Delsoline, a major alkaloid of Delphinium anthriscifolium Hance, has both a curare-like effect and a ganglion-blocking effect and is used to relieve muscle tension or hyperkinesia. A ultraperformance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method was established for the determination of delsoline in mouse blood, and the pharmacokinetics of delsoline after intravenous administration (1 mg/kg) and intragastric administration (9, 6, and 3 mg/kg) were studied. Gelsenicine served as an internal standard, and a UPLC BEH C18 chromatographic column was used. The mobile phase consisted of acetonitrile and 0.1% formic acid; the gradient elution flow rate was 0.4 mL/min. The MRM model was used for the quantitative analysis of delsoline m/z 468.3⟶108.1 and the internal standard m/z 327.1⟶296.1. Mouse blood samples were treated with acetonitrile precipitation to remove proteins. In the concentration range of 0.1–1000 ng/mL, delsoline in mouse blood showed a good linearity (r2 > 0.995), and the lower limit of quantitation was 0.1 ng/mL. The intraday precision relative standard deviation (RSD) was below 14%, and the interday precision RSD was below 15%. The accuracy ranged between 94.3% and 110.1%, the average recovery was above 90.8%, and the matrix effect ranged between 97.0% and 102.5%. The UPLC-MS/MS method was sensitive, rapid, and selective in the study of pharmacokinetics of delsoline. The absolute bioavailability of delsoline was 20.9%.

In health, food carbohydrate is stored as glycogen in muscle and liver, preventing a deleterious rise in osmotically active plasma glucose after eating. Glycogen concentrations increase sequentially after each meal to peak in the evening, and fall to fasting levels thereafter. Skeletal muscle accounts for the larger part of this diurnal buffering capacity with liver also contributing. The effectiveness of this diurnal mechanism has not been previously studied in Type 2 diabetes. We have quantified the changes in muscle and liver glycogen concentration with 13C magnetic resonance spectroscopy at 3.0 T before and after three meals consumed at 4 h intervals. We studied 40 (25 males; 15 females) well-controlled Type 2 diabetes subjects on metformin only (HbA1c (glycated haemoglobin) 6.4±0.07% or 47±0.8 mmol/mol) and 14 (8 males; 6 females) glucose-tolerant controls matched for age, weight and body mass index (BMI). Muscle glycogen concentration increased by 17% after day-long eating in the control group (68.1±4.8 to 79.7±4.2 mmol/l; P=0.006), and this change inversely correlated with homoeostatic model assessment of insulin resistance [HOMA-IR] (r=−0.56; P=0.02). There was no change in muscle glycogen in the Type 2 diabetes group after day-long eating (68.3±2.6 to 67.1±2.0 mmol/mol; P=0.62). Liver glycogen rose similarly in normal control (325.9±25.0 to 388.1±30.3 mmol/l; P=0.005) and Type 2 diabetes groups (296.1±16.0 to 350.5±6.7 mmol/l; P<0.0001). In early Type 2 diabetes, the major physiological mechanism for skeletal muscle postprandial glycogen storage is completely inactive. This is directly related to insulin resistance, although liver glycogen storage is normal.

In a double-blind comparative study, 135 depressed patients were treated in 20 centers. Inclusion diagnoses were typical depressions with single episode (296.2), several episodes (296.3), depressive neurosis (300.4), and adjustment disorder with depressed mood (309.0) in accordance with DSM-III-R. The dosage was 3 x 300 mg hypericum extract LI 160 or 3 x 25 mg imipramine daily. The treatment lasted for 6 weeks. Main assessment criteria were the Hamilton Depression Scale (HAMD), the Depression Scale according to von Zerssen (D-S) and the Clinical Global Impressions (CGI). In both treatment groups, a parallel reduction of the Hamilton score from 20.2 to 8.8 (LI 160, n = 67) or from 19.4 to 10.7 (imipramine, n = 68), and the transformed D-S point values from 39.6 to 27.2 (LI 160) and 39.0 to 29.2 (imipramine) were found. The analysis of CGI revealed comparable results in both treatment groups. Clinically relevant changes of the safety parameters were not found. In the LI 160 group fewer and milder side effects were found as compared to imipramine.

Telah berhasil dibuat Dye Sensitized Solar Cell dengan menggunakan pewarna dari ekstrak biji kakao dengan variasi lama perendaman semikonduktor dalam pewarna. Hasil pengujian XRD menunjukkan bahwa Kristal TiO2 berada difase anatase dengan ukuran Kristal 25.99228 nm, 12.94122 nm dan 12.94136 nm yang dihasilkan dengan metode Debye Scherrer. Karakterisasi UV-Vis dye pada panjang gelombang 300 nm - 800 nm menunjukkan bahwa pewarna dapat bekerja pada cahaya UV dan cahaya tampak sedangkan untuk hasil UV-Vis pada semikonduktor TiO2 hanya memiliki puncak absorbansi pada cahaya UV pada panjang gelombang 296.0 nm yaitu 2.5938 a.u. Nilai tegangan dan arus yang diperoleh akan diakumulasi untuk mengetahui efisiensi dari rangkaian DSSC. Masing –masing efisiensi() yang dihasilkan untuk lama perendaman 6 Jam, 12 Jam, 18 Jam dan 24 Jam secara berurut adalah 0,0197%; 0.0258%; 0.0397% dan 0.0426%.

An efficient and regioselective synthetic reaction friendly to the environment has been described to synthesize various derivatives of pyrazolo[1,5-a]quinozolinone. Condensation of aminopyrazole (4a-m) with formylated dimedone (3) in the presence of KHSO4, under ultrasonic irradiation furnished 2/3-substituted 8,8-dimethyl-8,9-dihydropyrazolo[1,5-a]quinazolin-6(7H)-one (6a-m). This is a clean reaction, giving excellent yields with short reaction time. The structures were elucidated with the help of spectral and analytical data. X-ray crystallographic studies of a model compound 6a ascertained its structural configuration, crystal data for C12H12BrN3O (M =294.152 g/mol): Triclinic, space group P-1 (no. 2), a = 5.872(4) Å, b = 10.870(8) Å, c = 19.523(15) Å, α = 90.013(10)°, β = 90.009(11)°, γ = 93.838(11)°, V = 1243.3(16) Å3, Z = 4, T = 296.15 K, μ(Mo Kα) = 3.293 mm-1, Dcalc = 1.571 g/cm3, 37271 reflections measured (4.18° ≤ 2Θ ≤ 52.7°), 5073 unique (Rint = 0.2404, Rsigma = 0.2366) which were used in all calculations. The final R1 was 0.0596 (I≥2σ(I)) and wR2 was 0.1759 (all data).

Background: Crescentic IgA nephropathy (CreIgAN) has a poor prognosis despite aggressive immunosuppressive therapy. The efficacy of plasma exchange (PE) in CreIgAN is not well defined. Methods: Twelve patients with severe CreIgAN who received PE as addition to routine immunosuppressive therapy, followed for more than 6 months, were involved. Twelve matched historical controls who received immunosuppressive therapy alone were selected by propensity score matching. Renal survival, plasma IgA-IgG complex and active complement products were assessed. Results: Nine men and 3 women received a median of 7 PE courses (range 5-10). Their baseline urine protein excretion rate was 5.8 (4.5-8.7) g/day, and their serum creatinine level was 705.3 ± 296.4 μmol/l. During a mean follow-up of 15.6 months (6-51 months), 6 of the 12 PE group patients were free of dialysis, while all the control patients were dialysis dependent (6 of 12 vs. 0 of 12, p = 0.014). In the PE group, dialysis had to be restarted for 1 patient owing to the development of severe pneumonia and pulmonary failure. PE was associated with a higher kidney survival rate (log rank test, p = 0.026) during follow-up. It also significantly decreased plasma IgA-IgG complex levels (pre-PE: 85.3 ± 25.9% vs. post-PE: 38.4 ± 12.4%, p < 0.001) and plasma and urinary active complement product levels, including C3a, C5a and soluble C5b-9. The latter levels remained low until the last follow-up. Conclusion: This study indicated that PE could increase renal recovery rates in severe CreIgAN.

The crystal data for C19H15ClN4O2: monoclinic, space group P21/n (no. 14), a = 7.3314(15) Å, b = 12.459(3) Å, c = 18.948(4) Å, β = 98.322(9)°, V = 1712.5(6) Å3, Z = 4, T = 296.15 K, μ(MoKα) = 0.245 mm-1, Dcalc = 1.423 g/cm3, 5082 reflections measured (3.926° ≤ 2Θ ≤ 38.556°), 1428 unique (Rint = 0.0545, Rsigma = 0.0574) which were used in all calculations. The final R1 was 0.0423 (I >2σ(I)) and wR2 was 0.1145 (all data). The Density functional theory optimized molecular geometries in TMQ agree closely with those obtained from crystallographic studies. The Highest Occupied Molecular Orbital (HOMO) and Lowest Unoccupied Molecular Orbital (LUMO) energy levels and energy gap were calculated by experimental (UV absorption & Cyclic voltammetry) and theoretical studies in two different solvents. The natural bond orbital analysis was performed to understand the molecular interaction on the basis of stability of molecule arising from hyper-conjugative interaction and charge delocalization. Hirshfeld surface and their related fingerprint plots enabled the identification of significant intermolecular interaction. The molecular electrostatic potential analysis provides the visual image of the chemically active sites and comparable reaction of atoms.

The storage experiment examined the effects of storage temperatures, seed moisture contents (SMC), and storage durations (SD) on the viability and vigour of sunflower seeds. Certified sunflower seeds were evaluated for seed germination, seedling vigour, and fungal infection. A 3x3x6 factorial experiment in Completely Randomized Design was set out in four replicates. The three factors were temperature (5, 20, 35 ºC), SMC (6,8,10%), and SD (15, 30, 45, 60, 75, 90Days After Storage, DAS). Before storage (0 DAS), the initial germination percentage (GP=94%), Germination index (GI=322), Seedling vigour index (SVI=2118), and fungal infection incidence (FII=52%) were determined and used as control. The lowest GP of 67%, GI (269.2), SVI (1295), and FII (78.0%), was found in seeds stored at a temperature of 35 ºC and SMC of 10% at 90DAS. Upon the interaction effect at the end of 90DAS, the temperature of 5 ºC with SMC of 6% followed by the temperature of 20 ºC with SMC of 6% recorded the greatest GP of 87.5 and 87.0%, GI of 304.3 and 296.8 SVI of 1910 and 1877 and fewer fungi incidence (%) of 61.5 and 63 respectively. The germination percentage was above the minimum certification standards of 85%, as per the Tanzania Official Seed Certification Institute. Conclusively, our results discovered that the optimum storage conditions-Temperature of 5 and 20 ºC with SMC of 6 and 8% could be suitable for maintaining the viability and vigour of sunflower seeds throughout the storage duration (90 DAS).

