Objective: Current guidelines recommend home blood pressure (HBP) monitoring in patients diagnosis with hypertension and those with white coat hypertension or masked hypertension. Delicate classification of these patients are important because the intensification of antihypertensive drug therapy for patients with white coat uncontrolled hypertension or white coat hypertension may not be needed because cardiovascular risk seems not to be greater. Therefore, its detection is important to avoid overtreatment. Meanwhile patients with masked hypertension or masked uncontrolled hypertension and uncontrolled hypertension need further intensification of antihypertensive treatment. Dr. Answer project consists of 21 artificial intelligence (AI) medical software that cover eight major diseases including hypertension. The main object of Dr. Answer hypertension project is to predict the patients next visit office BP according to the home BP data and provide effective and precise treatment plan. Design and method: 2,057 patients and 156,117 pure HBP data were collected in the Dr. Answer hypertension project. Patients were enrolled in 1 tertiary center (Chonnam National University Hospital) and other 21 non-tertiary centers from January 2022 to September 2022. 1,447 patients and 153,712 were excluded due to insufficient HBP data and follow up loss. Finally 610 patients and 2,405 HBP data were evaluated in the AI program model. Results: Patients were 66.7 ± 14.4 years and proportion of women was 24.1%. and patients enrolled in tertiary center was 73.4%. Mean HBP 8weeks of systolic blood pressure and 4 weeks of diastolic blood pressure data were evaluated in the AI program model. Recurrent Neural Network, Long Short-term Memory, Attention algorism were selected in AI model program. Mean Absolute Error (MAE) and Root Mean Square Error (RMSE) and Accuracy data were used as the AI model parameter. MAE of systolic BP was 3.96 and MAE of diastolic BP was 2.1. Meanwhile, RMSE of systolic BP was 5.81 and RSME of diastolic BP was 2.88. Accuracy data showed 74.1% and 91.4% respectively. Conclusions: Dr. Answer hypertension AI medical software showed promising results for predicting future blood pressure data in hypertension patients. Further advancement in the AI program model would be needed.

Introduction: Invasive vulvar cancer is very small. Usually occurs in women, but at the same time. An average of 90% - 92% of vulvar cancer patients are squamous cell carcinomas. Treatment of vulvar cancer is surgery, radiotherapy, chemotherapy or even combination according to the stages and references of each patient. In Dr. Cipto Mangunkusumo, Radiotherapy was selected to be one of the treatment options for stage III-IV vulvar cancer because it can provide maximum value on target and minimum on healthy tissue by using IMRT technique and frog leg position with special sterofoam feed fixation tool. Objective: Describe the procedure for managing external radiation therapy in cases of vulvar cancer with IMRT technique and a special fungicide feeding device at the Radiotherapy Department Dr. Rsupn. Cipto Mangunkusumo. Method: The type of descriptive skin research by observing directly and recording the required data and approach with case study, discussion and question and answer with radiotherapist, radiation oncologist and patient at the Department of Radiotherapy RSUPN. Cipto Mangunkusumo in march 2018. The sample used is 1 primary data sample of patients with vulvar carcinoma squamosa cell IVA. Results: Patients with IV carcinoma of squamous cell carcinoma A treatment plan with qualitative RT objective, using 10 MV energy, radiation plan 25 x @ 2 Gy. With IMRT technique and special fixture fixture tool from sterofoam to position the patient to form the frog spring. Conclusion: The use of IMRT techniques and semi frog leg positioning is highly appropriate and effective for vulvar cancer. It comes from DVH achievement doses at very maximum targets and on the minimal surrounding organs and patient comfort. The cost value of the IMRT technique is 0.2 cm

