Shock remains a major cause of intensive care unit admission. Initially categorized into hypovolaemic, cardiogenic, and distributive shock, understanding of the pathophysiology has recently evolved such that tissue hypoperfusion in all shock states leads to a dysregulated inflammatory response. After 24 hours, septic shock and ischaemiareperfusion related to hypovolaemic and cardiogenic shock share similar haemodynamic and pro-inflammatory profiles. Vascular hyporesponsiveness to catecholamines is a major consequence of this common pathophysiology, which is focused upon activation of NF-κ‎b with subsequent NO overproduction. Myocardial dysfunction is a frequent complication of the cytokine storm that follows septic shock and ischaemiareperfusion. It may worsen haemodynamic status, but nevertheless, remains transient and totally reversible.

For the past three decades scientists have urged us to abandon fossil fuels as rapidly as possible. Despite these pleas, the global energy sector has retained a familiar profile, dominated by the use of oil and natural gas. Only states have powers that are commensurate with rapidly reshaping societies in sustainable ways, but how do their politics enable these surges in sustainability? Shocks, States, and Sustainability answers this question through a comparative historical study of four radical environmental reforms: in the Dust Bowl during the New Deal, in Britain after World War II, in Cuba after the Soviet collapse, and in the Gulf of Maine after the Depression. This analysis suggests that states reform environmental practices in the aftermath of focusing events that draw popular attention to environmental degradation and suggest sharp limits in the availability of natural resources. These crises prompt the creation of encompassing coalitions of diverse peoples who push through laws and regulations that conserve natural resources.

Many people express shock and horror when they hear of a wealthy or famous person killing another person. As a society, we seem to expect the rich and famous to behave better, to commit fewer crimes, to be immune to the passions that inspire other, less prominent people to kill. After all, the rich and famous have everything—why would they need to murder? But the rich and famous kill for the very same reasons others do: love, power, money, jealousy, greed, revenge, and rage. Here, Scott takes us on a tour of murders committed by the rich and famous during the last century, looking at the motives, the responses of the community and local law enforcement, the media, and the outcomes. She argues that the rich and famous may kill for the same reasons as others, but they receive vastly different treatment and are often able to get away with murder. Homicide by the rich and famous is not new in this country, nor is fascination with the crimes committed by our most revered citizens. But being among the upper echelon of society does afford such suspects with a greater ability to escape punishment. Haemodynamic monitoring facilitates effective resuscitation and the rapid assessment of the response to time-dependent vasoactive and fluid therapyin different shock states. Since the introduction of the pulmonary artery catheter, several minimally and non-invasive CO monitoring devices have been introduced to provide continuous monitoring and a dynamic profile of fluid responsiveness. Several of these monitors provide additional haemodynamic parameters including dynamic indices of preload and volumetric indices. Patient outcome is dependent accurate acquisition and interpretation of data and subsequent management. Whilst data from CO monitors offer valuable information on global hamodynamics, they do not preclude tissue hypoperfusion. Furthermore, there is no ‘ideal’ CO value for an individual patient, and the trend in haemodynamic parameters in response to therapy may be more informative than the absolute values. CO monitoring should be based upon the patient’s needs, the clinical scenario, and the experience of the treating physician.

Under normal conditions, the right ventricle (RV) virtually acts as a passive conduit. In critically-ill patients many situations induce uncoupling between the right ventricle and pulmonary circulation, leading to RV systolic dysfunction, then failure. Mechanical ventilation has a major impact by decreasing RV preload, but also significantly increasing RV afterload. RV function should thus always be interpreted and re-evaluated in the light of respiratory mechanics and ventilator settings. RV systolic function is key to the patient’s haemodynamic profile and must be monitored to achieve optimal haemodynamic management. Echocardiography is the best compromise between clinical effectiveness and invasiveness to monitor RV function. A limitation is its inability to monitor haemodynamics continuously. Acute cor pulmonale is defined by the combination of RV dilatation with paradoxical septal motion during systole. In conclusion, RV function monitoring is strongly recommended in many situations encountered in the intensive care unit, such as ARDS, septic shock, and pulmonary embolism. Many devices are available, but echocardiography constitutes the best compromise between accuracy and invasiveness.

This paper adds quantitative analysis to the study by Crompton et al. (2020), in which various alternative regulatory arrangements regarding the petrol price in South Africa were explored. We use a multi-sector dynamic computable general equilibrium model for South Africa to conduct our economic impact analysis. Five scenarios are modelled, first individually to correctly calibrate the shocks, and then cumulatively to find the overall economy-wide effects of the proposed reforms. Under the most comprehensive set of reforms to the determination of petrol prices, which seeks to emulate market forces, the South African economy is seeing substantial benefits. GDP is expected to rise by 0.67 per cent and real wages by over 1.1 per cent relative to the baseline. Refineries are assumed to shrug off reforms targeted at removing pure profits earned via the import parity price (Basic Fuel Price) methodology by accepting a slightly lower rate of return, enabling them to meet the expected increase in demand for petrol on the back of the lower consumer prices achieved via the reforms. Whilst job losses at fuel service stations may be expected as a result of reduced revenues and margins, increased activity and job opportunities in the rest of the economy, facilitated through cheaper trade and transport margins, will more than offset those losses.
