Ever since Star Wars became a franchise -- immediately after the release of A New Hope - -there have been a wide variety of efforts to structure, explain, and systematize its ever-expanding narrative universe. Going under various names, including the "Extended Universe," the Star Wars franchise beyond the core films has always been somewhat contentious, both as markers of fan knowledge and as potential for nearly endless corporate profit. What was originally kept in a vast archive of narrative data at Lucasfilm led to a hierarchy of canons and canonicity, which was distributed back to fans via elaborate reference books and contained within Star Wars plots as holocrons kept as lore in Jedi and Sith temples. This paper examines the fate of that canonicity in the wake of the 2012 purchase of Lucasfilm by Disney, exploring how various aspects of these already commodified franchise elements have been contained and redeployed as Disney both continued the Skywalker saga of films and elaborated the Star Wars franchise in new ways.

The importance of science fiction in contemporary cultural studies can hardly be underestimated, no more than it can be denied. Many narratives emerging out of the world of science fiction have become fully integrated within the contemporary cannon of popular understanding, mythology and reference. Amongst these narratives, perhaps no story is more fully integrated with contemporary culture than the original Star Wars saga. More current in the contemporary social imagination than the norse sagas, or those of ancient greece, Star Wars shares many of their epic qualities. The focus on the heroic characteristics of individuals, for example, against the backdrop of a great conflict between forces of good and evil, in which the righteous and the virtuous prevail is the standard narrative of many epic cultures. Indeed, this is the origin of classic notions of virtue, which stay with us to this day (MacIntyre, 2007). In that sense, this saga could be understood as yet another permutation of a story which has been told since time immemorial. Yet, as with the classical sagas, one must be sensitive to problematic aspects within their narratives; to the version of morality which they promote, and the ways in which they do so. This main focus in this essay will be just one such problem: the (mis)use of memory within the narrative of the original Star Wars saga, and deception as it relates to the myth of the Jedi.

The Mandalorian is a very popular science-fiction show (two seasons, 2019–2020) set in the famous Star Wars Universe. Studies have shown that myth and religious thought played a crucial role in the creation of the Star Wars Universe. This article continues that tradition, albeit from a particular perspective that highlights religious language: by viewing The Mandalorian through a New Testament lens, it is argued that while many elements of popular culture reference Biblical or mythological sources, The Mandalorian’s use of these referents illustrates the way in which ancient religious and New Testament literature are still very much a shared phenomenon. Both The Mandalorian and the New Testament share certain timeless topoi: a mysterious character with extraordinary abilities, a connection to life-giving powers of the universe that give extraordinary abilities, choosing a certain way of life and the costs thereof, and also themes such as “debt”, “redemption”, and “beliefs” and how these are challenged. By using these themes, The Mandalorian presents itself as a modern myth.

Abstract Popular culture can influence debates over security policy. This article studies the use of Star Wars in the debate over Reagan's Strategic Defense Initiative (SDI). The term Star Wars was widely used to refer to SDI during debates in the 1980s. Scholars have pointed to both disabling and enabling effects of popular culture on political debates. This article refines these effects and introduces a third effect that comes from the widespread use of popular culture: a neutralizing effect that turns popular culture references into descriptive shorthands. Studying these dynamics contributes to our understanding of why popular culture is or is not used as a framing device and how it is used and perceived by decision-makers. I rely on content analysis of newspaper articles, congressional records, and political speeches and incorporate findings from elite interviews and the norm literature to conduct my analysis. I find that critics capitalized on linkages between the movie franchise and the policy to frame Reagan's missile defense system as Star Wars and to criticize his policy proposal. This science-fictionalization soon constituted the sociopolitical context surrounding Reagan's missile defense initiative. Most decision-makers perceived the Star Wars label to have a disabling effect as it cast doubt on the viability and desirability of SDI. Opponents initially tried to mobilize this effect, whereas most proponents argued against the label or refrained from using it. Few tapped into the enabling potential of Star Wars to communicate the potential benefits of SDI. The introduction of the professional term SDI helped proponents mitigate science-fictionalization in expert settings, while among the public the widespread use of the Star Wars label neutralized its meaning and turned it into a descriptive shorthand.

The article discusses inspirations from Far Eastern cultures present in George Lucas’ Star Wars saga. This cinematic story is treated as a postmodernist text, in which references to earlier cultural works appear. More attention is given to the sequel trilogy, not explored in academic circles as much as the two earlier trilogies. These inspirations are analysed through the lens of Edward Said’s concept of orientalism, with additional focus on the specific ways in which it was employed in the United States of America. In the conclusion, some reflections are devoted to audience reactions to the newest trilogy and the Eastern elements present therein.

In 2015, a series of memes appeared on Twitter under the hashtag #HumanitarianStarWars. Combining still images from the original Star Wars movies with ironic references to humanitarian/development jargon and institutions, the memes presented a humorous reflection on the modern aid industry. While memetic content has become an increasingly scrutinized area in digital culture studies—particularly with regard to unbounded and anonymous online communities, and popular discursive contestation—this article examines #HumanitarianStarWars to shed light on the possibilities and problematics of social media auto-critique undertaken by “insiders” in a particular professional realm. Keeping in mind critiques of the racial and imperial connotations of the (Western) pop-culture mythology itself, the article explores the use of the Star Wars franchise as a vehicle for commentary on an industry at work in the “Global South.” It highlights an ambiguous process of meaning-making that can be traced through the memes’ generation, circulation, and re-mediation. Although the memes provide a satirical self-reflection on practitioners’ experiences and perspectives of power relations in the global development industry, certain tendencies emerge in their remixing of this Hollywood universe that may reinforce some of the dynamics that they ostensibly critique. The article argues that examination of the ideological ambivalence of an institutional micro-meme can yield valuable insights into tensions playing out in professional social media spaces where public/private boundaries are increasingly and irrevocably blurred.

Aim Our goal was to audit the quality of the ward round documentation in our Plastic Surgery department by using the SAFE Ward Round Tool of the RCS Edinburgh’s as a reference standard, and to create an in-house pro-forma based on results and discussion. Method An initial cycle based on the SAFE Tool was undertaken with prospective audit of individual daily ward round entries. A sticker pro forma was introduced and re-audit was done using the same criteria. Based on results and discussion, the pro-forma was further improved. Re-audit was performed to assess percentage of completion of its contents. Results The first cycle showed 47% (n = 42) completion rate and re-audit after implementation of the sticker found a rise up to 70% (n = 42). The third cycle examining solely sticker completion yielded a compliance of 88% (n = 61). This improvement reflected to the enthusiastic comments received from staff working in allied specialties. Conclusions Significant lapses in daily ward round documentation were revealed by our methodology. A sticker pro-forma, which we have named the Surgical Tool for the Assessment of Rounds (STAR), was introduced and provided measurable and sustainable improvements on our daily ward round practice. That had as a result the safeguarding of patient safety in the frame of Good Medical Practice. We suggest same methodology to be followed based on the SAFE Ward Round Tool for surgical ward rounds improvement in all the surgical and interventional specialties particularly when there is a component of emergency admission in their daily practice

