Zusammenfassung Hintergrund Zwanghaftes Sexualverhalten (ZS) ist gekennzeichnet durch intensive, repetitive sexuelle Fantasien, dranghaftes Erleben und Verhaltensweisen, die zu klinisch signifikanten psychischen Beeinträchtigungen führen. Neue Untersuchungen zeigen, dass ZS mit interpersonellen Konflikten assoziiert ist. Die Beziehung von ZS zu sexuellen Grenzverletzungen und Paraphilien bleibt jedoch unklar. Fragestellung Das Ziel der vorliegenden Untersuchung war es, das Wissen über die Rolle von sexuellen Grenzverletzungen und Paraphilien bei Männern mit ZS zu erweitern. Material und Methoden Eine Gruppe von Männern mit ZS (n = 47) wurde mit einer Gruppe gesunder Kontrollprobanden (GK, n = 38) verglichen. Anhand von Fragebogen wurde untersucht, ob sexuelle Grenzverletzungen und Paraphilien als sexuelle Vorlieben vorliegen, und ob sie in der Realität ausgelebt wurden. Ergebnisse Es zeigte sich, dass Männer mit ZS eine stärkere sexuelle Erregung durch sexuelle Grenzverletzungen in Masturbationsfantasien erlebten und in der Vergangenheit häufiger sexuelle Grenzverletzungen wie nichtkonsensuelles Eindringen auslebten als GK (Männer mit ZS 17 % vs. GK 3 %). Des Weiteren waren die paraphilen Interessen bei Männern mit ZS stärker ausgeprägt im Vergleich zu GK. Schlussfolgerung Die Befunde unterstreichen die Wichtigkeit der Arbeit mit Patienten mit ZS und haben weitreichende Implikationen für die therapeutische Arbeit, insbesondere was die therapeutische Grundhaltung gegenüber Patienten mit ZS betrifft.

The DSM-II and DSM-III both allow for multiple diagnoses. The DSM-III acknowledges that a patient may have multiple paraphilias but the true nature and extent of the multiplicity has only been documented recently. In order to study the degree of crossover between the various paraphiliac acts, a study of men who admitted to at least one paraphilia was conducted. A proportional index of multiple deviation was obtained. In addition, the mean number of admitted sexually deviant incidents per paraphilic category was computed as an estimate of the extent of deviant acts committed by this population. The results indicated that paraphiliacs tend to have multiple types of sexual aberrations as well as a high frequency of deviant acts per individual.

There is no accepted definition of the term paraphilia despite its being listed as an essential feature of a class of mental disorders known as the paraphilic disorders. The origin of the term, history of its inclusion as a diagnosis, and logical flaws inherent in the various definitions are discussed in this review. We examine the basis for pathologizing individuals with paraphilias, consider what paraphilias can tell us about how humans develop their sexual interests, and question the usefulness of dividing sexual interests into paraphilias and normophilias. The construct of the paraphilias appears to be poorly conceived and has outlived its usefulness.

The term paraphilia refers to the sexual preferences and conducts that divert from what is generally accepted for a certain society in a given historic and cultural period. It demonstrates the practices that involve the use of non-human objects, mandatory humiliation and sexual suffering or non-consensual involvement of sexual partners.A paraphilic disorder is a paraphilia, which, presently, causes uneasiness and damages not only the patient but also others, as these behaviours exclude or damage the other affecting the patient's social relationships.Paraphilias are only practiced by a small percentage of the world's population. However, the causes are only reported if there is a search for treatment or if there are any legal complications. By which is believed that the prevalence is higher than the number of diagnosed cases.Although there are already a few paraphilia types registered, new forms of practice of this disturbance are emerging, mainly associated to the use of new technologies, as the Internet.The authors propose to produce a bibliographic review concerning the concept of paraphilic disorders and its exhibition forms; identify therapeutic strategies; perform a time frame regarding paraphilias and analyze the influence that the new technologies have in paraphilic disorders.Disclosure of interestThe authors have not supplied their declaration of competing interest.

The considerations made in this article justify the conclusion that a paraphilic assesses the marriage from a selfish perspective, because everything he does is aimed at satisfying his pleasure. He does not consider the other person or the possibility of treating him objectively - in other words, a parafilik treats his partner as a thing to satisfy his selfish need for pleasure. Meanwhile, in the marriage relationship, closeness and realization of the good of the other party are sought as an equal partner. On the other hand, paraphilism, which is a sexual impulse, destroys not only the marriage but the spouse and, to a greater or lesser extent, the paraphilic himself. In a marriage with a paraphilic, there is no stability, no realization of the spouse's good, no realization of the community of life and love, only instinctive satisfaction, which is also unnatural. Nevertheless, regardless of the definition of paraphilia in medical sciences and psychology, when conducting a trial in an ecclesiastical court, it should be verified in the light of the concept of Christian anthropology, proper to canon studies. The paraphilia itself does not nullity it, it all depends on the severity of the disorder in question and its impact on the functioning in marriage.

