The anonymity that the internet and social media affords users means that suicidal thoughts and/or behaviours can be talked about with a sense of freedom and disinhibition that is often not possible in face-to-face contexts. Better understanding online suicidal thoughts and/or behaviour talk is critical as more people turn to online spaces for support. Without this the potentiality of such spaces as sites for suicide prevention and intervention is likely to remain unrealised. Currently there are no scoping or systematic review syntheses focusing on internet and/or on social media suicidal thoughts and/or behaviour talk. This lack of synthesis is problematic as it makes it more difficult for online suicide prevention and intervention practices, policies, and our understanding of suicide to advance in a coherent and evidence-based manner. A scoping review protocol following Arksey and O’Malley’s six-step modified framework has been developed to address this synthesis gap. It aims to systematically map the empirical literature that has investigated online suicidal thoughts and/or behaviours talk. It is anticipated that review outcomes could inform the training of health practitioners and peer/professional online moderators in how to best talk with people experiencing suicidal thoughts and/or behaviours. Outcomes could also form an evidence-base for developing policies and practices that focus on online places as safe spaces to talk about suicidal thoughts and/or behaviours. Developers of safe language guidelines could also use the outcomes to audit how well current guidelines reflect empirical evidence. Outcomes could enable researchers to design future online suicidal thoughts and/behaviours talk studies that extend our understandings of suicide leading to potential refinements of contemporary suicide theories/models.

Objective: We aimed to examine whether suicidal thoughts and behaviour were independently associated with a wide range of health and social risk behaviours. Methods: We conducted cross-sectional analyses of data collected from 13,763 adult males who participated in The Australian Longitudinal Study on Male Health. We fit generalised linear models to estimate the relative risk of engaging in a range of health and social risk behaviours across several domains by suicidal thoughts and behaviour status. Results: Men with recent suicidal ideation (relative risk range, 1.10–5.25) and lifetime suicide attempts (relative risk range, 1.10–7.65) had a higher risk of engaging in a broad range of health and social risk behaviours. The associations between suicidal thoughts and behaviour and health and social risk behaviours were typically independent of socio-demographics and in many cases were also independent of depressive symptoms. Conclusion: Suicidal thoughts and behaviour overlaps with increased risk of engagement in a wide range of health and social risk behaviours, indicating the need for an alignment of broader public health interventions within clinical and community-based suicide prevention activities. The experience of suicidality may be an important catalyst for a broader psychosocial conversation and assessment of health and social risk behaviours, some of which may be modifiable. These behaviours may not carry an imminent risk of premature death, such as from suicide, but they carry profound health and social consequences if left unaddressed.

Substance abuse is common issue in youth and may influence youth's suicide thoughts or actions. It can lead to social isolation, low self-esteem, loss of work or school, estrangement from family and friends – all these may create a core of stresses that may lead to suicidal tendencies.Analysis addictive factors correlating with occurrence of suicidal thoughts and behaviours among adolescents at the age of 18–20.Study was based on authorial, previously validated questionnaire, included 16 questions about suicidal thoughts. Questionnaires were filled by adolescents (age 18–20) of 21 Secondary Schools in Katowice.From the group of 965 adolescents, 28.8% had suicidal thoughts. From all respondents: 31.3% smoked, 92.7% drunk alcohol, 16.0% used legal hights and 30.9% – drugs; 35.8% of smoking adolescents had suicidal thoughts. In group of non-smoked adolescents – 25.6%. There was statistical significant difference (P = 0.0012) between these groups. Among adolescents who drunk alcohol, 28.8% had suicidal thoughts. In the group of non –drinking alcohol adolescents – 27.9%. There was no statistical significant differences (P = 0.88233) between these groups. There were 43.7% adolescents with suicidal thoughts who used legal hights. In the group of non-users of legal hights, 26.0% adolescents had suicidal thoughts.Adolescents that are using examined stimulants are in the group risk of suicidal thoughts and autodestructive behaviours. Results show the need of psychiatric and psychotherapeutic support that is aiming to prevent suicides and autodestructive behaviours in this group. Screening questionnaires that are assessing the problem of addiction within the adolescent may help to identity persons with suicidal tendencies.Disclosure of interestThe authors have not supplied their declaration of competing interest.