AbstractIn star-forming environments, shock-compressed magnetic fields occur in cloud-cloud collisions, in molecular clouds exposed to supernova remnants (SNRs), and in photo-dissociation regions (PDRs). Besides their dynamical role, they increase the cosmic ray flux above the Galactic average, and the trapped particles contribute to the heating of the shocked gas. The associated dust emission is polarized perpendicularly to the sky plane projection of the field, Bsky. In edge-on viewed shock planes, highly ordered polarization patterns are expected. In search of such a signature, the dust emission from the Orion bar (a prototypical PDR) and from a molecular cloud/SNR interface (IC443-G) was studied with a λ870μm polarimeter at the APEX (Wiesemeyer etal 2014 and references therein). While our polarization map of OMC1 confirms the hourglass shape of Bsky (e.g., Schleuning 1998, Houde etal 2004), a deep integration towards the Orion bar reveals an alignment of Bsky with the shock forming in response to the wind and to the ionizing radiation from the Trapezium cluster (Fig. 1). This structure suggests a compressed magnetic field accelerating cosmic-ray particles, a scenario proposed by [Pellegrini et al. (2009)] to explain the high excitation temperature of rotationally warm H2 and CO (Shaw et al. 2009, Peng et al. 2012, respectively).

Feast scenes from the main Star Wars episodes play an important role the creation of the plot and characters. In the paper I divide them into three types. In the first part I discuss the scenes in which a feast or a meal is a pretext for the dialogues revealing the characters’ past (the conversation between Luke and his uncle) or predict their future (little Anakin and Qui-Gon during the dinner; Anakin and Padmé’s conversation predicting their fall). In the second part I present eating scenes through the authors create or underline the characteristic features of the protagonists (comism of Jar Jar Binks and Chewbacca; Jabba’s gluttony; dehumanization of the black characters by the fact that they lack any contact with food) or build their social status (Rey’s scavenger meal; feasts at tha palaces of Jabba and Amidala; Jedi master’s ascetic meal on Dagobah; clones’ canteen). To analyse the scenes I use theories developed by food anthropologists (Mary Douglas, Claude Levi-Strauss, and Katarzyna Łeńska-Bąk) and make references to classical mythology and the Bible. In the last part I raise questions about consumer’s the identity of the character who eats juxtaposing devouring and eating scenes (why are Eworks considered humanoids whereas Wampa and Rancor are considered monsters since they all eat human flesh?).

In contemporary consumer culture, branding is the term given to the creation of an image or text (visual, aural, textural or multi-sensory) intended to represent a commodity or product sold by a producer or service provider. This product’s commercial viability depends largely on the way it is presented (via branding) to its target market. The aim of this research report is to show that music used consciously as a branding medium, with special reference to film music (in its commodified form), has become a brand in itself, as opposed to merely a component of a multi-modal commercial product. Through analyses of a central film music theme from Star Wars: Episode IV, composed by John Williams, I aim to identify what I will term `audio-branding techniques’ within the music, thereby showing how music has come to be regarded as a brand. The audio branding techniques will relate directly to the four levels of analysis that I propose to conduct. The nature of branding implies the presence of three entities in the cultural and commercial `transaction’ that takes place: namely, the service provider (creator), the product (commodity) and the target market (consumer). I intend to argue that, as a result of powerful creative collaborations between John Williams and his various directors (not to mention his own unique talent), this composer’s film music has increasingly become an audio brand which is almost commensurate with the brand status of the film itself. Williams’s ability to create a symbiotic relationship between a music brand and that of a film has set him apart from most other contemporary art and commercial composers. As a result, it is not simply the actors, directors and producers associated with a movie that induce one to buy tickets to see it, but Williams’s independent audio branding style as well. I thus aim to prove that his film music is an audio brand independent of, and yet also allied with, other brands.

AbstractStarting sometime in 2008/2009 one expects to be able to take a glimpse at physics at the TeV scale. This will be done through the Large Hadronic Collider (LHC) at CERN, Geneva. It will be a result of an unprecedented coordinated international scientific effort. This chapter is written in 2007. It is essentially inviting disaster to spell out in full detail what the current various theoretical speculations on the physics are, as well motivated as they may seem at this time. What I find of more value is to elaborate on some of the ideas and the motivations behind them. Some may stay with us, some may evolve and some may be discarded as the results of the experiments unfold. When the proton antiproton collider was turned on in the early eighties of the last century at Cern the theoretical ideas were ready to face the experimental results in confidence, a confidence which actually had prevailed. The emphasis was on the tremendous experimental challenges that needed to be overcome in both the production and the detection of the new particles. As far as theory was concerned this was about the physics of the standard model and not about the physics beyond it. The latter part was left safely unchallenged. That situation started changing when the large electron positron (LEP) collider experiments also at Cern were turned on as well the experiments at the Tevatron at Fermilab. Today it is with rather little, scientifically based, theoretical confidence that one is anticipating the outcome of the experiments. It is less the method and foundations that are tested and more the prejudices. It is these which are at the center of this chapter. Some claim to detect over the years an oscilatory behavior in the amount of conservatism expressed by leaders in physics. The generation in whose life time relativity and quantum mechanics were discovered remained non-conservative throughout their life. Some of the latter developed eventually such adventurous ideas as to form as a reaction a much more conservative following generation. The conservative generation perfected the inherited tools and has uncovered and constructed the Standard Model. They themselves were followed by a less conservative generation. The new generation was presented with a seemingly complete description of the known forces. In order to go outside the severe constraints of the Standard Model the new generation has drawn upon some of the more adventurous ideas of the older generation as well as created it own ideas. In a way almost all accepted notions were challenged. In the past such an attitude has led to major discoveries such as relativity and quantum mechanics. In some cases it was carried too far, the discovery of the neutrino was initially missed as energy conservation was temporarily given up.

AbstractThis chapter explores the big-data-based production of knowledge on migration. Following Mol (2002) and Scheel et al. (2019), it is analysed how migration and migrants are enacted through big-data-based research papers. The emerging sub-discipline of big-data-based migration research enacts migration and migrants in multiple ways that open up possibilities to rethink migration. However, this multiplicity of migration is held together by reference to three migration narratives—demography, integration and humanitarianism—which stand in stark contrast to these alternative enactments, as they all frame migration as something that needs to be governed. As the research papers aim at contributing to these research fields, they inscribe themselves into these migration narratives and thereby adopt the assumption of migration as an object of government.