IntroductionPsychiatry's viewpoint of sexual deviance has waved between the normal and the pathological. “Normal” is not determined by nature but by the values of a specific society.AimsTo review the main landmarks in paraphilias history and the importance of social and cultural dimensions to it.MethodsPubMed database was searched using the keywords perversion, sexual deviance, paraphilia, culture and society.ResultsThroughout Middle Age and Renaissance any sexual act that differed from the natural/divine law was considered a vice. Unnatural vices (masturbation, sodomy, bestiality) were the most severely punished, as they could not result in conception. In 1886, Krafft-Ebing stated perversions were functional diseases of the sexual instinct caused by “hereditary taintedness” in the family pedigree and worsened by excessive masturbation. Proper perversions were sadism, masochism, antipathic sexuality (homosexuality, transvestism, transsexuality) and fetishism. Later, Havelock Ellis and Hirschfeld claimed sexual interest in the population followed a statistical norm, opposed the idea that masturbation led to diseases and demanded the decriminalization of homosexuality. Freud believed the “perverse disposition” to be universal in the childhood giving rise to healthy and pathological adult behaviors. In 1950's, Albert Kinsey surprised America when he proved many supposedly deviant sexual practices were quite common. The first Diagnostic and Statistical Manual (1952) was mainly psychoanalytic. Later, by 1973, homosexuality was removed from classifications. Recently, DSM-5 distinguishes between paraphilias and paraphilic disorders.ConclusionA progress in the paraphilic instincts’ acceptance has occurred. Zusammenfassung. Trotz des öffentlichen Interesses an der Thematik gibt es nur wenige aussagekräftige Studien über psychisch gestörte Sexualstraftäter. Letztere werden in der Regel nach Delikten (Vergewaltigung/Kindesmißbrauch) und nicht nach psychischen Störungen kategorisiert. Untersuchungsgruppen sind dadurch psychologisch inhomogen und die Ergebnisse vielfach nicht vergleichbar. In der vorliegenden Studie wird stattdessen vorgeschlagen, psychodiagnostische Kategorien heranzuziehen und danach zu unterscheiden, ob eine Paraphilie oder eine nicht näher bezeichnete Störung der Impulskontrolle vorliegt. Stichproben von n = 42 Paraphilen, n = 30 impulskontrollgestörten Sexualstraftätern und n = 30 Gewalttätern ohne Sexualdelikt werden hinsichtlich psychologischer Variablen verglichen, die für die Störungsdeskription von Sexualstraftätern als relevant gelten: Impulsivität, soziale Angst, Konfliktvermeidung und dysfunktionale Selbstaufmerksamkeit. Die Ergebnisse zeigen, daß die vorgenommene störungsbezogene Kategorisierung der Stichproben eine bessere Differenzierung hinsichtlich der gemessenen Variablen erlaubt als eine deliktbezogene. Paraphile lassen sich durch höhere soziale Angst (Interaktionsangst), Konfliktvermeidung und dysfunktionale Selbstaufmerksamkeit kennzeichnen, nicht jedoch durch geringere Impulsivität.

Background: The Paraphilia now around the world occupy an important position in medicine. Term “paraphilia” (from the Greek “para,” meaning “beside, aside,” and “philia,” meaning “love”) is currently used in psychiatry to define “anomalous” sexual interests. Objectives: Studying prevalence of paraphilic behaviors and interests in a sample of 200 males and 200 females in the population aged from (18 – 45). Methods: Communication was done with different online groups and sites to conduct this cross sectional study. These aimed to orient them about the aim and procedures of the study and explained its benefits. Admins of these groups were helpful but refused to put the name of the groups in the work. admins collected the answered questionnaire to protect identity and privacy of respondents. This study was approved by ethical committee of the Faculty of Medicine Menoufia University. Results: The overall prevalence of paraphilic behavior experience was found 21% with prevalence significantly higher in men than women. The most common paraphilic experience was frotteurism then voyeurism followed by fetishism then pedophilia. Conclusion: It was concluded that paraphilia is not an uncommon behavior in our community. It exists in both sexes but higher among men.

