Магістерська дисертація викладена на 83 сторінках, літературних джерел 37, серед них 4 іноземних; рис. 2, табл. 11. Актуальність теми. Ревматоїдний артрит – є одним з найпоширеніших запальних хвороб суглобів, що завдає значну як фізичну, психічну та соціально-економічну шкоду хворим. На сьогоднішній день хвороби ревматоїдного походження залишаються найбільш поширеними патологіями в світі, в Україні зокрема. З кожним роком з’являється велика кількість сучасних засобів, які доцільно включати до програми фізичної терапії. Все викладене вище визначає актуальність та необхідність подальшого вивчення данного питання у відновленні пацієнтів при ревматоїдному артриті, що і стало підставою для виконання цієї магістерської дисертації. Зв’язок роботи з науковими програмами, планами, темами. Магістерська дисертація виконана відповідно до наукової тематики кафедри біобезпеки і здоровя людини НТУУ «КПІ ім. Сікорського». Мета роботи: розробити програми фізичної терапії при ревматойдному артриті та оцінити її ефективність. Завдання: за даними спеціальної літератури вивчити сучасний стан і напрямки вдосконалення програми фізичної терапії при ревматойдному артриті; дослідити функціональний стан суглобів; розробити програму фізичної терапії при ревматойдному артриті та перевірити її ефективність. Об’єкт дослідження –хворі на ревматойдний артрит. Предмет дослідження – фізична реабілітація хворих на ревматойдний артрит. Методи дослідження: аналіз спеціальної науково-методичної літератури та інформаційних джерел; клінічні методи дослідження (контент-аналіз історій хвороби, огляд); візуально-аналогова шкала болю (VAS); інструментальні методи дослідження та методи математичної статистики. Наукова новизна одержаних результатів: вивчені функціональні особливості суглобів у хворих на ревматойдний артрит, що відкрило можливості для розробки програми фізичної терапії; визначені особливості змісту та спрямованості програми фізичної терапії, підібрані засоби відновлення з урахуванням ступеня функціональних порушень. Практичне значення отриманих результатів. Розроблена програма фізичної терапії при ревматойдному артриті, дозволила підвищити ефективність відновного лікування. Дані магістерської дисертації можуть бути використанні в роботі медичних закладів, реабілітаційних центрах, санаторіях та під час навчального процесу у вищих навчальних закладах з фізичного виховання і спорту, кафедрах фізичної терапії та ерготерапії. За результатами дослідження було опубліковано: 1 стаття в Международной научной интернет-конференции «Тенденции и перспективы развития науки и образования в условиях глобализации»; 1 тезиси в I Международная научно-практическая конференция “PRIORITY DIRECTIONS OF SCIENCE AND TECHNOLOGY DEVELOPMENT”, 27-29 сентября 2020 года Киев, Украина.
The master's dissertation is presented on 83 pages, literary sources 37, among them 4 foreign; Fig. 2, table. 11, Annex 1. Actuality of theme. Rheumatoid arthritis is one of the most common inflammatory diseases of the joints, which causes significant physical, mental and socio-economic damage to patients. Today, diseases of rheumatoid origin remain the most common pathologies in the world, in Ukraine in particular. Every year there are a large number of modern tools that should be included in the physical therapy program. All of the above determines the relevance and need for further study of this issue in the recovery of patients with rheumatoid arthritis, which became the basis for this master's thesis. Connection of work with scientific programs, plans, topics. The master's dissertation is made in accordance with the scientific theme of the Department of Biosafety and Human Health NTUU "KPI. Sikorsky ». Purpose: to develop physical therapy programs for rheumatoid arthritis and evaluate its effectiveness. Objectives: according to the special literature to study the current state and areas of improvement of physical therapy for rheumatoid arthritis; to investigate the functional state of the joints; develop a program of physical therapy for rheumatoid arthritis and test its effectiveness. The subject of the study is patients with rheumatoid arthritis. The subject of research - physical rehabilitation of patients with rheumatoid arthritis. Research methods: analysis of special scientific and methodical literature and information sources; clinical research methods (content analysis of case histories, review); visual-analog pain scale (VAS); instrumental research methods and methods of mathematical statistics. Scientific novelty of the obtained results: the functional features of the joints in patients with rheumatoid arthritis were studied, which opened opportunities for the development of a physical therapy program; the peculiarities of the content and direction of the physical therapy program are determined, the means of recovery are selected taking into account the degree of functional disorders. The practical significance of the results. Developed a program of physical therapy for rheumatoid arthritis, increased the effectiveness of rehabilitation treatment. The data of the master's dissertation can be used in the work of medical institutions, rehabilitation centers, sanatoriums and during the educational process in higher educational institutions of physical education and sports, departments of physical therapy and occupational therapy. Розглянуто варіант пошуку ефективних методів на суб’єктному та апаратно-програмному рівні комунікації в системі «Розробник ресурсу – Комп’ютерний засіб – Мережа – Користувач» на основі адаптації складових системи передачі мультимедійного контенту засобами мережних технологій. Для дослідження обраний наскрізний канал передачі аудіо інформації від оцифрування, сегментації цифрового масиву, вибору формату даних та файлу оброки з використанням прямих та зворотних швидких алгоритмів на основі системи неортогональних базисних функцій. Запропоновано здійснювати адаптацію на різних рівнях передачі інформації: на сервері мультимедійного контенту; на проміжних програмованих мережних вузлах; на терміналах користувачів з урахуванням сервісних запитів та різного апаратно-програмного забезпечення.