Objective:Cobalamin (B12) deficiency has been reported in infants born to mothers with low cobalamin intake. Early diagnosis of vitamin B12 deficiency in infants is critical for the prevention of neurobehavioral disorders. We investigated the relationship between serum vitamin B12 level in newborns and in their healthy mothers who consumed an omnivorous diet. Anthropometry was studied longitudinally to assess the growth velocity of the infants. Urinary methylmalonic acid (MMA) excretion of 6-month old infants was compared retrospectively as the biomarker correlated with the initial serum vitamin B12 concentrations. Methods: Serum cobalamin and blood hemoglobin were determined in 84 pairs of newborns and their mothers. Urinary MMA excretion was measured in the same subjects during the first 6 months of the post partum period. Results: At birth, median serum cobalamin levels were 152.0 pmol/L in the mothers and 296.6 pmol/L in the newborns. Maternal and neonatal serum cobalamin levels had no effect on growth velocity during the first six months of postnatal life. Serum maternal and neonatal cobalamin levels were inversely associated with urinary MMA excretion. Conclusion: Early diagnosis of vitamin B12 status in neonates and infants is crucial, particularly in nutritionally deprived areas. Biochemical measurement of plasma cobalamin or its metabolic marker MMA is highly recommended. Urinary MMA measurement in cobalamin diagnostics provides an advantage in that blood sampling is not required. A vitamin B12 taskforce should be created to alleviate vitamin deficiency and its negative consequences.

The main objectives of this study were to estimate the age and growth of reared Atlantic bluefin tuna and to reveal possible differences in growth between reared and wild specimens. A total of 806 specimens, ranging from 102 to 284 cm in length, were sampled from a Greek tuna farm in the Mediterranean Sea. The first spine of the first dorsal fin was used to estimate the age. Estimated ages ranged from 3 to 20 years, and the reproducibility of the ageing method was high. The proportion of the mean number of missing annuli per age due to vascularization ranged from 27.8 to 40.0%, with an average loss of 33.5%. No significant differences in the number of missing annuli were observed between 6–month and 18–month reared specimens of the same age group. The von Bertalanffy growth model was fitted a) to mean FL-at-age data and b) to mean RW-at-age data, and the growth equations found were: Lt = 296.6[1 – e(−0.114) (t + 1.026)] and Wt = 591.6[1 – e(−0.100)(t + 1.755)]3. Our results showed that the rearing process accelerates the growth of the Atlantic bluefin tuna and this was also evident in the calcified structure. In comparison with previous studies of growth in wild bluefin tuna from the Mediterranean Sea, mean weights at age of the reared fish were higher than those from wild fish in all age groups and lengths at age of the reared bluefin tuna were higher in younger fish, up to 6 years old.

Background: PPH is the prime cause of maternal mortality worldwide. The incidence of PPH can be drastically combatted by application of uterotonic in active management of third stage labour. Timing of its administration is a matter of concern. This study aimed to assess whether timing of administration of oxytocin would have any impact on incidence or mean blood loss of PPH.Methods: It was a single blinded randomized controlled trial conducted in the department of Obstetrics and Gynecology, Bankura Sammilani Medical College for 6 months where 100 antenatal mothers admitted for normal delivery in labour room were allocated randomly in study and control group considering inclusion and exclusion criteria. Incidence of PPH and mean blood loss had been identified clinically by following them for 24 hours.Results: Only 9% of study population had experienced PPH. There was no statistically significant difference in incidence of PPH with difference in timing of administration of oxytocin (p >0.05). But there was statistically significant decrease in mean blood loss if oxytocin was administered before the placental delivery. The mean blood loss with oxytocin administered before placental delivery was 296.8 ml (102.45) and after placental delivery was 452.0 (128.87) ml respectively.Conclusions: Policy makers should keep in mind not only the incidence of PPH, but the mean blood loss amount too in a setting where anaemia in pregnancy is quite prevalent.

Most hospitals use protocols to manage diabetic ketoacidosis using laboratory criteria to monitor the response to insulin and fluid therapy. This study analyzed information collected prospectively on a group of patients with diabetic ketoacidosis to review outcomes and management details. The study included 37 patients with a mean age of 38.1 ± 18.5 years. The initial blood glucose was 546.4 ± 296.3 mg/dL. The initial anion gap was 31.8 ± 7.8 meq/L. The mean time to anion gap closure twice was 19.2 ± 12.8 hours. The mean fluid administered until anion gap closure was 3694.9 ± 2484.6 mL. The insulin dose during the first day of management was 69.5 ± 49.9 units; the mean number of point-of-care glucose levels during the first 24 hours was 21.2 ± 6. The number of basic metabolic panels collected during hospital management was 12.2 ± 13.7. The mean ICU length of stay was 2.5 ± 3.8 days with no mortality. This study indicates that patients with diabetic ketoacidosis had a large number of basic metabolic panel tests and a large number of point of care glucose measurements using this protocol. This protocol needs review to determine whether or not the number of tests can be reduced, and transfer out of the ICU can occur more quickly. Background: Functional parameter in Chronic Obstructive Pulmonary Disease (COPD) can be evaluated withPeak Cough Flow (PCF), as an effective tool to measure cough ability. Patients with COPD can not do cougheffectively, that leads to sputum retention and finally functional capacity. The aimed of the study to foundimprovement of cough ability (PCF) and COPD symptom.Methods:This was a retrospective cohort study. The researcher did the measurement for PCF, COPD assesmenttool (CATTM ) score and the sixt minute walking test (6MWT) on COPD patients, after rehabilitation programthree times a week, for four weeks. The program consisted of infrared diathermy, breathing exercise, chestmobility exercise, the active cycle breathing technique and static cycle or treadmill exercise program betweenJanuary-June 2018.Results: Subjects are ten COPD patients, the mean of age was 70 years old. The data of PCF, CATTM, anddistance of 6MWT before intervention were 246± 57.19 L/ minute, 13.1± 6.36, and 296.4± 79.30. Thereforethe PCF, CATTM, and distance in 6MWT after four weeks intervention were 269 ± 70.78 (p< 0.05), 15.2 ± 8.56(p> 0.05), and 339.3 ± 62.55 (p> 0.05).Conclusion: Rehabilitation program on COPD patient along four weeks improve the PCF, however did notimprove the CATTM score and the distance of 6MWT.Keywords: 6 Minute Walk Test (6MWT), Chronic Obstructive Pulmonary Disease (COPD), COPD Assesment Test(CAT) Score, Functional Parameter, Peak Cough Flow (PCF).

Objective: To investigate a predictive role for the protein S-100b and serum circulating levels of Th1/Th2 cytokines in patients with chronic hepatitis C virus (HCV) infection with and without mixed cryoglobulinemia (MC). Methods: Sixty chronically HCV-infected patients were divided into two groups: 30 with and 30 without MC. Patients with MC presented detectable mixed cryoglobulins and clinical weakness, purpura and arthralgias. HCV-RNA and genotype, serum levels of cryoglobulins, principal hepatic indexes and levels of IL-6, IL-18 and S-100b protein were evaluated. Twenty uninfected healthy subjects were a control group to evaluate serum levels of S-100b protein. Results: IL-6 and IL-18 serum levels were higher in the MC+ group than the MC- group (8.7 ± 4.5 pg/mL versus 4.6 ± 2.3 pg/mL P < 0.0001 and 743.5 ± 128.2 pg/mL versus 578.5 ± 296.5 pg/mL P < 0.001 respectively). S-100b serum levels were higher in HCV+ with MC (0.23 ± 0.07 microg/L) respect to HCV+ patients without MC (0.17 ± 0.05 microg/L, P < 0.0001) and were statistically higher than in the control group (0.08 ± 0.03 microg/L, P < 0.0001 and P < 0.0001, respectively). A positive correlation was shown between serum levels of S-100b protein and levels of cryoglobulins in the group of HCV+ patients MC+ (r=0.72 and P < 0.0001). Conclusion: HCV patients with MC have a worse inflammatory condition than those without MC. Moreover, S-100b protein seems to be a sensitive marker of endothelial and tissue damage in chronic HCV hepatitis with cryoglobulinemia.

Using bovine pulmonary artery intimal explants mounted in Boyden chambers and homologous separated granulocytes, concentrations of thromboxane B2 and 6-keto-PGF1 alpha in the upper-well fluid were measured by radioimmunoassay over a three-hour period under the following conditions: (1) granulocyte chemotaxis (zymosan-activated plasma in the lower well, granulocytes in the upper well); (2) unstimulated granulocyte migration (serum or plasma in the lower well, granulocytes in the upper well); (3) granulocyte activation without migration (zymosan-activated plasma and granulocytes in the upper well); (4) granulocyte chemotaxis in the absence of endothelium (identical to condition 1 above except that endothelium was scraped from the explant surface); and (5) explants incubated in the absence of granulocytes. Minimal increases in thromboxane B2 concentrations in upper-well fluid occurred under all conditions. In contrast, granulocyte chemotaxis was accompanied by large increases in concentrations of 6-keto-PGF1 alpha evident by two hours of incubation and increasing markedly by three hours, to 524.3 +/- 69.0 ng/mL (m +/- SEM). Unstimulated migration of granulocytes toward serum or plasma and granulocyte activation without migration were accompanied, at three hours, by more modest increases in 6-keto-PGF1 alpha (296.5 +/- 46.4; 128.0 +/- 38.6, and 236.7 +/- 47.0 ng/mL, respectively) and, in the absence of granulocytes or in the absence of endothelium, only minimal increases in this prostacyclin metabolite occurred (137.2 +/- 16.9 and 53.9 +/- 12.6 ng/mL, respectively). The large rises in prostacyclin metabolite occurred at a time when the majority of granulocytes had migrated through the endothelial layer rather than during their adherence or transendothelial passage. We conclude that chemotaxis of granulocytes through pulmonary vascular endothelium causes endothelial production of large amounts of prostacyclin, but this occurs late in the chemotactic process, after granulocytes have transversed the endothelium.

