If one disregards the several petty, and often blatantly unfair, complaints voiced by Dr. Ellen Klein, she raises an issue that is worth considering: Who is qualified to teach business ethics? Her answer is plain and straightforward: only philosophers need apply.

Objective: Current guidelines recommend home blood pressure (HBP) monitoring in patients diagnosis with hypertension and those with white coat hypertension or masked hypertension. Delicate classification of these patients are important because the intensification of antihypertensive drug therapy for patients with white coat uncontrolled hypertension or white coat hypertension may not be needed because cardiovascular risk seems not to be greater. Therefore, its detection is important to avoid overtreatment. Meanwhile patients with masked hypertension or masked uncontrolled hypertension and uncontrolled hypertension need further intensification of antihypertensive treatment. Dr. Answer project consists of 21 artificial intelligence (AI) medical software that cover eight major diseases including hypertension. The main object of Dr. Answer hypertension project is to predict the patients next visit office BP according to the home BP data and provide effective and precise treatment plan. Design and method: 2,057 patients and 156,117 pure HBP data were collected in the Dr. Answer hypertension project. Patients were enrolled in 1 tertiary center (Chonnam National University Hospital) and other 21 non-tertiary centers from January 2022 to September 2022. 1,447 patients and 153,712 were excluded due to insufficient HBP data and follow up loss. Finally 610 patients and 2,405 HBP data were evaluated in the AI program model. Results: Patients were 66.7 ± 14.4 years and proportion of women was 24.1%. and patients enrolled in tertiary center was 73.4%. Mean HBP 8weeks of systolic blood pressure and 4 weeks of diastolic blood pressure data were evaluated in the AI program model. Recurrent Neural Network, Long Short-term Memory, Attention algorism were selected in AI model program. Mean Absolute Error (MAE) and Root Mean Square Error (RMSE) and Accuracy data were used as the AI model parameter. MAE of systolic BP was 3.96 and MAE of diastolic BP was 2.1. Meanwhile, RMSE of systolic BP was 5.81 and RSME of diastolic BP was 2.88. Accuracy data showed 74.1% and 91.4% respectively. Conclusions: Dr. Answer hypertension AI medical software showed promising results for predicting future blood pressure data in hypertension patients. Further advancement in the AI program model would be needed.

This study is concerned with the search for narrative language in the Diwan (for whom the birds sing); the idea of research based on the study of the mechanisms and elements of the narrative, used by the poet in the poems of the Diwan, and the extent of the poeticty of the narrative structures in the Diwan? The study's underlying approach is the analytical descriptive approach; to reveal narrative language in the Diwan, and to clarify the elements and mechanisms of the narrative: (time, place, events, characters, dramatic conflict). Through this study, I have tried to answer the following problems: What buildings and linguistic phenomena did the poet rely on to build her narrative poetry language? What narrative mechanisms did the poet use in shaping their texts?  To what extent is the Najrani dialect used in the Diwan? Was the poet able to reach her narrative language into poetry? Research goals: By answering research problems; at the end, I aim to:  Study Diwan (For whom the birds sing) narrative study. - Disclosure of the poet's use of narrative language; Through the extrapolation of narrative mechanisms in the Diwan's poems. Illustrate the extent of narrative language poeticity in narrative structures in the Diwan. The Arabic novel has witnessed great demand at the present time by readers, critics, and researchers. This is due to the importance of the novel; In that it was one of the most famous types of prose literary arts in absorbing reality and portraying it in a brilliant artistic manner. Given the remarkable development that the Saudi novel has witnessed and its prominent position in the Arab literary space; I devoted my studies to talking about a novel from Najran literature (Shayuot) by the writer (Hatem Hussein Hatem). In my research, I focused on the personality element. Its role in constructing the novel and presenting its events. It also focused on the mechanisms of recruiting the main characters in the novel, and the relationship of the character element to other narrative elements: (place, time, language, and plot). Based on the structural approach; In that it was the most appropriate method for analyzing and studying fictional works. In this study, I tried to answer the following problems: Mechanisms for the writer’s use of novel characters in his novel? How were the main characters embodied in the novel?  The extent to which the writer linked the main characters of the novel to the other artistic elements of the narrative, and how did that linking affect the pace of narration in the novel? Research goals:  Introduction to storytelling, and clarifying the elements of narrative in the Arabic novel.  The study of the character in the Arabic novel: its concept, its dimensions.  Extrapolation of the main characters in the novel (Shayout) by Hatem Hussein Hatem.  Clarification of the character's relationship - being an essential element of the narrative - to other narrative elements (place, language, events).  Revive the Najran environment by revealing the accent, and the Najran heritage of the main characters in the novel. Research literature: The idea of the project has come to support the link between literature and society, as the specialized studies in Najran literature are almost too few, including:  The Poetry Movement in Najran in Pre-Islamic times and the beginning of Islam, Fayza Raddad Al-Otaibi, published by the Najran Literary Club 1430 (published master’s thesis).  Boys' games in ancient Arabic poetry and the extent to which they are associated with Najran games in modern times, Dr. Essam Muhammad Qabisi, Dr. Zuhair bin Hassan al-Omari, Journal of the Faculty of Science of the University of Fayoum, 2014.  The Proverbs of Najran region: Objective study, Dr. Zuhair bin Hassan Al-Omari, Journal of the Faculty of Dar Al-Ulum, Cairo University, 2016.  Zaid bin Abdul Madan Al-Najrani: His political role and literary heritage between two times, Dr. Awad bin Abdullah bin Nahi, dr. Al-Habishi, Rasha Abdul Rauf, e Wafa Alayan Elias Al-Shudayfat. "The Semiotics of the Title in the Diwan (Qadas Jasad) By the writer: Mona Bint Mohammed". International Journal of Religion 5, n.º 1 (24 de janeiro de 2024): 460–73. http://dx.doi.org/10.61707/4j5zng63.