The option of finding effective methods at the subject and hardware-software level of communication in the system "Resource Developer - Computer - Network - User" based on the adaptation of the components of the system of multimedia content transmission by means of network technologies. The end-to-end channel of audio information transmission from digitization, digital array segmentation, data format selection and installment file using direct and inverse fast algorithms based on the system of non-orthogonal basic functions is chosen for the research. It is offered to carry out adaptation at different levels of information transfer: on the server of multimedia content; on intermediate programmable network nodes; on user terminals, taking into account service requests and various hardware and software.

Cette thèse traite des rapports noués entre le livre et la photographie en France au cours du dernier tiers du XIXe siècle. Elle dégage les enjeux recouverts par cette association pour les acteurs collaborant à sa réalisation, à la croisée desquels a été progressivement inventé un objet inédit : un livre dont le discours s’élabore à partir de la combinaison de textes et de photographies entre ses plats de couverture au moins, mais aussi sur ses pages si l’envie ou le besoin le commande. Pour cela, elle circonscrit les contours et les tendances de la production moyenne entre 1867 et 1897, en construisant un corpus qui en reflète l’étendue et les ambitions. De type systémique, son analyse démontre que le livre n’a été ni le berceau, ni le foyer, non plus que le vecteur de diffusion principal des images photographiques en France au XIXe siècle. En révélant l’extrême diversité de la production, elle prouve par contre que le livre a constitué un laboratoire d’expérimentations des modalités de l’association des textes et des images photographiques, dans le but de formaliser un discours pour le pérenniser. Par ce biais, les auteurs et les éditeurs ont progressivement défini les spécificités de la photographie dans le champ des moyens disponibles pour l’illustration. Permettant notamment aux imprimeurs-éditeurs de réaffirmer leur position dominante sur un monde de l’édition en mutation, les processus de cette appropriation se distinguent toutefois par la très faible implication de la communauté photographique. Dès lors, cette thèse propose une périodisation des liens des photographes au livre, lorsque celui-ci est utilisé comme un levier dans leurs réclamations statutaires
This thesis is about the relationship between books and photography in France during the last three decades of the 19th century. It reveals what was at stake in this specific association for the diverse actors involved in its creation. Articulating multiple issues, a new object was gradually invented: the photographically illustrated book, that is a book discursively produced through combining texts and photographic images between its covers, if not on the very pages themselves. To this end, the thesis charts the boundaries and tendencies of the average production of photographically illustrated books between 1867 and 1897, building a corpus that reflects its extent and its ambitions. Through a systemic analysis, it demonstrates that books were neither the breeding grounds nor the main sites nor the main vehicles for the dissemination of photographic images in 19th century France. By showing the extreme diversity of the production, it argues that books have constituted an experimental ground where modalities of texts’ and photographic images’ associations were tested. Through this process, authors and editors progressively defined what would become the specific characteristics of photographic images as means available for illustration. Allowing publishers and printers to reaffirm their dominant position in the world of publishing, the processes of this appropriation is particular because of the very low involvement of the photographic community itself. Therefore, the thesis proposes a periodization of the links of photographers to the book, analysing its role in the statutory claims they made through time

Esse trabalho vem no seguimento do projeto “Sence4me – Sistema de monitorização e controlo de desconforto em deficientes físicos motores, acamados e pacientes cirúrgicos”, representando mais uma etapa em todo o trabalho realizado ao longo dos anos. Numa primeira fase, realizou-se um estudo sobre úlceras de pressão. Este estudo visou, não só o conhecimento sobre o que é esta patologia mas também, as circunstâncias e principais locais onde este tipo de feridas é mais comum. Com isto, foi-nos permitido observar quais os comportamentos a obedecer para evitar o seu aparecimento. Da mesma forma e como a energia pneumática seria a base para todo o projeto, relembraram-se alguns conceitos e alguns elementos que dependem deste tipo energia para posteriormente a empregar. Juntamente com este estudo, partiu-se para a análise dos estudos anteriormente apresentados ao abrigo do projeto “Sence4me”, analisando o que de bom ele possui e o que seria desejável melhorar. Foi possível estabelecer o ponto de partida para o trabalho a desenvolver, aferir quais os materiais que se poderiam aproveitar e os que se teriam mudar em termos tecnológicos e, de uma forma mais específica, começar a projetar o colchão inteligente. Após estas fases mais preliminares, iniciou-se a fase de projeto do protótipo. Definiram-se os princípios bases a obedecer por forma a proceder à escolha de materiais que pudessem responder de forma afirmativa às premissas lançadas. No seu término conseguimos ter definidos todos os materiais, peças e instrumentos que necessitaríamos para obtenção de um projeto o mais próximo possível do pretendido. Por forma a dotar o protótipo de mais funções em relação aos projetos anteriores utilizou-se o programa informático Labview para elaborar um software que permitiria o controlo da pressão ao longo do tempo nas células pneumáticas do protótipo. Numa última fase, testou-se o protótipo em laboratório. Com estes testes pretendeu-se aferir da funcionalidade do software e da importância que algumas variáveis podem ter para o processo de enchimento e esvaziamento das células. A interpretação destes resultados possibilitou não só apresentar os resultados deste trabalho mas também possíveis melhorias a implementar em trabalhos futuros.
