaliação inicial, estava hemodinamicamente estável, afebril, taquipneica (28 irpm), com saturação de oxigênio (SatO2) de 98% e estertores em base pulmonar direita. Exame obstétrico e avaliação fetal sem alterações. Evoluiu com letargia, SatO2 95% em cateter de O2 3 L/min com sinais de esforço respiratório, pressão arterial de 150×90 mmHg, gasometria arterial com hipoxemia e acidose respiratória compensada. Laboratório com linfopenia, 120.000 plaquetas/mm3, lactato desidrogenase (LDH) de 259 U/L, bilirrubina direta 1,87 mg/dL e indireta de 1,06 mg/dL. Fez uso de azitromicina, ceftriaxone, oseltamivir e clexane com aumento progressivo no suporte de oxigênio e necessidade de reposição de bicarbonato. Tomografia de tórax com padrão de vidro fosco com acometimento de 50‒75%. Em função da piora do quadro respiratório e persistência da acidose metabólica, foi transferida para Unidade de Terapia Intensiva (UTI) para planejamento de parto ainda em ventilação não invasiva. Parto cesáreo realizado, recém-nascido (RN) com Apgar 8/9, encaminhado à UTI neonatal. Piora ventilatória e laboratorial no quarto dia pós-parto (LDH 877 U/L e transaminase glutâmico-oxalacética — TGO 372 mg/dL) com necessidade de suporte com ventilação mecânica (VM), posição prona, corticoterapia, infusão de aminas e diálise. Permaneceu em UTI por 14 dias, com posterior alta para enfermaria. Discussão: A paciente apresentou quadro de infecção por Covid-19 confirmada por polymerase chain reaction (PCR) positivo durante internação, com evolução grave e necessidade de UTI e VM, conforme a maioria dos pacientes de estudos publicados. Além da gestação, fator de risco já estabelecido, a paciente era hipertensa crônica e obesa, sendo ambas as condições associadas a casos severos. A indicação do parto foi iminentemente materna, sem evidências de sofrimento fetal, em função da piora progressiva do quadro materno, sendo a via alta escolhida e ocorrendo antes do termo, conforme relatos publicados. A evolução laboratorial apresentada tem dados que podem gerar dúvidas quanto a uma pré-eclâmpsia (PE) grave associada (relação proteína/creatinina urinária 0,79), e esse diagnóstico diferencial deve ser avaliado, pois 75% dos casos mais graves desenvolveram sinais e sintomas de PE. Porém, em razão da piora clínica e da instabilidade hemodinâmica com necessidade de múltiplas intervenções, essa avaliação foi prejudicada. RN com PCR para Covid-19 negativo. Altas hospitalares do RN e da puérpera com 8 e 35 dias, respectivamente, ambos em boas condições clínicas e com agendamento de seguimento.

Steam turbine valves are the most essential components of modern steam turbines from an operation, performance, reliability and safety aspects of a modern power plant. Current designs are pushing the operational envelope and it is not uncommon for large ultracritical plants to run on pressures exceeding 4500 psi and 1200 °F. These conditions are not only challenging for materials of construction for turbines and boilers but also for main steam turbine valves. The tendency of materials to oxidize at these temperatures is all too common causing problems for valve heads, stems, discs, bushings and seats. OEMs around the world are pushing to develop valve components with 9–12% Cr martensitic steels and nickel based alloys which offer better creep strength at elevated temperatures. For existing power plants at temperatures of a 1000 to 1050 °F range there is a push to retrofit valve components with Incolloy 901 type, Inconel 718 and Stellite alloys. Scale build up in traditional alloys happens too quickly for the usual two year maintenance cycle. The application of better alloys for steam turbine valves makes it possible to increase the maintenance cycle from two to four or even six years, while increasing the operational reliability of the valve. Elimination of main steam valve failures removes risks of turbine overspeed events and increases plant availability. Solid particle erosion is not forgiving on valve parts such as stems, discs and valve seats and over a period of time, excessive wear causes the valve to be rendered unsafe to continued service. Nitrided materials and chrome-carbide-coated materials are much harder than the stem base material; and to slow down wear, a nitriding process is used to develop a thin, hard, wear-resistant surface. Some of the material often used for Stellite liners are Nitralloy 135M, 410 SS, 422 SS Nitrided, Incolloy 901 Nitrided, 347 SS, 13Cr-13Ni-10Co-3Nb-2.5W-2Mo. Different OEMs use a variety of alloys for valve seats, discs and stems. Antigalling characteristics are particularly favorable. Valve casings are cast materials and usually specifications include the ASTM A217 and ASTM A356. The ASTM A217 cast steels are typically, 1.25Cr-0.5Mo Grade WC6 and the 2.25Cr-1Mo Grade WC9 materials. Some of the problems experienced with steam turbine valves, are sticking to the valve seat requiring excessive pull-out force, wear of the seat surface, valves not closing properly due to oxidation build up, Stellite weld cracking, cutting or gouging due to solid particle erosion. The material presented in this paper is of interest to fossil power plant personnel experiencing challenges on valve performance and maintenance. The paper looks at all aspects of steam turbine valves as far as current trends in valve material, operation and maintenance and lastly, looks at recent occurrences of valve failures leading to steam turbine overspeed catastrophic failures around the world.