AbstractFemale-oriented male–male erotica is a genre of popular culture often know as Boys’ Love (BL), yaoi, and danmei. It is one of the largest by-and-for women sexual subcultures and a global phenomenon. With the largest data sets in the field, we ask: Which risqué sexual content do Sinophone (Chinese-speaking) and Anglophone (English-speaking) participants particularly enjoy in BL and does this differ between cultures?, and Are there sub-demographics in Sinophone and in Anglophone culture who enjoy particular forms of risqué sexual content in BL and do these forms relate also to enjoyment of particular storylines and concern with legal issues? The material studied meets the DSM-5 definition of the paraphilic, and little is known about paraphilias in women or in the general population. Using Categorical Principal Component Analysis we explored one 15-response question from our Sinophone (N = 1922) and Anglophone (N = 1715) BL fandom surveys: Which risqué sexual content do you particularly enjoy in BL? We also tested for associations with seven demographic and other BL content-related questions. Notably, the component structure was nearly replicated between the two independent samples, in order of strength: BDSM Specialist, Mechanoid/Animal Sex Specialist, Underage Sex Specialist, and Minority Paraphilia Specialist. In both samples, it was the avid BL fans and/or those who liked explicitly sexual stories, a largely overlapping demographic, who most engage the risqué content, while, for the Sinophone, this included also more non-heterosexual and/or other-gendered people. We conclude that women’s paraphilias have been largely overlooked because they might be expressed more commonly through fantasy than action, that their mass expression has awaited both the means and the market force, and that current conceptualization of, and assumptions about, paraphilias is overly modeled on that of men.

ZusammenfassungSexuelle Probleme sind in der Allgemeinbevölkerung wie auch in der psychiatrisch-psychotherapeutischen Praxis häufig. Die ICD-10 listet die Mehrzahl der sexuellen Störungen im F-Kapitel der psychischen Störungen auf. In der psychiatrisch-psychotherapeutischen Aus-, Fort- und Weiterbindung nehmen sie jedoch keinen großen Raum ein. Viele Ärzte setzen sich nur wenig mit der Sexualanamnese der Patienten auseinander. Der Artikel erläutert an drei Beispielen die Relevanz der Sexualmedizin für Psychiatrie, Psychosomatik und Psychotherapie: Sexuelle Funktionsstörungen als häufigste Problemstellungen, Paraphilien als forensisch relevante Symptomatik mit großen Vorurteilen in der Bevölkerung und Transsexualität, deren Behandlung konsequent interdisziplinäre Ansätze erfordert und lebensverändernde Konsequenzen für die Betroffenen hat. Aus diesen Beispielen wird die Wichtigkeit der vertieften Beschäftigung mit der Thematik und der Auseinandersetzung mit diesem Aspekt der Psychosomatik für den Praktiker ersichtlich. Das gilt jedoch auch für das gesamte Fach mit seinen entsprechenden Curricula.

ZUSAMMENFASSUNG Gegenstand und Ziel Die digitale Transformation verändert die Sexualität und Beziehungsgestaltung vor allem junger Menschen in vielfältiger Weise. Material und Methoden Klinische Beobachtungen aus der Praxis der Paar- und Sexualtherapie werden vor dem Hintergrund aktueller Forschungsergebnisse dargestellt. Ergebnisse Es zeigt sich ein Anstieg sexueller Funktionsstörungen (erektile Dysfunktion, Hemmung oder Ausbleiben des Orgasmus und partnerbezogene Lustlosigkeit) besonders bei jungen Männern. Zudem entstehen zwanghafte oder süchtige Verhaltensweisen um Pornografie und Sexualität sowie sexuelle Abweichungen (Paraphilien) nehmen zu. Schlussfolgerungen Der problematische und frühzeitige Konsum von starken sexuellen Stimuli (Pornografie, ständig wechselnde sexuelle Partner sowie leistungsstarke Sex-Toys) verursacht neue Probleme und Krankheitsbilder, die nicht genügend erforscht sind. Für wissenschaftlich fundierte präventive, diagnostische und therapeutisch interdisziplinäre Maßnahmen ist diese Forschung dringend erforderlich. Klinische Relevanz Zur Förderung sexueller Gesundheit und Zufriedenheit müssen neue sexuelle Störungen bzw. problematische Erlebnis- und Verhaltensweisen im digitalen Kontext frühzeitig fachgerecht erkannt, diagnostiziert und therapiert werden.

AbstractParaphilias are recurrent and intense sexually arousing fantasies, sexual urges, or behaviors generally involving nonhuman objects. These paraphilias cause the suffering or humiliation of the patient or patient's partner, or children, or other nonconsenting persons. However, in many patients symptoms involve more culturally acceptable patterns (eg, repetitive masturbation, Internet pornography); such hypersexual symptoms have been labeled as compulsive, addictive, or impulsive. Growing evidence supports the existence of a discrete syndrome characterized by recurrent and intense sexually arousing fantasies, sexual urges, or behaviors involving patterns that fall outside the definition of paraphilia. There is, however, high comorbidity with paraphilia. While such symptoms have been labeled as sexual compulsion or addiction, these terms are problematic in this context. Modern nosology has neglected this entity, although the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV), category of sexual disorders not otherwise specified includes hypersexual behaviors as an example. We suggest that the DSM-IV category of sexual disorders be modified to include explicitly diagnostic criteria for a disorder characterized by hypersexual symptoms involving patterns that fall outside of the current definition of paraphilia. The disorder might be classified as one of the paraphilias, or as paraphilia-related. In the absence of a comprehensive understanding of the pathogenesis of this disorder, we suggest that it simply be termed hypersexual disorder.