This work follows on from the project "Sence4me - System monitoring and control discomfort engines disabled, bedridden and surgical patients", representing another step in all the work done over the years. Initially, we carried out a study on pressure ulcers. This study aimed, not only the knowledge about this disease but it is also the circumstances and main places where this type of wound is most common. With this, we were allowed to observe which behaviours to conform to prevent its onset. Likewise, and as pneumatic power would be the basis for the entire project, recalled certain concepts and elements that make this form of energy to be able to use later in this paper. Along with this study, broke for the analysis of previously published studies under the project "Sence4me", analysing the good that he has and it would be desirable to improve. It was possible to establish the starting point for the work to develop, assess what materials could be enjoy and that would change in terms of technology and, in a more specific start designing the mattress intelligent. After these preliminary stages, it moved to the stage of prototype design. Defined the principles bases to obey in order to proceed to the choice of materials that could answer in the affirmative to the assumptions launched. At its end we managed to have all materials, parts and tools we would need to obtain a project as close to the desired. In order to provide more functions of the prototype compared to previous designs used the computer program to produce a Labview software which enables control of the pressure over time in cells pneumatic prototype. In the last phase, the prototype was tested in the laboratory. With these tests intended to verify the functionality of the software and the importance that some variables can have on the process of filling and emptying of the cells. Interpretation of these results has led not only to present the results of this work but also possible to implement improvements in future work.

Mycobacterium tuberculosis (Mtb), one of the most successful human pathogens in the world, is initially sensed in the lung by pathogen recognition receptors such as TLRs, localized in macrophages, dendritic cells, and monocytes recruited from the bloodstream. This recognition via TLRs initiates a signal transduction pathway in the host cell that culminates with the induction of cytokines and chemokines that are critical for initiation of the innate immune response and the subsequent activation of adaptive immune defence against the pathogen. TLR2, TLR4 and TLR9 have been implicated as the main TLRs in the recognition of Mtb. However, the specific role of these receptors during Mtb infection remains unclear despite several studies that have been done. These studies have shown conflicting results regarding the role of individual TLRs or even in their cooperative work. One of the most important and intriguing characteristics of tuberculosis (TB) is the wide spectrum of the disease. After infection the variety of possible outcomes is undetermined and seems to depend on the particular genetic interplay between host and pathogen. Individual characteristics of each one could influence in a positive or negative way the final result of the infection. The variability in the immune response induced by different strains of Mtb, the distinct interaction with innate immune cells and their heterogeneous responses are other related issues, which could also impact the pathogenesis of the infection. This work was focused on understanding the molecular bases that determine the heterogeneity of the immune response induced by different strains of Mtb and to provide new insights on how the Mtb/host interaction can impact the immune response and pathogenesis. To achieve this, we selected Beijing (Bj) strains, a group of conserved genotypes of Mtb strains, included in the East- Asian lineage which have been associated with multidrug resistance and have been suggested to be more virulent and to elicit non-protective immune responses.Initially we confirmed the wide heterogeneity of the immune response induced by a panel of Bj strains using an in vitro model. We also hypothesized that TLRs may play an important role in the process. Using Mtb Bj strains and a mice model of infection we showed a differential recognition by the host to strains that belong to the same phylogenetic family. Depending on the infecting strain, recognition can occur preferentially via either TLR2 or TLR4, with a differential impact on the progression and outcome of the infection. Moreover, we showed that TLR4 has a critical role in the recognition of certain strains, namely 02-171 Bj strain. Our analysis showed a strong dependency on TLR4 recognition, inducing a particularly robust profile of cytokines in vitro and in vivo, with a prominent induction of IFN-β, which was not observed in strains preferentially recognized by TLR2. Moreover, in the absence of TLR4 we observed a higher bacterial burden in the lungs of mice infected with the 02-171 Bj strain as well a notable decrease of almost all measured cytokines, supporting a protective role for TLR4. Subsequently, differential activation of TLR by Mtb Bj strains has impact in the induction of molecules related with microbicidal activity such as a NO and LRG-47. An important differential role of TLR4 was detected during activation of these microbicidal mechanisms in macrophages. Importantly, this differential recognition was observed also in strains that belong to other phylogenetic families. Altogether, data presented throughout this work provide relevant input to better understand the molecular basis of the host immune innate cells during infection with Mtb strains, particularly how the Mtb/host interaction can impact on the heterogeneity of pathogenesis of TB.
