Nonunion fractures and aseptic bone necrosis are two pathological conditions having some impairment of the cellular part of the repair: a reduction of MSC and of the osteoblastic activation. Both are good candidates for cell-based therapies using stem cells. We made a review of the published human trials. Only autologous bone marrow aspirate implantation was until now used. In Nonunion, a direct injection—15 to 150 ml—was made in 4 case series studies. In another, the bone marrow aspirate was concentrated before injection. The results were good. In bone necrosis, only one level 1 study was published. The results at 24 months were positive in terms of reduction of the necrosis and appearance of collapse. In 3 case series studies, a treatment with concentrated bone marrow aspirates was deemed useful with good results in 76 to 96%. These results are interesting but need confirmation by controlled studies.

Antecedents. Hi ha pocs estudis sobre la regulació emocional en membres dels cossos de seguretat i emergències. Així i tot, hi ha estudis científics que indiquen els beneficis i riscos de l’ús de determinades estratègies en la regulació emocional. L’objectiu d’aquest estudi és validar i adaptar el Qüestionari de regulació emocional cognitiva (Cognitive Emotion Regulation Questionnaire, CERQ). Mètode. Hi van participar 239 aspirants a bomber (92,89 % homes i 7,11 % dones), d’entre 23 i 43 anys. Resultats. Els resultats van mostrar una estructura de 9 factors i 2 suprafactors, amb una reducció a 32 ítems. El nou model explica el 63 % de la variabilitat de les dades. L’escala té bona consistència interna, tot i els canvis de dimensionalitat. Pel que fa a la fiabilitat dels factors, té estabilitat temporal, així com entre els 2 suprafactors, amb correlacions altes entre les administracions. Conclusions. Els resultats obtinguts són similars a la versió original i a la validació en espanyol, i mantenen l’estructura factorial. Una diferència observada és el comportament dels suprafactors en els aspirants a bomber, la qual cosa ens porta a inferir que per superar el curs de formació bàsica és necessari combinar ambdues estratègies (adaptatives i menys adaptatives). [Background. There are few studies on emotional regulation in members of safety and emergency services. However, those that are to be found in the scientific literature indicate the benefits and risks that the use of certain strategies exerts on emotional regulation. Here, a preliminary study for the validation and adaptation of the Cognitive Emotion Regulation Questionnaire is presented. Methods. The study sample comprised 239 aspiring firefighters (92.89 % male and 7.11 % female), aged 23-43 years. Results. The results revealed a structure of nine factors and two suprafactors, which reduced the number of items to 32. The new model explains 63 % of the variability of the data. The scale used has good internal consistency despite the changes in dimensionality. As for the reliability of the factors, analysis indicates temporal stability, and also temporal stability between the two suprafactors, showing high correlations between administrations. Conclusions. The results obtained are similar to those recorded for the original version and for the validation in Spanish, maintaining the original factor structure. One difference observed is the behaviour of the suprafactors in aspiring firefighters, suggesting that in order to pass the basic training course, candidates need to combine both strategies (adaptive and less adaptive).] [Antecedentes: El objetivo principal es realizar un estudio preliminar para la validación y adaptación del Cognitive Emotion Regulation Questionnaire a partir del análisis de su estructura factorial, fiabilidad y consistencia interna en una muestra de aspirantes a bomberos. Método: Participaron 239 aspirantes a bomberos (92,89 % hombres y 7,11 % mujeres), entre los 23 y 43 años (M = 32,61 años; SD = 4,829). Resultados: Los resultados mostraron una estructura de nueve factores y dos suprafactores, reduciendo a 32 ítems en vez de 36. El nuevo modelo consigue explicar el 63 % de la variabilidad de los datos. La escala utilizada cuenta con una buena consistencia interna a pesar de los cambios de dimensionalidad. En cuanto a la fiabilidad de los factores del cuestionario tiene estabilidad temporal, analizando las 125 respuestas obtenidas, así como entre los dos suprafactores mostrando correlaciones altas entre las administraciones. Conclusiones: Los resultados obtenidos son parecidos a la versión original y a la validación en español, manteniendo su estructura factorial. Una diferencia que se observa es el comportamiento de los suprafactores en los aspirantes a bomberos llevándonos a inferir que para superar el curso de formación básica es necesario combinar ambas estrategias.]

Background: Thyroid lump is a common clinical problem in Nepal. The risk of malignancy has to be judged prior to surgery for which fine needle aspiration cytology is widely used. To bring the uniform reporting of thyroid aspirates, the Bethesda system for reporting thyroid cytopathology (TBSRTC) was introduced in 2007. This study was done to evaluate the various cytomorphological patterns of thyroid aspirate and to correlate the Bethesda system with final histopathology report.Materials and Methods: Fine needle aspiration were performed and were evaluated as per TBSRTC criteria. Fine needle aspiration cytology report were compared with biopsy specimen whenever possible. Result: A total of 386 cases were studied. The sensitivity, specificity, positive predictive value, negative predictive value and overall accuracy of TBSRTC were 75.2%, 98.2%, 90.0%, 94.7% and 94.03% respectively. Conclusion: Use of TBSRTC in reporting thyroid cytopathology has a greater accuracy; however, benign cases need regular follow up as there are false negative cases.

Historically Black Colleges and Universities (HBCUs) often come under criticism for being havens of conservatism (Harper and Gasman 2008). This conservatism can be found intertwined in some HBCUs’ presidential hiring processes. Focusing on the lack of gender parity in the HBCU presidency, through a Black Feminist Theory lens, I argue that HBCUs using these practices for the selection of Black women presidents create a conflict of self for aspirants who do not authentically subscribe to or perform conservatism. The philosophical ideas of authenticity, self‐esteem, and self‐respect are explored to explain how these expectations create barriers to aspirants achieving their goals and their authentic selves. Subjecting Black women leaders to these practices oppresses aspirants’ need for authenticity and leads to the replication of these conservative ideologies. I conclude that these barriers, in turn, narrow the HBCU presidential pipeline and perpetuate a lack of gender parity in HBCU leadership.

