AbstractLung transplantation in the United States, under oversight by the Organ Procurement Transplantation Network (OPTN) in the 1990s, operated under a system of allocation based on location within geographic donor service areas, wait time of potential recipients, and ABO compatibility. On May 4, 2005, the lung allocation score (LAS) was implemented by the OPTN Thoracic Organ Transplantation Committee to prioritize patients on the wait list based on a balance of wait list mortality and posttransplant survival, thus eliminating time on the wait list as a factor of prioritization. Patients were categorized into four main disease categories labeled group A (obstructive lung disease), B (pulmonary hypertension), C (cystic fibrosis), and D (restrictive lung disease/interstitial lung disease) with variables within each group impacting the calculation of the LAS. Implementation of the LAS led to a decrease in the number of wait list deaths without an increase in 1-year posttransplant survival. LAS adjustments through the addition, modification or elimination of covariates to improve the estimates of patient severity of illness, have since been made in addition to establishing criteria for LAS value exceptions for pulmonary hypertension patients. Despite the success of the LAS, concerns about the prioritization, and transplantation of older, sicker individuals have made some aspects of the LAS controversial. Future changes in US lung allocation are anticipated with the current development of a continuous distribution model that incorporates the LAS, geographic distribution, and unaccounted aspects of organ allocation into an integrated score.

Background: Takotsubo syndrome is an increasingly recognised cardiac condition that clinically mimics an acute coronary syndrome, but data regarding its prognosis remain controversial. It is currently unknown whether acute coronary syndrome risk scores could effectively be applied to Takotsubo syndrome patients. This study aims to assess whether the Global Registry of Acute Coronary Events (GRACE) score can predict clinical outcome in Takotsubo syndrome and to compare the prognosis with matched acute coronary syndrome patients. Methods: A total of 561 Takotsubo syndrome patients was included in this prospective registry. According to the GRACE score, the population was divided into quartiles. The primary endpoint was all-cause mortality and the secondary endpoints were cardiocerebrovascular events (a composite of all-cause mortality, cardiovascular death, recurrence of Takotsubo syndrome and stroke). Results: The median GRACE risk score was 139±27. Takotsubo syndrome patients with a higher GRACE risk score mostly have a higher rate of physical triggers and lower left ventricular ejection fraction on admission. During long-term follow-up, all-cause mortality rates were 5%, 11%, 12% and 22%, respectively, in the first, second, third and fourth quartile ( P<0.001). After multivariate analysis, the GRACE risk score was found to be a strong predictor of all-cause mortality (odds ratio (OR) 1.68, 95% confidence interval (CI) 1.28–2.20; P=0.001) and cardiocerebrovascular events (OR 1.63, 95% CI 1.26–2.11; P=0.001). Moreover, all-cause mortality in Takotsubo syndrome patients was comparable with the matched acute coronary syndrome cohort. Conclusion: In Takotsubo syndrome, the GRACE risk score allows us to predict all-cause mortality and cardiocerebrovascular events at long-term follow-up.

AbstractImage search re-ranking is one of the most important approaches to enhance the text-based image search results. Extensive efforts have been dedicated to improve the accuracy and diversity of tag-based image retrieval. However, how to make the top-ranked results relevant and diverse is still a challenging problem. In this paper, we propose a novel method to diversify the retrieval results by latent topic analysis. We first employ NMF (Non-negative Matrix Factorization) Lee and Seung (Nature 401(6755):788–791, 1999) to estimate the initial relevance score to the query q. Then, the initial relevance score is fed into an adaptive multi-feature fusion model to learn the final relevance score. Next, the diversification process is conducted. We group all the images by semantic clustering and estimate the topic distribution of each cluster by topic analysis. The clusters are ranked based on the topic distribution vector and the final retrieval image list is obtained by a greedy selection mechanism based on the estimated relevances. Experimental results on the NUS-Wide dataset show the effectiveness of the proposed approach.