Mycobacterium tuberculosis (Mtb), um dos agentes patogénicos com maior impacto em humanos, é inicialmente detectado nos pulmões por receptores específicos, como TLRs, localizados em macrófagos, células dendríticas e monócitos recrutados da corrente sanguínea. O reconhecimento via TLRs inicia uma via de transdução de sinal intracelular na célula hospedeira que conduz a uma indução da produção de citocinas e quimiocinas, moléculas cruciais para iniciar a resposta imune inata e subsequente ativação da resposta imune adaptativa contra o agente infeccioso. Dentro dos TLRs, TLR2, TLR4 e TLR9 são moléculas chave no reconhecimento de Mtb. No entanto, o papel específico desses receptores durante a infecção por Mtb permanece incerto uma vez que estudos recentes têm presentado resultados contraditórios, quer sobre o papel individual destes TLRs quer sobre a sua ação conjunta durante a infecção. Uma das características mais importantes e mais intrigantes da tuberculose (TB) é o amplo espectro da doença. De facto, após a infecção a grande diversidade de presentação de TB parece estar ligado à interação específica entre o hospedeiro e Mtb, sendo que características individuais podem influenciar de forma positiva ou negativa o resultado final da infecção. A variabilidade na resposta imune induzida por diferentes estirpes de Mtb, a interação com diferentes células imunes inatas e as suas respostas heterogéneas são outros factos que podem influenciar o processo patogénico. O objetivo deste trabalho era estudar as bases moleculares que determinam a heterogeneidade da resposta imune induzida por diferentes estirpes de Mtb e, assim, elevar o nível de conhecimento sobre a interação Mtb/hospedeiro e o impacto que esta relação tem sobre a resposta imune e a patogénese. De modo a atingir este objetivo, foram selecionada estirpes Beijing (Bj), um grupo de genótipos conservados de estirpes de Mtb que pertencem à linhagem Este-Asiática e foram previamente associados a fenómenos de multirresistência, virulência aumentada e induzir respostas imunes não protetoras. Os resultados iniciais, utilizando um modelo in vitro de infeção, confirmaram a grande heterogeneidade da resposta imune induzida por diferentes estirpes Bj. Adicionalmente, colocamos a hipótese de que os TLRs poderiam despenhar um papel importante durante este processo. Utilizando um modelo animal de infeção, verificámos a existência de um reconhecimento diferencial pelo hospedeiro a estirpes que pertencem à mesma família filogenética. De facto, dependendo da estirpe o reconhecimento pode ocorrer preferencialmente via TLR2 ou TLR4 com um impacto diferente na progressão e resultado final da infeção. Adicionalmente, demonstrámos que TLR4 tem um papel crucial no reconhecimento de estirpes específicas, nomeadamente a estirpe Bj 02-171. A nossa análise revelou uma forte dependência no reconhecimento via TLR4, induzindo um perfil de produção de citocinas, tanto in vitro como in vivo, com particular relevo para a produção de IFN-β, um fenómeno não observado em infeções por estirpes preferencialmente reconhecidas por TLR2. Além disso, observámos, na ausência de TLR4, uma maior carga bacteriana nos pulmões de ratinhos infectados com a estirpe 02-171 Bj assim como uma diminuição acentuada dos níveis de citocinas medidos, sugerindo um papel protetor para TLR4 durante a infeção. A ativação diferencial de TLR por estirpes Mtb Bj também tem um impacto na indução de produção moléculas com atividade microbicida como NO e LRG-47. De facto, foi detectado um papel diferencial de TLR4 durante a ativação destes mecanismos microbicidas em macrófagos, facto também observado em estirpes que pertencem a outras famílias filogenéticas. Ao longo deste trabalho foram presentados dados importantes que contribuem para compreender melhor as bases moleculares da resposta imune inata a infeções com Mtb, particularmente no que diz respeito à interação Mtb/hospedeiro o seu impacto na heterogeneidade da patogénese de TB.