The title is considered an essential element of literary text; it is the first threshold through which the reader enters the text. The title has evolved in literary text to become an independent science itself, and the first way to decipher the text and interpret it. It is considered a sign among the semiotic signs that appear at the beginning of the text, indicating its content as a whole. It is like a key that refers to the world of the text and becomes a reference for it. Given the importance of the title in literary texts, my study is dedicated to discussing the semiotics of the title in the collection "Qadas Jasad" by the writer (Mona Bint Mohammed), in an attempt to answer the following questions: - What is the semiotics of the title? To what extent does the semiotic approach apply to the collection "Qadas Jasad"? - What is the referential significance of the semiotics of the title in the collection? - Has the semiotic approach achieved different literary critical results? Research Objectives: My research study aims to achieve a set of objectives, the most important of which are: - Clarifying the nature of semiotics and the principles of modern semiotic methodology. - Presenting the most important contemporary semiotic trends and their characteristics. - Conducting a semiotic study of the title in the collection of poems "Qadas Jasad" and explaining the important dimensions included in the titles of the collection. - Explaining the semantic structure of the title by presenting the structural and semantic aspects of the title and revealing the relationship between the titles of the poems and their contents. Research Literature: The idea of the project came to support the connection between literature and society, as specialized studies in Najrani literature are very few, including: - The poetry movement in Najran during the Jahiliyyah and the early Islamic period, by Faiza Raddad Al-Otaibi, published by Najran Literary Club in 1430 AH (published master's thesis). - Games for boys in ancient Arabic poetry and their connection to games in the Najran region in modern times, Dr. Essam Mohammed Qubaisi, Dr. Zuhair bin Hassan Al-Amri, Journal of the Faculty of Dar Al-Uloom at Al-Fayoum University, 2014. - Folk proverbs in the Najran region: An objective study, Dr. Zuhair bin Hassan Al-Amri, Journal of the Faculty of Dar Al-Uloom at Cairo University, 2016. - Yazid bin Abdul Madaan Al-Najrani: His political role and literary heritage between two eras, Dr. Awad bin Abdullah bin Nahi, Dr. Essam Mohammed Qubaisi, Journal of Bisha University for Humanities, 2022. Methodology of Research: In my research, I have adopted the semiotic approach; because the title is a semiotic sign that needs to be explored in terms of its dimensions and meanings; through: deconstruction, interpretation, and analysis. Therefore, I have relied on both: the analytical method in analyzing the structural and semantic composition of the title, and the inductive method in extracting the relationship between the content of the poem and the title. Research Plan: I have divided my study into: - The first topic: The nature of semiotic methodology, its direction, and its origin. - The first subtopic: Definition of semiotic methodology in language and terminology. - The second subtopic: Semiotic directions. - The third subtopic: The emergence of semiotics in contemporary critical studies. - The second topic: The title and its functions. - The first subtopic: Definition of "title" in language and terminology. - The second subtopic: The importance, types, and functions of the title. - The third topic: The semiotics of the title in the Diwan (Qadas Jasad). - The first subtopic: The structural organization of the title in the Diwan (Qadas Jasad). - The second subtopic: The semantic structure of the title in the Diwan (Qadas Jasad). - Then the conclusion, which includes the most important results. Then the books and references. - The first topic: The nature of semiotic methodology, direction, and origin.