Bestämmelsen om tvångsinlösen i 22 kap. 1 § ABL är tvingande, vilket innebär att avsteg från vad som föreskrivs inte får göras genom bolagsordningen. Mot bakgrund av Högsta domstolens avgörande i NJA 2011 s. 429, synes bestämmelsens tvingande natur vara än mer vidsträckt. I förevarande fall ansågs nämligen ett aktieägaravtal, varigenom en majoritetsägare avstått från sin rätt att påkalla tvångsinlösen, sakna aktiebolagsrättslig relevans. Domstolen framhöll emellertid att sådana aktieägaravtal som huvudregel binder parterna. Författarna är av uppfattningen att blotta förekomsten av tvångsinlöseninstitutet utgör en inskränkning av principen om avtalsfrihet. Därutöver innebär Högsta domstolens avgörande att den välgrundade principen inskränks ytterligare. I förevarande uppsats utreds huruvida den sistnämnda inskränkningen är motiverad. Därutöver utreds även hur ett eventuellt avtalsbrott och en därtill inträdande skadeståndsskyldighet förhåller sig till det faktum att avtalsvillkoret saknar aktiebolagsrättslig relevans. Inledningsvis anser författarna att inskränkningen av avtalsfriheten som följer av NJA 2011 s. 429 är motiverad. Vidare har författarna konstaterat att skadeståndsskyldighet kan aktualiseras de lege lata eftersom parterna torde vara obligationsrättsligt bundna mot bakgrund av Högsta domstolens uttalande i NJA 2011 s. 429. Yttermera är författarna av uppfattningen att ett föreliggande skadeståndsansvar inte kan begränsas genom tillämpning av adekvansläran eller normskyddsläran. De lege ferenda anser författarna att ett aktieägaravtal som inskränker tvångsinlösenrätten inte bör tillerkännas obligationsrättslig verkan. Detta främst mot bakgrund av att det är en orimlig följd att ett avtal som inte kan göras gällande gentemot bolaget trots allt kan föranleda obligationsrättslig bundenhet och skadeståndsansvar.
The provision in chapter 22. 1 § of the Swedish companies act regarding compulsory purchases and sales of shares is imperative, meaning that derogations from the provision not is allowed through a company’s articles of association. The imperative nature of the provision has been further strengthened through the Swedish Supreme Court ruling NJA 2011 s. 429. In the mentioned case, a shareholders’ agreement in which a party has waived its right to claim a compulsory purchase, was found to lack affect in relation to the company. However, the court stressed that shareholders’ agreements, in principle, are valid and enforceable between the contractual parties, i.e. the shareholders. The authors’ opinion is that the mere existence of compulsory purchases and sales of shares constitute an infringement of the freedom of contract. Moreover, the mentioned court ruling entails an extension of the already existing infringement of this freedom. This thesis examines whether this, later infringement, is motivated. Furthermore, this thesis examines how a breach of a shareholders’ agreement – and liability to pay damages – relates to the fact that the contractual term that has been breached lacks affect toward the company.  According to the authors, the infringement of the freedom of contract derived from the mentioned court ruling is motivated. Furthermore, the authors have concluded that liability to pay damages may arise de lege lata, due to the fact that the contractual parties ought to be bound by the shareholders’ agreement according to the mentioned court ruling. In the authors’ opinion, the methods used to limit liability are not capable of doing so in this case. The authors suggest, de lege ferenda, that a term of a shareholder’s agreement where a party refrains from his right to claim a compulsory purchase or sales of shares not shall be binding between the parties.