The purpose of the research is to study the biotransformation of fenbendazole in the body of sheep after administration of fenbendazole solid dispersion (FSD) prepared by mechanochemical technique with arabinogalactan. Materials and methods. The FSD at a dose of 2.0 mg/kg for the active substance was administered orally to sheep. Animal blood serum samples were studied by high performance liquid chromatography / tandem mass spectrometry (HPLC-TMS) to determine the concentration of fenbendazole (FBZ) and its sulfoxide and sulfone metabolites at 0, 1, 2, 4, 6, 8, 12, 24, 33, 48 and 72 hours after administered FSD and initial FBZ (substance). FBZ and its metabolite residual quantity in the organs and tissues of the sheep was determined at 1, 3, 6, 11, and 21 days after the drug administration. The prepared sample and validated method were described. Results and discussion. A significant difference was found in the metabolism, pharmacokinetics, and timing of the FBZ and its metabolite elimination after the base drug (FBZ substance) and FSD were administered to sheep in an equal dose of 2.0 mg/kg for the active substance. FBZ and its metabolites began to be detected in blood serum 2 hours after the FSD and 4 hours after the base FBZ. Pharmacokinetic parameters of FBZ and its metabolites characterize a higher drug concentration in the blood and a longer retention time in the circulation after the FSD as compared with the base drug parameters. The FBZ and its metabolite maximum concentration was found in the organs and tissues of the sheep that received the FSD on day 3 in the liver amount of 4862.3±296.2 ng/g of FBZ; 18243.5±486.1 ng/g of sulfoxide; and 2482.3±132.4 ng/g of sulfone; and tens of times lower concentration after the base FBZ on day 6. FBZ and its metabolites were not detected in the organs and tissues of the sheep on day 16 after the base FBZ, and on day 21 after the FSD.

AMAÇ: Glukokinaz (GCK) gen mutasyonu pozitif olan Genç Erişkin Dönemde Başlayan Diyabet (Maturity Onset Diabetes of Young - MODY) tanısı alan olgularla sağlıklı bireylerin koroid tabakası kalınlıklarının karşılaştırılması amaçlandı.GEREÇ VE YÖNTEM: MODY tanısı GCK gen mutasyonu bakılarak doğrulanmış olan ama diyabetik retinopatisi olmayan 30 hasta (MODY grubu) ve 30 sağlıklı kişi (kontrol grubu) çalışmaya dahil edildi. Kontrol grubu, MODY grubu ile yaş, cinsiyet, refraktif kusur ve aksiyel uzunluk açısından uyumlu seçildi. Koroid kalınlığı hastaların sağ gözünde foveada, foveadan 1500 μm ve 3000 μm uzaklıklarda nazalde ve temporalde RTVue-100 Fourier-domain optik koherens tomografi (OKT) kullanılarak ölçüldü.BULGULAR: MODY tanı zamanı 4.1±5.8 yıl (aralık; 0-17 yıl) idi. Ortalama HBa1c düzeyi 5.1 (aralık: 4.4-6.2, referans aralık: 4-6) iken hiçbir hastada diyabetik retinopati yoktu. Ortalama subfoveal koroid kalınlığı subfoveal alanda MODY grubunda 370.09± 86.66 μm iken kontrol grubunda 354.44 ± 76.70 μm idi (p=0.456). Koroid kalınlığı nazal 1500 μm’de (sırasıyla 300.75 ± 67.02 ve 296.13 ± 73.45 μm, p=0.672), nazal 3000 μm’de (184.56 ± 45.53 ve 192.94 ± 57.63 μm, p=0.485), temporal 1500 μm’de (325.16 ± 59.41 ve 299.59 ± 67.81 μm, p=0.093) ve temporal 3000 μm’de (261.88 ± 43.30 ve 256.75 ± 50.83 μm, p=0.582) istatistiksel anlamlı değişiklikler göstermedi.SONUÇ: Literatürde diyabetin koroid üzerine etkilerini araştıran birçok çalışma olmasına rağmen, bu çalışma spesifik olarak MODY’li hastalarda koroid kalınlığının değerlendirildiği ilk çalışmadır. Diyabetik retinopatisi olmayan MODY’li hastalarda koroid kalınlığı etkilenmeyebilir. Bu MODY’li hastaların genç olmasına, MODY süresinin kısa olmasına ve diyabet regülasyonunun iyi olmasına bağlanabilir.

A multisensor control system based on the LabVIEW myRIO programming has been created for monitoring indoor air quality. The purpose of this research is to create a system to monitor and control the levels of CO and CO2 in the room so that it remains within the threshold of healthy air and does not endanger users. The research phase began with the manufacture of a multisensor circuit and a relay module for the air purifier system and connected to the input and output ports of the myRIO module as a processor programmed with LabVIEW. The process of testing the multisensor response and the activation response of the air purifier on-off is carried out in open areas and indoors by adding artificial air pollution. Besides, air quality control and monitor is also carried out when the level of CO or CO2 gas exceeds the threshold by increasing the number of users and set the air conditioner activation. From the results and data analysis, it was found that the system could be used as a monitor and control the indoor air quality as expected. The range of CO sensor readings is 7.46 ppm - 27.65 ppm and CO2 296.8 ppm - 1190.5 ppm. Air purifier on-off control response time to change of CO and CO2 are 7 and 6 seconds. The air purifier system is able to clean indoor air with a long activation time depending on the number users and the room air conditioner activation settings.

The persistence of immunity after hepatitis B vaccination is still under investigation in adults. In Chaoyang District, Beijing, people who were aged ≥ 18 years and completely immunized with HBV vaccine according to the standard procedure (0–1–6 months) were enrolled. Three groups were set for 1 (Y1), 5 (Y5) and 10 (Y10) years after the hepatitis B vaccination. The following data was collected and analyzed: antibody against hepatitis B virus surface antigen(anti-HBs) positive rates and geometric mean concentration (GMC) between the different compared groups through questionnaires and laboratory detection, including hepatitis B virus surface antigen (HBsAg), anti-HBs and antibody against hepatitis B virus core antigen(anti-HBc). All 600 subjects completed the questionnaires and serological tests. Among all subjects, the positive rates of HBsAg, anti-HBs and anti-HBc were 0, 70.5% (423/600) and 2.5% (15/600), respectively. The anti-HBs positive rates in Y1, Y5 and Y10 groups were 86.5% (173/200), 71.0% (142/200) and 54.0% (108/200) (χ2 = 50.8, p < 0.001) and showed a linear decreasing trend year by year (trend χ2 = 50.7, p < 0.001). The GMC in Y1, Y5 and Y10 groups were 296.6 mIU/mL, 51.6 mIU/mL and 25.5 mIU/mL (H = 64.8, p < 0.001), respectively. The anti-HBs positive rates and GMC decreased rapidly after the vaccination of adults against hepatitis B. Screening after 5–10 years and booster vaccination for the unprotected population is recommended.

To characterize fluid and ion shifts during prolonged whole-body immersion, 16 divers wearing dry suits completed four whole-body immersions in 5 degrees C water during each of two 5-day air saturation dives at 6.1 msw. One immersion was conducted at 1000 (AM) and one at 2200 (PM) so that diurnal variations could be evaluated. Fifty-four hours separated the immersions, which lasted up to 6 h; 9 days separated each air saturation dive. Blood was collected before and after immersion; urine was collected for 12 h before, during, and after immersion for a total of 24 h. Plasma volume decreased significantly and to the same extent (approximately 17%) during both AM and PM immersions. Urine flow increased by 236.1 +/- 38.7 and 296.3 +/- 52.0%, urinary excretion of Na increased by 290.4 +/- 89.0 and 329.5 +/- 77.0%, K by 245.0 +/- 73.4 and 215.5 +/- 44.6%, Ca by 211.0 +/- 31.4 and 241.1 +/- 50.4%, Mg by 201.4 +/- 45.9 and 165.3 +/- 287%, and Zn by 427.8 +/- 93.7 and 301.9 +/- 75.4% during AM and PM immersions, respectively, compared with preimmersion. Urine flow and K excretion were significantly higher during the AM than PM. In summary, when subjects are immersed in cold water for prolonged periods, combined with a slow rate of body cooling afforded by thermal protection and enforced intermittent exercise, there is diuresis, decreased plasma volume, and increased excretions of Na, K, Ca, Mg, and Zn.

Aim To assess the response of postprandial (PP) hypertriglyceridemia to genfibrozil in healthy male subjects with isolated low HDL-Cholesterol but without the metabolic syndrome (MS). Patients and methods 14 male subjects with isolated low HDL (HDL-C ≤ 33), normal fasting triglycerides and LDL-C levels and without any feature of the MS, were studied. 13 male subjects with HDL-C > 38 mg/dl served as controls. They also had normal fasting triglycerides and LDL-C levels and without any feature of the MS. The 2 groups were statistically similar with respect to age, blood pressure, BMI, body fat composition, waist circumference, waist to hip ratio, fasting insulin, fasting and PP blood sugar, baseline fasting TG level and baseline LDL-C. Postprandial TG levels were measured at 2, 4, and 6 hours following a morning meal. Ten of the patients with PP hypertriglyceridemia were treated with gemfibrozil 600 mg PO twice/day for one month. Results Patients had markedly higher levels of the peak PP TG at 4 hours compared to controls (296.0 ± 37.7 vs. 206.7 ± 29.9 mg/dl; P < 0.05) the other two postprandial levels were also higher in patients but the difference was not significant. Treatment with gemfibrozil significantly decreased the levels of fasting and postprandial TG and increased HDL-C by around 3.6 mg/dl (11.7%) without affecting LDL-C. Conclusion Postprandial increase in serum TG may be present in patients with isolated HDL-C without the MS. Treatment of patients with PP hypertriglyceridemia with gemfibrozil improves the low HDL-C and postprandial rise in TG.

