O cultivo de células tronco (CT) e a descoberta de técnicas que promovem a diferenciação tecidual apontam para novos domínios da medicina: a terapia celular e a engenharia de tecidos (ET). As limitações da medicina atual encontram na ET uma nova possibilidade terapêutica, com tratamentos regenerativos ou substitutivos para os tecidos danificados. Dentre as limitações médicas encontram-se as doenças arteriais obstrutivas periféricas (DAOP). A DAOP está presente em 5% da população e cerca de 20 a 30% dos casos podem evoluir para amputação do membro inferior acometido, mesmo com os tratamentos hoje existentes. A ET apresenta-se como uma ferramenta promissora para a resolução dos problemas cardiovasculares, particularmente para os que afetam as artérias de pequeno diâmetro (<5 mm), sendo factível a produção de enxertos vasculares autólogos, sob a ótica da medicina personalizada. O objetivo deste trabalho foi construir de um modelo experimental de vaso sanguíneo para enxerto, utilizando técnicas de ET para a diferenciação de células tronco mesenquimais de tecido adiposo (CTMta) em células endoteliais. Inúmeras etapas metodológicas foram necessárias: identificação da melhor forma de descelularizar as veias de animais doadores para confecção do arcabouço (scaffold), onde foram comparados 3 diferentes protocolos utilizando os detergentes Triton X-100, deoxicolato de sódio (DS) e dodecil sulfato de sódio (SDS); obtenção, expansão e caracterização por citometria de fluxo com anti-CD90 de CTMta; comprovação da adesão das CTMta sobre o scaffold utilizando imunofluorescência e o kit Qtracker; estabelecimento do índice de apoptose das CTMta sobre o scaffold descelularizado por eventual resíduo químico; análise histológica por HE e realização de imunohistoquímica com...
Stem cells (SC) cultivation and the development of new techniques to promote tissue differentiation evolved to new medicine fields of research: cell therapy and tissue engineering (TE). The current limitations in medicine rely on TE for new therapies as they provide regeneration and substitution of the damaged tissue. Within therapeutic limitations are the peripheral arterial obstruction diseases (PAD). PAD affects 5% of population and about 20-30% of cases may progress to amputation of the limb, even if the treatments that exist today. TE is a promising tool for cardiovascular diseases, especially those where the vessel is a small caliber artery (<5 mm), as it is possible to recreate the structure of the vessel with autologous vascular grafts from the perspective of personalized medicine. This research aimed to propose the creation of an experimental blood vessel using TE techniques and adipose-derived stem cell differentiation into endothelium. Several techniques were required: to gather enough veins and remove its cells, using three different protocols with Triton X 100, sodium deoxycholate (SD) and sodium dodecil sulfate (SDS); to obtain, cultivate and characterize adipose-derived stem cells with the CD90 marker and flow citometry; to prove the cells could adhere to the scaffold using immunofluorescence and Qtracker kit; to establish the cell apoptosis on the scaffold for possible chemical residue; histology and immunohistochemistry on the cell seeded scaffold using the fascin marker. Were obtained 90 jugular and cava vein segments and were decellularized, withing 11 different protocols. The best protocols were 2h of 1% SD, 1h of 2% SDS and 2h of 1% SDS. Texto completo da fonteThis thesis adds to the limited existing evidence on SHS as an independent risk factor for PAD. There was an overall association between exposure to SHS and PAD, with suggestion of a dose-response relationship. However, the association varied by age category. Individuals aged <60 were more strongly associated with the prevalence of IC. SHS was significantly associated with incident PAD only in men. This thesis further demonstrates that exposure to SHS carries a disproportionately higher cardiovascular risk than active smoking for a given level of smoke exposure. Telomere shortening per year of age may be an intermediate step between SHS and CVD including PAD. This also supports the association between SHS exposure and the atherosclerosis-related biomarkers, which play an important role in the pathophysiology of PAD. Further research is needed in the future to better understand the association between SHS and PAD, and the underlying mechanisms. The research in this thesis supports the need to protect the general public from exposure to SHS.

Estudos epidemiológicos mostraram que a prevalência de doença arterial periférica (DAP) é alta e tem como fator etiológico principal a aterosclerose. Os fatores de risco para o desenvolvimento da aterosclerose são bastante conhecidos, e nos últimos anos, foi identificado, além destes, a homocisteína. Embora ainda não haja consenso sobre a dose exata e a forma de utilização, principalmente do folato na forma de suplementos, adequação alimentar, fortificação de cereais, para o tratamento da hiper-homocisteinemia, diversos estudos realizados em pacientes com doença vascular coronariana, cerebral e periférica, mostraram que o folato, isoladamente ou em combinação com a vitamina B6 e B12 pode reduzir as concentrações sanguíneas homocisteína e também diminuir a concentração de alguns marcadores de biológicos do processo de aterosclerose. No entanto, estudos recentes não comprovaram este benefício sobre o processo inflamatório associado à hiper-homocisteinemia. Portanto, a suplementação ou, a fortificação, ou a adequação dietética isolada do folato é uma terapêutica custoefetiva na prevenção e no controle da homocisteinemia, mas ainda persiste inconclusiva quanto ao impacto sobre a evolução das doenças vasculares.
Epidemiological studies have shown that the prevalence of peripheral arterial disease (PAD) is high and has atherosclorosis as its etiological factor. Risk factors for the development of atherosclerosis are widely known, and, for the past years, homocysteine has been identified as one of them. Although there is still no consensus as to the exact dose and manner of use, mainly of folate in the form of supplements, eating adjustment, cereal strengthening, for the treatment of hyperhomocysteinemia, several studies done in coronary, cerebral and peripheral vascular disease patients have shown that folate, isolatedly or in combination with vitamins B6 and B12, may reduce blood concentrations of homocysteine as well as the concentration of some biological markers in the process of atherosclerosis. However, recent studies have not corroborated this benefit for the inflammatory process associated with hyperhomocysteinemia. Consequently, supplementation, strengthening and diet adjustment devoid of folate are not cost-effective therapies in the prevention and control of homocysteine, but it is still inconclusive regarding the impact on the evolution of vascular diseases.

Texto completo da fontePeripheral Artery Disease (PAD) is a cardiovascular condition with increasing morbidity and mortality rates. Despite this, it remains a neglected condition among cardiovascular diseases, as it does not consistently receive optimal medical treatment. Even within PAD, certain populations are overlooked in epidemiological research, although they appear to be increasingly affected by PAD. The general objective of this thesis is thus to study the epidemiology of PAD and its progression within specific neglected populations (women, a rural sub-Saharan African population, and the very elderly). We also aim to assess whether influenza vaccination is as overlooked among PAD patients as their overall medical management. Our work consisted of four complementary studies. Factors associated with PAD, its management, and their temporal evolution were examined in women and elderly subjects using data from a French cohort. Women are significantly more affected by critical limb ischemia than men, yet are less likely to receive revascularization procedures or optimal medical treatment. Very elderly subjects are increasingly presenting with critical ischemia, with a rise in revascularization interventions, though optimal medical treatment remains insufficiently followed over time. Influenza vaccination coverage remains limited among PAD patients, highlighting the need to increase patient awareness about the importance of this vaccination. Finally, the Ankle-Brachial Index (ABI) appears associated with cardiovascular events and 5-year mortality in the general population
