An 82-year-old female patient with painful, watery, and itchy eyes was admitted for examination at the Sri Guru Ramdas Institute of Medical Sciences and Research. During the examination, a number of tick larvae were found attached to the patient’s upper and lower eyelid margins. The ticks were manually removed and the patient was treated with topical ciprofloxacin eye ointment. Microscopic studies revealed that all specimens belonged to a single species Rhipicephalus (Boophilus) microplus Canestrini (Acari: Ixodidae) which parasitizes livestock. Although other cases of ocular infestations have been reported, this is the first in North India involving R. (B.) microplus. It is speculated that the patient’s occupation, surrounding conditions and living propensities made her susceptible to contamination by ticks.

Background: Hernia may be generally defined as a protrusion of viscus or part of a viscus though an abnormal opening in the walls of its containing cavity. The Greek word ‘hernia’ means an offshoot, a budding or bulge. But the Latin word ‘hernia’ means a rupture or tear. The most important principle in reconstruction surgery is to avoid tension when restoring the muscular or elastic structures. The modern biologically based concept for repair of groin hernia, acquired during adult life, is application of a patch, avoidance of tension and use of local, spinal or epidermal anaesthesia.Methods: Present study is a Prospective study conducted in the Department of General Surgery in Sri Guru Ramdas Medical College and Hospital. Patients admitted in wards with inguinal hernia from General Surgery out-patient Department were selected for study. The study consisted of 60 patients between 15 to 74 years; The study period is from April 2015 to September 2016. The main aim and objective of any hernia repair is to avoid any recurrence and also acceptability to the patient in terms of pain, discomfort, other complications during follow up and obviously in terms of cost- effectiveness of repair.Results: The present study comprises 60 cases of inguinal hernia who were admitted in the wards of Department of General Surgery in the Sri Guru Ramdas Institute of Medical sciences and research. The study was carried out during span of 1½ years. All of these cases were studied prospectively and results were analysed from various angles. Among total 60 cases in this study, maximum cases belonged to age group 35-44 years ;15 (25%) and minimum to age group 65-74 years 5 (8.3%). Case distribution as per type of hernia, it was observed that among total 60 cases 34 (56.67%)had inguinal hernia and rest 26 (43.3%) had inguino scrotal hernia. Most 0f the cases had right sided hernia 35 (58.33%), left sided hernia was seen only in 23 cases (38.33%) whereas bilateral hernia was present in 2 (3.33%).Conclusions: From our results of observations in the study we can conclude that-The methods of Lichtenstein’s tension free inguinal hernioplasty under local anaesthesia is safe, simple, effective, economical, and without any side effects such as hypotension, nausea, vomiting, urinary retention, and spinal headache.

Background: The role of serum uric acid as a risk factor for acute ischaemic stroke is controversial and there is little information about it. Present study was done to estimate serum uric acid levels in patients of acute ischaemic stroke and to assess its risk factor potential.Methods: It was a prospective case control study carried out in the department of Medicine at Sri Guru Ramdas Institute of Medical Sciences and Research, Vallah, Sri Amritsar, Punjab, India from January 2015 to July 2016. 50 cases of acute ischaemic stroke were enrolled and were compared with same number of age and sex matched healthy controls. Serum uric acid levels were measured in cases (within 24 hours of stroke evolution). Glasgow coma scale (GCS) score was calculated for cases at time of admission. The results were statistically analysed.Results: Mean serum uric acid level in cases was 6.15±1.91mg/dl whereas it was 5.1±1.4 mg/dl in controls. The difference of serum uric acid levels between cases and controls was statistically significant (p = 0.0054). Patients with poor GCS had higher mean serum uric acid levels as compared to patients with mild or moderate GCS score which was statistically significant(p = 0.0426).Conclusions: Serum uric acid can be used as a marker for increased risk of stroke. Furthermore, serum uric acid can also be used for risk stratification after stroke.

Background: Acute appendicitis is the most frequent cause of persisting progressive abdominal pain in all ages. However, there is no way to prevent the development of appendicitis, the only way to reduce the morbidity and to prevent the development of appendicitis, is to perform appendicectomy before perforation or gangrene has occurred. In this study, we have done a randomized, prospective comparative study of laparoscopic appendicectomy versus open appendicectomy in the Department of Surgery, Sri Guru Ramdas Hospital and Research Institute, Sri Amritsar from August 2014 to December 2016 a period of two years and four months.Methods: This prospective randomized controlled study was carried out in the department of General Surgery. The total population group included 144 patients with a mean age of 39 years (Age group between 18-60 years). Patients between 18 years and 60 years of age were candidates for randomization. The study was carried out as an open randomized single centre study.Results: In the present study patients were not blinded to the surgical technique employed but were equally informed to resume normal activity and work as soon as possible at their discretion. The results show that time to return to heavy work was significantly reduced by the laparoscopic approach. Less pain in the post-operative period was the major contributing factor. In the present study, laparoscopic appendicectomy was associated with improved cosmesis when compared with open appendicectomy (P <0.01).Conclusions: Laparoscopic appendicectomy is a safe procedure with lower morbidity it is also an excellent training tool in laparoscopic technique and with sufficient experience takes no longer than open appendicectomy.