Purpose This study analyzes (1) the value of tyrosinase reverse-transcriptase polymerase chain reaction (RT-PCR) of aspirates obtained by ultrasound-guided fine-needle aspiration cytology (US-FNAC) of sentinel nodes (SNs) in patients with melanoma before sentinel lymph node biopsy (SLNB) and (2) the value of RT-PCR of blood samples of all SLNB patients. Patients and Methods Between 2001 and 2003, 127 patients with melanoma (median Breslow depth, 2.1 mm) underwent SLNB. FNAC was performed in all SNs of all patients pre- and post-SLNB. The aspirates were partly shock-frozen for RT-PCR and were partly used for standard cytology. Peripheral blood was collected at the time of SLNB and at every outpatient visit thereafter. Results Thirty-four (23%) of 120 SNs were positive for melanoma. SN involvement was predicted by US-FNAC with a sensitivity of 82% and a specificity of 72%. Additional tyrosinase RT-PCR revealed the same sensitivity of 82% and a specificity of 72%. At a median follow-up time of 40 months from first blood sample, peripheral-blood RT-PCR was a significant independent predictor of disease-free survival (DFS) and overall survival (OS; P < .001). Conclusion US-FNAC is highly accurate and eliminates the need for SLNB in 16% of all SLNB patients. RT-PCR of the aspirate or excised SN does not improve sensitivity or specificity. RT-PCR of blood samples predicts DFS and OS.

Abstract: The Union Public Service Commission (UPSC) conducts one of the most challenging and competitive exams in India. Preparing for the UPSC exam requires immense hard work, dedication, and perseverance, which often puts a significant amount of stress on the aspirants' mental health. In this study, we conducted an online survey to assess the mental health status of UPSC aspirants. The survey was conducted on 203 participants, who were either preparing for the UPSC exam or had attempted it multiple times. The survey included questions on their attempts, coaching institutes, physical and mental health, emotional problems, sleep hours, and preparation status. The results indicate that while the majority of the participants rated their physical health as good, their mental health status was somewhat poor or poor. A significant number of participants reported facing emotional problems, feeling low or down, and sleep disturbances. The study highlights the need for mental health support for UPSC aspirants, including counselling services and awareness programs.

There is an increasing need for musical aspirants to have access to cheaper musical instruments. This study explores the opportunities to utilize image recognition algorithms via OpenCV to port this technology into readily available modern devices, which will enable inexpensive yet authentic methods of playing a piano. Through OpenCV and Pygame libraries, one can set up a rigid camera that will trace the player’s fingers. The fingers if they cross or hover over a specific coordinate of a key, the piano note (.wav file) will be played by Pygame’s mixer module. This simple yet inexpensive option might help first-time musical aspirants experience music in an affordable and accessible way. Furthermore, this article explores the future scope of accommodating other musical instruments.

The attitude of higher education aspirants of the II master's level to the use of innovative approaches and health technologies in physical education of higher education institutes is considered in the article. The system of higher education needs constant improvement, which is specified to the increase in requirements for the future specialists’ vocational training, in particular masters of education in the field of physical education and sports. At this time, the introduction of innovative approaches to the training process of higher education aspirants in the field of physical education and sports is relevant and necessary. It will allow future specialists to meet the needs of modern society, to be confident, more flexible in the system of training of higher education aspirants of a pedagogical profile, to have a sufficient level of adaptation to the fast-changing requirements of the world labor market, to be mobile to changing directions of the pedagogical activity, and to have the opportunity to choose new modern professions that arise with the advent of new sports. The analysis of the results of the research shows that respondents consider the decisive motive for higher education to have the possibility of employment in higher education institutions; young professionals note the need to reform the existing system of physical education in higher education institutions, offering for this the widespread use of innovative technologies and the exchange of international experience; point out the desirability of complementing the process of practical training of masters with combined and dance innovations and modern unconventional motor activity such as mixed martial arts (MMA), TRX and CrossFit; note the preparedness to introduce innovative technologies in their future professional activity. The main barriers to the introduction of innovative approaches and health technologies in the process of physical education of higher education institutions, according to aspirants, are excess of document circulation, reporting, and insufficient funding. The analysis of the questionnaire shows the need to modernize the modern system of physical education in higher education institutions through the use of innovative approaches and health technologies.

Abstract Background.—The current recommendation for the management of juvenile hemangiomas (JH) is to delay treatment in the hope of spontaneous regression. However, accurate diagnosis is necessary before considering conservative management. Traditionally, the diagnosis of JH has required excisional biopsy. The cytology literature on this relatively rare neoplasm is sparse. Objective.—To present our experience with fine-needle aspiration in the diagnosis and management of JH. Design.—Three cases with a cytologic diagnosis consistent with JH of the parotid gland and cheek were identified from our cytopathology files. Aspirate smears, immunohistochemical studies, computed tomographic scan findings, and clinical follow-up were reviewed. Results.—Patients were female infants ranging in age from 3 to 9 months and presented with an oval firm mass (size range, 2.0–5.0 cm) involving the parotid gland (2 cases) and cheek (1 case). Computed tomographic scan with contrast demonstrated homogeneous enhancement. Aspirate smears revealed spindle-shaped cells in sheets and clusters in a background of blood. The parotid gland aspirates and cell block preparations revealed ductal structures entrapped in sheets of spindle-shaped cells. Immunohistochemical studies revealed prominent vascular spaces lined by CD34 and factor VIII–positive flattened endothelial cells. The diagnosis of JH was rendered on the basis of the cytologic findings in conjunction with the radiologic and clinical findings. On clinical follow-up (8–24 months), none of the patients has shown any progression of the lesion. Conclusions.—Fine-needle aspiration, in conjunction with imaging studies, is a useful tool in the diagnosis and management of JH. It eliminates the need for surgical excision for diagnostic purposes and allows for clinical follow-up of patients with JH.

AbstractIntroduction:Peritonsillar abscess (quinsy) is one of the most common ENT emergencies. A 2002 UK audit of quinsy management revealed that an average ENT department treated 29 cases annually; the most common treatment was needle aspiration with intravenous antibiotics, and culture of the aspirate was often performed routinely. The aims of our study were to evaluate the value of routine culture of quinsy aspirates, and to establish whether the information thus gained was clinically useful.Methods:We examined the notes of patients admitted with quinsy to two hospitals in south-west England, from January 1998 to January 2004 in one hospital and from January 1995 to January 2005 in the other. A total of 577 cases was found. Aspirated pus had been sent for culture in 119 (21 per cent). These cases were examined in more detail.Results:Of the 119 patients, 78.2 per cent (93/119) were treated with either a cephalosporin or penicillin, plus metronidazole. Streptococcal species were cultured in 43.7 per cent (52/119) and anaerobes in 23.5 per cent (28/119; of these cultures, 5.9 per cent (7/119) were pure anaerobes only). All the anaerobes were sensitive to metronidazole. One of the 119 cultures, growing aerobic bacteria, was resistant to penicillin; however, this patient improved clinically on a combination of penicillin and metronidazole. No patients had their treatment changed because of culture results.Conclusions:There appears to be no need to routinely culture quinsy aspirates, based upon our findings (of 16 hospital years) and previous studies (which found no recorded episodes of treatment change as a result of culture sensitivities). The combination of penicillin or a cephalosporin, plus metronidazole appeared to be theoretically effective in 99.2 per cent (118/119) of our specimens; this finding is supported by other studies. However, the rare but potentially life-threatening complications of quinsy must be recognised.