Crystal data for C11H9N3O: Monoclinic, space group P21/c (no. 14), a = 7.8975(2) Å, b = 11.6546(4) Å, c = 11.0648(3) Å, β = 105.212(2)°, V = 982.74(5) Å3, Z = 4, T = 296.15 K, μ(MoKα) = 0.091 mm-1, Dcalc = 1.346 g/cm3, 13460 reflections measured (5.174° ≤ 2Θ ≤ 64.72°), 3477 unique (Rint = 0.0314, Rsigma = 0.0298) which were used in all calculations. The final R1 was 0.0470 (I > 2σ(I)) and wR2 was 0.1368 (all data). The experimentally determined data was supported by theoretically optimized calculations processed with the help of Hartree-Fock (HF) technique and Density Functional Theory with the 6-311G(d,p) basis set in the ground state. Geometrical parameters (Bond lengths and angles) as well as spectroscopic (FT-IR, 1H NMR, and 13C NMR) properties of 2PPT molecule has been optimized theoretically and compared with the experimentally obtained results. Hirshfeld surface analysis with 2D fingerprinting plots was used to figure out the possible and most significant intermolecular interactions. The electronic characterizations such as molecular electrostatic potential map (MEP) and Frontier molecular orbital (FMO) energies have been studied by DFT/B3LYP approach. The MEP imparted the detailed information regarding electronegative and electropositive regions across the molecule. The HOMO-LUMO energy gap as high as 5.3601 eV was found to be responsible for the high kinetic stability of the 2PPT.

Amblyopia is the most common cause of monocular visual impairment in both children, and young to middle-aged adults, affecting 2%–5% of the general population. The objective of this study was to compare the peripapillary nerve fiber thickness and macular thickness in amblyopic eyes, fellow eyes and normal control eyes using spectral domain optical coherence tomography. This was a cross-sectional observational study conducted at R M Kedia Eye Hospital, Birgunj, Nepal from February 2020 to July 2020. Pediatric patients with unilateral amblyopia (anisometropic amblyopia, strabismic amblyopia or both) among the age group of 6-18 years attending pediatric department of RM Kedia Eye Hospital were enrolled for the study. All patients underwent a full ophthalmological assessment, including visual-acuity testing, anterior segment evaluation with Topcon slit lamp and fundus examination with Volk +90D lenses. All statistical analysis was done in SPSS V. 20. The average peripapillary retinal nerve fiber layer thickness was 120.6 μm (SD=14.6 μm) in the amblyopic eye, 118.1 μm (SD=15.6 μm) in the fellow eye and 113.2 μm (SD=9.4 μm) in the normal eye (p=0.104) respectively. The average macular thickness was 298.6 μm (SD=19.1 μm) in the amblyopic eye, 296.9 μm (SD=11.2 μm) in the fellow eye and 303 μm (SD=12.4 μm) in the normal eye (p=0.260) respectively. In conclusion, our study did not find any significant difference in the peripapillary retinal nerve fiber thickness or macular thickness when compared between amblyopic eyes, fellow eyes, gender and age matched normal eyes.

Purpose. To provide quantitative measurements for the foveal avascular zone (FAZ) and to describe its morphological characteristics by optical coherence tomography angiography (OCT-A). Design. Cross-sectional observational case series. Methods. Healthy volunteers were recruited and evaluated using Triton-DRI SS-OCT Angiography. A 4.5 × 4.5 mm square was evaluated by OCT-A center around the fovea. Superficial and deep capillary plexus were identified, and different quantitative measurements were conducted along with categorization of the FAZ pattern. Results. Eighty-two eyes (41 volunteers) were evaluated. Mean age was 30.59 ± 7.6 years (23–42 years). Mean subcentral retinal thickness was 200.1 ± 5.66 um (192–210 um). The number of terminal vessels was variable (range, 8–11). Mean maximum terminal vessel-to-vessel intervening distance was 527.8 ± 60.3 um (471–674 um). Mean minimum terminal vessel-to-vessel intervening distance was 296.7 ± 45.8 um (233–373 um). Mean maximum horizontal diameter of FAZ (superficial) was 716.17 ± 124.09 um, while mean maximum vertical diameter of FAZ (superficial) was 667.76 ± 131.28 um. Mean maximum horizontal diameter of FAZ (deep) was 823.19 ± 144.92 um, while mean maximum vertical diameter of FAZ (deep) was 794.03 ± 150.28 um. OCT-A detected different FAZ patterns; horizontally oval configuration in 32 eyes (39%), rounded configuration in 24 eyes (29.3%), pentagon configuration in 14 eyes (17.1%), and vertically oval and nonspecific configuration in 6 eyes each (7.3%). Conclusion. OCT-A could efficiently provide both quantitative and qualitative assessment of normal FAZ; such characterizations were difficult by standard FAZ assessment procedures like FFA.

We investigated the time course of exercise-induced lipoprotein lipase activity (LPLa) and reverse cholesterol transport (RCT) during the 24-h postexercise period. Subjects were 10 sedentary normolipidemic males [NTG; fasting triglyceride (TG) = 89.1 ± 8.6 mg/dl] and 6 hyperlipidemic males (HTG; fasting TG = 296.8 ± 64.0 mg/dl). Each subject performed a control trial (no exercise) and 4 exercise trials. In the exercise trials, a subject jogged on a treadmill at 60% of his maximal O2 consumption for 1 h. Pre- and postheparin blood samples were taken before exercise (baseline) and at 4, 8, 12, and 24 h after exercise. There was no group difference in LPLa ( P > 0.05) over the time points. When the LPLa data from the two groups were combined, LPLa at 24 h after exercise was higher than baseline or at 4, 8, 12 h after exercise ( P < 0.05). Plasma TG and lecithin-cholesterol acyltransferase activity (LCATa) were higher in HTG than in NTG, and the total high-density lipoprotein-cholesterol (HDLtot-Chol) was lower in HTG than in NTG ( P < 0.05). HDL2-Chol, LCATa, and cholesterol ester transfer protein activity did not differ during the 24-h postexercise period ( P > 0.05). These results suggest that LPLa is still increasing 24 h after an acute aerobic exercise and that the magnitude of the increase in exercise-induced LPLa in HTG was similar to that in NTG. Furthermore, in the sedentary population with or without HTG, the variables related to RCT do not change during the 24-h period after exercise.

The present study concerned the production of glucose-based porous carbon materials by a one-step acid-catalyzed HTC. The samples were characterized by elemental analysis (EA), scanning electron microscope (SEM), Brunauer–Emmett–Teller (BET), Fourier transform infrared (FTIR) and point of zero charge (pzc). Experimental results showed that the addition of sulfuric acid (SA) with different dosages in the HTC system could improve the yield of products and reduce chemical oxygen demand (COD) of the process water. When the glucose and acid was at a mass ratio of 1:4 (glucose: SA = 1:4), the hydrochar obtained (H-G9) had a larger specific surface area (SBET = 296.71 m2/g) and higher abundance of functional groups on the surface than that of other samples, such as sulfur-containing functional groups and carboxylic groups, belonged to the mesoporous material with highly negatively surface charged. H-G9 exhibited the optimum adsorption for methylene blue (MB). H-G9 adsorbed MB with an initial concentration of 10 mg/L at pH 6 and 25 °C. The adsorption isotherm of MB on H-G9 demonstrated that Freundlich isotherm could be better applied. Regeneration efficiency of 88% was achieved by HTC process for saturated H-G9. This study prepared a porous carbon material by the simple one-step hydrothermal carbonization of glucose in the presence of SA. The maximum monolayer adsorption capacity as high as 332.46 mg/g for MB, which was well beyond that of commercial activated carbon (259.37 mg/g). This indicates that H-G9 has great potential for the removal of MB from wastewater.

AbstractIntroductionHeat waves pose a serious public health risk to particular patient populations, especially in urban areas. Emergency Medical Services (EMS) in many urban areas constitute the first line of regional preparation and response to major heat wave events; however, little is known on heat wave operational impact to the EMS system, such as call volume or demand.ObjectiveTo examine the effect of heat wave periods on overall urban EMS system call volume and transport volume as well as the nature of the call types.MethodsRetrospective review of all emergency medical calls to an urban, two-tiered EMS system performed over a 5-year period from 2006–2010. Heat wave days (HWD) defined as two or more consecutive days of hot weather >32.2°C (90°F) were compared with similar non-heat wave days (nHWD) of the previous year to also include two calendar days prior to and after the heat wave. National Weather Service (NWS) temperature data, daily EMS call volume data, and call type codes were collected and underwent descriptive analysis.ResultsThirty-one HWD were identified and compared with 93 nHWD. The mean maximum temperature for HWD was 34°C (93.2°F) compared with 25.3°C (77.6°F) for nHWD (P< .001). Average daily medical emergency calls (318.4 vs 296.3,P< .001) and actual patients transported per day (247.5 vs 198.3,P< .001) were significantly higher during HWD. There was no difference in daily medical emergency call volume or EMS transports between weekdays or weekend days. Objectives: To investigate serum uric as an indicator of fetomaternal complicationsin women with late pregnancy. Study Design: Cross sectional study. Setting: Shalamar Medicaland Dental College Lahore ethical committee. Period: One year from October 2016 to October2017. Methodology: Continuous variables were presented as mean and standard deviationlike age and Serum malonaladehyde concentration; categorical variables were presented asnumbers and percentages like intra uterine growth restriction, Mild to moderate and sever preeclampsia and fetal distress. One way ANOVA, independent sample t test and chi square testwere applied to check significance of results, p value less than or equal to 0.05 was consideredas significant. Results: Overall, 100% (n=400) patients were included in this study. The meanage of the patients was 25.81±4.33 years. The age difference was not statistically significant,in groups (p=0.383). The mean serum uric acid levels of the patients for maturity (weeks)36, 37, 38, 39 and 40 was 247.96±2.52 (nmol/l), 253.95±2.04 (nmol/l), 261.19±3.15 (nmol/l),263.95±2.75 (nmol/l) and 296.19±2.55 (nmol/l) respectively. The differences were statisticallysignificant (p=0.000). Fetal distress, number with rising levels were (n=23) 71.8%, number withno change in levels were (n=3) 9.4% and number with falling levels were (n=6) 18.8%. Thedifferences were statistically significant. (p=0.000). Conclusion: The observations of our studyconcluded that serial increase of serum uric acid from 36th week to 40th week was observedin pregnancy induced hypertion patients, and raised serum level of uric acid had significantrelation with fetomaternal complications. like Fetal distress, preeclampsia, and intrauterinegrowth retardation. So serum uric acid can be used an indicator for fetometernal complicationsin late pregnancy.