In the academic field of sentiment analysis, knowing how to correctly classify the articles to the corrective sentiment-orientation is a hot research topic. If we can correctly classify the critiques, we can create more applied development. Sentiment detection mainly detects useful information in the articles, from the articles concerning mining or extract useful information, including viewpoint, fancy, and attitude. Although, these articles are valuable, but a lot of articles are dispersal, if reading all articles will take more time and human. If we only read a few articles, this may cause prejudice. Therefore, automatic sentiment detection and classification has become one of hot topics of the day. SentiWordNet is important vocabulary resource in the sentiment analysis. SentiWordNet is based on WordNet. SentiWordNet gives three sentiment scores to each synset. Sentiment scores are representative sentiment intensities on the synset. Through the three sentiment scores one can know the sentiment-orientation of word. However, in the past, sentiment analysis of the study focuses on how to build vocabulary resources and find out the articles that are sentiment-oriented, or use the sentiment vocabulary resources classifying the articles. Few scholars in the academic field of sentiment analysis have explored sentiment objective words influential sentiment classification. Therefore, this study uses vocabulary relevance to revise SentiWordNet objective sentiment score and improve the text classification.

Because of the execution of purchasing of weapon system is according to the weapon equipment systems of military purchasing of Ministry of National Defense. They don’t have an expressly stipulated in writing for the details of purchasing operation. In addition, the purchasing operation of weapon systems was conducted by project situation. It’s difficult to find an experience of execution can be reference. Therefore, the key of the project will be success or not is always depend on the abilities and experiences of the executors. In this research, a framework to support the reasoning of the casual relations of a malfunction work to assist the purchasing project management will be proposed. The purchasing process in a supply chain of a UAV (Unmanned Aerial Vehicle) cell will be modeled according SCOR(Supply Chain Operations Reference-model). Then, the causal relations among the purchasing activities based on the knowledge of military product purchasing experts will be quantified by using fuzzy method. DWR (Discourse, Worlds, Relations ontology)will be used to represent the ontology of the purchasing process knowledge. Finally, a reasoning engine will be constructed in the Protege by integrating the DWR ontology and SWRL rules.

The category of Registered Counsellors was created to provide psychological service at primary level to previously disadvantaged communities, yet there is lack of public and professional knowledge concerning this category. This study aimed at finding reasons why people choose to become registered counsellors and what their relevance and future is in South Africa. Social constructionism is the epistemological framework for this qualitative investigation. Three registered counsellors aged between 27 and 31 were chosen for this study using a combination of purposive and snowball sample technique. Rich data were collected through open ended interviews. This approach was chosen as the most relevant because it helped in giving a voice to the three ‘registered counsellors’ which in turn helped in getting a holistic understanding of the participants’ point of view. Thematic analysis technique was used to identify key themes. The main themes identified in comparative analysis were the following: the need to help, feeling of fulfillment, working in a multidisciplinary team, registered counsellors as first point of entry, experiencing barriers in terms of lack of recognition, confusion between registered counsellors and other healthcare professionals; and the role of HPCSA. The results showed that the participants became registered counsellors because of the need to help others. The results also showed that registered counsellors are relevant in South Africa because their services are needed for the well-being of society. There is a future for registered counsellors in South Africa to close the gap in terms of mental health workforce shortage.

Today, Duo to Taiwan's diversification of education and social diversity, students and parents no longer see high school as the only option. In 2009, the Ministry of Education proposed "Technical and vocational education is the main driving force for cultivating talents and improving national competitiveness. " in the International Symposium on Technical and Vocational Education. Students' achievements in learning have been one of the important issues of common concern for domestic and foreign educational research institutes for a long time. In this study, we use the "technical high school design group learning achievement information system" to establish factors that affect the learning achievement model based on the technical high school design group learning achievement survey data, combined with the use of SQL to view the relationship between family status, Economic status, father education, mother education, student gender and student learning achievement for technical high school students. This study integrates examination materials of students from one to three grade, school award-winning materials, and study materials into a the database system so that teachers can quickly enter and query the students' learning and achievement, which is used as the adjustment of counseling important reference.