This paper examined the link between political party financing and the ineffectiveness of the NTYTRA in Nigeria and contends that the poor regulation of party financing, which is one of the features of the post-colonial state of Nigeria, not only led to certain alterations in the age reduction bill but also undermined the purpose of the final act. While our analysis is based on the framework provided by the post-colonial state theory, a mixed method of data collection was adopted for the study. The paper argued that the continuous overbearing influence of party financiers on electoral outcomes is a possible cause of alienation of young political aspirants from electoral positions. We therefore recommend the need for effective regulation of relevant laws on party financing and a review of the identified provisions in the NTYTRA that undermine the fair participation of young political aspirants in electoral contests.

<p><em>Political interviews are one of the ways by which political office-seekers in Nigeria sell themselves to the electorates. Extant studies have examined the discourse of political interviews from conversational, philosophical, rhetorical, stylistic and pragmatic perspectives with insufficient attention paid to grammatical forms and communicative intentions of the interviews granted by the two presidential aspirants in 2015 Nigerian General election. This study therefore fills this scholarly gap with the aim of unmasking their grammatical forms and communicative styles, intention and credibility. The paper adopts Halliday’s Systemic Functional Grammar specifically interpersonal function coupled with Searle’s model of Speech Acts Theory as theoretical framework. A total of six interviews granted by the two presidential aspirants in media serve as the source of our data. It is discovered that, in most cases, politicians’ communicative intention is to “pull down” their political opponents. While declaratives and interrogatives are simple, direct and straightforward, the intention is to condemn, lambast and castigate their opponents. This communicative style does not allow the general populace to decipher the political manifestoes of the political aspirants and the party they represent. The paper recommends that before Nigeria can boast of any sustainable growth and development, there is the need for her political office-seekers to adopt effective communication strategies and styles to unveil their intention and manifestoes so that electorates can evaluate their performance after their tenure of office.</em></p>

This study explores burnout among nursing students in Bangalore, India, focusing on Exhaustion and Disengagement scores. A cross-sectional design was applied using the Oldenburg Burnout Inventory modified for nursing students, collecting data using a survey that was conducted between October and December 2023. The sample consisted of 237 female nursing students from the Bachelor of Science in Nursing program at Bangalore College of Nursing, South India. The study integrated the t-distributed Stochastic Neighbor Embedding (t-SNE) procedure for data simplification into three t-SNE components, used in a hierarchical clustering analysis, which identified distinct student profiles: “High-Intensity Study Group” and “Altruistic Aspirants”. While burnout scores were generally high, students with high study hours (“High-Intensity Study Group”) reported greater Exhaustion, with a mean score of 26.78 (SD = 5.26), compared to those in the “Altruistic Aspirants” group, who reported a mean score of 25.00 (SD = 4.48), demonstrating significant differences (p-value = 0.005). Conversely, those motivated by altruism (“Altruistic Aspirants”) showed higher Disengagement, with a mean score of 19.78 (SD = 5.08), in contrast to “High-Intensity Study Group”, which reported a lower mean of 17.84 (SD = 4.74) (p-value = 0.002). This segmentation suggests that burnout manifests differently depending on the students’ academic load and intrinsic motivations. This study underscores the need for targeted interventions that address specific factors characterizing the clusters and provide information for designing future research and interventions. This study was not registered.

Abstract Objectives To retrospectively assess the utility of flow cytometry on bone marrow aspirates with respect to the initial diagnosis of myelodysplastic and myeloproliferative neoplasms. Methods Flow cytometry results at Brooke Army Medical Center from January 1, 2016, to January 1, 2019, were reviewed on all bone marrow biopsies performed for the initial diagnosis of myelodysplastic (MDS) or myeloproliferative neoplasms (MPN). Results were categorized as normal, abnormal myeloid population, or abnormal lymphoid population. Results A total of 197 cases were identified (134 for MDS and 76 for MPN). Of the biopsies for MDS, 27% showed an abnormal myeloid population, and 1% showed an abnormal lymphoid population. Three cases were diagnosed as acute myeloid leukemia. Of the biopsies for MPN, 8% showed an abnormal myeloid population, and 1% showed an abnormal lymphoid population. Cases with incidental abnormal lymphoid populations were small (<5% of events). Conclusion In all cases except those diagnosed as acute myeloid leukemia (1.4% of cases), flow cytometry findings did not affect the diagnosis. Based on these findings and the diagnostic criteria in the updated WHO 2016 revision, flow cytometry does not need to be routinely performed on the bone marrow specimens for the diagnosis of MDS or MPN. However, identification of possible cases of acute myeloid leukemia is critical since flow cytometry is required in those cases. Screening of a bone marrow aspirate slide by a pathologist would allow for canceling of unnecessary flow cytometry in these cases and allow for better test utilization.

This paper seeks to know the awareness level of Indian graduate aspirants towards Entrepreneurship and Intrapreneurship concepts. Under descriptive research, survey method was conducted at a selective institution in Andhra pradesh. A sample of 129 out of 573 was drawn using proportionate stratified random sampling technique, out of which 113 had filled the forms and the leftover 16 were absent at the time of survey. The data was collected using schedule with 13 questions and the data analysis was done using descriptive statistics measure and percentages. From the analysis, it was found that about 53.98% of graduates heard about Entrepreneurship and only 18.58% heard about Intrapreneurship. Very few graduates have designated that certain most significant motives, characteristics and skills required for becoming successful Entrepreneurs or Intrapreneurs. It was also identified that only 23.89% and 7.9% of the total 113 graduate aspirants were willing to become Entrepreneurs and Intrapreneurs correspondingly. The major limitation of the study was that the survey meant for the chosen institution and had excluded the discussion of financial sources for Entrepreneurs or Intrapreneurs, left for further research. Lastly, it was concluded that the graduates were not clearly aware of the concepts, there arises a need to implant the concepts for their growth and our nation aswell.

Gender differences in managerial aspirations and managerial potential have been advanced as possible explanations for the structuring of organizations along gender lines, with women concentrated in lower level jobs and under-represented in managerial positions. These hypothesized gender differences were examined in a sample of male and female physical education and non-physical education students. Analysis of variance results showed that the effects of gender, faculty, or their interaction on managerial aspirations were not significant. The main effects of aspiration level, faculty, and gender on the set of managerial potential variables were significant. Aspirants scored higher than nonaspirants on self-assurance, decisiveness, and need for dominance. Non-physical education students scored higher on need for dominance than did physical education students. Males were higher in need for autonomy and need for dominance, while females were higher in decisiveness.