AbstractThe Antikythera Mechanism is the oldest extant complex geared device, and an amazing example of an early analogue computer. It was built approximately 2150 years ago. The device was operated manually by a user, who would set a date on a dial. All necessary calculations were made using a set of gears (at least 39), while the results were displayed on several scientific scales. The Mechanism was used to calculate the diurnal and annual motion of the Sun, the Moon and probably the planets among the stars. It implemented the astronomical knowledge of ancient Greeks about the motion of these celestial bodies with astonishing accuracy, taking into account the anomalous orbit of the Moon using a system of eccentric gears. It could also predict eclipses of the Sun and the Moon from the Saros period, which was found in one of its scales. It calculated the dates of the major crown games that took place in ancient Greece (e.g. the Olympic Games). Finally, it was accompanied by an extended User’s Manual. More than 20 references to astronomical mechanisms can be found in classical literature from 50 BC to 500 AD. In this study, the first approach for a holistic documentation of the Antikythera Mechanism is presented.

AbstractWhile all international human rights treaties apply to children, only the Convention explicitly elaborates who is defined as a child. Article 1 defines the child as a human being who is below the age of 18 years. Majority is set at age 18 unless, under domestic law, it is attained earlier. During the negotiations of the text of the Convention, there was significant debate regarding definitions of both the commencement and the ending of childhood. The initial text, proposed by the Polish Government, drawing on Principle 1 of the UN Declaration of the Rights of the Child, 1959, provided no definition of childhood at all (Office of the United Nations High Commissioner for Human Rights and Rädda barnen (Society: Sweden), 2007, p. 301). However, government delegates on the Working Group immediately highlighted the need for clarification. The first revision of the text therefore proposed that a child is a human being from birth to the age of 18 years unless majority is attained earlier. However, with regard to the beginning of childhood, the Working Group were unable to come to a consensus. Rimmer's major sculptural works, such as St. Stephen, Falling Gladiator, Dying Centaur, and Osirus (destroyed), were created for exhibition and in response to the international neoclassical movement. In different ways, they are actually critiques of the rage for neoclassicism. Much of what Rimmer was trying to do is conveyed in his teaching, and he used his exhibited art as an extension of this. He wanted an art based not on copying from antique casts or from life but, rather, on the artist's own imagination so that the work is self-expressive. The fact that the man in Falling Gladiator assumes an impossible position is an instance of his insistence on the imaginative. The St. Stephen and a cast of the Falling Gladiator were exhibited in Paris at the Salon des Refusés, where the Gladiator created a stir as it seemed, wrongly, to be a cast of a live person. Rimmer broke new ground in producing fragmented human figures with an antique reference, such as his Osiris, a classical-Greek-looking nude male without parts of his arms. They resembled the broken ancient sculpture of the present rather than of the revered past. Originally Osiris had the head of a hawk. As with his pictures, Rimmer also was unusual in frankly accepting and portraying abnormalities as in his Seated Man (Despair). The late Fighting Lions, showing a male and female in vicious combat is arguably an allegory of male dominance. As an original thinker, Rimmer, more than once, explored the problem of expressing the spiritual in the material, most effectively in his relatively abstract Torso, which is an attempt to show the divine awakening or creation of a human soul. Following the Bible, the plaster cast retains the effect of a man’s torso having been crudely fashioned from clay. Perhaps just as unexpected was his plan for a colossal sculpture, Tri Mountain (never executed), which amalgamated the effect of three men and three hills as a symbol of the city of Boston. His one major public statue is the over-life-size Alexander Hamilton on Commonwealth Mall in Boston.

AbstractThe colonial experience has profoundly marked both Europe and Africa, the coloniser and the colonised. From the start of the scramble for Africa in the fifteenth century, African culture, language, identity, and agency have been systematically minimised and discounted while European supremacy has been amplified. This has set a tone that has persisted as the two struggle to achieve an equitable partnership. To this day, the language used by African and European leaders, writers, and influencers reflects that many are trapped in an outdated worldview that masks the true colonial legacy.It takes conscious work to do away with the heavy presence of colonial narratives and perceptions. This chapter reviews the works of many scholars who have attempted to do just this. From seminal books such as Born in Blackness (French, Born in Blackness: Africa, Africans, and the Making of the Modern World, 1471 to the Second World War, 2021), African Europeans (Otele, African European: An Untold History, 2020), and The EU and Africa (Adebajo & Whiteman, 2012) to others that frame the discussion in terms of the historical reasons for continuity in the stigmatisation of Africa, this chapter seeks to capture some of the critical insights of those who can help us construct a new narrative. Frantz Fanon’s and Amilcar Cabral’s theories on decolonising minds are also mentioned. Additionally, references to the current post-colonial academic debates are included.

Іn the scientific work, the authors made the first warning that during the recodification of civil legislation, it is first necessary to create a new version of the Civil Code of Ukraine (hereinafter – the Civil Code) and only after that start work on the project of the Civil Procedure Code of Ukraine (hereinafter – the Civil Procedure Code of Ukraine). However, work on the improvement of the Central Committee can be carried out in parallel and coordinated with the work on the improvement of the Central Committee, but this is extremely difficult to ensure. The authors believe that when starting work on a new edition of the CPC, one should first form its new structure with the systematization of relevant material, since the provisions of the current CPC are not laid out consistently.The judicial system of Ukraine should work within the limits of the law, but the state should find measures of influence on offenders and the demonstration of the will of the authorities is needed not for a short demonstration period, but on a permanent basis.Access to justice in civil proceedings in Ukraine should be discussed through the prism of whether a person with a minimum wage can protect his rights in court. Obviously not, because he cannot pay the large court fee. And this provision does not meet the requirements of Articles 3 and 55 of the Constitution of Ukraine, nor international standards, in particular, Articles 6 and 13 of the European Convention on the Protection of Rights and Fundamental Freedoms, since low-income citizens are, in fact, deprived of the right to access to justice and protection of their rights. Therefore, it is necessary to revise not only the norms of the Code of Civil Procedure, but also the laws of Ukraine «On court fees» and «On free legal aid».The right to appeal to the court for protection should not depend on the level of the plaintiff’s financial condition, if Ukraine is a rule of law state, but should be guaranteed to every person who needs it.Оne of the main issues of the new CPC project should be coordination and interaction of certain types of civil court proceedings, in particular, such as injunctive and claim, therefore the authors proposed ways to solve these issues.The authors paid a lot of attention to the analysis of minor disputes, the criteria for classifying certain categories of cases as minor. The impossibility of classifying labor disputes as minor is substantiated with reference to the Constitution of Ukraine. A reasoned opinion was expressed regarding the impracticality of paying the same court fee, which is paid in both «significant» and minor cases, taking into account the order of their consideration. It is proposed to improve the civil justice system taking into account Recommendation No. R (95) 5 of 02.07.1995 of the Committee of Ministers of the Council of Europe.list of shortcomings of the current legislation on civil procedure is given. When applied by the court, they should be considered as an abuse of law by the court. If the CPC does not record the court’s duty to respond to every argument of the party, then all other norms are a camouflage for the legally guaranteed arbitrariness of the judicial branch of government, no matter what proceeding we are talking about. If a person is limited in the right to be heard, then what judicial procedure can we talk about...Certain debatable issues are pointed out, which are subject to discussion and resolution when creating a new version of the CPC. The authors believe that the given arguments and arguments should be submitted for public discussion in order for the new version of the Code of Civil Procedure to be better, not worse, than the current regulatory act.But when implementing the idea of updating and recodifying the Central Committee, making changes to the Central Committee will become objectively inevitable, and not only to the Central Committee. The same applies to Ukraine’s accession to the EU, as it will be necessary to bring the CPC into compliance with the recommendations existing in the European community. no But you cannot make random amendments to the adopted draft law, and the introduction of fragmentary amendments quite often leads to an imbalance of the regulatory act as a whole.

