Resumo:
L’objectif du présent article est de résumer les aspects centraux des soins aux patients atteints d’un cancer avancé de la bouche avec métastases dans la région de la tête et du cou. Ces patients sont traités par approche multimodale. L’intervention chirurgicale limite leur capacité de parler et d’avaler, tandis que la chimiothérapie et la radiothérapie peuvent entraîner nausées et vomissements, diarrhée, mucite, cystite, déshydratation, néphrotoxicité, dermatite, dépression médullaire et carences alimentaires entraînant une perte de poids. Il est possible de contrer ces effets toxiques à l’aide de mesures d’autosoins. Dans le cadre de leurs interventions d’enseignement, les infirmières en oncologie pourraient transmettre aux patients et aux familles les connaissances nécessaires à l’application de ces mesures d’autosoins. Background:Correct nasal spray techniqueand patient compliance are the two mostimportant factors in the use of steroid nose spray. Objectives:To find out if patientsuse steroid nasal spray with a correctspray technique and if they are compliant in using the spray. Design: Patient-reported survey Participants: 100 participants (1 excluded) Main outcome measures 1) Nasal spray technique 2) Patient compliance Results: Out of 99 participants included in this study only one patient used steroid nose spray with a completely correct spray technique. 40% of patients received guidance instructions from the prescriber about the spray technique. 70% of patients reported using their spray regularly and 53% of patients were found using the correct dosage of the spray. Conclusions: Wrong spray technique and patient noncompliance is common and can be corrected. This survey emphasizes the importance of giving written and verbal instructions about the use of spray to the patients.

Background: Proactive involvement of patients in the design of safe care is a WHO-defined goal of the current decade. This paper investigated (a) which patient involvement activities and measures are implemented in German health care facilities for patient safety, and (b) which recommendations for action can be derived for our health care facilities. Aim and method: A literature and Internet search was conducted on institutional practices and organizational activities in Germany to improve patient safety with the involvement of patients. Interviews with five experts were conducted who advised our approach. An agenda for future research projects and first recommendations for clinical practice was formulated afterwards. Results: The evidence base on established measures to incorporate patient perspectives into the strategic promotion of patient safety in healthcare facilities in Germany is scarce and inconsistent. Patient surveys and the use of patient advocates are common. It remains unclear how patient councils as well as self-help groups have the potential to contribute to the promotion of patient safety in practice. Conclusions and agenda: An agenda is proposed. A more in-depth empirical survey and description of the situation in Germany, taking into account the experience of experts from clinical practice, is necessary to obtain a complete picture of the currently established measures for patient involvement, to uncover potential for realization and to define necessary framework conditions.

This study was to analyze the effect on patient safety and image quality improvement by lowering the arm and applying a self-made device during chest CT examinations of unconscious patients. From April 1 to September 30, 2022, 78 patients (male: 42, female: 36, mean age: 67 years old) among intensive care unit patients who underwent chest CT scan were retrospectively reviewed. The patients were classified into three positions as follows: both arms raised (A position), both arms straight out using the self-made device (B position), and both arms straight out to the side (C position). For radiation dose comparison, dose length product (DLP) values were measured and compared to each other. For image quality evaluation, image noise, signal to noise ratio (SNR), contrast to noise ratio (CNR), and contrast to dose ratio (CNRDR) values were compared, and qualitative evaluation was also performed. The average DLP value of the B position was significantly lower than that of the C position, and it was relatively higher than that of the A position, but there was no significant difference statistically.(p=0.051) In the quantitative evaluation, it was evaluated that the B position maintains a quality quite similar to that of the A position despite the lowering of the both arms. In addition, in the qualitative evaluation, an average score was obtained between A and C. In conclusion, it was confirmed that the use of the self-made device for rapid and safe chest CT examination of unconscious patients is useful in improving patient dose and image quality.