AbstractSeventy patients with nasal polyps were studied. Forty-seven of these had pre-operative sinusradiographs and all seventy patients had antral washouts at the time of nasal polypectomy. One hundred and seventeen antral aspirates were sent for culture and microscopy. The naked eyeappearance of aspirates were turbid in 34 patients. Bacteria were cultured from 15 of the specimens in 10 patients.It was concluded that antral washout should be recommended in all patients who have nasal polypectomy and that there is no need for sinus radiographs in those patients who have uncomplicated nasal polyps.

The Competency Framework of the South African Institute of Chartered Accountants (SAICA) details specific competencies, but also places emphasis on the pervasive skills that need to be attained by candidates in order to qualify as South African chartered accountants (CAs (SA)). This article presents the results of a literature review and empirical work that provides conflicting results as to whether the education, the training or a combination of the programmes are responsible for ensuring that aspirant CAs (SA) are equipped with pervasive skills. This study returned significant findings, indicating that SAICA-accredited academics are not aware of their responsibility to ensure that aspirant CAs (SA) have achieved all the competencies as set out in SAICA’s Competency Framework. While this apparent shortcoming may exist, it is the responsibility of the Heads of Academic Departments and SAICA alike to further inform academics that the onus to transfer both competencies (specific and pervasive) rests predominantly with them.

Background: Patients with multiple endocrine neoplasia (type 1) often present with multiple pancreatic endocrine tumours, such as insulinomas. A major difficulty in the surgical treatment of such patients is identifying which tumours are functionally active and therefore need to be resected for a cure. The objective of this study was to develop a rapid insulin assay that could be used intraoperatively for identifying insulinomas from pancreatic aspirates. Methods: By reducing the incubation time and by increasing the sample volume, a rapid insulin assay was developed on the IMx analyser. This was used to measure insulin from tissue aspirates collected from suspected insulinomas under ultrasound guidance. Results: The rapid insulin assay ( y) could be performed in 14 min and showed a good correlation with the standard IMx ( x) insulin assay ( y=0·808 x+0·04; r2=0·986). Using the rapid insulin assay on pancreatic tissue aspirates, insulinomas could be readily distinguished from normal pancreatic tissue or from non-functional adenomas based on a marked increase in insulin content. Conclusion: In summary, a new rapid assay for insulin is described that compares favourably to the standard IMx insulin assay and can potentially be used intraoperatively on pancreatic aspirates for identifying functionally active insulinomas.

Background As untreated visceral leishmaniasis (VL) is fatal, reliable diagnostics are pivotal for accurate treatment allocation. The current diagnostic algorithm for VL in Ethiopia, which is based on the rK39 rapid diagnostic test and microscopy of tissue smears, lacks sensitivity. This probably leads to missed cases and patients not receiving treatment. Methodology We conducted a retrospective study on stored microscopically negative spleen and bone marrow smears from suspected VL patients collected at the Leishmaniasis Research and Treatment Center (LRTC) in Gondar, northern Ethiopia between June 2019 and November 2020. Sociodemographic, clinical and treatment data were collected and samples were tested by real-time PCR targeting kinetoplast DNA. Principle findings Among the 191 eligible samples (135 spleen and 56 bone marrow) with a microscopically negative and valid PCR result, 119 (62.3%) were positive by PCR, although Ct values for some were high (median 33.0). Approximately three quarters of these undiagnosed primary VL (77.3%) and relapse (69.6%) patients did not receive antileishmanial treatment. Of the 56 microscopically negative bone marrow samples, 46 (82.1%) were PCR positive, which is considerably higher compared to the microscopically negative spleen samples, for which 73 out of 135 (54.1%) were PCR positive. The odds of being PCR positive were significantly higher for bone marrow aspirates and higher when white blood cell values were lower and splenomegaly (in cm) was more pronounced. Conclusions This study demonstrates that a lot of suspected VL patients remain undiagnosed and untreated. This indicates the urgent need for better diagnostics for VL in the East-African region. The outcomes of PCR positive should be closely monitored and treatment should be provided if the patient deteriorates. In resource limited settings, implementation of PCR on bone marrow aspirate smears of patients with low WBC values and splenomegaly could lead to considerable improvements in patient management.

This study aims to examine the marketability of homemade food and to understand the need for an entrepreneurial venture start-up in Oman. In addition, it explores barriers to buying or selling homemade food. The research is based on a qualitative study using purposive sampling techniques by an indepth interview with 83 aspirant buyers and sellers of homemade food. The study’s findings confirm that the marketability of homemade food can be achieved by developing a unique application for trading these products, that will help to avoid barriers to buying or selling. Most respondents preferred eating homemade food rather than restaurant or commercially prepared food. These research findings provide entrepreneurs with some insight in terms of new venture start-ups for marketing homemade food in niche markets. This research comprises a specific approach to the marketability of homemade food and suggests a unique mobile app for trading homemade food. The data was gathered in Nizwa, Oman, however it could be broadened to encompass more geographic regions, particularly urban areas. The research results, achieved using qualitative methodologies, contribute significantly to the body of existing literature. It enhances value in analysing probable barriers among aspirant buyers and sellers of homemade food.

Abstract Abstract 4526 Introduction: Prolonged and persistent neutropenia (white blood cell (WBC) ≤1000/μL) after chemotherapy is associated with increased risk for infection in cancer patients and is often an indication for a diagnostic bone marrow aspirate and biopsy. A test that predicts WBC recovery ≥ 1000/μL and that obviates the need for marrow sampling would be of clinical value. The immature reticulocyte fraction (IRF) reflects erythroid production and hence a recovering marrow. Materials and Methods: We identified 17 myeloma patients with prolonged pancytopenia after either myeloablative or non-myeloablative chemotherapy between March 2010 and February 2011, and compared the time of occurrence of IRF doubling (IRF-D) to the findings on bone marrow examination. Results: The time to IRF doubling preceded increase of WBC ≥ 1000/μL in 15 of 17 patients by a mean of 4.5 days (range 0–18 days). In two patients, the IRF-D coincided with WBC ≥ 1000/μL. The IRF doubled 2–4 days before the bone marrow examination in four patients and at a mean of 3.7 days (range 1–13 days) after the marrow examination in the remaining 13. Conclusion: We conclude that the IRF-D is a simple, inexpensive and widely available test that can precede marrow recovery by several days and may therefore obviate the need for a diagnostic bone marrow aspirate and biopsy in patients with prolonged and persistent neutropenia. Our case report describes a patient with a common presenting complaint yet an uncommon infection. Our patient presented with a fluctuant breast mass diagnosed as a breast abscess. An aspirate sample was sent for culture and sensitivities, which revealed the presence of Actinomyces turicensis and the anaerobe Peptoniphilus harei. She was therefore prescribed several weeks of amoxicillin and metronidazole, and made a full recovery. There are only three case reports describing A. turicensis as a causative organism for breast abscess, one of which had also occurred in our department. One case also showed the additional presence of P. harei. Our findings reveal a growing need for increasing clinician awareness of A. turicensis and the importance of aspirate sample culture and sensitivity.

