Background: Sickle cell disorders are structural hemoglobinopathies, rendering red blood cells sickle shaped, less deformable and sticky leading to microvascular vaso-occlusion and premature red blood cells destruction which leads to varied clinical manifestations. It leads to lifelong morbidity thus affecting quality of life and contributes to early mortality thereby reducing the key national resources- the healthy workforce. This study was done to evaluate epidemiological and clinical profile of sickle cell disease attending the centre. Methods: This study was cross-sectional, observational study conducted at tertiary care hospital in Gujarat. After taking ethical clearance patients were enrolled as per inclusion and exclusion criteria and epidemiological and clinical profile of sickle cell disease patients was studied. Results: Mean age of sickle cell disease was 22.58 years. It was found in tribal communities of Gujarat like Rathwa, Baria, Tadvi etc. Commonest symptom was musculoskeletal pain (86.84%), followed by jaundice (71.05%), fever, dyspnoea, abdominal pain and chest pain. Most common systemic manifestation was pain crises (60.66%), followed by hemolytic anemia (31.15%), acute chest syndrome (30%), consolidation (11.67%), hepatopathy (10%) and avascular necrosis of hip. (6.56%).Conclusions: Sickle cell disease is seen in younger patients. In Gujarat mainly tribal communities are affected. Major systemic manifestations of sickle cell disease include pain crisis followed by hemolytic crisis, acute chest syndrome, hepatopathy and AVN of hip.

Putting things off as long as possible (procrastination) is a well-known tendency. Less well known is the tendency to attempt to get things done as soon as possible, even if that involves extra effort ( precrastination). Since its discovery in 2014, precrastination has been demonstrated in humans and animals and has recently been revealed in an analogous tendency called the mere-urgency effect. Trying to get things done as soon as one can may reflect optimal foraging, but another less obvious factor may also contribute—reducing cognitive demands associated with having to remember what to do when. Individual differences may also play a role. Understanding precrastination will have important implications for explaining why hurrying happens as often as it does and may help reduce the chance that haste makes waste.

What is usually referred to as "culture wars" increasingly determines online and offline ambient, especially after the onset of COVID-19 pandemic, followed by waves of internet-enhanced moral panic. In this paper, we aim to understand the intensification of moral panic not simply as a phenomenon that depends on the inflammatory online fears, the way digital natives or baby boomers use the Internet, or the rise of so-called "populism", but as a dynamic arising from the neoliberal politics itself. Instead of taking for granted that current flammable ambience is the result of a "crisis of neoliberal consensus", this article, through example of the United States, shows that culture wars are not only "cultural", but are a vehicle for neoliberal goals, in this case, the privatization of elementary education.

AbstractPeter van Inwagen (2001) has given a probabilistic answer to the fundamental question ‘why is there something rather than nothing?’: There is something, because the probability of there being nothing is 0. Some philosophers have recently examined van Inwagen's argument and concluded that it does not really work. Three points are central in their criticism: (i) the premise which states that there is only one empty possible world is false, (ii) the premise which states that all possible worlds have the same probability is not plausible and (iii) the argument is not significant for the question it sets out to answer. In this paper, I shall show that (i) even if there are many empty worlds, this does not necessarily invalidate the argument in its general lines, (ii) the examples they offer to support the intuition that possible worlds may have different probabilities fail, and (iii) even if the conclusion of the argument does not really answer the question van Inwagen sets out to answer, it is still not an insignificant response to the question.

