ObjectivesMany studies suggest that infective endocarditis (IE) in people who inject drugs is predominantly right sided, while other studies suggest left sided disease; few have differentiated by class of drug used. We hypothesised that based on differing physiological mechanisms, opioids but not stimulants would be associated with right sided IE.MethodsA retrospective case series of 290 adult (age ≥18) patients with self-reported recent injection drug use, admitted for a first episode of IE to one of three hospitals in London Ontario between April 2007 and March 2018, stratified patients by drug class used (opioid, stimulant or both), and by site of endocarditis. Other outcomes captured included demographics, causative organisms, cardiac and non-cardiac complications, referral to addiction services, medical versus surgical management, and survival.ResultsOf those who injected only opioids, 47/71 (69%) developed right-sided IE, 17/71 (25%) developed left-sided IE and 4/71 (6%) had bilateral IE. Of those who injected only stimulants, 11/24 (46%) developed right-sided IE, 11/24 (46%) developed left-sided IE and 2/24 (8%) had bilateral IE. Relative to opioid-only users, stimulant-only users were 1.75 (95% CI 1.05 to 2.93; p=0.031) times more likely to have a left or bilateral IE versus right IE.ConclusionsWhile injection use of opioids is associated with a strong predisposition to right-sided IE, stimulants differ in producing a balanced ratio of right and left-sided disease. As the epidemic of crystal methamphetamine injection continues unabated, the rate of left-sided disease, with its attendant higher morbidity and mortality, may also grow.

Five month old female baby presented to us for evaluation of asymptomatic right sided pleural effusion. Pleural fluid analysis revealed it as chylous effusion. Radiological investigations revealed gross right side pleural effusion with partial collapse of right lung. After investigations, diagnosis of asymptomatic idiopathic chylothorax was made. Baby was treated with Inj. Octreotide alone. Pleural drain was not placed. After three doses of Octreotide, there was complete clearance of pleural effusion.J Nepal Paediatr Soc 2015;35(1):64-66

Honour-related violence is increasingly recognised as a human rights problem in a number of countries. However, the scope of State obligations to prevent such acts remains largely unexplored, with the exception of so-called honour killings. This article analyses other forms of honour-related violence from the perspective of the right to private life. It argues that a positive obligation to prevent honour-related violence arises under this right. The extent of the obligation is exemplified by demonstrating to which honour-related acts the right to private life is applicable and which measures authorities can be expected to take in order to prevent these acts.

The issue of justice in housing is rarely discussed in political philosophy. Yet, housing crises such as that currently faceSd by the UK highlight the importance of work in this area. In this article, I address the issue of housing justice by defending a basic right to housing. The right is in the first instance specific to a particular context, that of a Western liberal society. I defend a positive right to exercise a set of property rights called ‘lease rights’ over a self-contained living space of a certain standard, for a minimum term of 3 years. An important feature of this right is that it is a right to live alone. Where individuals have to live in communal housing because they lack the resources to do otherwise, their basic rights are violated. The account provides a distinctive understanding of how a government’s housing policy can fail its citizens.

This qualitative case study analyzed the United States Supreme Court's opinion in Bowers v. Hardwick, 478 U.S. 186 (1986), and the historical and legal background leading up to the case. Often characterized as a decision representing an emotional rejection of homosexuality rather than a reasoned application of constitutional privacy precedent, this inquiry sought to identify and document the determinants of the outcome in Bowers, in which a slim majority of the Court ruled that the constitutional right of privacy did not prohibit states from regulating homosexual sodomy. The study demonstrated that although homophobia certainly played a part in the Bowers decision, that the opinion was not necessarily inconsistent with previous privacy decisions such as Griswold v. Connecticut , 381 U.S. 479 (1965), and Roe v. Wade, 410 U.S. 113 (1973). The author concluded that the dominant insight gleaned from Bowers is that there is no such thing as a constitutionally protected right of privacy, at least not in the way that privacy is conventionally understood. The Bowers opinion illuminates that the Court's privacy jurisprudence has been more about the privileging of certain relationships (such as that between husband and wife or doctor and patient) than it has been about personal privacy. Such relationships serve an important limiting principle. The author concluded that the outcome in Bowers was not the insufficiency of the claim of a right to privacy, but the insufficiency of any limiting principle. The research documented and analyzed history of the two bodies of law most relevant to the Bowers opinion: state law which criminalized sodomy; and constitutional protection of individual privacy.