AbstractBuilding on the two rounds of exposure studies with human milk coordinated by the World Health Organization (WHO) in the mid-1980s and 1990s on polychlorinated biphenyls (PCB), polychlorinated dibenzo-p-dioxins (PCDD), and polychlorinated dibenzofurans (PCDF), five expanded studies on persistent organic pollutants (POPs) were performed between 2000 and 2019. After the adoption of the Stockholm Convention on POPs (the Convention) in 2001, WHO and the United Nations Environment Programme (UNEP) collaborated in joint studies starting in 2004. The collaboration aimed at provision of POPs data for human milk as a core matrix under the Global Monitoring Plan (GMP) to assess the effectiveness of the Convention as required under Article 16. Over time, the number of analytes in the studies expanded from the initial 12 POPs targeted by the Convention for elimination or reduction to the 30 POPs covered under the Stockholm Convention and two other POPs proposed for listing as of 2019. Many of these chemicals have numerous congeners, homologous groups, isomeric forms, and transformation products, which significantly extends the number of recommended analytes.In the studies between 2000 and 2019, 82 countries from all five United Nations regions participated, of which 50 countries participated in more than one study. For the human milk samples of the 2016–2019 period, results are available for the full set of 32 POPs of interest for the Convention until 2019: (i) the 26 POPs listed by the start of the study in 2016; (ii) decabromodiphenyl ether [BDE-209] and short-chain chlorinated paraffins [SCCP] as listed in 2017; (3) dicofol and perfluorooctanoic acid [PFOA] as listed in 2019; (4) medium-chain chlorinated paraffins [MCCP] and perfluorohexane sulfonic acid [PFHxS] as proposed for listing. This is a unique characteristic among the core matrices under the GMP.Four key messages can be derived: These studies are an efficient and effective tool with global coverage as key contributor to the GMP. After collection of a large number of individual samples (usually 50) fulfilling protocol criteria, pooled samples are prepared using equal aliquots of individual samples (physical averaging) and are considered to be representative for a country, subregion or subpopulation at the time of the sampling. The analysis of pooled representative human milk samples by dedicated Reference Laboratories meeting rigorous quality criteria contributes to reliability and comparability and reduces uncertainty of the analytical results. Additionally, this concept is very cost-effective. These studies can be used for regional differentiation based on concentrations of individual POPs between and within the five UN Regional Groups (African Group, Asia-Pacific Group, Eastern European Group, Group of Latin American and Caribbean Countries; Western European and Others Group). For some POPs, a wide range of concentrations with up to three orders of magnitude between lower and upper concentrations was found, even for countries in the same UN region. Some countries had levels within the usual range for most POPs, but high concentrations for certain POPs. Findings of concentrations in the upper third of the frequency distribution may motivate targeted follow-up studies rather than if the observed level of a POP is found in the lower third of frequency distribution. However, the concentration of a POP has also to be seen in context of the sampling period and the history and pattern of use of the POPs in each country. Therefore, results are not intended for ranking of individual countries but rather to distinguish broader patterns. These studies can provide an assessment of time trends, as possible sources of variation were minimized by the survey concepts building on two factors (sampling design; analysis of the pooled samples by dedicated Reference Laboratories). The estimation of time trends based on comparison of median or mean concentrations in UN Regional Groups over the five surveys in five equal four-year periods between 2000 and 2019 provides a first orientation. However, the variation of the number of countries participating in a UN Regional Group in a certain period can influence the median or mean concentrations. Thus, it is more prudent to only use results of countries with repeated participation in these studies for drawing conclusions on temporal trends. The reduction rates in countries should be seen in context with the concentration range: A differentiation of high levels and those in the range of the background contamination is meaningful. If high levels are found, sources might be detected which could be eliminated. This can lead to significant decrease rates over the following years. However, if low background levels are reported, no specific sources can be detected. Other factors for exposure, e.g. the contamination of feed and food by air via long-range transport and subsequent bioaccumulation, cannot be influenced locally. However, only very few time points from most individual countries for most POPs of interest are available, which prevents the derivation of statistically significant temporal trends in these cases. Yet, the existing data can indicate decreasing or increasing tendencies in POP concentrations in these countries. Furthermore, pooling of data in regions allows to derive statistically significant time trends in the UN Regional Groups and globally. Global overall time trends using the data from countries with repeated participation were calculated by the Theil–Sen method. Regarding the median levels of the five UN Regional Groups, a decrease per 10 years by 58% was found for DDT, by 84% for beta-HCH, by 57% for HCB, by 32% for PBDE, by 48% for PFOS, by 70% for PCB, and by 48% for PCDD and PCDF (expressed as toxic equivalents). In contrast, the concentrations of chlorinated paraffins (CP) as “emerging POPs” showed increasing tendencies in some UN Regional Groups. On a global level, a statistically significant increase of total CP (total CP content including SCCP [listed in the Convention in 2017] and MCCP [proposed to be listed]) concentrations in human milk of 30% over 10 years was found. The studies can provide the basis for discussion of the relative importance (“ranking”) of the quantitative occurrence of POPs. This, however, requires a differentiation between two subgroups of lipophilic substances ([i] dioxin-like compounds, to be determined in the pg/g [=ng/kg] range, and [ii] non-dioxin-like chlorinated and brominated POPs, to be determined in the ng/g [=μg/kg] range; both groups reported on lipid base) and the more polar perfluorinated alkyl substances (PFAS); reported on product base [as pg/g fresh weight] or on volume base [ng/L]. For this purpose, results for the complete set of the 32 POPs of interest for the 2016–2019 period were considered. By far, the highest concentrations of lipophilic substances were found for DDT (expressed as “DDT complex”: sum of all detected analytes, calculated as DDT; maximum: 7100 ng/g lipid; median: 125 ng/g lipid) and for chlorinated paraffins (total CP content; maximum: 700 total CP/g lipid; median: 116 ng total CP/g lipid). PCB was next in the ranking and had on average an order of magnitude lower concentrations than the average of the total CP concentrations. The high CP concentrations were caused predominantly by MCCP. If the pooled samples from mothers without any known major contamination source nearby showed a high level of CP, some individual samples (e.g. from local population close to emission sources, as a result of exposure to consumer products or from the domestic environment) might even have significantly higher levels. The lactational intake of SCCP and MCCP of the breastfed infant in the microgram scale resulting from the mothers’ dietary and environmental background exposure should therefore motivate targeted follow-up studies and further measures to reduce exposure (including in the case of MCCP, regulatory efforts, e.g. restriction in products). Further, due to observed levels, targeted research should look at the balance among potential adverse effects against positive health aspects for the breastfed infants for three groups of POPs (dioxin-like compounds; non-dioxin-like chlorinated and brominated POPs; PFAS) regarding potentially needed updates of the WHO guidance. As an overall conclusion, the seven rounds of WHO/UNEP human milk exposure studies are the largest global survey on human tissues with a harmonized protocol spanning over the longest time period and carried out in a uniform format. Thus, these rounds are an effective tool to obtain reliable and comparable data sets on this core matrix and a key contributor to the GMP. A comprehensive set of global data covering all POPs targeted by the Stockholm Convention, in all UN Regional Groups, and timelines covering a span of up to three decades allows to evaluate data from various perspectives. A widened three-dimensional view is necessary to discuss results and can be performed using the three pillars for assessments of the comprehensive data set, namely: analytes of interest; regional aspects; time trends. This can identify possible problems for future targeted studies and interventions at the country, regional, or global level. Long-term trends give an indication of the effectiveness of measures to eliminate or reduce specific POPs. The consideration of countries with repeated participation in these studies provides the best possible database for the evaluation of temporal trends. The continuation of these exposure studies is important for securing sufficient data for reliable time trend assessments in the future. Therefore, it is highly recommended to continue this monitoring effort, particularly for POPs that are of public health concern.