Biological factors are likely predisposing and modulating elements in sexually deviant behavior. The observation that paraphilic behavior tends to cluster in some families is intriguing and potentially raises questions as to whether shared genetic factors may play a role in the transmission of paraphilia. This pilot study introduces five families in which we found presence of paraphilia over generations. We constructed genograms on the basis of a standardized family history. Results document the aggregation of sexual deviations within the sample of families and support a clinical/phenomenological heterogeneity of sexual deviation. The concept of paraphilia in relation to phenotypic expressions and the likelihood of a spectrum of related disorders must be clarified before conclusions can be reached as to family aggregation of paraphilia based on biological factors.

(1) Background: Paraphilic disorders, marked by intense sexual fantasies and behaviors, present formidable challenges. This review addresses concerns fueled by scandals and child abuse. Emphasizing paraphilias’ complexity, it systematically reviews the pharmacotherapy literature, aiming to enhance understanding and guide future research. (2) Methods: A comprehensive search from 1990 to 2023 across major databases identified 28 relevant English-language studies. Inclusion criteria focused on adult pharmacotherapy for paraphilias, and results were evaluated using the Newcastle–Ottawa Scale. (3) Results: Synthesizing data from selected studies, diverse treatments such as SSRIs and antiandrogens were analyzed, revealing variable effectiveness and side effect profiles. Poor quality of the current literature has been reported. (4) Conclusions: Highlighting the pivotal role of the serotonergic system, this review underscores the efficacy of SSRIs and androgen deprivation therapy. GnRH analog-associated side effects and the importance of a combined assessment approach are discussed. Critical insights contribute to understanding and ethical considerations in paraphilic disorders.

DSM-5 distinguishes between paraphilias and paraphilic disorders. Paraphilias are defined as atypical, yet not necessarily disordered, sexual practices. Paraphilic disorders are instead diseases, which include distress, impairment in functioning, or entail risk of harm one’s self or others. Hence, DSM-5 new approach to paraphilias demedicalizes and destigmatizes unusual sexual behaviors, provided they are not distressing or detrimental to self or others. Asphyxiophilia, a dangerous and potentially deadly form of sexual masochism involving sexual arousal by oxygen deprivation, are clearly described as disorders. Although autoerotic asphyxia has been associated with estimated mortality rates ranging from 250 to 1000 deaths per year in the United States, in Italy, knowledge on this condition is very poor. Episodes of death caused by autoerotic asphyxia seem to be underestimated because it often can be confounded with suicide cases, particularly in the Italian context where family members of the victim often try to disguise autoerotic behaviors of the victims. The current paper provides a review on sexual masochism disorder with asphyxiophilia and discusses one specific case as an example to examine those conditions that may or may not influence the likelihood that death from autoerotic asphyxia be erroneously reported as suicide or accidental injury.

Paraphilias are a group of disorders related to sexual activity that society considers strange or abnormal. This causes many people with paraphilia to experience ostracism and hide their condition. Even though social media and news report on the increasing phenomenon of paraphilia, there are still many people who don't know how to deal with this condition. Economic limitations and embarrassment discourage many people from visiting specialists such as psychologists, especially since most of them live in big cities. This makes it difficult for the public to access accurate information and early diagnosis. This factor causes people with possible paraphilia to be less aware of their condition and reluctant to seek treatment. This study uses additional methods such as forward chaining and Bayes' theorem to overcome the uncertainty of diagnosis and increase the level of accuracy. This expert system can assist the community in carrying out an accurate self-diagnosis to detect possible paraphilia. This system is capable of diagnosing 8 types of paraphilia with 31 symptoms.

