Abstract: Rather than attempting to prevent all overdoses, the Prescription Pill Dispenser Vial (RPPDV) aims to prevent accidental overdoses by controlling how many pills a user can access within a twenty-four hour period. The RPPDV is much like a traditional pill vial. The body is similar to a conventional pill vial; however, it is modified to prevent the cap's opening without a special screw. The cap is heavily modified and uses a CR2032 or CR2025 coin cell battery to power a gate and the unlocking of the pill cap. The RPPDV works first when the user clicks the red button on the cap of the RPPDV, which opens the pill gate and opens space into a pill container section. This section is large enough for only one pill (or two if multiple pills are required simultaneously). Once a tablet has entered the pill container section, the button must be pressed again to close the pill gate. The user cannot open the lid to the pill container section until the pill gate is closed (see Fig 1.4.1). The RPPDV is similar to an escrow service, with the pill holder section acting as a middleman. The size of the pill holder container prevents access to multiple pills at the same time. The number of times the user presses the button is recorded and timed with a 555 timer set for twenty-four hours. For example, if a drug has a maximum dose of three pills per day, the button could be capped at four cycles or eight total pushes (to allow a grace cycle if a tablet is broken or stuck). Similarly, the gate cannot be opened unless the lid to the pill container section is closed. The production price of the RPPDV is significantly higher than a traditional pill vial and can get higher depending on the additional features added to the RPPDV. The necessary components to function include CR2032 or CR2025 lithium batteries, a 555 timer, and a coin battery holder, along with plastic and metal molds. The base model alone produces an estimated average cost per unit of $1.33 USD (see Fig 4.2.1). Additional features include adding a Bluetooth Low Energy (Bluetooth LE) component to customize the number of unlocks per day and RFIDs for storage and tracking. Prices vary depending on the specifications of the RPPDV.

AbstractObjectives: To investigate whether placebo effects may vary in strength, depending on appearance of the medication.Method: Polish subjects were randomly given different kinds of pills and told that the medicine was produced abroad or in Poland and that it was cheap or very expensive. Measures of different forms of well-being were taken directly after the ‘therapy’.Results: Subjective ratings were higher for subjects who received big white or small red medicine compared to subjects who received small white or big red pills. The results are discussed in the light of the subject's previous experience with drugs of different kinds.Conclusion: It was shown that placebo effects do vary in strength, depending on the appearance of the pill.

Academic research is dedicating growing attention to the phenomenon known as the manosphere, where genders and (hetero)sexual relations are explained according to the so-called Red Pill philosophy and LMS theory. In this article, we critically analyze these theories and their pseudoscientific assumptions using a Bourdieusian framework, and we analyze the empirical effects they have in the Incel and Red Pill Italian communities through the example of the “Aranzulla case”. By exploring the “Redpiller” blog and two Incel forums (“Forum of the uglies” and “Forum of the Incel”), we found that, on one hand, the Red Pill and LMS theories are strongly contradictory in themselves because, even if they combine socio-economic and biological theories to explain sexual relations, they assume a fixed vision of gender based on (supposedly) natural inclinations. On the other hand, our results show that these theories promote misogynistic content blaming women for men’s unsatisfaction, but they actually unveil intragender dynamics. Indeed, they produce tensions inside the manosphere, by creating hierarchies of masculinities, and around the possibility of escaping the biological destiny linked to the Incel condition.