Tenaga pengajar (ustadz dan ustadzah) di pondok pesantren menghadapi tantangan dalam pembinaan santri. Perkembangan teknologi informasi pada era revolusi industri 4.0 telah berdampak pada pola pendidikan pondok pesantren. Oleh karena itu, diperlukan pelatihan Kepemimpinan Bertumbuh untuk meningkatkan kompetensi kepemimpinan para asatidz. Program pengabdian kepada masyarakat ini bermitra dengan Yayasan Al Asror Patemon Gunungpati Semarang. Metode yang digunakan adalah Seminar Pendidikan Strategi Kepemimpinan Bertumbuh dalam Menghadapi Tantangan Era Digital. Kegiatan ini juga bekerjasama dengan LP3 UNNES yang telah menjadikan Yayasan Al Asror sebagai mitra. Kegiatan pengabdian dilaksanakan di aula Madrasah Aliyah (MA) Al Asror, pada hari Sabtu, 8 Agustus 2020. Kegiatan diawali dengan penandatanganan dokumen rencana pelaksanaan kegiatan antara FE UNNES dan Yayasan Al Asror sebagai tindak lanjut Kerjasama (MOU) antara UNNES dan Yayasan. Narasumber kegiatan adalah Plt. Wakil Rektor Bidang Kerjasama dan Perencanaan, Dr. Hendi Pratama, MA. yang memaparkan materi mengenai “Menguatkan Reputasi Pribadi dan Lembaga Al Asror”. Narasumber kedua adalah Ketua LP3 UNNES, Dr. Ngabiyanto, M.Si. yang menyampaikan materi “Strategi Kepemimpinan Bertumbuh dalam Menghadapi Tantangan Era Digital”. Peserta kegiatan sangat antusias mengikuti kegiatan ini. Hal ini tampak pada partisipasi peserta saat narasumber mengajak dialog langsung dengan peserta. Peserta juga sangat antusias pada sesi tanya jawab. Teachers (ustadz and ustadzah) in boarding schools face challenges in the construction of students. The development of information technology in the era of industrial revolution 4.0 has had an impact on the pattern of boarding school education. Therefore, it is necessary to train “Kepemimpinan Bertumbuh” to improve the leadership competence of the asatidz. This community service program is partnered with Yayasan Al Asror Patemon Gunungpati Semarang. The method used is The Education Seminar of Growing Leadership Strategy in Facing the Challenges of the Digital Era. This activity also collaborates with LP3 UNNES which has made Al Asror Foundation as a partner. The service was held in madrasah Aliyah (MA) Al Asror hall, on Saturday, August 8, 2020. The activity began with the signing of a document on the implementation plan of activities between FE UNNES and Al Asror Foundation as a follow-up to the Cooperation (MOU) between UNNES and the Foundation. The speakers of the activity were Plt. Vice Rector for Cooperation and Planning, Dr. Hendi Pratama, MA. which presents material on "Strengthening the Personal Reputation and Institution of Al Asror". The second speaker was the Chairman of LP3 UNNES, Dr. Ngabiyanto, M.Si. who delivered the material "Leadership Strategy to Grow in Facing the Challenges of the Digital Era". Participants of the activity are very enthusiastic to participate in this activity. This is evident in the participation of participants when the speaker invites direct dialogue with the participants. Participants were also very enthusiastic at the question and answer session.

This study is concerned with the search for narrative language in the Diwan (for whom the birds sing); the idea of research based on the study of the mechanisms and elements of the narrative, used by the poet in the poems of the Diwan, and the extent of the poeticty of the narrative structures in the Diwan? The study's underlying approach is the analytical descriptive approach; to reveal narrative language in the Diwan, and to clarify the elements and mechanisms of the narrative: (time, place, events, characters, dramatic conflict). Through this study, I have tried to answer the following problems: What buildings and linguistic phenomena did the poet rely on to build her narrative poetry language? What narrative mechanisms did the poet use in shaping their texts?  To what extent is the Najrani dialect used in the Diwan? Was the poet able to reach her narrative language into poetry? Research goals: By answering research problems; at the end, I aim to:  Study Diwan (For whom the birds sing) narrative study. - Disclosure of the poet's use of narrative language; Through the extrapolation of narrative mechanisms in the Diwan's poems. Illustrate the extent of narrative language poeticity in narrative structures in the Diwan. Research literature: The idea of the project has come to support the link between literature and society, where studies specializing in the Najrani literature are almost too few, and from it: Najran Poetry Movement in Pre- Islamic times and the beginning of Islam, Fayza Radad al-Uteibi, Najran Literary Club 1430 AH (Master's thesis published). Boys' games in ancient Arabic poetry and the extent to which they are associated with Najran games in modern times, Dr. Essam Muhammad Qabisi, Dr. Zuhair bin Hassan Al-Amri, Journal of the Faculty of Dar Al-Ulum of the University of Fayoum, 2014. Folk proverbs in the Najran region: an objective study, Dr. Zuhair bin Hassan Al-Omari, Journal of the Faculty of Dar Al-Ulum, Cairo University, 2016. Zaid bin Abdul Madan Al-Najrani: His political role and literary heritage between two times, Dr. Awad bin Abdullah bin Nahi, d. Essam Muhammad Qubaisi, Journal of the University of Pesha for Humanities, 2022. Research methodology: In my research, I relied on the descriptive approach to the definition of narrative poetry and the analytical approach to clarifying elements of the narrative in the poems of the Diwan; through analysis of narrative structures in the Diwan. Search Plan: Introduction: A brief overview of the writer, Noha Ali.  The first section: Definition of narrative, and the poetic mechanisms of narrative structures. The second topic: the poetic mechanisms of narrative in the self-presence, and the movement of characters in the poems of the Diwan. Third topic: The mechanisms of narrative poetry in (time, space) in the Diwan. Fourth topic: Poetry Mechanisms of Dramatic Attendance in (Conflict, Dramatic Monologue) in the Diwan. The conclusion: with the most important findings and recommendations, then books and references.