To facilitate computational toxicology, we developed an approach for generating high-resolution lung-anatomy and particle-deposition mouse models. Major processing steps of our method include mouse preparation, serial block-face cryomicrotome imaging, and highly automated image analysis for generating three-dimensional (3D) mesh-based models and volume-based models of lung anatomy (airways, lobes, sublobes, and near-acini structures) that are linked to local particle-deposition measurements. Analysis resulted in 34 mouse models covering 4 different mouse strains (B6C3F1: 8, BALB/C: 11, C57Bl/6: 8, and CD-1: 7) as well as both sexes (16 male and 18 female) and different particle sizes [2 μm ( n = 15), 1 μm ( n = 16), and 0.5 μm ( n = 3)]. On average, resulting mouse airway models had 1,616.9 ± 298.1 segments, a centerline length of 597.6 ± 59.8 mm, and 1,968.9 ± 296.3 outlet regions. In addition to 3D geometric lung models, matching detailed relative particle-deposition measurements are provided. All data sets are available online in the lapdMouse archive for download. The presented approach enables linking relative particle deposition to anatomical structures like airways. This will in turn improve the understanding of site-specific airflows and how they affect drug, environmental, or biological aerosol deposition. NEW & NOTEWORTHY Computer simulations of particle deposition in mouse lungs play an important role in computational toxicology. Until now, a limiting factor was the lack of high-resolution mouse lung models and measured local particle-deposition information, which are required for developing accurate modeling approaches (e.g., computational fluid dynamics). With the developed imaging and analysis approach, we address this issue and provide all of the raw and processed data in a publicly accessible repository.

This study focused on the fact that multiple living facilities (gosiwons) can have more rooms on the same site by taking advantage of the fact that the parking area is smaller than the parking lot area (as stipulated by the parking lot law) and multifamily housing, and further evaluated the performance-oriented design of a gosiwon's firefighting facilities. By creating a scenario using standard guidelines (2021), fire and evacuation simulation programs were used to calculate the evacuation time according to life safety standards, with the number of people who could evacuate being calculated according to the change in the width of the escape stairs at the Gosiwon facility. As a result of this analysis, the group that underwent evacuation training and Scenario 6, in which the width of the evacuation stairs was expanded to 2m, showed the shortest evacuation time of 296.3 seconds. As a result of conducting an evacuation simulation for the same building without changing the width of the stairs and fire drills, the evacuation time was shortened by 165.2 seconds from the evacuation time of 461.5 seconds. As a result of this analysis, it is necessary to add the width of the evacuation stairs to the types of safety facilities under the Multi-Use Business Act so as to improve the evacuation safety of gosiwons. In addition, multi-use establishments with 11 or more floors were added to the fire training and education targets as stipulated in the Fire Prevention Act, allowing training to be conducted together with fire agencies, thereby securing evacuation safety and laying the foundation for the revision of the laws.

Background: An excessive immune response during coronavirus disease (COVID-19) can induce cytokine release syndrome (CRS), which is associated with life-threatening complications and disease progression. Methods: This study was aimed to investigate the differences and similarities between CRS induced by COVID-19 and CAR-T therapy, then provide valuable experiences for early identification and controlling CRS progression in COVID-19. We retrospectively evaluated the clinical characteristics of severe CRS (sCRS, grade 3-4) induced by COVID-19 (40 patients) or chimeric antigen receptor T-cell (CAR-T) therapy as a comparator (41 patients). Results: Grade 4 CRS was significantly more common in the COVID-19 group (15/40 [35.7%] vs. 5/41 [12.2%], P=0.008). CAR-T group had more more dramatic increase in cytokine than COVID-19 group (Figure1), including IL-2 (7.3pg/mL [IQR: 2.0-12.7] vs.1.7 [0.7-2.7], P&lt;0.001), IL-6 (7120.6 pg/mL [1066.8-15 136.4] vs. 110.3 [41.7-728.1], P&lt;0.001), IL-10 (174.5pg/mL [61.7, 434.6] vs. 10.1 [6.3-20.6], P&lt;0.001) and IFN-γ (1308.5pg/mL [296.6, 3108.2] vs .35.0 [16.9-60.8], P&lt;0.001). Interestingly, COVID-19 group had significantly higher levels for TNF-α (31.1 pg/ml [16.1-70.0] vs. 3.3 [1.8-9.6], P&lt;0.001).The correlations between viral load/ tumor burden and various cytokine levels were shown in Figure 2. Lg viral loads were correlated with lg IL-6 (R2=0.101; P&lt;0.001) and lg IL-10 (R2=0.105; P&lt;0.001) .In CAR-T group, LDH was a common indicator related to tumor burden among patients with ALL, NHL, and MM. The lg LDH concentration was correlated with the lg serum concentration of IL-6 (R2=0.161; P=0.01). The independent risk factors for COVID-19-related sCRS were hypertension history (OR: 7.167, 95% CI: 2.345-21.903; P=0.001) and minimum platelets &lt;100×109 /L during disease course (OR: 9.237, 95% CI: 2.544-33.546; P=0.001). Conclusion: Our study demonstrated that there were similar processes but different intensity of inflammatory responses of sCRS in COVID-19 and CAR-T group. The diagnose and management of COVID-19 related sCRS can learn lessons from treatment of sCRS induced by CAR-T therapy. AimsImproving access to Hackney Integrated Learning Disability Service (ILDS) using quality improvement (QI) methodology by reducing the time taken to complete eligibility assessment by 50% by April 2021, while improving service user and staff experience.BackgroundReferrals to ILDS require assessment of eligibility. It was noted that there was significant waiting time between referral and eligibility assessment, exacerbated by the COVID-19 pandemic. Quality Network for Community Learning Disability Services guidelines suggest waiting times for those accessing adult learning disability (LD) community services be locally agreed, although there is limited literature on this topic.MethodsAll staff members across the multidisciplinary team were invited to participate in the QI project. We defined outcome measures as days from referral to allocation of eligibility assessment to staff member and to completion of eligibility assessment, comparing referrals received before and after start of the project. The key change ideas tested using Plan-Do-Study-Act cycles were: (1) eligibility screening checklist, (2) eligibility assessments drop-in sessions for staff, (3) formal training for ILDS staff, (4) eligibility screening allocation system, (5) template letters for eligibility decisions, (6) new ILDS referral form, (7) workshops for local general practitioners.ResultsTime taken to eligibility assessment allocation decreased from median of 184 (mean=183.5±109.8) to 13 days (mean=19.9±26.4) (93% reduction). Time to completion of eligibility assessment decreased from a median of 271 (mean=296.0±133.8) to 63 days (mean=75.7±34.8) (77% reduction). We received positive feedback from staff and service users regarding the new eligibility process.ConclusionsWe report waiting times for accessing our community adult LD services and effective strategies for reducing this. We recommend similar services use a QI methodology to reduce waiting times and improve the experience of staff and service users.

Global warming and the associated climate change are imposing abiotic stress on plants. Abiotic factors are crucial for plant productivity, survival, and reproduction. Eight sunflower hybrids were tested in conditions of different water availability and with combinations of different temperatures and irradiation. The changes in the photosynthetic efficiency were measured in the morning (control conditions: 2013, 25.8 °C and 349.1 W m−2; 2014, 21.8 °C and 296.4 W m−2) and afternoon (the combination of increased temperatures and high irradiation: 2013, 34 °C and 837.9 W m−2; 2014, 29.4 °C and 888.9 W m−2) at a flowering stage in rainfed or irrigated conditions. The measurement time (morning and afternoon conditions) had a statistically significant effect on all the tested parameters. The performance index (PIABS) in 2013 and the maximum quantum yield of photosystem II (TR0/ABS) in 2014 are the only parameters significantly affected by the irrigation. As a result of the combined effect of increased temperatures and high irradiation, PIABS values decreased by 73–92% in rainfed conditions and by 63–87% in irrigated conditions in 2013, depending on the hybrid, while in 2014, the decrease varied between 70 and 86%. The TR0/ABS decrease was 7–17% in 2013, depending on the hybrid, and 6–12% in 2014, both in rainfed and irrigated conditions. The principal component analysis confirmed the effect of the combination of increased temperatures and high irradiation on hybrids, sorting them exclusively according to the time of measurement. All investigated parameters highly fluctuated between hybrids but without observable trends for the morning and afternoon conditions, as well as for irrigation. Plants’ reaction to the combination of increased temperatures and high irradiation manifested as a change in their photosynthetic efficiency, i.e., the photosynthetic apparatus’ functioning was impaired.

Amyotrophic lateral sclerosis (ALS) is the most common motor neuron disease, but its definitive diagnosis delays around 12 months. Although the research is highly active in the biomarker field, the absence of specific biomarkers for diagnosis contributes to this long delay. Another strategy of biomarker identification based on less specific but sensitive molecules may be of interest in clinical practice. For example, markers related to other neurodegenerative diseases such as Alzheimer’s disease (AD) could be fully explored. Here, we compared baseline levels of amyloidβ1-42 (Aβ1-42), total Tau, and phosphorylated-Tau (phospho-Tau) protein in the cerebrospinal fluid (CSF) of ALS patients to controls and correlated it with clinical parameters of ALS progression collected over 12 months. We observed increased levels of Aβ1-42 (controls: 992.9 ± 358.3 ng/L; ALS: 1277.0 ± 296.6 ng/L; p < 0.0001) and increased Aβ1-42/phospho-Tau ratio and Innotest Amyloid Tau Index (IATI) (both p < 0.0001). IATI and the phospho-Tau/total Tau ratio correlated positively with ALSFRS-R and weight at baseline. Multivariate analysis revealed that baseline ALSFRS-R was associated with Aβ1-42 and phospho-Tau/total Tau ratio (p = 0.0109 and p = 0.0013, respectively). Total Tau and phospho-Tau levels correlated negatively with ALSFRS-R variation at months 6 and 9, respectively (p = 0.02 and p = 0.04, respectively). Phospho-Tau/total Tau ratio correlated positively with ALSFRS-R variation at month 9 (p = 0.04). CSF levels of Aβ1-42 could be used as a complementary tool to ALS diagnosis, and total Tau and phospho-Tau levels may help establishing the prognosis of ALS. Further studies merit exploring the pathophysiological mechanisms associated with these markers. Despite their lack of specificity, phospho-Tau/total Tau and Aβ1-42 should be combined to other biological and clinical markers in order to improve ALS management.