Despite significant advances in drug discovery and vaccination strategies during the last decades, infectious diseases are still a major health problem worldwide. Among these, infections by mycobacteria are particularly deadly, and Mycobacterium tuberculosis alone – the causative agent of tuberculosis – is estimated to infect a third of the world population and is responsible for 9 million new cases and 1.5 million deaths every year. Interestingly, most people do not develop active disease following infection, but instead are capable of controlling infection and remain in an asymptomatic state termed latent infection. Although how the immune system controls infection is unclear, it is clear that T cells are a key component of a successful response against mycobacteria, since infection with human immunodeficiency virus (HIV) greatly increases susceptibility to infection. Because T cells play such an important role in controlling mycobacterial infections, there is considerable interest in understanding which subsets are necessary for protection, which mechanisms are employed by these cells to limit bacterial growth, and whether such cells can be elicited by vaccination strategies. To clarify the role of T cells during immunity to tuberculosis, we addressed two aspects of T cell biology: how infection impacts the thymus, the organ where T cells differentiate; and how CD8+ T cells mediate protection during tuberculosis. Our lab had previously shown that mycobacteria could disseminate to the thymus and interfere with thymic function. However, it was unclear whether the immune system could control thymic infection and halt bacterial growth in this organ. Here we show that an immune response is recruited to the infected thymus and controls infection. This response involves the production of pro-inflammatory cytokines by antigen-specific CD4+ and CD8+ T cells, which stimulate infected cells and leads to control of bacterial replication. Interestingly, these T cells are not cells undergoing differentiation in the thymus, but rather are activated cells that recirculate from infected peripheral organs to the thymus to fight infection. This recruitment is associated with the production of the chemokines CXCL9 and CXCL10 in the infected thymus, and recirculating antigen-specific T cells that express CXCR3 and preferentially traffic to the thymus. In a second set of studies, we investigate how CD8+ T cells contribute to immunity against M. tuberculosis infection. Although CD8+ T cells are essential for optimal immunity during tuberculosis, how they exert protection is unclear. Here we describe the generation of a new mouse model, the first carrying T cells expressing a recombinant T cell receptor (TCR) specific for a M. tuberculosis antigen recognized by CD8+ T cells – TB10. By combining single cell sorting of antigen-specific cells with PCR amplification of their TCRs, we selected 4 TCR candidates specific for TB10. These TCRs were cloned into retroviral vectors and used in retrovirus-mediated stem cell gene transfer to generate retrogenic mice. These mice express CD8+ T cells that recognize and respond to their cognate peptide. More importantly, adoptive transfer of retrogenic cells provides protection against aerosol infection with M. tuberculosis. We then use these mice to investigate the mechanism by which CD8+ T cells mediate protection. By using retrogenic mice that are deficient in the production of interferon-γ or perforin, we show that protection mediated by CD8+ T cells requires cytokine production but is independent of their cytotoxic capacity. Collectively, the studies presented in this thesis clarify different aspects of T cell-mediated immunity to mycobacteria; they reinforce that the thymus is not an immune privileged organ and should be studied in the context of infection, and strengthen the notion that CD8+ T cells are protective after M. tuberculosis infection
Nas últimas décadas observamos incríveis avanços tanto no desenvolvimento de vacinas como de antibióticos. No entanto, as doenças infecciosas continuam a ser um problema de saúde global. Entre estas, as infecções por micobactérias são particularmente mortiferas, e é estimado que um terço da população mundial esteja infectada com Mycobacterium tuberculosis – a bacteria responsável pela tuberculose. Esta doença causa 9 milhões de novos casos todos os anos, resultando em cerca de 1.5 milhões de mortes anuais. No entanto, a maioria dos indivíduos infectados não desenvolve a doença, e permanence num estadio clinico assimptomatico designado por infecção latente. Apesar de ainda não ser claro como é que o sistema imunitário controla estas infecções, as células T são um componente essencial neste processo, já que infecção com o virus de imunodeficiencia humana (VIH) aumenta significativamente a susceptibilidade à infecção com micobactérias. Devido à importância das células T nestes casos, é importante clarificar que subconjuntos destas células são necessários para controlar a infecção, como é que essas células são protectoras, e se é possível gerá-las por intermédio de vacinas. Para melhor entender qual o papel das células T durante infecções por micobactérias, focamo-nos em dois pontos: qual o impacto destas infecções no timo, o orgão em que as células T se diferienciam; e qual o papel das células T CD8+ durante a tuberculose. Quando iniciamos estes estudos, era sabido que micobactérias são capazes de disseminar para o timo e interferir com a função deste orgão. No entanto não era claro se o timo