Systematic pursuits are being developed to set forth the framework for the Fifth Generation (5G) wireless standards. This paper emphases on the most extensively deployed technology - Orthogonal Frequency Division Multiplexing (OFDM) that has outpaced other waveform aspirants for Fourth Generation (4G) communication standards. Irrespective of the beneficial features, it does possess a number of significant limitations that mark it as an incompatible candidate for the upcoming 5G standard. This paper highlights on its major drawback i.e high Peak-to-Average Power Ratio (PAPR). Results state that PAPR does cause sudden upsurge to the output signal envelope causing further other damages. There exists a need for more flexible waveforms to replace the conventional OFDM in order to address the unprecedented challenges. The future research directions in the domain are presented.

Mesenchymal stromal cells (MSC) harvested in different tissues from the same donor exhibit different phenotypes. Each phenotype is not only characterized by a certain pattern of cell surface markers, but also different cellular functionalities. Only recently were different harvesting and processing techniques found to contribute to this phenomenon as well. This study was therefore set up to investigate proteomic and functional properties of human bone marrow-derived MSCs (hBM-MSC). These were taken from the same tissue and donor site but harvested either as aspirate or bone chip cultures. Both MSC populations were profiled for MSC markers defined by the International Society for Cellular Therapy (ISCT), MSC markers currently under discussion and markers of particular interest. While classic ISCT MSC markers did not show any significant difference between aspirate and outgrowth hBM-MSCs, our additional characterization panel revealed distinct patterns of differentially expressed markers. Furthermore, hBM-MSCs from aspirate cultures demonstrated a significantly higher osteogenic differentiation potential than outgrowth MSCs, which could be confirmed using a transcriptional approach. Our comparison of MSC phenotypes obtained by different harvesting techniques suggests the need of future standardized harvesting, processing and phenotyping procedures in order to gain better comparability in the MSC field.

Introduction Hemophagocytic lymphohistiocytosis (HLH) is a rare condition resulting from a dysregulated inflammatory response. It can prove difficult to diagnose and portends a poor prognosis. Bone marrow (BM) biopsy is an easily accessible test that is often used to identify the presence of hemophagocytosis and assess for underlying malignancy. Currently there are no evidence-based guidelines on the reporting of hemophagocytosis on BM biopsy and no reports of a correlation between hemophagocytosis with the clinical diagnostic criteria for HLH. We therefore aimed to assess if the amount of hemophagocytosis identified in the BM biopsy correlates with HLH-2004 criteria. Secondary aims were to evaluate inter-observer variability in reporting hemophagocytosis, and to formulate recommendations for screening in BM specimens. Method A retrospective review of bone marrow biopsies from adult patients under investigation for HLH was undertaken independently by 2 hematopathologists who were blinded to the original biopsy report. Relevant clinical and laboratory data was extracted from medical records. The average number of actively hemophagocytic cells in each slide prepared from BM aspirates were quantified into 0, 1, 2-4 and ≥5. On trephine samples, hemophagocytosis was reported as either 'present' or 'absent', with the assistance of the CD68 immunohistochemical stain. Cases with discordance pertaining to the degree of hemophagocytosis were reviewed by both assessors to reach a consensus. Results Sixty-two specimens from 59 patients were available for assessment. An underlying hematological condition was identified in 34 cases (58%). The most common underlying hematological condition was lymphoma, found in 15 cases (25%). There was a significant association between the amount of hemophagocytosis identified on the aspirate samples and the number of HLH-2004 criteria met (p&lt;0.05). In patients where hemophagocytosis was present (n=31), there was a significant correlation between the amount of hemophagocytosis and ferritin levels (p&lt;0.05). Interobserver variability was present in 63% of cases. Based on our review, we make the following recommendations for reporting of hemophagocytosis in the BM samples:&gt; 1. Count only macrophages ingesting intact hemopoietic cells. W2. Quantify the average number of active histiocytes per aspirate slide. W3. Count histiocytes away from particles where the cellular outline is clear. W4. Avoid counting conglomerates of histiocytes where the cellular margins are indistinct W5. On the aspirate specimen, assess for hemophagocytosis on both the trail and squash preparations. W6. Delineating hemophagocytosis on trephine samples is difficult without the use of a CD68 immunohistochemical stain. Interestingly, a study by Ho et al found no association between the BM histologic findings and the probability of hemophagocytosis (Ho et al, American Journal of Clinical Pathology, 2014). This difference highlights the need for standardised reporting of BM specimens. Conclusion Our findings indicate that the amount of hemophagocytosis present on BM samples correlates with the number of HLH-2004 criteria met. We found marked interobserver variability which we anticipate can be rectified with our recommendations on the reporting of hemophagocytosis. Migrainous headaches and transient ischaemic attacks are amongst the commonest acute neurological presentations seen in the UK. Our case highlights a rare complication of a rare disorder presenting as a mimic for these common neurological conditions.A 68 year-old woman presented with gradual onset severe frontotemporal headache with vomiting, mild photophobia and transient aphasia. Previously, she had experienced episodes of sudden onset left arm weakness and numbness spreading to the face, associated with complete aphasia, lasting for ten minutes. Examination was unrevealing.Contrast head MRI showed enhancing leptomeningeal disease. An IgM kappa paraprotein was detected in serum, with a lymphoplasmacytic lymphoma diagnosed on bone marrow aspirate/trephine. CSF was highly cellular containing a population of predominantly CD20+ lymphocytes, in keeping with a B-cell lymphoplasmacytic lymphoma infiltrating the leptomeninges. A diagnosis of Waldenstrom’s macroglobulinaemia (WM) presenting with Bing-Neel syndrome was made. She was started on chemoimmunotherapy with MATRix regimen.Bing-Neel syndrome is a rare complication of WM that results from infiltration of the central nervous system, and rarely peripheral nervous system, by malignant lymphoplasmacytic cells. Neurological manifestations are diverse and protean. The diagnosis should be considered with unexplained neurological symptoms in the presence of macroglobulinaemia.