We assess the potential effect of large language models (LLMs) on the labor markets of Chile, Mexico, and Peru using the methodology of Eloundou et al. (2023). This approach involves detailed guidelines (rubrics) for each job to assess whether access to LMM software would reduce the time required for workers to complete their daily tasks. Adapting this methodology to the Latin American context necessitated developing a comprehensive crosswalk between the Occupational Information Network (O*NET) and regional occupational classifications, SINCO-2011 and ISCO-2008. When we use this adaptation, the theoretical average task exposure of occupations under these classifications is 32% and 31% for each classification. Recognizing the unique characteristics of each country's labor market, we refined these ratings to reflect better each nation's capacity to adopt and effectively implement new technologies. After these adjustments, the task exposure for SINCO-2011 drops to 27% and for ISCO-2008 to 23%. These adjusted exposure ratings provide a more accurate depiction of the real-world implications of LLM integration in the Latin American context. According to this methodology, the LLM-powered exposure using GPT-4 estimates suggests that the percentage of jobs with task exposure exceeding 10% is 74% in Mexico, 76% in Chile, and 76% in Peru. When we raise the exposure threshold to 40% or more, the proportion of affected occupations significantly decreases to 9% in Mexico, 20% in Chile, and 6% in Peru. The exposure is close to zero after this threshold. In other words, the exposure would only affect less than half of the total labor force in these countries. Further analysis of exposure by socioeconomic conditions indicates higher exposure among women, individuals with higher education, formal employees, and higher-income groups. This suggests a potential increase in labor inequality in the region due to adopting this technology. Our findings highlight the need for targeted policy interventions and adaptive strategies to ensure that the transition to an AI-enhanced labor market benefits all socio-economic groups and minimizes disruptions.

Objectives. To update the evidence on benefits and harms of cannabinoids and similar plant-based compounds to treat chronic pain using a living systematic review approach. Data sources. Ovid® MEDLINE®, PsycINFO®, Embase®, the Cochrane Library, and SCOPUS® databases; reference lists of included studies; and submissions received after Federal Register request were searched to October 24, 2022. Review methods. Using dual review, we screened search results for randomized controlled trials (RCTs) and observational studies of patients with chronic pain evaluating cannabis, kratom, and similar compounds with any comparison group and at least 1 month of treatment or followup. Dual review was used to abstract study data, assess study-level risk of bias, and rate the strength of evidence (SOE). Prioritized outcomes included pain, overall function, and adverse events. We grouped studies that assessed tetrahydrocannabinol (THC) and/or cannabidiol (CBD) based on their THC to CBD ratio and categorized them as comparable THC to CBD ratio, high-THC to CBD ratio, and low-THC to CBD ratio. We also grouped studies by whether the product was a whole-plant product (cannabis), cannabinoids extracted or purified from a whole plant, or a synthetic product. We conducted meta-analyses using the profile likelihood random effects model and