Objective: The therapeutic efficacy and molecular mechanisms of traditional Chinese medicines (TCMs), such as Liuwei Dihuang pills (LWDH pills), in treating osteoporosis (OP) remain an area of active research and interest in modern medicine. This study investigated the mechanistic underpinnings of LWDH pills in the treatment of OP based on network pharmacology, bioinformatics, and in vitro experiments. Methods: The active ingredients and targets of LWDH pills were retrieved through the TCMSP database. OP-related targets were identified using the CTD, GeneCards, and DisGeNET databases. The STRING platform was employed to construct a protein–protein interaction (PPI) network, and core targets for LWDH pills in treating OP were identified. The GO functional and KEGG pathway enrichment analyses for potential targets were performed using the R package “clusterProfiler”. A “drug–target” network diagram was created using Cytoscape 3.7.1 software. The viability of MC3T3-E1 cells was evaluated using the CCK-8 method after treatment with various concentrations (1.25%, 2.5%, 5%, and 10%) of LWDH pill-medicated serum for 24, 48, and 72 h. Following a 48 h treatment of MC3T3-E1 cells with LWDH pill-medicated serum, the protein levels of collagen Ⅰ, RUNX2, Wnt3, and β-catenin were quantified using the Western blot analysis, and the activity of alkaline phosphatase (ALP) was measured. Results: A total of 197 putative targets for LWDH pills for OP treatment were pinpointed, from which 20 core targets were singled out, including TP53, JUN, TNF, CTNNB1 (β-catenin), and GSK3B. The putative targets were predominantly involved in signaling pathways such as the Wnt signaling pathway, the MAPK signaling pathway, and the PI3K-Akt signaling pathway. The intervention with LWDH pill-medicated serum for 24, 48, and 72 h did not result in any notable alterations in the cell viability of MC3T3-E1 cells relative to the control group (all p > 0.05). Significant upregulation in protein levels of collagen Ⅰ, RUNX2, Wnt3, and β-catenin in MC3T3-E1 cells was observed in response to the treatment with 2.5%, 5%, and 10% of LWDH pill-medicated serum in comparison to that with the 10% rabbit serum group (all p < 0.05). Furthermore, the intervention with LWDH pill-medicated serum resulted in the formation of red calcified nodules in MC3T3-E1 cells, as indicated by ARS staining. Conclusions: LWDH pills may upregulate the Wnt/β-catenin signaling pathway to elevate the expression of osteogenic differentiation proteins, including collagen Ⅰ and RUNX2, and to increase the ALP activity in MC3T3-E1 cells for the treatment of OP.

Este ensayo analiza el discurso del Mr. Misterios, un autoproclamado “macho alfa” que promueve la fi losofía red pill en su canal de YouTube. Se argumenta que este discurso surge como reacción a cambios sociales que han empoderado a las mujeres, especialmente el feminismo y la liberación sexual femenina; para ello, se usa como base teórica el concepto de “contrato sexual” de Carol Pateman (1995). Se plantea que el discurso red pill va más allá de la misoginia tradicional, convirtiéndose en un “sexismo hostil” que presenta a las mujeres como peligrosas y socialmente poderosas. Además, se enmarca a Mr. Misterios dentro del contexto más amplio de la manósfera, argumentando que este tipo de contenido online sirve como puerta de entrada a grupos y foros con retórica aún más violenta y radical. En este sentido, se busca dar cuenta de los distintos conceptos que caracterizan el discurso de la manósfera, como la hipergamia femenina, el ginocentrismo y la filosofía red pill. Se concluye resaltando la necesidad de prestar atención a los mecanismos mediante los cuales la manósfera difunde y normaliza discursos sexistas y agresivos, aprovechando la vulnerabilidad de hombres que se sienten insatisfechos con su masculinidad.

Zuo Gui Wan is an important Chinese herbal medicine with potent effects on the Zang-fu organ, Kidney. The chief herb, Shu Di Huang (Rehmannia) tonifies Kidney Yin and Jing (Essence) and fills the Marrow. Every song has an important meaning that the singer intends to communicate to the audience. To have a thorough understanding of the music you wish to express, you might delve into its connotative meaning. The aims of this study were to identify the types of connotative meaning from the lyrics in selected song albums by Maroon 5 “Red Pill Blues”. The data were analyzed uses descriptive qualitative method based on the theory proposed by Leech (1974). This study found 25 data, based on the theory used there are three categorize of connotative meaning from the data above namely; positive connotative, negative connotative and neutral connotative with positive connotation as a highest occurrence. The finding show that connotative meaning is becoming increasingly. Common and maybe as a suitable learning medium. This research will provide learning with deep understanding about the meaning of the word.