Resumo:
Abstract The retinal disease Diabetic retinopathy (DR) is one of the most probable causes of blindness. Automatic detection of DR is mostly done using convolutional neural networks (CNNs) on colour retinal images. This work in contrast uses stochastic variational deep kernel learning (SVDKL) for DR grading, combining a deep CNN with Gaussian processes (GPs) into a single end-to-end trainable model, which promises to provide predictions with a reliable uncertainty estimate exploiting approximate Bayesian inference. Evaluating the performance and uncertainty calibration of SVDKL on DR grading compared to a plain CNN, the EfficientNet-B0, preliminary results on a subset of the Kaggle DR dataset show a naturally enhanced uncertainty calibration for SVDKL over the plain CNN as well as a good diagnostic performance. Despite SVDKL achieving a slightly reduced accuracy, incorrect predictions were in closer proximity to the target stages, which is beneficial for clinical diagnosis due to minimizing the cost related to severe misclassifications.

e13064 Background: Breast cancer patients (BCP) and family caregivers (CG) describe their experience as a difficult hike, without access to a map, relevant gear, or a guide. The shrinking oncologist workforce has ~24 minutes per patient to communicate the condition, treatment plan and answer questions. Many use hand-drawn visuals – analogous to a map or process flow diagrams. 97% of BCP use Dr. Google™, which results in “Cancer Information Overload” and non-evidence-based activities. A team of survivors, cartographers, and oncologists developed an interactive cancer map and self-management platform to coach BCP to navigate treatment. Using equitable design, features include: (a) 7th grade reading level; (b) written content and audio/video assets; (c) supporting the user from diagnosis through survivorship to make the platform accessible to a diverse population. After acceptance testing showed high willingness to use & recommend (ASCO Quality 2023;552), we tested the platform’s usability through observed use and task-analysis. Methods: A convenience sample of BCP, CG and oncology HCP [including advanced practice providers (APP)] underwent a usability test in two waves. We defined a set of tasks the users should be able to accomplish. We observed the users during a 30-minute virtual session and recorded the session, noted the task accomplishments, and any issues. Issues from wave 1 were addressed and incorporated into the test for wave 2. Results: We tested two waves of users between November 30, 2023 and February 1, 2024. The results are shown in the table. After wave 1, we modified the platform and tested the maps with wave 2 users. Two core usability features were added between waves: (a) user drill down to access a more detailed level of the map; (b) a product tour guiding the user through the tool, which addressed qualitative user observations that showed initial confusion as to how to begin navigation. Conclusions: Cartographically represented maps of the cancer journey and the self-management tools are acceptable to BCP, survivors, and CG and are usable. Per the ASCO Quality 2023;552 poster, the platform is acceptable to HCPs and not perceived to increase work, and APPs found value in the platform to enable education. Features have been added to ensure a diverse population can easily use the platform including guided walk through and welcome videos. [Table: see text]

AbstractTabling is a powerful resolution mechanism for logic programs that captures their least fixed point semantics more faithfully than plain Prolog. In many tabling applications, we are not interested in the set of all answers to a goal, but only require an aggregation of those answers. Several works have studied efficient techniques, such as lattice-based answer subsumption and mode-directed tabling, to do so for various forms of aggregation. While much attention has been paid to expressivity and efficient implementation of the different approaches, soundness has not been considered. This paper shows that the different implementations indeed fail to produce least fixed points for some programs. As a remedy, we provide a formal framework that generalises the existing approaches and we establish a soundness criterion that explains for which programs the approach is sound.