BackgroundDespite efforts to predict suicide risk in children, the ability to reliably identify who will engage in suicide thoughts or behaviours has remained unsuccessful.AimsWe apply a novel machine-learning approach and examine whether children with suicide thoughts or behaviours could be differentiated from children without suicide thoughts or behaviours based on a combination of traditional (sociodemographic, physical health, social–environmental, clinical psychiatric) risk factors, but also more novel risk factors (cognitive, neuroimaging and genetic characteristics).MethodThe study included 5885 unrelated children (50% female, 67% White, 9–11 years of age) from the Adolescent Brain Cognitive Development (ABCD) study. We performed penalised logistic regression analysis to distinguish between: (a) children with current or past suicide thoughts or behaviours; (b) children with a mental illness but no suicide thoughts or behaviours (clinical controls); and (c) healthy control children (no suicide thoughts or behaviours and no history of mental illness). The model was subsequently validated with data from seven independent sites involved in the ABCD study (n = 1712).ResultsOur results showed that we were able to distinguish the suicide thoughts or behaviours group from healthy controls (area under the receiver operating characteristics curve: 0.80 child-report, 0.81 for parent-report) and clinical controls (0.71 child-report and 0.76–0.77 parent-report). However, we could not distinguish children with suicidal ideation from those who attempted suicide (AUROC: 0.55–0.58 child-report; 0.49–0.53 parent-report). The factors that differentiated the suicide thoughts or behaviours group from the clinical control group included family conflict, prodromal psychosis symptoms, impulsivity, depression severity and history of mental health treatment.ConclusionsThis work highlights that mostly clinical psychiatric factors were able to distinguish children with suicide thoughts or behaviours from children without suicide thoughts or behaviours. Future research is needed to determine if these variables prospectively predict subsequent suicidal behaviour.

ObjectivesSeveral studies have shown increased suicide rates among veterinarians. We investigated the self-reported prevalence of suicidal thoughts and behaviours and contributing and independent factors associated with suicidal thoughts and behaviours among veterinarians in Norway.DesignCross-sectional, nationwide survey.Participants2596 veterinarians in Norway (response rate: 75%).Main outcome measurePaykel’s five-item questionnaire.ResultsIn total, 27% (n=682/2567) of veterinarians in Norway felt that life was not worth living during the last year, 5% (n=139/2562) had serious suicidal thoughts, and 0.2% (n=6/2537) had attempted suicide. Female veterinarians reported significantly higher prevalence of suicidal feelings and thoughts than males. For serious suicidal thoughts, women had nearly twice the prevalence as their male colleagues (6.2% (n=108/1754) vs 3.6% (n=28/766), χ2: 6.5, p=0.011). Independent factors associated with serious suicidal thoughts were being single (OR 1.76, 95% CI 1.13 to 2.72, p<0.05), negative life events (OR=1.43, 95% CI 1.22 to 1.68, p<0.001) and the presence of mental distress (OR 2.75, 95% CI 2.14 to 3.52, p<0.001). The veterinarians related their serious suicidal thoughts to work and personal problems, and a lesser degree to family, social and other problems. Nearly twice as many women (53%, n=57/108) as men (28%, n=7/25) reported work problems as the most important contributing factor to their serious suicidal thoughts (χ2: 4.99, p=0.03). 4% (n=6/139) reported work problems as the only factor of importance.ConclusionsVeterinarians in Norway have relatively high prevalence of suicidal feelings and thoughts, including serious suicidal thoughts. In multivariable analyses, the individual factors were more important than work-related ones, while work problems were the most reported contributing factor to serious suicidal thoughts by the veterinarians themselves. The role of gender and specific work-related factors should be further investigated to better understand the complexity of suicidal behaviour among veterinarians.

Tinnitus (the perception of sound in the absence of any corresponding external source) is highly prevalent and can be distressing. There are unanswered questions about how tinnitus, suicidal thoughts, and suicidal behaviours co-occur and interact. To establish the extent of scientific literature, this scoping review catalogued primary reports addressing the associations between tinnitus, suicidal ideation, attempted suicide, and death by suicide. We searched OvidSP, Medline, EMBASE, PsycINFO, CINAHL, Google Scholar, EThoS, and ProQuest for all studies and case reports on ideation and/or attempted and/or completed suicide in the context of tinnitus. Twenty-three studies were included, and data were charted according to study type. Several epidemiological and other observational studies gave evidence of risk factors and an association between suicidal ideation, suicidal behaviour, and tinnitus. However, there was no evidence of the direction of causality. Qualitative studies are indicated to explore the patient’s experience and understand the dynamics of any interaction between tinnitus and suicidal thoughts and behaviours. A theory-informed model of tinnitus and suicide needs to be developed to inform the development of interventions and how tinnitus patients are supported clinically.