Introduction The gap between clinical trials and real-world care continues to grow in the field of multiple myeloma (MM). Randomized controlled trials (RCTs) cannot account for every patient scenario. Modern clinical decision support systems (CDSSs) that leverage machine learning (ML) models offer an alternate path to precision oncology. However, little work has been done to understand how clinicians reconcile RCT with ML data, particularly when results are discordant. Aim We designed a CDSS that displays simulated survival and adverse event data from an RCT and ML model and conducted a survey study to evaluate how clinicians incorporate the available data to make treatment decisions for 12 patients with MM. Methods Participants were presented with varying combinations of RCT and ML results in increasing “tiers” of information for 12 patients (A-L) with MM (Table 1). In tier 1, participants were provided RCT data only. In tier 2, participants were given outcomes of an ML model and in tier 3, they were provided with information about how the model was trained and validated. At each tier, participants were asked to select a treatment (“red pill” or “blue pill”), rate their confidence in treatment on a Likert scale from 1-10, and when ML data was available, rate their perceived reliability of the model on a Likert scale from 1-10. Participants were recruited from internal medicine and hematology/oncology departments via email between January and April 2023, and were offered a $50 Amazon gift card as incentive. We used descriptive statistics to analyze respondent characteristics. For each scenario, we ran two-sample paired t-tests to compare the change in confidence and reliability between tier 2 and tier 1 (ML versus RCT data) and tier 3 versus tier 2 (ML data with information about training and validation versus without). We utilized a Bonferroni correction to adjust the alpha level for significance. We also ran McNemar's tests with a Bonferroni correction to assess the difference in proportions of blue pill selection at different tiers to characterize the extent of treatment switching. Results A total of 284 physicians were invited to participate in the study and 32 participated, for a response rate of 11.3%. Half were internal medicine residents and half were hematology/oncology fellows and attendings. A majority were male (72.0%), white (69.0%) and all were less than 40 years of age. For scenarios A-D, the patient met inclusion criteria for the RCT and was well represented in training data for the ML model. Confidence was highest for scenario A where survival results between the RCT and ML model were concordant (Figure 1). Across scenarios B, C, and D, confidence dropped after seeing the ML results (B: p=0.05, C: p=0.05, D: p=0.36) but increased after participants learned the ML model was trained on patients like theirs (B: p=.002, C: p=0.17, D: p=0.06). In scenarios C and D, a majority of participants switched treatment after learning the ML model showed no benefit with the red pill (C: p=5.1x10 -4, D: p=1.5x10 -5). In scenarios E-H, the patient did not meet inclusion criteria for the RCT but was well represented in the training data for the ML model. When the ML model showed worse adverse events (F) or no benefit with the red pill (G, H) the majority of participants switched treatment choice (F: p=0.27, G: p=0.002, H: p=1.9x10 -4). There was an increase in confidence when participants learned the ML model had been trained on representative patients (E: p=0.008, F: p=0.01, G: p=0.18, H: p=2.0x10 -3). In scenarios I-L, the patient did not meet inclusion criteria for the RCT and was not well represented in the training data for the ML model. There was a decrease in confidence when participants learned this in tier 3 (I: p=0.18, J: p=3.0x10 -4, K: p=0.05, L: p=0.009). However, the majority of participants still switched to the blue pill when the ML model showed no benefit with the red pill (K: p=1.9x10 -4). Conclusions Confidence in treatment was highest when RCT and ML findings were concordant. Participants chose treatments based on ML model estimates even before assessing how the ML model was trained or validated. Participants preferred the treatment that demonstrated a survival benefit, regardless of whether it was supported by RCT data or an ML model.