The Arabic novel has witnessed great demand at the present time by readers, critics, and researchers. This is due to the importance of the novel; In that it was one of the most famous types of prose literary arts in absorbing reality and portraying it in a brilliant artistic manner. Given the remarkable development that the Saudi novel has witnessed and its prominent position in the Arab literary space; I devoted my studies to talking about a novel from Najran literature (Shayuot) by the writer (Hatem Hussein Hatem). In my research, I focused on the personality element. Its role in constructing the novel and presenting its events. It also focused on the mechanisms of recruiting the main characters in the novel, and the relationship of the character element to other narrative elements: (place, time, language, and plot). Based on the structural approach; In that it was the most appropriate method for analyzing and studying fictional works. In this study, I tried to answer the following problems: Mechanisms for the writer’s use of novel characters in his novel? How were the main characters embodied in the novel?  The extent to which the writer linked the main characters of the novel to the other artistic elements of the narrative, and how did that linking affect the pace of narration in the novel? Research goals:  Introduction to storytelling, and clarifying the elements of narrative in the Arabic novel.  The study of the character in the Arabic novel: its concept, its dimensions.  Extrapolation of the main characters in the novel (Shayout) by Hatem Hussein Hatem.  Clarification of the character's relationship - being an essential element of the narrative - to other narrative elements (place, language, events).  Revive the Najran environment by revealing the accent, and the Najran heritage of the main characters in the novel. The title is considered an essential element of literary text; it is the first threshold through which the reader enters the text. The title has evolved in literary text to become an independent science itself, and the first way to decipher the text and interpret it. It is considered a sign among the semiotic signs that appear at the beginning of the text, indicating its content as a whole. It is like a key that refers to the world of the text and becomes a reference for it. Given the importance of the title in literary texts, my study is dedicated to discussing the semiotics of the title in the collection "Qadas Jasad" by the writer (Mona Bint Mohammed), in an attempt to answer the following questions: - What is the semiotics of the title? To what extent does the semiotic approach apply to the collection "Qadas Jasad"? - What is the referential significance of the semiotics of the title in the collection? - Has the semiotic approach achieved different literary critical results? Research Objectives: My research study aims to achieve a set of objectives, the most important of which are: - Clarifying the nature of semiotics and the principles of modern semiotic methodology. - Presenting the most important contemporary semiotic trends and their characteristics. - Conducting a semiotic study of the title in the collection of poems "Qadas Jasad" and explaining the important dimensions included in the titles of the collection. - Explaining the semantic structure of the title by presenting the structural and semantic aspects of the title and revealing the relationship between the titles of the poems and their contents. Research Literature: The idea of the project came to support the connection between literature and society, as specialized studies in Najrani literature are very few, including: - The poetry movement in Najran during the Jahiliyyah and the early Islamic period, by Faiza Raddad Al-Otaibi, published by Najran Literary Club in 1430 AH (published master's thesis). - Games for boys in ancient Arabic poetry and their connection to games in the Najran region in modern times, Dr. Essam Mohammed Qubaisi, Dr. Zuhair bin Hassan Al-Amri, Journal of the Faculty of Dar Al-Uloom at Al-Fayoum University, 2014. - Folk proverbs in the Najran region: An objective study, Dr. Zuhair bin Hassan Al-Amri, Journal of the Faculty of Dar Al-Uloom at Cairo University, 2016. - Yazid bin Abdul Madaan Al-Najrani: His political role and literary heritage between two eras, Dr. Awad bin Abdullah bin Nahi, Dr. Essam Mohammed Qubaisi, Journal of Bisha University for Humanities, 2022. Methodology of Research: In my research, I have adopted the semiotic approach; because the title is a semiotic sign that needs to be explored in terms of its dimensions and meanings; through: deconstruction, interpretation, and analysis. Therefore, I have relied on both: the analytical method in analyzing the structural and semantic composition of the title, and the inductive method in extracting the relationship between the content of the poem and the title. Telling the story of how Christians learned to stop worrying and embrace the probable, this chapter considers how thinkers evaluated the certainty and truth of Jesus revelations, both canonical and contemporary. The latter included revelations made to women, with gender rendering their accounts especially unreliable, or, in the plain ken, especially trustworthy. Some writers used Jesus as a tool to demolish the claims of reason; others found in Jesus the compelling certainty they sought. Throughout the East, from Greece to Russia, hesychasts combined the Jesus prayer with breathing techniques to compensate for the instability of knowledge. Jean Gerson raised a difficult question: Should a priest celebrate the eucharist—that is, affect the presence of the body and blood of Jesus—after a nocturnal emission? His investigations led to the development of a plain-ken approach that celebrated a messy probabilism as an acceptable answer to uncertainty, first in ethics, and later more generally. In these ways a plain-ken vision of history, a messy world of contingency, arose between what was necessarily true and what was impossible.