SummaryAsymmetric dimethylarginine (ADMA), myeloperoxidase (MPO) and paraoxonase 1 (PON1) are directly involved in the pathogenesis of atherosclerosis by modulation of oxidative stress and/or NO bioavailability. We aimed to assess whether endurance exercise which is known to be cardioprotective could beneficially affect these novel risk markers. Thirty-two subjects (31–68 yrs, 56% males) with elevated cardiovascular risk including ten patients with coronary artery disease volunteered for a supervised 12-week endurance training (196 ± 15 min/week).Their fitness evaluated by 2 km test runs improved significantly after training (pre: 17.3±0.8 vs. post: 15.7±0.9min, p<0.001). ADMA (pre: 0.94±0.03 vs. post: 0.75±0.04 μmol l−1) and MPO (pre:296.8±22.2 vs.post:185.7±19.5 ng ml−1) serum levels decreased significantly by 17.6±4.6% and 28.5±7.5%, respectively, after training (both p<0.001).Their down-regulation was inversely correlated (ADMA: r=-0.609, p<0.001, MPO: r=-0.437, p=0.014) with the up-regulation of plasma cGMP levels (Cyclic-guanosine 3`,5`-monophosphate; pre: 1.6±0.12 vs. post: 2.21±0.2 μmol ml−1, p=0.001) reflecting NO production. PON1 activity towards phenylacetate was not significantly influenced by training (pre: 133±6 vs. post: 130±5 μmol ml−1 min−1, p=0.375). In a matched inactive control group (n=16) ADMA, MPO, cGMP levels and PON1 activity did not change over time. ADMA, MPO and cGMP changes were significantly different between participants and controls (all p<0.05). Regular endurance exercise was successful in reducing the circulating levels of two promising cardiovascular risk markers, ADMA and MPO, in persons prone to cardiac events. These changes may result in numerous antiatherosclerotic effects such as improvement of NO bioavailability, reduction of oxidative stress and lipid peroxidation.

INTRODUCTION: Patients with glioblastoma experience a wide variation in response to standard treatment, with nearly 30% experiencing tumour progression during treatment, and nearly 6% surviving more than 5 years. To date, there are few non-invasive clinical biomarkers to predict response to firstline treatment. Chemical exchange saturation transfer (CEST) MRI may have the potential to fill this gap. CEST MRI is sensitive to treatment-induced changes and changes in tumor metabolism1. Our team has obtained CEST data for patients before, during and after standard chemoradiation treatment, and found that CEST provides markers of early response and can identify early, standard and late progressors before treatment initiation.2 METHODS: Patients (n = 104) with primary, IDH wild-type glioblastoma were imaged with CEST-MRI at multiple time points throughout standard chemoradiation treatment. DNA and RNA were co-extracted from matched normal and tumour pairs and processed for whole genome sequencing and gene expression analysis using Nanostring. Clinical variables such as age, extent of resection, sex, ECOG status and MGMT promoter methylated were also collected. A survival analysis was conducted using the Kaplan-Meier method with log-rank tests. Univariate and multi-variate hazard ratios for clinical variables were calculated by fitting Cox Proportional Hazards Models. RESULTS: Early progressors reported a median progression-free survival (PFS) of 142 days compared to 296.5 in standard and 832 days in late progressors (p < 0.0001). Early progressors also harbored distinct and statistically significant differences in gene expression and genomic alterations, namely in DNA damage repair, glucose transport and arginine metabolism pathways. A gene signature was prognostic of PFS and overall survival in a Cox proportional hazards model. CONCLUSIONS: This data may serve as a radiogenomic biomarker to assess treatment response within early phases of treatment and allow for a personalized treatment plan.

BackgroundTakayasu arteritis (TAK) is a chronic systemic vasculitis that mainly affects the aorta and its major branches. This chronic relapsing disease is relevant to significant morbidity and treatment remains challenging [1]. Early identification of refractory TAK is helpful to improve the long-term prognosis of the disease. In recent years, platelets have been recognized as important markers for various types of diseases [2]. Platelet counts may indicate the activity of autoimmune disease as well as responsiveness to anti-inflammatory therapy and presence of various comorbidities [3]. Multiple studies have demonstrated that platelet count of TAK patients was significantly increased, especially in the active phase, which was significantly higher than that in the inactive phase [4-8].ObjectivesPlatelets have been recognized as important markers for various types of diseases. The aim of our study was to investigate whether platelet count could be the risk factor of refractory Takayasu arteritis (TAK).MethodsIn this retrospective study, 57 patients were divided into groups with or without refractory TAK. We compared the clinical manifestations, laboratory parameters, and medication between the two groups. The logistic regression analysis was used to identify the risk factors of refractory TAK.ResultsAmong the 57 patients, 18 cases (31.6%) were considered to have refractory TAK within 1 year of initiation of medication in our hospital. Refractory TAK patients had higher level of platelet (PLT) than non-refractory TAK patients (305.5 vs. 272.0, 109/L, P=0.043). PLT was positively correlated with ESR (r=0.502, p<0.001), hs-CRP (r=0.529, p<0.001), IgA (r=0.322, p=0.016), IgG (r=0.419, p=0.001), IgM (r=0.343, p=0.010), C3 (r=0.554, p<0.001). For PLT, the area under the ROC curve was 0.668 (95% CI 0.515 to 0.822, p=0.043) and the best cut-off value was 296.5×109/L. The level of PLT greater than 296.5×109/L was found to be statistically related to refractory TAK (OR [95%CI] 4.000 [1.233-12.974], p=0.021).ConclusionClinicians should pay close attention to platelet levels in patients with TAK. Background: Millions of individuals worldwide suffer from septic shock and sepsis each year, with fatalities reaching one in four and often higher. This work was conducted to evaluate the impact of Ascorbic acid, hydrocortisone, and thiamine treatment on the 28th day mortality rate and the outcomes (changes in SOFA score, incidence of organ dysfunction, alterations in serum procalcitonin level, changes in serum lactate level and total dose of vasopressor therapy. Methods: This prospective randomised controlled double-blinded work was performed on 80 individuals ranging in age from 18 to 65 years old and presented with sepsis who were diagnosed based on the SSC 2016 and The SSC Bundle 2018 Update. The participants were divided into two groups at random. Group I (placebo): The conventional approach was used to treat those who were in this group. The participants were given 50 ml of normal saline intravenously within 30 minutes, 5 ml intravenously within 6 hours, and 10 ml intravenously within 12 hours. Group II: the participants had standard sepsis treatment as well as a combination of ascorbic acid 1.5 gm diluted in 50 ml of normal saline IV within 30 minutes of 6 hours and thiamine 200 mg diluted in 10 ml of normal saline IV over a 12-hour period. If the admission term was shorter than 4 days, this combination treatment was provided until the time of release for a total of 4 days. Results: No statistically substantial variation was existed among both groups as regard incidence of 28th day mortality and incidence of organ dysfunction. At day one, the comparison of the mean values of mean arterial pressure (MAP) and cardiac index was comparable between the two groups. At day 2,3 and 4, the mean values of MAP were greater in group II in contrast to group I while cardiac index was greater in group I compared to group II. The mean value (± SD) of the total dose of noradrenaline was 824.1 ± 296.8 mg in group I, and 591.9 ± 343.2 mg in group II, with statistically substantial variation among the two groups (P<0.001) Conclusions: The combined use of ascorbic acid, thiamine, and hydrocortisone has no impact on the 28th day mortality and organ dysfunction among individuals with septic shock and sepsis but has a good impact on haemodynamic parameters and inflammatory markers during the period of drug administration.

AbstractThe new mineral joanneumite was found at Pabellón de Pica Mountain, Iquique Province, Tarapacá Region, Chile, where it occurs as violet microcrystalline aggregates up to 2 mm in size in small cracks in a gabbroic rock, which is covered by a guano deposit. Associated minerals are salammoniac, dittmarite, möhnite and gypsum. Joanneumite is non-fluorescent and the Mohs hardness is 1. The calculated density is 2.020 g cm–3. The infrared spectrum of joanneumite shows the frequencies of NH3 and isocyanurate groups and the absence of absorptions of H2O molecules and OH– ions. The chemical composition (electron microprobe data, the hydrogen was calculated from the structural formula, wt.%) is C 20.33, N 31.11, O 28.34, Cu 17.27, Zn 0.24, H 2.82, total 100.11. The empirical formula is Cu0.96Zn0.01N7.84C5.98O6.25H9.96 and the idealized formula is CuN8C6O6H10 with the structural formula Cu(C3N3O3H2)2(NH3)2. Due to the lack of suitable single crystals the synthetic analogue of joanneumite was prepared for the single-crystal structure refinement. The crystal structure was solved and refined to R = 0.025 based upon 1166 unique reflections with I > 2σ (I). Joanneumite is triclinic, space group P1̄, a = 4.982(1), b = 6.896(1), c = 9.115(2) Å, α = 90.53(3), β = 97.85(3), γ = 110.08(3)°, V = 290.8(1) Å3, Z = 1 obtained from single-crystal data at 100 K, which are in good agreement with cell parameters from powder diffraction data of joanneumite at 293 K: a = 5.042(1), b = 6.997(1), c = 9.099(2) Å, α = 90.05(3), β = 98.11(2), γ = 110.95(3)° and V = 296.3(1) Å3. The eight strongest lines of the powder X-ray diffraction pattern are [d, Å (I,%) (hkl)] 6.52 (68) (010), 5.15 (47) (011), 4.66 (21) (100, 110), 4.35 (9) (1̄11), 3.29 (6) (1̄20), 3.22 (7) (1̄1̄1), 3.140 (100) (1̄21), 2.074 (7) (1̄32). The crystal structure of joanneumite is identical with the structure of synthetic bis(isocyanurato) diamminecopper(II).