consegue controlar estas infecções, e como o faz. Nos estudos aqui apresentados demonstramos que o timo controla a infecção, num processo dependente do recrutamento de uma resposta imunitária adquirida para este orgão. Esta resposta envolve a produção de citocinas pró-inflamatórias por parte de células T, tanto CD4+ como CD8+. Estas citocinas activam as células infectadas, resultando no controlo do crescimento bacteriano no timo. Apesar do timo ser o orgão em que as células T se diferenciam, os nossos estudos revelam que as células responsáveis pelo controlo bacteriano neste orgão não são células que se encontram neste orgão no processo de diferenciação, mas antes células T activadas que recirculam dos orgãos periféricos para o timo para controlar a infecção. Este processo depende da produção das quimoquinas CXCL9 e CXCL10 no timo infectado, e da expressão do receptor CXCR3 por parte das células que recirculam. Num conjunto de estudos diferentes debruçamo-nos sobre a contribuição das células T CD8+ durante a tuberculose. Apesar de ser aceite que estas células são importantes durante a infecção com M. tuberculosis, o mecanismo pelo qual elas protegem indivíduos infectados é ainda desconhecido. Para investigar esta questão criamos um novo modelo animal, o primeiro ratinho que expressa células T CD8+ específicas para um antigénio de M. tuberculosis, o antigénio TB10. Para tal, células específicas para TB10 foram isoladas individualmente por citometria de fluxo, os seus receptores de células T (TCR) amplificados por reacção em cadeia de polimerase (PCR) e sequenciados. Das sequências detectadas, 4 foram seleccionadas e clonadas em vectores retrovirais, que foram depois usados para gerar animais retrogénicos, via transferência adoptiva de células estaminais infectadas com os retrovirus. Estes animais expressam células T CD8+ que reconhecem o antigénio de interesse, e transferência adoptiva de células retrogénicas é suficiente para proteger significativamente animais de infecção aerogénica com M. tuberculosis. Para investigar qual o mecanismo responsável pela protecção observada, geramos animais retrogénicos incapazes de produzir interferão-γ ou perforina, e demonstramos que as células T CD8+ precisam de produzir citocinas, mas não de ser citotóxicas, para proteger animais contra infecção por tuberculose. Em conjunto os estudos apresentados nesta tese clarificam diferentes aspectos acerca da imunidade mediada por células T durante infecções com micobactérias: re-enforçam a ideia de que o timo não é um orgão imuno-priveligiado e deve ser estudado no contexto de doenças infecciosas; e que as células T CD8+ são protectoras no contexto de tuberculose.

Worldwide, Mycobacterium tuberculosis infections are responsible for approximately 2 million deaths each year and the control of tuberculosis remains one of the most important challenges in the fight against infectious disease. The struggle against this mycobacterial infection has been performed using the vaccine M. bovis Bacille Calmette-Guérin (BCG) and the administration of anti-mycobacterial drugs. However it is clear that vaccination has to be improved for the following reasons: i) the efficacy of BCG vaccination is not satisfactory, ranging from 0-80%, ii) there has been an increase of mycobacterial strains resistant to the most common drugs actually used. The development of new vaccines requires however that significant effort be put into the study of the host-parasite interactions in this infection. It is with this in mind that I have investigated the cellular response to the vaccine BCG. It is known that interferon (IFN)-γ, associated with the development of acquired immune responses mediated by T helper (Th)1 lymphocytes, plays an essential role in the control of infection by mycobacteria. IFN-γ-gene-deficient ( ifn2-/-) mice are highly susceptible to mycobacterial infection and exhibit enhanced accumulation of activated effector T cells and neutrophils within granulomatous lesions. Nevertheless these T cells are not associated with control of bacterial growth and are associated with the development of pathology. Recently a new phenotype of T lymphocytes (Th17), which is associated with the development of inflammatory type of responses and pathology, has been described. Th17 cells produce mainly interleukin (IL)- 17, IL-6 and tumour necrosis factor (TNF). IL-17 can act on macrophages inducing the expression of TNF, granulocyte macrophage colony stimulatin factor (GM-CSF) and IL-8, resulting in the maturation and recruitment of neutrophils. The dynamics and regulation of these cells following infection by bacteria are poorly understood. I have therefore examined the kinetics of Th1 and Th17 cellular response following infection by BCG. My data show that in C57BL/6 mice infected with BCG, both IFN-γ and IL-17-producing T cells develop early after infection, however only IFN-γ-producing T cells expand as infection progresses. In contrast, in ifn2-/- mice infected with BCG the number of IL-17-producing T cells increases dramatically. These observations point to a regulatory role of IFN-γ on the differentiation of Th17 cells. To elucidate the mechanism underlying this observation, CD4 T cells from OT II transgenic mice were cultured in the presence of bone marrow derived dendritic cells (BMDC) infected with BCG and the cognate antigen. I observed that in these conditions a significant number of Th17 cells differentiate and that the differentiation of these cells can be inhibited by the presence of IFN-γ. Furthermore