assessed between-study heterogeneity using Cochran’s Q statistic chi square test and the I2 statistic. Magnitude of benefit was categorized as no effect or small, moderate, and large effects. Results. From a total of 3,568 abstracts, 21 RCTs (N=1,905) and 9 observational studies (N=15,079) assessing different cannabinoids were included; none evaluated kratom. Studies were primarily short term, and 60 percent enrolled patients with neuropathic pain. Comparators were primarily placebo or usual care. The SOE was low unless otherwise noted. Compared with placebo, comparable THC to CBD ratio oral spray was associated with a small benefit in pain severity (7 RCTs, N=632, 0 to 10 scale, mean difference [MD] −0.54, 95% confidence interval [CI] −0.95 to −0.19, I2=39%; SOE: moderate) and overall function (6 RCTs, N=616, 0 to 10 scale, MD −0.42, 95% CI −0.73 to −0.16, I2=32%). There was no effect on study withdrawals due to adverse events. There was a large increased risk of dizziness and sedation, and a moderate increased risk of nausea (dizziness: 6 RCTs, N=866, 31.0% vs. 8.0%, relative risk [RR] 3.57, 95% CI 2.42 to 5.60, I2=0%; sedation: 6 RCTs, N=866, 8.0% vs. 1.2%, RR 5.04, 95% CI 2.10 to 11.89, I2=0%; and nausea: 6 RCTs, N=866, 13% vs. 7.5%, RR 1.79, 95% CI 1.19 to 2.77, I2=0%). Synthetic products with high-THC to CBD ratios were associated with a moderate improvement in pain severity, a moderate increase in sedation, and a large increase in nausea (pain: 6 RCTs, N=390, 0 to 10 scale, MD −1.15, 95% CI −1.99 to −0.54, I2=48%; sedation: 3 RCTs, N=335, 19% vs. 10%, RR 1.73, 95% CI 1.03 to 4.63, I2=28%; nausea: 2 RCTs, N=302, 12.3% vs. 6.1%, RR 2.19, 95% CI 0.77 to 5.39; I²=0%). We also found moderate SOE for a large increased risk of dizziness (2 RCTs, 32% vs. 11%, RR 2.74, 95% CI 1.47 to 6.86, I2=40%). Extracted whole-plant products with high-THC to CBD ratios (oral) were associated with a large increased risk of study withdrawal due to adverse events (1 RCT, 13.9% vs. 5.7%, RR 3.12, 95% CI 1.54 to 6.33) and dizziness (1 RCT, 62.2% vs. 7.5%, RR 8.34, 95% CI 4.53 to 15.34); outcomes assessing benefit were not reported or insufficient. We observed a moderate improvement in pain severity when combining all studies of high-THC to CBD ratio (8 RCTs, N=684, MD −1.25, 95% CI −2.09 to −0.71, I2=58%; SOE: moderate). Evidence (including observational studies) on whole-plant cannabis, topical or oral CBD, low-THC to CBD, other cannabinoids, comparisons with active products or between cannabis-related products, and impact on use of opioids was insufficient to draw conclusions. Other important harms (psychosis, cannabis use disorder, and cognitive effects) were not reported. Conclusions. Low to moderate strength evidence suggests small to moderate improvements in pain (mostly neuropathic), and moderate to large increases in common adverse events (dizziness, sedation, nausea) with high and comparable THC to CBD ratio extracted cannabinoids and synthetic products during short-term treatment (1 to 6 months); high-THC to CBD ratio products were also associated with increased risk of withdrawal due to adverse events. Evidence for whole-plant cannabis and other comparisons, outcomes, and plant-based compounds was unavailable or insufficient to draw conclusions. Small sample sizes, lack of evidence for moderate and long-term use and other key outcomes, such as other adverse events and impact on use of opioids during treatment, indicate that more research is needed.