‘Deconstructing The Starships’ references a speech originally given at the June 1988 presentation of the Arthur C. Clarke Award, at the Institute of Contemporary Arts. The speech discusses science fiction’s penetration into popular culture and its inclusion in 20th century mainstream fiction. It also analyses the structure and methodology of a science fiction novel, looking at the characterisation, narrative and literary conventions used in order to develop an understanding of the requirements of a science fiction text. The chapter references Star Wars and Star Trek throughout, and uses the two franchises to associate the Starship Enterprise with US Navy nuclear submarines in the Cold War, thus mirroring science fiction with reality.

Wavefront sensors currently used in adaptive optics operate only on relatively bright sources. One of the most sensitive sensor ever built is the Hartmann sensor of the European COME-ON adaptive optics system developed for infra-red astronomy [1]. It is equipped with an electron-bombarded CCD and can stabilize images on a 3.6 m telescope with a mag 13 star. With some improvement in the optical transmission, the sensor should be able to operate up to a mag. 15 star. Because only a few astronomical objects are bright enough or are close enough to a bright star to enable the sensor to work, it is generally thought that artificial laser guide stars are necessary to benefit from the full potential of adaptive optics in astronomy. However the aforementioned sensors are far from the theoretical sensitivity limit An image stabilizer developed for the CFH telescope (HR Cam [2]) was recently equipped with four photon-counting avalanche photodiodes in a quadrant detector, and image stabilization was demonstrated on a mag 19.5 star. Such a sensitivity allows images to be sharpened over the whole sky. The UH wavefront sensor was developed to reach HR Cam performance on tip/tilt compensation together with higher order compensation using brighter natural reference sources. It is estimated that under average Mauna Kea seeing conditions a Strehl ratio improvement by a factor 10 can be obtained over 30% of the sky at 1.6μm with a 3.6 m telescope.

<p>The Third Panama Crossing, also known as the Atlantic Bridge, is a stay cable bridge that crosses the Panama Straight on the eastern coast of the Panama. It features double pylon, double stay cable plan structure, a concrete deck and a main span of 530m built, which makes it the world longest stay cabled concrete span.</p><p>The full concrete design was required due to the excessive corrosiveness of the atmosphere at the Limon Bay location. This required a special approach to design a durable stay cable system, which included the qualification of new thick anticorrosion coatings, as well as specific anchorage set up allowing a long lasting and renewable protection of the critical anchorage components.</p><p>All cables, the longest ones reaching almost 300m, were installed with internal dampers. A tailor made set up allowed to deliver large strokes, ranging from 50 to 100mm, required to cater for cable rotations under service loads. They were qualified through testing for both an increased durability and resistance against corrosion and tested in situ for high performance with an energy dissipation ranging between 5% and 6.5%.</p>

Optical frequency standards operating at 1.55 µm are required for fiber-optic wavelength division multiplexed communication systems and for 'cold start' coherent optical communication. Unfortunately, transitions near 1.55 µm are rare and difficult to probe. One may overcome the difficulties associated with frequency references at 1.55 µm by second harmonic generation (SHG) and locking to Rb D2 line near 780 nm. This reference was already used to stabilize 1560 nm laser diodes using the internally generated second harmonic of diode lasers and by bulk external SHG in KNbO3, however the SHG power levels were in the pW range and the nW range, respectively, hence locking to Doppler broadened line was possible, but the power level was not sufficient for locking to sub-Doppler lines.

Honeywell's Earth Reference Attitude Determination Sensor (ERADS) is a wide field of view ultraviolet imaging sensor for satellite attitude determination. The sensor was designed for small satellite applications1, where size, weight, and cost are the primary drivers. Use of the ultraviolet region of the spectrum provides a unique capability to simultaneously image stars, Earth's limb (day or night), and other celestial objects, and to tolerate the sun in the field of view. This is because the solar output is lower relative to the other objects of interest in this waveband, and eliminates the need for expensive and cumbersome sun shields. The field of view consists of a 10° annular region at 71° from axis, and a 10 degree center field. Higher accuracy attitude information is obtained from measurements of stellar location. The accuracy with which these measurements can be made is limited by the number of pixels on the focal plane, the blur spot dimensions, and the photon count rates. The detector array of the sensor was a megapixel CCD. The field of view of the sensor was effectively increased by making use of an array of six folding mirrors, and the field remapping effectively increased the pixel density in the area of interest. The optimal blur diameter for the centroiding algorithms proved to be the width of three pixels, if a photon rate of 50 per star for fourth magnitude stars could be maintained.

Development of a robust and cost-effective method of joining dissimilar materials could provide a critical pathway to enable widespread use of multi-material designs and components in mainstream industrial applications. The use of multi-material components such as steel-aluminum and aluminum-polymer would allow design engineers to optimize material utilization based on service requirements and could often lead to weight and cost reductions. However, producing an effective joint between materials with vastly different thermal, microstructural, and deformation responses is highly problematic using conventional joining and/or fastening methods. This is especially challenging in cost sensitive, high volume markets that largely rely on low cost joining solutions. Friction stir scribe technology was developed to meet the demands of joining materials with drastically different properties and melting regimes. The process enables joining of light metals like magnesium and aluminum to high temperature materials like steel and titanium. Viable joints between polymer composites and metal can also be made using this method. This paper will present the state of the art, progress made, and challenges associated with this innovative derivative of friction stir welding in reference to joining dissimilar metals and polymer/metal combinations.