ABSTRACT Patient satisfaction reflects the quality of an institution’s delivery of health care services. Patient satisfaction is a critical factor in improvement or complete recuperation to wellness because satisfied patients will mean better compliance to treatment or follow up. As reported in the 2017 Annual OPD census of the Department of Obstetrics and Gynecology, there were 2617 total patient consultations. Majority of these (2138 or 81.7 %) were obstetrical cases. The average number of consultations per day (for Obstetrics and Gynecology patients combined) is 91. Therefore, the Out-Patient Clinic plays an important role in prenatal care specifically the prevention of adverse maternal and neonatal morbidity and mortality. This study aimed to assess the level of patient satisfaction among obstetrical patients in the Out Patient Department of the Obstetrics and Gynecology Section at Metropolitan Medical Center from July 1, 2018 to July 31, 2018 and to see the relationship between socio-demographic characteristic and the level of patient satisfaction. This is a descriptive, cross sectional study. The level of patient satisfaction assessed as to physical facilities, interaction between doctor and patient, interaction between nurses and patient, and registration service. There were sixty three subjects in this study. Sixty three were satisfied with inclusion criteria. The validated questionnaire was the pretested questionnaire which was 96 % valid and reliable by Cronbach’s analysis. Statistical analysis was done which showed respondents were generally satisfied. There were significant relationship association between socio-demographic characteristics and level of patient satisfaction with P value < 0.05.Key Word : Patient Satisfaction, Socio Demographic Characteristic

BACKGROUND: Acute pulmonary oedema is a life-threatening syndrome diagnosed based on radiological and clinical findings. However, to our knowledge, no studies have investigated this syndrome in critically ill patients. OJECTIVE: To describe the prevalence of radiologically and clinically diagnosed pulmonary oedema (RCDPO) in critically ill patients, characteristics of diagnosed patients, and treatments and outcomes in this patient population. METHODS: We conducted a retrospective study using natural language processing to identify all radiological reports of pulmonary oedema among patients who had been admitted to single tertiary intensive care unit (ICU) over a 1-year period (January 2015 to January 2016). We reviewed clinical data, discharge diagnosis, treatment and outcomes for such patients, and used multivariable logistic regression analysis to identify the association of RCDPO with various outcomes. RESULTS: Out of 2001 ICU patients, we identified 238 patients (11.9%) with RCDPO. Patients with RCDPO were more acutely ill, had more chronic liver disease and had more chronic renal failure than critically ill patients who did not have RCDPO. They were typically admitted with acute cardiovascular disease; were more likely to receive invasive mechanical ventilation and continuous renal replacement therapy; had longer duration of ICU and hospital stay; were more likely to die in hospital; and, if discharged alive, were more likely to be admitted to a chronic care facility. In total, 46 RCDPO patients (19.3%) died in hospital. On multivariable analysis, only age and continuous renal replacement therapy were independently associated with mortality. In contrast, invasive mechanical ventilation was associated with a 2.5 times greater odds of radiological resolution. CONCLUSION: RCDPO affected about one in eight ICU patients. Such patients were sicker and had more comorbidities. The presence of RCDPO was independently associated with higher risk of death. Invasive mechanical ventilation was the only intervention independently associated with greater odds of radiological resolution.

BackgroundThe presentation of multiple simultaneous trauma patients to an Emergency Department is likely to place significant stress and strain on trauma care resources. Currently there is limited literature and no UK or multicentre data available to understand this impact. The aim of this study was to identify patient outcomes when there are simultaneous major trauma patients. We hypothesised that with increasing numbers of simultaneous trauma patients an increase in mortality may be seen.MethodsThe Trauma Audit and Research Network (TARN) database was interrogated from 2010–2015 to identify simultaneous major trauma patients. We defined simultaneous trauma as occurring when there was more than one trauma patient within an Emergency Department at any one time.Patient age, sex, Glasgow Coma Scale and Injury Severity Score (ISS) were recorded. A standardised comparison using a stratified Ws statistic was conducted to compare mortality between groups. Secondary outcomes included length of hospital and intensive care (ICU) stay.ResultsOf 2 07 094 patients, 33.7% were eligible simultaneous trauma patients. 55.7% of patients were male, median age was 61 and median ISS was 9. COVID-19 has now been declared a global pandemic with evolving incidence rates and fatalities. It is important to identify vulnerable populations who will be impacted most by this pandemic leading to higher mortality rates compared to the general healthy population. Although older patients and patients with co-morbidities fall into this vulnerable group, patients with hematologic and oncologic malignancies on active cytotoxic treatments are at even greater risk as they are both myelosuppressed and immunosuppressed. In addition to following the universal guidelines recommended by the Centers for Disease Control (CDC), it is important to also institute guidelines for cancer centers to help protect this vulnerable population. We review the current data, risks, and recommendations for COVID-19 in cancer patients.

Vascular diseases of the brain due to their high prevalence and severe consequences for the health of the population represent a major medical and social problem. In the structure of cerebrovascular diseases significant weight have chronic forms of vascular lesions of the brain, which in the domestic literature is traditionally treated as a discirculatory encephalopathy. In Uzbekistan the number of sufferers with chronic forms of cerebrovascular diseases is increasing, therefore the urgency of solving this problem in Uzbekistan can be defined as an extreme one, requiring the concentration of efforts of specialists in different fields.