IFN-γ, by increasing the production of IL-12 and decreasing the synthesis of IL-23 (a cytokine important for the differentiation of Th17 cells), modulates the differentiation of Th1 and Th17 cells. These data reinforce the necessity to perform further studies to understand the dynamics of Th1/Th17 cells following infection by mycobacteria as well as the regulatory role of IFN-γ. Although IL-17 is induced soon after mycobacterial infection, the effect of IL-17 on the bactericidal activity of macrophages was not known. To address this question bone marrow derived macrophages (BMDM) were infected with M. tuberculosis or M. bovis BCG, and treated with IFN-γ, IL-17 or both. I have shown that IL-17 increases the susceptibility of macrophages to the intracellular infection with mycobacteria and that it inhibits the macrophage bactericidal activity induced by IFN-γ. This effect of IL-17 is not dependent on the macrophage effector mechanisms related with nitric oxide production or phagosome maturation. Surprisingly, IL-17 alone but not in the presence of IFN-γ, increases macrophage survival which is related to an increased expression of IL-6, a cytokine known to induce cell survival. These data illustrate the potential for IL-17 to have an impact on disease progression and support the necessity to elucidate the role of IL-17 on mycobacterially-infected macrophages. The differentiation and persistence of Th17 cells has been considered responsible for several pathologies, including those associated with autoimmune and chronic infectious diseases. This is of interest as a well-known phenomenon of mycobacterial infection is the Koch reaction. This reaction occurs in mycobacterially-infected hosts re-exposed to mycobacterial antigens and is characterized by a massive influx of neutrophils and damaging pathologic consequences. As a result of the low level of protection afforded by BCG vaccination, and because protection is not long-lived, it has been proposed that re-vaccination should be considered. In view of the potential for Koch reaction in response to re-vaccination it is therefore essential that we have a better understanding of the molecular mechanisms leading to the Koch reaction; specifically whether Th17 cells play a role. To address this question, C57BL/6 mice were infected with M. tuberculosis. Fifteen days after infection mice were vaccinated sub-cutaneously once, twice or three-times with BCG, with an interval of fifteen days between each vaccination. Importantly I determined that the number of Th17 cells increases with the number of M. bovis BCG revaccinations. Since the presence of Th17 cells has been associated with the induction of cytokines and chemokines responsible for the differentiation and recruitment of neutrophils, I postulate that neutrophils are responsible for the pathology and tissue damage associated to the Koch reaction. In summary, my work has identified potential new mechanisms into the complex network of immune responses, occurring in response to infection and vaccination with mycobacteria. Specifically, I have demonstrated a role for IFN-γ in the regulation of Th17 cells differentiation, and the relevance of IL-23 in the sustained differentiation of Th17 cells. I have also shown that IL- 17 inhibits the bactericidal activity of infected macrophages. This has the potential to be important and of biological relevance but the role of this molecule in bacterial control must be clarified. Finally, my data support a role for IL-17 in the pathology associated to the Koch reaction and this is important for the understanding of this unique reaction. Altogether my results point to the fact that the dynamics and effector function of Th17 cells have to be taken into consideration for a better understanding of the mechanisms leading to resistance and/or pathology following infection by M. tuberculosis, as well as for the design of new vaccines.
As infecções por Mycobacterium tuberculosis constituem um grave problema de saúde pública a nível mundial, sendo responsáveis por cerca de 2 milhões de mortes por ano. O controlo da infecção por M. tuberculosis tem vindo a ser feito pelo recurso à vacinação com M. bovis Bacilo Calmette-Guérin (BCG) e pela administração de fármacos anti-micobacterianos. Contudo é cada vez mais evidente que o desenvolvimento de novas vacinas é fundamental tendo em consideração: i) que a eficácia da vacinação por BCG é muito variável, com um grau de protecção que varia entre 0-80%, dependendo de factores genéticos das populações e também de factores ambientais, ii) que se tem assistido nas últimas décadas a um aparecimento cada vez mais acentuado de estirpes de M. tuberculosis multi-resistentes aos fármacos disponíveis. Neste contexto, um conhecimento mais profundo das interacções parasita-hospedeiro é necessário para que se possam desenvolver estratégias preventivas e terapêuticas mais eficazes. Tendo por base este conhecimento, fui estudar a resposta celular à vacinação por BCG. A produção de interferão (IFN)-γ, associada ao desenvolvimento de respostas imunes adquiridas do tipo de linfócitos T de ajuda (Th)1, desempenha um papel fundamental no controlo das infecções por micobactérias, pelo seu papel na potenciação dos mecanismos efectores dos macrófagos e na formação de granulomas. A formação de granulomas também ocorre em ratinhos deficientes em IFN-γ infectados por micobactérias. No entanto, estas estruturas exibem uma acumulação de neutrófilos e células T efectoras. Apesar da presença destas células no local