Abstract: The "sentimental" narrator of Hari Kunzru's Red Pill is starting to feel like a self-described "waster."1 Away on fellowship in Berlin at the interdisciplinary Deuter Center for Social and Cultural Research, our writer-in-residence narrator ingests the eponymous capsule and "wakes up" to the obsolescence of literary humanism, a historical "period that was drawing to a close" ( RP 46).2 Instead of using the time afforded by the fellowship to work on his grant winning project––notunironically titled "The Lyric I," and which aims to achieve poetic transcendence through a better understanding of "the construction of the self in lyric poetry" ( RP 15)––the narrator whiles away his days on violent police procedurals and social media doom-scrolling.3 "I was like a miser, fretting about his emotional hoard," he confesses: "I frequently found myself hunched over my laptop, my eyes welling with tears … If the world changed, would I be able to protect my family?" ( RP 6-7). Alternating between sentimental musings and apocalyptic fantasies, he slides into a "mad" state of internet-fueled paranoia and begins to see signs and symbols of "red pill" and Alt-Right ideology everywhere he looks ( RP 280). The "sleepy-eyed cartoon frog" on a stranger's tee shirt slyly signals right-wing in-group belonging, while the OK sign made by the right hand of Carl Spitzweg's "Poor Poet" is reinterpreted as an allusion to contemporary fascist iconography, a cryptic communication from the not-so distant past ( RP 27; 9-10).

Background. This study aimed to explore the function of modified Yuejuwan (MYJ) on THP-1 macrophage-derived foam cells. Methods. First, human THP-cells were obtained, and then, grouping was made to the following: control group, foaming group, foaming group +0.2 mg/mL Jiawei Yueju pill, foaming group +0.5 mg/mL Jiawei Yueju pill, and foaming group +1 mg/mL Jiawei Yueju pill. An Oil Red O staining assay was used to examine the uptake of oxidatively modified low-density lipoprotein (oxLDL). The secretion of interleukin (IL)-1β and tumor necrosis factor (TNF)-α were determined using an enzyme-linked immunosorbent assay (ELISA). Real-time quantitative PCR (qRT-PCR) and Western blot were used to quantify genes and proteins expression levels. Results. Our results indicated that MYJ inhibited the accumulation of total cholesterol (TC), free cholesterol (FC), and cholesteryl ester (CE) in foam cells. Moreover, the secretion of IL-1β and TNF-α also downregulated in foam cells after treatment of MYJ. Furthermore, we found that tripartite motif-containing 37 (TRIM37) was significantly upregulated in foam cells. Knockdown of TRIM37 promoted cholesterol efflux and presented an anti-inflammation effect in foam cells. Furthermore, TRIM37 positively mediated the translocation of NF-κB to nuclear. It negatively regulated its ubiquitination in foam cells after interacting with TRAF2. Importantly, MYJ profoundly suppressed the function of TRIM37 in foam cells and functioned as a TRIM37 inhibitor. Conclusions. This study demonstrated that MYJ might alleviate oxLDL-induced foam cell formation by inhibiting the TRIM37/TRAF2/NF-κB pathway activity. MYJ was a potential agent in preventing atherosclerosis and indicated its potential signaling pathway in foam cells.

Desde a propagação dos discursos neoliberais de poder no Brasil, tem-se observado uma crescente proliferação de discursos e comportamentos misóginos online, que ameaçam a segurança e o bem-estar das mulheres. Esta problemática é muitas vezes alimentada por movimentos como o Red Pill, assim como pela falta de legislação adequada para combater essa forma de discriminação de gênero. Os "red pills", por sua vez, são homens que se posicionam contra o que consideram um "sistema pró-mulheres", acreditando que as mulheres carecem de lealdade e bom caráter em relação aos homens. Este estudo adota uma abordagem metodológica descritiva e qualitativa para traçar um panorama histórico que investigue as origens desse discurso misógino e seu impacto na internet. Concluímos que o machismo é um problema que requer uma abordagem ativa, juntamente com a necessidade de preencher as lacunas legislativas relacionadas à misoginia online. Encapsulating antioxidant-rich plant extracts, such as those found in red clover, within microcapsules helps protect them from degradation, thus improving stability, shelf life, and effectiveness. This study aimed to develop a microencapsulation delivery system using chitosan and alginate for microcapsules that dissolve in both the stomach and intestines, with the use of natural and synthetic emulsifiers. The microcapsules were formed using the extrusion method and employing alginate or chitosan as shell-forming material. In this study, all selected emulsifiers formed Pickering (β-CD) and traditional (white mustard extract, polysorbate 80) stable emulsions. Alginate-based emulsions resulted in microemulsions, while chitosan-based emulsions formed macroemulsions, distinguishable by oil droplet size. Although chitosan formulations with higher red clover extract (C1) concentrations showed potential, they exhibited slightly reduced firmness compared to other formulations (C2). Additionally, both alginate and chitosan formulations containing β-CD released bioactive compounds more effectively. The combined use of alginate and chitosan microcapsules in a single pill offers an innovative way to ensure dual solubility in both stomach and intestinal environments, increasing versatility for biomedical and pharmaceutical applications.