Sibling caretaking, although common across time and cultures, has not been well researched from the carer’s point of view. In Nicaragua, ranked as one of the poorest countries in the Americas, sibling caretaking is common. The country’s historical background and its state of chronic poverty, widespread unemployment, loose family structures, and migration and mobility makes of the old practise of shared management child care a necessity. Households headed by sing¬le mothers constitute a particular Nica¬raguan charact¬eristic. Many children are expected to help in their own families and care for their siblings and other children living in their households. In its broadest sense sibling caretaking is a public health concern, and we conducted this study to widen the understanding of the phenomenon as it is represented in a setting undergoing a rapid social transition. The main objectives were to identify, describe and analyse the life situation of sibling caretakers in poor areas in León, Nicaragua, with focus on how they perceived it themselves. A combined qualitative and quantitative methodological design was used, mainly applying an ethnographic approach. A further ambition was to explore involvement of children in a participatory research process in accordance with the ‘Convention on the Rights of the Child’. The overall emotion expressed among the caretakers was pride, even if their situation often was characterized by stress and coping problems. They perceived their work as important for their families and they appreciated to fend for their siblings. Household work and nurturing of siblings were shaping the future lives of the caretakers and constituted part of their socialization. Even if many of these children achieve essential life skills as caretakers, they are at risk of falling behind as they grow older. Their long-term personal development is likely to be hampered by the obligations they have as caretakers. The carers' awareness of missing out on education was the most problematic issue for them. From a societal point of view, caretaking has negative consequences. The individual child is marginalised with limited access to basic education, contributing to overall low educational levels in Nicaragua. While the structuring conditions leading to sibling caretaking may be difficult to change, awareness of how these can affect children might make way for improvements in terms of access to school education and support from the society. The knowledge gained from this study should be further utilised to plan for interventions that take children’s perspectives into consideration.

I investigated percent coverage, plant height, species richness, and woody stem density in plant communities in ten study plots during spring and fall (2010–2012) within 3 different treatments (continual mowings, one fall mowing, and one fall mowing with native wildflower seeds) on Highway 25 right-of-way in Oktibbeha and Winston counties, Mississippi. I recorded 277 plant species including native and non-native forbs, legumes, grasses, rushes/sedges, and woody plants. Non-native agronomic grasses exhibited greatest coverage greater than 90 percent occurring in all treatments. Percent coverage of plants less than 0.46m height category exceeded 100 while, greater than 0.46m plant height categories averaged 55 percent. Woody stem density ranged from 7,772 year 1 to 10,025 stems/hectare year 2. I detected no significant differences in plant height or woody stems among treatments. One mowing per year retained agronomic plant cover for erosion control and annual cost savings up to 75 percent for roadside maintenance.

La notion juridique de vie privée au sens d’un droit générale de mener sa vie comme on l’entend est une notion très récente en droit français. Avant la Seconde Guerre mondiale, la vie privée est essentiellement protégée au travers des supports fonctionnels (le domicile, les correspondances, etc.) et des concepts précis (l’obligation de confidentialité, la diffamation etc.). Cette thèse entend néanmoins montrer par l’histoire la cohérence ancienne qui existe entre ces multiples protections qui entendent protéger la faculté de l’individu de se conduire comme il l’entend dans la mesure où son action n’est pas publique. Les juristes modernes sont marqués par l’idée selon laquelle l’homme doit se voir reconnaître tant par de nombreux juristes modernes un droit naturel à la maîtrise de ses affaires privées. Au-delà du droit de propriété, cette sûreté, qui couvre l’ensemble de la vie privée, est présente dans la property lockéenne et donc dans les différentes déclarations des droits du XVIIIe siècle qui sert de fondement direct à la privacy américaine. Elle désigne initialement une liberté éminemment négative ayant pour vocation de laisser l’homme tranquille contre les intrusions dans la vie privée sans décision politique, sans causes légitimes et de manière à remettre en cause un certain partage fonctionnel entre ce qui relève du public et du privé
"Privacy" or "private life" is regarded as a general notion of the french law. This words appears for the first time France during the French Constitution to protect individual reputations. Under this report, private protection is an extension of the concept of defamation as it was understood under ancien law. According to old law conception, infamous revelations must be repressed even if they are true. This approach presupposes that personal information has no place in the public space. In the same way the rule of secrecy is institutionalized in order to protect specific relationships: the secret of confession, secret of letters, confidential medical information, etc. This thesis intends to link the concrete protections of the private sphere to Western history in general