We still lack knowledge about what implications gender has in suicidal behaviours. This is particularly true when it comes to women's suicides and suicidal attempts. In this article I argue that one reason for this is that suicidology has had a "gender neutral" approach to it's research object. This has often meant that studies on men have been generalized to the whole "suicide population". When gender has been on the agenda, it has been treated in a problematic way. Through a critical analyses of the studies that have been done in this field, the article is able to show that the knowledge gap existing in suicidology is filled with traditional theories of gender roles. That is, mythology enters the knowledge-empty spaces. The article also discusses the serious consequenses this has for women seeking help fortheir suicidal thoughts. In spiteofthefact that women are overrepresented concerning both suicidal attempts and serious sucicidal thoughts, suicidology still doesn't picture womens suicidal behaviours as one of its primary issues.

AimMental Health workers bear responsibility for preventing suicide in their client group. Survey studies have indicated that staff can be seriously adversely affected when a client suicides. The aim of the current study is to describe and evaluate the effects on mental health (MH) workers of their ongoing role in managing suicidal behaviours and to identify the thoughts and feelings associated with this role.MethodA survey was administered to 135 MH workers via an on-line self-report vehicle. The survey comprised standardised measures of anxiety and burnout as well as a questionnaire developed for this study concerning perceptions and attitudes to suicide and suicide prevention.ResultsFactor analysis of 12 retained items of the questionnaire identified three factors: 1) preventability beliefs (beliefs about suicide being always and/or permanently preventable); 2) associated distress (stress/anxiety about managing suicidal behaviour); and 3) the prevention role (covering views about personal roles and responsibilities in preventing suicidal behaviours). Analysis of these factors found that many MH workers experience an elevation of stress/anxiety in relation to their role in managing suicidal behaviours. This distress was associated with the emotional exhaustion component of burnout. Measures showed adverse responses were higher for outpatient than inpatient workers; for those who had received generic training in suicide prevention: and for those who had experienced a workplace related client suicide.ConclusionThere is a need for the development of appropriate self-care strategies to alleviate stress in MH workers exposed to suicide.

BackgroundPrevious literature has demonstrated a strong association between cigarette smoking, suicidal ideation and suicide attempts. This association has not previously been examined in a causal inference framework and could have important implications for suicide prevention strategies.AimsWe aimed to examine the evidence for an association between smoking behaviours (initiation, smoking status, heaviness, lifetime smoking) and suicidal thoughts or attempts by triangulating across observational and Mendelian randomisation analyses.MethodFirst, in the UK Biobank, we calculated observed associations between smoking behaviours and suicidal thoughts or attempts. Second, we used Mendelian randomisation to explore the relationship between smoking and suicide attempts and ideation, using genetic variants as instruments to reduce bias from residual confounding and reverse causation.ResultsOur observational analysis showed a relationship between smoking behaviour, suicidal ideation and attempts, particularly between smoking initiation and suicide attempts (odds ratio, 2.07; 95% CI 1.91–2.26; P < 0.001). The Mendelian randomisation analysis and single-nucleotide polymorphism analysis, however, did not support this (odds ratio for lifetime smoking on suicidal ideation, 0.050; 95% CI −0.027 to 0.127; odds ratio on suicide attempts, 0.053; 95% CI, −0.003 to 0.110). Despite past literature showing a positive dose-response relationship, our results showed no clear evidence for a causal effect of smoking on suicidal ideation or attempts.ConclusionsThis was the first Mendelian randomisation study to explore the effect of smoking on suicidal ideation and attempts. Our results suggest that, despite observed associations, there is no clear evidence for a causal effect.