The high creep-fatigue loading of fast starting steam turbines is investigated. Based on real turbine operation, but assuming worse conditions like increased notch factors, typical stress-strain cycles of the rotor are calculated. These an-isothermal cycles consists of phases of different start-types (cold start, warm start and hot start), of steady-state operation, shut-down and stand-still. Experiments, performed with these cycles, are evaluated with different methods. For the LCF fraction of the lifetime consumption, several reference temperatures are compared, whereby the so-called ‘corner temperature’ is preferably incorporated, which is the temperature at the end of the compression phase. The creep fraction of the lifetime consumption is firstly assessed with the time fraction rule, either considering the midlife cycles only or considering each individual cycle, either considering all phases of the cycles or the steady state operation phase only. Then, the ductility exhaustion method for the creep damage is applied, after defining the ductility for several experiments.

The field of entrepreneurial start-ups is broadly studied by scholars. To increase the amount of successful entrepreneurs, the stimulation for more starting entrepreneurs is a referenced field of research and subject to governmental stimulation. More entrepreneurs lead to a bigger gross national product, which has a positive impact on an economic region. More new entrepreneurs do not mean more successful entrepreneurs. According to prior research, in half of the cases of entrepreneurial exit, the exit was more or less avoidable. Beside the stimulation of more people to become an entrepreneur the reduction of the entrepreneurial failure in the post start-up phase of a venture is worth full to consider in order to get more successful entrepreneurs. This PhD proposal wants to shed light on that phase of the entrepreneurial process and want to identify effective interventions to prevent Entrepreneurial Failure.

Abstract. In metal casting simulation the Heat Transfer Coefficient (HTC) is unknown as it depends on melt and mold materials, on the casting modulus at different regions of the casting and on local conditions at the mold-casting gap. In this paper, thermocouple measurements at three regions of a brass investment casting provided reference cooling curves. A genetic algorithm (GA) determined the optimum 3-step time-dependent HTC for the whole of the casting in a simulation program for which cooling curves are as close as possible to the reference curves. The resulting prediction of solidification times is satisfactory but prediction of qualitative characteristics such as start / end of solidification in different regions was not accurate enough.

Introduction: In 2021, it is estimated that Brazil will have 66,280 new cases of breast cancer. Unfortunately, around 55% of the cases treated by the Brazilian Unified Health System (SUS) are symptomatic and are in advanced stages. The long time before biopsy and the late start of treatment contribute to the progression of these tumors, with worsening prognosis. In order to reduce the time to start therapy, we implemented a model of integrated multidisciplinar care with high resolution (CARE). Objectives: The purpose of this work is to demonstrate the main barriers and the solutions found to reduce the time of treatment start at the institution, according to Law no. 12,732, which recommends the start of therapy within 60 days of diagnosis, preventing the progression of the disease and reducing mortality. Methods: The information was registered at the Women´s Health Reference Center (CRSM) data colletion system from the first day of care until the start of therapy (surgery or chemotherapy), in a cross-sectional cohort of 8,357 patients from January 2011 to December 2018. Patients referred were seen at CARE and underwent consultation with an anesthesiologist or oncologist in the service having a previous appointment to start treatment. Results: After the integration of anestheologists and clinical oncologists, in 2014, there was a significant reduction (from 55 to 38 days) in the time of treatment start. Discussion: CARE started integrated care with biopsy on the same day as the initial consulation in 2006, reducing returns and time to diagnosis from 90 to 15 days. CRSM serves more than 1,200 new cases/year, about 30% of SUS cases in the city of São Paulo (SP). A 12-week surgical delay for all breast cancer patients from one year (for example in the COVID 19 pandemic) would lead to 1,400 excess deaths in the United Kingdon and 6,100 in the United States, assuming surgery was the first choice of treatment for 83% of the patients. Conclusion: Our experience shows that the training of mastologists to perform a biopsy in the first consultation, that the public/private partnership to obtain the anatomopathological and immunohistochemical results in 15 days and that a multidisciplinary integrated care is possible, allowing the average onset of treatment to be close to 30 days, which may reduce the proportion of advanced cases in SUS.

Abstract In 2009, the Vito field was discovered in more than 4,000 ft of water approximately 150 miles offshore from New Orleans, Louisiana. The project produces from reservoirs nearly 30,000 feet below sea level. This paper outlines the approach taken to develop the Vito facilities operating model (Operating Model) and prepare for start-up and ramp-up. This paper is part of a Vito Project series at OTC 2023, and the other papers are listed in the references. In 2015 the project faced significant financial hurdles and went through a refresh of the concept design to reduce cost and simplify while maintaining safety as a top priority. This re-design resulted in reduced redundancy and operational flexibility including reducing host personnel on board (POB) capacity to 60 personnel. To prepare to start-up and operate the facility with this simplified design, a proactive approach was adopted across four key areas: Operating Model, maintenance strategy, digital building blocks, and start-up ramp-up (SURU) planning. An Operating Model was developed to enable efficient execution of maintenance and operations activities with the reduced POB. This model leverages multiskilling of onsite personnel and enabling digital technologies. The limited POB necessitated development of a maintenance strategy that is both lean and comprehensive with high utilization of the base crew. With local performance standards as the foundation of the strategy, a Reliability Centered Maintenance (RCM) study was performed to validate and further optimize the defined strategy, which relies heavily on asst specific maintenance tasks and onshore sparing due to minimal redundancy and space on host for spares. Digital Twin technology has been leveraged to create a virtual mirror image of the Vito facility that is a central repository of equipment data and documentation. This enables virtual planning of work which reduces POB needs by providing the ability to perform walkdowns, take measurements and identify access issues without being on site. Augmented reality technology supplements this by streaming the viewpoint of offshore staff directly to onshore teams to perform troubleshooting, diagnose issues, and inspect without requiring physical presence. To manage well start-up and to optimize long term recovery, a start-up ramp-up model was implemented with a focus on proactive actions to optimize start-up. This led to long term lease of onshore caverns to eliminate requirements for temporary unloading equipment during start-up. A phased commissioning and start-up plan was developed with gas buyback brought on ahead of well start-up to pressure up vessels and commission compressors. Additionally, a real time simulator was built to test procedures, train personnel and walkthrough the start-up plan.

The Nordic Council of Ministers’ vision is for the Region to be the most sustainable and integrated region in the world by 2030. Cross-border labour market mobility in the Nordic Region will play an important role in achieving that goal. In this working paper, we share the latest data on labour market mobility across national borders in the Nordic Region in the form of both migration and commuting. We also present findings from a review of current literature on labour market mobility in the Nordic Region and present an analytical framework for exploring potential improvements to it. The working paper was written by Anna Lundgren, Senior Research Fellow and Ágúst Bogason, Research Fellow at Nordregio. A reference group comprising stakeholders from cross-border regions and Info Norden (see Appendix) provided input. The paper represents our contribution to research in this area and we invite others to comment on it. The project will present its final results in 2023. This working paper is part of the research project “Re-start Nordic competence mobility” under the thematic group of Green, resilient and innovative regions, which is part of the regional co-operation programme funded by the Nordic Council of Ministers. The first phase of the project resulted in a chapter, “Labour market mobility between the Nordic countries” in State of the Nordic Region 2022.