Hiding in Plain Sight is an illustrated history of the former and present cinemas in the city of Leeds and an interactive website that engages Leeds residents in a participatory reminiscence project about cinemas and cinema-going. Launched in the summer of 2020, it is the most recent output of an ongoing cinema history research project at the Hyde Park Picture House, a much-loved 106 year-old Grade 2 listed independent cinema. The Hiding in Plain Sight project was one of a series of activities hosted by the organisation in line with their objective to engage as many people as possible with the cinema’s valuable heritage. The author of this paper, Dr Laura Ager, was employed by the Hyde Park Picture House as their Creative Engagement Officer between 2019 and 2020 and in this role she developed project’s framework and its research strategy. In this article she outlines the project’s origins and stages of development and considers how the methods used in the research phase have interacted with the design and production of the Hiding in Plain Sight website to give unexpected insights. She also reflects on some essential stages of project re-negotiation during the extraordinary and turbulent summer of 2020.

AbstractAn answer set is a plain set of literals which has no further structure that would explain why certain literals are part of it and why others are not. We show how argumentation theory can help to explain why a literal is or is not contained in a given answer set by defining two justification methods, both of which make use of the correspondence between answer sets of a logic program and stable extensions of the assumption-based argumentation (ABA) framework constructed from the same logic program.Attack Treesjustify a literal in argumentation-theoretic terms, i.e. using arguments and attacks between them, whereasABA-Based Answer Set Justificationsexpress the same justification structure in logic programming terms, that is using literals and their relationships. Interestingly, an ABA-Based Answer Set Justification corresponds to an admissible fragment of the answer set in question, and an Attack Tree corresponds to an admissible fragment of the stable extension corresponding to this answer set.

‘So why do you want to do Acute Medicine?’ – it’s a common interview question, and one for which most prospective trainees will have a well-prepared answer. Sometimes the candidate will focus on the opportunities for multi-professional working and teamwork, as well as interactions with critical care and other specialities; however almost all will mention the wide variety of conditions with which acute physicians are faced in their daily practice. What may appear a simple problem at the time of presentation may have a twist in the tail which will catch out the unwary. The need to maintain a broad differential diagnosis is a key skill for the acute physician: I frequently need to remind trainees that ‘Query ACS – measure 12 hour troponin’ is not an acceptable management plan. It is crucial to remember that even when a condition like acute coronary syndrome has been ‘excluded’, a serious cause may still be lurking. Several of the articles in this edition illustrate such caveats. In the review of the diagnosis and management of aortic dissection, the variability of presentation is illustrated with a case where the onset of symptoms preceded the hospital admission by several weeks and was initially labelled as ‘musculoskeletal’. In this case it was the chest x-ray which raised the clinical suspicion when a change in the mediastinal contour was identified. However, serious spinal causes of chest pain, such as the spontaneous extradural haematoma on p76, will only be diagnosed by more complex imaging; this will be hard to arrange without a clear clinical indication. The rarity of this condition means that most will not have come across a case and will therefore be unaware of the need to consider this diagnosis. The importance of a careful history, refreshingly emphasised in the recent NICE guidance on chest pain of possible cardiac origin as summarised by Charlotte Cannon in this edition, cannot be understated. Many readers will have come across patients with erythema nodosum, referred as ‘unresolving cellulitis’ – often these turn out to have sarcoidosis as described in Dr Chakraborty’s case series, although other underlying causes need also to be considered. Cellulitis may also be a manifestation of a subcutaneous source of infection, particularly when it appears in an unusual site; Denzil May’s case illustrates the need to image the abdomen when presented with abdominal wall cellulites – on this occasion an underlying psoas abscess was the cause although appendix abscesses, inflammatory bowel disease, diverticular abscesses and pelvic collections also need to be considered. The opportunity to learn a practical procedure has been a subject of considerable debate amongst acute medical trainees over recent years, and has been highlighted in this journal in the past. Much of the focus in previous editions has been on echocardiography. Ultrasound is another practical procedure which is listed in the training curriculum for Acute Medicine, although the feasibility of training acute medical trainees in this area has not been fully evaluated. Dr Ismaeel’s study comparing the outcome of ultrasound undertaken at the bedside by an acute medical trainee and that undertaken in the radiology department may stimulate some debate on the value of this. This study looked specifically at diagnostic ultrasound of the abdomen and demonstrated that some conditions could be identified at the bedside, though probably not with enough reliability to prevent the need for a subsequent departmental ultrasound. The key is whether the specific questions can be answered by the ‘occasional’ sonographer using portable equipment: ‘are the bile ducts dilated?’, ‘are the kidneys obstructed?’, ‘are there metastases in the liver?’, ‘is there ascites or is it just fat?’ – knowing the answers to such questions can inf luence management out of hours, although currently most patients will have to wait until the morning for a scan. A formal training programme in acute medical ultrasound will need to focus clearly on these specific areas, rather than generic diagnostic ultrasound skills. Many readers will be familiar with the rapid turnover of AMU nursing staff, particularly at a junior level, which can undermine the ability to develop a stable workforce. In a questionnaire study in Birmingham and Dundee, consultant nurses Liz Lees and Liz Myers have produced some interesting data which may help to improve recruitment and retention of nurses on Acute Medical Units. It seems that, as with junior doctors, it is the variety which is the main attraction, as well as the being the main factor which keeps nurses interested in working on the AMU. Correspondence relating to this and any other issues relating to articles in this journal would be most welcome.