In der vorliegenden Studie wurde geprüft, ob Alkoholeinfluss bei Sexualdelikten mit der Art der psychopathologischen Störung des Täters zusammenhängt. Bei vorliegender Störung der Impulskontrolle wurde erwartet, dass Alkoholeinfluss abweichendes Verhalten begünstigt und dass Delikte von Impulskontrollgestörten deshalb häufiger unter Alkoholeinfluss begangen werden. Dies sollte nicht für Delikte von paraphilen Sexualstraftätern gelten, da ein theoretisch begründbarer Zusammenhang zwischen Alkoholisierung und paraphiler Betätigung fehlt. Es wurden die Gerichtsurteile von N=72 impulskontrollgestörten und paraphilen Sexualstraftätern daraufhin untersucht, ob Alkohol im Tathergang vorkam. Dies war bei Impulskontrollgestörten dreimal häufiger der Fall als bei Paraphilen (p &lt;.05). Die Ergebnisse legen nahe, die Alkoholisierung des Täters bei Sexualdelikten vor dem Hintergrund psychopathologischer Störungen differenziell zu betrachten. </P><P> Außerdem wurde gezeigt, dass die Variable »Alkoholeinfluss zur Tatzeit« besser diskriminiert, wenn die oben genannte diagnostische Einteilung der Sexualstraftäter-Gesamtgruppe herangezogen wird und nicht die kriminologische zwischen »child molesters« und »rapists«.

Background: Paraphilic interest is when sexual arousal occurs in response to objects that are not part of normal sexual arousal. One of the rare paraphilic interests is emetophilia, in which an individual experiences sexual pleasure by observing people vomiting. Case Report: We report a 29 year old, unmarried woman, who came alone to the psychiatrist due to sexual arousal by observing others vomiting since childhood. The patient has never had sexual intercourse, so she does not know whether this condition affects her sexual activities or not. Meanwhile, she experienced job-related distress due to her illness. Conclusion: Paraphilic interest or anomalous is one paraphilic disorder form which the person experiences less intense and frequent unusual sexual interest without significant distress or impairment in daily life. In this case, further evaluation concerning this patient’s level of distress, including her sexual intercourse behaviour, was still needed. Keywords: paraphilia, emetophilia, paraphilic disorder

ZusammenfassungDer vorliegende Artikel präsentiert empirische Daten sowie drei Fallbeispiele zur medikamentösen Behandlung von Menschen mit Persönlichkeitsstörungen und komorbiden Störungen in der forensischen Nachsorge. In einer Studie wurden Daten von insgesamt 117 Patienten der Forensisch-Therapeutischen Ambulanz in Berlin ausgewertet. Die Stichprobe zeigte mit 60 % eine hohe Prävalenz von Persönlichkeitsstörungen, wobei die dissoziale und die narzisstische Persönlichkeitsstörung bei Weitem den größten Anteil stellten. Gehäufte Komorbiditäten bestanden für Suchterkrankungen und Paraphilien. Die Personen, bei denen die Diagnose einer Persönlichkeitsstörung vorlag, wurden in 60 % der Fälle medikamentös behandelt. In Bezug auf die Dauer der Therapie bis zum Stichtag im Dezember 2020 sowie die Anzahl der Krisen und Weisungsverstöße im Jahr 2020 ließen sich keine signifikanten Unterschiede im Vergleich zu Patienten mit anderen Störungsbildern feststellen. Die drei Fallberichte beschreiben Personen, die Sexualstraftaten begangen haben. Für sie bedeutete die kombinierte psychopharmakologische Behandlung aus Antiandrogenen und Psychopharmaka eine wirksame Unterstützung im Rahmen des spezifischen Gesamtbehandlungsplans in der forensischen Nachsorge. Die erhobenen Daten und Fallberichte werden hinsichtlich ihrer praktischen Relevanz für die Arbeit in forensischen Ambulanzen diskutiert.

Introduction ADHD is a neurodevelopmental disorder displaying inattention, hyperactivity, and impulsivity as core symptoms. It can affect several areas of life including sexual health. Clinicians have often made assumptions concerning the bound of specific ADHD symptoms affecting sexual desire by increasing its frequency and intensity. Yet, there is still a lack of knowledge about the comorbidity between ADHD, hypersexuality, and paraphilias. A recent literature review could show that some individuals who suffer from ADHD report about hypersexual and paraphilic fantasies and behaviors, but as far as we know, no clear empirical data has emerged supporting the idea that hypersexuality and paraphilias are more frequent in individuals with ADHD. Objectives The present investigation aimed to compare several sexuality related aspects between individuals with and without ADHD. Methods Therefore, we designed an extensive online survey based on established questionnaires, such as the Hypersexual Behavior Inventory (HBI). The survey was implemented in a outpatient sample, ADHD specific fora as well as other general online channels. Results In total, N = 238 individuals participated in the survey (n = 160 with ADHD). Thereby, individuals with ADHD reported significantly more often about a wide range of hypersexual fantasies and behaviors in comparison to individuals without ADHD. Furthermore, individuals with ADHD reported significantly more often about paraphilic fantasies and behaviors including fetishistic and sadistic sexual fantasies. No differences were found concerning other paraphilias. Further results regarding other facets of sexuality, such as sexual orientation, are to be presented and discussed. Conclusions The present study contributes to closing the knowledge gap regarding sexuality in individuals with an ADHD. Disclosure No significant relationships.