My thesis project, Better Alone focuses on single person households from a perspective of loneliness. The objective has been to find an architectural solution to decrease experienced loneliness. In this project I intend to address the issue and design a way of living that can relieve the experience of involuntary loneliness. I have used two main strategies: the first is to provide space for social interaction in connection to the home and to create reasons to enter and linger in these spaces. The second main strategy is to blur or stretch the border that separates the home into privacy through visibility. The project's site is  in southern Stockholm, in between the neighborhoods Hammarbyhöjden and Hammarby sjöstad, a fairly central location. The site's most prominent characteristic is the fact that it is a steep slope. At the bottom of the slope runs Hammarbyvägen, which is a fairly busy road. Together these two aspects create a barrier between the neighborhoods of Hammarby sjöstad and Hammarbyhöjden, these are challenges that I have addressed in the project.

However, getting the basics right will reduce the pain of EMC testing and increase the chances of right-first-time. And that is all that we want, right?

Paul Muldoon is an outstanding Irish poet. One of his poetry's characteristics can be coined as the 'intricate spontaneity,' which does the most that it does, inspires. This paper deals with the problem of privacy in his poem the Hedgehog. Privacy is an essential right that everyone must have. Its importance is huge as it relates to such areas as identity, sovereignty... etc. The paper is an attempt to follow the brush touches by which the poet draws the object of his painting, the hedgehog. It aims at observing the animal closely both externally as well as internally. In respect to the external part, the symbolism the poet makes use of through this object of painting is impressive and urges upon examining. As for the internal part, the poem is to be mystically considered for a better understanding of its purport concerning the concept of privacy. The paper setting benefits of Imagism tools to assess the poem properly.

Tetralogy of Fallot (TOF), also called blue-baby syndrome is one of the most common congenital heart defects in children after infancy and is estimated to account for 10% of all congenital heart defects [3]. TOF consists of four interrelated lesions: i) ventricular septal defect ii) Pulmonary stenosis iii) Right ventricular (RV) hypertrophy and (iv) Overriding Aorta [3]. TOF has been successfully repaired for several decades (Fig. 1). There are now an estimated 100,000 adult “repaired TOF” patients in the United States alone. As a result, long-term sequelae of the disease and repair have become important clinical issue. Specifically, residual pulmonary valve insufficiency (PI) is one such accepted and often unavoidable sequela. PI, when severe, abnormally alters the RV loading conditions, thereby triggering RV hypertrophy and dilatation. In turn, RV dilatation can evolve into irreversible RV myocardial contractile dysfunction, and has been related to sudden death in many “repaired TOF” patients. To normalize RV loading conditions, pulmonary valve replacement is often necessary and should be performed prior to the onset of irreversible RV myocardial damage.