Collecting patient opinions is fundamental to personalizing care, promoting shared decision-making, and fostering patient engagement in care. The systematic, large-scale collection of these opinions also enables to evaluate the impact of care on health and the extent to which care is responding to people’s needs. The SCAPE surveys, introduced in Switzerland in 2018, aim to systematically collect and analyze the experiences of cancer patients, providing essential data for improving the quality of oncology care. An important feature of the SACPE surveys is the involvement of patient partners in research right from the start. Throughout the partnership, their experiential knowledge was fully recognized, enriching the quality, relevance, and value of SCAPE. The quality of their engagement will be assessed to appreciate their work and valuable involvement, and to draw lessons to guide future projects aimed at improving the quality of care, in partnership with patients. Recueillir l'opinion des patientes et patients est fondamental pour personnaliser les soins, promouvoir la prise de décisions partagée et favoriser l'engagement des patientes et patients dans les soins. La collecte systématique et à large échelle de ces opinions permet également d’évaluer l’impact des soins sur la santé et à quel point les soins répondent aux besoins des personnes. Les enquêtes SCAPE, introduites en Suisse en 2018, ont pour objectif de recueillir et analyser systématiquement les expériences de soins des patientes et patients atteints de cancer, fournissant des données essentielles pour améliorer la qualité des soins en oncologie. Une spécificité importante des enquêtes SCAPE est d’avoir impliqué des patientes et patients partenaires de recherche dès le début du processus de mise en place des enquêtes. Tout au long de ce partenariat, leur savoir expérientiel a été pleinement reconnu, enrichissant ainsi la qualité, la pertinence et la valeur de SCAPE. La qualité de leur engagement sera évaluée afin de valoriser leur travail ainsi que leur implication précieuse et de tirer des enseignements pour guider de futurs projets visant à améliorer la qualité des soins, en partenariat avec des patientes et patients.

After treatments for loss of large amounts of weight, the dissatisfaction with body image and the lack of recognition of the body itself is recurrent. These factors are elements of study due to the difficulties encountered in maintaining the results, in the long term, also in the surgical treatment of obesity. The lack of identity with the new form and dissatisfaction with body image have been pointed out as maintaining factors of poor eating habits and emotional eating that leads to weight recurrence. In this sense, the interest was raised as to whether: Would small mindfulness interventions have the ability to improve body image and favor the recognition of body distortions, in order to prevent weight recurrence in patients undergoing bariatric surgeries? This article aims to investigate the applicability of small mindfulness meditative strategies in the prevention of weight recurrence in bariatric patients. The method presented is a case report through 12 weekly meetings with a female client, 48 years of age, preoperative BMI of 42 Kg/m2 and postoperative BMI of 21.67 Kg/m2. The themes of the meetings were: psychoeducation about bariatric surgery, family approach, evaluation and psychoeducation about cognitive patterns, assessment of body image and body identity through photo composition. The other meetings were distributed among the small practices of mindfulness: breathing, movement, eating with mindfulness and body scanning. The practices demonstrated efficacy in reducing the discrepancy between the internal subjective perception of the body (interoception) and body image (exteroception). There was a positive experience of the body outside the aesthetic requirements, achievement of greater identity with body shape, exit from the perfectionist pattern and evaluation improvement of body shape and size. The small mindfulness-like meditative strategies have been shown to be effective in cognitive behavioral therapy for the prevention of weight recurrence in bariatric patients.

This paper is a literature review that addresses themes related to a sense of connection with a being of a higher nature by cancer patients. Objective: to seek the connection relationship “I-God” in cancer patients. Methodology: The searches were performed from publications indexed in the Databases Scielo, Lilacs and Medline, crossing the terms: câncer/espiritualidade/saúde, cancer/spirituality/health and cáncer/espiritualidad /salud. The research was limited to English, Portuguese and Spanish from 2015 to 2020, from September/2019 to June/2020. We found 934 articles, of which seven investigations constituted the final selection for meeting the objective of the study. The analyses for the selection of the articles occurred in three distinct stages. The first phase was carried out by the researchers, based on the subsidies provided by the title and keywords, the second, the second was the reading of the selected abstracts and the third last selection phase, where finally the reading of the papers that provided subsidies to the study occurred. Conclusion: It is observed that in most articles it is reported the belief in God based on faith and spirituality, which shows that it is a favorable support, stimulates people to cure fear and anxiety and is a source of strength for the participants of the researchfound.