Resumo:

Este artigo analisa a aparente crise de masculinidades na cultura Sigma brasileira através de memes de internet. A cultura Sigma se destaca pela rejeição das normas sociais convencionais e pelo respeito pelas tradições, frequentemente associada à perspectiva Red Pill. O discurso desse grupo, muitas vezes altamente misógino, é ocasionalmente disfarçado por meio de memes, tornando-o facilmente compartilhável. Realizou-se uma análise crítica de conteúdo visual dos memes, combinando análise crítica de imagens com contextualização cultural, focando na prática visual. A pesquisa concentrou-se na hashtag #redpillbrasil e nas contas do Instagram @brazilian_sigma e @wojakemes_sigma para identificar temas e discursos culturais. Os resultados evidenciam que a percepção de tal crise se expressa como uma mentalidade Sigma, promovendo misoginia, antifeminismo, nacionalismo e valores convencionais.

O artigo visa a problematizar como discursos misóginos e machistas se disseminam a partir de movimentos do masculinismo, como a Red Pill. Parte-se do conceito de gênero (Scott, 1995) e de violência simbólica (Bourdieu, 2002), com enfoque na história da sexualidade (Foucault, 1988). Discute-se, ainda, um prelúdio sobre o conceito de masculinidade (Nolasco, 2001; Connel e Messerschmidt, 2013). Pretende-se recorrer à perspectiva teórico-metodológica da Análise de Conteúdo (Bardin, 2011) para tentar identificar o discurso misógino e sexista usado por Thiago Schutz na busca por deslegitimar a autonomia das mulheres, no programa Jovem Pan Morning Show. O texto identifica que discursos masculinistas, como os do influenciador digital Thiago Schutz, podem incitar ódio e violência contra mulheres e perpetuar estereótipos de gênero.

Abstract The article analyzes how allusive cognitive metaphors (ACMs) function as a persuasive narrative strategy in contemporary social media–fueled storytelling cultures. The ACM is a concise way of combining intertextual and metaphorical meaning-making for use in viral storytelling. Well-known works of fiction function as a shared baseline that can be easily alluded to. This narrative-metaphorical strategy has been adopted especially frequently by populists and online groups advocating extreme ideologies, one of the prominent and influential cases being “the red pill,” coined by the antifeminist manosphere. The popularity of ACMs suggests that the interpretive contexts and target texts that narrative scholarship has grown accustomed to are changing, and that scholars of narrative and fiction need to adapt to the new challenges stemming from the ever-expanding digital sphere.

