Occupational health nurses must apply the universal principles of Total Quality Management so that health services are customer driven, value added, process focused, and statistically measured. They must elicit customer input prior to, along with, and after program implementation. Programs must be directed toward doing the right things and doing things right. Measurement must be a priority of any program in health services. If occupational health nurses do all this, they will be at the forefront of the American quality movement, especially as it relates to health care.

The number of doctoral programmes in nursing has rapidly multiplied in many countries of the world, with each doctoral programme varying not only between countries, but also between higher education institutions within countries. This has led to a concern about the quality of doctoral education in nursing and the development of quality criteria, standards and indicators (QCSI) for doctoral education in nursing that can be applied globally. This study is part of an international collaborative study to compare the quality of doctoral education in nursing among Australia, Japan, Korea, South Africa, Thailand, United Kingdom (UK) and the United States of America (USA) using the QCSI criteria, i.e. the nature of the mission, the quality of academic personnel, doctoral students, curriculum, programme administration and infrastructure, the availability of institutional resources and evaluation of the programme. In South Africa, no study has ever been conducted at a national level to evaluate the quality of doctoral education in nursing. Linking to the global need, this research aimed to explore and describe the quality of doctoral education in nursing in South Africa and to develop a strategy to improve the quality of doctoral education in nursing in South Africa. The aim was achieved through five objectives: exploring and describing the quality of doctoral education in nursing in South Africa from the perspectives of nursing deans, academic personnel, doctoral graduates and doctoral students with regard to the nature of the mission, the quality of academic personnel, doctoral students, curriculum, programme administration and infrastructure, availability of institutional resources, and evaluation of the programme. A further objective was to develop a strategy to improve the quality of doctoral education in nursing in South Africa. The design of the study was quantitative, with exploratory, descriptive and contextual research strategies. The research followed specific steps that consisted of two phases. Phase one consisted of the first four objectives of the study and entailed a comprehensive literature review and empirical research using four structured (Likert-type) internet-mediated (by v email) questionnaires to explore and describe the quality of doctoral education in nursing in South Africa from the perspectives of nursing deans, academic personnel, doctoral graduates and doctoral students. Sixty-two (62) problems were identified from the literature review and the empirical research, which served as the evidence base towards developing a strategy for improving the quality of doctoral education in nursing in South Africa in phase two, which consisted of objective five of the study. The strategy for improving the quality of doctoral education in nursing in South Africa was developed using a strategic process to develop a vision, mission, values, principles, assumptions, strategy objectives, and functional tactics, based on Total Quality Management (TQM) philosophy_ Finally, the research was evaluated, limitations were identified and recommendations were formulated for practice, education, research and policy.

During the past decade, nurses in the clinical setting have began making a paradigm shift from Quality Assurance to Total Quality Management, or as it is commonly referred to within health care facililties, Continuous Quality Improvement.In contrast, scant attention has been paid to quality management practices in nursing in the higher education sector. This study provides an applied example of where it investigates quality management practices in the context of organisational culture and human resource management with the aim of developing a quality culture model for a school of nursing in higher education.The research study that was conducted produced several major findings from the views of nurse academics who participated in it. Several issues associated with nurse academics' opinions of quality management practices utilised in schools of nursing have been unravelled.The fundamental issue is that procedures and policies formulated for nurses in the hospital setting do not serve the needs of nursing education.The most crucial factor to be considered in policy developments and future research is that it needs to be contextualised in the culture of nursing in higher education.It has become imperative that a transparent quality culture reflects contemporary nursing in Australia and the proposed model in this thesis provides nurses with an opportunity to shape a quality system for the nursing profession.