The chapter discusses how Anna kept her pretense understand Arabic, while Dr. Helmy pretended to be the ideal pro-Nazi Arab envisioned by Goebbels’ propaganda office. It talks about Dr. Helmy’s outward show of absolute loyalty to Adolf Hitler, which is an act he had perfected in Gestapo custody together with his Egyptian friends.

A medical resident was on duty at New York Hospital one night in 1979 when a 27-year-old security guard was admitted with a rare form of pneumonia. As inexperienced as the resident was, he knew that this very rare condition was usually diagnosed only in people with a history of cancer, organ transplantation, or other conditions involving immune system suppression. This otherwise healthy young man had none of those. Weeks later, when the resident presented this case at inner-city rounds, a number of hands shot up. These clinicians had seen similar cases. The resident's report of this New York City outbreak was one of three that formed the basis for the first published report of the disease we would come to know as AIDS. Three years later, this physician, Dr. Henry Masur, arrived at the National Institutes of Health where he joined established NIH researchers already anxiously trying to understand this deadly new condition. At key communications offices on the NIH campus, phones were ringing incessantly. Reporters all around the world wanted to know what NIH was doing about it. As public communicators, we were learning about the disease along with the researchers. Why did it seem to disproportionately affect gay men? Why were people with the disease dying from ordinary infections? We were learning the answers to these questions as they unfolded, translating what we learned into plain English, and getting the information out to the public. A prominent AIDS researcher came into the office of one of my colleagues, sat next to her, and made a simple drawing of how immune cells appeared to be affected by the new virus, explaining it to her at the same time scientists themselves were just beginning to understand it. Dr. Masur is now chief of the Critical Care Medicine Department of the NIH Clinical Center. I covered his account of his first meeting with an AIDS patient for an NIH employee newsletter when he delivered the NIH Astute Clinician Lecture in 2002, which honors scientists who observe and investigate unusual clinical occurrences.