Background: Urinary bladder lesions are a great health concern as it lies among the top ten most common cancers in the world. These range from benign, harmless lesions that do not recur to life threatening tumors. The present study was undertaken to study incidence of various urothelial cancer in patients undergoing transurethral resection of bladder tumor in tertiary care hospital, as the treatment, follow up and prognosis is highly variable with different subtypes of bladder cancer.Methods: A hospital based descriptive cross-sectional study was conducted on all patients undergoing transurethral resection of bladder tumor from December 2018 to May 2020 in the Department of General Surgery of Sri Guru Ramdas Institute of Medical Sciences and Research, Amritsar. Based on World Health Organization (WHO) classification incidence of various urothelial malignancy was calculated.Results: Out of 80 patients with growth urinary bladder, 4 patients (5% of total patient) were diagnosed as case of papilloma, 26 patients (32.5% of total patients) were diagnosed as low grade papillary urothelial carcinoma, 50 patients (62.5% of total patients) were of high grade papillary urothelial carcinoma. There were no case of PUNLMP in our study.Conclusions: It can be concluded that majority of the patient undergoing transurethral resection of bladder tumor are diagnosed with high grade papillary carcinoma.

Background: Basal cell carcinoma (BCC) is a slow growing, locally invasive and most common skin cancer. Its etiology is mostly linked to exposure to U.V. rays in sunlight. Diagnosis is mostly clinical and wide local excision is main treatment modality.Methods: The study was conducted on 50 cases of BCC diagnosed at Sri Guru Ramdas University of Health Sciences during a study period extending from January 2013 to January 2021. In the present study various variables associated with BCC i.e. age, gender, size, site, and histopathology were studies.Results: The present study was conducted on 50 patients of BCC. Total number of males was 32 (64%) and there were 18 (36%) females. Mean age of patients presented with BCC was 65 years and SD=14.054. It was observed that most common site of lesion was infra orbital region 12 (24%). Size of lesion varied from 1 to 3 cm and wide local excision with flap was most commonly performed procedure at our Institute. Nodular sclerosing was observed to be the most common histopathological type accounted for 35 (70%) of total cases.Conclusions: BCC is a common surface malignancy amenable to early detection and potential cure. Infra orbital region is most common site and nodular sclerosing is most common histopathological type. Wide local excision remains the heart of treatment.

The reliable diagnosis of tuberculous lymphadenitis by FNAC has important implications in a developing country like India. The aim is to study various cytomorphological patterns seen in tuberculous lymphadenitis and their correlation with AFB positivity. Fine needle aspiration cytology (FNAC) was performed on three hundred and twenty two patients with lymphadenopathy referred to the cytopathology section of pathology department, Sri Guru Ramdas Institute if Medical sciences and research Amritsar from August 2010 to July 2012. The patients with cytological diagnosis of tuberculous lymphadenitis were followed for their response to antitubercular treatment. A total of 322 FNACs were performed on patients with lymphnode lesions; out of which the most common (119) cytological diagnosis was tuberculous lymphadenitis. The most common pattern observed was (group II) presence of epitheloid granuloma with caseation necrosis which was seen in about half (50.5%) of the cases followed by smears with caseation necrosis only (group III) (27.7%) and smears with epitheloid granulomas only (group I) (21.8%).The overall AFB positivity was seen in 19.3% cases of tuberculous lymphadenitis. Maximum (30.3%) AFB positivity was seen in smears (group III) with only necrotic debris without granuloma and was least in group I (7.6%).While comparing Group I smears with that of group III using Fisher's exact test, the difference in AFB positivity between the smears was statistically significant as the two-tailed p value was 0.0496. Thus FNAC is safe, easy, quick reliable as well as conclusive for the diagnosis of tuberculous lymphadenitis when done along with Zeihl Neelsen stain for acid fast bacilli.

Background: Urinary bladder lesions are a great health concern as it lies among the top ten most common cancers in the world. The present study was undertaken to assess the quality of life among patients with high-grade non-muscle invasive carcinoma of the urinary bladder receiving immunotherapy. Methods: A hospital-based experimental study was conducted in the Department of General Surgery of Sri Guru Ramdas Institute of Medical Sciences and Research, Amritsar from April 2021 to July 2022. To examine the Quality of Life of 60 patients before and after immunotherapy, the WHOQOL-BREF tool was employed. The participants were followed for a total of six months, with two follow-ups carried out at intervals of three months each. Results: The pre-test physical scores (20.70±8.66) were substantially lower than the other domains. At three and six months, the psychological scores were 21.93±8.51 and 49.37±12.27 respectively, while social scores were 23.93±12.16 and 49.15±24.09 correspondingly. Environment scores (p=0.45) showed no significant difference between groups after BCG treatment. Conclusions: It was vivid that the negative effects of NMIBC on QOL are most pronounced in the physical realm. Quality of life for those with high-grade NMIBC who had BCG treatment improved significantly across all dimensions except the environment. The quality of life of high-grade NMIBC patients was not affected by any of the socio-demographic characteristics except for education and occupational status.