Old people's life situation when receiving municipal help and care in theirlast period of life is sparsely investigated from their own perspective. Thepurpose of this study was focused on the thoughts of the aged people andtheir personal experiences on what quality is within the geriatric care. Inthis qualitative study, 10 elderly people aged 75-90 years wereinterviewed from 3 different nursing homes within Solna Municipality.Qualitative interviews, with the emphasis on their present life situationespecially what brought about a good life, were performed. The interviewswere analysed using qualitative content analysis. The implication of theterm “meaningful existence” is individual and differs from person toperson. This study has identified six categories that could lead to ameaningful existence for the elderly living in nursing homes. These were:to feel secure, maintaining self determination, maintaining relations, to berespected, to feel comfort and to be active. In conclusion it was found thata meaningful existence, what ever it may be to the single individual, isimportant the quality of life for the elderly person residing in nursing homes.

Syftet med detta examensarbete är att bidra med kunskap om hur kundfokus enligt Total Quality Management kan förstärkas i socialtjänstens uppföljning av insatsen vård-och omsorgsboende. Genom kvalitativ metod har dokumentanalys och kartläggning av dokument från socialstyrelsen och tre olika kommuner genomförts för att undersöka vilka verktyg för kvalitetssäkring som används av socialtjänsten idag. Verktygen som inhämtades var identiska i form av kvantitativa uppföljningsfrågor. De bedömdes inte vara tillräckligt kundorienterade ur TQM perspektiv och därför modifierades dessa till en ny intervjumall med öppna frågor. Den modifierade intervjumallen testades på fyra informanter som bor på vård-och omsorgsboende för att undersöka på vilket sätt kundperspektivet i uppföljningsprocessen kan bidra till högre kvalitet på insatsen. Slutsatsen är att uppföljningar med öppna frågor var ett steg närmare kundperspektivet, men dock inte tillräckligt då uppföljningsfrågorna endast undersökte informanternas basbehov. Förbättringsområden på insatsen vård-och omsorgsboende som identifierats från informanternas utsagor stämmer med vad tidigare forskning definierat som största svårighet med implementering av TQM i offentlig sektor: begränsade ekonomiska resurser samt att tillgodose en stor grupps varierade och individuella behov. Begränsade ekonomiska resurser leder till personalbrist, brist på utbud på aktiviteter och hur ofta de kan genomföras samt mindre varierade måltider och sämre kvalitet på råvaror.
The purpose of this report is to contribute with knowledge of how customer focus from a perspective of Total Quality Management can be strengthened in the social services' follow-up of care and care housing. Qualitative document and survey analysis of documents from the Swedish National Board of Social Affairs and three different municipalities were carried out to investigate which tools for quality assurance are used by the social service today. The tools obtained were identical in the form of quantitative follow-up questions. They were not considered to be sufficiently customer-oriented from TQM's perspective and therefore these were modified to a new interview template with open questions. The modified interview template was tested on four informants who live in nursing homes to investigate how the customer perspective in the follow-up process can contribute to higher quality of the intervention. The conclusion is that follow-ups with open questions were one step closer to the customer perspective, but not enough, since the follow-up questions only examined the informants' basic needs. Areas of improvement in nursing home care services identified from the informants' statements are consistent with what previous research has defined as the greatest difficulty in implementing TQM in the public sector: limited financial resources and meeting a large group's varied and individual needs. Limited financial resources lead to staff shortages, lack of range of activities and how often they can be carried out as well as less varied meals and poor quality of raw materials