The authors describe a 43-year-old patient who had a mediastinal mass that became infected after a transbronchial needle aspirate biopsy. A paraspinal, extrapleural window with a saline-lidocaine mixture was created that allowed the placement of a percutaneous drainage catheter into the infected lesion. This procedure resulted in an excellent clinical outcome, and obviated the need for a thoracotomy and more invasive surgical management.

Aim: Osteoarthritis (OA) is a highly prevalent and costly condition, rooted in cartilaginous defects. Despite various causes, the inability for chondrocytes to regenerate prohibits these lesions from self-healing. Debridement commonly provides symptomatic relief but does not target the underlying disease process, necessitating investigation into possible treatments. Intraosseous and intraarticular bone marrow aspirate concentrate (BMAC) injection is a new promising therapy aimed at repairing these cartilage defects. Methods/materials: We retrospectively reviewed patients who underwent BMAC chondroplasty and examined the efficacy in delaying need for further intervention. Results: Only 5 of 23 procedures (21.7%) required postoperative intervention within the 2-year follow-up period. Only one request for total knee arthroplasty was made, but the procedure has not been done. Conclusion: This study demonstrates that BMAC chondroplasty may be an efficacious method to delay need for total knee arthroplasty.

Pediatric chronic myelogenous leukemia is uncommon. We report a pediatric patient with chronic myelogenous leukemia presenting with a normal white blood cell count and no circulating immature myeloid cells. The patient presented with extreme thrombocytosis (platelet count range: 2175–3064 × 109/L) noted incidentally. No splenomegaly was found. Examination of the bone marrow aspirate revealed normal cellularity and normal myeloid: erythroid ratio with marked megakaryocytic hyperplasia. Molecular studies on the bone marrow aspirate detected both the major BCR/ABL1 p210 fusion transcript (9280 copies; p210/ ABL1 ratio: 38.2%) and the minor p190 transcript (below limit of quantitation). The platelet count normalized within 2 weeks after treatment with the second-generation tyrosine kinase inhibitor dasatinib. Follow-up after 3 months revealed a 1.87 log reduction in p210 transcripts compared to diagnosis and no detectable p190 transcripts. This case highlights the need to include BCR/ABL1 fusion testing to accurately diagnose pediatric patients presenting with isolated thrombocytosis.

As part of our research study we are considering domain of information provided by the university to the public i.e. the aspirants applying to the university as well as the students undergoing the curriculum. We are developing an advanced chat-bot which assists the students to gather information about the university. Since the information which can be provided from the official website is not up to the mark, there is a need for developing another medium for providing the necessary information. Whenever a student requires some assistance of any matter regarding university they can gather information from helpdesk, website or billboards. Sometimes the information may not be accurate and insufficient. So, in order to make the required information accessible to the students we are developing this software where students can interact with the software assistant through text and gain the information that is required by them. The software allows the student to interact with it in text basis and provides almost any information regarding University.

This paper argues that the characteristic lack of engineering representation in British senior management is partly a consequence of the prevailing conception of what management is actually about. As compared to certain other capitalist economies, British conditions have favoured such management activities as the search for longterm finance and strategic marketing over product and process improvement. This system of priorities, massively perpetuated in management writings and education, is now embedded in the British definition of what management is. Aspirants to senior positions, which necessarily involve considerable decision-making discretion, need to demonstrate their `trustworthiness' in such terms. Insofar as the managerial credentials of professional engineering rest upon its position of authority within productive labour, they are out of key with the conception of management dominant in Britain. For many years the profession has tried to overcome this by adding `managerial' subjects to engineering education. However, so long as management is conceived of as a distinct field of study in its own right, such a strategy can do no more than place engineers in the position of comparative amateurs competing with full-time `specialists'.

In 2019 a newly identified coronavirus, designated as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has spread rapidly from the epicenter in Wuhan (China) to more than 150 countries around the world, causing the Coronavirus disease 2019 (COVID-19) pandemic. In this study, we describe an extraction-less method based on reverse transcriptase loop-mediated isothermal amplification (RT-LAMP) intended for the rapid qualitative detection of nucleic acid from SARS-CoV-2 in upper respiratory specimens, including oropharyngeal and nasopharyngeal swabs, anterior nasal and mid-turbinate nasal swabs, nasopharyngeal washes/aspirates or nasal aspirates as well as bronchoalveolar lavage (BAL) from individuals suspected of COVID-19 by their healthcare provider. The assay’s performance was evaluated and compared to an RT quantitative PCR-based assay (FDA-approved). With high sensitivity, specificity, and bypassing the need for RNA extraction, the RT-LAMP Rapid Detection assay is a valuable and fast test for an accurate and rapid RNA detection of the SARS-CoV-2 virus and potentially other pathogens. Additionally, the versatility of this test allows its application in virtually every laboratory setting and remote location where access to expensive laboratory equipment is a limiting factor for testing during pandemic crises.

Relevance. Traumatic and ischemic injury of the limbs is accompanied by damage of the skeletal muscles and peripheral nerves of the limbs. The dynamics and consequences of ischemic lesions remain poorly understood and need to be corrected. Objective: using quantitative morphological and sonographic methods, to study the dynamics of skeletal muscle damage of the limb after traumatically induced ischemia with and without the injection of platelet-rich plasma, bone marrow aspirate, and adipose tissue fraction. Materials and Methods. In 3 experiments, rabbits were modeled with 6-hour limb ischemia by applying an elastic tourniquet. After compartment syndrome detection, based on the assessment of subfascial pressure, cell suspensions were injected into the leg muscles. Sonographic and histological examination of the muscles was performed on days 5, 15, and 30. The results of sonography and morphometry were evaluated by statistical methods. Results. The developed model of ischemia consists of 6-hour immobilization of the limb, on тwhich medical elastic tourniquets were imposing. The action of the tourniquets causes high subfascial pressure and necrosis of the superficial muscle groups of the lower third of the thigh and lower leg. According to sonography, the δ-entropy of damaged tissues on day 5 is reduced relative to the intact limb, as in the case of administration of bone marrow aspirate cells. On days 15 and 30, sonography showed no difference between the comparison groups. The dynamics of morphological features of limb tissue damage consist of necrosis of superficial muscle groups, atrophy in the middle layers, and almost intact deep muscle groups. Necrosis was replaced by scar tissue, the density of which increases 11-14 times, and does not differ in the period 5-30 days. The administration of platelet plasma, bone marrow aspirate, and adipose tissue fraction did not change the dynamics of fibrotic changes in ischemic damaged muscles. Muscle atrophy is accompanied by activation of endogenous repair of single muscle fibers, which tended to intensify after injection of bone marrow aspirate. The sciatic nerve of the injured limb was not structurally damaged according to the deep topography, while the nerves of the tibia develop degenerative changes from the 15th day.