AbstractBackgroundDeath by suicide is often preceded by attempted suicide, suicidal ideation and non-suicidal self-injury. These extreme thoughts and behaviours have been considered in terms of a continuum of suicidality. Little known research, however, has considered a suicide continuum that extends beyond these extreme thoughts and behaviours and incorporates a much wider array of phenomena that may vary in severity and may constitute a broader negative self-evaluation (NSE) continuum.MethodHarvesting key indicators of NSE from a British epidemiological survey (N = 8580), the current study used exploratory factor analysis, confirmatory factor analysis and factor mixture modelling to (i) identify the dimensional structure of NSE in the general population and (ii) profile the distribution of the resultant NSE dimensions. Multinomial logistic regression was then used to differentiate between classes using an array of risk variables, psychopathology outcome variables and a suicide attempt indicator.ResultsA 4-factor model that reflected graded levels of NSE was identified; (F1) Low self-worth & subordination (F2) depression, (F3) suicidal thoughts, (F4) self-harm (SH). Seven classes suggested a clear pattern of NSE severity. Classes characterised by higher levels across the dimensions exhibited greater risk and poorer outcomes. The greatest risk for suicide attempt was associated with a class characterised by engagement in SH behaviour.ConclusionsLow self-worth, subordination and depression, while representative of distinct groups in the population are also highly prevalent in those who entertain suicidal thoughts and engage in SH behaviour. The findings promote further investigation into the genesis and evolution of suicidality and internal threat.

Suicide is the second leading cause of death among 15- to 29-year-olds. Male youths have higher risk for suicide compared to females, whereas females are at greater risk for suicidal ideation and suicide attempts. Mental disorders have been identified as one of the strongest predictors of suicidal thoughts and behaviours (STB). Among youth, sexual minority (LGB) individuals are a high-risk subpopulation for STB. Evidence about suicide risk and whether there are possible mechanisms by which some factors increase or diminish the risk according to gender or sexual orientation is scarce. The general aim of this thesis is to provide new evidence about the risk for STB and potential risk and protective factors for STB among adolescents/young adults; and how these vary according to gender and sexual orientation. To achieve this aim, we carried out a systematic review of the literature, followed by a meta-analysis, and analysed empirical data from a cohort study (UNIVERSAL: University and Mental Health) of Spanish university students. The results show that female adolescents/young adults are at greater risk for suicide attempts compared to males. As already well known through information provided by vital statistics registries, males are at higher risk for suicide. Risk and protective factors for STB differ by gender. Internalising disorders and interpersonal difficulties increase the risk among females, while externalizing disorders, hopelessness and some stressful life events (e.g. death of any of the parents) increase the risk for males. In addition, family and peer support are found to be protective factors for suicidal ideation, but only among females. LGB youth have higher risk for STB compared to heterosexuals. This risk is mediated by childhood maltreatment, bullying and a previous history of any mental disorders. Perceived sexual orientation discrimination increases the risk for any mental disorder, which, in turn, carries risk for suicidal ideation. This thesis has provided new evidence on a wide range of individual and community risk and protective factors for STB among adolescents and young adults, and has detected important differences across gender and sexual orientation with regard to STB risk. These results suggest that there is a need to combine preventive strategies focusing on individual risk factors (e.g. early diagnosis and treatment of mental disorders) with those with a public health population-level risk prevention approach (e.g. reinforcing community protective factors), as well as to consider the specific needs of high-risk groups.
El suicidio es la segunda causa principal de muerte entre los jóvenes de 15 a 29 años. Los hombres jóvenes tienen un mayor riesgo de suicidio en comparación con las mujeres, mientras que las mujeres tienen un mayor riesgo de ideación suicida e intentos de suicidio. Los trastornos mentales se han identificado como uno de los predictores más importantes de los pensamientos y la conducta suicida. Entre los jóvenes, los gais, lesbianas y bisexuales (LGB) son una subpoblación de alto riesgo para los pensamientos y la conducta suicida. La evidencia sobre el riesgo de suicidio y los posibles mecanismos por los cuales algunos factores aumentan o disminuyen el riesgo según el género o la orientación sexual es escasa. El objetivo general de esta tesis es proporcionar nueva evidencia sobre el riesgo de los pensamientos y la conducta suicida y sus posibles factores de riesgo y protección en los adolescentes/adultos jóvenes; y cómo estos varían según el género y la orientación sexual. Para lograr este objetivo, se realizó una revisión sistemática de la literatura, seguida de un meta-análisis y se analizaron datos empíricos de un estudio de cohorte (UNIVERSAL: Universidad y Salud Mental) de estudiantes universitarios españoles. Los resultados muestran que las mujeres adolescentes/adultas jóvenes tiene mayor riesgo de intentos de suicidio en comparación con los hombres. Como se conoce a través de la información proporcionada por los registros de estadísticas de mortalidad, los hombres tienen mayor riesgo de suicidio. Los factores de riesgo y protección para los pensamientos y la conducta suicida difieren según el género. Los trastornos mentales internalizantes y las dificultades interpersonales aumentan el riesgo entre las mujeres, mientras que los trastornos externalizantes, la desesperanza y algunos eventos vitales estresantes (p. ej. la muerte de cualquiera de los padres) aumentan el riesgo en los hombres. Además, el apoyo familiar y de los pares son factores protectores para la ideación suicida, pero solo entre las mujeres. Los jóvenes LGB tienen un mayor riesgo de pensamientos y conducta suicida en comparación con los heterosexuales. Este riesgo está mediado por el maltrato infantil, el bullying y los antecedentes de cualquier trastorno mental. La discriminación debida a la orientación sexual aumenta el riesgo de cualquier trastorno mental, lo que, a su vez, conlleva un mayor riesgo de ideación suicida. Esta tesis proporciona nueva evidencia sobre una amplia gama de factores de riesgo y protección a nivel individual y comunitario para los pensamientos y la conducta suicida en los adolescentes/adultos jóvenes, y ha identificado diferencias importantes de acuerdo al género y la orientación sexual en el riesgo de los pensamientos y la conducta suicida. Los resultados sugieren que es necesario combinar estrategias preventivas centradas en factores de riesgo individuales (p. ej. Diagnóstico temprano y tratamiento de los trastornos mentales) con aquellas con un enfoque de salud pública y de prevención a nivel poblacional (p. ej. Reforzar los factores protectores a nivel de la comunidad), así como considerar las necesidades específicas de los grupos de alto riesgo.