In the age of globalisation and digitalisation, the amount of information is constantly increasing. The society’s responsibility is also increasing, as there is a large part of society that needs special support in order to make this information accessible. One way to do that is being able to communicate in easy or plain language. The aim of this article is to reveal easy and/or plain language as one of the measures for an inclusive society, and pinpoint the possibilities of expanding it in Latvia, where there has been little discussion about the different use and text creation methods of easy and plain language. Therefore, professor Dr. paed. Sarmīte Tūbele draws attention to the specifics of plain language target groups from a speech therapist’s point of view and summarizes the diverse characteristics of learning disabilities. When discussing the possibilities of adjusting learning materials, she emphasises the importance of integrating quality illustrative elements into easy language texts in order to encourage the reader’s perception and interest. Dr. paed. Ieva Sproģe discusses methods of text creation/adaptation in the context of translatology and lists criteria for processing and creating texts, as well as suitable classic translation methods, emphasising the main goal of easy and plain language – improving the ability of perceiving a text by simplifying it, i. e., making it more understandable. Subsequently, a summary of suggestions for easy and plain language text requirements and using them with target groups is provided. Methodology: The research was carried out using literature review and a study of easy language organisations, as well as events organised by them and their guidelines. Results: The article provides a brief summary of easy language history and describes current international discussions about easy language, pointing out both positive development tendencies and problematic aspects; the most common classifications of easy language and its target groups are described, as well as the interpretations and possibilities of classification for the concept “easy to read”; conclusions are made about the possibilities of developing easy language text creation in Latvia. Easy language is described in the context of translating/adapting and speech therapy by collecting suggestions for easy and plain language text requirements and adapting them for the target groups. This is the first time that an easy and plain language description in a scientific, multidisciplinary context has been provided in Latvia.

Abstract Film cooling is one of the most critical technologies in modern gas turbine engine to protect the high temperature components from erosion. It allows gas turbines to operate above the thermal limits of blade materials by providing the protective cooling film layer on outer surfaces of blade against hot gases. To get a higher film cooling effect on plain surface, current study proposes a novel strategy with the implementation of hole-pair into ramp. To gain the film cooling effectiveness on the plain surface, RANS equations combined with k-ω turbulence model were solved with the commercial CFD solver ANSYS CFX11.0. In the numerical simulations, the density ratio (DR) is fixed at 1.6, and the film cooling effect on plain surface with different configurations (i.e. with only cooling hole, with only ramp, and with hole-pair in ramp) were numerically investigated at three blowing ratios M = 0.25, 0.5, and 0.75. The results show that the configuration with Hole-Pair in Ramp (HPR) upstream the cooling hole has a positive effect on film cooling enhancement on plain surface, especially along the spanwise direction. Compared with the baseline configuration, i.e. plain surface with cylindrical hole, the laterally-averaged film cooling effectiveness on plain surface with HPR is increased by 18%, while the laterally-averaged film cooling effectiveness on plain surface with only ramp is increased by 8% at M = 0.5. As the blowing ratio M increases from 0.25 to 0.75, the laterally-averaged film cooling effectiveness on plain surface with HPR is kept on increasing. At higher blowing ratio M = 0.75, film cooling effectiveness on plain surface with HPR is about 19% higher than the configuration with only ramp.

Vertol Aircraft Corporation's Vectodyne concept (based on the theories of re-known aerospace engineer Dr. Alexander Lippisch and other lesser known inventors) was a tubular wing, dual contra-rotating, propeller driven aircraft design that never left the drawing board. It was created in answer to a 1956 U.S. Army request for unconventional VTOL Tactical Transport weight and performance proposals that would offer direction in the development of a new type of cargo aircraft that would potentially be deployed within the 1962-1965 timeframe. This paper will address the aerodyne origins of the Vectodyne, its strengths, weaknesses and dismissal, and finishing with the vindication flight of the propulsive technology behind its design via a single engine surveillance drone, ending with the question, "After fifty years of false starts, is the Vectodyne a viable concept with today's technology?"