We present a rare case of extramedullary plasmacytoma of the palatine tonsil with cervical lymph node involvement treated by surgical resection. A 58-year-old Caucasian male presented with a solitary 3 cm×3 cm jugulodigastric lymph node and was found to have an ipsilateral tonsillar swelling. The involved tonsil and lymph node were surgically resected after two inconclusive fine-needle aspirates, and plasmacytoma was confirmed histologically and by immunocytochemistry. Adjuvant radiotherapy was not indicated as adequate resection was achieved at surgery. We also highlight the challenges of diagnosis when fine-needle aspiration is inconclusive and the need for careful planning before surgery.

6083 Background: Receptor tyrosine kinase (RTK) fusions may be targeted by small molecule inhibitors to treat various advanced tumors, including thyroid cancer. Clinical trials have studied selective inhibitors of ALK, BRAF, NTRK and RET, leading to several FDA-approved therapies. The Afirma Genomic Sequencing Classifier (GSC) classifies cytologically indeterminate thyroid nodules as molecularly benign or suspicious. The Xpression Atlas reports 905 genomic variants and 235 fusion pairs on GSC Suspicious, Suspicious for Malignancy (SFM), and Malignant FNA samples at the time of diagnosis. Here we report the prevalence of these fusion genes in real-world clinical practice. Methods: We analyzed anonymized data from 50,644 consecutive Bethesda III-VI nodule FNA samples submitted to the Veracyte CLIA laboratory for molecular testing using whole transcriptome RNA sequencing (RNA-Seq). Gene pairs are listed alphabetically. Results: 32,080 Bethesda III/IV nodules were classified as GSC Benign and 278 were Parathyroid Classifier positive. No ALK, BRAF, NTRK1/3, or RET fusions were identified among these samples. Among 16,594 Bethesda III/IV GSC Suspicious FNAs, 3% (n = 529) were positive for ALK, BRAF, NTRK1/3 or RET fusions. Among the 1,692 Bethesda V/VI FNAs, the proportion of positive nodules was 8% (n = 135). Among these combined cohorts of Bethesda III/IV GSC Suspicious and Bethesda V/VI, the most common gene fusions observed for each of the 5 studied RTK genes was: ETV6/NTRK3 (n = 164, 72% of NTRK3 fusions), CCDC6/RET (n = 104, 55% of RET), BRAF/SND1 (n = 32, 20% of BRAF), ALK/STRN (n = 20, 37% of ALK), and NTRK1/TPM3 (n = 14, 50% of NTRK1). BRAF showed the highest diversity of fusions, with 80 gene partners. Different gene partners with RET, ALK, NTRK1, and NTRK3 numbered 25, 11, 9, and 5 , respectively . Conclusions: Whole-transcriptome RNA-seq on small sample thyroid FNA specimens can identify clinically relevant ALK, BRAF, NTRK, and RET fusions across Bethesda categories. The prevalence ranges from 3% in Bethesda III/IV Afirma GSC Suspicious specimens to 8% among Bethesda V/VI specimens. Future studies need to determine if detection of precision medicine candidates by pre-operative FNA can optimize initial treatment, predict response to treatment, and prioritize selective targeted therapy should systemic treatment be needed.[Table: see text]

Investigating the rationale for a particular technique can reveal the surprising finding that some techniques are not based on scientific research. Aspiration of the insulin syringe is an example of a technique that evolved based on assumption. In a study designed to challenge the need to aspirate during insulin administration, 204 injections that were aspirated yielded no blood return. The historical and research literature supports the finding that aspiration never was proven to be a reliable indicator of correct subcutaneous needle placement. Because inadvertent intramuscular injection of insulin has been documented, diabetes educators need a reliable method to individualize insulin administration techniques to decrease the risk of incorrect needle placement. Until a reliable method is available, the literature supports lifting a skinfoldfor all subcutaneous insulin injections.

INTRODUCCIÓN: En la actualidad, se busca un modelo de profesorado competente, capaz de sortear los retos de la sociedad del conocimiento. Entre las competencias “deseables” destaca la científica y tecnológica, a fin de explorar e incluir todas las posibilidades ofrecidas por las Tecnologías de la Información y Comunicación (TIC) en sus prácticas docentes. MÉTODO: El presente estudio responde a un diseño descriptivo-correlacional, cuya finalidad es analizar la percepción de los futuros docentes sobre su competencia científica en el uso de las TIC. Para ello, se elaboró un cuestionario ad hoc, de 30 ítems y cuatro opciones de respuestas. Las cuestiones incluían aspectos como su actitud hacia el uso de las TIC, qué importancia otorgaban a su inclusión en el aula, así como su opinión sobre si realmente estaban preparados para afrontar el reto de utilizarlas en su posterior ejercicio profesional. RESULTADOS: Los resultados revelan que los aspirantes a maestros se sienten tecnológicamente competentes, pero admiten que no son capaces de integrar las TIC en el aula pese a conocer las posibilidades didácticas que ofrecen. A su vez, los participantes poseen una valoración positiva hacia el uso de las TIC en el aula, aunque reconocen ciertas carencias formativas para asegurar su éxito cuando trabajen como maestros. Se establece una relación positiva entre la valoración de las TIC en la enseñanza, sus efectos en el aprendizaje, el conocimiento de su uso con fines didácticos y cómo incluirlas en las prácticas docentes. DISCUSIÓN: Se evidencia la necesidad de cambiar los programas formativos sobre tecnología educativa para alcanzar una integración equilibrada entre el conocimiento científico de las herramientas tecnológicas y el conocimiento didáctico-pedagógico. INTRODUCTION: We are currently looking for a competent teaching model able to face the new challenges of the knowledge society. Among the "desirable" competences, scientific and technological stands out, in order to explore and include all the opportunities offered by Information and Communication Technologies (ICT) in their teaching practices. METHOD: This study responds to a descriptive-correlational design, whose purpose is to analyse the perception of future teachers about their scientific competence regarding the use of ICTs. For this purpose, an ad hoc questionnaire was developed, with 30 items and four response options. The questions included aspects such as their attitude towards the use of ICTs, what importance they attached to their inclusion in the classroom, as well as their perception about whether they were really prepared to face the challenge of using them in their subsequent professional practice. RESULTS: The results reveal that teacher aspirants feel technologically competent, but admit that they are not able to integrate ICTs into the classroom even though they know what teaching opportunities they offer. In turn, the participants have a positive appreciation for the use of ICT in the classroom, although they recognise a number of training needs to ensure their success when working as teachers. A positive relationship is established between the assessment of ICT in teaching, its effects on learning, knowledge of its use for teaching purposes and how to include it in teaching practices. DISCUSSION: There is an evident need to reform training programmes on educational technology in order to achieve a balanced integration between scientific knowledge of technological tools and didactic-pedagogical knowledge.