BACKGROUND: Paraphilia or deviance is sexual stimulation or action that deviates from normal sexual behavior, but is necessary for some people to experience arousal and orgasm. People with paraphilia have unusual sexual interests, including inanimate objects, places, or individuals (children and corpses), and animals. Another paraphilic disorder involving a disapproving individual is characterized by a persistent and intense pattern of atypical sexual arousal manifested by sexual thoughts, fantasies, compulsions, or behavior in which the focus of the arousal pattern involves the other person being unwilling or unable to consent. However, it is not specifically described in any of the other so-called Paraphilic Disorder categories (e.g., arousal patterns involving corpses or animals). CASE REPORT: Here, we report a case of a male patient aged 37 years, unmarried status, his last education graduated from elementary school, the patient complained that he had frequent contact with cows, he had been experiencing it for 2 years, he felt lonely and had no close friends. As a child, he was often scolded and beaten by his father. He is also often teased by his friends. CONCLUSION: Zoophilia is another paraphilic disorder involving disapproving individuals characterized by a persistent and intense pattern of atypical sexual arousal manifested by sexual thoughts, fantasies, drives, or behaviors in which the focus of the arousal pattern involves the animal. This case is rarely reported because sufferers think their actions are not harmful, lack of parental attention and supervision may be one of the causes of this disorder.

Paraphilia is a term used to describe different sexual deviations involving unconventional methods of sexual gratification. Paraphili as indulged in abnormal and orthodox sex play by using objects as parts of the body or fantasies in which complete satisfaction is sought and obtained without sexual intercourse. Perversions are acts that are socially prohibited or unacceptable, or biologically undesirable. In sexual paraphilia, usually, person psychopaths suffer from mental aberrations and conflicts, socio-sexual maladjustments, psychosexual imbalance, and abnormalities. Paraphilia-induced suicide is a type of anomic suicide, and anomic suicide is due to a lack of social regulation that occurs during high levels of stress and frustration. Paraphilia may induce suicide even though paraphilia is not punishable. It is just due to fear of social defamation and social taboo. Case presentation: A 59-year-old male committed suicide on the railway track, and thenthe body was sent to the mortuary room for an autopsy examination. History was taken from the police and relatives. He was a widower for the last 15 years. Conclusions: Paraphilia is the form of the insertion of foreign bodies in the rectum, and when a person cannot remove foreign bodies himself, attending a hospital is very humiliating. To prevent the social integrity or selfesteem, the person committed suicide. Where this type of paraphilia is not listed as a crime in India, for sexual gratification, a person may use it by any means, which may be natural or unnatural form. Many unnatural forms of paraphilia are used by many people, which is not punishable. Sexual gratification is done by most of normal people and a few abnormal psychiatric people by any sexual perversion. All paraphilia that is not punishable by the law should not be considered a social taboo.

Background and Objective Paraphilia is one of the disorders without precise statistics in Iran. One of the most important sexual injuries is the lack of abilities and skills required in the sexual management of children and adolescents. Paraphilic behavior, especially child sexual abuse, produces dire consequences in children and adolescents. Materials & Method The research was done by searching in PubMed, CINAHL, Web of Science, Google Scholar, ScienceDirect, Scopus, Magiran, and SID using the keywords of “sexual deviance,” “consequence,” “victim,” “sexual education,” “child,” “adolescent,” “COVID-19,” and “reproductive and sexual health.” Results The consequences of deviant behaviors of paraphilia in child and adolescent victims include various sexually transmitted diseases, unwanted pregnancy, obstetric complications, reluctance to marry and have children, posttraumatic stress syndrome, and sexual dysfunction. Preventive solutions include school-based sex education with the family-school-peer triangle, teaching parenting skills to parents, using family-centered sex education models, promoting sexual restraint in adolescents, monitoring the impact of the media on child sex education, paying attention to the early diagnosis of disorders, such as depression or anxiety in children and parents, avoiding destructive parenting styles, such as controlling or banning extreme freedom and teaching life skills to children and adolescents. Conclusion Considering the dreadful consequences of paraphilic behaviors in children and adolescent victims, there is a need for sexual education by taking preventive measures at the level of macro-policies in families, kindergartens, and schools through relevant experts, training child sex educators.

