Texto completo da fonteIn social engineering interventions, and typically in development , participation is presented as a lever, even a democratic and managerial ideal; moreover with the use of digital devices. U Report Cameroon follows this trend. By collecting, through its mobile telephone platform, the opinions of young people and adolescents on subjects such as sexual and reproductive health, this project provides for “effective and successful participation”. These logics idealizing participation and digital technology suggest that the realization of these projects goes without saying. Such linearity takes for granted the appropriation, by its actors, of the directives issued during the framing. But what about the “disenchantment” faced with the implementation gaps between what was planned and what is happening?To identify the stakes of these gaps, we combine multidisciplinary contributions mobilized in research on development, digital health and organization. Our vision of development is less marked by the deconstruction of the mechanisms for strengthening the Western model of well-being. It is viewed as a form of social change and internationalized public action; i.e. an object of study of the dynamics at work in the project ecosystem. Within this framework, social interactions take place between actors in the production driven by varied logics and strategies. Anything that contributes to conceptual shifts relating to the consideration of the device studied from “traveler model” to “organization” via “IAP” and “DISTIC”.This study focuses on gaps between what is achieved and what is planned for participation according to the 'participative norm', in the light of “rationalizations as a capacity for action”. The ACO/CCO articulation contributes to a multilevel analysis of the practices of the actors in the production chain, the structuring framework and the discourses. This crossed lens focused on the symbolic dimension of relationships to participation concretizes “methodological and interpretive eclecticism” and supports “empirical adequacy” in the interpretation of discursive, observational and written field data collected from December 2020 to August 2022.Between prevalence and interdependence, the respondents try to optimize their presence in the project ecosystem. They reinterpret the reference framework and the operational variations of the project, the ‘linkage notions’ at the center of the “hybrid” semiotic foci activated according to fluctuating expectations, values and motivations. Forms of ‘nuanced submission’ emerge, between resignation and rupture. This ‘dialectization-semiotization-redefinition triad’ questions the “rationalization in action” due to the dissonances between what is thought, what is said and what is done and makes the gaps in realization appear as gaps in translation and appropriation

Texto completo da fonteIn order to better understand the strategies used by farmers in chemical weed control and with the aim of comparing the economic returns of direct sowing and herbicide application with tillage, a research was conducted in two benchmark villages (Mafa Kilda and Pandjama) in North Cameroon. Comprehensive approach was adopted in order to better understand farmers practically minded. Farmer’s social representations of their profession about direct sowing with herbicides and till practice have been analysis. An analysis concerned the reasons for adoption of direct sowing and herbicide use by farmers, and farmer’s perception of the health and environmental risks. A follow-up technico-economic study of the two farming system was undertaken. The factorial correspondence analysis was used to analyse the typology of farms. Economical analysis was employed. Results show that with direct sowing 0. 7 man-days/ha and 1. 6 man-days/ha are required to establish crops in the field whereas with regular tillage 4 to 5 man-days/ha are required. This gain in time with direct sowing can vary between 60 to 80% depending on the crop. The productivity and revenue from farms under direct sowing and herbicide application are higher compared with tillage. For cotton, maize and groundnut cultivation, the application of a medium dosage of herbicide combined with early weeding using animal traction, can permit to have acceptable profit margin with direct sowing and herbicide application. Due to social and cultural capital differences, 94 % of farmers at Pandjama were aware of the potential health and environmental hazards related to heavy use of herbicides, compared to 70 % at Mafa Kilda

Texto completo da fonteThe increasing growth of health expenditure, accentuated by changes in health needs, has inevitably led to the introduction of a budgetary constraint within hospitals. These budgetary restrictions, denounced by the health professionals, caused a real discomfort within the hospitals imposing to envisage organizational reforms. Managerial innovations have thus become a possible response to the hospital crisis. However, their implementation may be hampered by obstacles related to the structural complexity of health organizations and the plurality of actors involved in them. The literature highlights the key role of the proximity manager in helping to overcome resistance by supporting the creation of a sense of innovation.Our research aims to specify the methods of influence of the proximity manager on the implementation of managerial innovation and its adoption by the actors of the organization. Based on an intervention research carried out in immersion for six months in a Luxembourg hospital, we support the implementation of a managerial innovation, Lean Management, within an outpatient chemotherapy service faced with organizational difficulties.The results of the research highlight the influence of the local manager on the adoption of managerial innovation and reveal factors that moderate the ability to mobilize these levers. Structural adjustments and the introduction of managerial support for the manager's autonomy support his missions to deploy innovation

Texto completo da fonteThe health care system is facing increasing problems due to changes in the social, technological, demographical and economical environments. The use of telecommunication technology to assist in the delivery of health care and related services has increased over the past few decades. It is called telemedicine or telehealth. This research deals with two main question : do telehealth networks allow for a greater efficiency in the use of the available resources? do they constitute a breaking-down point with the existing health care paradigm? three disciplinary points of views are adopted. The firts historical approach traces the development of telehealth in the united-states. Their technical characteristics and their role are analyzed. The second perspective investigates the efficiency of telehealth networks. Concerning medical services the impact of telehealth along two major dimensions is measured : accessibility to health care (spatial and temporal barriers) and the patient trajectory in the health care system. For image-data management, the cost price of a centralized digital image management system is compared to the cost price of a peripheralized one (pacs). And concerning educational services and according to the results of our survey, the savings associated with continuing medical education using telecommunication technology are put forward. In the last systemic approach, the similarities between this innovation and the medical ideology are analyzed, as well as the social and psychological issues at stake

In particular, the Electronic Health Record (EHR) is increasingly being deployed within healthcare organizations. The ability to share EHR’s underlying knowledge both internally and externally within healthcare organizations has been accepted as a method to improve the quality and delivery of care; however it has also raised important questions related to legal and privacy issues.This research aims to explore the critical factors that impact knowledge sharing in the French healthcare sector. Our main research focus is to answer the question of how to improve Knowledge sharing in the healthcare field?A qualitative exploratory study was handled to investigate EHR’s underlying Knowledge sharing in French hospitals.Three major issues were identified, namely the need for: a common healthcare terminology, the interoperability among healthcare information systems and the patient’s informed consents before sharing his sensitive data.In the end, this research purposes both a conceptual research model for explaining organizational value of knowledge sharing in healthcare sector and an ontological framework that extends the Systematized Nomenclature of Medicine Clinical Terms (SNOMED CT) with privacy dimension to secure access to sensitive patient’s data

Texto completo da fonteOn 1 June 2007 the European Union set up REACH (Registration, Evaluation, Authorisation of Chemicals), one of the most ambitious regulations. This regulation establishes a new philosophy of how to design environmental protection and health. According to the preamble to the Regulation, the objective of REACH is "to ensure a high level of protection of human health and the environment while enhancing competitiveness and innovation." REACH has been designed to balance environmental objectives with competitiveness aims, and has the scope to induce the adoption of eco-Innovation as a side effect of the regulation itself. For this reasons, REACH appears as a privileged object of study to analyse the effects of environmental regulation on the innovation. In this regard, we analyse the innovation-Friendly mechanisms of REACH to promote the development of environmental innovations. From an unique original survey on REACH regulation, we study, one the hand, mechanisms to promote new opportunities in the field of environment and health, and, the other hand, those that stimulate demand for environmental quality. Finally, we analyse the impact of the attributes of the REACH regulation on technological and industrial dynamics from an agent-Based model (ABM). The ABM provide a powerful tool for exploring such complex and stochastic systems as innovation, and allow modelling the behaviour of heterogeneous agents, technological diversity and the change in selection environment that result from policy measures