Observers widely acknowledged the lack of divisive Islamist populism in Indonesia’s 2024 Presidential Elections. This was in stark contrast to the 2019 elections in which Prabowo Subianto, the case study of this article and new leader of Indonesia, led a campaign that overtly supported Islamist interests and actors, and deepened religio-ethnic tensions in society. Despite this acknowledgement, it remains unclear if religion was still operationalized – albeit differently – in his most recent campaigning efforts. This article therefore seeks to examine if religion was politicized and performed by Prabowo in 2024 and contrast the findings with 2019 to address how and why his instrumentalization of religion varied significantly. Applying a discursive-performative lens, discourse analysis will be used to determine if and how religion featured in a sample of Prabowo Subianto’s speeches (six speeches in total, three from each election campaign). Specifically, this analysis will explore how references to religion and a religious community reflect a) his political goals and b) the political community he is attempting to engage. It will also discuss these findings in the context of contemporary populism studies.

Objectives. To review the association between malnutrition and clinical outcomes among hospitalized patients, evaluate effectiveness of measurement tools for malnutrition on clinical outcomes, and assess effectiveness of hospital-initiated interventions for patients diagnosed with malnutrition. Data sources. We searched electronic databases (Embase®, MEDLINE®, PubMed®, and the Cochrane Library) from January 1, 2000, to June 3, 2021. We hand-searched reference lists of relevant studies and searched for unpublished studies in ClinicalTrials.gov. Review methods. Using predefined criteria and dual review, we selected (1) existing systematic reviews (SRs) to assess the association between malnutrition and clinical outcomes, (2) randomized and non-randomized studies to evaluate the effectiveness of malnutrition tools on clinical outcomes, and (3) randomized controlled trials (RCTs) to assess effectiveness of hospital-initiated treatments for malnutrition. Clinical outcomes of interest included mortality, length of stay, 30-day readmission, quality of life, functional status, activities of daily living, hospital acquired conditions, wound healing, and discharge disposition. When appropriate, we conducted meta-analysis to quantitatively summarize study findings; otherwise, data were narratively synthesized. When available, we used pooled estimates from existing SRs to determine the association between malnutrition and clinical outcomes, and assessed the strength of evidence. Results. Six existing SRs (including 43 unique studies) provided evidence on the association between malnutrition and clinical outcomes. Low to moderate strength of evidence (SOE) showed an association between malnutrition and increased hospital mortality and prolonged hospital length of stay. This association was observed across patients hospitalized for an acute medical event requiring intensive care unit care, heart failure, and cirrhosis. Literature searches found no studies that met inclusion criteria and assessed effectiveness of measurement tools. The primary reason studies did not meet inclusion criteria is because they lacked an appropriate control group. Moderate SOE from 11 RCTs found that hospital-initiated malnutrition interventions likely reduce mortality compared with usual care among hospitalized patients diagnosed with malnutrition. Low SOE indicated that hospital-initiated malnutrition interventions may also improve quality of life compared to usual care. Conclusions. Evidence shows an association between malnutrition and increased mortality and prolonged length of hospital stay among hospitalized patients identified as malnourished. However, the strength of this association varied depending on patient population and tool used to identify malnutrition. Evidence indicates malnutrition-focused hospital-initiated interventions likely reduce mortality and may improve quality of life compared to usual care among patients diagnosed with malnutrition. Research is needed to assess the clinical utility of measurement tools for malnutrition.

The article is devoted to youth preparation for family life issue and the necessity in gender approach application for this issue in the domestic scientific discourse. The relevance of the problem in gender aspect is highlighted from the perspective of the working group of Ministry of Education and Science of Ukraine development of “Strategy of Gender Equality in Education”. Based on references to the publications of educators and psychologists, it was concluded that scientists consider the problem of youth preparation for family life, mainly from the point of psychological readiness of young men and women to fulfill marital obligations. In the pedagogical aspect, scientists focus on creating pedagogical conditions and using the system of educational influences on the formation of youth readiness to start a family. The analysis of the views presented in the scientific discourse on the youth preparation for family life issue gave us grounds to find a few contradictions between the psychological and pedagogical consideration of the problem and the modern life realities. Traditional notions of the family are in significant transformation state. The approaches proposed by psychological and pedagogical thought are aimed to preserve traditional notions of the family which do not stand the test of time. In the system of youth preparation for family life, the authors identified a few gender issues: education according to traditional role behavior models, the effect of hidden curriculum in education, pressure on girls about marriage, the system of requirements for the role and "place" of women in the family according to her status (daughter, wife, daughter-in-law, mother), etc. It is suggested that science should be more flexible on this issue and should propose models and approaches that ensure the sustainable development of the State (including in demographic terms) while guaranteeing fundamental rights and freedoms, including the right to free development and self-realization without discriminatory restrictions