In order to solve the problem of the low qualification rate of the pilling and coating of small-grain forage seeds, a vibration force field is introduced to the traditional vertical disk coating machine to promote the mixing of materials and improve the qualification rate of the pilling. Using the typical small-grain forage seed red clover as an example, we used the vibration force field after adding seed powder particles to a coating pot for the theoretical analysis of the force situation, using the discrete element software EDEM to construct a red clover seed simulation model with the coefficient of discretization as the evaluation index. We studied the effects of the rotational speed of the coating pot, the vibration frequency of the pot, the amplitude of the vibration of the pot, and the other operating parameters of the pot on the uniformity of the seed powder mixing, with the pelletization of the pass rate as the physical evaluation standard, using a one-way test to study the effect of operating parameters on the pelletization pass rate. We used the qualified rate as the physical evaluation standard, through a single-factor test, to study the influence of the working parameters on the qualification rate of the pilling. The results show that the simulation test evaluation index of the discrete coefficient and the physical test evaluation index of the pilling qualification rate with the change rule of the working parameters is consistent with the discrete coefficient, and can be used as an indirect evaluation index of the quality of pilling. To further determine the optimal combination of working parameters, a quadratic regression orthogonal design test was conducted with the discrete coefficients as the evaluation index, and the second-order regression equations of the red clover seeds were established and analyzed by ANOVA using Design-Expert software. The study shows that, when the rotational speed of the coating pot is 307.204 rpm, the vibration frequency is 2.526 Hz, and the vibration amplitude of the coating pot is 5.843 mm, the predicted coefficient of dispersion at this time is 8.1%. Simulation using the best combination of parameters to obtain the average value of the dispersion coefficient of 8.4%, relative to the predicted value of 3.7%, indicated that the optimization of the experimental regression model is accurate, and the results obtained for the vibration of small seeds under the conditions of the design of the pellet granulation coating machine and the optimization of the pelletization coating process parameters have a certain degree of reference significance.

Foods must have attractive color, flavor and texture to be acceptable to consumers. Carminic acid (C.I. (1956) 75470; C.I. Natural Red 4; EEC No. E120) is a natural red dye extracted from cochineal, the desiccated bodies of female Dactylopius coccus Costa insects. Carminic acid produce pink shades in candy, confections, chewing gum, concentrated fruit juice, smoked fish, liqueurs and alcoholic cordials, soft drinks, yoghurt, ice cream, tomato ketchup, baked goods, dairy products, jams, jellies, eye shadow, and pill coatings. The impact of the presence of aspartame, citric acid and vitamin C on the spectrophotometric parameters of carminic acid was investigated. These results enabled us to evaluate the possibility of using UV-Vis spectrophotometric method for easily identifying and quantifying carminic acid present in a wide variety of foodstuffs in the presence of additives. It was established that the investigated additives do not affect the position of absorbance maximum of the dye nor cause changes in its intensity. The value of molar absorptivity, ε, at 518 nm was 1265.68 ± 49.90 dm3/mol∙cm. The calculated value is necessary for the quantitative spectrophotometric determination of carminic acid food dye. The FT IR spectra of the dye was also recorded.

Algae have the potential as a very useful resource for humans. Some types of algae have been known to have antioxidant, anticancer, anti-inflammatory, and antifungal properties that can help in the treatment of diseases. This research was conducted in traditional markets and modern markets in Medan City, especially on Jl. Jamin Ginting, North Sumatra. The purpose of this study was to identify the types of algae-based medicinal products (microalgae or macroalgae) available in traditional/modern markets in Medan city. The sampling method used in this study was the cruise method with purposive sampling technique. The results showed 3 types of medicines in traditional and modern markets in Medan City that are made from algae, namely Ever E 250 (Seaweed Capsules), Algae Calcium (Red Algae) and Brown Seaweed (Brown Algae). Some of the health benefits of red algae include maintaining thyroid function, lowering cholesterol and as a healthy diet menu. The benefits of brown algae include algin or alginate acid from brown algae used in ice cream formation, pill formation, ointments, tooth cleaners, and lotions. Objective. To research the efficacy of Biejiajian pill (BJJ) on diabetes-associated atherosclerosis and explore its subsequent mechanisms. Methods. Diabetes-associated atherosclerosis (AS) was established in apolipoprotein E knockout (ApoE−/−) mice using high-fat diet and streptozotocin. Atorvastatin (ATV, 10 mg/kg/day) or BJJ-L (BJJ low-dose, 0.9 g/kg/day), BJJ-M (BJJ medium-dose, 1.8 g/kg/day), and BJJ-H (BJJ high-dose, 3.6 g/kg/day) were administered to diabetic ApoE−/− mice for 12 continuous weeks. The normal control group consisted of 10 male C57BL/6J mice. Atherosclerosis plaques, vascular endothelial function, fasting blood glucose, lipid metabolism, inflammatory factors, NLRP3 inflammasome expression, and mitochondria and autophagy changes were evaluated. Results. The atherosclerotic lesions areas in the aortas were analyzed through Oil Red O and H&E staining, and they were reduced in the BJJ-H and BJJ-M groups. In the BJJ group, endothelin-1 (ET-1) levels were decreased, whereas endothelial nitric oxide synthase (eNOS) was increased. Fasting blood glucose levels in the BJJ and ATV groups were gradually decreased. Lipid metabolism parameters such as TG, TC, and LDL-C were reduced, while HDL-C was elevated in BJJ groups. The serum IL-1β and IL-18 were decreased under BJJ therapy. The aortic mRNA and protein expressions of NF-κB, TXNIP, NLRP3, ASC, caspase-1, and IL-1β were inhibited in BJJ-H and BJJ-M groups, especially in the BJJ-H group. Electron microscopy revealed an increase in autophagy in each treatment group. Conclusions. The findings reveal that BJJ could alleviate diabetic atherosclerosis in diabetic ApoE−/− mice by inhibiting NLRP3 inflammasome.