The utilization of in vitro tests with a tiered testing strategy for detection of mild ocular irritants can reduce the use of animals for testing, provide mechanistic data on toxic effects, and reduce the uncertainty associated with dose selection for clinical trials. The first section of this thesis describes how in vitro methods can be used to improve the prediction of the toxicity of chemicals and ophthalmic products. The proper utilization of in vitro methods can accurately predict toxic threshold levels and reduce animal use in product development. Sections two, three and four describe the development of new sensitive in vitro methods for predicting ocular toxicity. Maintaining the barrier function of the cornea is critical for the prevention of the penetration of infections microorganisms and irritating chemicals into the eye. Chapter 2 describes the development of a method for assessing the effects of chemicals on tight junctions using a human corneal epithelial and canine kidney epithelial cell line. In Chapter 3 a method that uses a primary organ culture for assessing single instillation and multiple instillation toxic effects is described. The ScanTox system was shown to be an ideal system to monitor the toxic effects over time as multiple readings can be taken of treated bovine lenses using the nondestructive method of assessing for the lens optical quality. Confirmations of toxic effects were made with the utilization of the viability dye alamarBlue. Chapter 4 describes the development of sensitive in vitro assays for detecting ocular toxicity by measuring the effects of chemicals on the mitochondrial integrity of bovine cornea, bovine lens epithelium and corneal epithelial cells, using fluorescent dyes. The goal of this research was to develop an in vitro test battery that can be used to accurately predict the ocular toxicity of new chemicals and ophthalmic formulations. By comparing the toxicity seen in vivo animals and humans with the toxicity response in these new in vitro methods, it was demonstrated that these in vitro methods can be utilized in a tiered testing strategy in the development of new chemicals and ophthalmic formulations.

Indeed, a particularly salient characteristic of the papers presented here is their strong focus on interdisciplinarity and their breadth of scope, both in terms of methodology and content. The cross-pollination between different fields of Byzantine Studies is also a major point of the volume. Archaeology and art history have pride of place; it is especially in archaeological papers that one can grasp the vital importance of the interaction with the so-called hard sciences and with new technologies for contemporary research. This relevance of science and technology for archaeology, however, also applies to, and have significant repercussions in, historical studies, where – for example – the study of climate change or the application of specific software to network studies are producing a major renewal of knowledge. In more traditional subject fields, like literary, political, and intellectual history, the contributions to the present volume offer some important reflections on the connection between Byzantium and other cultures and peoples through the intermediary of texts, stories, diplomacy, trade, and war.

Hinchey’s classification of diverticulitis has become the most widespread system and while the Hinchey score may currently have less clinical relevance as it did in his time, its publication and eventual adoption marked a practice-changing paradigm shift in the way diverticulitis is viewed and managed today.

This chapter describes how to standardize electroencephalographic (EEG) interpretation and reporting in clinical practice. The Standardized Computer-Based Organized Reporting of EEG (SCORE) software program was developed by an international taskforce under the auspices of the International League Against Epilepsy and the International Federation of Clinical Neurophysiology. Clinically relevant features are scored by choosing predefined terms in the software. This process automatically generates a report and at the same time builds up a database for education, quality assurance, and research. SCORE is the template used for the interactive online educational EEG platform of this textbook.

A bilateral investment treaty (BIT) is relevant for a particular arbitration case only when the case falls within its scope of application. Normally the scope of application of a treaty includes four aspects: subject matter (ratione materiae), covered persons and entities (ratione personae), territorial application, and temporal application. The Chinese BITs are not an exception to this but they do, however, raise some special, perhaps unique, questions. This chapter deals with the four aspects on the scope of application, with special attention to those issues with “Chinese characteristics”. It also deals briefly with the issue of investment admission, which has been left, by and large, within the discretion of the host state.