Trata-se de um estudo que tem como objeto a elaboração de uma ferramenta computacional para coleta de dados e cálculo de indicadores, para avaliação da qualidade da assistência de enfermagem, que permitirá fazer análise de causa e consequência, e caracterizar o nível de atuação da enfermagem, identificando aspectos positivos ou negativos da assistência que vem sendo prestada ao paciente, com o objetivo de melhorá-la. Objetivos: Desenvolver uma ferramenta computacional para mensuração de indicadores de qualidade da assistência de enfermagem; identificar os indicadores de qualidade da assistência de enfermagem na ótica dos enfermeiros das clínicas médica feminina e masculina, estabelecendo articulação com os indicadores do manual de indicadores de enfermagem do Núcleo de Apoio à Gestão Hospitalar (NAGEH); construir uma ferramenta computacional para o registro das ocorrências relacionadas à assistência de enfermagem e cálculo dos indicadores de qualidade de enfermagem; analisar os resultados dos testes com a ferramenta, a partir dos dados coletados nas clínicas médicas. Método: Trata-se de uma pesquisa metodológica, que se refere às investigações dos métodos de obtenção, organização e análise de dados, para a elaboração, avaliação dos instrumentos e técnicas. Após aprovação no Comitê de Ética em Pesquisa com o CAAE nº 21758113.9.0000.5243, se deu a coleta de dados com a técnica de entrevista semiestruturada, realizada no período de fevereiro a maio de 2014, com os enfermeiros das clínicas médicas de um hospital universitário. Para este estudo o caminho metodológico foi constituído de cinco fases: revisão bibliográfica, entrevista com os participantes da pesquisa, organização dos dados, análise dos dados, estruturação da planilha e teste do protótipo impresso e eletrônico nas clínicas médicas. Resultados: Os dados obtidos com a entrevista evidenciaram os indicadores que poderiam ser elaborados, medidos e controlados nas clínicas médicas de acordo com a visão dos enfermeiros. Também possibilitou o registro de outras informações e impressões acerca do trabalho e da gestão do cuidado desenvolvida nas clínicas, com a evidência de que a qualidade do cuidado decorre da avaliação das três dimensões: estrutura, processo e resultado. Na ótica dos enfermeiros, os três indicadores de qualidade da assistência de enfermagem, prioritários para serem medidos e monitorados, compondo a ferramenta computacional proposta neste estudo foram: incidência de úlcera de pressão, incidência de queda e incidência de flebite. Conclusão: O uso de uma ferramenta computacional de indicadores possibilitará, através da construção de um aplicativo WEB, pelo setor de Gerência e Tecnologia e Informação do Hospital, o armazenamento dos dados referentes aos processos de assistência de enfermagem, e o mesmo ficará disponível para que a avaliação dos resultados dessa assistência seja realizada em tempo real, e o gerenciamento das informações possa ser compartilhado entre diretor de enfermagem, gerentes e coordenadores da assistência, educação permanente e núcleo da segurança do paciente.
This is a study that has as its object the development of a computational tool for data collection and calculation of indicators, to assess the quality of nursing care, that will allow the analysis of cause and consequence, and to characterize the level of performance of nursing, identifying positive or negative aspects of care that is being provided to patients in order to improve it. Objectives: Develop a computational tool to measure quality of nursing care indicators; identify indicators of quality of nursing care in the perspective of nurses in female and male medical clinics, establishing liaison with indicators of the manual of indicators nursing of the Support Center for Hospital Management (NAGEH, in Portuguese); build a computational tool for documenting events related to nursing care and calculation of nursing quality indicators; analyze test results with the tool, from the data collected in medical clinics. Method: It is a methodological research, which refers to investigations of obtaining methods, organization and analysis of data for the preparation, evaluation of tools and techniques. After approval by the Ethics Committee on Research with CAAE No 21758113.9.0000.5243, the data was collected by semi-structured interview technique, performed in the period from February to May 2014, with the nurses of medical clinics of a university hospital. For this, the methodological study consisted of five phases: literature review, interviews with participants, data organization, data analysis, spreadsheet structuring, test of the printed and electronic prototype in medical clinics. Results: The data obtained from the interview shows the indicators that could be developed, measured and controlled in medical clinics according to the view of nurses. Also enabled the recording of other information and impressions about the work and care management developed in clinics, with evidence that the quality of care results from the evaluation of three dimensions: structure, process and outcome. In the view of nurses, the three priority indicators of quality of nursing care to be measured and monitored, making the computational tool proposed in this study were: pressure ulcer incidence, incidence of falls and incidence of phlebitis. Conclusions: The use of a computational tool of indicators by building a web application will allow the storage of data on nursing care processes, by the management, Technology and Information sector of the Hospital, which will be available for the evaluation the results of this assistance performed in real time, and will allow the management of information can be shared between the director of nursing, managers and coordinators of care, continuing education and patient safety center.