Background: The effects of physical illness and psychiatric disorders have been extensively investigated in fatal and non-fatal suicidal behaviour. Although different study designs have focused on the independent roles of physical and mental illness in suicidality, few studies have examined the relationship between co-occurring physical and mental health conditions (multimorbidity) and suicide risk. Considering the paucity of research, the present doctoral programme of research aimed to investigate if populations with physical/mental multimorbidity have an increased risk of suicidal thoughts and behaviours, compared to those with neither physical nor mental health conditions, and further investigate if this effect is stronger than either of the health conditions alone. Methods: Based on the well-established terms of comorbidity and multimorbidity, the effect of co-occurring health conditions in the risk of suicidality was investigated by undertaking an overview of reviews and a systematic review. Four empirical studies of existing datasets were conducted in order to explore if suicidality (suicidal thoughts and suicide attempts) varies as a function of physical/mental multimorbidity. Two national mental health surveys from the United Kingdom (National Psychiatric Morbidity Survey 2000, n = 8575; Adult Psychiatric Morbidity Survey 2007, n = 7389) and one cohort study carried out in Scotland (West of Scotland Twenty-07 study, n = 4510) were used and both cross-sectional and prospective study designs were employed. Results: Findings indicated that those with physical/mental multimorbidity are more likely to have suicidal thoughts and attempt suicide, compared to those with neither physical nor mental health conditions. The results were consistent: having both physical and mental health conditions did not increase risk of suicidal thoughts or attempts, beyond the risk conferred by mental illness alone. Having only physical health conditions was not associated with either suicidal thoughts or suicide attempts. Conclusions: Overall, the findings suggest a potential risk of suicidality for populations with physical/mental multimorbidity. Although multimorbidity was a predictor of suicidality, it did not increase the risk of any suicide-related outcome more than mental health conditions alone. The current findings highlight that health care professionals should focus on populations who have reached services for non-mental health issues, but subsequently develop mental illness, as this pattern of multimorbidity could potentially be a risk factor for suicidal thoughts and behaviours. Further research is needed to better understand the risk of suicide in individuals with physical/mental multimorbidity.

Researchers have previously identified substance use and borderline personality disorder as factors that increase risk for suicidal thoughts and behaviors. This study explored the relationship between these factors in samples of students and individuals seeking outpatient treatment. Supplemental data collected via the internet (MTurk) also looked at experiential avoidance (EA) with the Avoidance and Fusion Questionnaire for Youth. The Structured Clinical Interview for the DSM-IV, Alcohol Use Disorders Identification Test, Scale for Suicide Ideation, and Personality Assessment Inventory- Borderline Features Scale elicited information regarding severity and/or frequency of substance use, suicidal thoughts, and borderline features respectively. Additionally, the Psychiatric Diagnostic Screening Questionnaire was administered to the UNT sample. The UNT sample analyses indicate substance use moderates, strengthening, the relationship between borderline features and current suicidal thoughts. However, severity of suicidal thoughts was lower for individuals high in both borderline features and substance use disorder symptoms compared to those low in borderline features and high in substance use symptoms. The MTurk sample analyses suggest substance use functions as a mediator. A robust relationship existed between substance use severity and EA, showing substance use as a behavioral marker for EA. In conclusion, concurrent treatment of substance use and borderline personality features would be beneficial in reducing risk for suicidal thoughts. Further investigation into the role and utility of addressing EA is warranted.