Most people recognize that incarceration alone will not solve sexual violence. Indeed, treating the offenders is critical in an approach to preventing sexual violence and reducing victimization. In most cases, a diagnosis of paraphilia is associated with sexual violence. This review is intended to present and summarize the first recently published international guidelines about pharmacological treatment of paraphilias. Pharmacological interventions should be part of a more comprehensive treatment plan including psychotherapy and, in most cases, behavior therapy. Antiandrogens, and mostly GnRH analogues, significantly reduce the intensity and frequency of deviant sexual arousal and behavior. GnRH analogue treatment constitutes the most promising treatment for sex offenders at high risk of sexual violence, such as pedophiles or serial rapists, however informed consent is necessary. SSRIs remain an interesting option in adolescents, in patients with depressive or OCD disorders, or in mild paraphilias such as exhibitionism.Disclosure of interestThe author has not supplied his declaration of competing interest.

IntroductionLa prise en charge des patients paraphiles peut nécessiter la mise en place de traitement pharmacologique : antidépresseurs ou traitements anti-androgènes. Le but de notre étude est de mieux connaître cette population d’agresseurs sexuels et d’évaluer l’efficacité et la tolérance des traitements pharmacologiques dans cette indication.MéthodeCette étude s’appuie sur l’analyse rétrospective des dossiers médicaux. Tous les patients paraphiles ayant commis une agression sexuelle en France sont potentiellement incluables.RésultatsVingt-trois patients ont été inclus avec un âge moyen de 44 (± S.D. = 12) ans. Les paraphilies retrouvées sont les suivantes : pédophilie 61 % (n = 14), exhibitionnisme 35 % (n = 8) et un violeur en série. Les comportements sexuels déviants sont décrits comme exclusifs pour 39 % (n = 9) de l’échantillon. Dix-sept pour cent (n = 4) des patients rapportent une hypersexualité associée et 39 % (n = 9) rapportent un abus sexuel durant leur enfance. Cinq patients (22 %) recevaient un traitement antidépresseur, six patients (26 %) recevaient quotidiennement de l’acétate de cyprotérone et 12 patients (52 %) une injection d’analogues de la GnRH tous les trois mois. Deux patients ont récidivé après la mise en route du traitement pharmacologique. Un patient exhibitionniste a récidivé durant le traitement par acétate de cyprotérone et un patient pédophile a récidivé durant une interruption du traitement par analogue de la GnRH due à un désir de paternité. Tous les patients décrivent une diminution de leur activité sexuelle déviante durant le traitement pharmacologique (antidépresseurs et antiandrogènes). Quatre patients (17 %) présentaient une ostéoporose, toutefois, la densité osseuse n’a pas été mesurée chez 14 patients (61 %).ConclusionLe traitement pharmacologique semble diminuer l’intensité des comportements et des fantasmes sexuels déviants parmi les patients paraphiles agresseurs sexuels. Notre étude permettra aux cliniciens d’avoir accès à des informations complémentaires à propos de l’efficacité et de la tolérance des traitements pharmacologiques dans cette indication.

Objective: According to Kernberg’s psychodynamic model of personality organization (2012), identity integration, defense mechanisms and reality testing allow the classification of personality functioning in three main organizations lying on a continuum of severity. A specific relation is hypothesized to exist between the continuum of personality organization and the spectrum of sexual pathology, ranging from some degree of sexual inhibition in the presence of the capacity for a stable love relationship (neurotic personality organization), to inordinate and chaotic sexual life combined with paraphilic features (borderline personality organization). The aim of the present study is to assess the relationship between features of BPO and the clinical relevance of paraphilic interests in a sample of women from the community. Design and Method: We sampled 224 women voluntarily recruited through adds posted on different websites. Volunteers were invited to participate in a research project on the “relationship between personality characteristics and sexual life”. After providing consent, all participants were invited to fill in a set of questionnaires including: - the Inventory of Personality Organization (Lenzenweger et al., 2001), to assess the main domains of personality structure according to Kernberg’s model; - a modified version of the Paraphilia Scale (Dawson et al., 2014). Results: We found that the presence of three markers of borderline personality organization (Instability of self/others, Instability of behaviour and Psychosis) is associated, in a linear fashion, to the presence and clinical relevance of paraphilic interests. Conclusions: Results support the importance of an accurate assessment of sexual life in order to obtain essential clinical information on personality functioning in clinical settings.

Introduction: This is a single‐patient case report of a (now) 75-year old married male with long-standing Peyronie’s disease (PD). The patient’s main symptoms were painful erections as well as secondary anxiety and depression. Resolution of these symptoms was achieved with transdermal estradiol treatment. Objectives: To describe a patient with PD, whose persistently painful erections and psychologic distress were eliminated by a novel treatment involving transdermal estradiol. This report also aims to present a novel treatment for patients with paraphilias. Methods: An interview with the patient, chart review, and a literature search were conducted. Informed consent was obtained from the patient and this report was approved by the Research Ethics Committee at the Royal Hospital in Ottawa. Results: Treatment with transdermal estradiol decreased the patient’s sex drive and virtually eliminated his erections with no undesirable side effects. The patient and his wife still enjoy sexual relations without the need for penile erection. Conclusions: This case report presents a novel treatment for PD. It also introduces a novel method to treat men who wish to decrease their sex drive (e.g. men with paraphilic disorders). Replication of this treatment intervention in men with PD and new studies of its use in men with paraphilic disorders are warranted.