Abstract Background: Exposure to oral 6MP for ~2 years is critical for durable remissions in children with ALL. We have previously shown that poor 6MP adherence (rates <95%) is prevalent and increases relapse risk (JCO 30[17]:2094-101). 6MP is supplied in 50 mg tablets that may be swallowed, crushed, or chewed; patients are generally instructed to take 6MP with non-dairy liquid, in the evening on an empty stomach (~1-2h after a meal). These restrictive instructions could impose challenges in 6MP ingestion; the need for such restrictions with respect to clinical outcomes has not been examined systematically. This study examines the pill-taking habits of children with ALL and their association with adherence, risk of relapse and red cell TGN levels. Methods: Participants included 441 patients with ALL in first remission receiving oral 6MP (75 mg/m2/d) during maintenance. Adherence was measured using Medication Event Management System (MEMS) that recorded dates/times of each 6MP bottle opening for 6 mo/pt (58,086 patient-days of MEMS data). Average adherence rate (days MEMS bottle opened/days 6MP prescribed) over the 6m study period was computed to define non-adherence (adherence rate <95%). A 37-item questionnaire elicited pill-taking habits: i) 6MP never with food vs. with food; ii) 6MP never with dairy vs. with dairy; iii) 6MP pill swallowing (whole, crushed, chewed); iv) time of day 6MP always taken (evening, night, morning, midday, varying times); v) use of a routine: established routine vs. no routine. Monthly TGN levels (~6/pt) yielded 1,395 measurements. 6MP dose intensity [DI] was defined as dose prescribed/ protocol dose, and inherited capacity to metabolize 6MP was characterized by thiopurine methyltransferase (TPMT) genotype. Only TPMT wild-type patients were included in this analysis. 38 relapses occurred after a median follow-up of 6.1 years from start of maintenance (cumulative incidence of relapse: 8.9±1.4% at 6 years). Results: Median age at diagnosis was 6y (2-20); 67% were males; 40.3% had high-risk disease. 43.8% of the patients were non-adherent. Pill-taking habits and non-adherence: Time-varying generalized estimating equations (adjusted for age at study, ethnicity/race, income, and parental education) identified the following predictors of non-adherence: i) Taking 6MP with dairy (Odds Ratio [OR]=1.9, 95% Confidence Interval [CI]=1.3-2.9, p=0.003); ii) Taking 6MP at varying times vs. non-varying times (OR=2.4, 95%CI=1.2-4.7, p=0.008); iii) No routine (OR=1.7, 95%CI=1.1-2.7, p=0.02). Crushing the pill vs. swallowing whole was less likely to be associated with non-adherence (OR=0.7, 95%CI=0.4-0.99, p=0.05) [Fig 1A].Pill-taking habits and relapse: A proportional sub-distribution hazards model (adjusted for time from start of maintenance, NCI risk group, cytogenetics, race/ethnicity, age at study, and adherence) demonstrated no association between pill-taking habits and relapse risk: i) 6MP with food (Hazard Ratio [HR]=0.7, 95%CI=0.3-1.9, p=0.5); ii) 6MP with dairy (HR=0.3, 95%CI=0.07-1.5, p=0.2); iii) Swallowing tablet whole vs. crushing/chewing (HR=0.8, 95%CI=0.3-2.1, p=0.7); iv) 6MP in the evening/night vs. morning/mid-day (HR=1.1, 95%CI=0.2-7.5, p=0.9); v) 6MP at varying times vs. non-varying times (HR=1.0, 95%CI=0.2-4.5, p=0.9) [Fig 1B].Pill-taking habits and DI-adjusted TGN levels: Among adherent patients, there was no association between DI- and age-adjusted TGN levels and: i) 6MP with food (p=0.5); ii) 6MP with dairy (p=0.7); iii) Swallowing 6MP whole vs. chewing/crushing (p=0.4); iv) 6MP in the morning/midday vs. evening/night (p=0.5); v) 6MP at varying times vs. non-varying times (p=0.5) [Fig 1C]. Conclusions: The association between non-adherence and certain pill-taking habits identifies a subgroup of patients that either did not receive appropriate instructions or did not adhere to them; our study was not designed to discern between the two scenarios. However, pill-taking habits were not associated with either relapse risk or TGN levels. These findings suggest that the commonly-practiced restrictions surrounding 6MP ingestion (i.e., taking only in the evening without food/dairy) might not influence outcome. Figure 1A Figure 1A. To effectively manage, empathize with and respond to the implications and impact of the so-called ‘Manosphere,’ this paper is centered on the premise that researchers require an understanding of the draw factors that lead individuals to engage, affiliate with, and contribute to the various groups that constitute this wider movement. This paper seeks to contribute to the growing body of knowledge around the Manosphere by exploring how thought leaders propagate symbiotic cycles of ontological security and insecurity through YouTube in a manner that resembles a protection racket. It argues that these constructed ontological security cycles provide a powerful impetus to not only draw individuals into the Manosphere, but also to extract material and social resources out of them that can be reinvited to retain them within the movement.