Objectives. To assess the use, effectiveness, and implementation of telehealth-supported provider-to-provider communication and collaboration for the provision of healthcare services to rural populations and to inform a scientific workshop convened by the National Institutes of Health Office of Disease Prevention on October 12–14, 2021. Data sources. We conducted a comprehensive literature search of Ovid MEDLINE®, CINAHL®, Embase®, and Cochrane CENTRAL. We searched for articles published from January 1, 2015, to October 12, 2021, to identify data on use of rural provider-to-provider telehealth (Key Question 1) and the same databases for articles published January 1, 2010, to October 12, 2021, for studies of effectiveness and implementation (Key Questions 2 and 3) and to identify methodological weaknesses in the research (Key Question 4). Additional sources were identified through reference lists, stakeholder suggestions, and responses to a Federal Register notice. Review methods. Our methods followed the Agency for Healthcare Research and Quality Methods Guide (available at https://effectivehealthcare.ahrq.gov/topics/cer-methods-guide/overview) and the PRISMA reporting guidelines. We used predefined criteria and dual review of abstracts and full-text articles to identify research results on (1) regional or national use, (2) effectiveness, (3) barriers and facilitators to implementation, and (4) methodological weakness in studies of provider-to-provider telehealth for rural populations. We assessed the risk of bias of the effectiveness studies using criteria specific to the different study designs and evaluated strength of evidence (SOE) for studies of similar telehealth interventions with similar outcomes. We categorized barriers and facilitators to implementation using the Consolidated Framework for Implementation Research (CFIR) and summarized methodological weaknesses of studies. Results. We included 166 studies reported in 179 publications. Studies on the degree of uptake of provider-to-provider telehealth were limited to specific clinical uses (pharmacy, psychiatry, emergency care, and stroke management) in seven studies using national or regional surveys and claims data. They reported variability across States and regions, but increasing uptake over time. Ninety-seven studies (20 trials and 77 observational studies) evaluated the effectiveness of provider-to-provider telehealth in rural settings, finding that there may be similar rates of transfers and lengths of stay with telehealth for inpatient consultations; similar mortality rates for remote intensive care unit care; similar clinical outcomes and transfer rates for neonates; improvements in medication adherence and treatment response in outpatient care for depression; improvements in some clinical monitoring measures for diabetes with endocrinology or pharmacy outpatient consultations; similar mortality or time to treatment when used to support emergency assessment and management of stroke, heart attack, or chest pain at rural hospitals; and similar rates of appropriate versus inappropriate transfers of critical care and trauma patients with specialist telehealth consultations for rural emergency departments (SOE: low). Studies of telehealth for education and mentoring of rural healthcare providers may result in intended changes in provider behavior and increases in provider knowledge, confidence, and self-efficacy (SOE: low). Patient outcomes were not frequently reported for telehealth provider education, but two studies reported improvement (SOE: low). Evidence for telehealth interventions for other clinical uses and outcomes was insufficient. We identified 67 program evaluations and qualitative studies that identified barriers and facilitators to rural provider-to-provider telehealth. Success was linked to well-functioning technology; sufficient resources, including time, staff, leadership, and equipment; and adequate payment or reimbursement. Some considerations may be unique to implementation of provider-to-provider telehealth in rural areas. These include the need for consultants to better understand the rural context; regional initiatives that pool resources among rural organizations that may not be able to support telehealth individually; and programs that can support care for infrequent as well as frequent clinical situations in rural practices. An assessment of methodological weaknesses found that studies were limited by less rigorous study designs, small sample sizes, and lack of analyses that address risks for bias. A key weakness was that studies did not assess or attempt to adjust for the risk that temporal changes may impact the results in studies that compared outcomes before and after telehealth implementation. Conclusions. While the evidence base is limited, what is available suggests that telehealth supporting provider-to-provider communications and collaboration may be beneficial. Telehealth studies report better patient outcomes in some clinical scenarios (e.g., outpatient care for depression or diabetes, education/mentoring) where telehealth interventions increase access to expertise and high-quality care. In other applications (e.g., inpatient care, emergency care), telehealth results in patient outcomes that are similar to usual care, which may be interpreted as a benefit when the purpose of telehealth is to make equivalent services available locally to rural residents. Most barriers to implementation are common to practice change efforts. Methodological weaknesses stem from weaker study designs, such as before-after studies, and small numbers of participants. The rapid increase in the use of telehealth in response to the Coronavirus disease 2019 (COVID-19) pandemic is likely to produce more data and offer opportunities for more rigorous studies.

Objective. To understand the evidence base for nutrition interventions delivered prior to or during cancer treatment for preventing and treating negative cancer and cancer treatment–related outcomes among individuals with or at risk for malnutrition. The primary purpose was to inform the National Institutes of Health (NIH) Pathways to Prevention workshop Nutrition as Prevention for Improved Cancer Health Outcomes, held July 26–28, 2022. Data sources. We searched Ovid Medline, Ovid Embase, and Cochrane Central Register of Controlled Trials to identify studies from 2000 through July 2022. We conducted grey literature searches to identify additional resources relevant to the associated costs or value (e.g., cost-effectiveness, cost-benefit) of nutrition interventions. Review methods. The review was guided by a set of Key Questions established by the NIH planning committee for the Nutrition as Prevention for Improved Cancer Health Outcomes workshop. We searched for studies that evaluated a broad range of nutrition interventions (e.g., dietary supplements, nutrition support, nutrition counseling) for preventing and treating negative outcomes of cancer and cancer-related treatment. Eligible studies included randomized controlled trials (RCTs) with enrollment ≥50 participants. We extracted basic study information from all eligible studies, then grouped studies by broad intervention and cancer types. We provide a detailed evidence map for all included studies, but conducted risk of bias and additional qualitative descriptions of outcomes for only those intervention and cancer types with a larger volume of literature. Results. We identified 9,798 unique references, with 206 studies from 219 publications reporting RCTs of nutrition interventions to potentially improve negative outcomes of cancer and cancer-related treatment. Two decades of randomized trial evidence on nutrition interventions for adults prior to and/or during cancer treatment primarily focused on dietary supplements, nutrition support (including oral nutrition supplements), and the route or timing of nutrition interventions for gastrointestinal and head and neck cancers in the inpatient setting. Most studies evaluated changes in body weight/composition, adverse events, length of hospital stay, and quality of life. Few studies were conducted within the U.S. setting. Among intervention and cancer types with a high volume of literature (n=114), which predominantly included studies in dietary supplements and nutrition support in gastrointestinal and head and neck cancers, 11 percent (n=12) were rated as low risk of bias (higher quality), 40 percent (n=46) medium risk of bias, and 49 percent (n=56) high risk of bias (lower quality). Low and medium risk-of-bias studies reported mixed results on the effect of nutrition interventions across cancer and treatment-related outcomes. Although the evidence map shows a large volume of studies evaluating nutrition interventions and outcomes, these studies showed high heterogeneity across study populations, interventions, and outcomes (measure definitions, timing of measurements), even within nutrition intervention categories; as a result, we could not aggregate results. While studies enrolled individuals from multiple cancer types, treatments, and stages, across the lifespan, with varying degrees of muscle wasting, and in those with a range of comorbid conditions, no eligible studies specifically evaluated whether the effects of nutrition interventions on preventing negative outcomes varied across these characteristics. Among studies included in our Key Questions, we found that few (4%, n=8) published cost or value (e.g., cost-effectiveness, cost-benefit) information related to the intervention. In our grey literature search of additional studies examining cost or value of nutrition interventions, we found few studies that conducted cost-effectiveness or cost-benefit analyses; among those that did, we found the studies were conducted in non-U.S. health systems and demonstrated mixed results on the value of nutrition interventions. Conclusions. Although overall RCT evidence focused on a wide range of nutrition interventions, studies were concentrated in use of dietary supplements, nutrition support, and the route or timing of nutrition interventions within gastrointestinal and head and neck cancers in inpatient settings. Among interventions with the highest volume of literature, the majority of studies were rated as high risk of bias. Our findings point to the need for rigorous new research to bolster the evidence base. Specifically, the field needs a more detailed future evaluation of a subset of nutrition interventions contained in this evidence map that focuses on priorities most relevant to specific stakeholders (e.g., oncologists, patients, dietitians, researchers, policymakers). Further, studies should be specifically designed to evaluate the main outcomes of interest for clinical practice. Future research would also benefit from creation of standardized taxonomies for interventions and outcomes as well as more rigorous design and reporting of nutrition interventions. As mentioned, heterogeneity of populations, interventions, comparators, and outcomes precluded aggregation. Currently, the quality and heterogeneity of the studies limit translation of findings into clinical practice or guidelines. In order to inform development of these guidelines, coordinated efforts are required to develop detailed conceptual frameworks for mechanisms of nutrition interventions most relevant to clinical care providers and patients. Such frameworks would help inform priorities for future research as well as guide practice and policy.