This chapter considers the relevance of international law within the U.S. legal system to the United States’ initiation and conduct of war. After briefly reviewing some of the most relevant treaties relating to war and warfare, the chapter considers the Constitution’s distribution of war authority between Congress and the President. It then discusses how international law, including the provisions in the UN Charter relating to the authority of the Security Council, as well as collective self-defense treaties, might affect the President’s war authority. The chapter then shifts to the “war on terrorism” and discusses the relevance of international law, including the Geneva Conventions, to issues concerning the scope of the military’s detention authority in that conflict, with particular reference to the Supreme Court’s 2004 decision in Hamdi v. Rumsfeld. International law and other issues relating to the use of military commissions to try terrorist suspects are also considered. The chapter concludes by discussing legal debates relating to coercive interrogation and targeted killing.

Purpose: The purpose of this study was to validate the second version of the Prostate Imaging Reporting and Data System (PI-RADSv2) scores in predicting positive in-bore MRI-guided targeted prostate biopsy results across different non-university related institutions. The study focuses on PI-RADS v2 scoring because during the study period, PI-RADS v2.1 had not been released. Materials and Methods: This was a retrospective review of 147 patients who underwent multiparametric magnetic resonance imaging (mpMRI) of the pelvis followed by in-bore MRI-guided targeted prostate biopsy from December 2014 to May 2018. All lesions on mpMRI were rated according to PI-RADS v2 criteria. PI-RADS v2 scores were then compared to MR-guided biopsy results and pre-biopsy PSA values. Results: Prostate Cancer (PCa) was detected in 54% (80/147) of patients, with more prostate cancer being detected with each subsequent increase in PI-RADS scores. Specifically, biopsy results in patients with PI-RADS 3, 4, and 5 lesions resulted in PCa in 25.6% (10/39), 58.1% (33/55), and 86.0% (37/43) respectively. Clinically significant PCa (Gleason score ≥7) was detected in 17.9% (7/39), 52.7% (29/55), and 72% (31/43) of cases for PI-RADS 3, 4, and 5 lesions respectively. When the PI-RADS scoring and biopsy results were compared across different institutions, there was no difference in the PI-RADS scoring of lesions or in the positive biopsy rates of the lesions. The sensitivity, specificity, PPV, and NPV for PI-RADS 3-4 lesions were also not statistically different across the institutions for detecting Gleason 7 or greater lesions. Conclusion: Our results agree with prior studies that higher PI-RADS scores are associated with the presence of clinically significant PCa and suggest prostate lesions with PI-RADS scores 3-5 have sufficient evidence to warrant targeted biopsy. The comparison of PI-RADS score across different types of non-university practices revealed no difference in scoring and biopsy outcome, suggesting that PI-RADS v2 can be easily applied outside of the university medical center setting. Clinical Relevance: PI-RADS v2 can be applied homogeneously in the non-university setting without significant difference in outcome.

The 2008-2012 Country Program Evaluation for Guyana concludes that the Bank's strategy was aligned with the development objectives and economic priorities of the government. Nevertheless, its program has produced mixed results. In infrastructure, progress was achieved by consolidating the primary road network and extending the electrical grid to unserved areas; however, efforts to reduce losses in the electricity sector did not meet their target. In competitiveness, the financial and judicial sectors benefitted from institutional and structural reforms, and the business environment was strengthened through the adoption of streamlined procedures. However, key initiatives to deepen the financial market and increase access to credit have met with less success. Gains have also been made towards the achievement of the Millennium Development Goals; however, the absence of updated data in the social sectors hinders the evaluation of results. The average age of the loan portfolio is 4.56 years, the oldest in the Bank. With the launch of the Low Carbon Development Strategy, Guyana has an opportunity to benefit from global commitments for rainforest conservation and climate change. The Bank should continue to support new initiatives that lie within the LCDS, and should work closely with Guyana to enhance its national capacity to monitor and report the results. Although Guyana has shown the greatest improvement among D-2 countries in terms of its Country Institutional and Policy Evaluation score, FSO resources available to the country under the current program were 32% less than the allocation under the previous program. The sustainability of the Bank¿s work and its relevance in Guyana could be affected by a further reduction in future FSO allocations. The CS emphasized the strengthening and use of national systems by Bank projects. Several operations use the Integrated Financial Management System for accounting, and the intention is to use this for all new Bank-funded operations. Given the findings in this CPE, OVE recommends that the Administration: (i) take advantage of the current period of stability and growth to support Guyana on its path toward becoming a green economy; (ii) seek to leverage declining FSO resources; and (iii) continue strengthening Guyana's national systems.