Objective: The relationship between alcohol use and suicidal behaviors is well-accepted, but less is known about the contribution of its early initiation. This study was designed to test the association of early alcohol initiation versus later initiation with suicidal ideation and attempt in an ethnically diverse sample. Methods: The Collaborative Psychiatric Epidemiology Surveys (CPES), 2001-2003 (n = 20,013), database was used. A total of 13,867 participants were selected included 56.9% females and 43.1% males. Race and ethnicity were reported as 28.8% non-Hispanic White, 39.1% Black, 20.3% Latino, and 11.9% Asian. Logistic regression analyses tested the associations between early (< =14 years) and later (> =15) age alcohol initiation with suicide ideation and attempts. Alcohol initiation was indexed by self-report of the first time that any alcohol product was consumed. Potential confounders were controlled. Results: Early alcohol initiation was associated with higher odds (AOR = 3.64, 95% CI [2.51, 5.28]) of suicide ideation as compared with adults who had initiated > = age 15 (AOR = 2.11, 95% CI [1.46, 3.04]). Early age initiation was also associated with higher odds (AOR = 3.81, 95% CI [2.02, 7.18]) of lifetime suicide attempt versus later age initiators (AOR = 2.03, 95% CI [1.08, 3.79]). Significant differences were found between early and later age of initiation. Conclusion: Early age of alcohol initiation has profoundly increased odds of suicide ideation or attempt. It is critical that effective prevention programs for children and their caregivers be implemented to prevent or delay alcohol initiation and lessen the risk for future suicidal behaviors.

The purpose of this investigation was to examine the role of distress tolerance in suicidality among Black college students. It was hypothesized that (1) individuals with low levels of distress tolerance would report higher levels of suicide ideation; (2) individuals with high levels of distress tolerance would report greater suicide attempts; (3) social support would moderate the relationship between distress tolerance and suicide ideation; (4) social support would moderate the relationship between distress tolerance and suicide attempts; and that (5) family and peer support would act as distinct buffers against suicidality. These hypotheses were tested by surveying 47 undergraduate university students (female = 49%; mean age = 22.45). Participants completed packets with self-report measures that included: the Self-Harm Behavior Questionnaire, the Distress Tolerance Scale, the Child and Adolescent Social Support, and demographics. Results suggested that individuals with low levels of distress tolerance showed greater history of self-harm behavior when compared to individuals with high levels of distress tolerance. Results indicated that social support moderated the relationship between distress tolerance level and history of self-harm behaviors. Results also indicated that family support acted as significant protective factor against suicidality.

El objetivo general de esta tesis es estudiar la frecuencia (prevalencia, incidencia y persistencia) de los pensamientos y conductas suicidas entre los universitarios españoles, así como aportar evidencia sobre su asociación con una amplia gama de factores de riesgo y protección. Presentamos los resultados de las encuestas en línea basal (T1) y de seguimiento 12 meses (T2) del proyecto de investigación UNIVERSAL “Universidad y Salud Mental”, del que se han analizado 2.118 estudiantes de primer año de 5 universidades españolas. La prevalencia a los 12 meses es del 9,9% para idea suicida, 5,6% plan y 0,6% intento. La incidencia a los 12 meses de idea suicida por primera vez en la vida es del 3,2%; y la persistencia del 21,2% de los casos con ideas suicidas previas. El principal factor asociado con la idea suicida es la presencia de un posible trastorno del estado de ánimo los últimos 12 meses, junto con: psicopatología de los padres, haber sufrido maltrato emocional o negligencia en la infancia o sufrir una agresión sexual o violación. Las relaciones positivas en la infancia y el sentido de pertenencia a la universidad se asociaron con una menor presencia de idea suicida. Los resultados de esta tesis señalan una elevada prevalencia e incidencia de pensamientos y conductas suicidas en universitarios de primer año en España. Estos resultados sugieren la necesidad de llevar a cabo estrategias de motorización y prevención dirigidas a tanto a los estudiantes con posibles trastornos del estado de ánimo como al conjunto de la población de universitarios de primer año.
The main objective of this thesis is to assess the frequency (prevalence, incidence and persistence) of suicidal thoughts and behaviors among Spanish university students, as well as to provide evidence about their association with a wide range of risk and protective factors. Baseline (T1) and 12-month follow-up (T2) online surveys data from the UNIVERSAL research project "University and Mental Health" were used for this thesis. A total of 2,118 first-year students from 5 Spanish universities were analyzed. Prevalence at 12 months is 9.9% for suicidal ideation, 5.6% for plan and 0.6% for attempt. 12-month first-onset incidence is 3.2%; and persistence of suicidal ideation among those with previous suicidal ideation is 21.2%. The main factor associated with suicidal ideation is presence of a possible mood disorder in the previous 12 months, along with: parents’ psychopathology, having suffered emotional abuse or neglect in childhood, or suffering a sexual assault or rape. Positive childhood relationships and a higher sense of university membership are associated with a lower likelihood of suicidal ideation. Results from this thesis show that there is a high prevalence and incidence of suicidal thoughts and behaviors among first-year Spanish university students. These results suggest to the need to carry out motorization and prevention strategies focused on students with mood disorders as well as on the entire population of first-year university students