Today, obtaining field information in traditional pipeline design workflows rely on the use of conventional aerial photography, mapping and field visits. As a Designer, Engineer or Project Manager, a field visit can answer many questions and perhaps be the key to achieving a sound, successful design and installation. While conventional aerial orthophotography and mapping is invaluable during the design, it lacks a dimension that allows you to visualize the right-of-way the way you would if you were there. The use of Airborne Video and Oblique Imagery is not intended to replace conventional aerial orthophotography or mapping, but augment its use by providing a rich visualization that, in conjunction with the planimetric data sets, can aid the project team immensely during the design and permitting phase of a project. Currently, there are Airborne Data services available serving the pipeline industry that contain custom tools that either integrate into GIS platforms or operate as stand-alone proprietary software. These tools aid designers, engineers and constructors navigate through the video without having to sit and watch the whole video. In addition, video can be geocoded to specific pipeline or right-of-way features, such as a valve sites or stream crossings. Some tools even allow you to take rough measurements from visible features like fences or roads. Having such a dataset enables project team members to revisit any location along the right-of-way as many times as required to get information to finalize a design, permit application or to estimate the cost of construction. As Project Managers, we regularly face having new team members join the project at all stages of the project. Having a video to hand to new team members reduces the time they will take to get on board and familiarize themselves with the right-of-way. It also reduces the amount of field trips required just for this purpose. Another great use of Airborne Datasets is to use it during a pre-bid conference and provide it to the contractors bidding on the job. On large pipeline projects, it is unfeasible and impractical to have the project management team tour the entire right-of-way during a job walk, and there is likely areas along the right-of-way that are not very accessible thus requiring a flyover to review it with others.

The idea that children have a “right to education” has been widely accepted since the Universal Declaration of Human Rights in 1948 (United Nations, 1948) and periodically reinforced since. The “right to education” has always, explicitly or implicitly, encompassed a “right to learn.” Measures of schooling alone, such as enrollment or grade attainment, without reference to skills, capabilities, and competencies acquired, are inadequate for defining education or education poverty. Because of education’s cumulative and dynamic nature, education poverty needs an “early” standard (e.g., Grade 3 or 4 or age 8 or 10) and a “late” standard (e.g., Grade 10 or 12 or ages 15 and older). Further, as with all international poverty definitions, there needs to be a low, extreme standard, which is found almost exclusively in low- and middle-income countries and can inform prioritization and action, and a higher “global” standard, against which even some children in high income countries would be considered education poor but which is considered a reasonable aspiration for all children. As assessed against any proposed standard, we show there is a massive learning crisis: students spend many years in school and yet do not reach an early standard of mastery of foundational skills nor do they reach any reasonable global minimum standard by the time they emerge from school. The overwhelming obstacle to addressing education poverty today is not enrollment/grade attainment nor inequality in learning achievement, but the fact that the typical learning profile is just too shallow for children to reach minimum standards.

Three sector disruptions alone driven by just eight technologies can directly eliminate over 90% of net greenhouse gas (GHG) emissions worldwide within 15 years. Market forces can be leveraged to drive the bulk of global GHG emissions mitigation because the technologies required are either already commercially available and competitive today, or can be deployed to market before 2025 with the right societal choices. The same technologies will also make the cost of carbon withdrawal affordable, meaning that moonshot breakthrough technologies are not required to solve the ‘Last Carbon Problem’ and go beyond net zero from 2035 onwards.

Arkansas law is clear: every arrested person has the right to an attorney’s help the first time they see a judge. But across the state, people often face a judge at first appearance without a lawyer by their side. Even worse, a shortage of attorneys means people sometimes wait months for a lawyer’s help. The Constitution promises that every person in jail will have access to the courts and to counsel. Yet far too often, Arkansas allows people to languish in jail alone, afraid, and undefended. This policy brief outlines research-based solutions for Arkansas to honor the Constitution’s promises by guaranteeing counsel at first appearance, ensuring appointment of defense counsel within 72 hours of arrest, and adequately funding public defense. These reforms can end the first appearance crisis, reduce court backlogs, and ease jail overcrowding.

Longwall mining induces large surface motion that may impact active pipelines. Typical remediation for longwall mining involves shutting down and exposing the pipeline. The use of InSAR has the potential to provide accurate measurements confirming the expected ground movement that will occur with the mining operations. Used correctly, with an appropriate survey design, InSAR can provide extremely high densities of ground movement over time. Exploiting the wide-area capabilities of InSAR could become an important part of integrity management for pipelines where longwall mining is a consideration. InSAR surveys are well suited to the observation of spatially and temporally smooth movements. These movements can be very small (millimetres in months) or larger in areal extent and movement. We have previously shown the ground movement (of 9.8 ft in 12 years) along a pipeline associated with an enhance oil recovery operation. This work examines some of the design considerations necessary to observe fast, large scale deformation with InSAR. This is accomplished through modelling and through the examination of data captured over a pipeline/longwall mine in Pennsylvania. The qualitative description of the passage of the miner is very good. The local ground conditions in Pennsylvania make a more thorough examination of the ground movement available from SAR less accurate than it would be in regions better suited to InSAR measurements (for instance Wyoming).