Background: The word anastomosis originates from the Greek word (ἀναστόμωσις) meaning communicating opening. Gut anastomosis is one of the frequently performed surgeries in both emergency and elective setup. Anastomosis following gut resections in emergency set up is mostly done due to traumatic rupture, benign or malignant perforation or obstruction and in certain other inflammatory conditions. Anastomosis is also done in some elective conditions like mostly due to malignancy of GI system. As conventional practice following gut anastomosis, patients are kept “NIL BY MOUTH” till bowel sounds return.Methods: It’s a prospective study, carried out over period of 18 months in Department of General Surgery, Sri Guru Ramdas Institute of Medical Sciences and Research, Vallah, Amritar. The objective of this study was to whether early enteral feeding within 48 hours of small gut anastomosis is tolerable to the patient. Whether early enteral feeding within 48 hours of small gut anastomosis is beneficial to the patient.Results: This prospectively conducted comparative study was carried out on 60 patients, meeting inclusion criteria, undergoing gastrointestinal anastomosis either elective or emergency, in the Department of General Surgery, SGRD Medical College, between Jan 2012 to June 2013. Random selection of patients into group A (30) and group B (30) was done after having fulfilled inclusion and exclusion criteria. The group A was fed via enteral route within 48 hrs of enteric anastomosis. The group B was fed via enteral route after 48-72 hours or appearance of full peristaltic sounds following enteric anastomosis. These patients were followed in post-operative period for their drain output, any nausea, vomiting, or significant abdominal distension, prolonged ileus, clinical leakage, infective complications, hospital stay.Conclusions: The following inferences can therefore be drawn from this study: Appearance of intestinal peristaltic sounds is earlier in early enterally fed group, Mean duration of post-operative hospital stay is lower in early enterally fed group, mean post-operative day 4 albumin level is higher in early enterally fed group. The rate of infective complications (UTI, RTI, wound complications) is equal in both the groups. The rate of clinical leakage, nausea/vomiting are equal in both the groups. The rate of re-exploration for anastomotic leakage is equal in both the groups.

Kumpulan Sri Gabus dan Bunga Emas Seri Temenggung merupakan dua buah kumpulan Makyung yang terkenal di Kelantan. Kumpulan Sri Gabus berasal dari Kampung Gabus, Pasir Mas, manakala Bunga Emas Seri Temenggung kini bertapak di Universiti Malaysia Kelantan, Malaysia. Boleh dikatakan bahawa Kumpulan Sri Gabus berfungsi di luar bandar dan Bunga Emas Seri Temenggung pula di kawasan bandar. Kedua-dua kumpulan ini mempunyai sejarah perkembangan dan gaya persembahan yang tersendiri. Objektif kajian ini adalah untuk mengenalpasti persamaan dan perbezaan dari segi gerak tari Makyung dan nilai estetika yang terdapat daripada intepretasi guru tari kedua kumpulan. Analisis kajian ini berdasarkan Wiraga, yang merupakan salah satu prinsip estetika tari yang digunapakai di Indonesia. Kajian ini menggunakan pendekatan kualitatif iaitu kajian kepustakaan dan lapangan yang melibatkan kaedah pemerhatian turut serta dan temubual. Hasil kajian mendapati terdapat beberapa persamaan dan perbezaan yang ketara di antara kumpulan luar bandar dan kumpulan bandar ini dari aspek gerak dan nilai estetika yang terdapat daripada setiap tarian Makyung. The Sri Gabus and Bunga Emas Seri Temenggung groups are two well-known Makyung groups in Kelantan. Sri Gabus Group is from Kampung Gabus, Pasir Mas, while Bunga Emas Seri Temenggung is currently based at the Universiti Malaysia Kelantan, Malaysia. It can be said that the Sri Gabus Group works in the rural areas and Bunga Emas Seri Temenggung in the urban areas. Both groups have their own developmental history and presentation styles. The objective of this study is to identify similarities and differences in terms of Makyung dance movements and the aesthetic value of the two dance teachers' interpretation. The analysis of this study is based on Wiraga, which is one of the principles of dance aesthetics applied in Indonesia. This research uses the qualitative research method, namely library and field studies that involve participatory observation and interviews. The results show that there are some similarities and differences between the rural and urban groups in terms of movement and the aesthetic value of each Makyung dance