da infecção, não se verifica o controlo do crescimento bacteriano, observando-se o desenvolvimento de patologia. Recentemente, foi descrito um novo fenótipo de linfócitos T designado por Th17. Estas células produzem essencialmente interleucina (IL)-17, IL-6 e factor de necrose tumoral (TNF) e estão associadas ao desenvolvimento de respostas inflamatórias. A acção da IL-17 sobre os macrófagos induz a expressão de TNF, factor estimulador de colónias de granulócitos e macrófagos (GM-CSF) e IL-8. Contudo, a dinâmica, regulação e função efectora destas células após uma infecção por micobactérias não são conhecidas. Neste sentido, decidimos estudar a dinâmica das células Th1 e Th17 após uma infecção por BCG. Os resultados mostram que a infecção de ratinhos selvagens por BCG induz a diferenciação de células T produtoras de IFN-γ e de células T produtoras de IL-17. No entanto, apenas o número de células T produtoras de IFN-γ expande durante o curso da infecção. Pelo contrário, a infecção por BCG em ratinhos deficientes em IFN-γ resulta numa expansão do número das células T produtoras de IL-17. Esta observação sugere que o IFN-γ regula negativamente a diferenciação de células Th17. Para elucidar o mecanismo por detrás desta observação, células T CD4+ de ratinhos transgénicos (OT II) foram cultivadas com células dendríticas derivadas da medula óssea (BMDC) infectadas com BCG e antigénio. Nestas condições, observou-se a diferenciação de um elevado número de células Th17, sendo esta diferenciação inibida pela presença de IFN- γ. Observou-se, ainda, que o IFN-γ, aumentando a produção de IL-12 e inibindo a produção da IL-23 (uma citocina importante para a diferenciação de células Th17), modela a diferenciação de células Th1 e Th17. Os meus resultados mostram, pela primeira vez, que a IL-17 pode estar envolvida na patologia associada a infecções por micobactérias e que as células Th17 são reguladas pelo IFN-γ. Estes resultados realçam a importância de estudar a dinâmica das células Th1/Th17, assim como o papel regulador do IFN-γ. No decurso das infecções experimentais por micobactérias a IL-17 é induzida numa fase precoce, no entanto, o seu papel na actividade bactericida dos macrófagos não é conhecido. Para avaliar o efeito da IL-17 na actividade dos fagócitos, macrófagos derivados da medula óssea (BMDM) foram infectados com M. tuberculosis ou M. bovis BCG e tratados com IFN-γ ou IL-17. Os resultados mostram que a IL-17 aumenta a susceptibilidade dos macrófagos a infecções por micobactérias e que, adicionalmente, inibe a acção bactericida induzida pelo IFN-γ. Observei ainda que estes efeitos não são dependentes da produção de óxido nítrico nem da maturação do fagosoma, dois mecanismos efectores do macrófago descritos como intervindo no controlo das infecções por micobactérias. Verificou-se, ainda, que a IL-17 aumentava a sobrevivência in vitro dos macrófagos infectados. Dada a capacidade da IL-17 induzir nos macrófagos um aumento da expressão de IL-6, uma citoquina conhecida pelo seu efeito na sobrevivência de várias populações de células, podemos especular que o aumento da sobrevida dos macrófagos induzido pela IL-17 estará relacionado com o aumento da expressão de IL-6. Estudos adicionais serão necessários para elucidar o papel da IL-17 na progressão da infecção por micobactérias em macrófagos. A diferenciação e persistência de células Th17 estão associadas à patologia observada em algumas doenças auto-imunes e infecções crónicas. A exposição a antigénios micobacterianos em hospedeiros previamente infectados por M. tuberculosis resulta no desenvolvimento de uma patologia severa associada a um influxo massivo de neutrófilos, fenómeno designado por reacção de Koch. Como a protecção conferida pela vacinação por BCG é variável e tem, segundo alguns autores, uma duração de apenas 10 a 20 anos, tem-se vindo a sugerir que a re-vacinação pelo BCG seria uma estratégia para o controlo da tuberculose. Neste sentido, é necessário um conhecimento mais profundo dos mecanismos moleculares e celulares associados ao desenvolvimento da reacção de Koch. Para elucidar esta questão, ratinhos selvagens foram infectados com M. tuberculosis. Quinze dias depois, foram vacinados uma, duas ou três vezes com M. bovis BCG, com um intervalo de quinze dias entre cada vacinação. Os meus resultados mostram que o número de células Th17 aumentava com o aumento do número de revacinações. Uma vez que a presença de células Th17 está relacionada com o recrutamento massivo de neutrófilos, é de esperar que estes possam ser responsáveis pela patologia associada ao desenvolvimento da reacção de Koch. Em resumo, este trabalho introduz novos mecanismos na já complexa rede do desenvolvimento e regulação de respostas imunológicas, nomeadamente a regulação das células Th17 pelo IFN-γ, e a relevância da IL-23 na diferenciação das células Th17. Mostrei ainda que a IL-17, actuando nos macrófagos, inibe a sua actividade bactericida. Contudo, a relevância biológica desta observação tem que ser clarificada. Os meus resultados sugerem adicionalmente que a IL-17, associada a um recrutamento massivo de neutrófilos, poderá ser responsável pelo desenvolvimento da patologia associada à reacção de Koch. Os resultados apresentados nesta tese abrem novas perspectivas sobre a importância do estudo da dinâmica e função das células Th17 nas infecções por micobactérias, nomeadamente nos mecanismos envolvidos na resistência e desenvolvimento de patologia, tendo implicações para o desenvolvimento de novas estratégias preventivas.