This report presents the data collected in 2020 as part of the long-term terrestrial vegetation monitoring program at Channel Islands National Park. The purpose of this monitoring program is to document long-term trends in the park’s vegetation communities. Data are collected from 30-m-long transects using a point-line intercept method. In the past, each transect was sampled annually. However, beginning in 2012 the program began adding randomly located transects to improve the representativeness of the sampling, and transitioned to a rotating panel design. Now only a core subset of the transects are surveyed annually. Non-core transects are assigned to one of four panels, each of which is surveyed once every four years. Coronavirus disease (COVID-19) caused a global pandemic that prevented field data collection from most of the transects scheduled for 2020. As a result, just 29 of the 127 transects scheduled were visited, distributed as follows: Santa Barbara Island (n = 18), Santa Cruz Island (n = 11). The small size of this 2020 dataset offers limited comparisons to previous years. The program also planned to install 19 new transects on Santa Rosa and San Miguel islands to complete the probabilistic component of the study design (Tiszler et al. 2016a), but the safety response to COVID-19 did not permit these installations. Key 2020 results and interpretation: For the transects visited at both islands in 2020, a long-term decline in mean absolute cover, measured as the mean number of taxa intersecting each point along a transect, was apparent. Annual rainfall correlated positively with mean absolute cover of vegetation. Six vegetation communities were visited at Santa Barbara Island compared to nine at Santa Cruz Island, despite the smaller number of transects surveyed at the latter. Mean absolute cover at both islands together averaged 87%, with native plants covering 44% and non-natives covering 39%. Among vegetation communities, mean absolute cover ranged from a high of 137% in a Lyonothamnus grove to a low of 32% in a riparian community where few plants were encountered along one transect. Mean absolute cover was higher at Santa Cruz Island (105%) than Santa Barbara Island (77%), and mean absolute native cover followed the same pattern (57% vs 36%). For both islands combined, mean richness per transect was 11 taxa. Compared to Santa Barbara Island, Santa Cruz Island supported higher overall mean species richness (14 vs 9 taxa) and higher species evenness (0.70 vs 0.55). Santa Cruz Island’s higher absolute cover, higher native species cover, higher species richness, and higher species evenness indicate that its vegetation is in better overall condition compared to Santa Barbara Island, and that the rate of recovery after overgrazing is faster at Santa Cruz Island than Santa Barbara Island.

BACKGROUND: Chronic pain affects a large proportion of the general population – at least 34% of adults in the UK - and it has a huge impact on people’s lives as well as the workplace and health services, accounting for more than 75% of the years lived with disability. Despite this, chronic pain is poorly captured in clinical data which makes it difficult to identify appropriate patient cohorts especially for retrospective observational studies. A different model for pain-related projects is to include the patient voice directly into the team with the addition of funded patient members. Here we present our positive experiences from within the Alleviate Pain Data Hub. AIMS: To demonstrate how the patient voice is an intrinsic part of chronic pain research through the experience of the Alleviate Pain Data Hub and its pain community. To highlight the lived experience of people with chronic pain to inform the project and to respond to important issues. METHODS: Throughout the lifetime of the Alleviate project two patient representatives have been included as part of the core project team. Using their input, alongside the input of a wider Chronic Pain Advisory Group, a continuous theme of outputs was generated to inform current and future research. For example, in 2023, a chronic pain survey based on a set of questions developed and used by Chronic Pain Australia (CPA) in 2023 was adapted for the UK population. Ethics approval was gained by the University of Dundee, School of Medicine ethics committee. The survey was open for a month between 16th November – 15th December 2023 and was shared via social media by the Alleviate Data Hub, the Advanced Pain Discovery Platform (APDP), Pain UK and HDR UK, plus direct contact was made with various large organisations, trade unions and charities. The survey was open to all adults across the UK. RESULTS: The patient members of the team have been included throughout the project lifecycle and have been instrumental in delivering several of Alleviate’s outputs. Together with a public/patient involvement and engagement (PPIE) coordinator the patient members developed a recruitment strategy for the Alleviate pain community which includes 336 individuals. From this pool we recruited a panel of pain forum members who join in regular project meetings, PPIE specific events and have contributed to the direction taken in Alleviate. This includes the initiation of a collaboration between Alleviate, APDP, Pain UK and CPA on the reuse of the Australian survey in the UK. The questions covered general demographics of the participants and covered topics around an individual’s experience with pain and subsequent contact with health professionals, what research topics are important, and their emotional experiences relating to chronic pain research. 623 responses from across the UK four nations were received with a dominance of people living in England (71%), female (80%), with a median age of 53 and 9% from an ethnic minority background. Some key responses were that 75% of respondents felt ignored or dismissed by healthcare services and upto 44% were stigmatised by GPs or other health professionals. Further analysis will be presented covering aspects of the patient journey, management of their pain and views on most important research topics. CONCLUSIONS: Being able to include the knowledge and experience of those living with pain and act on their needs and/or requests highlighted how impactful partner patient partners can be. The patient voice is a powerful director and motivator within Alleviate which has instigated several activities that would not have otherwise occurred: 1) raised the importance of chronic pain; 2) produced instructive materials for the public and researchers; and 3) instigated new collaborations.