Based on the proposal by the European Council, more than 25 heads of state and the World Health Organization (WHO) support development of an international treaty on pandemics, that planned to be negotiated under the auspices of WHO, will be presented to the World Health Assembly in May 2021. Given that the treaty alone is not enough to ensure compliance, triggers for a high-level political response is required. To this end, to inform the design of a support system, we explored institutional mechanismsi with a mandate to review compliance with key international agreements in their signatory countries and conduct independent country investigations in a manner that manages sovereign considerations. Based on our review, there is no single global mechanism that could serve as a model in its own right. There is, however, potential to combine aspects of existing mechanisms to support a strong, enforceable treaty. These aspects include: • Periodic review - based on the model of human rights treaties, with independent experts as the authorized monitoring body to ensure the independence. If made obligatory, the review could support compliance with the treaty. • On-site investigations - based on the model by the Committee on Prevention of Torture according to which visits cannot be blocked by state parties. • Non-negotiable design principles - including accountability; independence; transparency and data sharing; speed; emphasis on capabilities; and incentives. • Technical support - WHO can provide countries with technical assistance, tools, monitoring, and assessment to enhance emergency preparedness and response.

A novel algorithmic framework for the robust detection and classification of machinery threats and other potentially harmful objects intruding onto a pipeline right-of-way (ROW) is designed from three perspectives: visibility improvement, context-based segmentation, and object recognition/classification. In the first part of the framework, an adaptive image enhancement algorithm is utilized to improve the visibility of aerial imagery to aid in threat detection. In this technique, a nonlinear transfer function is developed to enhance the processing of aerial imagery with extremely non-uniform lighting conditions. In the second part of the framework, the context-based segmentation is developed to eliminate regions from imagery that are not considered to be a threat to the pipeline. Context based segmentation makes use of a cascade of pre-trained classifiers to search for regions that are not threats. The context based segmentation algorithm accelerates threat identification and improves object detection rates. The last phase of the framework is an efficient object detection model. Efficient object detection �follows a three-stage approach which includes extraction of the local phase in the image and the use of local phase characteristics to locate machinery threats. The local phase is an image feature extraction technique which partially removes the lighting variance and preserves the edge information of the object. Multiple orientations of the same object are matched and the correct orientation is selected using feature matching by histogram of local phase in a multi-scale framework. The classifier outputs locations of threats to pipeline.�The advanced automatic image analysis system is intended to be capable of detecting construction equipment along the ROW of pipelines with a very high degree of accuracy in comparison with manual threat identification by a human analyst. �

Ground displacement along pipeline corridors has the potential to compromise pipeline integrity. The Appalachian Mountain region is almost entirely classed as high landslide susceptibility by the United States Geological Survey (USGS) and crossed by numerous transmission pipelines and gathering lines. In phase I of this research project, 3vGeomatics successfully demonstrated a proof of concept on the effectiveness, reliability, and precision of using L-band ( ) SAR satellites for InSAR displacement monitoring of vegetated areas. The second phase of this project improved operational monitoring utility of long wavelength InSAR by leveraging a two and a half year dataset of long-wavelength satellite radar data with improved product formats to facilitate management of pipeline threats posed by both geohazards and third party encroachment. Potential encroachment threats are highlighted by new object detection capabilities that use the same raw SAR data to produce additional intelligence. As with the previous year's results, these phase-2 InSAR and new object detection results were compared with measurements from other sensors including differential light detection and ranging (LiDAR), visual field inspections, and aerial photographs. This project demonstrated the technological readiness and streamlined product formats delivering actionable intelligence from L-band SAR data for operational monitoring of ground displacement and other hazards over entire pipeline networks and associated infrastructure in vegetated areas.