Abstract STUDY QUESTION What are patients’ and fertility staff views of talking about possible IVF/ICSI failure and need for multiple cycles in treatment planning? SUMMARY ANSWER Healthcare professionals (HCPs) typically plan treatment on a cycle-by-cycle basis but HCPs and patients see benefits in talking about possible IVF/ICSI failure and the consequent need for multiple cycles to better prepare patients for this possibility, to support them through treatment challenges and to foster a sense of collaboration with the clinic in achieving the shared goal of treatment success. WHAT IS KNOWN ALREADY Many patients need more than one round of IVF/ICSI stimulation to achieve their parenthood goals. About 60% of patients are willing to plan for multiple cycles of treatment in advance of treatment engagement. However, it is not clear how patients are informed about the high possibility of failure and the subsequent need for multiple cycles during their treatment planning consultations, and how approaches could be optimized. STUDY DESIGN, SIZE, DURATION Qualitative focus groups with HCPs working at fertility clinics, patient advocates employed by patient charities (April 2020) and patients (July and August 2020). Patients were eligible if they had had a consultation to start a first/repeat stimulated IVF/ICSI cycle in the 8 weeks prior to participation, were aged 18 or older (upper age limit of 42 years for women), in heterosexual relationships and fluent in English. Eligible HCPs and patient advocates were those employed at a fertility clinic or charity, respectively. PARTICIPANTS/MATERIALS, SETTINGS, METHOD Focus group topic guides progressed from general questions about fertility consultations to if and how the possibility of treatment failure and need for multiple cycles was introduced and discussed in (attended/own) clinics. After, preferences regarding planning IVF/ICSI on a multi-cycle or cycle-by-cycle basis were explored. Focus groups were recorded, and recordings transcribed and analysed using framework analysis to identify shared, unique and incongruent themes across participant groups. MAIN RESULTS AND THE ROLE OF CHANCE Twelve HCPs, 2 patient advocates and 10 patients participated in six semi-structured online focus group discussions. All patients were childless and had been trying to conceive for ∼3 years. Framework analysis generated four themes and one meta-theme across participant groups. The meta-theme showed planning IVF on a cycle-by-cycle basis is the norm at clinics and that this affects how treatment is planned and the acceptability of a shift towards planning for multiple cycles, which was perceived as beneficial despite some apprehension. The four themes were: (i) heterogeneity in information provision during treatment planning; (ii) the need for improved HCP-patient collaboration; (iii) the need to temper optimism about treatment success; and (iv) apprehension, benefits and preferences regarding multi-cycle planning. LIMITATIONS, REASONS FOR CAUTION Most patients were women from private fertility clinics with no previous treatment experience recruited from social media websites, mainly associated with patient support groups. Similarly, most HCPs were women from private fertility clinics. WIDER IMPLICATIONS OF THE FINDINGS The findings suggest that shifting from cycle-by-cycle to multi-cycle approaches in IVF planning is possible. Achieving this shift, like other shifts in IVF (e.g. single embryo transfer), is likely to require collaboration among all stakeholders (e.g. users, staff, policymakers, regulators) to ensure that costs and benefits are balanced through using appropriate benchmarks, avoiding deflating optimism, fostering a sense of collaboration and supporting patients through challenges of multi-cycle IVF. We address the problem of evolutionary system design (ESD) by means of answer set programming modulo difference constraints (AMT). The goal of this design approach is to synthesize new product variants or generations from existing products. We start by formalizing the underlying system synthesis problem and design space exploration process, which consists of finding the Pareto front with respect to latency, cost, energy, and similarity measures between the two designs. We then present AMT-based encodings to capture all of these aspects. The idea is to use plain ASP for conflict detection and resolution and for routing and to use difference constraints for scheduling. Moreover, we propose a new approach for expressing the similarity that we use at three alternative levels of AMT-based design space exploration, namely, at the strategic, heuristic, and objective levels, which is performed to guide the exploration towards designs of high interest. Last but not least, we systematically evaluate the emerging techniques empirically and identify the most promising AMT techniques.