Background : Nonalcoholic fatty liver disease (NAFLD) is increasingly recognized as a cause of chronic liver disease, and often results in the devastating outcomes of decompensated liver cirrhosis and hepatocellular carcinoma and is an important feature of metabolic syndromes and insulin resistance. The assessment of liver brosis is essential for predicting the prognosis and outcome of all forms of chronic liver disease. A liver biopsy is the gold standard for the assessment of liver brosis, but it has its limitations, which include life-threatening complications. Alternative methods of non-invasive laboratory and radiological testing for the assessment of liver brosis in NAFLD have evolved during the past decade, and these methods may be able to overcome the limitations of liver biopsy. These methods include the AST/ALT ratio, the AST platelet ratio index (APRI), and the Fibrosis 4 (FIB-4) score. This study was conducted in order to assess liver brosis using Fibroscan, and to compare these results to the use of AST platelet ratio index (APRI scores), and the AST/ALT ratios on NAFLD patients. METHODS: This was a cross sectional study conducted in King George Hospital Visakhapatnam,A total 122 patients were studied of which 65 were males and 57 were females.all the patients were subjected to relevant investigations including ultrasound abdomen,serum liver enzymes,broscan. The (SPSS) version 20 was used for the analysis.A Student's t-test was used to compare the AST/ALT ratio to the APRI scores between patients with advanced brosis higher than F2 and patients with mild to moderate brosis of F2 or less. RESULTS :The data showed that a high percentage of the NAFLD patients exhibited advanced stages of liver brosis based on the Fibroscan examinations. These results were supported by the strong correlation between the Fibroscan results and the AST/ALT ratio and APRI scores. Correlation analysis showed a signicant positive correlation between age and brosis scores (r = 0.27 with P = 0.004 for Pearson correlations). On the other hand, a signicant negative correlation between platelet count and stiffness scores was obtained (r = - 0.315 with P= 0.001 for Pearson correlations). Serum ALT level was determined to be signicantly negatively correlated with age by using Spearman correlations (r = - 0.232, and P = 0.022). A signicant positive correlation was observed between serum ALT and hepatic stiffness measurements using Spearman correlations (r = 0.284, and P = 0.005). This study has shown that the combination of Fibroscan CONCLUSION: and AST/ALT and APRI methods provides a valuable approach for assessing liver brosis in NAFLD patients. This can eliminate the need for liver biopsy in patients without clear indication

Age-based voting eligibility in Mexicos 2018 general election shows that the just-eligible score higher on several measures of low-cost political engagement compared to the just-ineligible. Exposure to information that the youth vote will be pivotal in the election increases eligible respondents interest in the presidential debate and in the election result. Information about current policy outcomes affects eligible voters future policy priorities in ways consistent with the incentives of eligible respondents to collect and process electorally relevant information on salient policy issues.

Resumo:
This paper defines the scope and methodology for the evaluation of the IDB Group's country program with the Government of the Dominican Republic for the period 2013-2016. Taking an independent and holistic view, the CPE will analyze the IDB Group's relationship with the country, in particular the relevance, efficiency, effectiveness, and sustainability of the program. The 2013-2016 CPE will be the fourth independent evaluation conducted by the Office of Evaluation and Oversight (OVE) of the Bank's country program with the Dominican Republic and the first to cover the work of the entire IDB Group in the country.