AbstractWe are now witnessing a major change in the world’s population. Many people globally grow very old: 80, 90, and 100 years. Increased age is followed by an increased incidence of functional and chronic comorbidities and diverse disabilities, which for many leads to the need for long-term care in a nursing home. Quality of life and health promotive initiatives for older persons living in nursing homes will become ever more important in the years to come. Therefore, this chapter focuses on health promotion among older adults living in nursing homes. First, this chapter clarifies the concepts of health, salutogenesis, and pathogenesis, followed by knowledge about health promotion. Then insight and knowledge about the nursing home population is provided; what promotes health and well-being in nursing home residents?Health promotion in the health services should be based on integrated knowledge of salutogenesis and pathogenesis. The salutogenic understanding of health is holistic and considers man as a wholeness including physical, mental, social, and spiritual/existential dimensions. Research indicates that various health-promoting interventions, specifically the nurse–patient interaction, influence on older adults in nursing homes as a wholeness of body–soul–spirit, affecting the whole being. Hence, dimensions such as pain, fatigue, dyspnea, nausea, loneliness, anxiety, and depressive symptoms will be influenced through health-promoting approaches. Therefore, two separate studies on the health-promoting influences of nurse–patient interaction in nursing home residents were conducted. In total, nine hypotheses of directional influence of the nurse–patient interaction were tested, all of which finding support.Along with competence in pain and symptom management, health-promoting nurse–patient interaction based on awareness and attentional skills is essential in nursing home care. Thus, health care workers should be given the opportunity to further develop their knowledge and relational skills, in order to “refine” their way of being present together with residents in nursing homes. Health professionals’ competence involves the “being in the doing”; that is, both the doing and the way of being are essential in health and nursing care.

Resumo:
One of the forms of care for the elderly are the nursing homes, long-term care homes. Still, in many countries the low level of quality of such services is still the main criterion for the perception of objects as a final option, in the absence of alternative forms of care for an older person. The aim of the article is to seek answers to the questions about the expected quality of the services offered by nursing homes. The article presents the results of research on the expectations of the society in terms of quality of services, carried out on a sample of 602 Polish citizens. The study allowed to identify the key characteristics that determine the quality of services from the perspective of the future decisions related to the choice of the resort.

Abstract One of the pitfall of total quality management (TQM) is that it is based on a single measurement, which has mostly been “quality” or “quality-driven.” Today manufacturing sectors are much more fiercely competitive and global than ever before. Consumers are more demanding, competition is more global, fierce and ruthless, and technology is advancing (and changing) rapidly. The quality-based philosophy inherent in TQM does not account for the shrinking time factor inherent in today’s complex product design, development and delivery (PD3) process. The competitors are always finding better and faster ways of doing things. Catching up in quality is not enough to be a world-class leader in manufacturing. It only makes a company at par with its competitors in terms of inheriting some of their product’s quality characteristics. But relatively speaking it gets you there only after a few years later. What is required is a total control of one’s own process — that is to identify and satisfy the needs and expectations of consumers better than the competitions and to do so profitably faster than any other competitor. Competition has driven organization to consider concepts such as time compression (fast-to-market), concurrent engineering, design for X-ability, and tools and technology (such as Taguchi, Value Engineering, QFD, etc.) while designing and developing an artifact. Quality — as in TQM — addresses aspects of “quality” with reference to the functions a product has to perform. But, this is one of the many value characteristics that need to be considered by a world-class manufacturer. With conventional TQM process, it is difficult to address all aspects of Total Values Management (TVM) such as X-ability, cost, tools and technology, responsiveness and organization issues. It is not enough to include “Quality” into a product or process and expect the outcome to be a world-class. TVM efforts are vital in maintaining a competitive edge in today’s global marketplace. The question is how to address all value aspects of this TVM? This is what discussed in this paper. A new concurrent PD3 process for TVM methodology is proposed, which accounts for concurrency — paralleling of value characteristics — along with a methodology for their systematic deployments.