Background: Recent studies have reported promising clinical results after osteochondral autograft transplantation (OAT) for the treatment of large osteochondral lesions of the talus (OLT). However, no study has yet compared clinical outcomes between primary and secondary OAT for large OLT. Purpose: To compare clinical outcomes among patients with large OLT who receive primary OAT versus those who receive secondary OAT after failure of marrow stimulation and to identify factors associated with clinical failure. Study Design: Cohort study; Level of evidence, 3. Methods: From 2005 to 2014, 46 patients with large OLT (≥150 mm2) underwent OAT: 18 underwent OAT as initial surgical management (primary OAT group), and 28 patients underwent secondary OAT after failure of previous arthroscopic marrow stimulation (secondary OAT group). In both groups, OAT procedures included arthroscopic inspection and debridement of concomitant soft tissue injuries. Clinical outcomes were assessed using pain visual analog scale (VAS), the Roles and Maudsley score, Foot and Ankle Outcome Scores (FAOS), and revisional surgery rates. Factors associated with clinical failures were evaluated using bivariate and logistic regression analyses. Survival outcomes were compared using Kaplan-Meier analysis. Results: Mean follow-up time was 6 years (range, 2-10.8 years). Mean lesion size was 194.9 mm2 (range, 151.7-296.3 mm2). There were no significant differences between groups in patient demographics and preoperative findings. Postoperative pain VAS, Roles and Maudsley score, FAOS, and revisional surgery rates were not significantly different at last follow-up. Prior marrow stimulation was not significantly associated with clinical failure on bivariate analysis. Lesion size greater than 225 mm2 on preoperative magnetic resonance imaging was significantly associated with clinical failure. Survival probabilities from Kaplan-Meier plots were not significantly different between the primary and secondary OAT groups ( P = .947). Conclusion: Clinical outcomes of patients with large OLT treated with secondary OAT after failed marrow stimulation were found to be comparable with those who were treated with primary OAT. These results may be helpful to orthopaedic surgeons deciding appropriate surgical options for patients with large OLT.

AbstractWe evaluated the concentrations of brain natriuretic peptide in the plasma as a marker of systolic ventricular function before and after maximal exercise in 15 surgically palliated patients with functionally univentricular hearts, with apparently good ventricular function. Of the patients, 6 with median age of 14.6 years, and a range from 12.5 to 17.9 years, had been palliated by construction of a total cavopulmonary connection, while the other 9 patients, with a median age of 32.1 years, and a range from 15.6 to 54.2 years, had undergone the classical Fontan procedure. We used 8 healthy individuals, with a median age of 13.9 years, and a range from 12.8 to 14.2 years, as a control group for the measurements of brain natriuretic peptide. The values of the peptide were significantly higher in those with the classical Fontan procedure, both before, when the median value was 131.8 nanogram per litre, with a range from 0.5 to 296.4, and after maximal exercise, when the median value was 108.1, with a range from 0.1 to 235.9. The comparable values in those with a total cavopulmonary connection were a median of 12.8, and a range from 0.5 to 39.1 before, and a median of 9.7, with a range from 2.7 to 26.2 after maximal exercise. The median value for the control group was 13.1, with a range from 2.6 to 38.7 before exercise (p = 0.016), and a median of 24.1, with a range from 5.8 to 66.7 after maximal exercise (p = 0.03), respectively. In the control subjects, the level of the peptide increased by a median of 9.7 nanograms per litre, with a range from 1.2 to 28.0 after maximal exercise (p = 0.008). The level was unchanged after maximal exercise in those with classical Fontan procedures and total cavopulmonary connections, with a difference between levels before and after exercise of a median of 5.9 nanogram per litre, and a range from −23.7 to 31.0 (p = 0.96), and a median of −1.0 nanogram per litre, with a range from −12.0 to 3.9 (p > 0.99), respectively. We conclude that maximal exercise did not increase the level of brain natriuretic peptide level in those patients with the classical Fontan procedure, nor those with a total cavopulmonary connection, findings which may indicate that systolic ventricular dysfunction is not the major cause of the decreased working capacity observed in patients with well functioning palliated functionally univentricular hearts.

Black spot, also known as Ascochyta blight, is the most important disease on field pea (Pisum sativum). It is caused by a complex of pathogens, the most important of which in Australia include Didymella pinodes, Phoma pinodella, and P. koolunga. The relative proportions of these and other component pathogens of the complex fluctuate widely across time and geographic locations in Australia, limiting the ability of breeders to develop varieties with effective resistance to black spot. To address this, 40 field pea genotypes were tested under controlled environment conditions for their individual stem and leaf responses against these three pathogens. Disease severity was calculated as area under disease progress curve (AUDPC), and subsequently converted to mean rank (MR). The overall rank (OR) for each pathogen was used to compare response of genotypes under inoculation with each pathogen. The expressions of host resistance across the field pea genotypes were largely dependent upon the individual test pathogen and whether the test was on stem or leaf. Overall, P. koolunga caused most severe stem disease; significantly more severe than either D. pinodes or P. pinodella. This is the first report of the host resistance identified in field pea to P. koolunga; the five genotypes showing highest resistance on stem, viz. 05P778-BSR-701, ATC 5338, ATC 5345, Dundale, and ATC 866, had AUDPC MR values <250.4, while the AUDPC MR values of the 19 genotypes showing the best resistance on leaf was less than 296.8. Two genotypes, ATC 866 and Dundale, showed resistance against P. koolunga on both stem and leaf. Against D. pinodes, the four and 16 most resistant genotypes on stem and leaf had AUDPC MR values <111.2 and <136.6, respectively, with four genotypes showing resistance on both stem and leaf including 05P770-BSR-705, Austrian Winter Pea, 06P822-(F5)-BSR-6, and 98107-62E. Against P. pinodella, four and eight genotypes showing the best resistance on stem and leaf had AUDPC MR values <81.3 and <221.9, respectively; three genotypes, viz. 98107-62E, Dundale, and Austrian Winter Pea showed combined resistance on stem and leaf. A few genotypes identified with resistance against two major pathogens of the complex will be of particular significance to breeding programs. These findings explain why field pea varieties arising from breeding programs in Australia fail to display the level or consistency of resistance required against black spot and why there needs to be a wider focus than D. pinodes in breeding programs.

Sweet basil (Ocimum basilicum L.) is an annual aromatic and medicinal plant in the Lamiaceae that is originally native to India but is grown in warm regions all over the world. It is a popular culinary herb used fresh and dried, and is used in traditional folk medicine. In the Czech Republic, sweet basil is grown commercially in South Moravia or by home gardeners as a potted plant. In 2012, severe downy mildew was observed in a field of basil plants (cv. Dark Green) at the Crop Research Institute (CRI) in Olomouc, Czech Republic. Infected leaves each exhibited large, interveinal, chlorotic lesions, and violet-gray, fuzzy growth on the lower leaf surface. Within a few days, lesions turned necrotic and severely infected leaves dropped prematurely. Microscopic observations revealed hyaline conidiophores typical of Peronospora Corda, emerging from stomata. Conidiophores (n = 100) were usually 239.9 to 296.5 × 8.7 to 10.6 μm, straight, and were branched 4 or 5 times submonopodially at the upper ends. Ultimate branchlets (n = 100) were slightly curved and obtuse, with the longer branchlets usually 17.8 to 22.7 μm and the shorter branchlets 10.0 to 12.9 μm, and each bearing a single conidium. Conidia (n = 100) were olive-brown, mostly ellipsoidal to subglobose, and typically 29.0 to 31.0 × 23.2 to 25.4 μm, with a length/width ratio of 1.2 to 1.3. Oospores were not observed. Based on these morphological characteristics, the pathogen was identified as Peronospora belbahrii Thines (5). The specimen was deposited in a local herbarium at the CRI in Olomouc, as voucher PB-1. Genomic DNA was extracted from conidia, and the internal transcribed spacer (ITS) region of ribosomal DNA (rDNA) amplified with primers DC-6 (1) and LR-0 (4). A sequence was deposited in the NCBI database (GenBank Accession No. KJ960193). A BLAST search of the NCBI database revealed 99% identity to the deposited ITS sequences of P. belbahrii from basil and other host species (EU863410, FJ394334-7, GQ390794, GQ390795, HM462241, HM462242, HM486901, HQ702191, HQ730979, KC756923, KF419289, and KF419290). P. belbahrii was first described by Thines et al. (5) as a pathogen of sweet basil and coleus (Solenostemon scutellarioides), but can also infect Agastache spp. (2). There are many reports indicating the pathogen is spreading throughout the world (5). In Europe, chronologically, basil downy mildew has been reported from Italy, France, Switzerland, Germany, Hungary, and Cyprus (2,3,5). Background COVID-19 has caused the worst international crisis since World War II. Italy was one of the countries most affected by both the pandemic and the related infodemic. The success of anti–COVID-19 strategies and future public health policies in Italy cannot separate itself from the containment of fake news and the divulgation of correct information. Objective The aim of this paper was to analyze the impact of COVID-19 on web interest in conspiracy hypotheses and risk perception of Italian web users. Methods Google Trends was used to monitor users’ web interest in specific topics, such as conspiracy hypotheses, vaccine side effects, and pollution and climate change. The keywords adopted to represent these topics were mined from Bufale.net—an Italian website specializing in detecting online hoaxes—and Google Trends suggestions (ie, related topics and related queries). Relative search volumes (RSVs) of the time-lapse periods of 2016-2020 (pre–COVID-19) and 2020-2021 (post–COVID-19) were compared through percentage difference (∆%) and the Welch t test (t). When data series were not stationary, other ad hoc criteria were used. The trend slopes were assessed through Sen slope (SS). The significance thresholds have been indicatively set at P=.05 and t=1.9. Results The COVID-19 pandemic drastically increased Italian netizens’ interest in conspiracies (∆% ∈ [60, 288], t ∈ [6, 12]). Web interest in conspiracy-related queries across Italian regions increased and became more homogeneous compared to the pre–COVID-19 period (average RSV=80±2.8, tmin=1.8, ∆min%=+12.4, min∆SD%=–25.8). In addition, a growing trend in web interest in the infodemic YouTube channel ByoBlu has been highlighted. Web interest in hoaxes has increased more than interest in antihoax services (t1=11.3 vs t2=4.5; Δ1%=+157.6 vs Δ2%=+84.7). Equivalently, web interest in vaccine side effects exceeded interest in pollution and climate change (SSvaccines=0.22, P<.001 vs SSpollution=0.05, P<.001; ∆%=+296.4). To date, a significant amount of fake news related to COVID-19 vaccines, unproven remedies, and origin has continued to circulate. In particular, the creation of SARS-CoV-2 in a Chinese laboratory constituted about 0.04% of the entire web interest in the pandemic. Conclusions COVID-19 has given a significant boost to web interest in conspiracy hypotheses and has made it more uniform across regions in Italy. The pandemic accelerated an already-growing trend in users’ interest toward some fake news sources, including the 500,000-subscriber YouTube channel ByoBlu, which was removed from the platform by YouTube for disinformation in March 2021. The risk perception related to COVID-19 vaccines has been so distorted that vaccine side effect–related queries outweighed those relating to pollution and climate change, which are much more urgent issues. Moreover, a large amount of fake news has circulated about COVID-19 vaccines, remedies, and origin. Based on these findings, it is recommended that the Italian authorities implement more effective infoveillance systems, and that communication by the mass media be less sensationalistic and more consistent with the available scientific evidence. In this context, Google Trends can be used to monitor users’ response to specific infodemiological countermeasures. Further research is needed to understand the psychological mechanisms that regulate risk perception.