In this article singular points of a parallel manipulator are obtained based on a novel geometrical method. Here we introduce the constrained plain method (CPM) and some of its application in parallel mechanism. Given the definition of constraint plane (CP) and infinite constraint plane (ICP) the dependency conditions of constraints is achieved with the use of a new theorem based on the Ceva geometrical theorem. The direction of angular velocity of a body is achieved by having three ICPs with the use of another theorem. Finally, with the use of the above two novel theorems singularities of the 3UPF_PU mechanism are obtained. It should be emphasized that this method is completely geometrical, involving no complex or massive calculations. In the previous methods based on the Grassmann Geometry, the mechanism needs to be statically analyzed at first, so that the Inverse Jacobian Matrix is achieved, and then the Plucker-Vector is derived. This task is somewhat inconvenient and in the end there are plenty of conditions remained to be pondered in order to obtain the singularity conditions, while the novel method introduced here, involves no tiring calculations neither the analysis of numerous conditions and yields the answer quickly.

This paper focuses on the legitimacy of using conventional predictions, based on adiabatic wall temperature (Taw) and heat transfer coefficient (HTC) augmentation, of heat flux into a film cooled leading edge. To answer this question, the heat flux predicted using Taw was compared to the heat flux into a conducting leading edge. The study used numerical simulations with the k-ε turbulence model of FLUENT. The model simulated was a three-row leading edge with one row of holes on the stagnation line and two additional rows located at ±25°, which has been experimentally studied extensively. The adiabatic wall temperature was obtained from an adiabatic simulation. External heat transfer coefficients were determined from a constant heat flux simulation using a density ratio of DR = 1.0, as is commonly done experimentally. A conjugate heat transfer simulation was also run to give the surface temperature and heat flux into the conducting leading edge. Overall, the heat transfer was well predicted with the use of Taw and HTC augmentation. However, between the holes, conventional predictions of heat transfer were poor, with disparity up to 30% when compared with the conducting wall heat flux obtained from the conjugate heat transfer simulation. Thermal boundary layer profiles were used to understand the disparity between the heat fluxes obtained from the conventional prediction and the conducting wall simulation.

In this module, students have the opportunity to discover the hidden world of parasites: they will come face to face with living parasites, learn about what differentiates parasites from free-living species, observe some common adaptations to a parasitic lifestyle, explore the ecological role of parasites in food webs, and assess how parasite abundance might change in a changing world. To accomplish these goals, this module includes an introductory PowerPoint presentation (including a video of parasite ecologist Dr. Chelsea L. Wood delivering this introductory lecture) and two exercises. The first exercise is a wet lab that involves dissecting an easy (and disturbing) source of live parasite material: fresh fish from your local seafood market. The second exercise is a computer lab that will allow students to engage with real data to answer the question: how do human impacts on ecosystems change the abundance of parasites in wildlife? This module will introduce students to the basics of parasite ecology and provide an opportunity to practice their data analysis and interpretation skills.

Eliciting lived experiences, we seek to identify those experiences that have shaped the lived practice of a career and desire to answer: (1) How have experiences informed a career? (2) What does the lived educational experiences of an educator portend for the future of education? This inquiry incorporates the lived past experiences of the co-authors. Additionally, we seek to offer a path forward for the future of the profession that we love and have been grateful to experience. This data was part of a hermeneutic inquiry research project involving myself and my father, Dr. Leonard L. Gregory. The data was recorded utilizing a phone recording and then setting up a zoom meeting whereby the audio was converted onto a word document. From there, the data was uploaded into Dedoose where it was stored and coded for initial themes. The journal entries that I made were from a personal print copy journal. I utilized this journal after the initial round of recordings with my father. I used it to jot down questions that I had from the previous meeting, and we utilized it to make notations of thoughts that we both had afterwards from the meetings we had. The text Conducting Hermeneutic Research: From philosophy to Practice by Nancy J. Moules, et al., was a key influence and outlines the use of my data according to hermeneutic standards.

Oxon Run Parkway (OXRN) is a 51-hectare (126-acre) natural area within Washington, D.C. administered by the National Park Service under National Capital Parks East (NACE). The original plan called for a road, slated to follow Oxon Run stream, but this never came to fruition; despite this, the moniker stuck. The majority of the original Oxon Run Parkway is managed by the District of Columbia. The section of Oxon Run Parkway under NPS jurisdiction contains wetlands and forests, as well as the only McAteean magnolia bogs still remaining in the District. The lower Cretaceous Potomac Group, known as one of the few dinosaur-bearing rock units on the east coast of North America, crops out within Oxon Run. One of the most prevalent fossil-bearing resources are the siderite, or “bog iron” sandstone slabs that sometimes preserve the footprints or trackways of various vertebrates, including dinosaurs. Such trackways have been reported from Potomac Group outcrops throughout the Atlantic Coastal Plain of Maryland and Virginia. In 2019, National Capital Parks-East took possession of such a track, referred to a dinosaur, collected by paleontologist Dr. Peter Kranz. This report was compiled after a paleontological survey of Oxon Run Parkway and is intended as a supplement to the National Capital Parks East Paleontological Resource Inventory (Nelson et al. 2019). This report contains information on the history of Oxon Run Parkway and its geology, as well as discussion of the fossil track.