Since a patient had recounted to the A (he means a bien agé gentleman) that when he was younger., had noticed that a flic in Paris, who was imposing to him a fine for speeding, tried to hide an odd and unjustifiable penis erection beneath his trousers, the A has been brooding that that phenomenon was reputable a paraphilia that could reveal even other psycological and physiological conditions in man.(many novelists have written that when Nazis used to shoot dissidents and homosexulas in the extermination camps, showed awkwardly erections underneath their uniforms). Manifold are the paraphilias, from the simplest masturbation to the voyeurism, from the frottage to the toucherism, and the A discovered that these mental statuses could imply some hidden manifestations and/or benefits to health and skin beauty.

JustificatifLa prise en charge thérapeutique des patients paraphiles auteurs de violence sexuelle est un enjeu médical et sociétal majeur car les conséquences à long terme pour les victimes de violence sexuelle sont importantes.MéthodologieL’étude EPIPARA (étude épidémiologique descriptive de sujets atteints de paraphilie ayant commis un délit sexuel) a été menée de décembre 2012 à septembre 2014 dans 17 centres : Bordeaux, Dieppe, Fort-de-France (deux centres), la Guyane, Lille, Mulhouse, Nancy, Marseille, Paris (3 centres : un CMP, l’hôpital Paul-Guiraud et l’hôpital Cochin), Perpignan, Montpellier, Rouen, Rennes, Strasbourg. Il s’agit d’une multicentrique rétrospective observationnelle (financement d’un programme hospitalier de recherche clinique national en 2010) et a été réalisée à partir des dossiers de sujets paraphiles ayant commis un délit sexuel. L’objectif de cette étude était de faire un état des lieux descriptif de cette population (recherche de comorbidité, niveau socio-économique, histoire de vie notamment traumatique…). Ainsi, nous avons notamment recueilli les histoires traumatiques de ces sujets.RésultatsCent soixante-seize dossiers ont été analysés (89 % des sujets sont pédophiles). Trente-sept pour cent ont été agressés dans l’enfance dont un tiers par des hommes et 8 % par une femme (dont dans la moitié des cas par leur mère). Cinq pour cent des sujets pédophiles disent avoir été agressés par leur mère.ConclusionNous observons donc que plus d’un tiers de cette population a une histoire traumatique, le plus souvent non prise en charge. Une meilleure connaissance de cette population permettra de mieux prendre en charge les auteurs de violences sexuelles.

Purpose Adverse childhood experiences (ACEs) are a broad conceptual framework in the social sciences that have only recently been studied within criminology. The purpose of this paper is to utilize this framework by applying it to one of the most potentially dangerous forensic populations. Design/methodology/approach Archival data from 225 federal sex offenders was used to perform descriptive, correlational, and negative binomial regression models. Findings There was substantial evidence of ACEs including father abandonment/neglect (36 percent), physical abuse (nearly 28 percent), verbal/emotional abuse (more than 24 percent), and sexual abuse (approximately 27 percent). The mean age of sexual victimization was 7.6 years with the youngest age of victimization occurring at the age of 3. Offenders averaged nearly five paraphilias, the most common were pedophilia (57 percent), pornography addiction (43 percent), paraphilia not otherwise specified (35 percent), exhibitionism (26 percent), and voyeurism (21 percent). The offenders averaged 4.7 paraphilias and the range was substantial (0 to 19). Negative binomial regression models indicated that sexual sadism was positively and pornography addiction was negatively associated with serious criminal violence. Offenders with early age of arrest onset and more total arrest charges were more likely to perpetrate kidnaping, rape, and murder. Originality/value ACEs are common in the life history of federal sex offenders, but have differential associations with the most serious forms of crime.

Frotteurism / frotteuristic disorder seems to be a quite rare paraphylic disorder. This comes from a relatively low prevalence (variation in the literature on this matter) and especially by the lack of data on it in the literature. Interestingly, DSM-III did not include Frotteurism in the 8 paraphiles listed with criteria for a diagnostis. Frotteurism first appeared in DSM-III-R. The frotteuristic disorder is included in DSM 5. The use and meaning of the word frotteurism in sexual terms comes from a French psychiatrist Valentin Magnan in 1890. He described men doing something that he called rubbing - rubbing their penis by women's back without them noticing. The word comes from "frotter" a french word, that means rubbing or putting pressure on someone, and has no sexual connotation. Lussier P., et al. (2008). Keywords: frotteurism, paraphilia, sexology.