Clinicians caring for pregnant adolescents recognize their patients' risk for having behavioral problems.1 Not all pregnant adolescents, however, experience such problems. In their article in this issue, Elster and colleagues2 reports that young urban mothers who engaged in three or more problem behaviors were more likely than were urban women who reported no problem behaviors to have had a child before they were 19 years of age. The interrelationship of adolescent problem behaviors was originally described approximately 20 years ago by Jessor et al.3,4 High-risk behaviors did cluster and young persons involved in one problem behavior were likely to engage in others (sexual intercourse, marijuana use, and problem drinking).4

Abstract Edith Wharton composed many dramatic monologues in her lifetime, and from the beginning of her poetic output, her personae tend to speak from a point of extremity between the living and the dead—often choosing, in fact, to narrate the moments of their own deaths. From her 1878 book of juvenilia, in a poem called “The Last Token,” Wharton made her interest in this particular placement evident; she herself, as evidenced by her reading and letters, was often strung between religious and secular modes of life, value, and belief. This article examines two of Wharton's “deathbed monologues” in detail: “The Leper's Funeral and Death,” unpublished in Wharton's lifetime, and—primarily—“Margaret of Cortona,” published in 1901. In each, a person speaks not only at the cusp of their bodily death but also from a vantage point past a symbolic death. In dividing and extending these speakers' deaths in this way, Wharton draws attention to the simultaneous fragility and persistence of the human: between and beyond traditional Christian divisions of body and soul.

Purpose The purpose of this paper was to study the unfolding of an urgent and extensive decommissioning program in Sweden, focusing on the public’s reactions and their arguments when opposing the decommissioning activities. Design/methodology/approach The public’s responses were studied through local media. Its content was surveyed and divided into actions and arguments. The arguments were further analyzed and categorized into inductively developed themes. Findings Protest activities, such as demonstrations, meetings and petitions, were not coordinated, but mostly carried out for withdrawals of unique services and services in remote areas. The public questioned the decision makers’ information, calculations and competence, the adequacy of the consequence analyses and whether the decommissioning activities would lead to any real savings. Patient and public safety, the vulnerable in society, and effects on the local areas were important topics. Thus, it seems the decision makers did not fully succeed in communicating the demonstrable benefits or create clarity of the rationales for decommissioning the particular services. Furthermore, it seems the public has a more inclusive approach to health services and their value compared to decision makers that need to keep the budget. Originality/value Decommissioning is an emerging field of research, and this study of the unfolding of an urgent and extensive decommissioning program contributes with evidence that may improve decommissioning policy and practice. The study illustrates that it may be possible to implement a decommissioning program despite public protest, but that the longer-term effects on the health system’s legitimacy need to be studied.

Small case reports have described infected RCCs, but this phenomenon remains uncharacterized. OBJECTIVE We reviewed RCCs over 23 years at our institution to determine factors predicting infection and recurrence. METHODS We retrospectively reviewed the magnetic resonance images, laboratory results, microbiology, and pathology of 176 RCC patients (1985–2008) who underwent initial operation at our institution (n = 170) or at another institution followed by recurrence managed at our institution (n = 6). RESULTS There were 3 RCC categories: cysts cultured intraoperatively during initial surgery (n = 21), cysts not cultured during initial surgery but cultured during subsequent surgery (n = 9), and cysts that were never cultured (n = 146). Cultured cysts were larger (1.6 vs 1.2 cm; P = .002) and had more frequent pituitary dysfunction (76% vs 30%; P < .001) than noncultured cysts. Restricted diffusion was also more common in cultured cysts (50% vs 0%; P = .02). Of cysts cultured at initial or subsequent surgery, 48% and 44%, respectively, had positive cultures (n = 14) and were treated with antibiotics. The most common organisms were Staphylococcus epidermidis (64%) and Propionibacterium acnes (57%). Kaplan-Meier recurrence rates were 13% (culture positive/antibiotic treated), 31% (culture negative/not antibiotic treated), and 9% (noncultured) (P = .002, cultured vs noncultured; P = .002, culture negative/not antibiotic treated vs non-cultured; P = .5 culture positive/antibiotic treated vs noncultured). CONCLUSION Suspected RCC infection, regardless of culture results, is a strong predictor of recurrence and may warrant antibiotic treatment. With antibiotic treatment, the recurrence rate of infected RCC approaches that of noninfected cysts. The higher recurrence rates reported in other series may reflect underrecognition of occult infection.