AbstractInsight from individuals with lived experience suggests that unnecessary hospitalization decreases the likelihood that individuals will disclose suicidal thoughts and behaviors to providers. To improve engagement and create better outcomes for individual who experiences a suicidal crisis, clinicians should receive suicide-specific training that emphasizes personalized care for each patient that matches their level of risk. Individualized safety planning and options other than inpatient hospitalization should be prioritized when suicidal thoughts and behaviors are disclosed to providers and when the level of risk does not warrant hospitalization. This will allow youth who make such disclosures to get access to effective and minimally restrictive interventions while preventing trauma and harm. Finally, healthcare systems must focus on investing resources and research into alternatives to inpatient care as treatment for suicidal crises, and individuals with lived experience will be an essential asset to this process.

AbstractPharmacologic and non-pharmacologic treatment of children and adolescents with suicidal thoughts and behavior have lagged behind the already sparse treatments for suicidal adults, leaving many at-risk youths undertreated. The following is a review of the neurobiological research literature focused on suicide risk in children and adolescents. Topics include the relationship of suicide risk to neuroimaging findings, impulsivity, genetics, and treatment approaches, including selective serotonin reuptake inhibitors (SSRIs), lithium, ketamine, and transcranial magnetic stimulation (TMS).

Cannabis is the most commonly used substance that remains federally illegal in the United States. With its rising legality in many states across the nation, it is important to understand the influence that cannabis can have on a user's physical and mental well-being. According to the Centers for Disease Control and Prevention (2022), suicide is among the top nine leading causes of death in the United States for individuals aged 10 to 64. Research suggests that individuals diagnosed with depression who experience suicidal ideation, plans, and attempts is positively correlated with both daily cannabis use and non-daily cannabis use and that this correlation is more significant in women than in men (Han et al., 2021). This study seeks to generalize this finding by investigating both the prevalence and intensity of 30-day cannabis use amongst individuals who endorse suicidal thoughts in the last 12 months and the moderating effect of gender identity on the relationship between 30-day cannabis use and suicidal thoughts in the last 12 months. This study will utilize a sample of college students who completed the American College Health Association-National College Health Assessment Survey sometime between the Fall academic semester of 2019 to the Spring academic semester of 2021. The survey was completed on campuses across the United States with data collected on students’ health habits, behaviors, and perceptions. This data set consists of 198,848 participants aged 18 to 98 years of age (MAge = 23.16), with 66.27% of participants identifying as cisgender women and 30.83% identified as cisgender men. Within the dataset, 40.30% of participants endorsed thinking about or planning suicide in the last 12 months and 19.34% of participants reported using cannabis in the last 30 days. We anticipate that individuals who reported cannabis use in the last 30 days will show an increase in suicidal ideation in the last 12 months compared to individuals who reported never using cannabis. Similarly, we anticipate that gender identity will moderate and strengthen the relation between cannabis use and suicidal ideation in such that individual who identify as women will have a stronger and more significant correlation between cannabis use and suicidal ideations compared to individuals who identify as men. If these results are supported, future research will be warranted to further investigate how the nuanced interaction between gender identity and cannabis use increases our understanding of the upward trend in suicidality among individuals with and without mental health diagnoses.