8029 Background: Despite treatment with high-dose chemotherapy followed by autologous stem cell transplantation (AHCT), MM patients invariably relapse. MRD-negativity post-AHCT has emerged as the most important prognostic marker. Currently, MRD in MM is monitored via bone marrow aspirate sampling. Marrow MRD assays are limited by the spatial heterogeneity of marrow MM localization; extramedullary disease and sampling variability of marrow aspiration. Sensitive, non-invasive blood-based MRD assay is an unmet need. ctDNA as a noninvasive biomarker can be utilized to predict relapse in MM. Here we attempt to evaluate MRD using ctDNA in AHCT recipients with MM. Methods: In this retrospective, single-center study, we analyzed ctDNA MRD in blood samples collected from 28 patients with MM after upfront AHCT. A total of 80 plasma timepoints were available pre and post AHCT with a median follow-up of 92.4 months. Multiparameter flow cytometry (MFC) at 10-4 level was used to assess the MRD from the BM biopsy. Individual bone marrow aspirates or FFPE slides from the time of MM diagnosis and matched normal blood were whole-exome sequenced, and somatic mutations were identified. MRD assessment at 3 months post-AHCT was performed by ctDNA analysis using a personalized, tumor-informed (SignateraTM, bespoke mPCR NGS assay). The prognostic value of ctDNA was evaluated by correlating MRD status with clinical outcomes. Results: Table provides the baseline disease characteristics. Median age was 67 [41-75] years and 16 [57.1%] were males. ctDNA was detectable in 70.8% (17/24) of pre-AHCT, 53.6% (15/28) of ̃3 months post-AHCT, and 39.2% (11/28) of patients during the surveillance phase post-AHCT. Of the 15 ctDNA MRD positive patients, 93.3% (n=14) experienced relapse on follow-up (hazard ratio: 5.64; 95% CI: 1.8-17; p=0.0003). Patients negative for ctDNA at 3 months post-AHCT had significantly superior progression-free survival (PFS) compared to positive (median PFS, 84 months vs. 31 months; p=0.003) The positive predictive value (PPV) for relapse among patients positive for ctDNA at 3 months post-AHCT was 93.3%, and significantly higher than marrow MFC of 68.4%. Conclusions: Our study shows the feasibility that a tumor-informed assay on archival blood samples is predictive of relapse post-AHCT. Future prospective studies with real-time marrow NGS and ctDNA samples are needed to define the role of ctDNA in MM and its prognostic significance.[Table: see text]

Background: Antimicrobial resistance poses a major threat in the treatment of respiratory disease especially in developing countries like Bangladesh. Multidrug-resistant (MDR) bacteria along with extremely drug resistant (XDR) bacteria have emerged as major clinical and therapeutic dilemma in the treatment of tracheal infections here. Thus, the aim of this study is to assess multidrug resistance among clinical strains isolated from tracheal aspirates of patients in Dhaka, Bangladesh.Methods: Total 200 clinical isolates from tracheal aspirates were identified and their antibiotic susceptibility profiles were evaluated by using the VITEK 2 system following the Clinical and Laboratory Standards Institute guidelines. Patient information on diagnosis, sex, age was obtained from hospital data.Results: Of 200 clinical isolates obtained, Pseudomonas aeruginosa was the most frequent pathogens (30.5%) followed by Acinetobacter baumannii (29%), Klebsiella pneumoniae (22.5%), Streptococcus pneumoniae (7.5%), Escherichia coli (5%), Staphylococcus aureus (2%), Proteus spp (1.5%), Enterobacter spp (1%), Citrobacter spp (0.5%), Providencia spp (0.5%). Of 20 different antibiotics tested, highest number of isolates (86%) showed resistance to third generation cephalosporin cefixime, however least number of isolates showed resistance to polymixin antibiotics- colistin (12.5%) and polymixin B (6%). Tracheal infection was found to be more prevalent in males rather than in females although this difference was not statistically significant. The prevalence of infections was highest among the patients of age-group (old adults) ≥60 years (61.5%). Of 200 clinical isolates, 43 (21.5%) were XDR and 125 (62.5%) were MDR bacteria. Of 200 clinical isolates, the synthesis of extended spectrum β-lactamases (ESBL) and carbepenemase were detected in 59 (29.5%) and 98 (49%) strains respectively.Conclusions: Tracheal infections caused by β-lactamase producing MDR and XDR pathogens among patients are high in Dhaka, Bangladesh. Therefore, there is an urgent need for constant surveillance and interventions in Bangladesh in order to prevent further spreading of those resistant organisms.

Loop-mediated amplification has been promoted for SARS-CoV-2 screening, however, antigen tests are preferred in low-income countries and remote zones. Poor training in molecular biology, plus the need for RNA purification or reading instruments to overcome issues of sensitivity in colorimetric detection, are some of the reasons limiting the use of this technique. In this study, nasopharyngeal swabs, aspirates and saliva were amplified in an in-house LAMP assay and subject to colorimetric detection, achieved by the naked eye and by image analysis with a mobile application. Accuracy of detection by the naked eye ranged from 61–74% but improved to 75–86% when using the application. Sensitivity of the digital approach was 81% and specificity 83%, with poor positive predictive value, and acceptable negative predictive value. Additionally to the reported effect of some transport media’s pH, the presence of mucus and warming up of reagents while setting up the reaction critically affected performance. Accuracy per type of sample was 55, 70 and 80%, for swabs, aspirates and saliva, respectively, suggesting potential to improve the test in saliva. This assay, carried out in a closed tube, reduces contamination, has few pipetting steps and requires minimal equipment. Strategies to improve performance and implications of the use this sort of colorimetric LAMP for massive testing are discussed.

AbstractDuring a 2-week period, three infants with a cough lasting at least 8 days with whoops, were admitted to the pediatric unit; Bordetella pertussis was isolated from nasopharyngeal aspirates collected from the three infants. Approximately 1 week later, a nurse working on the same unit developed influenza-like symptoms followed by whooping cough; B pertussis was isolated. Isolates from the nurse and from one of the infants were shown to be indistinguishable by pulsed-field gel electrophoresis. These data demonstrate that B pertussis transmission to healthcare workers is possible and emphasize the need to use respiratory protection devices (Droplet Precautions) for healthcare workers having close contact with infected children.