The broad objective of the project was to develop a feasible approach to combat diarrheal disease caused by ETEC through the development of a low-cost oral immunogen in tomato fruit, expressed in the context of a prototype tomato that would answer the shortcomings of plant oral vaccines, especially in terms of produce handling and control of gene escape. Specifically, the goals for Boyce Thompson Institute (BTI) on this project were to develop transgenic tomato lines that express the enterotoxigenic E. coli (ETEC) heat-labile enterotoxin (LT) subunits A and/or B for use in oral edible vaccines, and to optimize expression and assembly of these antigens in tomato fruits.LT-B is a useful vaccine antigen against ETEC disease, since antibodies against LT-B can prevent binding and delivery of the holotoxinLT. Mutant forms of the toxic LT-A subunit that have reduced toxicity can be co-expressed and assembled with LT-Bpentamers to form mutant LT (mLT) complexes that could be used as mucosaladjuvants for other oral vaccines. Work on the project is continuing at Arizona State University, after Dr. Mason moved there in August 2002. A number of approaches were taken to ensure the expression of both subunits and bring about their assembly inside the transgenic fruits. Initially, expression was driven by the fruit-specific E-8 promoter for LT-B and the constitutive CaMV 35S promoter for LT-A(K63). While LT-B accumulated up to 7 µg per gram ripe fruit, assembled LT-K63 was only 1 µg per gram. Since promoter activities for the two genes likely differed in cell type and developmental stage specificity, the ratios of A and B subunits was not optimal for efficient assembly in all cells. In order to maximize the chance of assembly of mLT in fruit, we focused on constructs in which both genes are driven by the same promoter. These included co-expression plasmids using the 35S promoter for both, while switching to attenuated mLTs (LT-R72 and LT-G192) that have shown greater potential for oral adjuvanticity than the initial LT-K63, and thus are better candidates for a plant-derived adjuvant. Other, more novel approaches were then attempted, including several new vectors using the tomato fruit-specific E8 promoter driving expression of both LT-B and mutant LT-A, as well as a dicistronic construct for co-expression of both LT-B and mutant LT-A genes from a single promoter, and a geminivirusreplicon construct. We describe in the Appendix the results obtained in transgenic tomato lines transformed with these constructs. Overall, each contributed to enhanced expression levels, but the assembly itself of the holotoxin to high levels was not observed in the fruit tissues. The Israeli lab’s specific objective was to develop transgenic tomato lines expressing the LTholotoxin antigen bearing attributes to prevent gene escape (male sterility and orange fruit color) and to improve the dissemination of the oral vaccine (long shelf-life tomato cherry fruit or tomato processing background). Breeding lines bearing a number of attributes to prevent gene escape were developed by combining material and backcrossing either to a tomato cherry background, or two different processing backgrounds. Concomitantly, (these lines can be utilized for the creation of any future oral vaccine or other therapeutic-expressing tomato, either by crosses or transformation), the lines were crossed to the holotoxin-expressing tomatoes received from the United States, and this transgenic material was also incorporated into the backcrossing programs. To date, we have finalized the preparation of the cherry tomato material, both non-transgenic (bearing all the desired attributes), and transgenic, expressing the holotoxin. The level of expression of LT-B in the cherry fruits was comparable to the original transgenic tomatoes. Since it was not higher, this would necessitate the consumption of more fruits to reach a desired dose. A final backcross has been made for both the non-transgenic and the transgenic material in the processing lines. Auxin sprays resulted in high percentages of fruit set, but the processing genotypes gave many puffed fruits.