The Covid-19 pandemic has posed great challenges for all social work clients and their families, caregivers, medical and mental health care providers, and support systems. Social work clients are vulnerable to the detrimental effects of restrictions (isolation) and are confronted with adverse consequences from distancing and new rules, which may trigger or worsen psychiatric disorders (anxiety, depression, self-harm, substance abuse, suicidal behaviour and thoughts), according to recent literature (March-May 2020). This paper presents the results of a survey of Romanian social workers aimed at identifying both positive and negative effects of the Covid-19 pandemic on both social workers and their clients.

Introduction: Comorbidity of personality and substance use disorders, including prescription drug abuse, is common in clinical practice. We present a case report of a patient with a diagnosis of a personality disorder and symptoms of modafinil dependence. Case report: 33-year-old male, single, unemployed for over 3 years. He started psychological treatment at the age of 17 due to anxiety symptoms. Since the age of 21 he had irregular consultations with several psychiatrists in private practice. He received several diagnoses, including anxiety, depression, obsessive compulsive disorder and personality disorder. In one of the consultations, modafinil 100mg was prescribed twice daily to alleviate depressive symptoms. The patient gradually increased the dose to up to 1000mg a day. He presented for a consultation in our psychiatric hospital claiming he had been trying to reduce the dose of the drug. He had a fast speech and showed aggressive behavior, reported intrusive suicidal and homicidal thoughts, and had recent episodes of aggressive behavior requiring police intervention. Hospitalization was proposed for discontinuation of the psychostimulant medication. Discussion and conclusions: Modafinil is a central nervous system stimulant, pharmacologically different from other stimulants. It is approved in Portugal for the treatment of excessive somnolence associated with narcolepsy. However, it is often used off label in several countries as an adjunctive treatment for symptoms of depression or fatigue associated with cancer or neurologic diseases. Although modafinil is typically associated with low abuse potential, case reports have been presented in the literature describing patterns of abuse and dependence of this drug. It is important that clinicians prescribe it with caution, bearing in mind patients' past consumption patterns and traits and that may increase the risk of abuse, such as high novelty seeking and reward sensitivity and low agreeableness and conscientiousness.

Discharge from hospital following a suicidal crisis is a crucial time for the provision of high-quality aftercare. Providing aftercare services has been demonstrated to reduce the risk of further suicide attempts, deaths, and to increase the likelihood that people will seek professional support when experiencing suicidal thoughts. this Evidence Check aims to provide a review of the evidence for suicide aftercare services and effective characteristics of these services, focusing on comprehensive aftercare services being provided in Australia. This review identified 53 relevant studies in the peer reviewed literature. As a search of the grey literature did not uncover a comprehensive overview of available services, an online survey of PHNs was undertaken to address this gap. Most studies found that comprehensive aftercare programs and brief intervention programs are effective (63% and 70% of studies respectively). There have been no comprehensive dismantling studies published which evaluate the effectiveness of individual components or characteristics of suicide aftercare programs. However, this review identified components and characteristics that were common to programs that effectively reduced suicide-related outcomes such as reattempts.

Review question / Objective: Is electroacupuncture a safe therapy for the treatment of depression? Is electroacupuncture effective for the treatment of depression, as compared with sham control, or conventional drugs？ Condition being studied: Depression is a mood disorder that causes sufferers to feel sadness, decreased interest, guilt, self-blame, loss of energy, and experience sleep disorders such as insomnia. People suffering from depression even feel they have no way out and have suicidal thoughts. In the United States, the prevalence of a major depressive disorder is 16.2%1-3. The 2010 Global Burden of Disease Study identified major depression as the second leading cause of disability worldwide and a leading cause of the burden of suicide and ischaemic heart disease. At present, depression patients are mainly treated with antidepressants, but the efficacy is extremely unstable. Studies have shown that acupuncture can help improve symptoms in patients with depression, but these clinical studies have not been systematically evaluated, and further confirmation is needed to confirm the efficacy of electroacupuncture in treating depression.

Emerging data suggest that strengthening positive social bonds and improving social integration might reduce suicidal behaviours in youth to date; little research has studied the effect of social integration, on suicide behaviours, with reference to a young person’s social network structure — namely, an individual’s position within their network and the patterns of relationships among members of the network.

In 2011, Esposito-Smythers et al. reported that integrated outpatient cognitive-behavioural therapy (I-CBT) significantly reduced substance use, suicidal behaviours, and the rate of health service use compared with enhanced treatment-as-usual (E-TAU) in adolescents with co-occurring alcohol or drug use disorder and suicidality.