Abstract Because of methodologic problems, platelet function in thrombocytopenic patients has never been adequately studied, and therefore individual differences in bleeding risk are poorly understood. Whole blood flow cytometry analyzes the function of individual platelets, thereby enabling assessment of platelet function in severe thrombocytopenia (Michelson, A. Platelets, 2nd ed, Elsevier, 2007). In this study, platelet function was compared in 31 ITP patients, 14 CIT patients, and 16 healthy controls. Clinical bleeding was related to platelet function testing. The immature platelet fraction (IPF, or reticulated platelets) was measured in a Sysmex XE-2100. Platelet surface P-selectin and activated integrin αIIbβ3 (reported by monoclonal antibody PAC1) were measured by whole blood flow cytometry in the presence and absence of 0.5 μM ADP, 20 μM ADP, 1.5 μΜ TRAP, or 20 μM TRAP. Bleeding was quantified by a comprehensive score that allocates grades of 0 (no), 1 (minor) or 2 (marked) bleeding at 10 anatomic sites (Page, L.K. Br J Haematol 2007). Mean platelet volume (MPV) and IPF were higher in ITP than CIT, reflecting, as expected, a higher rate of platelet production (Table 1). Platelet surface P-selectin without added agonist (i.e. circulating activated platelets) was significantly higher in both ITP and CIT patients than in controls (Table 2). However, upregulation of platelet surface P-selectin and activated αIIbβ3 in response to ADP and TRAP (platelet ‘activatability’) was reduced in CIT patients compared with ITP patients and controls (Table 2). Stratification of bleeding scores by platelet count showed that CIT patients had more GI, urinary, and pulmonary bleeding at platelet counts &lt;20 × 109/L whereas ITP patients had more skin/oral bleeding (Table 1). In sum, the higher platelet surface P-selectin and activated αIIbβ3 in CIT and ITP patients than in controls is consistent with a role for circulating activated platelets in maintenance of vascular integrity in thrombocytopenia. However, platelet activation in response to ADP and TRAP is reduced in CIT compared with both ITP and controls, which may reflect the relative senescence (as evidenced by lower IPF) of CIT platelets and/or effects of chemotherapy or the underlying leukemia. These data demonstrate that bleeding in different thrombocytopenic conditions is not entirely explained by the thrombocytopenia per se. Reduced responses to ADP and TRAP in CIT patients compared with ITP patients may be clinically significant, given that, at equivalent degrees of thrombocytopenia, CIT patients had more significant bleeding (GI, urinary, pulmonary) than ITP patients. A 42-day feeding trial was conducted to evaluate the simple and adaptable feed ingredient mix for broiler chickens' production. The four formulated diets were used as a straight diet with the fish meal (FM), groundnut cake (GNC) and soya bean meal (SBM) having the varying ratio of inclusion as in diet 1 (1:4.5:2.5), diet 2 (1:3.5:3.5), diet 3 (1:2.5:4.5) and diet 4 (1:3:4), respectively. These were compared with two primary commercial feeds which were coded as AF and TF and used for starter and finisher phases. Three hundred, one-day old Marshall breed chicks were randomly assigned to six dietary treatments (50 birds/treatment; 5 replicate/treatment) in a Completely Randomized Design. Parameters such as feed intake, weight gain, carcass traits, and relative weight of the internal organs and economic analysis of the chicks' production were measured. The daily weight gain of the broiler chickens fed diet 3 was significantly (P<0.05) higher than those fed other diets, while the least daily feed intake was recorded in chicks fed diet 1. The broiler chickens fed with diet 5, a commercial diet consumed higher (P<0.05) quantity of feed, compared to those fed rest experimental diets; while the bird fed diet 1 recorded the least (P<0.05) quantity of feed consumed. The broiler chickens fed diet 3 had better (P<0.05) feed conversion to weight, when compared to those fed the rest diets. All the carcass traits and relative organs measured varied significantly (P<0.05) with diets except the dressed weight. The test diets 3 and 4 had better cost/weight gain (N239 and N228.9) compared to the two commercial diets 5 (N269.4) and 6 (N296.8). It is therefore concluded that the simple mix ratio with FM:GNC: SBM ratios of 1:2.5:4.5 and 1:3:4 could be measured using local measuring items such as congos, milk tin, and tablespoon by peri-urban and backyard poultry farmers. Un essai d'alimentation de 42 jours a été mené pour évaluer le mélange d'ingrédients alimentaires simple et adaptable pour la production de poulets de chair. Background: Our group recently reported, in a prospective multicentre phase 2 study of 31 patients with higher-risk (HR)-MDS or CMML receiving CPX-351 as first-line induction treatment 87% of overall responses (including 64.5% of CR/ CR according to ELN2017 criteria), and 93.5% of the patients could be bridged to transplant (GFM-CPX MDS trial, Peterlin et al. Lancet Haematol 2023). We had also previously reported, in AML, that the soluble forms of FLT3 ligand (FL) and IL6 could be prognostic factors of refractoriness to chemotherapy and overall survival (OS) (Peterlin et al. Cancer Medicine, 2021). A secondary objective of the GFM-CPX MDS trial was to assess the prognostic value of these two cytokines in HR-MDS/CMML receiving CPX-351. Methods: All patients included in the GFM-CPX MDS trial (clinicaltrials.gov#NCT04273802), induction chemotherapy with CPX-351 (44 mg Daunorubicin/100 mg Aracytine/m 2 on days 1, 3, and 5) also consented for the present biological FLAM-MDS sub-study. Using a new cytokine multiplex assay from Milliplex (MILLIPLEX®), FL levels (expressed in pg/mL) were evaluated on Day (D) 1, D8, D15 and D22 of the first cycle of induction; IL-6 (expressed in pg/mL) and CRP (known to be correlated with IL6, expressed in mg/L, normal value 0-5) levels were evaluated on D1 and D22 only. Patients who failed to achieve response (CR or CRi or MLFS, ELN2017 criteria) after the first induction cycle could receive a second one. The impact on CR/CRi rate after one cycle of CPX-351, OS and EFS of FL, IL-6 and CRP dosages and their kinetic profile during this first cycle were studied. Results: Dosages could be performed in 28 of the 31 patients included ( Table 1). Overall response (CR+CRi+MLFS) after cycle 1 was achieved in 24 (87%, MLFS n=7, CR+CRi n=17) patients; 3/4 non-responders received a second induction (1 achieved CR). Allograft was performed in 26/28 (93%). With a median follow-up of 18 months for surviving patients, 1y and 18-month OS were 79% and 75%, respectively. 1y and 18-month EFS were both 76%, respectively. Achieving CR/CRi versus MLFS/non-response was associated with better 1-y EFS (71% (52-96) vs 27% (10-72), p=0.009) but not 1-y OS (82% (66-100) vs 73% (51-100), p=0.8). Median (range) FL levels were 4.5 [0- 33.5] on D1, 74.1 [4.1-296.6] at D8, 326.6 [47.4-678] at D15 and 381.3 [4.4-800.1] at D22. As in AML (Peterlin et al, 2021), patients could be categorized according to three FLc kinetic profiles: i) sustained increase from D 1 to 22 (FLI group n=17, 61% of the cohort), ii) increase from D 1 to 15, then decrease on D 22 (FLD group, n=9, 32% of the cohort) and iii) persistent low levels (&lt;100 pg/mL from D 1 to 22, FLL group, n=2, 7% of the cohort). Median (range) IL6 levels were 11 (0-587.8) on D1 and 179.8 (0-531) on D22. Increase of IL6 levels between D1 and D22 was observed in 19 patients (68%) and decrease in 7, while 2 patients had no IL6 detectable at either times. Finally, median (range) CRP levels were 2.3 (0-108) on D1 and 40.5 (7.2-323) on D22. Increase of CRP level was observed in most patients between D1 and D22 (n=25, 96%). There was no correlation between IL6 and CRP levels on D1 (p=0.64) or D22 (p=0.48). CR/CR rates, 1-y OS and EFS were not correlated with FL levels (&lt; vs &gt; median) at the 4 time points considered. However, a FLD kinetic profile was significantly associated with higher CR/CRi rates (100% vs FLI 47% vs FLL 50%, p=0.002) and 1-y EFS (78% (55-100) vs FLI 47% (28-78) vs FLL: 0%, p=0.003), Figure 1). 1-y OS was similar between the 3 FL kinetic profile groups (FLD: 89% (71-100) vs FLI 76% (59-100) vs FLL 50% (13-100), p=0.7)). In multivariate analysis, taking into account age, gender, R-IPSS and the FL kinetic profile during cycle 1 of CPX-351, a FLD kinetic profile was the only factor associated with higher CR/CRi rate (OR: 24.2, 95%CI: 2.4-3278, p=0.004) and better EFS (OR: 21.91; 95%CI: 2.47-194.77, p=0.006). Levels and kinetic profiles of IL6 and CRP had no impact on CR/CRi rates, OS and EFS. Conclusion: As in AML, FL kinetic profile predicts response and survival in HR-MDS/CMML. A FLD kinetic profile was the only factor independently associated with higher CR/CRi rates and better EFS. These results have to be confirmed on larger cohorts. Other treatment approaches may be of interest in patient with a non FLD profile.