The ceramic material discussed in the following paper was collected during a survey conducted by the Dept. of Archaeology and History of Art at Bilkent University, Ankara, Turkey. The survey, under the direction of Doctors Ilknur Özgen and Marie-Henriette Gates, was undertaken in the summer of 1991 and covered the coastal area from the site of Yumurtalık, southeast of Misis and Misis Dağ, and continued along the coastal plain to include the site of Kinet Höyök, near Dörtyol north of the Iskenderun Plain (The Hatay). This entire region can be considered the eastern half of the Cilician coastal plain (Fig. 1).The objectives of the survey were two-fold: to surface collect ceramic remains from mounds in the investigation area, and to conduct a geomorphological study of the plain and its relation to the surrounding topographical regions (alluvial plains to the east and west-foothills of the Amanus and Anti-Taurus to the north and northeast). A detailed report on the geomorphology of this region has been published by the geomorphologist who accompanied the survey team, Dr. Sancar Özaner, of the Maden Tetkik ve Araştırma Enstitüsü (Mineral and Research Exploration Institute).

Background: The high use and pressure on the knee cause osteoarthritis to occur frequently. Prolotherapy is a non-surgical injection therapy used to treat chronic pain in musculoskeletal conditions, including knee OA. This solution can create local irritation, inflammation, and tissue regeneration, thereby increasing the strength of damaged ligaments, tendons, and intra-articular structures. Therefore, the aim of this research is to assess the changes in VAS and WOMAC scores, as well as plain radiographs of knee OA patients after hypertonic dextrose prolotherapy. Methods: This experimental case series was done at the Physical Medicine and Rehabilitation Polyclinic of dr. Ramelan Navy Hospital Surabaya from December 2021 to April 2022, by giving hypertonic dextrose prolotherapy injection intraarticularly to 3 knee OA subjects using a 22G or 23G needle. This procedure was performed 3 times on each subject, with an interval of 4 weeks each. The assessments were carried out with VAS and WOMAC scores, as well as plain radiographs of the knees before the procedure and 1 month after the third injection for evaluation. Results: In all 3 subjects, the knee joint spaces were slightly widened on plain radiographs, as well as decreased VAS and WOMAC scores after injection. Conclusion: Hypertonic dextrose prolotherapy here had minimal side effects, affordable prices, and a fast process. It also causes a reduction in pain and joint stiffness and causes functional improvement. Prolotherapy may be considered a therapy for knee OA patients.