ABSTRACT The human pathogen Eikenella corrodens synthesizes type IV pili and exhibits a phase variation involving the irreversible transition from piliated to nonpiliated variants. On solid medium, piliated variants form small (S-phase), corroding colonies whereas nonpiliated variants form large (L-phase), noncorroding colonies. We are studying the molecular basis of this phase variation in the clinical isolate E. corrodens VA1. A genomic fragment encoding the major type IV pilin was cloned from the S-phase variant of strain VA1. Sequence analysis of the fragment revealed four tandemly arranged potential open reading frames (ORFs), designatedpilA1, pilA2, pilB, andhagA. Both pilA1 and pilA2 predict a type IV pilin. The protein predicted by pilB shares sequence identity with the Dichelobacter nodosus FimB fimbrial assembly protein. The protein predicted by hagAresembles a hemagglutinin. The region containing these four ORFs was designated the pilA locus. DNA hybridization and sequence analysis showed that the pilA locus of an L-phase variant of strain VA1 was identical to that of the S-phase variant. An abundantpilA1 transcript initiating upstream of pilA1and terminating at a predicted hairpin structure betweenpilA1 and pilA2 was detected by several assays, as was a less abundant read-through transcript encompassingpilA1, pilA2, and pilB. Transcription from the pilA locus was nearly indistinguishable between S- and L-phase variants. Electron microscopy and immunochemical analysis showed that S-phase variants synthesize, export, and assemble pilin into pili. In contrast, L-phase variants synthesize pilin but do not export and assemble it into pili. These data suggest that a posttranslational event, possibly involving an alteration in pilin export and assembly, is responsible for phase variation in E. corrodens.
